Abstract:Objective To explore characteristics and management of bile duct injury caused by laparoscopie cholecystectomy and emphasize the importance of intraoperative cholangiography.Meth- ods Clinical data of 31 cases of bile duct injury caused by laparoscopic eholecystectomy appeared in our hospital during 1994—1999 were retrospectively analyzed.Results All these 31 cases recovered well and no biliary stricture occurred during the follow-up ranging from 1 month to 6 years.Conclu- sions Comparing with injuries caused by laparotomic cholecystectomy,the bile duct injury caused by laparoscopie cholecystectomy has its own characteristics.Most of the cases of the injury can be detec- ted intraoperatively by intraoperative cholangiography.If the injury could be found introperatively,the outcome of immediate repair would be satisfactory.Meanwhile,the methods of management are multi- ple and flexible.

Abstract:To evaluate the place of primary repair of bile duct injury during laparoscopic cholecystectomy (LC).Between 1994 and 2004, 53 of a total 14 302 cases were identified with bile duct injury. The indication, methods and outcome of primary repair for 17 cases of bile duct injury were analyzed.Forty-Two of 53 cases of bile duct injury were identified at the time of operation, thirty three involved a tear of the common, hepatic or right hepatic bile duct, and 17 cases received primary repair. Drainage tubes were placed in 7 cases, and biliary drainage through the cystic duct was performed in 10 cases. All patients recovered well and got out of bed within 6 hours postoperatively. The alimentary tract recovered in 24 hours after operation in all cases, and their mean hospitalization was 4.6 days. Sixteen patients were followed up, and no stricture of bile duct was found.Repair of bile duct injury during LC is safe and effective in selected cases.
Robotic-assisted Laparoscopic Repair of a Penetrating Extraperitoneal Bladder Injury

Abstract:Background: Traumatic intraperitoneal or complicated extraperitoneal bladder injuries are conventionally managed with open exploration and repair. There are rare reports in the literature of laparoscopic repair of intraperitoneal bladder injury secondary to blunt abdominal trauma, as well as two reports of laparoscopic repair of extraperitoneal bladder injuries from blunt abdominal trauma. There are no reported cases of a minimally invasive surgical repair of a penetrating bladder injury. There are also no reported cases of a robotic-assisted laparoscopic repair of a traumatic bladder injury, regardless of the injury mechanism. Objective: In this video, we demonstrate a surgical technique for a robotic-assisted laparoscopic repair of a penetrating traumatic bladder injury. Methods: We present a case of a 43-year-old male with a penetrating extraperitoneal bladder injury secondary to a gunshot wound. Our patient underwent emergent primary vascular repair of an associated vascular injury. Hemodynamic instability delayed immediate exploration and bladder repair. Cross-sectional imaging and flexible sigmoidoscopy ruled out further visceral injury. Unfortunately, difficulty maintaining catheter patency prompted further surgical intervention. An attempt to evacuate all clots by rigid cystoscopy was unsuccessful, and the decision was made to proceed with a robotic-assisted laparoscopic cystorrhaphy. Results: The retropubic space was developed and the extraperitoneal bladder injury was identified. All clot was evacuated and no active bleeding was noted. The bladder mucosa was inspected confirming no additional injury. The cystotomy was closed in two running layers using absorbable sutures. Two leak tests were performed confirming a water-tight repair. The bladder was reapproximated to the anterior abdominal wall to reestablish the retropubic space. A cystogram 1week postoperatively confirmed a successful bladder repair. Conclusion: Robotic-assisted laparoscopic cystorrhaphy may be a feasible approach for a penetrating extraperitoneal bladder injury in highly select, hemodynamically stable patients.
Uroperitoneum as an Atypical Source of Ascites: A Case Report

