Abstract:PurposeThe purpose of this study was to develop a discharge assessment scale tailored for outpatients undergoing sedative anesthesia treatment in the ambulatory postanesthesia care unit and validate its agreement with the Post-Anesthetic Discharge Scoring System.DesignThe Delphi method.MethodsA Delphi survey was conducted with 30 experts focusing on the evaluation of outpatient discharges following treatment under ambulatory anesthesia. Subsequently, a cross-sectional observational study employing convenience sampling selected 2,579 outpatients who had undergone painless ambulatory gastrointestinal endoscopy at a tertiary hospital to analyze the level of agreement with the Post-Anesthesia Discharge Scoring System.FindingsThe study conducted three rounds of expert consultations to create the ambulatory discharge assessment scale. Twenty-five experts from 12 provinces and municipalities in our country were interviewed. The discharge assessment form encompassed five aspects: consciousness level, vital signs, directional stability, mobility, and adverse reactions. According to the scale, if the total score exceeded 9 points, with none of the items scoring 0 points, the ambulatory patient could be discharged from the hospital with the accompaniment of family members. Patients assessed using this newly constructed scale were able to leave the hospital earlier compared to those assessed using the comparative scale. No significant differences were observed in vital signs at the time of discharge or the occurrence of adverse events within 24 hours after the procedure.ConclusionsThis assessment tool for discharging ambulatory patients after the ambulatory anesthesia from the postanesthesia outpatient care unit can be considered a valuable addition to formalize the discharge process in outpatient services.

Abstract:Heart to Heart is a computer-based decision aid for patients and providers that provides personalized, evidence-based information about coronary heart disease (CHD) risk and potential risk-reducing interventions. To develop Heart to Heart, the authors used Framing-ham risk equations and systematic reviews of risk-reducing interventions. The Web version was programmed using PHP: Hypertext Processor, a Web-based programming language, and has separate interfaces for providers and patients. The authors subsequently developed a modified version for personal digital assistants. Heart to Heart uses information about a patient's CHD risk factors (age, gender, total and high-density lipoprotein cholesterol levels, diabetes, smoking, systolic blood pressure, and left ventricular hypertrophy) to calculate risk of total CHD events over 5 or 10 years. Patients and providers can then examine the effect of introducing one or more risk-reducing interventions (aspirin, lipid-lowering drug therapy, antihypertensive medication, or smoking cessation) on the patient's CHD risk. Future research will be directed to determining whether Heart to Heart can improve utilization of effective CHD risk-reducing interventions.

Abstract:Attention is called to the development of severe pulmonic-valve regurgitation during withdrawal of a balloon-tipped catheter that had been in place for six days. The resulting pulmonic regurgitation produced a loud (grade 5/6), musical diastolic murmur recorded by phonocardiogram at the time of its initial production. Autopsy disclosed a noninfected tear in one of the three pulmonic valve cusps.

Abstract:BACKGROUND:The Child Hospital Consumer Assessment of Healthcare Providers and Systems (Child HCAHPS) is a standard instrument to measure pediatric inpatients' experience of care. Currently, no Chinese version of the Child HCAHPS exists for Chinese patients. Therefore, this study aimed to create a Chinese version of the Child HCAHPS and investigate its validity and reliability in a Chinese setting.METHODS:Using the approach recommended in guidelines from the Agency for Healthcare Research and Quality for translating HCAHPS surveys, we produced a Chinese version of the Child HCAHPS. A two-month field test with seven hospitals across five provinces in China was performed to assess its validity. Construct validity was assessed using confirmatory factor analysis. We evaluated convergent validity by factor loading, average variance extracted (AVE), and construct reliability (CR). Cronbach's alpha and corrected item-total correlation (CITC) were used to reflect hospital-level unit reliabilities for the survey's item composites. The correlation of the measure score with the overall rating was calculated to evaluate criterion validity.RESULTS:An overall response rate of 63% was achieved, and 2,258 respondents completed the questionnaire. Confirmatory factor analysis showed a comparative fit index (CFI) of 0.905, a non-normed fix index of 0.886, and a root mean square error of approximation (RMSEA) of 0.089. Most items had factor loadings over 0.7. Cronbach's alpha coefficient on the overall level was 0.981, and all measures' CITC exceeded 0.6, demonstrating good to excellent hospital-level reliability of the composite and single-item measures. All composite measures had good to excellent internal consistency reliability (0.716 to 0.994). Item-to-composite correlation ranged from 0.510 to 0.997. Composite-to-composite correlations ranged from 0.488 to 0.997. According to the survey result, for all the 18 composite or single-item measures, mean top-box scores ranged from 56% ("Involving teens in care") to 87% ("Informed in Emergency Room").CONCLUSIONS:The Chinese version of the Child HCAHPS demonstrated acceptable validity and reliability. The application of this tool can help benchmark ongoing healthcare improvement initiatives in China.
The Development and Assessment of Simple Patient Satisfaction Questionnaire

