Abstract:This review aims to characterize the current landscape of exoskeletons designed to promote medical care and occupational safety in industrial settings. Extensive exploration of scientific databases spanning industries, health, and medicine informs the classification of exoskeletons according to their distinctive attributes and specific footholds on the human physique. Within the scope of this review, a comprehensive analysis is presented, contextualizing the integration of exoskeletons based on different work activities. The reviewers extracted the most relevant articles published between 2008 and 2023 from IEEE, Proquest, PubMed, Science Direct, Scopus, Web of Science, and other databases. In this review, the PRISMA-ScR checklist was used, and a Cohen's kappa coefficient of 0.642 was applied, implying moderate agreement among the reviewers; 75 primary studies were extracted from a total of 344. The future of exoskeletons in contributing to occupational health and safety will depend on continued collaboration between researchers, designers, healthcare professionals, and industries. With the continued development of technologies and an increasing understanding of how these devices interact with the human body, exoskeletons will likely remain valuable for improving working conditions and safety in various work environments.

Abstract:The research community has conducted several controlled "in -lab" assessments on the effectiveness of industrial exoskeletons, paving the way for their adoption. However, field testing, focusing on ergonomics and the user experience, could serve to enhance both end-users' awareness and address open doubts concerning true effectiveness of industrial exoskeletons. This study presents an analysis of qualitative data regarding the use of back-support exoskeletons during field trials in harsh civil engineering environments. This work evaluates the StreamEXO's (an active back-support exoskeleton) efficacy in reducing fatigue and the evolution of its perceived usefulness. This is achieved using qualitative data collection tools, during real scenarios testing over multiple-day trials. Collected data shows a positive correlation between self-reported fatigue, measured on a four verbal anchors-based Borg CR10 scale, and the use of the exoskeleton during physically demanding movements. Moreover, the evolution of scores throughout the testing sessions (90 minutes of exoskeleton use for three nonconsecutive days) suggests a trend due to the adaptation and learning curve of workers during the exoskeleton experience. The analysis of the open-ended answers highlights that the adaptation to physical interaction has a negative oscillation on day two to rise back during the third day, possibly correlated to a change in muscle pattern. The main critical factors affecting comfort during the exoskeleton experience are weight balance, body pressure, and thermal comfort, which can strongly affect device acceptance.
Exoskeletons for workers: A case series study in an enclosures production line

Abstract:This case-series study aims to investigate the effects of a passive shoulder support exoskeleton on experienced workers during their regular work shifts in an enclosures production site. Experimental activities included three sessions, two of which were conducted in-field (namely, at two workstations of the painting line, where panels were mounted and dismounted from the line; each session involved three participants), and one session was carried out in a realistic simulated environment (namely, the workstations were recreated in a laboratory; this session involved four participants). The effect of the exoskeleton was evaluated through electromyographic activity and perceived effort. After in-field sessions, device usability and user acceptance were also assessed. Data were reported individually for each participant. Results showed that the use of the exoskeleton reduced the total shoulder muscular activity compared to normal working conditions, in all subjects and experimental sessions. Similarly, the use of the exoskeleton resulted in reductions of the perceived effort in the shoulder, arm, and lower back. Overall, participants indicated high usability and acceptance of the device. This case series invites larger validation studies, also in diverse operational contexts.

Abstract:Work-related musculoskeletal disorders (WMSDs) have been a concern among healthcare workers, impacting their well-being and patient safety. Exoskeleton technologies have gained a growing interest as an ergonomic intervention for WMSDs. This scoping review explores exoskeleton effects on WMSDs among healthcare workers. A comprehensive search identified 9 eligible studies published in English between 2013 and 2023. Exoskeletons showed promising effects on objective measures, selectively reducing muscle activation in a task-dependent manner. They also improved force exertion and body posture parameters in specific scenarios. Subjectively, exoskeletons reduced discomfort, pain, fatigue, and received positive perceptions with acceptable usability. However, the impact on perceived exertion varied with tasks. While exoskeletons have shown potential in enhancing healthcare workers' well-being and performance, more work is required to refine their effectiveness and maximise benefits in different healthcare settings. The study revealed the need for standardised methodologies, consideration of participant characteristics, and optimisation of exoskeleton design. This scoping review investigated the effects of exoskeleton technologies on WMSDs among healthcare workers. The review identified 9 studies published between 2013 and 2023. Overall, exoskeletons showed promise in reducing muscle activation, improving force exertion, posture, and subjective measures. Standardised methodologies and design optimisation are needed to maximise benefits.

