Abstract:Abstract Background Available data proving the value of DWI for breast cancer diagnosis is mainly for enhancing masses; DWI may be less sensitive and specific in non-mass enhancement (NME) lesions. The objective of this study was to assess the diagnostic accuracy of DWI using different ROI measurement approaches and ADC metrics in breast lesions presenting as NME lesions on dynamic contrast-enhanced (DCE) MRI. Methods In this retrospective study, 95 patients who underwent multiparametric MRI with DCE and DWI from September 2007 to July 2013 and who were diagnosed with a suspicious NME (BI-RADS 4/5) were included. Twenty-nine patients were excluded for lesion non-visibility on DWI ( n = 24: 12 benign and 12 malignant) and poor DWI quality ( n = 5: 1 benign and 4 malignant). Two readers independently assessed DWI and DCE-MRI findings in two separate randomized readings using different ADC metrics and ROI approaches. NME lesions were classified as either benign (> 1.3 × 10 −3 mm 2 /s) or malignant (≤ 1.3 × 10 −3 mm 2 /s). Histopathology was the standard of reference. ROC curves were plotted, and AUCs were determined. Concordance correlation coefficient (CCC) was measured. Results There were 39 malignant (59%) and 27 benign (41%) lesions in 66 (65 women, 1 man) patients (mean age, 51.8 years). The mean ADC value of the darkest part of the tumor (Dptu) achieved the highest diagnostic accuracy, with AUCs of up to 0.71. Inter-reader agreement was highest with Dptu ADC max (CCC 0.42) and lowest with the point tumor (Ptu) ADC min (CCC = − 0.01). Intra-reader agreement was highest with Wtu ADC mean (CCC = 0.44 for reader 1, 0.41 for reader 2), but this was not associated with the highest diagnostic accuracy. Conclusions Diagnostic accuracy of DWI with ADC mapping is limited in NME lesions. Thirty-one percent of lesions presenting as NME on DCE-MRI could not be evaluated with DWI, and therefore, DCE-MRI remains indispensable. Best results were achieved using Dptu 2D ROI measurement and ADC mean.

Abstract:Objective:This article is to investigate the potential of DWI in cervical lymph node lesions and focus on the technique parameters that affect the quality of DW imaging.Methods:Conventional MR imaging(spin echo T_1WI,T_2WI and STIR) was used to scan 30 patients with histologically proven malignant tumors(16) and inflammatory lesions(14) on a GE 1.5T scanner using cervical phased array coil.Six B values(30 cases) were acquired with diffusion gradients applied in all three orthogonal directions using body coil.Diffusion scan time was 40～52 seconds.Results:All benign and malignant lymph nodes were visible in DWI and showed significant hyperintensity signal.Calculated ADC malignant lymph nodes was statistically different from that of benign lymph nodes.When various of scan parameters were mixed rationally,MRI may improve signal-noise-ratio of image quality and lessen geometric malformation.Conclusion:Diffusion weighted imaging is a rapid and feasible method in detecting cervical lymph node lesions and shows a potential for distinguishing between benign and malignant lymph nodes.
Prediction of Prognostic Factors and Genotypes in Patients With Breast Cancer Using Multiple Mathematical Models of MR Diffusion Imaging

Abstract:Purpose: To explore the clinical value of apparent diffusion coefficient (ADC), intravoxel incoherent motion (IVIM), and diffusion kurtosis imaging (DKI) based on diffusion-weighted MRI (DW-MRI) for predicting genotypes and prognostic factors of breast cancer. Materials and methods: A total of 227 patients with breast cancer confirmed by pathology were reviewed retrospectively. Diffusion-weighted imaging (DWI), IVIM, and DKI were performed in all patients. The corresponding ADC, true diffusion coefficient (D), perfusion-related diffusion coefficient (D*), perfusion fraction (f), mean diffusion rate (MD), and mean kurtosis value (MK) were measured. Multivariate logistic regression analysis and receiver operating characteristic (ROC) curve were used to analyze the diagnostic efficacy in predicting the Nottingham prognostic index (NPI), the expression of antigen Ki-67, and the molecular subtypes of breast cancer. The nomogram of the combined genotype-prediction model was established based on the multivariate logistic regression model results. Results: D* and MK values were significantly higher in the high-grade Nottingham group (NPI ≥ 3.4) than the low-grade Nottingham group (NPI < 3.4) (p < 0.01). When D* ≥ 30.95 × 10-3 mm2/s and MK ≥ 0.69, the NPI tended to be high grade (with areas under the curve (AUCs) of 0.712 and 0.647, respectively). The combination of D* and MK demonstrated the highest AUC of 0.734 in grading NPI with sensitivity and accuracy of 71.7% and 77.1%, respectively. Additionally, higher D*, f, and MK and lower ADC and D values were observed in the high Ki-67 than low Ki-67 expression groups (p < 0.05). The AUC of the combined model (D + D* + f + MK) was 0.755, being significantly higher than that of single parameters (Z = 2.770~3.244, p = 0.001~0.006) in distinguishing high from low Ki-67 expression. D* and f values in the Luminal A subtype were significantly lower than in other subtypes (p < 0.05). Luminal B showed decreased D value compared with other subtypes (p < 0.05). The HER-2-positive subtype demonstrated increased ADC values compared with the Luminal B subtype (p < 0.05). Luminal A/B showed significantly lower D, D*, MD, and MK than the non-Luminal subtypes (p < 0.05). The combined model (D + D* + MD + MK) showed an AUC of 0.830 in diagnosing the Luminal and non-Luminal subtypes, which is significantly higher than that of a single parameter (Z = 3.273~4.440, p < 0.01). f ≥ 54.30% [odds ratio (OR) = 1.038, p < 0.001] and MK ≥ 0.68 (OR = 24.745, p = 0.012) were found to be significant predictors of triple-negative subtypes. The combination of f and MK values demonstrated superior diagnostic performance with AUC, sensitivity, specificity, and accuracy of 0.756, 67.5%, 77.5%, and 82.4%, respectively. Moreover, as shown in the calibration curve, strong agreements were observed between nomogram prediction probability and actual findings in the prediction of genotypes (p = 0.22, 0.74). Conclusion: DWI, IVIM, and DKI, as MR diffusion imaging techniques with different mathematical models showed potential to identify the prognosis and genotype of breast cancer. In addition, the combination of these three models can improve the diagnostic efficiency and thus may contribute to opting for an appropriate therapeutic approach in clinic treatment.

