Abstract:OBJECTIVES The aim of this study was to assess the feasibility and efficacy of left bundle branch pacing (LBBP) using a novel intraseptal technique to deliver cardiac resynchronization therapy (CRT) in patients with left bundle branch block (LBBB) and nonischemic cardiomyopathy. BACKGROUND His bundle pacing to correct LBBB is a viable alternative approach to achieve CRT but is limited by suboptimal lead delivery and high thresholds. METHODS This was a prospective, multicenter study performed between June 2017 and August 2018 at 6 centers. Patients with nonischemic cardiomyopathy, complete LBBB, and left ventricular ejection fractions (LVEFs) <= 50% who had indications for CRT and/or ventricular pacing in whom LBBP was attempted were included. Success rates, QRS duration, LVEF, left ventricular end-systolic volume, and improvement in functional class were assessed. RESULTS LBBP was successful in 61 of 63 patients (97%, mean age 68 +/- 11 years, 52.4% men). During LBBP, QRS duration narrowed from 169 +/- 16 to 118 +/- 12 ms (p < 0.001). Pacing threshold and R-wave amplitude remained stable at 1-year follow-up compared with implantation values (0.5 +/- 0.15 V/0.5 ms vs. 0.58 +/- 0.14 V/0.5 ms and 11.1 +/- 4.9 mV vs. 13.3 +/- 5.3 mV, respectively). LVEF increased significantly (33 +/- 8% vs. 55 1 10%; p < 0.001), with a reduction in left ventricular end-systolic volume (123 +/- 61 ml vs. 67 39 ml; p < 0.001). LVEF had normalized (>= 50%) in 75% of patients at 1 year. New York Heart Association functional class improved significantly from 2.8 +/- 0.6 at baseline to 1.4 +/- 0.6 at 1 year. No deaths or heart failure hospitalizations were observed during follow-up. CONCLUSIONS LBBP is a feasible and effective method for achieving electric resynchronization of LBBB, with resultant improvements in left ventricular structure and function. Low and stable pacing thresholds may be advantageous over His bundle paring for CRT in patients with LBBB and nonischemic cardiornyopathy. (C) 2020 by the American College of Cardiology Foundation.
A New Method to Recommend Left Ventricular Lead Positions for Improved CRT Volumetric Response and Long-Term Prognosis

Abstract:Using ECG-gated single-photon emission computed tomography (SPECT) myocardial perfusion imaging (MPI), we sought to develop and validate a new method to recommend left ventricular (LV) lead positions in order to improve volumetric response and long-term prognosis after cardiac resynchronization therapy (CRT). Seventy-nine patients received gated SPECT MPI at baseline, and echocardiography at baseline and follow-up. The volumetric response referred to a reduction of ≥ 15% in LV end-systolic volume 6 months after CRT. After excluding apical, septal, and scarred segments, there were three levels of recommended segments: (1) the optimal recommendation: the latest contracting viable segment; (2) the 2nd recommendation: the late contracting viable segments whose contraction delays were within 10° of the optimal recommendation; and (3) the 3rd recommendation: the viable segments adjacent to the optimal recommendation when there was no late contracting viable segment. After excluding 11 patients whose LV lead was placed in apical or scarred segments, 75.6% of the patients concordant to recommended LV segments (n = 41) responded to CRT while 51.9% of those with non-recommended LV lead locations (n = 27) were responders (P = .043). Response rates were 76.9%, 76.9% , and 73.3% (P = .967), respectively, when LV lead was implanted in the optimal recommendation (n = 13), the 2nd recommendation (n = 13), and the 3rd recommendation (n = 15). LV leads placed at recommended segments reduced composite events of all-cause mortality or heart failure (HF) rehospitalization compared with pacing at non-recommended segments (log-rank χ2 = 5.623, P = .018). Pacing in the recommended LV lead segments identified on gated SPECT MPI was associated with improved volumetric response to CRT and long-term prognosis.
Quadripolar Left Ventricular Lead in 20 Cases with Cardiac Resynchronization Therapy