Abstract:Uroperitoneum secondary to spontaneous bladder rupture is a rare cause of ascites associated with significant morbidity and mortality. It can be difficult to detect and is often initially mistaken for other, more common etiologies. We present the case of a 56-year-old female with a history of cervical cancer treated with chemotherapy and radiation, radiation proctitis, and diabetes mellitus who presented with subacute onset abdominal pain and distension, urinary retention, and nausea. She had been diagnosed with cervical squamous cell cancer 12 years prior to presentation and was successfully treated with two months of chemotherapy and radiation, and a presumed recurrence five years later was treated to remission with chemotherapy. The golden-yellow appearance of her ascitic fluid during diagnostic paracentesis raised suspicion for urinary ascites that was confirmed by an elevated ascites-to-serum creatinine ratio and computed tomography (CT) cystography. Subsequent CT cystogram demonstrated leakage of contrast from the bladder with a 0.5 cm irregularity noted at the bladder dome, potentially representing the site of extravasation. A Foley catheter was placed at the time of admission with an immediate output of 1 L of fluid. Subsequently, her abdominal distension significantly improved, and her creatinine began to downtrend. Gynecologic oncology and urology were consulted and determined that she was not a candidate for surgical intervention given the significance of her bladder scarring. Positron emission tomography (PET)/CT was performed and revealed no active cancer. At the time of discharge, she had no episodes of emesis. Additionally, her creatinine had fallen to 1.0 mg/dl. She was discharged with a Foley catheter with plans to follow up with outpatient urology. While relatively uncommon, uroperitoneum should be suspected in patients presenting with new-onset ascites who have risk factors for spontaneous bladder rupture such as pelvic irradiation. Uroperitoneum has a significant rate of mortality and morbidity. Ascites urea and creatinine studies, followed by a CT cystogram if these studies are abnormal, should be performed in any patient with risk factors for uroperitoneum. Patients should be managed with the placement of a Foley catheter and urology consultation for surgical evaluation.

Abstract:Peroral cholangioscopy-guided lithotripsy is highly effective in clearing difficult bile duct stones. It can cause adverse events, such as cholangitis and pancreatitis; however, gallbladder perforation is extremely rare. Herein, we describe the case of a 77-year-old woman who developed gallbladder perforation following peroral cholangioscopy -guided lithotripsy. She was referred to our hospital to treat multiple large bile duct stones. She underwent peroral cholangioscopy-guided lithotripsy because of conventional lithotripsy failure. After a cholangioscope was advanced into the bile duct, saline irrigation was used for visualization. Electronic hydraulic lithotripsy was performed, but it took time for fragmentation because the calculus was hard. The 2-h endoscopic procedure did not completely remove the stone, and treatment was discontinued after placing a biliary plastic stent and nasobiliary tube. After the endoscopic procedure, she started experiencing right hypochondrial pain, which worsened the next day. Computed tomography showed a gallbladder wall defect in the gallbladder fundus with pericholecystic fluid. She was diagnosed with gallbladder perforation and underwent emergency surgery. A perforation site was found at the gallbladder fundus. Open cholecystectomy, choledochotomy, and extraction of residual bile duct stones were performed. The patient was discharged 9 days post-surgery without any complications. The saline irrigation used for visualization may have caused a surge in intra-gallbladder pressure, resulting in gallbladder perforation. Therefore, endoscopists may need to conserve irrigation water during peroral cholangioscopy-guided lithotripsy.

Abstract:Traumatic intraperitoneal bladder rupture (IBR) requires surgical repair. Traditionally performed via laparotomy, experience with laparoscopic bladder repair (LBR) after blunt trauma is limited. Benefits of laparoscopy include decreased length of stay (LOS), less pain, early return to work, fewer adhesions, and lower risk of incisional hernia. The aim of this series is to demonstrate the potential superiority of LBR in select trauma patients. This is a retrospective review performed of all IBR patients from 2008 to 2016. Demographics, clinical management, outcomes, and follow-up were compared between LBR and open bladder repair (OBR) patients. Twenty patients underwent OBR, and seven underwent LBR. There was no significant difference in gender, age, or Injury Severity Score. There were no deaths or reoperations in either group. Average hospital length of stay and ICU days were similar between groups. There was one patient with UTI and one with readmission in each group. There were two incisional hernias and two bowel obstructions in the OBR group, with one patient requiring operative intervention. No such complications occurred in the LBR group. LBR for traumatic IBR can be safely performed in select patients, even in those with multiple extra-abdominal injuries.

