Abstract:This overview addresses for whom and under what conditions psychotherapy produces change, the course of change over sessions, and why change happens. Specifically, the most confirmed moderators are pretreatment symptoms level, readiness for change, assertiveness, agency, defense maturity, quality of object relationships, mentalization, and interpersonal functioning. Regarding the course of change, processual models better describe change than stage ones, capturing its nonlinear and time-embedded nature. Moreover, trajectories of change describe different classes of change by clients (i.e., from three to five) and might be helpful feedback for clinicians. Furthermore, the most confirmed change mechanisms in literature for individual psychotherapy are insight, affective awareness, reflective functioning, externalizing difficulties, self-talk, coping, emotion regulation, the reappraisal of threat, fear extinction, change of interpersonal cognitions/avoidance, compensatory skills, mood regulation expectancies, diet habits, and self-efficacy. Conversely, emotional closeness, couple satisfaction, blamer softening, family functioning, and parental competence are confirmed mechanisms for couple/family psychotherapy. Finally, transversal mechanisms common to many approaches should not be considered equal to common factors, such as alliance, emotional regulation, and insight.

Abstract:This paper lays out a model of mental schemes and schematic processes and uses it to offer an alternative "schematic synthesis" model of psychological change—a model that is at variance with most conventional theories. We argue that a theory of the internal workings of the mind, which provides a theoretical explanation of how clients' in-session experiences and performances are constructed, moment-by-moment, would enhance therapists' effectiveness and provide a guide for intervention. In this paper we describe how Pascual Leone's Theory of Constructive Operators (TCO) (Pascual-Leone & Johnson, 2021) can be applied to understand therapeutic change. TCO describes a mind that works by scheme activation, in combination with mental operators that act on schemes, to facilitate their synthesis. It offers a theoretical view of the internal workings of the mind that can causally explain how change occurs. This differs from standard accounts of mechanisms of change, such as learning or insight, which predominantly provide descriptions of how change occurs from an external vantage point. We use the TCO model to explain how emotional change takes place in therapy by a developmental process of transformation by “synthesis” of schemes, rather than by learning or insight. We illustrate application of the model in the context of a therapy transcript.

Abstract:OBJECTIVE: The present paper elaborates a process perspective of change in psychotherapy for personality disorders (PDs). Firstly, the paper reviews the literature of mechanisms of change in treatments of PD, with the main focus on emotional processing and socio-cognitive processing. Secondly, it proposes an illustrative case-series analysis of eight cases, drawn from a mediation analysis conducted within the context of a randomized controlled trial for borderline personality disorder (BPD).METHOD: As such, cases with good and poor outcomes are compared, as are cases with poor and good intake features and cases with poor and good process markers across treatment.RESULTS: The results illustrate possible pathways to healthy change over the course of four months of treatment, and possible pathways of the absence of change.CONCLUSIONS: These results are discussed with regard to three main research perspectives: the combination of qualitative and quantitative methodology in psychotherapy research may be applied to case study research, a neurobehavioral perspective on change may incorporate the individualized experience in the laboratory and therapist responsiveness to patient characteristics may be a core feature of fostering change. Clinical or methodological significance of this article: The present paper illustrates individual pathways to change in personality disorders. It illustrates how coping capacities influence the process of psychotherapy and outcome in personality disorders. It demonstrates the relevance of individualizing treatments for personality disorders. It demonstrates several integrative features of psychotherapy research, in particular the use of neurobehavioral paradigms and the integration of single-case research within randomized controlled trials.