Abstract:Objective The purpose of this study was to evaluate the application value of new type of quadripolar left ventricular (LV) lead in patients received cardiac resynchronization therapy (CRT).Methods Retrospective analysis was performed on heart failure patients following successful implantation of a quadripolar LV lead from March 2014 to April 2015 at The Second Affdiated Hospital,Zhejiang University College of Medicine.Clinical data like the anatomy characteristics of coronary vein,location of LV lead and the selected pacing vector were collected intraoperatively.During follow-up,clinical data such as the stability of LV lead,pacing threshold,phrenic nerve stimulation (PNS) and CRT response were collected postoperatively.Results Totally 20 consecutive heart failure patients [15 dilated cardiomyopathy,3 ischemic cardiomyopathy,2 others;mean age:(65.6±8.7) years] were selected in our study.All the 20 patients were implanted the quadripolar LV lead,in which 19 succeeded in the first target vein.The average length of LV lead target veins were(71.5±15.2) mm.The pacing sites of LV lead of the 20 cases were all at middle/basal segment(100％).Among them,the tip electrode (D1) of 12 cases were implanted at the apex of heart,7 at the middle segment and 1 at the basal segment with the proximal fourth electrode (P4) left in the coronary main vein.The tip of LV lead of 19 cases were implanted into small tributary,while in another one case,target vein was lack of tributary,leading to the lead tip simply located in the distal end of lateral vein,and one month later,LV lead was dislocated.During implantation,PNS was occurred in 7 cases and high pacing threshold in 3 cases,which were all overcome by reprogramming pacing vector.After a median follow-up of 6.7 months,the threshold of tip electrode and ring electrode remained stable(P＞0.05),LVEF and NYHA improved significantly(P＜0.05).Conclusion The Quartet quadripolar LV lead proved to be better at choosing optimal pacing thresholds,avoiding apex pacing and PNS,improving the success of LV lead implantation in chronic heart failure patients receiving CRT.In our opinion,the quadripolar LV lead was suitable to the target vein with small tributary at the branch vein.The threshold of tip electrode and ring electrode remained stable at short-time follow-up.

Abstract:Background Cardiac resynchronization therapy(CRT)is an effective treatment for chronic heart failure patients.However,approximately 30% of recipients exhibit no response to CRT and the position of left ventricular(LV)lead is thought to be an important factor associated with no response. Majority of the related studies indicated that implantation of LV leads at the sites of the latest activation and away from scar can significantly improve the response to CRT. Single photon emission computed tomography (SPECT)myocardial perfusion imaging(MPI)phase analysis(PA)is a novel method to assess LV mechanical dyssynchrony,the site of the latest activation and scar burden to recommend the suitable position for LV lead. Therefore,randomized,controlled trials using SPECT to guide LV lead implantation to improve CRT response is needed. Rationale GUIDE-CRT study applies pre-operation SPECT MPI AP to evaluate LV mechanical dyssynchrony and distribution of scar and identify the site of the latest activation segment away from scar as optimal pacing site. The feasibility and effectiveness of SPECT MPI PA in guiding LV lead implantation to improve CRT response will be verified. Design Patients with standard CRT indications will be enrolled in 19 centers in China and randomized to two groups(Guided vs.Control). All the patients will undergo baseline echocardiographic assessment,and SPECT imaging to identify the optimal LV lead positions from 13 LV segments. In the Guided group,the implanters will receive SPECT imaging guidance and attempt to place the LV leads to the target positions define as the LV lead is at or close to the latest activation segment away from scar. In the Control group,the patients will undergo routine LV lead implantation. All patients receive 6-month follow-up echocardiographic assessment to evaluate CRT response. The primary endpoint is reduction of LV end-systolic volume(LVESV). The secondary endpoints include reduction of LV end-diastolic volume, changes of LV ejection fraction,and CRT response rate defined as a relative decrease of LVESV by ≥ 15% from baseline to 6-month follow-up. Conclusion SPECT guided LV lead implantation will obviously improve CRT efficacy.