Abstract:Abstract Background This study aimed to evaluate the utility of radiomics-based machine learning analysis with multiparametric DWI and to compare the diagnostic performance of radiomics features and mean diffusion metrics in the characterization of breast lesions. Methods This retrospective study included 542 lesions from February 2018 to November 2018. One hundred radiomics features were computed from mono-exponential (ME), biexponential (BE), stretched exponential (SE), and diffusion-kurtosis imaging (DKI). Radiomics-based analysis was performed by comparing four classifiers, including random forest (RF), principal component analysis (PCA), L1 regularization (L1R), and support vector machine (SVM). These four classifiers were trained on a training set with 271 patients via ten-fold cross-validation and tested on an independent testing set with 271 patients. The diagnostic performance of the mean diffusion metrics of ME (mADC all b , mADC 0–1000 ), BE (mD, mD * , mf), SE (mDDC, mα), and DKI (mK, mD) were also calculated for comparison. The area under the receiver operating characteristic curve (AUC) was used to compare the diagnostic performance. Results RF attained higher AUCs than L1R, PCA and SVM. The AUCs of radiomics features for the differential diagnosis of breast lesions ranged from 0.80 (BE_D*) to 0.85 (BE_D). The AUCs of the mean diffusion metrics ranged from 0.54 (BE_mf) to 0.79 (ME_mADC 0–1000 ). There were significant differences in the AUCs between the mean values of all diffusion metrics and radiomics features of AUCs (all P < 0.001) for the differentiation of benign and malignant breast lesions. Of the radiomics features computed, the most important sequence was BE_D (AUC: 0.85), and the most important feature was FO-10 percentile (Feature Importance: 0.04). Conclusions The radiomics-based analysis of multiparametric DWI by RF enables better differentiation of benign and malignant breast lesions than the mean diffusion metrics.

Treatment of chronic urticaria with colchicine.

Abstract:BACKGROUND Chronic urticaria (CU) is a cutaneous disease that can be debilitating, difficult to treat, and sometimes life-threatening. Treatment with antihistamines is often ineffective. Immunosuppressants are second line therapy but can have significant side effects. Data is needed on effective therapies with safer profiles. OBJECTIVES To determine the efficacy and side-effects of colchicine in patients with CU. METHODS Patients were identified through retrospective chart reviews at the University of Utah from 2002-2007. We identified 36 patients with a diagnosis of chronic urticaria based on history, physical examination, and a skin biopsy. Length of treatment ranged from one month to 17 months. RESULTS Subjective clinical responses to colchicine therapy reported as complete (n=15) or partial (n=5) were found in 56 percent of patients. The mean±SD duration of treatment was 7±6 months. Three patients (15%) who had resolution of urticaria stopped colchicine secondary to diarrhea and hematuria. Of the complete responders, nine individuals (60%) have remained symptom free and four individuals (27%) had recurrence after colchicine was stopped. LIMITATIONS Short-term follow-up and retrospective study design. CONCLUSIONS This retrospective study demonstrated that colchicine was an effective and well-tolerated treatment for patients unresponsive to antihistamines. The data supports the use of colchicine for CU patients and further controlled studies are warranted to better characterize the use of colchicine in patients with CU refractory to antihistamines.

