Abstract:Age-related hearing loss (ARHL) is primarily attributed to inner-ear factors, yet the role of age-related middle ear characteristics remains elusive. Employing a finite element (FE) model, we conducted a comparative analysis with clinical data extracted from a retrospective cohort study involving 90 younger adults (mean age = 38.1 ± 7.7) and 111 older adults (mean age = 63.8 ± 8.4). The clinical dataset encompassed air-bone gap (ABG) measurements obtained through pure-tone audiometry (PTA) at frequencies of 0.5, 1.0, 2.0, and 4.0 kHz. FE results quantified the normalized stapes displacement value of the simulated form of air-bone gap ( ABGSim ) between the two age groups. The Mann-Whitney U test, with a significance threshold set at P < 0.05, was employed for statistical analysis. Furthermore, the study employs simulated auditory risk unit (ARU) results to evaluate basilar membrane (BM) damage. A significant intergroup discrepancy surfaces at 1.0 kHz ( ABGSim = 1.0; ABG: P = 0.008), with pronounced BM damage occurring within the speech frequency range (0.5–4.0 kHz) among older adults. The ARU consistently localizes within the 3–18 mm region from the base for both age groups. In conclusion, older adults exhibited significant conductive hearing loss (CHL) at 1.0 kHz but demonstrated a modest enhancement in middle ear sound transmission efficiency at 2.0 kHz. Furthermore, our research indicates that aging exacerbates damage to the BM when exposed to speech frequency excitation exceeding 90 dB sound pressure level (dB SPL).

Abstract:The difficulty to estimate the mechanical properties of the tympanic membrane (TM) is a limitation to understand the sound transmission mechanism. In this paper, based on finite element calculations, the sensitivity of the human hearing system to these properties is evaluated. The parameters that define the bending stiffness properties of the membrane have been studied, specifically two key parameters: Young’s modulus of the tympanic membrane and the thickness of the eardrum. Additionally, it has been completed with the evaluation of the presence of an initial prestrain inside the TM. Modal analysis is used to study the qualitative characteristics of the TM comparing with vibration patterns obtained by holography. Higher-order modes are shown as a tool to identify these properties. The results show that different combinations of elastic properties and prestrain provide similar responses. The presence of prestrain at the membrane adds more uncertainty, and it is pointed out as a source for the lack of agreement of some previous TM elastic modulus estimations.

Abstract:Establishment of three-dimensional finite-element model of the whole auditory system includes external ear, middle ear, and inner ear. The sound-solid-liquid coupling frequency response analysis of the model was carried out. The correctness of the FE model was verified by comparing the vibration modes of tympanic membrane and stapes footplate with the experimental data. According to calculation results of the model, we make use of the least squares method to fit out the distribution of sound pressure of external auditory canal and obtain the sound pressure function on the tympanic membrane which varies with frequency. Using the sound pressure function, the pressure distribution on the tympanic membrane can be directly derived from the sound pressure at the external auditory canal opening. The sound pressure function can make the boundary conditions of the middle ear structure more accurate in the mechanical research and improve the previous boundary treatment which only applied uniform pressure acting to the tympanic membrane.

Finite Element Modeling of Ossicle for Sound Transmission

Abstract:Objective:To evaluate the hearing improvement following partially ossicular replacement with different materials. Methods: The CT data of normal ossicle, were digitalized and imported into PATRAN( a world class commercial software. ) to build a 3 dimension finite element model. Then the ossicle model was analyzed based on dynamic transmitting vibration principles and the calculation results were evaluated and compared with various material preferences. Results:The ossicle 3D model established in the present study was in accord with biological structure of the human, the frequency reaction curves obtained by the model proposed in this study agreeed well with the fresh temporal bone ones. This research demonstrated that,with the increasing of elastic modulus,the amplitude of stapes footplate would also increase and get to the amplitude of stapes footplate more closely. After the elastic modulus getting bigger than 115000 gPa,the change rate of amplitude curve would be relatively small,which was almost a horizontal line. Conclusion:The model in this study is nicety. In stead of paying mainly attention to the material density when choosing the artificial replaced body materials in clinic,the physical preferences of the chosen material should focus mainly on the Young's modulus,when the elastic modulus increases so does,the hearing recovering effect,while as the elastic modulus comes to 115000 gpa,which means the elastic modulus arrive at the limit value for the artificial body, the hearing recovering effect no longer gets better.

