Abstract:In this paper I consider the individual nature of the way a person develops senses of time. I suggest that something of a patient's early, unvoiced, experience is revealed through the temporal rhythms of interaction with the therapist. I refer to development, cycles of experience, metabolism, transgenerational emotions, experiences of beginnings and endings, early interactions with (m)other, together with narrative and its relation to unconscious processes. Experiences of time within the session indicate the nature of the feelings that are being lived, or refused life. I consider the paradox of living in the present and its relation to chronology. Lastly I give an example of the difficulty of timing comments when there is no flow of feeling, which is, itself, a communication.

Abstract:This study aims to assess moments of meeting in psychotherapy -understood as moments of intense connectedness and intimacy, shared between patient and therapist during any therapeutic encounter that enable a spin in the therapy process- using a qualitative interview with patients who were undergoing or had finished psychological treatment. Micro-pehenomenological interviews were conducted with nine patients who were undergoing or had finished psychological treatment. Transcriptions of the interviews were subject to micro-phenomenological analysis. A general structure of episodes of meeting showing their temporal evolution was identified and divided into six consecutive phases. These findings suggest that episodes of meeting in psychotherapy are lived and remembered by patients in a significant way; they are emotionally charged and have a meaning for each participant. Also, emotional and nonverbal cues seem to be highly relevant features. Moments of meeting could be understood as implicit mechanisms that allow changes in the implicit relational domain.
Generalized granuloma annulare associated with gastrointestinal stromal tumour: case report and review of clinical features and management

Abstract:The paraneoplastic variant of granuloma annulare (GA) is a rare cutaneous manifestation of underlying malignancy that is most commonly associated with systemic lymphoma. We report an interesting case of a patient with gastrointestinal stornal tumour (GIST) of the stomach presenting with extensive generalized GA. GIST was diagnosed 2 months after the diagnosis of GA. Resolution of the GA was seen 1 month after surgical excision of GIST. The close correlation of the clinical courses of these two rare diseases suggests that their coexistence was more than a coincidental finding. This case highlights the importance of excluding paraneoplastic GA, especially in cases where the skin manifestations are extensive and resistant to treatment.

Abstract:Aim: Mixed-methods approaches promise a deep understanding of psychotherapeutic processes. This study uses qualitative and quantitative data from daily diary entries and daily self-assessments during inpatient treatment. The aim of the study is to get an insight into the similarities and differences between both types of data and how they represent self-organized pattern transitions in psychotherapy. While a complete correlation of results is not expected, we anticipate observing amplifying and subsidiary patterns from both perspectives. Materials and methods: Daily, five MDD patients wrote diaries and completed self-assessments using the Therapy Process Questionnaire, a questionnaire for monitoring the change dynamics of psychotherapy. The data were collected using the Synergetic Navigation System, an online tool for real-time monitoring. Diary entries of the patients described their experiences in everyday life. The qualitative text analysis was conducted using Mixed Grounded Theory, which provided categories representing the patients' ongoing experiences of transformation and stagnation. The time series data was analyzed using the dynamic complexity algorithm and the pattern transition detection algorithm. Results from qualitative and quantitative analyses were combined and compared. Following the process of data triangulation, the leading perspective came from the theory of self-organization. In addition to presenting the overall results for all five patients, we delve into two specific case examples in greater detail. Results: Specific and highly diversified diary entries of 5 patients were classified into the categories of perceived pattern stability, noticing improvement, broadening the perspective, critical instability, and experiencing moments of Kairos. Patients reported problems not only related to their disorder (e.g., lack of energy and hopelessness) but also to phases and steps of change, which could be related to the theory of self-organization (e.g., problem attractors, critical fluctuations, pattern transitions, and Kairos). Qualitative and quantitative analysis provide important supplementary results without being redundant or identical. Conclusion: Data triangulation allows for a comprehensive and multi-perspective understanding of therapeutic change dynamics. The different topics expressed in the diary entries especially help to follow micro-psychological processes, which are far from being a simple reaction to interventions. The way patients experience themselves being in stability or instability and stagnation or transformation is surprisingly close to the general features of self-organizing processes in complex systems.

Abstract:This paper offers a reflection and an open, experiential reading of one of Winnicott's most fascinating case studies. I try to show how the question of time in the psychoanalytical context emerges not only in relation to a deviation from the traditional setting but also concerns an axis inherent to some key therapeutic issues, such as the meaning of time and the temporal as a quality of holding in regression to dependency; or as a component in the transference experience or in the therapist's holding in mind of the patient's potential, through holding the future. The high frequency of meetings in psychoanalysis facilitates qualitative change, allowing patients and therapists to be in touch with deep, primitive strata of the psyche. Extending time in psychoanalytic treatment, therefore, whether as an ad hoc response to the patient's needs or as an extension of the setting, is tantamount to enabling a multi-layered experience of time across the spectrum of its manifestations. Attention to the temporal component in therapy and the experience of time in the transference is a valuable resource in our work. This paper contributes to the attempt to elaborate this dimension of psychoanalytic listening.