Abstract:Objectives: To develop a simple patient satisfaction questionnaire and to assess its reliability and validity. Methods: A 7-item patient satisfaction questionnaire was developed with reference to the Hospital Consumer Assessment of Healthcare Providers and Systems Survey(HCAHPS) and Press Ganey Survey, including questions about admission, skills, attitude, pain controlling, environment and overall assessment. The internal consistency and validity were evaluated by a survey on 151 inpatients, using HCAHPS and Press Ganey Survey as references. The reliability of the questionnaire was assessed by a test-retest survey conducted among 70 patients within an interval of 24-48 hours. Results:(1)Test-retest reliability: Weighed Kappa indexes for items were from 0.96 to 0.92. Intraclass correlation coefficients were from 0.80 to 0.92 for items.(2)Internal consistency: Cronbach's alpha was 0.86 for the whole scale.(3)Construct validity: The accumulated proportions of variances of the preceding three principal components was 79.06%.(4)Criterion validity: Spearman correlation coefficients of overall assessment of the questionnaire with those of HCAHPS and Press Ganey Survey were 0.58 to 0.82.(5)Acceptance: The response rate of the questionnaire was 90.10%, which cost 3.4 minutes to answer. Though people's willingness to use these three scales were similar, the abbreviated questionnaire was simpler and better completed. Conclusion: The abbreviated questionnaire satisfied test-retest reliability and was highly correlated with HCAHPS and Press Ganey Survey. It had much fewer items, took less time while satisfied construction validity, internal consistency and responding rate. We suggested this questionnaire be used as a replicable tool to assess patient satisfaction of in-patient medical service.

Abstract:Background Patient Reported Experience Measures (PREM) have become a critical component in assessing healthcare quality from the patient's perspective. Accurate and reproducible assessment tools are essential for generating robust and reliable results for evaluating peri-anesthesia patient experiences, identifying associated factors, and assessing the impact of healthcare interventions. However, there is currently no systematic review that consolidates all existing peri-anesthesia PREMs and evaluates their psychometric properties. Objective To identify and assess the psychometric properties of PREMs for peri-anesthesia patients. Design Systematic review of measurement properties following the COSMIN guidelines. Methods Systematic searches were conducted in China National Knowledge Infrastructure, Wanfang, PubMed, Embase, Web of Science, CINAHL, and PsycINFO databases from January 1, 1993, to April 15, 2024. Studies reporting on the development and/or validation of any PREMs for use in the peri-anesthesia period were considered eligible. The measurement properties extracted included data on the item development process, content validity, structural validity, internal consistency, cross-cultural validity, reliability, hypothesis testing and responsiveness. For the same PREM across different studies, reliability coefficients were analyzed using a meta-analysis. The quality assessment, rating of measurement properties, synthesis, and modified grading of the evidence were carried out following the COSMIN methodology for systematic reviews. Results A total of 26 studies encompassing 16 PREMs were included. Among them, the Patient Satisfaction with Perioperative Anesthetic Care questionnaire (PSPACq), Perception of Quality in Anesthesia (PQA), Sindhvananda General Anesthesia Satisfaction questionnaire, and Daycare Anesthesia Satisfaction (DAS) demonstrated moderate to high-quality evidence of adequate content validity and internal consistency, resulting in strong recommendations. Five PREMs exhibited high-quality evidence of inadequate structural validity and internal consistency, receiving a "not recommended" status. The remaining PREMs were weakly recommended. Conclusions This systematic review identified PSPACq and PQA as effective tools for assessing peri-anesthesia experiences in surgical patients, suitable for both research and clinical use. Future studies should focus on thoroughly evaluating the measurement properties of these two PREMs, as many aspects remain underexplored. A high risk of bias was noted in other PREMs, particularly in content validity, structural validity, and reliability, which increases uncertainty in the evidence base. Abstract:INTRODUCTION: Evaluating patient-reported outcomes is complex. These difficulties may explain weaknesses with some existing tools: mainly, they rely on expert instead of patient views or are not metrically sound. The purpose of this study was to develop and validate a multidimensional self-reported questionnaire, specifically assessing the satisfaction of patients undergoing regional anesthesia, Evaluation du Vécu de l'Anesthésie LocoRégionale (EVAN-LR).METHODS: Patients included underwent various surgical procedures under regional anesthesia. The questionnaire structure was identified by principal component factor analyses and interitem, item-dimension, and interdimension correlations. The authors assessed external validity by studying the relationships between potential dimensions of EVAN-LR and validated instruments such as Amsterdam Preoperative Anxiety and Information Scale, State Trait Anxiety Inventory, and specific visual analog scales. Internal consistency reliability was assessed by Cronbach α.RESULTS: We included 390 patients for the validation phase. The EVAN-LR comprises 19 items, structured in a global index and five dimensions: Attention, Information, Discomfort, Waiting, and Pain. The consequences of staying alert during regional anesthesia were specifically addressed by two items. Female sex was associated with significantly lower Information score. Patients with American Society of Anesthesiologists physical status below 2 had a significantly lower Attention score. Patients older than 55 years showed higher satisfaction scores for most dimensions. EVAN-LR poorly correlated with premedication.CONCLUSION: The authors have validated a new measuring tool assessing patient satisfaction within the perioperative period surrounding regional anesthesia. The multidimensional structure of EVAN-LR allows it to be used as a clinical tool for improving anesthesia management.