Abstract:Summary The effects of cerebral dysfunction upon responses to the Expanded Word Association Test were studied among psychiatric patients. Psychiatric patients suffering from impairments in cerebral functioning showed significantly (p < .001) more blocking, multi-words and repetitions, irrespective of the type of stimulus word, than psychiatric patients judged free of such impairment. By weighing these variables with discriminant function coeficients it was possible to effectively discriminate between the groups, with only four misclassifications out of ninety patients. The results were discussed in relation to a previous experiment where similar findings had been obtained, and to the use of the test as a clinical tool.

Abstract:Osteoporosis (OP) is common with advancing age. Several studies have shown a strong correlation between OP and otosclerosis. However, no studies have investigated OP of the malleus, incus or stapes in the human middle ear, its effect on middle ear transfer function. Here, we investigate whether these three ossicles develop OP, and how this affects middle ear transfer function. The effect of OP on middle ear transfer function was investigated in simulations based on a finite element (FE) method. First, the FE model used in our previous study was refined, and optimized by introducing viscoelastic properties to selected soft tissues of the middle ear. Then, the FE model was used to simulate OP of the three ossicles and assess its influence on middle ear transfer function. Other possible age-related changes, such as stiffness of the joints or ligaments in the middle ear, were also investigated. The results indicated that OP of the ossicles could increase the high frequency displacement of both the umbo and stapes footplate (FP). However, the stiffness of the middle ear soft tissue can lead to the decrease of middle ear gain at lower frequencies. Furthermore, loosening of these joints or ligaments could increase displacement of the umbo and stapes FP. In conclusion, although age-related hearing loss is most commonly conceived of as sensorineural hearing loss (SNHL), we found that age-related changes may also include OP and changes in joint stiffness, but these will have little effect on middle ear transfer function in elderly people.

Abstract:Damping plays an important role in the middle ear (ME) sound transmission system. However, how to mechanically characterize the damping of ME soft tissues and the role of damping in ME sound transmission have not yet reached a consensus. In this paper, a finite element (FE) model of the partial external and ME of the human ear, considering both Rayleigh damping and viscoelastic damping for different soft tissues, is developed to quantitatively investigate the damping in soft tissues effects on the wide-frequency response of the ME sound transmission system. The model-derived results can capture the high-frequency (above 2 kHz) fluctuations and obtain the 0.9 kHz resonant frequency (RF) of the stapes velocity transfer function (SVTF) response. The results show that the damping of pars tensa (PT), stapedial annular ligament (SAL) and incudostapedial joints (ISJ) can help smooth the broadband response of the umbo and stapes footplate (SFP). It is found that, between 1 and 8 kHz, the damping of the PT increases the magnitude and phase delay of the SVTF above 2 kHz while the damping of the ISJ can avoid excessive phase delay of the SVTF, which is important in maintaining the synchronization in high-frequency vibration but has not been revealed before. Below 1 kHz, the damping of the SAL plays a more important role, and it can decrease the magnitude but increases the phase delay of the SVTF. This study has implications for a better understanding of the mechanism of ME sound transmission.