Abstract:A recent meta-analysis revealed that mental health and baseline psychological impairment affect the quality of life and outcomes in different chronic conditions. Implementing mental health care in physical care services is still insufficient. Thus, interdisciplinary communication across treatment providers is essential. The standardized language provided by the diagnostic statistical manual favors a clear conceptualization. However, this approach might not focus on the individual, as thinking in categories might impede recognizing the continuum from healthy to diseased. Psychoanalytic theory is concerned with an individual's unconscious conflictual wishes and motivations, manifested through enactments like psychic symptoms or (maladaptive) behavior with long-term consequences if not considered. Such modifiable internal and external factors often are inadequately treated. However, together with the physical chronic condition constraints, these factors determine degrees of freedom for a self-determined existence. The effect of therapeutic interventions, and especially therapy adherence, relies on a solid therapeutic relationship. Outcome and process research still investigates the mechanism of change in psychotherapeutic treatments with psychanalysis's focus on attachment problems. This article examines existing knowledge about the mechanism of change in psychoanalysis under the consideration of current trends emerging from psychotherapy research. A clinical example is discussed. Additionally, further directions for research are given. The theoretical frame in psychoanalytic therapies is the affect-cognitive interface. Subliminal affect-perception is enabled via awareness of subjective meanings in oneself and the other; shaping this awareness is the main intervention point. The interactional ingredients, the patient's inherent bioenvironmental history meeting the clinician, are relevant variables. Several intrinsic, subliminal parameters relevant for changing behavior are observed. Therapeutic interventions aim at supporting the internalization of the superego's functions and at making this ability available in moments of self-reflection. By supporting mentalization abilities, a better understanding of oneself and higher self-regulation (including emotional regulation) can lead to better judgments (application of formal logic and abstract thinking). Thus, this facilitates enduring behavior change with presumably positive effects on mental and physical health.

Abstract:Objectives: Affective change has been considered the hallmark of therapeutic change in psychoanalysis. Psychoanalytic writers have begun to incorporate theoretically the advanced understanding of emotional processing and transformation of the affective neurosciences. We ask if this theoretical advancement is reflected in treatment techniques addressing the processing of emotion. Methods: We review psychoanalytic models and treatment recommendations of maladaptive affect processing in the light of a neuroscientifically informed model of achieving psychotherapeutic change by activation and reconsolidation of emotional memory. Results: Emotions tend to be treated as other mental contents, resulting in a lack of specific psychodynamic techniques to work with emotions. Manualized technical modifications addressing affect regulation have been successfully tested in patients with personality pathology, but not for psychodynamic treatments of axis I disorders. Conclusions: Emotional memories need to be activated in order to be modified, therefore, we propose to include techniques into psychodynamic therapy that stimulate emotional experience.

Abstract:Experiences of change from the perspective of both chronically conflictual couples and their therapist has not been explored, thus inhibiting current understanding of what is important for in-session change. The present study examines couple and therapist experiences of therapy for chronically conflictual couples. Sixteen interviews from four couples ( n = 8) and their therapists ( n = 4) were analyzed using Colaizzi's method of phenomenological inquiry. Results highlight environmental, couple, and therapist factors that are integral to experiencing change in session. Results show the importance of therapists facilitating intentional, non-conflictual, and connecting couple interactions. We discuss clinical implications of the findings.

Abstract:Introduction: This systematic review identified qualitative and mixed-methods empirical studies on psychotherapy from dialogical and narrative approaches, aiming to address the following questions: (1) How are subjectivity and intersubjectivity qualitatively understood in dialogical and/or narrative psychotherapies studied using dialogical and narrative approaches? (2) How do therapeutic changes occur, including their facilitators and barriers? (3) What psychotherapeutic resources are available for psychotherapists in these types of studies? Method: The articles were selected according to the Systematic Reviews and Meta-Analyses (PRISMA) guidelines and the eligibility criteria proposed by the PICOS strategy (participants, interventions, comparators, outcomes, and study design) from 163 records identified in the Web of Science Core Collection databases. Results: The systematic review process allowed the selection of 16 articles. The results provided insights into the understanding of subjectivity, intersubjectivity, change in psychotherapy, its facilitators, and barriers from these perspectives. It also offered some therapeutic interventions that can be implemented in psychotherapies, integrating dialogical and/or narrative aspects. Discussion: The centrality of dialogical exploration of patient/client resources, therapists as interlocutors fostering client agency, polyphony serving as scaffolding for change, and interconnection with the sociocultural environment are discussed. The integration of this latter topic has been a challenge for these types of studies, considering the active construction of shared meanings. The dialogical and narrative approaches focus psychotherapy on transforming meanings through dialogue and re-authoring stories, evolving within cultural and historical contexts. Thus, this study highlights the relevance of these perspectives in contemporary psychotherapy, emphasizing dialogue in co-creation within an intersubjective framework.
[Mid-term follow-up and analysis of the failure cases of interspinous implants for degenerative lumbar diseases].

