Abstract:Introduction and importance: Ectopic liver tissue (ELT), a rare anomaly distinct from accessory liver, challenges conventional embryonic morphogenesis. Unlike the latter, ELT lacks a connection to the main liver, showcasing an unusual growth of normal liver tissue beyond its customary location. This peculiarity poses clinical and radiological challenges for surgeons throughout their careers, particularly during laparoscopic or open procedures. Elevated clinical significance arises from ELT's potential to progress into hepatocellular carcinoma, necessitating heightened awareness among surgeons. Case report: This article presents a compelling case of ELT, discovered incidentally during a planned laparoscopic cholecystectomy. The patient, a 60-year-old female with a history of biliary colic, underwent a meticulous exploration revealing an undistended gallbladder with an unexpected brownish tissue fragment resembling hepatic parenchyma. Clinical discussion: Ectopic liver tissue, dating back to early 20th-century records, challenges precise incidence determination. Theories regarding embryonic development around the fourth week in utero provide insights into ELT's origins and displacement from the hepatic diverticulum. Varied attachment locations and potential manifestations in other intra-abdominal and intra-thoracic sites add layers to the complexity of its diagnosis. Radiological studies, though challenging, offer glimpses of ELT, cautioning against percutaneous biopsies due to associated risks. Conclusion: In conclusion, this case of ELT offers valuable insights into its diagnostic challenges and surgical considerations, underscoring the need for continued research and heightened awareness in the medical community. The rarity and varied presentations of ELT warrant ongoing exploration to refine diagnostic approaches and optimize patient outcomes.

Abstract:Abstract Background Ectopic liver tissue (ELT) is a rare developmental abnormality in which normally functioning liver tissue develops at an extra-hepatic site. Due to the atypical clinical symptoms of ELT, its clinical diagnosis is rarely easy, however, the risk of carcinogenesis is strongly increased due to its special pathological characteristics. So it should always be considered in the differential diagnosis of tumors or symptomatic mass. Case presentation Case 1：A 57-year-old female patient presented herself to the outpatient department of our hospital complaining of paroxysmal upper abdominal pain for one month. She was admitted due to "Multiple intrahepatic bile duct stones and chronic cholecystitis". During the surgery, an abnormally enlarged gallbladder of size about 12*8*5cmwas found. A tiny mass of about 1.2*0.6*0.3cm on the free wall of the gallbladder with a similar texture to the liver was also found. Case 2：An 87-year-old male patient presented himself to the emergency department of our hospital complaining of right upper abdomen pain for three days. He was admitted to the hospital due to "cholecystolithiasis and chronic cholecystitis". During the surgery, an abnormally enlarged gallbladder of size about 12*8*5cm. A tiny mass of about 1.2*0.6*0.3cm on the free wall of the gallbladder with a similar texture to the liver was also found. After surgery, the two patients are now in good overall condition. Discussion and conclusions In this two continuous cases of Ectopic liver tissue, the pathological examination and liver cancer markers “AFP” and “Glypican-3” did not support tumor lesions. Accordingly, we proposed a new hypothesis for this phenomenon, that cryptogenic functional excretory system (helps to excrete chemical carcinogens) from ELT into the gallbladder (a part of the biliary system) does exist but it cannot be found in ELT located in other organs. This phenomenon may provide a rational response to the question why does Ectopic Liver Tissue on the Gallbladder rarely have malignant transformation.
Radiological Insights of Ectopic Thyroid in the Porta Hepatis: A Case Report and Review of the Literature

Abstract:BACKGROUND Ectopic thyroid is defined as a rare developmental anomaly where thyroid tissues are atypically found in locations other than its normal anatomical position: Anterolateral to the second, third, and fourth tracheal cartilages. An intemperate descent or a migration failure of the thyroid anlage results in sub-diaphragmatic thyroid ectopia, a sparse clinical entity. CASE SUMMARY This case portrays a 63-year-old female patient presenting with chronic abdominal discomfort at a local hospital whereby a computed tomography (CT) scan revealed a well-defined mass in the hepatic entrance. For further examination, the patient underwent a CT scan with contrast, magnetic resonance imaging (MRI), and CT-angiography (CTA) at our department. The CT scan showed a well-defined and high attenuated mass measuring 43 mm × 38 mm in the hepatic entrance with calcification. The CTA revealed an additional finding: Blood supply to the mass from the right hepatic artery. MRI of the upper abdomen demonstrated a mass with mixed signal intensity on T1 and T2 weighted images in the hepatic entrance. The patient underwent surgery with resection of the mass which was sent for histopathology. Ectopic thyroid at the level of porta hepatis with nodules was the definitive diagnosis since histopathological report revealed presence of thyroid tissue in the resected liver mass. CONCLUSION This case delivers a rare insight of pre-operative radiological imaging of an ectopic thyroid located in the liver. These findings can aid in narrowing down potential differential diagnosis when managing a patient with those subsequent findings.