Abstract:Rationale and ObjectivesTo evaluate the value of adding Diffusion Weighted Imaging (DWI) with Apparent Diffusion Coefficient (ADC) mapping to dynamic contrast enhanced (DCE-MRI) to distinguish benign from malignant pathology subtypes and tumor recurrence.Method and MaterialsIn this retrospective IRB approved study, 956 consecutive patients underwent bilateral breast MRI between 1/2015 and 12/2015, with 156 BIRADS 4, 5, or 6 lesions detected in 111 patients. DWI imaging at B0, B100, B600, B1000 was performed with DCE-MRI. Values for diffusion and ADC images were recorded by two fellowship-trained breast radiologists. Mean ADC and signal intensity (SI) values were correlated with histology, tumor grade, hormone receptors (ER, PR, and HER-2)and Oncotype DX scores, when available. p ≤ 0.05 was considered significant.ResultsOf 156 lesions, there were 59 (38%) benign lesions, 24 (15%) Ductal Carcinoma In-Situ, 47 (30%) Invasive Ductal Carcinoma (IDC), 15 (10%) Invasive Lobular Carcinoma (ILC) and 2 (2%) Mucinous carcinoma (MC), five (5%) mixed IDC and ILC, and four (4%) other, including tubular and rare types of malignancy. Mean ADC values for malignancy were significantly lower than for benign lesions (1085 ± 343 × 10−6 vs 1481 ± 276 × 10−6 mm2/s), which is highly predictive (area under curve = 0.82). In addition, tumors with PR negativity and Oncotype score ≥18 (intermediate to high risk for recurrence) demonstrated significantly lower ADC values. SI at B100 and B600 was helpful in distinguishing benign versus IDC. There was no significant correlation between ADC values and tumor grade or ER/HER2 status.ConclusionADC value is important factor in distinguishing malignancy, differentiating tumors with higher Oncotype score, and PR negativity. Therefore, it can be used as an important tool to assist appropriate treatment selection.

Intravoxel Incoherent Motion Mr Imaging In Breast Cancer: Quantitative Analysis For Characterizing Lesions

Abstract:Objectives: We compared the monoexponential model diffusion weight imaging (DWI) and intravoxel incoherent motion DWI (IVIM-DWI) parameters for characterizing different subtypes and grades of breast cancer lesions. Methods: We analyzed 93 patients (51 with breast cancers and 47 with benign lesions) using monoexponential (b=0.800 s/mm(2)) and biexponential analysis (b=0-800 s/mm(2)) and 3.0T MRI. We compared the apparent diffusion coefficient (ADC), true diffusion coefficient (D), perfusion fraction (f) and pseudo-diffusion coefficient (D*) in different subtypes and grades of cancers. We used receiver operating characteristics (ROC) to identify the best cutoff for differentiation between lesions. Results: All data showed good fit (R-2>0.90). The D value was significantly lower than ADC in all the lesions except cysts (P<0.05). ADC, D and D* in malignant tumors were significantly lower than those in benign ones (P<0.001 for ADC and D; 0.002 for D*). The f value was higher than that of benign lesions (P<0.001). D showed higher accuracy than ADC in differentiating malignancy from benign lesions (AUC=0.945 for D; 0.931 for ADC), or ductal carcinoma in situ (DCIS) from invasive ductal carcinoma (IDC) (AUC=0.791 for D; 0.675 for ADC). Furthermore, the D value was significantly lower in high grade IDC (TNM III) than in low grade IDC (TNM I+II) (P=0.006), while no other parameters showed a difference between varied grades of IDC (P>0.05). Conclusions: IVIM provides quantitative measurement of cellularity and vascularity for characterizing breast lesions. In particular, D shows good potential for grading breast cancer.

Abstract:<h3 class="Heading">Background</h3><p class="Para">Diffusion-weighted imaging (DWI) can increase breast MRI diagnostic specificity due to the tendency of malignancies to restrict diffusion. Diffusion tensor imaging (DTI) provides further information over conventional DWI regarding diffusion directionality and anisotropy. Our study evaluates DTI features of suspicious breast lesions detected on MRI to determine the added diagnostic value of DTI for breast imaging</p><h3 class="Heading">Methods</h3><p class="Para">With IRB approval, we prospectively enrolled patients over a 3-year period who had suspicious (BI-RADS category 4 or 5) MRI-detected breast lesions with histopathological results. Patients underwent multiparametric 3 T MRI with dynamic contrast-enhanced (DCE) and DTI sequences. Clinical factors (age, menopausal status, breast density, clinical indication, background parenchymal enhancement) and DCE-MRI lesion parameters (size, type, presence of washout, BI-RADS category) were recorded prospectively by interpreting radiologists. DTI parameters (apparent diffusion coefficient [ADC], fractional anisotropy [FA], axial diffusivity [<em class="EmphasisTypeItalic">λ</em><sub>1</sub>], radial diffusivity [(<em class="EmphasisTypeItalic">λ</em><sub>2</sub> + <em class="EmphasisTypeItalic">λ</em><sub>3</sub>)/2], and empirical difference [<em class="EmphasisTypeItalic">λ</em><sub>1</sub> − <em class="EmphasisTypeItalic">λ</em><sub>3</sub>]) were measured retrospectively. Generalized estimating equations (GEE) and least absolute shrinkage and selection operator (LASSO) methods were used for univariate and multivariate logistic regression, respectively. Diagnostic performance was internally validated using the area under the curve (AUC) with bootstrap adjustment.</p><h3 class="Heading">Results</h3><p class="Para">The study included 238 suspicious breast lesions (95 malignant, 143 benign) in 194 women. In univariate analysis, lower ADC, axial diffusivity, and radial diffusivity were associated with malignancy (OR = 0.37–0.42 per 1-SD increase, <em class="EmphasisTypeItalic">p</em> < 0.001 for each), as was higher FA (OR = 1.45, <em class="EmphasisTypeItalic">p</em> = 0.007). In multivariate analysis, LASSO selected only ADC (OR = 0.41) as a predictor for a DTI-only model, while both ADC (OR = 0.41) and FA (OR = 0.88) were selected for a model combining clinical and imaging parameters. Post-hoc analysis revealed varying association of FA with malignancy depending on the lesion type. The combined model (AUC = 0.81) had a significantly better performance than Clinical/DCE-MRI-only (AUC = 0.76, <em class="EmphasisTypeItalic">p</em> < 0.001) and DTI-only (AUC = 0.75, <em class="EmphasisTypeItalic">p</em> = 0.002) models.</p><h3 class="Heading">Conclusions</h3><p class="Para">DTI significantly improves diagnostic performance in multivariate modeling. ADC is the most important diffusion parameter for distinguishing benign and malignant breast lesions, while anisotropy measures may help further characterize tumor microstructure and microenvironment.</p>