Abstract:OBJECTIVE To assess the indications,efficacy and safety of application of interspinous implants for degenerative lumbar diseases by mid-term follow-up and analysis of the failure cases. METHODS In our study, 52 cases of degenerative lumbar diseases treated with interspinous implants From September 2007 to September 2008 were divided into Wallis group (25 cases) and Coflex group (27 cases). The clinical results were assessed by lumbar pain visual analog score (VAS) and lower limber pain VAS, lumbar Japanese Orthopedic Association (JOA) score and Prolo functional score; the radiological results including implant positon, segmental lodosis and segment movement degree were assessed by lumbar X ray post-operation compared with pre-operation. For cases of discogenic low back pain, lumbar disc changes were assessed by lumbar MRI 12 months post operation. RESULTS Fifty-two cases had complete follow-up and the average time was 30.4 months. In the final follow up, lumbar pain VAS,lower limber pain VAS,lumbar JOA score and Prolo functional score of Wallis group were (2.5 ± 1.7),(2.7 ± 1.4),(23.4 ± 3.1)and(8.9 ± 1.4), which were better than pre-operation (5.3 ± 3.0),(7.4 ± 2.6),(13.5 ± 4.6)and(4.5 ± 2.6 ),lumbar pain VAS P=0.027, all others P<0.001 ;lumbar pain VAS,lower limber pain VAS,lumbar JOA score and Prolo functional score of Coflex group were (2.6 ± 2.9),(3.8 ± 1.9),(21.2 ± 3.5)and(8.5 ± 1.8),which were better than pre-operation (5.5 ± 3.2),(7.1 ± 2.8), (13.1 ± 4.8)and(4.2 ± 2.5), lumbar pain VAS P=0.036, all others P<0.001;in the final follow up,lower limber pain VAS of Wallis group was (2.7 ± 1.4), which was better than (3.8 ± 1.9) of Coflex group(P=0.039);in the final follow up, Segment lodosis angles of Wallis group was 14.3° ± 3.9°, which was larger than 13.2° ± 3.5° of Coflex group (P=0.028); Segment movement degree of Wallis group was 9.6° ± 2.8°, which was smaller than 12.8°±3.0°of Coflex group (P=0.019).In Coflex group,four cases of lumbar disc herniation relapsed and three cases received second operation. One case with lumbar stenosis suffered from lumbar disc herniation of the same segments and received second revision operation. CONCLUSION Treatments with interspinous implants for the degenerative lumbar diseases are effective, but we should pay attention to the indication and apply them for lumbar disc herniation with caution.

Abstract:We propose that key concepts from clinical psychotherapy can inform science-based initiatives aimed at building tolerance and community cohesion. Commonalities in social and clinical psychology are identified regarding (1) distorted thinking (intergroup bias and cognitive bias), (2) stress and coping (at intergroup level and intrapersonal level), and (3) anxiety (intergroup anxiety and pathological anxiety). On this basis we introduce a new cognitive-behavioral model of social change. Mental imagery is the conceptual point of synthesis, and anxiety is at the core, through which new treatment-based approaches to reducing prejudice can be developed. More generally, we argue that this integration is illustrative of broader potential for cross-disciplinary integration in the social and clinical sciences, and has the potential to open up new possibilities and opportunities for both disciplines.

Abstract:This study aims to assess moments of meeting in psychotherapy -understood as moments of intense connectedness and intimacy, shared between patient and therapist during any therapeutic encounter that enable a spin in the therapy process- using a qualitative interview with patients who were undergoing or had finished psychological treatment. Micro-pehenomenological interviews were conducted with nine patients who were undergoing or had finished psychological treatment. Transcriptions of the interviews were subject to micro-phenomenological analysis. A general structure of episodes of meeting showing their temporal evolution was identified and divided into six consecutive phases. These findings suggest that episodes of meeting in psychotherapy are lived and remembered by patients in a significant way; they are emotionally charged and have a meaning for each participant. Also, emotional and nonverbal cues seem to be highly relevant features. Moments of meeting could be understood as implicit mechanisms that allow changes in the implicit relational domain.