Abstract:Background and objective The fixation of ligament and tendon of the middle ear often occurs after chronic otitis media surgery. However, there are relatively few studies on the effect of ligament and tendon on sound transmission in the human middle ear. Here, the finite element model and lumped parameter model are used to study the effect of ligament and tendon fixation and detachment on sound transmission in human ear. Methods In this paper, the finite element model including the external auditory canal, middle ear and simplified inner ear is used to calculate and compare the middle ear frequency response of the normal and tympanosclerosis under pure tone stimulation. In addition, the lumped parametric model is taken into account to illustrate the effect of ligament and tendon stiffness on the human ear transmission system. Results The results indicate that the motion of the tympanic membrane and stapes is reduced by ligament and tendon fixation. Although ligament and tendon detachment have a limited effect in the piston-motion direction, the stability of motion in the plane perpendicular to the piston-motion direction is significantly reduced. Most significantly, the ligament and tendon fixation cause a hearing effect of about 18 dB, which is greater in the plane perpendicular to the piston-motion direction after ligament and tendon detachment than in the piston-motion direction. Conclusions In this study, the calculation accuracy of the lumped parameter and the finite element model is studied, and the effect of ligament and tendon on hearing loss is further explored through the finite element model with high calculation accuracy, which is helpful to understand the role of ligament and tendon in the sound transmission mechanism of the human middle ear. The study of ligament and tendon on conductive hearing loss provides a reference for clinical treatment of tympanosclerosis.

Abstract:Tympanosclerosis is an abnormal disorder of the middle ear or only the eardrum (i.e., myringosclerosis) in which there are calcium deposits. Normally, it is caused by recurrent middle ear infections. In this work, a 3D finite element model of the ear was developed, simulating different cases of tympanosclerosis. Through this model, the magnitude and the phase angle of the umbo and stapes displacement were obtained. The middle ear sound transfer function was determined for a stimulus of 80, 90 and 100 dB SPL, in a frequency range between 100 Hz and 10 kHz, applied on the outer surface of the eardrum in the external auditory canal. Depending on the tympanosclerosis affected area, the main conclusion is that worse results (leading to hearing loss) occur when all of the ossicular chain is affected.

Abstract:Three dimensional geometrical data of a tympanic membrane were obtained by using sections of undecalcified polymer-embedded temporal bone.An efficient and high precision method using mixed element of the quadrangle element and triangle element was introduced into the mesh of eardrum with HyperMesh software.The meshed model was imported into CAE software ANSYS.After the static analysis under different sound pressure as well as natural vibration characteristics and harmonic response analyses,the results were compared with the published data,the reliability based on the mixed element eardrum model was verified.This study provided a theoretic foundation for the subsequent study on the ossicular chain of the middle ear system.

Abstract:Hypothesis: The amount and viscosity of middle ear fluid probably influences the vibration function of the tympanic membrane (TM). Background: There has been much research into the mechanisms of hearing loss resulting from middle ear fluid in the previous study. However, further study is required to understand how the middle ear fluid affects the vibration function of the TM. Methods: Tests on a TM in a fresh cadaveric human temporal bone specimen under different simulated conditions were carried out. Saline (1 cSt) and silicone oil (100 cSt, 1 000 cSt, 12 500 cSt) were used to simulate middle ear fluid. The fluid approximately contacted 50% or 100% of TM, which was proportional to the fluid amount. Induced by stimulus signal with frequency domain from 0.25 to 8 kHz, the vibration at 6 points of the TM was measured by laser Doppler vibrometer (LDV), respectively. Data acquisition and processing were accomplished by self-developed software. Results: With the increase of the fluid amount, the magnitude of velocity transfer function reduced across all frequencies. The effect of fluid viscosity on the TM vibration varied and reduced in a complicated way when the fluid viscosity changed. Conclusion: Increasing fluid amount will significantly reduce the TM movement. The effect of fluid viscosity on the TM vibration was nonlinear and related to the fluid amount. The vibration at each point on the TM is frequency-dependent.

Abstract:The tympanic membrane (TM) is one of the most common routes to access the middle ear and inner ear for the treatment of hearing and balance pathologies. Since the TM is a soft thin biological tissue with small dimensions, using needles seems to be among the most practical interventional approaches. In this study, we proposed a finite-element (FE) analysis of needle-TM interactions that combines a 3D model of the TM and other main middle-ear structures in gerbil, and a 2D model of needle insertion into the TM based on the cohesive zone method (CZM). The TM was modelled using a 1st-order Ogden hyperelastic material and its properties were obtained by fitting to the experimental force-displacement plots of large deformation in the TM under needle indentation. The cohesive parameters were also acquired by calibrating the puncture force against the experimental data of needle insertion into the TM. These FE models were then used to obtain the deformation behaviour of the TM and other middle-ear structures due to the insertion force applied at different locations on the TM. Moreover, we investigated the effect of the TM thickness, the geometry of the needle (i.e., diameter and tip angle), and needle material on the insertion of needles into the TM. We also studied the penetration success of deformable needles.