Abstract:Up to one-third of patients who undergo cardiac resynchronisation therapy (CRT) are not responders. To identify potential responders to CRT may be sometimes difficult and time-consuming. Forty-five patients who had undergone CRT implantation for standard indications were evaluated. Electrical left ventricular (LV) lead location was assessed by left ventricular activation time (LVAT), LV lead electrical delay (LVLED), and RV-LV interlead electrical delay (RVsense-LVsense). Anatomic LV pacing location was assessed as basal or mid-ventricular between 3:00 to 5:00 (traditionally optimal site), and all the other positions (traditionally non-optimal site). CRT response was defined as a decrease in LV end-systolic volume (LVESV) exceeding 15% at six months. LVLED was larger in the responder group than that in the non-responder group (67.3 ± 8.5% vs. 55.3 ± 8.1%, P< 0.001). In the multivariate analysis, LVLED and cLBBB morphology were the two independent predictors of positive echocardiographic response to CRT (OR=1.180, P=0.003; OR=7.497, P=0.04, respectively). A cutoff value of LVLED> 54.82% predicted responders with 96.3% sensitivity and 75.2% specificity and the area under the receiver operating characteristic (ROC) curve was 0.844 for LVLED (P=0.002). No relationship was found between the anatomic LV pacing sites and response to CRT (P=0.188). The larger left ventricular lead electrical delay may predict response to cardiac resynchronisation therapy.

Abstract:Although cardiac resynchronization therapy (CRT) is a well-established treatment for a subset of patients with chronic heart failure, a considerable proportion of eligible patients still fail to benefit from this treatment. The aim of this study was to identify potential independent predictors for being a responder to CRT. A single-center, retrospective analysis was conducted in 193 consecutive patients with heart failure and wide QRS complex who successfully underwent CRT device implantation from January 2006 to October 2012. Clinical characteristics, left ventricular lead position (LV-Ps), electrocardiography and echocardiography were evaluated before and 12 months after CRT. Response to CRT was defined as an absolute increase of ≥ 5% in left ventricular ejection fraction (LVEF) compared with baseline at 12 months after CRT implantation without heart failure rehospitalization or any cause of death. There were 132 responders (68%) and 61 nonresponders (32%). By univariate logistic analysis, the presence of non-left bundle branch block (non-LBBB) and QRS duration, chronic atrial fibrillation (AF), history of ventricular tachycardia (VT), degree of tricuspid regurgitation and left atrium dimension (LAD) at baseline, ΔQRS duration, and LV-Ps were associated with predicting a response to CRT. However, on multivariate analysis, only optimal LV-Ps and presence of non-LBBB remained independently predictive for a CRT response, with an odds ratio of 2.53 (95% confidence interval [CI]: 1.13-5.66, P = 0.023), 0.15(95% CI: 0.05-0.45, P = 0.001), respectively. Kaplan-Meier analysis revealed that patients with nonoptimal LV-Ps or non-LBBB morphology had a significantly higher rate of mortality or heart failure rehospitalization as compared with those with optimal LV-Ps or LBBB morphology (P < 0.05).

Abstract:Background: Leadless cardiac resynchronization therapy (CRT) is an emerging heart failure treatment. An implanted electrode delivers lateral or septal endocardial left ventricular (LV) pacing (LVP) upon detection of a right ventricular (RV) pacing stimulus from a coimplanted device, thus generating biventricular pacing (BiVP). Electrical efficacy data regarding this therapy, particularly leadless LV septal pacing (LVSP) for potential conduction system capture, are limited. Objectives: The purpose of this study was to evaluate the acute performance of leadless CRT using electrocardiographic imaging (ECGi) and assess the optimal pacing modality (OPM) of LVSP on the basis of RV and LV activation. Methods: Ten WiSE-CRT recipients underwent an ECGi study testing: RV pacing, BiVP, LVP only, and LVP with an optimized atrioventricular delay (LV-OPT). BiV, LV, and RV activation times (shortest time taken to activate 90% of the ventricles [BIVAT-90], shortest time taken to activate 95% of the LV, and shortest time taken to activate 90% of the RV) plus LV and BiV dyssynchrony index (standard deviation of LV activation times and standard deviation of all activation times) were calculated from reconstructed epicardial electrograms. The individual OPM yielding the greatest improvement from baseline was determined. Results: BiVP generated a 23.7% improvement in BiVAT-90 (P = .002). An improvement of 43.3% was observed at the OPM (P = .0001), primarily through reductions in shortest time taken to activate 90% of the RV. At the OPM, BiVAT-90 improved in patients with lateral (43.3%; P = .0001; n = 5) and septal (42.4%; P = .009; n = 5) LV implants. The OPM varied by individual. LVP and LV-OPT were mostly superior in patients with LVSP, and in those with sinus rhythm and left bundle branch block (n = 4). Conclusion: Leadless CRT significantly improves acute ECGi-derived activation and dyssynchrony metrics. Using an individualized OPM improves efficacy in selected patients. Effective LVSP is feasible, with fusion pacing at LV-OPT mitigating the potential deleterious effects on RV activation.
A novel method combining gated SPECT and vectorcardiography to guide left ventricular lead placement to improve response to cardiac resynchronization therapy: A proof of concept study