Abstract:Objective To compare the value of diffusion kurtosis imaging (DKI) model with single-index DWI model parameters in the differential diagnosis of benign and malignant breast lesions,and to explore the correlation between the parameters and molecular subtypes and prognostic factors of breast cancer.Methods A retrospective analysis was performed with inclusion of 64 cases of breast diseases from January 2016 to May 2017 in Shanghai First People's Hospital.The patients were pathologically confirmed and typed, 30 cases are malignant tumors and 34 cases are benign lesions. DKI and DWI were performed within 2 weeks before the pathological examination. Invasive ductal carcinoma of grade Ⅰ, Ⅱ and Ⅲ were revealed in 1, 7 and 13 cases respectively. Luminal A breast cancer was found in 10 cases, Luminal B breast cancer was diagnosed in 11 cases, HER-2 positive breast cancer was 4 cases and triple negative breast cancer was 5 cases. The expressions of estrogen receptor (ER), Progesterone receptor (PR), and HER-2 positive were found in 20, 14 and 15 cases respectively. Ki67 was highly expressed in 24 cases and low expression in 6 cases. All patients underwent both plain and enhanced mammography scanning. The kurtosis (MK), mean diffusivity (MD) and ADC value were measured. Prognosis analysis was performed according to the maximum diameter (>2 cm, ≤2 cm), vascular or neurological invasion (positive, negative), lymph node metastasis (positive, negative), ER (positive, negative), PR (positive, negative), HER-2 (positive, negative),Ki67 (positive, negative), pathological grade (grade Ⅰ+Ⅱ,Ⅲ). Two independent samples t test was used to compare DKI and DWI parameters between benign and malignant lesions. ROC analysis was performed for assessing the values of parameters in discriminating benign and malignant breast lesions. Mann-Whitney U and Kruskal-Wallis H tests were used for the comparison of various prognostic factors or molecular subtypes.Spearman rank correlation analysis was used to explore the correlation of different prognostic factors and DKI and DWI parameters. Results The MK value of malignant group was higher than that of benign group,and the MD value and ADC value were lower than that of benign group (P<0.05). The area under the ROC for MK, MD and ADC were 0.897, 0.827 and 0.776, respectively. The area under the ROC was improved to 0.935 when three parameters were combined. The MK of ER positive group was higher than that of negative group (P<0.05). There was no significant difference of parameters among the other prognostic groups (all P>0.05). There was a low positive correlation between ER and MK (r= 0.417, P= 0.022). There was no correlation between the other prognostic factors and parameters (r=－0.086 to 0.313, all P>0.05). There was no significant difference in the MD, MK and ADC values among the four different subtypes of breast cancer (all P>0.05). Conclusions MK, MD and ADC values can be used to discriminate benign and malignant breast tumors, among which MK value has the best diagnostic performance. There is a certain correlation between DKI model parameters and prognostic factors.
Evaluation of breast cancer malignancy, prognostic factors and molecular subtypes using a continuous-time random-walk MR diffusion model

Abstract:PURPOSE: To assess the continuous-time random-walk (CTRW) model's diagnostic value in breast lesions and to explore the associations between the CTRW parameters and breast cancer pathologic factors.METHOD: This retrospective study included 85 patients (70 malignant and 18 benign lesions) who underwent 3.0T MRI examinations. Diffusion-weighted images (DWI) were acquired with 16b-values to fit the CTRW model. Three parameters (D<sub>m</sub>, α, and β) derived from CTRW and apparent diffusion coefficient (ADC) from DWI were compared among the benign/malignant lesions, molecular prognostic factors, and molecular subtypes by Mann-Whitney U test. Spearman correlation was used to evaluate the associations between the parameters and prognostic factors. The diagnostic performance was assessed by the area under the receiver operating characteristic curve (AUC) based on the diffusion parameters.RESULTS: All parameters, ADC, D<sub>m</sub>, α, and β were significantly lower in the malignant than benign lesions (P < 0.05). The combination of all the CTRW parameters (D<sub>m</sub>, α, and β) provided the highest AUC (0.833) and the best sensitivity (94.3%) in differentiating malignant status. And the positive status of estrogen receptor (ER) and progesterone receptor (PR) showed significantly lower β compared with the negative counterparts (P < 0.05). The high Ki-67 expression produced significantly lower D<sub>m</sub> and ADC values (P < 0.05). Additionally, combining multiple CTRW parameters improved the performance of diagnosing molecular subtypes of breast cancer. Moreover, Spearman correlations analysis showed that β produced significant correlations with ER, PR and Ki-67 expression (P < 0.05).CONCLUSIONS: The CTRW parameters could be used as non-invasive quantitative imaging markers to evaluate breast lesions.