Abstract:BACKGROUND: Spontaneous bladder rupture is relatively rare, and common causes of spontaneous bladder rupture include bladder diverticulum, neurogenic bladder dysfunction, gonorrhea infection, pelvic radiotherapy, etc. Urinary bladder perforation caused by urinary catheterization mostly occurs during the intubation process.CASE SUMMARY: Here, we describe an 83-year-old male who was admitted with 26 h of middle and upper abdominal pain and a history of long-term catheterization. Physical examination and computed tomography of the abdomen supported the diagnosis of diffuse peritonitis, most likely from a perforated digestive tract organ. Laparoscopic exploration revealed a possible digestive tract perforation. Finally, a perforation of approximately 5 mm in diameter was found in the bladder wall during laparotomy. After reviewing the patient's previous medical records, we found that 1 year prior the patient underwent an ultrasound examination showing that the end of the catheter was embedded into the mucosal layer of the bladder. Therefore, the bladder perforation in this patient may have been caused by the chronic compression of the urinary catheter against the bladder wall.CONCLUSION: For patients with long-term indwelling catheters, there is a possibility of bladder perforation, which needs to be dealt with quickly.

Abstract:Traumatic bladder injuries are rare, comprising only 0.78% to 1.6% of blunt abdominal injuries, with 85% of them being blunt injuries. [ 1 ] The rarity of bladder injuries is primarily attributed to the protective pelvic structure, typically stemming from high-energy trauma. Clinical signs of traumatic bladder injuries are nonspecific and mainly involve gross hematuria, suprapubic pain/tenderness, and urinary difficulties. [ 2 , 3 ] Clinicians can easily diagnose and treat bladder injuries when typical clinical signs are present, but patients with atypical symptoms, especially delayed bladder rupture, may be overlooked or misdiagnosed, potentially impacting treatment efficacy. In this study, we present a rare and easily overlooked case of delayed intraperitoneal bladder rupture resulting from blunt trauma. Ethical approval for this study was obtained from the ethics committee of the Chongqing Emergency Medical Center. The patient and her family were informed about emergency exploratory laparotomy procedure and relevant medical documents were signed after obtaining their consent.

Abstract:In Aberdeen the birth-rate fell from 17-6 in 1961 to its lowest level, 14-1, in 1969 and then rose slightly to 14-7 in 1971 (Medical Officer of Health Report, 1971). From the mid-sixties birth-rates generally were falling in Britain and in other western societies affected by similar demographic and socio-economic forces but Aberdeen's rate fell to a lower level than that of England and Wales (16-0) or of Scotland as a whole (16-6). There has been no deliberate policy in Aberdeen to aim for 'zero growth' but rather the objective has been that every woman should attain 'a fifth freedom?freedom from the tyranny of excessive fertility' (Baird, 1965). Thus birth control in all its aspects has long been accepted as an integral part of the maternity services. Aberdeen's reputation in this field has received much publicity, various aspects and particularly its policy on therapeutic abortion having been featured in television, radio, and the press. This article gives an overview and reports the outcome of pregnancies and the use of the family planning clinic and sterilization in Aberdeen City residents during the decade 1961-71 during which oral contraception was introduced (1964), the Abortion Act came into force (1968), and sterili zation increased, particularly vasectomy. Aberdeen, with a relatively unchanging population of 183,000 is a well-defined city set in a predomin antly rural area. The opportunities it provided for the development of a comprehensive maternity service were perceived many years ago by Sir Dugald Baird. Under his leadership as Professor of Obstetrics and Gynaecology from 1936, and that of his successor, Professor Ian MacGillivray, from 1965, the maternity service in Aberdeen has evolved. Baird (1969b) has described its development and the collaboration between hospital and local authority which has been a feature of it. [Analysis of Perioperative Complications of Laparoscopic Radical Cystectomy for Bladder Cancer].