Abstract:Background: The segment of the latest mechanical contraction (LMC) does not always overlap with the site of the latest electrical activation (LEA). By integrating both mechanical and electrical dyssynchrony, this proof-of-concept study aimed to propose a new method for recommending left ventricular (LV) lead placements, with the goal of enhancing response to cardiac resynchronization therapy (CRT). Methods: The LMC segment was determined by single-photon emission computed tomography myocardial perfusion imaging (SPECT MPI) phase analysis. The LEA site was detected by vectorcardiogram. The recommended segments for LV lead placement were as follows: (1) the LMC viable segments that overlapped with the LEA site; (2) the LMC viable segments adjacent to the LEA site; (3) If no segment met either of the above, the LV lateral wall was recommended. The response was defined as ≥15% reduction in left ventricular end-systolic volume (LVESV) 6-months after CRT. Patients with LV lead located in the recommended site were assigned to the recommended group, and those located in the non-recommended site were assigned to the non-recommended group. Results: The cohort comprised of 76 patients, including 54 (71.1%) in the recommended group and 22 (28.9%) in the non-recommended group. Among the recommended group, 74.1% of the patients responded to CRT, while 36.4% in the non-recommended group were responders (P = .002). Compared to pacing at the non-recommended segments, pacing at the recommended segments showed an independent association with an increased response by univariate and multivariable analysis (odds ratio 5.00, 95% confidence interval 1.73-14.44, P = .003; odds ratio 7.33, 95% confidence interval 1.53-35.14, P = .013). Kaplan-Meier curves showed that pacing at the recommended LV lead position demonstrated a better long-term prognosis. Conclusion: Our findings indicate that pacing at the recommended segments, by integrating of mechanical and electrical dyssynchrony, is significantly associated with an improved CRT response and better long-term prognosis.

Abstract:Background: Leadless left ventricular (LV) endocardial pacing is an emerging cardiac resynchronization therapy (CRT) technology. Predictors of response to leadless CRT are poorly understood. Implanting the LV endocardial pacing electrode in sites with increased electrical latency (Q-LV) may improve response rates. Objective: To examine the association between Q-LV and echocardiographic remodelling response to leadless CRT delivered with the WiSE-CRT system. Methods: A post-hoc analysis (n=122) of the SOLVE-CRT trial examined the relationship between LV pacing site Q-LV with rate of LV end-systolic volume (LVESV) reduction >15% at 6 months. Multivariable regression analysis, adjusting for age, sex, prior CRT non-response, cardiomyopathy aetiology, QRS morphology and QRS duration was performed, followed by ROC analysis and analysis of variance by Q-LV quartile. A subgroup analysis of the ischaemic cardiomyopathy cohort was undertaken. Results: Complete Q-LV data was available in 122/153 (80%) of patients in the active arms SOLVE-CRT. Overall, the 6-month LVESV response rate was 46%. Logistic regression identified Q-LV as an independent response predictor with borderline significance (adjusted odds ratio 1.015, p=0.05). Analysis by Q-LV quartile demonstrated a significant improvement in response rate in quartile 4 (longest Q-LV, 64%) compared to quartile 1 (shortest Q-LV, 28%), p<0.01. This association was primarily driven by strong Q-LV-response correlation in patients with ischaemic cardiomyopathy, demonstrated by subgroup logistic regression (adjusted odds ratio 1.034, p=0.004). Conclusion: Increased Q-LV was associated with improved reverse remodelling following leadless CRT. Targeting LV endocardial sites of high Q-LV may deliver additional benefit compared to empirical LV electrode implantation.