Abstract:OCCUPATIONAL APPLICATIONS This paper synthesizes the current literature on industrial-use, active and passive upper-limb exoskeletons, as an aid to health and safety specialists, ergonomics practitioners, and policy-makers that may be considering or regulating their use. Reported here are the effects of exoskeleton use on muscular activity; endurance and fatigue; kinematics and range of motion; productivity and quality; discomfort and pain; and usability and subjective feedback. A moderate level of evidence emerged, supporting the efficacy of passive upper limb exoskeletons in reducing muscular demands on the anterior and middle deltoid during occupational tasks, primarily those tasks involving overhead work. Given the nascence of research in this area, however, there is insufficient evidence to support the efficacy of active upper limb exoskeletons. Practitioners are encouraged to explore the effectiveness of passive exoskeleton use to reduce musculoskeletal disorder risk, particularly for overhead work. However, more research on the efficacy of active exoskeletons is recommended prior to field trials. TECHNICAL ABSTRACT Background: Recently, interest has surged regarding industrial-use exoskeletons as personal protective equipment to reduce work-related musculoskeletal disorders (MSDs). Purpose: Reviews existing evidence on the efficacy and effectiveness of industrial-use upper limb exoskeletons for reducing exposures to factors linked to MSD risk, and for improving productivity and quality. Methods: Scopus, PubMed, and CINAHL were searched systematically for relevant, English, peer-reviewed journal articles published between January 2000 and February 2019. Retained papers were quality-appraised using a modified CONSORT 2010 checklist, where an evidence synthesis algorithm was used to determine the level of evidence separately for active and passive exoskeletons. Results: A total of 799 articles were retrieved, 14 articles of which met inclusion criteria and were retained for analysis. Studies commonly reported on the effects of exoskeleton use on measures of muscular activity; endurance and fatigue; kinematics and range of motion; productivity and quality; discomfort and pain; and usability and subjective feedback. A moderate level of evidence was found supporting the efficacy of passive upper limb exoskeletons in reducing muscular demand on the anterior and middle deltoid muscles during occupational tasks, particularly overhead work; however, there is limited and insufficient evidence regarding impacts on measures related to kinematics and fatigue. There is insufficient evidence to support the use of active exoskeletons, due to the lack of medium-to-high quality studies with consistent findings. Conclusions: Considering the current state of evidence, those considering upper-limb exoskeletons for MSD prevention, particular for overhead work, may wish to consider passive systems rather than active systems at this time. High-quality research should continue to investigate the efficacy of active systems and effectiveness of both active and passive exoskeletons as an intervention to reduce MSDs. Exoskeleton efficacy and effectiveness research quality can be improved by including samples representative of industrial workers and by strengthening study power.
The Effects of Upper-Body Exoskeletons on Human Metabolic Cost and Thermal Response during Work Tasks—A Systematic Review

Abstract:Background: New wearable assistive devices (exoskeletons) have been developed for assisting people during work activity or rehabilitation. Although exoskeletons have been introduced into different occupational fields in an attempt to reduce the risk of work-related musculoskeletal disorders, the effectiveness of their use in workplaces still needs to be investigated. This systematic review focused on the effects of upper-body exoskeletons (UBEs) on human metabolic cost and thermophysiological response during upper-body work tasks. Methods: articles published until 22 September 2020 were selected from Scopus, Web of Science, and PubMed for eligibility and the potential risk of bias was assessed. Results: Nine articles resulted in being eligible for the metabolic aspects, and none for the thermal analysis. All the studies were based on comparisons between conditions with and without exoskeletons and considered a total of 94 participants (mainly males) performing tasks involving the trunk or overhead work, 7 back-support exoskeletons, and 1 upper-limb support exoskeleton. Eight studies found a significant reduction in the mean values of the metabolic or cardiorespiratory parameters considered and one found no differences. Conclusions: The reduction found represents a preliminary finding that needs to be confirmed in a wider range of conditions, especially in workplaces, where work tasks show different characteristics and durations compared to those simulated in the laboratory. Future developments should investigate the dependence of metabolic cost on specific UBE design approaches during tasks involving the trunk and the possible statistical correlation between the metabolic cost and the surface ElectroMyoGraphy (sEMG) parameters. Finally, it could be interesting to investigate the effect of exoskeletons on the human thermophysiological response.