Abstract:INTRODUCTION: This issue of In Session in The Journal of Clinical Psychology offers in depth case studies of psychotherapy for persons with differing types of schizophrenia spectrum disorders across international settings.METHOD: To understand the context and import of this work, this commentary explores commonalities of this work with past work including respect for patients' autonomy, compassion, and attention to the therapeutic relationship. The commentary then discusses themes which harken to the past including framing psychosis as something than can be understood jointly by therapist and patient rather than being unilaterally imposed.RESULTS: It is suggested that this work is emblematic of new movements in the literature which focus on the person who is experiencing schizophrenia.CONCLUSIONS: This focus extends to understanding the purposes, place of the person in the world and their possibilities, which enables them to make sense of what they need and how want to manage their own recovery.

Abstract:This study uses a hermeneutic phenomenological approach to better understand therapist experiencing of metaphor in non-directive person-centered relationships. Metaphor is ubiquitous in language. Meanwhile, literature in the field of psychotherapy posits it as indicative of a range of therapeutic attitudes, goals and outcomes. However, outside of noting its uses for communicating empathic understanding, and its function in process direction and diagnosis, the person-centered approach has no coherent account of it. Analysis of interviews with three therapists revealed experiencing related to relinquishing certainty, risking curiosity, and connecting briefly. These findings led to a brief consideration of the dialogical contact made when meeting in metaphor, with meaning noticed as an emergent quality of this contact. Whilst making a case for research that broadens the notion of rationality and embraces the complex ineffability of human experience, this article suggests some further questions, derived from the results, that might be addressed in future reflection.

Abstract:In dynamic psychotherapies, people work to address troubling thoughts, actions, and feelings by talking about themselves and their experiences. How can such talk ameliorate mental health symptoms? This chapter proposes an answer to this question that circumvents the central barrier to answering it, the assumption that the body and the mind are two separate systems. The chapter author calls upon recent work in cognitive science that emphasizes how human beings use their bodies and their environments in thinking; in such approaches mind and body are not separable. Such work helps us to conceptualize clients in psychotherapy as shaping the environments in which they dwell through the narratives they construct. The transformation that can occur through narrative is, from this perspective, in part a consequence of how persons constitute themselves differently in different environments.

Abstract:Background: The move from inconsistent and problematic autobiographical narrative to a more coherent and reality-based narrative construction of the Self has been discussed as potential mechanism of change in psychotherapies for personality disorders. So far, little empirical evidence exists that demonstrates in a time-dependent design the role of narrative construction in the treatment of borderline personality disorder, in particular when it comes to understanding the integration of body-related information from the affective system with the autobiographical narrative. The present study aims at demonstrating change in emotion-based narrative markers over brief psychiatric treatment and to assess the impact of these changes on subsequent symptom change. Methods: A total of N = 57 clients with borderline personality disorder were assessed at three timepoint over the course of four months of brief psychiatric treatment, within the context of a secondary process-outcome analysis of a randomized controlled trial. Symptom change was assessed using the OQ-45.2 and emotion-narrative change was assessed using the Narrative-Emotion Process Coding System to code client's in-session speech in terms of problem, transition and change markers. Results: All three emotion-based marker categories evidenced significant changes in the assumed direction. The reduction in problem emotion-based narrative markers (e.g., empty story telling) between session 1 and 5 into the treatment predicted the symptom reduction assessed between session 5 and 10. Conclusions: Emotion-based narrative construction may be a suitable method to study the pathway of change toward a more coherent and reality-based narrative construction of the Self-in-interaction-with-the-Other. Reduction of emotion-based problem-marker may be a promising candidate for a mechanism of change in treatments for personality disorders which should be tested in a time-dependent controlled design.