Abstract:BACKGROUND: Comprehensive medication management (CMM) services are a relatively new standard for clinical practice. A patient satisfaction tool for pharmacists providing comparable pharmacy services is essential for measuring quality and sustainability.OBJECTIVE: To develop a psychometrically valid questionnaire for measuring patient satisfaction for CMM services.METHODS: A patient satisfaction survey tool was developed through a multiphase development process. Validation studies were conducted across 2 urban ambulatory care health system settings providing CMM services. The survey consisted of 10 items related to 3 domains: medication-related needs, pharmacist-patient engagement, and overall satisfaction. Using a 4-point scale, the surveys were mailed, collected, and analyzed for descriptive statistics, internal consistency, and factorial composition.RESULTS: Total surveys returned for analysis numbered 195, with an overall survey response rate of 19.2%. Factor analysis and item analysis identified 1 factor of pharmacists' patient care services. The factor was named "patient satisfaction."CONCLUSIONS: The instrument that was developed provided 1 factor of CMM services. This brief patient satisfaction tool appears to be reliable and valid and may serve other CMM providers to assess 1 measure of quality assurance upon further evaluation.

Abstract:Aims: To develop and validate a new instrument to measure satisfaction with integral care (doctor-nurse) of the patient with type 2 diabetes mellitus, considering expectations-experiences together for the primary level of care.Methods: The instrument was constructed with questions regarding integral care to measure the satisfaction of the diabetes patient and was classified into four domains. The validity of the content was done through a panel of experts, apparent validity through a focus group, the validity of the construct through analysis of the main components and confirmatory factorial analysis, instrument reliability with internal consistency, determined by Cronbach alpha and temporal stability (test-retest).Results: The reliability of the questionnaire was 0.942. The intraclass correlation coefficient was 0.849. Validity of the construct showed acceptable goodness-of-fit and factorial structure with four factors: communication, empathy, technical care, care continuity, and 24 items for each domain, giving a Kayser-Meyer-Olkin index above 0.80 and a total variance above 73.Conclusions: The instrument is reliable and is also valid in terms of up into construct and content to evaluate satisfaction.Practice Implications: In addition, these results allow to have elements for the design of strategies aimed at improving the relationship of health personnel with the patient.