Abstract:OBJECTIVE:To evaluate operation process and perioperative complications of patients who underwent laparoscopic radical cystectomy (LRC).METHODS:The clinical data of 49 cases of LRC from October 2004 to June 2010 were reviewed retrospectively. Perioperative complications and mortality were analyzed, and so were the operative time, blood loss and postoperative hospital stay. Perioperative complications were defined as any adverse event within 30 days of surgery. All complications were graded according to an established five-grade modification of the original Clavien system.RESULTS:The mean operation time was 418 minutes, the mean blood loss was 514 mL, the transfusion rate was 36.7%, the mean transfusion volume was 578 mL, and the average postoperative hospital stay was 20 d. For urinary diversion, ileal conduits were constructed in 27 patients (55.1%), ileal neobladders in 16 patients (32.7%), and ureterocutaneostomies in 6 patients (12.2%). A total of 17 patients (34.7%) developed at least one perioperative complication. Complications of grades 1-2 occurred in 12 patients (24.5%), which included subileus, urinary tract infections, deep venous thrombosis of the lower limbs, pneumonia, etc. Complications of grades 3-5 occurred in 5 patients(10.2%), and one patient died of pulmonary embolism. Ileal neobladders and ileal conduits were similar at the operation time, blood loss, transfusion rates, postoperative hospital stay and morbidity of perioperative complications.CONCLUSION:Morbidity of perioperative complications following LRC was still high. The most frequent complications were subileus and urinary tract infections. The surgery should be performed on selected patients, and measures need to be taken to prevent these complications. Compared with ileal conduits, ileal neobladders did not increase the operation time, blood loss and morbidity.
Iatrogenic bile duct injury: The scourge of laparoscopic cholecystectomy

Abstract:BACKGROUND: Laparoscopic cholecystectomy (LC) has become the first-line surgical treatment of calculous gall-bladder disease and the benefits over open cholecystectomy are well known. In the early years of LC, the higher rate of bile duct injuries compared with open cholecystectomy was believed to be due to the 'learning curve' and would dissipate with increased experience. The purpose of the present paper was to review a tertiary referral unit's experience of bile duct injuries induced by LC.METHODS: A retrospective analysis was performed on all patients referred for management of an iatrogenic bile duct injury from 1981 to 2000. For injuries sustained at LC, details of time between LC and recognition of the injury, time from injury to definitive repair, type of injury, use of intraoperative cholangiography (IOC), definitive repair and postoperative outcome were recorded. The type of injury sustained at open cholecystectomy was similarly classified to allow the severity of injury to be compared.RESULTS: There were 131 patients referred for management of an iatrogenic bile duct injury that occurred at open cholecystectomy (n = 62), liver resection (n = 5) and at LC (n = 64). Only 39% of bile duct injuries were recognized at the time of LC. Following conversion to open operation, half the subsequent procedures were considered inappropriate. When the injury was not recognized during LC, 70% of patients developed bile leak/peritonitis, almost half of whom were referred, whereas the rest underwent a variety of operative procedures by the referring surgeon. The remainder developed jaundice or abnormal liver function tests and cholangitis. An IOC was performed in 43% of cases, but failed to identify an injury in two-thirds of patients. The bile duct injuries that occurred at LC were of greater severity than with open cholecystectomy. Following definitive repair, there was one death (1.6%). Ninety-two per cent of patients had an uncomplicated recovery and there was one late stricture requiring surgical revision.CONCLUSIONS: The early prediction that the rate of injury during LC would decline substantially with increased experience has not been fulfilled. Bile duct injury that occurs at LC is of greater severity than with open cholecystectomy. Bile duct injury is recognized during LC in less than half the cases. Evidence is accruing that the use of cholangiography reduces the risk and severity of injury and, when correctly interpreted, increases the chance of recognition of bile duct injury during the procedure. Prevention is the key but, should an injury occur, referral to a specialist in biliary reconstructive surgery is indicated.