Abstract:BackgroundPurposeBreast MRI is a sensitive imaging technique to assess breast cancer but its effectiveness still remains to be improved. To evaluate the diagnostic performance of diffusion-weighted imaging (DWI), diffusion kurtosis imaging (DKI), and quantitative dynamic contrast-enhanced (DCE)-MRI in differentiating malignant from benign breast lesions independently or jointly and to explore whether correlations exist among these parameters. Study TypePopulationRetrospective. In all, 106 patients with breast lesions (47 malignant, 59 benign). SequenceAssessmentDKI sequence with seven b values and quantitative DCE sequence on 3.0T MRI. Diffusion parameters (mean diffusivity [MD], mean diffusivity [MK], and apparent diffusion coefficient [ADC]) from DKI and DWI and perfusion parameters from DCE (K-trans, k(ep), v(e), and v(p)) were calculated by two experienced radiologists after postprocessing. Disagreement between the two observers was resolved by consensus. Statistical TestsResultsThe parameters in benign and malignant lesions were compared by Student's t-test. The diagnostic performances of DKI and quantitative DCE, either alone or in combination, were evaluated by receiver operating characteristic (ROC) analysis. The Spearman correlation test was used to evaluate correlations among the diffusion parameters and perfusion parameters. MK, MD, ADC, K-trans, and k(ep) values were significantly different between breast cancer and benign lesions (P < 0.05). MK from DKI demonstrated the highest AUC of 0.849, which is significantly higher than ADC derived from conventional DWI (z = 3.345, P = 0.0008). The specificity of DCE-MRI-derived parameters was improved when combining diffusion parameters, such as ADC and MK. The highest diagnostic specificity (93.2%) was obtained when k(ep) and ADC were combined. k(ep) was correlated moderately positively with MK (r = 0.516) and moderately negatively with MD (r = -0.527). K-trans was weakly positively correlated with MK with an r of 0.398 and weakly negatively correlated with MD with an r of -0.450. Data ConclusionDKI is more valuable than conventional DWI in distinguishing between benign and malignant breast lesions. DKI exhibits promise as a quantitative technique to augment quantitative DCE-MRI. Diffusion parameters derived from DKI were statistically correlated with perfusion parameters from quantitative DCE-MRI. Level of Evidence: 3 Technical Efficacy: Stage 2 J. Magn. Reson. Application of Monoexponential, Biexponential, and Stretched-Exponential Models of Diffusion-Weighted Magnetic Resonance Imaging in the Differential Diagnosis of Metastases and Myeloma in the Spine-Univariate and Multivariate Analysis of Related Parameters.

Abstract:Objective: To explore the value of related parameters in monoexponential, biexponential, and stretched-exponential models of diffusion-weighted imaging (DWI) in differentiating metastases and myeloma in the spine. Methods: 53 metastases and 16 myeloma patients underwent MRI with 10 b-values (O-1500 s/mm(2)). Parameters of apparent diffusion coefficient (ADC), true diffusion coefficient (D), pseudo-diffusion coefficient (D*), perfusion fraction (f), the distribution diffusion coefficient (DDC), and intravoxel water diffusion heterogeneity (alpha) from DWI were calculated. The independent sample t test and the Mann-Whiney U test were used to compare the statistical difference of the parameter values between the two. Receiver operating characteristics (ROC) curve analysis was used to identify the diagnostic efficacy. Then substituted each parameter into the decision tree model and logistic regression model, identified meaningful parameters, and evaluated their joint diagnostic performance. Results: The ADC, D. and alpha values of metastases were higher than those of myeloma, whereas the D* value was lower than that of myeloma, and the difference was significant (p < 0.05); the area under the ROC curve for the above parameters was 0.661, 0.710, 0.781, and 0.743, respectively. There was no significant difference in the f and DDC values (p > 0.05). D and a were found to conform to the decision tree model, and the accuracy of model diagnosis was 84.1%. ADC and a were found to conform to the logistic regression model, and the accuracy was 87.0%. Conclusion: The 3 models of DWI have certain values indifferentiating metastases and myeloma in spine, and the diagnostic performance of ADC, D, alpha and D* Was better. Combining ADC with a may markedly aid in the differential diagnosis of the two. Advances In knowledge: Monoexponential, biexponential, and stretched-exponential models can offer additional information in the differential diagnosis of metastases and myeloma in the spine. Decision tree model and logistic regression model are effective methods to help further distinguish the two.
Multiparameter MRI Model With DCE-MRI, DWI, and Synthetic MRI Improves the Diagnostic Performance of BI-RADS 4 Lesions