Abstract:e19739 Background: The standard therapy of thyroid carcinomas (TC) consists of total or nearly total thyroidectomy. Then the whole-life substitution therapy by oral thyroxine (T4) is given. Another goal of this therapy is to suppress serum thyroid stimulating hormone (TSH) to prevent the growth factor-like effect of TSH on TC cells. The recommendation is to administer supraphysiologic amounts of oral T4. This leads to hyperthyroidism, which is subclinical. Thyroid hormones are also involved in regulation of bone metabolism and longstanding subclinical hyperthyroidism may result in increased bone turnover and decreased bone mineral density (BMD). The aim of the study was to assign the damage of bone metabolism in TC patients. METHODS Bone mineral density (BMD) was measured by dual energy photon x-ray absorptiometry in the lumbar spine and hips. BMD was classified using T score in postmenopausal women and more than 50 years old men. We classified Z score in premenopausal women and younger than 50 years men. C-terminal cross-linked telopeptides of type I collagen (CTX) were measured using ELISA. Additionally serum TSH and fT4 (free T4) were measured. Relationships between baseline characteristics (age, menopausal status in women, TSH levels and duration of suppressive therapy) and BMD were assessed using univariate and multivariate analysis tools. RESULTS We included 165 TC patients in the study. There were 13 men and 152 women, the mean duration of thyroid suppressive therapy was 7. 2 years (0 - 24 years). The patients exhibit osteporosis in 31%, whereas 30% had normal BMD. The incidence of osteoporosis appeared to increase with age but did not correlate with duration of thyroid suppressive therapy. TSH levels slightly correlated with decrease in BMD. CONCLUSIONS The long term survivors from TC after total thyroidectomy on thyroid suppressive therapy have higher risk of osteoporosis and therefore we recommend BMD testing and appropriate measures according to results.
A rare case of gall bladder containing accessory lobe of liver stuck to its wall: A case report

Abstract:Introduction: Accessory liver lobe (AL) is a very rare congenital anomaly found during surgery incidentally and not usually possible to diagnose preoperatively. Here, we are reporting this rare case in which the accessory liver was diagnosed at the time of surgery and was ectopically connected to the gallbladder wall. Case summary: A 71-y-old woman, attended our outpatient department (OPD) with pain in the epigastrium. She was diagnosed early having gallstone disease. Ultrasound abdomen was advised, which revealed cholelithiasis. Laparoscopic cholecystectomy was performed and we discovered a small liver-looking tissue stuck to the anterior wall of the gall bladder, which was removed along with the gall bladder. Subsequently, the tissue was confirmed to be liver tissue. Conclusion: It is a rare variation of the accessory lobe of the liver attached to the gall bladder. It remains asymptomatic clinically and may pose a danger of transforming to hepatocellular carcinoma; hence, total excision of the lobe should be considered.

Abstract:Acute cholecystitis and appendicitis are among the most common conditions encountered by general surgeons; however, they are rarely described simultaneously. We are reporting a rare incidental finding of early acute appendicitis during laparoscopic cholecystectomy for a 36-year-old lady who presented to our emergency department with signs and symptoms of acute calculous cholecystitis. Laparoscopy has proved to be a highly useful and ideal tool in the diagnosis and treatment of such surgical situations.

Abstract:Background: Hepatocellular carcinoma (HCC) is one of the most frequent cancers and the main cause of cancer-related death worldwide. Ectopic HCC, an extremely rare type of HCC, exhibits a wide range of clinical signs and radiographic features, making preoperative identification challenging. Case summary: A 47-year-old man underwent routine abdominal color ultrasonography, which identified an asymptomatic tumor in the left upper abdomen. The patient had no history of hepatitis, did not drink alcohol, and had no family history of cancer. Abdominal contrast-enhanced computed tomography (CT) revealed a heterogeneously enhanced lesion between the spleen and stomach that had invaded the diaphragm, with blood supplied by the left inferior phrenic artery. The patient underwent laparoscopic surgery, and HCC was identified by postoperative pathology. Additionally, specific immunohistochemical staining was performed to assess the molecular biological characteristics of the HCC. The patient underwent two rounds of hepatic arterial interventional chemotherapy after surgery. Abdominal plain and enhanced magnetic resonance imaging and lung CT 3 mo postoperatively revealed no signs of local recurrence or distant metastasis. Conclusion: This asymptomatic ectopic HCC case described achieved an excellent result due to early detection, radical resection, and systematic surveillance.
Incidentally exploration of the fusiform cystic duct cyst during laparoscopic cholecystectomy of septate gallbladder - Case Report