Abstract:Purpose: Rectal injury is a potential complication of radical prostatectomy. Because laparoscopic radical prostatectomy is still a challenging procedure, we review the incidence and management of rectal injury in 1,000 cases of consecutive laparoscopic radical prostatectomy performed at our institution. Materials and methods: Of the first 1,000 laparoscopic transperitoneal radical prostatectomies performed between January 1998 and April 2002, 13 (1.3%) were complicated by rectal injury. Mean patient age was 66.5 years (range 58 to 76) and mean prostate specific antigen was 12.9 ng./ml. (range 2.9 to 26). Clinical stage was T1c, T2a and T2b in 5, 7 and 1 patient, respectively. Mean preoperative Gleason score was 5.8 (range 3 to 8). Once recognized the rectal defect was closed laparoscopically in 2 layers and tested for the absence of leakage. Broad-spectrum intravenous antibiotics were given for 7 days. Oral liquids were started the day after surgery with a low residue diet, and a regular diet was started on postoperative day 5. Healing of the vesicourethral anastomosis was confirmed by voiding cystourethrogram on postoperative day 5. Results: All patients underwent a non-nerve sparing procedure except 1 in whom unilateral neurovascular bundle preservation was done. Of 13 injuries 11 were diagnosed and repaired intraoperatively, and 2 were diagnosed postoperatively. Of the 11 cases of intraoperative diagnosis and repair 9 healed primarily without colostomy and peritonitis was diagnosed in the remaining 2 on days 3 and 4, respectively. Of the latter 2 patients 1 required repair of a small rectal defect without colostomy while the other required colostomy. Colostomy was performed in the 2 patients with delayed diagnosis on days 3 and 4 but even then a rectourethral fistula developed in 1, necessitating secondary repair. Average urethral catheterization time was 8.6 days for the 9 patients with an uneventful immediate postoperative course and mean hospital stay was 6.8 days. For the remaining 4 patients urethral catheterization duration was 12, 13, 15 and 120 days, and hospital stay was 7, 16, 21 and 27 days, respectively. There was no perioperative mortality. Conclusions: Rectal injury during laparoscopic radical prostatectomy requires meticulous intraoperative repair in 2 layers, which allows primary healing without diversion colostomy. For injury prevention scrupulous attention is required during non-nerve sparing radical prostatectomy, particularly at the posterior surface of the prostatic apex.
Bladder perforation due to laparoscopic peritoneal dialysis catheterization: A case report and literature review

Abstract:Peritoneal dialysis (PD), offering the benefits of stable hemodynamics, a low risk of bloodstream infection, preservation of residual renal function, and enhancement of quality of life, [ 1 ] has emerged as the preferred modality for renal replacement therapy in patients with end-stage renal disease. Percutaneous puncture catheterization, surgical catheterization, and laparoscopic catheterization are the most common methods for PD catheterization. Advancements in PD catheterization technology have led to a gradual decrease in the incidence of complications related to catheterization. However, these complications continue to be a primary cause of treatment failure in PD. The main complications include infection-related issues (exit site and tunnel infections) and mechanical problems (catheter displacement or occlusion, dialysate leakage, hernia, internal organ injury, and bleeding). [ 2 ] Hence, timely identification and correction of catheter-related complications are essential for enhancing the prognosis of PD patients.