Brief presential and online psychodynamic psychotherapy: patients' perception

Abstract:In the last decade, different types of online activities have been growing exponentially and online psychotherapy is moving in the same direction. It is crucial to recognize this line of work as a facilitator for people in the contemporary world, especially after the COVID-19 pandemic in 2020. The purpose of this study was to compare the perception of patients who underwent brief face-to-face psychodynamic psychotherapy and online psychotherapy by a single therapist. The study had a quantitative-qualitative approach, exploratory and comparative, which used the clinical methodology. The appointments were systematic and respective, consisting of six daily face-to-face sessions, on consecutive days, except on Sundays; and six online appointments, using the same process for data collection; the extent of each session was between 40 to 50 minutes. For the data collection regarding the perception of patients on the brief psychodynamic psychotherapy care in person versus online, were tested the Inventory of Therapeutic Alliance (Working Alliance Inventory) with questions about the therapeutic relationship, after the end of 6 online sessions, and the Questionnaire Session Evaluation Questionnaire right after the end of each psychotherapeutic session, both in face-to-face and online situations. The results obtained demonstrated that there is equivalence in face-to-face and online care, regarding the therapeutic relationship built in both moments.
Lost in transition? Professional perspectives on transitional mental health services for young people in Germany: a qualitative study

Abstract:BackgroundThe transition of young patients from child and adolescent to adult mental health services often results in the interruption or termination of care. At this intersection, mental health professionals function as gatekeepers between systems, and their personal views on current clinical practice can contribute to a broader understanding of procedures and help identify reasons for service gaps. This qualitative study investigated the views of mental health professionals on services for young people during the transition from child and adolescent to adult mental health care, as well as on factors which facilitate or hinder continuity of care.MethodsFour group discussions with 24 mental health professionals with various backgrounds were conducted. Groups were audio-taped, transcribed verbatim and analyzed following the reconstructive approach of R. Bohnsack’s documentary method.ResultsA main theme and six subthemes emerged. Participants’ overall concern was an increasing lack of patient centeredness in care provision. They criticized the limited flexibility and time constraints of their work, which was held to be incompatible with the time-consuming process of engaging young patients in care and coping with their individual needs. A lack of adequate interprofessional exchange and networking was seen as resulting in a diffuse sense of responsibility and a lack of clarity for all involved parties. Participants focused on the adverse impact of neglecting developmental characteristics in care procedures for young patients and revealed personal issues they experienced in their work with young patients (e. g. personal difficulties with diagnosing).ConclusionsMental health professionals at this transitional point face a number of complex tasks as well as limitations in terms of time and personal support. An emphasis should be placed on forming and maintaining partnerships within and between systems which could contribute significantly to relieving professionals’ workload. Furthermore, an open style of communication to engage young patients in care is essential. Strengthening communicative skills, improving knowledge about this life stage (especially when working in adult services), and promoting interprofessional encounters can help to develop new procedures in clinical practice. On higher system levels, heightened awareness of the need to reduce fragmentation of care and administrative barriers is needed.

Abstract:In recent years, especially in the era of the COVID-19 pandemic, we have witnessed a partial transition in the field of psychotherapy from in-person settings to remote ones (i.e., online therapy). This trend has raised the question of whether the two settings are interchangeable. With the research described here, the authors explore, within a transactional analysis theoretical framework, whether the therapeutic impact of in-person sessions, assessed by both patients and therapists, is different from that perceived with a remote setting. Using the Session Evaluation Questionnaire (SEQ), 20 transactional analysis psychotherapists and their patients evaluated the impact of two pairs of sessions—one in person and one online—for a total of 160 SEQs. The data were then analyzed through t -tests to verify the research hypotheses. For therapists only, the impact of remote sessions was perceived to be lower than that of in-person sessions. The implications are discussed from a clinical point of view.
Understanding psychoanalytic work online and back to the couch in the wake of the COVID-19 pandemic: an investigation among Italian psychoanalysts