Abstract:With rising manual work demands, physical assistance at the workplace is crucial, wherein the use of industrial exoskeletons ( i- EXOs) could be advantageous. However, outcomes of numerous laboratory studies may not be directly translated to field environments. To explore this discrepancy, we conducted a systematic review including 31 studies to identify and compare the approaches, techniques, and outcomes within field assessments of shoulder and back support i -EXOs. Findings revealed that the subjective approaches [i.e., discomfort (23), usability (22), acceptance/perspectives (21), risk of injury (8), posture (3), perceived workload (2)] were reported more common (27) compared to objective (15) approaches [muscular demand (14), kinematics (8), metabolic costs (5)]. High variability was also observed in the experimental methodologies, including control over activity, task physics/duration, sample size, and reported metrics/measures. In the current study, the detailed approaches, their subject-related factors, and observed trends have been discussed. In sum, a new guideline, including tools/technologies has been proposed that could be utilized for field evaluation of i -EXOs. Lastly, we discussed some of the common technical challenges experimenters face in evaluating i -EXOs in field environments. Efforts presented in this study seek to improve the generalizability in testing and implementing i -EXOs.

Abstract:This study investigates the biomechanical impact of a passive Arm-Support Exoskeleton (ASE) on workers in wool textile processing. Eight workers, equipped with surface electrodes for electromyography (EMG) recording, performed three industrial tasks, with and without the exoskeleton. All tasks were performed in an upright stance involving repetitive upper limbs actions and overhead work, each presenting different physical demands in terms of cycle duration, load handling and percentage of cycle time with shoulder flexion over 80°. The use of ASE consistently lowered muscle activity in the anterior and medial deltoid compared to the free condition (reduction in signal Root Mean Square (RMS) −21.6% and −13.6%, respectively), while no difference was found for the Erector Spinae Longissimus (ESL) muscle. All workers reported complete satisfaction with the ASE effectiveness as rated on Quebec User Evaluation of Satisfaction with Assistive Technology (QUEST), and 62% of the subjects rated the usability score as very high (>80 System Usability Scale (SUS)). The reduction in shoulder flexor muscle activity during the performance of industrial tasks is not correlated to the level of ergonomic risk involved. This preliminary study affirms the potential adoption of ASE as support for repetitive activities in wool textile processing, emphasizing its efficacy in reducing shoulder muscle activity. Positive worker acceptance and intention to use ASE supports its broader adoption as a preventive tool in the occupational sector.

Abstract:Background: The body of literature regarding the use of an upper limb exoskeleton during authentic working conditions is sparse. Objective: The aim of this study was to evaluate the effectiveness of an upper limb exoskeleton in reducing muscle strain during authentic industrial construction work. Methods: Fifteen male participants, comprising of roofers, scaffolders, builders, bricklayers, and graders performing overhead work participated in the study. During work without (REF) and with exoskeleton (EXO), muscle activity from 8 muscles, heart rate (HR), metabolic equivalent (MET), and upper arm elevation angles were recorded. Results: When using the exoskeleton, a significant reduction of 20.2% in average muscle activity of 8 muscles was found. The largest effect focused on m. deltoideus, where 46.2 and 32.2% reduction occurred in medial and anterior parts of the muscle, respectively. HR and MET were unaffected. Upper arm elevation angles were similar between REF and EXO, indicating equal biomechanical loading. Conclusions: This study indicates that exoskeletons show great promise in reducing the potential for musculoskeletal strain during authentic overhead construction work.