Abstract:Optimization of LV Lead Site by EP MappingIntroductionElectrophysiological mapping (EPM) in coronary sinus (CS) branches is feasible for guiding LV lead placement to the optimal, latest activated site at cardiac resynchronization therapy (CRT) procedures. However, whether this procedure optimizes the response to CRT has not been demonstrated. This study was to evaluate effects of targeting LV lead at the latest activated site guided by EPM during CRT.MethodsSeventy-six consecutive patients with advanced heart failure who were referred for CRT were divided into mapping (MG) and control groups (CG). In MG, the LV lead, also used as a mapping bipolar electrode, was placed at the latest activated site determined by EPM in CS branches. In CG, conventional CRT procedure was performed. Patients were followed for 6 months after CRT.ResultsBaseline characteristics were comparable between the 2 groups. In MG (n = 29), EPM was successfully performed in 85 of 91 CS branches during CRT. A LV lead was successfully placed at the latest activated site guided by EPM in 27 (93.1%) patients. Compared with CG (n = 47), MG had a significantly higher rate (86.2% vs. 63.8%, P = 0.039) of response (>15% reduction in LV end-systolic volume) to CRT, a higher percentage of patients with clinical improvement of 2 NYHA functional classes (72.4% vs. 44.7%, P = 0.032), and a shorter QRS duration (P = 0.004).ConclusionsLV lead placed at the latest activated site guided by EPM resulted in a significantly greater CRT response, and a shorter QRS duration.

Abstract:OBJECTIVE:To observe the clinical effects of cardiac resynchronization therapy (CRT) in patients with chronic heart failure, and compare the clinical characteristics and outcome between responders and non-responders to define factors related to the efficacy of CRT.METHODS:We retrospectively analyzed the data of patients underwent CRT-P/D implantation from January 2006 to December 2012 in our Hospital. All patients received long-term follow-up including NYHA classification, left ventricular ejection fraction (LVEF) and left ventricular internal dimension at end diastole (LVIDd).RESULTS:A total of 204 patients were included (130 males, mean age (64.8 ± 11.9) years). The total response rate of CRT was 61.3%. Women, QRS duration ≥ 150 ms, and left bundle branch block (LBBB) were related with better response after CRT (all P < 0.05). Multivariate regression analysis showed that QRS duration was an independent determinant for CRT response. All-cause mortality rate was significantly lower in responder group than in non-responder group (P < 0.001).CONCLUSIONS:In patients with chronic heart failure, women, QRS duration ≥ 150 ms, and LBBB are related with better CRT response rate post CRT. QRS duration ≥ 150 ms is an independent predictor of CRT response, and positive response is associated with lower all-cause mortality in this patient cohort.

Abstract:Cardiac resynchronization therapy (CRT) is an established therapeutic option in symptomatic heart failure with reduced ejection fraction and evidence of left ventricular (LV) conduction delay (QRS width ≥120 ms), especially when typical left bundle branch block is present. The rationale behind CRT is restoration of aberrant LV electrical activation. As there is considerable heterogeneity of the LV electrical activation pattern among CRT candidates, an individualized approach with targeting of the LV lead in the region of latest electrical activation while avoiding scar tissue may enhance CRT response. Echocardiography, electro anatomic mapping, and cardiac magnetic resonance imaging with late gadolinium enhancement are helpful to guide such targeted LV lead placement. However, an important limitation remains the anatomy of the coronary sinus, which often does not allow concordant LV lead placement in the optimal region. Epicardial LV lead placement through minimal invasive surgery or endocardial LV lead placement through transseptal punction may overcome this limitation, obviously with an increased complication risk. Furthermore, recent pacing algorithms suggest superiority of LV-only versus biventricular pacing in patients with preserved atrio ventricular (AV) conduction and a typical LBBB pattern. Finally, pacing from only one LV site might not overcome the wide electrical dispersion often seen in patients with LV conduction delays. Therefore, multisite pacing has gained significant interest to improve CRT response. The use of multiple LV leads may potentially lead to more favorable reverse remodeling, improved functional capacity and quality of life in CRT candidates, but adverse events and a shorter battery span are more frequent because of the extra lead. The use of one multipolar LV lead increases the number of pacing configurations within the same coronary sinus side branch (within small distances from each other) without the use of an additional lead. Small observational studies suggest that more effective resynchronization can be achieved with this approach. Finally, there are many reasons for non effective CRT delivery in carefully selected patients with an adequately implanted device. Multidisciplinary, post implantation care inside a dedicated CRT clinic ensures optimal CRT delivery, improves response rate and should be considered standard of care.