Abstract:Introduction and importance: Biliary cysts are rare congenital anomalies of the biliary ductal system. Cystic duct cysts in particular are not included in the widely used Todani classification and remain underreported. Case presentation: A 28-year-old male presented with intermittent right upper quadrant pain exacerbated by fatty foods. Ultrasound showed a septate gallbladder. Laboratory tests were normal. Laparoscopic cholecystectomy identified a 1.5 cm cystic duct cyst. Clinical discussion: Cystic duct cysts arise from anomalous ductal recanalization/dilatation. Presentation mimics cholelithiasis. Complete surgical excision is the treatment to prevent complications. Conclusion: This case presents an incidental finding of a cystic duct cyst resected during cholecystectomy for septate gallbladder. Increased recognition can improve the management of these rare biliary anomalies.
Complete Laparoscopic Cholecystectomy for a Duplicated Gallbladder A Case Report

Abstract:Introduction: Duplication of the gallbladder (GB) is a rare congenital abnormality occurring in 1 in 4000 to 5000 births. Three types have been reported: type I (split primordial GB), type II (2 separate GBs with their own cystic ducts), and type III (triple GBs drained by 1 to 3 separate cystic ducts). Patients with a duplicated GB are usually asymptomatic and are sometimes not diagnosed on preoperative imaging, which might increase the difficulty and risk of cholecystectomy. The key to successful treatment is total removal of the duplicated GB to avoid the recurrence of disease. Intraoperative cholangiography is recommended for identifying and resecting duplicated GBs. The final diagnosis depends on the histopathology. Patient concerns: A 62-year-old woman had recurrent upper abdominal pain and nausea for 1 year, with no fever, jaundice, or other symptoms. An ultrasound of the abdomen indicated polyps in the GB. Computed tomography (CT) revealed moderate dense structures attached to the wall of the GB and an unusual 47 x 21 mm elliptical structure with an extra tubule located above the main GB. Diagnosis: A diagnosis of duplicated GB was made based on the histopathology. Interventions: The patient underwent a laparoscopic cholecystectomy with total removal of the duplicated GB. Outcomes: The patient's postoperative course was uneventful and she was discharged from the hospital on the second postoperative day. She had no upper abdominal pain at the 6-month follow-up. Conclusion: Duplicated gallbladder is a rare congenital biliary anatomy, which is usually asymptomatic and sometimes cannot be diagnosed on preoperative imaging. With gallbladder disease, the duplicated GBs should be removed totally; a laparoscopic approach should be attempted first and cholangiography is recommended to aid in identifying and resecting the duplicated GBs. The final diagnosis depends on the histopathology. There is still insufficient evidence on the need to remove duplicated GBs found incidentally.
A case of clear cell-hepatocellular carcinoma accidentally found in the gallbladder wall after laparoscopic resection of gallbladder empyema: Beware of the liver bed

Abstract:Introduction and importance: Hepatocellular carcinoma (HCC) invading the gallbladder is a rare scenario, even in the context of an advanced tumor. The clear cell variant of HCC (CC-HCC) is also rare. However, CC-HCC is more frequent in women and those with liver cirrhosis, and the standard definition is the presence of transparent cytoplasm in at least 50 % of tumor cells. Case presentation: An accidental finding of HCC was made during histological examination of a gallbladder empyema specimen from a 74 year-old male patient. The tumor was found to be invading the wall of the gallbladder. On post-operative imaging, the HCC was identified in liver segments 4b, 5 and 6, with extensive portal vein thrombosis; without other distant spread. The tumor in our case was almost exclusively comprised of clear cells (>90 %); therefore, it was classified as CC-HCC. Clinical discussion: Tumors are rarely discovered along with acute cholecystitis. Furthermore, HCC rarely invades the wall of the gallbladder. HCC with clear cell histology should be distinguished from other regional and metastatic tumors of similar morphology. Conclusion: In this particular case, we provide an unusual circumstance in which HCC invaded the gallbladder wall and was found by chance in a specimen obtained after a cholecystectomy. Malignancy remains an under-recognized clinical scenario that is easily overlooked when assessing patients presenting with cholecystitis. We report our case with an additional focus on the findings of computed tomography (CT) as well as the clinicopathological characteristics pertinent to this distinctive histologic pattern of CC-HCC.