Abstract:Realizing secure human–robot interaction is a key aspect of Industry 5.0’s “human-centric”. As human involvement is essential for precise operations and wise decision-making in industrial production, industrial exoskeletons can assure worker safety while enhancing productivity during human–robot interaction. To promote industrial exoskeletons to better realize human–robot security in industrial interaction scenarios such as automobile assembly, logistics handling, etc., the key factors and challenges affecting industrial exoskeletons are analyzed, and the research progress is summarized. Industrial exoskeletons are classified into static-, dynamic-, and special-work scenario-oriented ones according to different work scenarios and needs. It also analyzes the industrial exoskeleton technology in five aspects: mechanism design, control system design, interactive information perception, variable stiffness drive and interactive interface design. On the basis of which, it analyzes the problems faced by the development of industrial exoskeletons at present, proposes the solutions to the problems, and looks forward to the development trend of realizing secure human–robot interaction with the help of industrial exoskeletons.
Barriers and facilitators to exoskeleton use in persons with spinal cord injury: a systematic review

Abstract:Purpose: Exoskeleton can assist individuals with spinal cord injuries (SCI) with simple movements and transform their lives by enhancing strength and mobility. Nonetheless, the current utilization outside of rehabilitation contexts is limited. To promote the widespread adoption of exoskeletons, it is crucial to consider the acceptance of these devices for both rehabilitation and functional purposes. This systematic review aims to identify the barriers or facilitators of the use of lower limbs exoskeletons, thereby providing strategies to improve interventions and increase the adoption of these devices. Methods: A comprehensive search was conducted in EMBASE, Web of Science, Scopus, Cochrane, and PubMed. Studies reporting barriers and facilitators of exoskeleton use were included. The studies' quality was assessed using the Mixed Methods Appraisal Tool and undertook a thematic content analysis for papers examining the barriers and facilitators. Results: Fifteen articles met the inclusion criteria. These revealed various factors that impact the utilization of exoskeletons. Factors like age, engagement in an active lifestyle, and motivation were identified as facilitators, while fear of falling and unfulfilled expectations were recognized as barriers. Physical aspects such as fatigue, neuropathic discomfort, and specific health conditions were found to be barriers. Conclusion: This systematic review provides a comprehensive overview of the barriers and facilitators to the use of exoskeleton technology. There are therefore still challenges to be faced, efforts must be made to improve its design, functionality, and accessibility. By addressing these barriers, exoskeletons can significantly improve the quality of life of people with SCI.

Abstract:Work-related musculoskeletal disorders (WRMSDs) are a leading cause of chronic conditions among working-age adults. Preventing these disorders is crucial to reducing their impact, and quantitative analysis through sensors can help identify their causes and guide ergonomic solutions. This systematic review aims to compile research from 2000 to 2023 published in English and sourced from Web of Science, Scopus, or PubMed that examines workers' movements during tasks using wearable sensor systems that are applicable in workplace settings. The goal is to identify the job sectors that have been studied and highlight tasks lacking ergonomic risk research. A total of 111 papers were selected through a screening process across three databases, assessed using the McMaster risk of bias tool. The studies span various job sectors and report on the use of different technologies for data collection and study population sizes. The review identifies existing research on WRMSD risks utilizing wearable systems in different job sectors, drawing attention to under-researched areas that warrant further study. It serves as a foundation for future research aimed at understanding the causes of WRMSDs and developing solutions supported by wearable technologies to mitigate these risks.

Abstract:Sometimes the automation equipment cannot solve all the problems for industrial enterprises, and human workers cannot be replaced by machines in production activities. The possibility that the workers develop work-related musculoskeletal disorders, while performing high intensity and repetitive installation and commissioning work over a long period of time, is very high. A mechanical design of a passive upper extremities exoskeleton suit to reduce the muscles effort of upper limbs is proposed in this article. Thereby, a decrease in the work-related musculoskeletal disorders risk is expected. To evaluate the ergonomic contribution of the passive upper extremities exoskeleton suit, both static and dynamic tool lift experiments were designed, in which 10 volunteers were asked to participate in the experiments. The surface electromyography is captured from these volunteers to measure the magnitude of muscle output forces that are applied with and then without passive upper extremities exoskeleton suit assistance during the process of manual handling, and the tests are collected for comparison. Results show that there is a significant decrease in the output force and fatigue in deltoid, biceps brachii, and brachioradiali, especially in biceps brachial which is up to 67.8%. The implementation of passive upper extremities exoskeleton suit is not only a benefit to reduce workers’ upper extremities fatigue but also ultimately increase the work efficiency by minimizing work-related musculoskeletal disorders and safety accidents.