Abstract:A three-dimensional geometrical model of middle ear including tympanic membrane,ossicles,tendoms and muscles was built up here using the serial tissue sections method.The lymph fluid of cochlear could be equivalent to a mass-stiffness-damping model,the physical parameters of the equivalent model were determined according to the published physical properties.Harmonic response analysis was made for the middle ear finite element model.The displacements of the eardrum umbo and the stapes footplate were compared with the experimental data taken from the literature.The reliability of the FE model was confirmed with the comparison.A further study by changing the physical parameters of the equivalent model was made then to observe the effect of the lymph fluid on the sound transmission.
Numerical model characterization of the sound transmission mechanism in the tympanic membrane from a high-speed digital holographic experiment in transient regime

Abstract:A methodology for the development of a finite element numerical model of the tympanic membrane (TM) based on experiments carried out in the time domain on a cadaveric human temporal bone is presented. Using a high-speed digital holographic (HDH) system, acoustically-induced transient displacements of the TM surface are obtained. The procedure is capable to generate and validate the finite element model of the TM by numerical and experimental data correlation. Reverse engineering approach is used to identify key material parameters that define the mechanical response of the TM. Finally, modal numerical simulations of the specimen are performed. Results show the feasibility of the methodology to obtain an accurate model of a specific specimen and to help interpret its behaviour with additional numerical simulations. STATEMENT OF SIGNIFICANCE: Improving knowledge of the dynamic behavior of the tympanic membrane is key to understanding the sound transmission system in human hearing and advance in the treatment of its pathologies. Recently we acquired a new tool to carry out experiments in transient regime by means of digital laser holography, capable of providing a large amount of information in a controlled transient test. In this work, these data are used to develop a methodology that generates a numerical model of the tympanic membrane based on numerical-experimental correlations. It is important to be able to develop models that fit specific patients. In this work, additional modal simulations are also presented that, in addition to validating the results, provide more information on the specimen.

Abstract:The ear canal (EC) is essential for sound transfer and crucial for hearing. Some pathological conditions may modify its morphology, leading to EC sound pressure redistribution, and stapes footplate displacement (FPD) gain alteration. However, no consensus regarding pathological EC and its impact on sound transfer has yet been achieved. To address the effect of morphology of EC on sound pressure redistribution and FPD gain. Varied pathological EC finite element (FE) models were constructed and analyzed based on FE analysis. The results indicated that canal wall down mastoidectomy decreases the second resonance frequency of the EC. The canal wall down mastoidectomy, with conchaplasty increased the first resonance frequency, but decreased the second along with the interval sound pressure gain increased, following which the FPD gain was altered. Stenosis of the EC at the internal portion decreased the second resonance frequency with minimal effect to the first part. When the stenosis moved to the outer portion of the EC, the first resonance frequency decreased, and the second one increased, along with the interval sound pressure gain decreased and FPD gain. Finally, the simplified EC model exerted a minimal effect on sound transfer. The minimal change in EC, such as simplification, straightening, canal wall down mastoidectomy, or enlargement, moderately affects the sound transfer; however, the EC stenosis deteriorates the sound transfer remarkably.