Abstract:Introduction: Cardiac resynchronisation therapy is ineffective in 30–40% of patients with heart failure with reduced ejection fraction. Targeting non-scarred myocardium by selecting the site of latest mechanical activation using echocardiography has been suggested to improve outcomes but at the cost of increased resource utilisation. The interval between the beginning of the QRS complex and the local LV lead electrogram (QLV) might represent an alternative electrical marker. Aims: To determine whether the site of latest myocardial electrical and mechanical activation are concordant. Methods: This was a single-centre, prospective pilot study, enrolling patients between March 2019 and June 2021. Patients underwent speckle-tracking echocardiography (STE) prior to CRT implantation. Intra-procedural QLV measurement and R-wave amplitude were performed in a blinded fashion at all accessible coronary sinus branches. Pearson's correlation coefficient and Cohen's Kappa coefficient were utilised for the comparison of electrical and echocardiographic parameters. Results: A total of 20 subjects had complete data sets. In 15, there was a concordance at the optimal site between the electrically targeted region and the mechanically targeted region; in four, the regions were adjacent (within one segment). There was discordance (≥2 segments away) in only one case between the two methods of targeting. There was a statistically significant increase in procedure time and fluoroscopy duration using the intraprocedural QLV strategy. There was no statistical correlation between the quantitative electrical and echocardiographic data. Conclusions: A QLV-guided approach to targeting LV lead placement appears to be a potential alternative to the established echocardiographic-guided technique. However, it is associated with prolonged fluoroscopy and overall procedure time.

Abstract:Objectives: We aimed to evaluate the feasibility of left ventricular electroanatomical mapping to choose between left bundle branch area pacing (LBBAP) or coronary venous pacing (CVP). Background: There are several ways to achieve left ventricular activation in cardiac resynchronization therapy (CRT): LBBAP and CVP are two possible methods of delivering CRT. However, the criteria for choosing the best approach remains unknown. Methods: A total of 71 patients with heart failure, reduced ejection fraction, and left bundle branch block (LBBB) were recruited, of which 38 patients underwent the three-dimensional electroanatomical mapping of the left ventricle to accurately assess whether the left bundle branch was blocked and the block level, while the remaining 33 patients were not mapped. Patients with true LBBB achieved CRT by LBBAP, while patients with pseudo-LBBB achieved CRT by CVP. After a mean follow-up of 6 months and 1 year, the QRS duration and transthoracic echocardiography, including mechanical synchrony indices, were evaluated. Results: Twenty-five patients with true LBBB received LBBAP, while 13 without true LBBB received CVP. Seventeen patients received LBBAP, and 16 patients received CVP without mapping. Paced QRS duration after the implantation of LBBAP and CVP was significantly narrower in the mapping subgroup compared to the non-mapping subgroup. A significant increase in post-implantation left ventricular ejection fraction was observed in patients with LBBAP or CVP, and the mapping subgroup were better than the non-mapping subgroup. After a 12-month follow-up, atrioventricular, intraventricular, and biventricular synchronization were significantly improved in the mapping subgroup compared to non-mapping groups in both LBBAP and CVP. Conclusion: In our study, three-dimensional electroanatomical mapping was used to choose LBBAP or CVP for heart failure patients, which proved feasible, with better cardiac resynchronization in the long-term follow-up. Therefore, three-dimensional electroanatomical mapping before CRT appears to be a reliable method for heart failure patients with LBBB who are indicated for CRT.