Abstract:Occupational exoskeletons hold promise in preventing musculoskeletal disorders, but their effectiveness relies on their long-term use by workers.This study aims to characterize the adoption process of occupational exoskeletons by analyzing the experiences of 25 operators. Using a mixed-methods approach, both quantitative and qualitative data were collected before and during a four-week familiarization period. We primarily focused on users' expectations, subjective assessments over time, and initial experiences.Findings elucidate shifts in operators' perceptions of the devices over time. Through their narratives, we highlight how exoskeleton use impact operators' movements and the subsequent adaptations. Operators demonstrated diverse exploratory behaviors, indicating their efforts to get to grips with the effects of exoskeletons in their own ways.This study offers insights into the initial stages of occupational exoskeleton adoption, thus enriching our comprehension of rejection patterns and pathways toward their widespread acceptance.

Abstract:Wearable passive upper-limb exoskeletons have been proposed and commercialized as tools to improve the ergonomics of workers in repetitive or physically demanding tasks. In the study presented here, an innovative upper-limb exoskeleton is presented, along with experimental tests with human subjects. The device, called proto-MATE, is characterized by two distinguishing design features: a highly ergonomic human-robot kinematics architecture and bioinspired assistance, created to partially compensate for the user's arm weight. Experimental tests investigated the device's effects on the physical strain of eight upper-limb muscles. These tests also quantified the kinematic coupling between the device and the user by means of specific kinematics-related parameters. The protocol included overhead tasks that are representative of the target application and tasks that generalize nontargeted upperlimb movements and may occur in real working conditions.

Abstract:Background: In order to reduce the risk of work-related musculoskeletal disorders (WMSDs) several methods have been developed, accepted by the international literature and used in the workplace. The purpose of this systematic review was to describe recent implementations of wearable sensors for quantitative instrumental-based biomechanical risk assessments in prevention of WMSDs. Methods: Articles written until 7 May 2018 were selected from PubMed, Scopus, Google Scholar and Web of Science using specific keywords. Results: Instrumental approaches based on inertial measurement units and sEMG sensors have been used for direct evaluations to classify lifting tasks into low and high risk categories. Wearable sensors have also been used for direct instrumental evaluations in handling of low loads at high frequency activities by using the local myoelectric manifestation of muscle fatigue estimation. In the field of the rating of standard methods, on-body wireless sensors network-based approaches for real-time ergonomic assessment in industrial manufacturing have been proposed. Conclusions: Few studies foresee the use of wearable technologies for biomechanical risk assessment although the requirement to obtain increasingly quantitative evaluations, the recent miniaturization process and the need to follow a constantly evolving manual handling scenario is prompting their use.

Supporting Surgical Teams: Identifying Needs and Barriers for Exoskeleton Implementation in the Operating Room

Abstract:Objective: The objective of this study was to identify potential needs and barriers related to using exoskeletons to decrease musculoskeletal (MS) symptoms for workers in the operating room (OR). Background: MS symptoms and injuries adversely impact worker health and performance in surgical environments. Half of the surgical team members (e.g., surgeons, nurses, trainees) report MS symptoms during and after surgery. Although the ergonomic risks in surgery are well recognized, little has been done to develop and sustain effective interventions. Method: Surgical team members (n = 14) participated in focus groups, performed a 10-min simulated surgical task with a commercial upper-body exoskeleton, and then completed a usability questionnaire. Content analysis was conducted to determine relevant themes. Results: Four themes were identified: (1) characteristics of individuals, (2) perceived benefits, (3) environmental/societal factors, and (4) intervention characteristics. Participants noted that exoskeletons would benefit workers who stand in prolonged, static postures (e.g., holding instruments for visualization) and indicated that they could foresee a long-term decrease in MS symptoms with the intervention. Specifically, raising awareness of exoskeletons for early-career workers and obtaining buy-in from team members may increase future adoption of this technology. Mean participant responses from the System Usability Scale was 81.3 out of 100 (SD = 8.1), which was in the acceptable range of usability. Conclusion: Adoption factors were identified to implement exoskeletons in the OR, such as the indicated need for exoskeletons and usability. Exoskeletons may be beneficial in the OR, but barriers such as maintenance and safety to adoption will need to be addressed. Application: Findings from this work identify facilitators and barriers for sustained implementation of exoskeletons by surgical teams.