Abstract:Background: Worldwide, psychotherapists' clinical experience went through rapid developments with transition to teletherapy during the COVID-19 pandemic. Literature on the use of remote psychoanalysis was not conclusive, leaving the issue of the consequences of the necessary setting alternation open. This study aimed to investigate the psychoanalysts' experiences of shifting to remote work and then returning to in-person setting, considering the effect of the patients' attachment styles and personality configurations. Method: Seventy-one analysts of the Italian Psychoanalytic Society were asked to fill out an online survey about patients who found the transition easier and patients who found it more difficult. General questions on therapeutic work, ISTS (Interpretive and Supportive Technique Scale) for interpretive and supportive aspects of technique, WAI-S-TR (Working Alliance Inventory-Short Revised-Therapist) for therapeutic alliance, RQ (Relationship Questionnaire) for attachment style, and PMAI (Prototype Matching of Anaclitic-Introjective Personality Configuration) for personality configurations were administered. Results: All of the analysts chose to continue the treatment using audio-visual tools. Patients with difficult transitions had a significantly higher frequency of insecure attachment and a higher score on RQ Dismissing scale than patients with easy transitions. No significant differences were found between the two groups in personality configurations, psychotherapeutic alliance, and psychotherapeutic technique. Moreover, a higher level of therapeutic alliance was positively correlated to RQ Secure scale and was negatively correlated to RQ Dismissing scale. Patients with easy transition both to remote work and back to in-person setting had higher scores of therapeutic alliances than those with difficult transition both to remote work and back to in-person setting. Conclusion: Online psychoanalytic therapy was widely used during the COVID-19 pandemic. Patients with insecure attachment styles had greater difficulties in adapting to setting alternations, thus confirming that insecure attachment is a vulnerability factor not only for psychopathological problems but also for a well-functioning therapeutic collaboration. Patient's personality configuration did not influence their adaptation to the setting alternation. The supportive and interpretive styles did not undergo significant changes in the transition from in-person setting to remote setting and vice versa, thus suggesting a continuity in the analysts' "internal setting."

Abstract:Journal of the American Psychoanalytic Association, Volume 70, Issue 4, Page 733-754, August 2022. This qualitative study focuses on the process of treatment over the internet from a psychodynamic perspective based on the experiences of seventeen patients in China who underwent online treatment by psychoanalysts based in the United States or other Western nations. The data were collected and analyzed using psychodynamic themes. Seventeen participants involved in online cross-national and cross-cultural psychodynamic treatment were recruited in China. Various themes in regard to the process of online treatment in a cross-national and cross-cultural context emerged. Findings indicate the following: (1) in contrast to traditional relational patterns, psychodynamic treatment provides a new relationship model; (2) psychodynamic treatment creates a space of self-reflection for Chinese patients in terms of their relationships; (3) the therapeutic setting emphasizes clear boundaries, free space, equal relationships, and emotional expression for patients. The results provide new insights into self-examination, which frees patients from the inflexible aspects of traditional relationships, family hierarchies, and rigid cultural values. In addition, psychodynamic treatment in a cross-national and cross-cultural setting not only enables patients to work through their trauma but also empowers them to explore their unique journey of individuation.

“Our Time is Up”: A Relational Perspective on the Ending of a Single Psychotherapy Session

Abstract:This paper, written from a relational perspective, examines the final minutes of an individual psychotherapy session, and is organized around the topics of boundary negotiation, unwitting self-disclosures, visual challenges, and countertransference. Attending to session-ending material is important because the separation involved lends heightened emotional intensity to the oftensignificant material that appears in the final minutes. This material often serves as a bridge to the psychotherapeutic work to be taken up in subsequent sessions. Session-ending dynamics call upon the therapist to prioritize empathy, validation, and support for the patient suffering from early deprivation; identify and heal narcissistic injury in the patient wishing to be special; judiciously alter the frame when doing so will benefit the patient and not constitute a boundary violation; avoid re-traumatization; admit mistakes; confront blatant denial; advance agency; address uncomfortable topics; set appropriate limits; and deal authentically with uncomfortable countertransference. Numerous clinical examples serve to illustrate these clinical phenomena.

Abstract:Time and the distribution of time have hardly been taken notice of up to the present although they are of great importance when considering the economic perspective. The present study reports on the results found between certain time parameters (e.g. total duration, total number of sessions, number of sessions per week, longer interruptions) and the outcome of psychoanalytic psychotherapies. The sample consists of 76 patients with neurotic, functional or psychosomatic diagnoses. The results support the thesis that not only the quantity of time (number of sessions, duration of therapy), but also the distribution throughout the therapy is associated with the success of therapy.

Abstract:Psychotherapy science and the practice of psychotherapy do not always co-exist. Rather, tension has developed overtime between researchers and practitioners. This tension, however, has not eroded all links between science and practice. In fact, many therapists pursue careers in which they can engage in psychotherapy research. This phenomenological study was undertaken to understand the meaning therapists ascribe to being a part of psychotherapy research while being full-time clinicians. To accomplish this task, we interviewed ten therapists who work in a college counseling center where the intersection of research and practice is evident. Using a semi-structured protocol, each participant was individually interviewed about their experiences of engaging in research while being a practitioner. Analyses of interviews led to the identification and interpretation of 450 statements that were grouped into six themes: (a) Making Research Feasible; (b) Ubiquitous Nature of Research in the Center; (c) Benefits of Doing Research; (d) Impediments to Psychotherapy Science; (e) Psychotherapy Science Beyond Data Collection; and (f) Scientist-Practitioner Values and Identity. Results highlight the value of embedding research in clinical settings; however, the impediments to engaging in research while providing therapy were clear. Results are discussed in the context of practice-oriented research and integrating science and practice.