Abstract:Lichtenberg's chapter on "Empathy, Motivational Systems, and a Theory of Cure" was a brilliant effort to integrate his new paradigm of multimotivational systems with Kohut's empathic approach to psychotherapy. Lichtenberg shows how the clinical dialogue with patients can be monitored by using empathic perception to track shifts in motivational systems. At any given moment, a particular motivational system may become dominant. Successfully identifying the dominant motivation through empathic perception creates in patients a feeling of being understood without judgment. This ambience of safety allows for a shift that activates an exploratory motivation with patients and a spirit of inquiry. An observational platform is created where experience from the past with attachment figures and interaction in the here and now in therapy can be explored from new perspectives. Recent research in psychotherapy that uses monitoring the clinical dialogue with patients by tracking shifts in motivational systems supports the view of its positive effects. Lichtenberg's "theory of cure" is based on the premise that an empathic perception that monitors shifts in motivational systems will mobilize intrinsic self-righting capacities in patients. I note that there is controversy about what mutative factors in psychotherapy are most important: building an ambience of safety and trust or creating new insights into personal and internal conflicts through the use of language and symbolic representations. Both factors are crucial for positive changes in psychotherapy, and their importance will vary from patient to patient and from one patient–therapist relation to another. I note that Wilma Bucci's multiple code theory that sees experience coded at subsymbolic and verbal symbolic levels supports this view. I close by proposing that we can build on Lichtenberg's multimotivational system by adding a trans-survival need to give meaning to our lives.

Facilitating the Client Change: A Perspective from the Therapeutic Zone of Proximal Development

Abstract:Therapeutic zone of proximal development (TZPD) is an extension of Vygotskys concept of the zone of proximal development in the field of psychotherapy. Based on the assimilation model, TZPD represents the space between the actual development level and the potential development level of clients within a developmental sequence. As a new concept to understand the change process of psychotherapy, it can enhance the understanding of setbacks in psychotherapy, provide an easy-to-operate theoretical framework for promoting the change of the client, and provide precise guidance for the therapist to choose supportive or challenging intervention strategies. In terms of research, TZPD theory is based on an unconventional methodology of Theory-Building Qualitative Research. TZPD theory is built upon accumulating observations in qualitative case studies in which TZPD helped the therapist understand and interpret the psychotherapy process. Under this methodology, researchers mainly use a therapeutic collaboration coding system (TCCS) as a research tool to analyze different case studies, understand the causes and characteristics of different counseling cases, and compare the effects of good and bad results based on TZPD, to describe the characteristics of the psychotherapy process. This kind of practice will provide a richer perspective for future research on the theoretical construction of psychological counseling and therapy and the understanding of psychological treatment mechanisms. In clinical practice, as the working area of psychotherapy, TZPD provides a new perspective for understanding the setback phenomenon in the psychotherapy process. Early researchers regarded setbacks as a hindrance in the psychotherapy process, but the TZPD suggests that setbacks may be therapists exploration of the clients TZPD boundaries and preparation for subsequent progress. The key point of therapists work is not to avoid setbacks, but to discover and differentiate the phenomenon of setbacks under the enlightenment of TZPD theory, and choose the appropriate intervention technique at the appropriate stage. The future of TZPD hinges on further exploration of the clinical wisdom of experienced therapists. Its also worthy of carrying out theoretical exploration from different approaches and perspectives, which might ultimately explain setbacks in the process of assimilation and lead to a better understanding of the dynamics of the psychotherapy process.

Abstract:The change that occurred during an analytic hour with a seven-year-old girl is explored by means of videotape observation and aspects of developmental theory drawn from the field of infant research. Detailed review of the videotape and transcript of twelve minutes of the analytic hour revealed significant change in the girl's attitude toward the loss represented by a cancelled session. The moment-to-moment process between two points, A and B—identified as representing the beginning and end of the change in the girl's ability to deal with loss in the analytic relationship—is studied and described in two forms: written transcript for the verbal material, and narrative description for the nonverbal material. The methodology is based on observed behaviors but also relies on the clinical experience of the analyst in the interpretation of affects and the recognition of intentions in both participants. The results suggest that, taken as a whole, many such small transitional episodes during the course of a treatment play an important role in the process of therapeutic change.