Abstract:The alkaline treatment of Guo, dGuo, dGMP and denatured DNA modified by N-acetoxyacetylaminofluorene (N-AcO-AAF) was performed in 0.1 M NaOH at 40 degrees C. The kinetics of the reaction were followed by ultraviolet absorption and by chromatographic methods and were found different for the four products under study. Circular dichroism spectra show differences in the environment of acetylaminofluorene residue in these products. The alkaline treatment of Guo-AAF (and dGuo-AAF) leads to the formation of three products. These products were separated by thin layer chromatography and by HPLC and were characterized by spectroscopic methods. One is the already known unstable Guo-AF (and respectively dGuo-AF) (1). The other two products are relatively stable products of the transformation of Guo-AF (or dGuo-AF). These last ones present almost identical ultraviolet absorption spectra, but very different circular dichroism spectra.
Confronting the uncommon: Cholecystitis and cholangitis in a young adult arising from a bilobed gallbladder with subvesicle duct - A case report and review of literature

Abstract:Introduction and importance: Bilobed gallbladder is a rare congenital anomaly characterized by the duplication of the gallbladder. It presents a unique challenge for surgeons due to its infrequency and the potential for perioperative complications. Anatomical variations, including the presence of the subvesicle duct of Luschka, should be suspected in these cases. Case presentation: In our case report, we present the clinical details of a **23-year-old male** who presented with symptoms of acute cholecystitis and cholangitis. Preoperative imaging revealed a bilobed gallbladder, and incidentally, intraoperative evidence of the subvesicle duct/duct of Luschka was also observed during a successful laparoscopic cholecystectomy. Importantly, the patient experienced no postoperative complications. Clinical discussion: This case highlights considering bilobed gallbladder as a differential diagnosis in patients with cholecystitis or cholangitis is crucial. Surgeons should be vigilant about this anomaly when evaluating patients with gallbladder-related symptoms. Conclusion: We understand precise imaging plays a pivotal role in guiding surgical planning. Detecting bilobed gallbladder preoperatively allows for better assessment of biliary structures and helps prevent bile leaks during surgery. Although rare, awareness of bilobed gallbladder and its associated anatomical variants is essential for optimal patient management and successful surgical outcomes. Surgeons should remain attentive to such anomalies to ensure safe and effective procedures.

Abstract:PURPOSE To determine the configuration of the anterior lamina cribrosa (LC) surface in eyes with different tilt axes. METHODS A total of 114 eyes (66 glaucomatous eyes and 48 healthy eyes) with tilted optic discs (ovality index > 1.3) were divided into vertical- and horizontal-tilt groups according to their optic disc tilt axis. Serial horizontal B-scan images of the ONH were obtained from each eye using enhanced-depth imaging spectral-domain optical coherence tomography. After three-dimensional reconstruction of the images, the anterior LC surface depth (LCD) was measured in each of the oblique scans containing the long disc axis and in the seven horizontal B-scans that divided the anterior LC surface vertically into eight parts. RESULTS The configuration of the anterior LC surface along the long disc axis differed markedly between groups. The vertical-tilt group had a larger LCD at superior locations than at inferior locations, while the pattern was the opposite in the horizontal-tilt group. The RNFL thinning in glaucomatous eyes was most prominent at both the superotemporal and inferotemporal sectors in the vertical-tilt group, while it was predominated at the inferotemporal sector in the horizontal-tilt group. CONCLUSIONS The configuration of the anterior LC surface in eyes with differed optic disc tilt axis suggested that the horizontally-tilted optic discs may not be resulted from the optic disc rotation, but from the optic disc tilt centered on the oblique axis. The preferential location of RNFL thinning could be explained by the LCD profile differing according to the tilt axis.
Spilled gallstones during laparoscopic cholecystectomy