Abstract:Enzymes have the remarkable ability to select the correct substrate from the pool of chemically similar molecules. The accuracy of such a selection is determined by differences in the free-energy profiles for the right and wrong reaction pathways. Here, we investigate which features of the free-energy landscape govern the variation and minimization of selectivity error. It is generally believed that minimal error is affected by both kinetic (activation barrier heights) and thermodynamic (binding stability) factors. In contrast, using first-passage theoretical analysis, we show that the steady-state selectivity error is determined only by the differences in transition-state energies between the pathways and is independent of the energies of the stable complexes. The results are illustrated for two common catalytic mechanisms: (i) the Michaelis-Menten scheme and (ii) an error-correcting kinetic proofreading scheme with tRNA selection and DNA replication as guiding biological examples. Our theoretical analysis therefore suggests that the selectivity mechanisms are always kinetically controlled.
Identifying a perforated prepyloric ulcer during laparoscopy in a patient presumed to have cholecystitis

Abstract:Abdominal pain in patients with significant alcohol use and smoking history presents diagnostic challenges due to overlapping clinical features of complications like acute cholecystitis and peptic ulcers. The unreliable physical examinations of intoxicated patients often complicate accurate diagnosis. We present a case of a 56-year-old male with a history of alcoholism and smoking, who presented to the emergency department with nonspecific abdominal pain. Initial imaging suggested cholecystitis, but due to the patient's intoxication, his physical examination was unreliable. During a laparoscopic cholecystectomy, a perforated prepyloric ulcer was unexpectedly discovered, sealed by the gallbladder. This case highlights the limitations of relying solely on imaging for diagnosing abdominal conditions in intoxicated patients. Iatrogenic ureteral injury diagnosed after colon cancer surgery: A case report of a rare and challenging complication

Abstract:Introduction: Iatrogenic ureteral injury (IUI) is an unfortunate and rare complication during colorectal surgery. While IUI remains a rare event, short and long-term complications are life-threatening ranging from intraperitoneal urinoma to septic shock and a serious risk of permanent renal failure. Case presentation: An 88-year-old patient was admitted with symptoms of large bowel obstruction and underwent a laparotomy with a discharge colostomy. A week later, a second laparotomy was required for a non-functional retracted stoma, revealing a perforation in a sigmoid tumor. The patient then had an oncological sigmoidectomy with Hartman's colostomy. Postoperative findings indicated a left ureteral injury. Three weeks later, a ureterostomy was performed. Unfortunately, the patient succumbed to heart failure one week after the ureterostomy. Discussion: Low anterior and abdominoperineal resection of the rectum, along with sigmoid resection are the most frequent causes of ureteral injury in digestive surgery. The primary objective of management is to establish a continuous flow of urine to avert potential complications. Preventing IUI in colorectal surgery is of paramount importance. This process initiates in the preoperative phase with a meticulous assessment of ureteral and colic anatomy through comprehensive review of preoperative imaging. Conclusion: IUI remains a seldom-seen, and yet a very serious complication in colorectal surgery. It is imperative to prioritize both preoperative and intraoperative measures to prevent IUI, ensuring optimal outcomes. When the diagnosis of a IUI is established, a treatment strategy should be meticulously devised and executed by a skilled and experienced surgeon.
Ureteral Ligation During Robotic-Assisted Laparoscopic Prostatectomy