Abstract:Background Heart failure patients with non-left bundle branch block (non-LBBB) QRS pattern have limited response rate to biventricular pacing (BVP). Objectives A personalized cardiac resynchronization therapy (CRT) implantation approach guided by real-time electrocardiographic imaging (ECGi) was studied. Methods Twenty patients with left ventricular ejection fraction (LVEF) ≤35%, QRS duration ≥120ms and non-LBBB (13 right bundle branch block and 7 intraventricular conduction delay) were recruited. During CRT implantation, right atrial, right ventricular, coronary sinus, His-bundle and/or Left-bundle leads were inserted. The total activation time (TAT) with different pacing combinations were measured real-time during implantation by ECGi. The configuration producing shortest TAT would be chosen. Clinical response was defined as ≥1 New York Heart Association (NYHA) class improvement. Echocardiographic response was defined as LV end systolic volume reduction ≥15% and/or LVEF improvement ≥10% at 6-months. Results After ECGi guided CRT implantation, LVEF improved from 26±6% to 34±11% (both p<0.01) and NYHA class improved from 3.0±0.5 to 2.0±0.6 (p<0.01). Both clinical and echocardiographic response rates were 70%. ECGi approach resulted in better acute electrical resynchronization over BVP as measured by TAT reduction (40% vs 14%, p<0.01). Percentage of TAT reduction was found to be a strong predicator for echocardiographic response (AUC for ROC curve 0.91, 95% CI 0.78 – 1.00). A strong positive correlation between percentage TAT reduction and percentage LVEF improvement (Pearson R 0.70, p=0.001) was found. Conclusions ECGi guided CRT implantation in non-LBBB patients generates superior acute electrical resynchronization compared with BVP and is associated with favorable clinical and echocardiographic outcomes.

Abstract:Cardiac resynchronization therapy (CRT) threw lights on heart failure treatment, however, parts of patients showed nonresponse to CRT. Unfortunately, it lacks effective parameters to predict CRT non-response. In present study, we try to seek effective electro-echocardiographic predictors on CRT non-response. This is a retrospective study to review a total of 227 patients of dyssynchronous heart failure underwent CRT implantation. Logistic analysis was performed between CRT responders and CRT non-responders. The primary outcome was the occurrence of improved left ventricular ejection fraction 1 year after CRT implantation. We concluded that LVEDV > 255 mL ( OR = 2.236; 95 % CI, 1.016 – 4.923 ) rather than LVESV > 160 mL ( OR = 1.18; 95 % CI, 0.544 – 2.56 ) and TpTe/QTc > 0.203 ( OR = 5.206; 95 % CI, 1.89 – 14.34 ) significantly predicted CRT non-response. Oppositely, S wave > 5.7 cm/s ( OR = 0.242; 95 % CI, 0.089 – 0.657 ), E/A > 1 ( OR = 0.211; 95 % CI, 0.079 – 0.566 ), E’/A’ > 1 ( OR = 0.054; 95 % CI, 0.017 – 0.172 ), CLBBB ( OR = 0.141; 95 % CI, 0.048 – 0.409 ), and QRS duration >160 ms ( OR = 0.52; 95 % CI, 0.305 – 0.922 ) surprisingly predicted low-probability of CRT non-response.

Abstract:Theoretically, the layout of fractional pitch permanent magnet motors should ensure the cancellation of most cogging torque harmonics. In reality, manufacturing errors lead to incomplete cancellation and higher than expected cogging torque. One potential manufacturing error is the variation in angle between neighboring magnets. This paper uses the superposition technique to analytically determine the expected magnitude of each cogging torque harmonic based on a Gaussian distribution of angular magnet placement variation. A simple expression is presented for the expected value of each harmonic's magnitude based upon the harmonic number, the number of magnets, and the standard deviation of the Gaussian placement distribution. The derived expression is validated using Monte Carlo simulations and experimental implementation.

Abstract:An index of equivalent performance in noise was developed to compare recognition in different forms of speech distortion. Speech-recognition performance of young and elderly listeners with and without hearing loss was evaluated for undistorted speech presented in quiet and noise, and for speech distorted by four time-compression ratios and by four reverberation times. The data obtained in noise on young subjects with normal hearing served to generate a normalized regression equation, which was used to convert percent-correct performance in different distortion conditions to equivalent performance for undistorted speech at a particular S/N ratio. Comparisons of the equivalent S/N ratios obtained in the various conditions allowed rank-ordering of speech recognition performance in different types of degradation. The data also show that age and hearing loss affect recognition of speech degraded by reverberation or time compression. However, age effects are evident primarily in the most severe distortion conditions. Recognition of undistorted speech in noise was affected by hearing loss but not by age. These findings support a hypothesis that stipulates that increased age produces a reduction in the functional S/N ratio.