Abstract:Introduction: Incidental gallbladder cancer is found in 0.6-2.1% of patients undergoing laparoscopic cholecystectomy for symptomatic gallstones. Patients with Tis or T1a tumours generally undergo no further intervention. However, spilled stones during surgery may have catastrophic consequences. We present a case and suggest aggressive management in patients with incidental gallbladder cancer who had spilled gallstones at surgery. Case history: A 37-year-old woman underwent a laparoscopic cholecystectomy for symptomatic gallstones, during which some stones were spilled into the peritoneal cavity. Subsequent histological examination confirmed incidental pT1a gallbladder cancer. Hepatopancreatobiliary multidisciplinary team discussion agreed on regular six-monthly follow-up. The patient developed recurrent pain two years after surgery. Computed tomography revealed a lesion in segment 6 of the liver. At laparotomy, multiple tumour embedded gallstones were found on the diaphragm. Histological examination showed features (akin to the original pathology) consistent with a metastatic gallbladder tumour. Conclusions: This case highlights the potential for recurrence of early stage disease resulting from implantation of dysplastic or malignant cells carried through spilled gallstones. It is therefore important to know if stones were spilled during original surgery in patients with incidental gallbladder cancer following a laparoscopic cholecystectomy. Aggressive and early surgical management should be considered for these patients.

Abstract:Introduction Laparoscopic cholecystectomy (LC) represents one of the most commonly performed routine abdominal surgeries. Nevertheless, besides bile duct injury, problems caused by lost gallstones represent a heavily underestimated and underreported possible late complication after LC. Methods Case report of a Clavien-Dindo IVb complication after supposedly straightforward LC and review of all published case reports on complications from lost gallstones from 2000-2022. Case Report An 86-year-old patient developed a perihepatic abscess due to lost gallstones 6 months after LC. The patient had to undergo open surgery to successfully drain the abscess. Reactive pleural effusion needed additional drainage. Postoperative ICU stay was 13 days. The patient was finally discharged after 33 days on a geriatric remobilization ward and died 12 months later due to acute cardiac decompensation. Conclusion Intraabdominal abscess formation due to spilled gallstones may present years after LC as a late complication. Surgical management in order to completely evacuate the abscess and remove all spilled gallstones may be required, which could be associated with high morbidity and mortality, especially in elderly patients. Regarding the overt underreporting of gallstone spillage in case of postoperative gallstone-related complications, focus need be put on precise reporting of even apparently innocuous complications during LC.

Primary Hepatic Ectopic Pregnancy: Diagnosis and Laparoscopic Management

Abstract:Objective: To demonstrate and discuss a case of primary hepatic ectopic pregnancy and laparoscopic management. Design: Case presentation with demonstration of surgical hepatic wedge resection. Setting: Tertiary referral center in Manchester, United Kingdom. Interventions: A 33-year-old women gravida 13 para 2 with a body mass index of 55 kg/m2 and previous 2 cesarean sections and a laparoscopic cholecystectomy presented to the emergency services after a private ultrasound scan showing a pregnancy of unknown location and a serum human chorionic gonadotropin (hCG) of 18 336 IU/mL. A diagnostic laparoscopy was performed but fallopian tubes were normal with no signs of ectopic pregnancy seen. An abdominal ultrasound scan was performed but did not identify the ectopic pregnancy. Owing to worsening symptoms of pain and rising hCG levels, she underwent a further laparoscopy converted to laparotomy and a left salpingo-oophorectomy for suspected left ovarian pregnancy. However, serum hCG levels continued to rise after the surgery, reaching 36 960 IU/mL. An magnetic resonance imaging scan of her abdomen and pelvis was arranged that showed a 4 cm cystic lesion in the segment V of the liver. Further ultrasound correlation showed a hyperechoic lesion with echogenic components suspicious of an ectopic pregnancy with a fetal pole. Fetal heart action was not visualized. A multidisciplinary team approach was adopted with involvement of the hepatobiliary surgical team, and the option of medical management with methotrexate and surgical excision was considered. A decision was made for surgical excision based on the accessible location of the ectopic pregnancy on segment V and the more controlled and predictable outcome with surgical excision. A preoperative computed tomography scan confirmed the lesion in segment V of liver in keeping with liver capsular implantation of ectopic pregnancy (Video still 1). At laparoscopy the ectopic pregnancy was visualized on the inferior surface of liver close to the inferior margin with a band of overlying omental adhesion (Video still 2). The overlying omental adhesions were sealed and cut with advanced bipolar diathermy, keeping a safe margin from the ectopic pregnancy to minimize any bleeding. The liver capsule was then opened with monopolar diathermy, and the small segment of liver with the ectopic pregnancy was excised using a combination of Bowa-Lotus liver blade (Bowa Medical Ltd). Hemostasis was controlled using Floseal hemostatic matrix and applied pressure laparoscopically. Total operating time was 80 minutes with an estimated blood loss of 500 mL. The patient was discharged on day 3 postoperatively, and follow-up serum hCG excluded residual trophoblastic disease. On review of the clinical case, earlier imaging of the upper abdomen when confronted by a persistent pregnancy of unknown location with high levels of serum hCG would have prevented the second laparoscopy, laparotomy, and salpingo-oophorectomy. In similar cases, it would also help exclude poorly differentiated malignancies as a source of serum hCG. Conclusion: Only 27 cases of ectopic pregnancy on the liver have been identified in English literature since 1952, based on a MEDLINE and Embase enquiry and further review of all case reports by the authors to avoid duplicates. Estimated incidence of hepatic implantation is 1 in 15 000 pregnancies; 4 case reports of laparoscopic liver resection have been identified and another case managed by suction from the liver surface [1,2]. The key principle demonstrated is to resect the ectopic pregnancy with a safe margin of liver tissue and any adhesions to avoid catastrophic bleeding from direct handling of the ectopic pregnancy.
Spilled gallstone mimicking intra-abdominal seeding of gallbladder adenocarcinoma: A case report