Abstract:Robotic-assisted laparoscopic prostatectomy (RALP) is the surgical standard of care for patients with localized prostate cancer. Although uncommon, the procedure involves a potential risk of injury to adjacent anatomical structures. We report on a unique case of iatrogenic ureteral injury during RALP that required subsequent robotic-assisted laparoscopic ureteral reimplantation for definitive repair. A 57-year-old male underwent RALP using the Da Vinci Xi system (Intuitive Surgical, Sunnyvale, CA). The procedure was unremarkable and a 20 French Foley catheter was placed with plans for removal after one week following a negative cystogram. On postoperative day two, his creatinine level elevated to 2.69 mg/dL from a baseline of 1.40 mg/dL, left-sided flank pain increased, and non-contrast CT imaging revealed moderate left proximal hydroureteronephrosis and no other abnormalities. Aside from mild nausea on postoperative day one, he had no other symptoms. An integrated stent was unable to be placed by urology at this time. Subsequently, a left percutaneous nephrostomy tube was placed under fluoroscopic guidance. After this intervention, the patient's symptoms improved and the decision was made not to proceed with operative re-exploration at this time to attempt identification of the obstruction. Three weeks later, the patient underwent cystoscopy with attempted left retrograde ureteropyelography and left ureteroscopy due to suspected distal obstruction. This revealed complete obstruction of the intramural portion of the ureter, presumed to be secondary to suture ligation at the time of the vesicourethral anastomosis. Seven weeks postoperatively, the patient underwent robotic-assisted laparoscopic left ureteral reimplantation. Thereafter, the patient had a resolution of his left hydroureteronephrosis and acute kidney injury. This case describes an intravesical ureteral ligation during RALP. An iatrogenic intravesical ureteral ligation has far less guiding literature than a more common ureteral transection. Additionally, ureteral transection is often identified and managed intraoperatively, while the ureteral ligation presented in this case is far less likely to be apparent during surgery. Early identification will allow for rapid reoperation to manage the injury. We hypothesize that during the vesicourethral anastomosis, the left intramural ureter was ligated. Importantly, with the use of a 3-0 V-Loc stitch for the vesicourethral anastomosis, its barbed nature would not facilitate simple surgical removal. In conclusion, when performing RALP, the depth of the bladder-sided vesicourethral anastomotic stitch should be carefully considered to avoid a similar injury.

Abstract:In a retrospective study 85 patients of our department with clinical oral leukoplakia occurring between 1980 and 1985 were checked histologically and reclassified. Compared to other studies and unusual high malignant transformation rate (14.1%) was found which seemed to be depended on the high rate of radiated patients. Four of twelve patients were healed due to early diagnosis and radical surgery performed consecutively. Using these data as a basis the different courses and therapies are demonstrated.
Nonoperative Management of a Spontaneous Perforation of Neobladder Secondary to Blackout From Solitary Binge Drinking: A Case Report and Current Literature Review

Abstract:The orthotopic ileal neobladder is becoming a popular technique of urinary diversion after radical cystectomy (RC) for localized muscle-invasive bladder cancer (MIBC), allowing patient continence, with a more desirable body image and good quality of life. Minimally invasive robot-assisted RC and neobladder have the potential to minimize physical and psychological trauma and are increasingly being adopted for patients with MIBC worldwide. Spontaneous perforation of orthotopic neobladder is uncommon;however, it represents serious complications. Solitary binge drinking can be dangerous in a patient with a neobladder because of reduced level of consciousness and overdistension of the neobladder. We report a case of spontaneous ileal neobladder perforations one year post-robotic RC secondary to blackouts from binge drinking. We also describe nonoperative active management and review the literature. A 66-year-old gentleman was brought by ambulance to our emergency department with a reduced level of consciousness, vomiting, and abdominal pain in the early hours of the morning. Collateral history revealed that he had drunk alcohol alone the night before at his home where he lives alone. Initial examination revealed tachycardia and hypotension. Immediate resuscitation using the sepsis six protocol included intravenous normal saline, blood culture, broad-spectrum antibiotic, lactate measurement, and insertion of a urethral catheter to monitor urine output. Following contrast-enhanced computed tomography (CECT) of the abdomen and pelvis, the patient was referred to a urologist. A diagnosis of spontaneous perforation of the neobladder was made. A nonoperative or 'conservative' management approach was adopted with careful active monitoring at the intensive care unit (ICU) involving a multidisciplinary team. Follow-up CT was performed to assess radiological recovery. The patient recovered successfully and was discharged home five weeks post-admission with an indwelling urethral catheter. The catheter was removed 10 weeks post-admission following a cystogram confirming the integrity of the neobladder. The patient has preserved neobladder function and continence and is doing well until the last follow-up at six months post-discharge. Patients with neobladder should be rigorously counseled about the importance of timed voiding, intermittent self-catheter, serious consequences of solitary binge drinking, and urinary retention.