The Development and Testing of the MISSCARE Survey OR

Abstract:Background: An acceptable, reliable, and valid survey instrument to measure missed nursing care in perioperative settings has not been developed. Purpose: To develop and conduct psychometric testing of the MISSCARE Survey OR. Methods: Data were collected nationwide from 1,693 operating room (OR) nurses who completed the MISSCARE Survey OR. The survey contained two sections: Part A, "Elements of perioperative nursing care" (32 questions) and Part B, "Reasons for missing nursing care" (17 questions). Results: The MISSCARE Survey OR demonstrated acceptability, as few respondents missed questions in Part A (0.1%-1.1%) and Part B (0.8%-1.3%). Exploratory factor analysis revealed five subscales in Part A (Legal, Preparation, Safety, Communication, and Closing) and four in Part B (Urgency, Staffing, Materials, and Teamwork). In Part A, the five-factor solution explained 44% of the variance. In Part B, the four-factor solution explained 53% of the variance. Alpha coefficients for subscales in Part A ranged from 0.71 to 0.84 and 0.74 to 0.90 for Part B. Validity was measured using content validity, criterion validity, and construct validity. A panel of OR nurse experts established content validity. Criterion validity compared hospitals with fewer than six ORs to hospitals with hospitals with more than six ORs where it was hypothesized aprior that nurses in hospitals with fewer ORs would have missed less care (X = -.123, standard error [SE] = .041, p = .003). Construct validity was tested through exploratory and confirmatory factor analyses (CFA). Correlation coefficients for Part A ranged from 0.34 to 0.73 and 0.60 to 0.73 for Part B. Overall model fit was acceptable: goodness-of-fit index (GFI) and CFA were greater than 0.90, standardized root mean square residual (SMRM) was less than 0.06, and root mean square error of approximation (RMSEA) less than 0.08. Conclusion: The MISSCARE Survey OR promises to be a reliable, valid indicator of the extent of and reasons for missed nursing care.
Development of a Artificial Intelligence Dental Anxiety Scale (AI -CDAS) For Children: Validation And Reliability

Abstract:Assessing children's pain using patient-based pain scales can be challenging. Hence, there is a need of new version Artificial Intelligence Child Dental Anxiety Scale (AI-CDAS) to scale and test its psychometric properties (validity and reliability). This study was conducted to evaluate the validity and reliability of Artificial Intelligence version of the Child Dental Anxiety Scale (AI-CDAS) for anxiety detection scale in children using face as a response set from October 2022 to December 2023. Aim of this study was to examine the reliability and validity of Artificial Intelligence based Child Dental Anxiety Scale (AI-CDAS) using a wide range of age samples in dental clinics. A total of 100 outpatients (Age 03-09 years) from Bharati Vidyapeeth Deemed to be University Dental College and Hospital, Pune, India participated in this study. Dental anxiety was assessed using the Artificial Intelligence Child Dental Anxiety Scale (AI-CDAS) and comparing with a valid and reliable scale of Colored Version of Modified Facial Affective Scale. Reliability and validity was good and significant correlations were found between the AI-CDAS and the Colored Version of Modified Facial Affective Scale. This study suggests that the Artificial Intelligence based Child Dental Anxiety Scale (AI-CDAS) is a valid and reliable measure for assessing children's dental anxiety and may help encourage dentists to formally assess dental anxiety scale in day to day practice. Self-report measures are commonly employed in dental anxiety assessments. One advantage of self report measures is the ease of administration, taking relatively short period of time to complete. They can also assess the reaction to different aspects of the dental experience.

Abstract:Moses was a lifelong preoccupation of Freud, representing a double and idealized self and object. Freud identified with different aspects of Moses during different periods of development, from concrete hero to abstract ideal. He turned to Moses in the concluding phase of his relationship with Fliess and his self–analysis, and then at other times of crisis. The Moses recreated by Freud is important to the evolution of the concepts of the superego, and his Moses studies simultaneously illuminate the developmental significance of internalization, identification, and abstract symbolic thought. Latently autobiographical, the Moses motif is related to the analysis of unconscious conflict and trauma and to issues of Jewish identity and analytic ideals.

Abstract:TSN agar was applicable for enumeration of Clostridium perfringens in fecal samples of adults but not in those of infants. It was demonstrated using TSN agar that some healthy aged adults had persistently carried C. perfringens at levels ranging from 10(7) to 10(9), while some others ranged from 10(3) to 10(6) per ml volume of fecal sample although all of these adults had the same diets. In the test for agglutinability of isolates of C. perfringens collected from two elderly adults, a younger adult and a baby, it was demonstated that most of the isolates obtained from an aged adult of high levels for 19 months belonged the same serotype, while rapid alteration of serotypes could be observed in three other persons with high or low levels. In spite of as many as 10(9) C. perfringens per ml of feces, no trace of a-toxin could be detected in the fecal samples. In in vitro tests, fecal suspension suppressed the production of a-toxin although it allowed the organism to grow sufficiently.