Abstract:Objectives To evaluate the value of synthetic magnetic resonance imaging (syMRI), diffusion-weighted imaging (DWI), DCE-MRI, and clinical features in breast imaging–reporting and data system (BI-RADS) 4 lesions, and develop an efficient method to help patients avoid unnecessary biopsy. Methods A total of 75 patients with breast diseases classified as BI-RADS 4 (45 with malignant lesions and 30 with benign lesions) were prospectively enrolled in this study. T1-weighted imaging (T1WI), T2WI, DWI, and syMRI were performed at 3.0 T. Relaxation time (T1 and T2), apparent diffusion coefficient (ADC), conventional MRI features, and clinical features were assessed. “T” represents the relaxation time value of the region of interest pre-contrast scanning, and “T+” represents the value post-contrast scanning. The rate of change in the T value between pre- and post-contrast scanning was represented by ΔT%. Results ΔT1%, T2, ADC, age, body mass index (BMI), menopause, irregular margins, and heterogeneous internal enhancement pattern were significantly associated with a breast cancer diagnosis in the multivariable logistic regression analysis. Based on the above parameters, four models were established: model 1 (BI-RADS model, including all conventional MRI features recommended by BI-RADS lexicon), model 2 (relaxation time model, including ΔT1% and T2), model 3 [multi-parameter (mp)MRI model, including ΔT1%, T2, ADC, margin, and internal enhancement pattern], and model 4 (combined image and clinical model, including ΔT1%, T2, ADC, margin, internal enhancement pattern, age, BMI, and menopausal state). Among these, model 4 has the best diagnostic performance, followed by models 3, 2, and 1. Conclusions The mpMRI model with DCE-MRI, DWI, and syMRI is a robust tool for evaluating the malignancies in BI-RADS 4 lesions. The clinical features could further improve the diagnostic performance of the model.

Abstract:Background: Imageology uses high-throughput and automatic computing methods to transform medical image data into quantitative data with feature space, and then makes accurate quantitative analysis, extracts features and builds models, which can intuitively observe the overall features of lesions and the surrounding tissues, and provide rich invisible information. At present, the research on the imaging features of dynamic contrast-enhanced (DCE) and diffusion-weighted imaging (DWI) to predict the molecular typing value has achieved results, but the imaging model based on DWI and DCE-magnetic resonance imaging (MRI) is not enough to predict the molecular subtypes, and the prediction value of the prediction model based on the three-dimensional volume of interest of the lesion to the four molecular subtypes of breast cancer has not been fully studied.Methods: The clinical data of 202 breast cancer patients at our hospital from October 2020 to November 2021 were collected. All patients were examined with multimodal MRI before surgery. Base on immunohistochemical recombinant Ki-67 protein (Ki-67), estrogen receptor (ER), human epidermal growth factor receptor-2 (HER-2) and progesterone receptor (PR) results, the tumors were divided into four types According to the results of the sentinel lymph node (SLN) biopsies, the patients were divided into SLN (+) and SLN (-) groups. 3-dimensional (3D) Slicer software was used to outline the region of interest (ROI), and AMni-Kinetics software was used for feature extraction. The imaging characteristics were screened using least absolute shrinkage and selection operator (LASSO)-Logistic regression model using R statistical software, and the receiver operating characteristic (ROC) curve was drawn using "pROC" software package to evaluate the prediction efficiency of the model.Results: The most efficacious model at predicting SLN (+) in breast cancer patients with different molecular subtypes and SLN metastasis was the model based on the imageological characteristics of fat inhibition, and T2-weighted imaging (T2WI), T1-weighted imaging + C (T1WI-C), and DWI combined sequences at the tumor + 2 mm periphery. AUC (sensitivity, specificity) of the validation group were 0.831 (0.856, 0.891), 0.832 (0.660, 0.877), 0.801 (0.772, 0.765), 0.904 (0.769, 0.873), and 0.819 (0.810, 0.500) respectively when the tumor was 2 mm around the tumor.Conclusions: The imaging features extracted from multi-parameter DWI, T1WI+C, and T2WI in breast cancer have certain value at predicting different molecular types and SLNs of breast cancer.

Abstract:ObjectiveTo determine the preoperative magnetic resonance imaging (MRI) findings of breast cancer on dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) and diffusion-weighted magnetic resonance imaging (DWI) in different molecular subtypes.Materials and methodsA retrospective study was conducted on 116 breast cancer subjects who underwent preoperative MRI and surgery or biopsy. Three radiologists retrospectively assessed the morphological and kinetic characteristics on DCE-MRI and tumor detectability on DWI, by using apparent diffusion coefficient (ADC) values of lesions. The clinicopathologic and MRI features of four subtypes were compared. The correlation between clinical and MRI findings with molecular subtypes was evaluated using the chi-square and ANOVA tests, while the Mann–Whitney test was used to analyze the relationship between ADC and prognostic factors.ResultsOne hundred and sixteen women diagnosed with breast cancer confirmed by surgery or biopsy had the following subtypes of breast cancer: luminal A (27, 23.3%), luminal B (56, 48.2%), HER2 positive (14, 12.1%), and triple-negative breast cancer (TNBC) (19, 16.4%), respectively. Among the subtypes, significant differences were found in axillary node metastasis, histological grade, tumor shape, rim enhancement, margin, lesion type, intratumoral T2 signal intensity, Ki-67 index, and paratumoral enhancement (p < 0.001, p < 0.001, p < 0.001, p < 0.001, p < 0.001, p < 0.001, p < 0.001, p < 0.001, and p = 0.02, respectively). On DWI, the mean ADC value of TNBC (0.910 × 10−3 mm2/s) was the lowest compared to luminal A (1.477×10−3 mm2/s), luminal B (0.955 × 10−3 mm2/s), and HER2 positive (0.996 × 10−3 mm2/s) (p < 0.001). Analysis of the correlation between different prognostic factors and ADC value showed that only axillary lymph node status and ADC value had a statistically significant difference (p = 0.009).ConclusionThe morphologic features of MRI can be used as imaging biomarkers to identify the molecular subtypes of breast cancer. In addition, quantitative assessments of ADC values on DWI may also provide biological clues about molecular subtypes.
Multiple mathematical models of diffusion-weighted imaging for endometrial cancer characterization: Correlation with prognosis-related risk factors