Abstract:BACKGROUND Gallbladder rupture is common in laparoscopic cholecystectomy because the gallbladder is usually in acute or chronic inflammation status. The gallstones may sometime be spilled into the peritoneal cavity, resulting in intra-abdominal abscess if the gallstones were not retrieved. The diagnosis of intra-abdominal abscess caused by unretrieved gallstone can usually be correctly identified in the routine imaging studies, such as abdominal ultrasonography or computed tomography (CT). Here we present a case of abscess formation from unretrieved gallstone following laparoscopic cholecystectomy, which mimics the imaging findings of metastatic gallbladder adenocarcinoma. CASE SUMMARY This case described a 78-year-old man who received laparoscopic cholecystectomy and gallbladder adenocarcinoma was diagnosed after surgery. After adjuvant chemotherapy, the following up abdominal CT showed several small nodules at right upper abdomen and peritoneal carcinomatosis is considered. Repeated laparoscopic surgery for the excision of seeding tumor was conducted and the pathological diagnosis of the nodules and mass was inflammatory tissues and gallbladder stone. CONCLUSION Spilled gallstones are a common complication during laparoscopic cholecystectomy and some gallstones fail to be retrieved due to the size or the restricted view of laparoscopic surgery. For spilled gall bladder stones, surgeons may consider regular computerized tomography follow-up, and if necessary, laparoscopic examination can be used as a means of confirming the diagnostic and treatment.

Abstract:Ectopic thyroid arises from abnormal development of thyroid primordial tissues as it migrates to the lower interstitium during the embryonic period, which can occur at various locations during the descent process. However, ectopic thyroid in the subdiaphragmatic area is extremely rare. In this case, we report a case of ectopic thyroid located in the hepatoduodenal ligament. The 60-year-old female patient was admitted to hospital with gallbladder stones and cholecystitis. Preoperative imaging showed a mass in the hepatoduodenal ligament. As the patient declined a needle biopsy of the mass, the nature of the mass remained unclear prior to surgery. The patient subsequently underwent laparoscopic cholecystectomy and exploratory resection of the mass. The histopathology of the resected mass showed the characteristics of ectopic thyroid, and immunohistochemical staining revealed positive expression of thyroid transcription factor-1 and thyroglobulin. The diagnosis of ectopic thyroid was established. Upon confirming the diagnosis, comprehensive neck examination revealed the presence of a normally functioning thyroid gland. Throughout the four-year follow-up period, the patient's thyroid ultrasonography and thyroid function tests indicated no abnormalities. Ectopic thyroid in the hepatoduodenal ligament and surrounding areas is an extremely rare clinical abnormality, achieving a clear diagnosis before initiating treatment offers diagnostic and treatment insights and clues for clinicians when differentiating masses within this region.

Abstract:Ectopic or heterotopic pancreas is defined as the presence of pancreatic tissue in an anatomical place not related to the pancreas, being it most frequent locations the stomach and small bowel. Its finding in the gallbladder is exceptional. Since the first case was reported by Otschkin in 1916, about 30 cases have been described in literature. We report the case of a 43 years-old male patient who had an urgent laparoscopic cholecystectomy with the diagnosis of acute cholecystitis, which pathological study showed the existence of chronic cholecystitis with heterotopic pancreatic tissue in the gallbladder wall.