Abstract:Nursing Ethics, Ahead of Print. BackgroundPatient dignity is sometimes neglected in intensive care unit (ICU) settings, which may potentially cause psychological harm to critically ill patients. However, no instrument has been specifically developed to evaluate the behaviors of dignified care among critical care nurses.AimThis study aimed to develop and evaluate ICU Dignified Care Questionnaire (IDCQ) for measurement of self-assessed dignity-conserving behaviors of critical care nurses during care.MethodsThe instrument was developed in 3 phases. Phase 1: item generation; phase 2: a two-round Delphi survey and a readability pilot study; phase 3: cross-sectional survey with model estimation. The questionnaire was evaluated by item analysis, exploratory and confirmatory factor analysis, assessment of internal consistency reliability, and test-retest reliability. The investigation was conducted using a convenience sample of 392 critical care nurses from 6 cities in Zhejiang Province, China, of which 30 participated in the test-retest reliability survey 2 weeks later.Ethical considerationsThe study was approved by ethics committee. All participants provided written informed consent before the survey. The questionnaire survey was anonymous.ResultsThe results showed acceptable reliability and validity of the IDCQ. The 17-item final version questionnaire was divided into 2 dimensions: absolute dignity and relative dignity. These two factors accounted for 62.804% of the total variance, and model fitting results were acceptable. The Cronbach's alpha coefficient of the questionnaire was 0.94, and the test-retest intraclass correlation coefficient (ICC) was 0.88 after 2 weeks.ConclusionsThis study developed a brief and reliable instrument (IDCQ) to assess dignified care in ICU nursing. It can help critical care nurses identify their behaviors in maintaining patient dignity and discover their deficiencies. It may also serve as a clinical nursing management tool to help reduce patient disrespect experience in ICU.

Psychometric properties were assessed after 193 school-aged children completed the needs of children's questionnaire in four paediatric areas in Australia and New Zealand. Results: The development and validation of the NCQ over two countries resulted in a 16-item, four-category tool to measure the self-reported importance and fulfilment of school-aged children's needs in hospital. Cronbach's alpha for the combined samples was 0.93. Conclusion: The NCQ bridges a gap to measure the level of importance and fulfilment of school-aged children's self-reported needs in hospital. Future testing and validation is needed in other paediatric areas and countries. Impact: The 16-item NCQ is a valid measure to evaluate if the quality of care delivered and received in hospital is in line with what children self-report as important and required and to date indicates good usability and utility. Child self-reports are essential to inform healthcare delivery, policy, research and theory development from a child and family-centred care lens that honours the United Nations Convention on the Rights of the Child and the best interests of the child.
Creating and Validating a Questionnaire for Assessing Dentists' Self-Perception on Oral Healthcare Management—A Pilot Study

Abstract:Background and Objectives: Questionnaires designed to test knowledge and self-perception can be valuable tools for diagnosing a dentist's understanding of the management and administration of a practice. The objective of this study was to create and authenticate a questionnaire for assessing dentists' self-perception on oral healthcare management developed from discussions with experts in this field. Material and Methods: In order to create and verify a questionnaire survey, a cross-sectional, descriptive, and analytical study was carried out. Participants' personal information and 31 statements across four categories made up the final questionnaire form. The answers to the questionnaire were in the form of a Likert scale. After refining the initial version, a total of 36 interviews were conducted at dental offices to verify the validity. For the Exploratory Factor Analysis (EFA), we used the Kaiser–Meyer–Olkin (KMO) index, the Bartlett sphericity test, and also Cronbach alpha coefficient for the validity of the questionnaire. Results: The accuracy of the instrument was measured by intrarater and interrater reliability. For the EFA, all the communalities exceeded the threshold of 0.05. With a Cronbach's alpha coefficient of 0.898, the questionnaire has sufficient internal consistency. Conclusions: The questionnaire demonstrates robust reliability and validity, thereby affirming its suitability for its intended purpose.

Abstract:This study evaluated the effect of a single exposure to sulfuric acid aerosol on six human biochemical blood parameters. A total of 18 human subjects were exposed to ambient air for 4 hr on the first day of exposure and to 100 microgram/m3 (0.033 microM) sulfuric acid aerosol (0.5 micrometer mean mass diameter) for 4 hr on the second day. A total of 17 human subjects were exposed for 4 hr to ambient air on both exposure days. On each exposure day, at 30 min and 90 min after exposure was initiated, all subjects engaged in a 15-min exercise period (walking 4 mph on a treadmill inclined at 10 degrees). Six biochemical blood parameters were measured before and after exposure: glutathione, lysozyme, glutathione reductase, serum glutamic oxaloacetic acid transaminase, serum vitamin E, and 2, 3-diphosphoglyceric acid. The results indicate no significant effect of one 4-hr exposure of humns to 100 microgram/m3 sulfuric acid aerosol. One significant post-exposure effect did occur--an increase in glutathione reductase--for both the control and acid-exposed group. It is concluded that at the levels studied, sulfuric acid aerosol is neutralized in the upper airways; consequently, no cellular damage is measurable.