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Purpose</h3><p>To investigate mono-exponential, bi-exponential, and stretched-exponential models of diffusion-weighted imaging (DWI) for evaluation of prognosis-related risk factors of endometrial cancer (EC).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Method</h3><p>Sixty-one consecutive patients with EC who preoperatively underwent pelvic MRI with multiple b value DWI between September 2016 and May 2018 were enrolled. The apparent-diffusion-coefficient (ADC), bi-exponential model parameters (D, D* and f) and stretched-exponential model parameters (DDC and α) were measured and compared to analyze the following prognosis-related risk factors confirmed by pathology: histological grade, depth of myometrial invasion, cervical stromal infiltration (CSI) and lymphovascular invasion (LVSI). A stepwise multilvariate logistic regression and the receiver operating characteristic (ROC) curves were performed for further statistical analysis.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Lower ADC, D, f, and DDC were observed in tumor with high grade compared with a low-grade group, and the largest area under curve (AUC) was obtained when combining f and DDC values. ADC, D, f, DDC, and α were significantly different in patients with deep myometrial invasion (DMI) compared to those without DMI; the combination of f, DDC and α showed the highest AUC. Significantly different ADC and f were found between patients' presence and absence CSI; the f values showed the highest diagnostic performance with an AUC of 0.825. Regarding the LVSI, ADC, D*, f, and DDC were significantly lower in tumors with LVSI compared to those without LVSI; the combination of f and DDC showed the largest AUC.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Multiple mathematical DWI models are a useful approach for the prediction of prognosis-related risk factors in EC.</p>

Diffusion-weighted MRI of the Breast: Lesion Characterization and Parameter Selection

Abstract:Objective To evaluate the b value of diffusion-weighted(DW)MRI in distinguishing between benign and malignant breast lesions.Methods Three diffusion-weighted sequences were implemented with 500,1000 and 2000 s/mm~2 b values respectively on 95 breast lesions in 83 patients.All lesions were confirmed by pathology.The apparent diffusion coefficient(ADC)values and signal intensity (SI)were recorded and compared in different lesions(breast cancer,benign lesion,cyst and normal beast tissue)with the same b value and the same lesions with the different b values.Results(1)The mean ADC value and SI of breast cancer were 1.375±0.378 and 839.713±360.493 respectively with b= 500 s/mm~2,1.176±0.311 and 459.314±229.609 with b=1000 s/mm~2,0.824±0.198 and 243.825± 110.616 with b=2000 s/mm~2.The differences in the mean ADC value were significant between two type lesions(cancer and benign lesion,cancer and cyst,cancer and normal breast tissue)with b values of 500 s/mm~2 and 1000 s/mm~2.But the significant differenee was only seen between cancer and benign lesions when b value was 2000 s/mm~2.(2)The one-side upper limits of 95% confidence interval of mean ADCs were adopted as the point to separate the malignant from the benign lesions,the sensitivity was 70.92%, 70.73% and 69.77%,the specificity was 77.19%,75.70% and 54.76%,the accuracy was 77.12%, 74.32% and 62.35% respectively with b values of 500 s/mm~2,1000 s/mm~2 and 2000 s/mm~2.The areas under ROC eurves were Az_(500)=0.775±0.046(P0.01),Az_(1000)=0.780±0.044(P0.01)and Az_(2000)=0.620±0.062(P0.05).Conclusion DWI MRI is useful for the differential diagnosis of breast lesions with b values of 500 s/mm~2 and 1000 s/mm~2.
Differentiation of Clinically Benign and Malignant Breast Lesions Using Diffusion-Weighted Imaging

Abstract:PURPOSE:To evaluate the value of diffusion-weighted imaging (DWI) in distinguishing between benign and malignant breast lesions.MATERIALS AND METHODS:Fifty-two female subjects (mean age = 58 years, age range = 25-75 years) with histopathologically proven breast lesions underwent DWI of the breasts with a single-shot echo-planar imaging (EPI) sequence using large b values. The computed mean apparent diffusion coefficients (ADCs) of the breast lesions and cell density were then correlated.RESULTS:The ADCs varied substantially between benign breast lesions ((1.57 +/- 0.23) x 10(-3) mm(2)/second) and malignant breast lesions ((0.97 +/- 0.20) x 10(-3) mm(2)/second). In addition, the mean ADCs of the breast lesions correlated well with tumor cellularity (P < 0.01, r = -0.542).CONCLUSION:The ADC would be an effective parameter in distinguishing between malignant and benign breast lesions. Further, tumor cellularity has a significant influence on the ADCs obtained in both benign and malignant breast tumors.