Abstract:Occupational Safety and Health (OSH) is defined through three objectives: (i) maintenance of workers’ health; (ii) improvement of working environment and safety; and (iii) promotion of a work culture that supports health and safety. In industry, the most frequent threat to workers’ health is musculoskeletal disorders. Furthermore, even routine tasks in certain work environments (nuclear, construction, disaster response, marine, chemical, etc.) expose workers to extreme risks like explosions, contaminations, fires, confined spaces, debris, toxic gases, etc. The goal of this research is to design and develop advanced collaborative robotic technologies towards: (i) reducing workers’ physical stress and improving their health through a novel modular full-body wearable exoskeleton with arm, lower-back, and leg modules, allowing full motion dexterity; and (ii) avoiding hazard-prone worker environments and improving safety through a new collaborative master-slave teleoperation system consisting of: (a) a hydraulicallydriven, quadruped field robot with a robotic manipulator arm for operation in hazardous environments; (b) a hand exoskeleton master device for teleoperation control. The article presents the current status of development of these technologies. Preliminary validation of the exoskeleton shows reductions in muscular efforts of up to 30% for the lower back. For the master-slave collaborative system, (a) the Hydraulic Quadruped robot prototypes can traverse rough environments through capabilities that include stair climbing, walking over obstacles, omni-directional trotting with step reflexes, running, jumping, and self-righting; (b) the 7 degrees-of-freedom (DOF) manipulator arm allows object manipulation in a large workspace with dexterous grasping of up to 160 N of payload; and (c) the novel HEXOTRAC 3-digit hand exoskeleton provides high-resolution tracking of fingers and provides force feedback for intuitive bilateral teleoperation of robotic manipulators. These next-generation industrial exoskeletons and collaborative teleoperation systems can address the emerging challenges in industrial workers’ health and safety.

Abstract:Exoskeletons are wearable devices that can enhance human strength and are used in various fields, such as healthcare and the manufacturing industry. However, poorly designed exoskeletons can strain the muscles and cause injuries to users. The objectives of this review paper are to review the ergonomics factors that contribute to a harmonious user-exoskeleton interaction and to explore the current trends, challenges, and future directions for developing ergonomically designed exoskeletons. In this review, 102 relevant papers published from 2015 to 2023 were retrieved from Web of Science, Scopus, and Google Scholar. These papers were considered in the analysis for gathering relevant information on the topic. The authors identified six ergonomics factors, namely kinematic compatibility, contact pressure, postural control, metabolic cost, cognitive workload, as well as task demands and workplace conditions, that can influence the interaction between users and exoskeletons. By understanding and addressing these ergonomics factors during the design and development process, exoskeleton designers can enhance the user experience and adoption of the devices in daily living activities and industrial applications.

Abstract:Work-related musculoskeletal injuries and disorders remain an important problem in the construction industry. Exoskeletons are an emerging wearable technology that assists or augments a user’s physical activity or capacity. This technology is a potential solution to reduce the physical demands and fatigue experienced by construction workers and help improve worker safety, health, and performance. As a first step towards enabling exoskeleton use in construction, we captured the perspectives of construction industry stakeholders regarding adopting exoskeletons and continued use in practice. Stakeholder responses highlighted several important questions and concerns, which were grouped using qualitative content analysis into three categories: (1) expected benefits, (2) exoskeleton technology adoption factors, and (3) perceived barriers to use. Uncertainties were expressed about the practical value and usability of exoskeleton technologies, and the impact of this technology on worker safety. Given this, and the limited state of current evidence, we summarize important research gaps that need to be addressed in future for successful adoption and use of exoskeleton technologies in the construction industry.