Abstract:A dynamic nonlinear equivalent magnetic network (NEMN) method for a fault-tolerant permanent magnet Vernier machine is proposed in this paper. The key of this paper is to develop two models for no-load and on-load conditions, respectively, because the flux distribution under the no-load condition is different from that under the on-load condition owing to the modulation effect. The no-load model is built based on the precise air-gap modeling method, while the on-load model is developed based on the simplified air-gap modeling method. Meanwhile, a new circuit transformation method is provided for the on-load model to solve the difficulty of low convergence speed in the iteration process. Moreover, iron saturation and fringing effect are considered in the proposed NEMN models. Then, it is used to predict the air-gap flux density, cogging torque, back-electromotive force, and inductance. Finally, both the finite-element method and experimental tests are used to confirm the accuracy and effectiveness of the proposed models.

Abstract:Implicitly or explicitly, time dominates the psychoanalytic situation. The precision, consistency of duration, and regularity of analytic sessions enhance the patient's ego boundaries, counteracting the regressive effects of timelessness induced by free association. The extended overall duration of psychoanalysis and the high frequency of sessions favor the development of transference neurosis. The interpretation of the transference in the here and now of the analytic situation illuminates the past, and as a result, the patient's self-image and that of the world become better integrated. The sense of time in the analytic situation for both patient and analyst varies along with the vicissitudes of transference and countertransference.
What makes psychotherapy sessions productive? a new approach to bridging the gap between process research and practice

Abstract:Abstract This paper proposes a new strategy for bridging the gap between psychotherapy process research and practice. Constructs covering the full spectrum of potential change mechanisms are derived from a general theory of human functioning (Grawe's Consistency Theory) and operationalized by Therapy Spectrum Analysis (TSA), a measure of the realization of therapeutic change mechanisms within individual therapy sessions. Using the data generated by TSA, we conducted an exploratory search for meaningful patterns of process and session‐outcome variables using Answer Tree as a context‐sensitive method of data analysis. Results show that process‐activated resources of the patients, in particular their responsiveness to interventions and motivation are especially predictive of the session productivity experienced. Further analyses were conducted to uncover how the patients' participation can be promoted and results translate into guidelines for the session‐to‐session decisions by clinicians. The experience of closeness and distance in the therapeutic relationship of patients with different attachment classifications: an exploration of prototypical cases

Abstract:Background: Individuals with different attachment classifications (Secure, Avoidant and Preoccupied) may experience emotional closeness differently, in their intimate relationships but also as clients in psychotherapy. However, evidence for this assumption almost exclusively comes from research with self-report questionnaires. Aims: In this paper, we use observer-rated measures to explore in depth how patients with different attachment classifications experience closeness and distance from the therapist in different phases of therapy. Method: Three patients' and their therapists' narratives about the therapeutic relationship at three time points during therapy were extracted and analyzed with two transcript-based observational measures: The Patient Attachment Coding System (PACS), which classifies patients' attachment according to their discourse behavior, and the therapeutic-Distance Scale-Observer version (TDS-O), which assesses the therapeutic relationship in terms of closeness, distance, autonomy and engagement. Cases were chosen from a larger research project due to their different prototypical attachment classification on the PACS. The narratives were obtained from Relationship Anecdote Paradigm (RAP) interviews in which the patients and their therapists narrated separately about meaningful interactions with each other, at early, middle and late phases of therapy. In addition, we followed patients self-report of the alliance and symptoms (OQ-45). Results: Although all patients reported experiencing discomfort with feeling distant from the therapist the therapeutic distance, the secure patient was able to reflect on his feelings and, in the therapist's recollection, was able to share them with the therapist. This allowed the therapist to harness these feelings for the benefit of the therapy. The avoidant and the preoccupied patients both experienced the therapist as distant, but the avoidant patient prevented closeness by a minimal expression of feelings, and the preoccupied described strong frustration with the therapist in a one-sided manner that prevented collaborative processing and left the therapist confused. Discussion: It appears that patient discourse is a stable (trait-like) component of attachment, while the therapeutic-distance is a process (state-like) component that may change along therapy. The discourse of insecure patients may hinder therapists' ability to adjust the therapeutic-distance to patients' needs. Therapists' knowledge about the ways patients with different attachment classifications communicate their proximity wishes may improve their attunement.