Abstract:BACKGROUND: Diffusion-weighted imaging (DWI) with apparent diffusion coefficient (ADC) mapping is one of the most useful additional MRI parameters to improve diagnostic accuracy and is now often used in a multiparameric imaging setting for breast tumor detection and characterization.PURPOSE: To evaluate whether different ADC metrics can also be used for prediction of receptor status, proliferation rate, and molecular subtype in invasive breast cancer.STUDY TYPE: Retrospective.SUBJECTS: In all, 107 patients with invasive breast cancer met the inclusion criteria (mean age 57 years, range 32-87) and underwent multiparametric breast MRI.FIELD STRENGTH/SEQUENCE: 3 T, readout-segmented echo planar imaging (rsEPI) with IR fat suppression, dynamic contrast-enhanced (DCE) T<sub>1</sub> -weighted imaging, T<sub>2</sub> -weighted turbo-spin echo (TSE) with fatsat.ASSESSMENT: Two readers independently drew a region of interest on ADC maps on the whole tumor (WTu), and on its darkest part (DpTu). Minimum, mean, and maximum ADC values of both WTu and DpTu were compared for receptor status, proliferation rate, and molecular subtypes.STATISTICAL TESTS: Wilcoxon rank sum, Mann-Whitney U-tests for associations between radiologic features and histopathology; histogram and q-q plots, Shapiro-Wilk's test to assess normality, concordance correlation coefficient for precision and accuracy; receiver operating characteristics curve analysis.RESULTS: Estrogen receptor (ER) and progesterone receptor (PR) status had significantly different ADC values for both readers. Maximum WTu (P = 0.0004 and 0.0005) and mean WTu (P = 0.0101 and 0.0136) were significantly lower for ER-positive tumors, while PR-positive tumors had significantly lower maximum WTu values (P = 0.0089 and 0.0047). Maximum WTu ADC was the only metric that was significantly different for molecular subtypes for both readers (P = 0.0100 and 0.0132) and enabled differentiation of luminal tumors from nonluminal (P = 0.0068 and 0.0069) with an area under the curve of 0.685 for both readers.DATA CONCLUSION: Maximum WTu ADC values may be used to differentiate luminal from other molecular subtypes of breast cancer.LEVEL OF EVIDENCE: 3 Technical Efficacy: Stage 2 J. Abstract:Objective To evaluate the diagnostic efficacy of MRI diffusion kurtosis imaging (DKI) and quantitative dynamic contrast enhancement MRI (DCE-MRI) in benign and malignant breast lesions, and to explore the differential diagnosis ability for different pathological types and molecular subtype lesions. Methods Sixty four females were retrospectively enrolled in the study of MRI diffusion kurtosis imaging and quantitative dynamic contrast enhancement between November 20 and May 2017. All of them were confirmed to have benign or malignant lesions after surgical resection or puncture. All patients underwent axial T1WI, DKI and DCE-MRI examinations. The mean kurtosis (MK) and mean diffusivity (MD) values were calculated by the DKI model, and the hemodynamic parameters were obtained by quantitative dynamic contrast enhancement, including volume transfer constant (Ktrans), rate constant (Kep), extravascular extracellular space distribute volume per unit tissue volume (Ve) and blood volume fraction (Vp). Pathological analysis was performed to monitor the expression of estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor 2 (HER-2) and nuclear proliferation index Ki67. The breast cancer was divided into Luminal A type, Luminal B type, HER-2 positive and triple-negative 4 subtypes. The differences of DKI parameters and DCE-MRI parameters between benign and malignant breast lesions were compared using two independent samples t test (normal distribution and homogeneity of variance) or Mann-Whitney U test (skewed distribution or variance). ROC analysis was used to evaluate the value of DKI and DCE-MRI parameters in differential diagnosis of benign and malignant breast lesions with pathological results as the gold standard. The Mann-Whitney U test and Kruskal-Wallis H test were used to compare the differences of DKI and DCE-MRI parameters among different prognostic factors and molecular subtypes of breast cancer. Spearman rank correlation analysis was used to evaluate the correlation between DKI and DCE-MRI parameters and different prognostic factors. Results Sixty-four cases were single lesions, with breast cancer in 23 cases and 41 cases of benign lesions. In breast cancer, there were 9 cases of Luminal A type, 7 cases of Luminal B type, 3 cases of HER-2 positive type and 4 cases of triple negative type. The positive numbers of ER, PR and HER-2 were 14, 11 and 10 cases respectively. Nineteen cases showed high expression of Ki67, while 4 cases showed low expression. There were significant differences in MK, MD, Ktrans and Kep between benign and malignant lesions (P<0.05). However, there was no significant difference between Ve and Vp (P>0.05). The area under the ROC for the differential diagnosis of benign and malignant breast lesions were 0.897, 0.808, 0.844 and 0.842, respectively. The combined multi-parameter differential diagnosis improved the efficacy. Combined with the above four parameters, the area under the ROC was 0.950. The diagnosis Sensitivity, specificity and accuracy were 0.870, 0.951 and 0.922 respectively. The Ktrans and Vp values of patients with ER positive and ER negative, as well as Ve value of deferent lymph node status, were significantly different (P<0.05), but there was no significant difference between the other prognostic factors (P>0.05). There was a moderate positive correlation between ER and Ktrans and Vp values. There was a low positive correlation between lymph node status and Ve value (r= 0.6, 0.5 and 0.4, respectively, P<0.05). No correlation was found among other parameters and prognostic factors (P>0.05). There were no significant differences in DKI and DCE-MRI parameters among different subtypes of breast cancer patients (all P>0.05). Conclusion DKI combined with DCE-MRI can improve the differential diagnosis of breast lesions and some DCE-MRI parameters are related to prognostic factors.