Abstract:Studies of genetic disorders of sensorineural hearing loss have been instrumental in delineating mechanisms that underlie the remarkable sensitivity and selectivity that are hallmarks of mammalian hearing. For example, genetic modifications of TECTA and TECTB, which are principal proteins that comprise the tectorial membrane (TM), have been shown to alter auditory thresholds and frequency tuning in ways that can be understood in terms of changes in the mechanical properties of the TM. Here, we investigate effects of genetic modification targeting CEACAM16, a third important TM protein. Loss of CEACAM16 has been recently shown to lead to progressive reductions in sensitivity. Whereas age-related hearing losses have previously been linked to changes in sensory receptor cells, the role of the TM in progressive hearing loss is largely unknown. Here, we show that TM stiffness and viscosity are significantly reduced in adult mice that lack functional CEACAM16 relative to age-matched wild-type controls. By contrast, these same mechanical properties of TMs from juvenile mice that lack functional CEACAM16 are more similar to those of wild-type mice. Thus, changes in hearing phenotype align with changes in TM material properties and can be understood in terms of the same TM wave properties that were previously used to characterize modifications of TECTA and TECTB. These results demonstrate that CEACAM16 is essential for maintaining TM mechanical and wave properties, which in turn are necessary for sustaining the remarkable sensitivity and selectivity of mammalian hearing with increasing age.
Material characterization of human middle ear using machine-learning-based surrogate models

Abstract:This study aims to introduce a novel non-invasive method for rapid material characterization of middle-ear structures, taking into consideration the invaluable insights provided by the mechanical properties of ear tissues. Valuable insights into various ear pathologies can be gleaned from the mechanical properties of ear tissues, yet conventional techniques for assessing these properties often entail invasive procedures that preclude their use on living patients. In this study, in the first step, we developed machine-learning models of the middle ear to predict its responses with a significantly lower computational cost in comparison to finite-element models. Leveraging findings from prior research, we focused on the most influential model parameters: the Young's modulus and thickness of the tympanic membrane and the Young's modulus of the stapedial annular ligament. The eXtreme Gradient Boosting (XGBoost) method was implemented for creating the machine-learning models. Subsequently, we combined the created machine-learning models with Bayesian optimization (BoTorch) for fast and efficient estimation of the Young's moduli of the tympanic membrane and the stapedial annular ligament. We demonstrate that the resultant surrogate models can fairly represent the vibrational responses of the umbo, stapes footplate, and vibration patterns of the tympanic membrane at most frequencies. Also, our proposed material characterization approach successfully estimated the Young's moduli of the tympanic membrane and stapedial annular ligament (separately and simultaneously) with values of mean absolute percentage error of less than 7%. The remarkable accuracy achieved through the proposed material characterization method underscores its potential for eventual clinical applications of estimating mechanical properties of the middle-ear structures for diagnostic purposes.

Abstract:As a bridge that transmits airborne sound signals to the auditory receptors of the inner ear, the eardrum and ossicular chain of the middle ear convert sound through two types of conversions: gas–solid (airborne sound signal–eardrum and ossicular chain) and solid–liquid (eardrum and ossicular chain–internal and external lymphatic fluid in the cochlea). This process concentrates and amplifies the sound to the inner ear through the lever principle structure formed by the three ossicles. However, diseases, hereditary factors, or trauma can reduce the sound transmission function of the middle ear. The effectiveness of middle ear replacement prostheses depends on their vibration response to the human auditory perception frequency, from the eardrum to the stapes plate. This response is influenced by the materials, geometry, and design of the replacement prosthesis and eardrum. This study explores the effects of different materials on hearing after artificial ossicular replacement. Usually, human temporal bone models are used for testing and validating numerical results. However, obtaining specimens from living humans is not always feasible. Therefore, we used three-dimensional printing technology to build a model of the middle ear to test the ossicular bone. Titanium alloy TC4, stainless steel 316L, and composite HA/PCL are chosen as materials for ossicular replacement. Using f&zwj;inite element analysis and an in vitro verification experiment, individual replacements of the ossicles and three bone material replacements were conducted for frequency response analysis. A Fiber-Reinforced Mesoscale Constitutive Model of Tympanic Membrane Considering Anisotropic Deformation

Abstract:The tympanic membrane (TM), located at the end of the ear canal, is a collagenous multi-layer soft tissue membrane with fibers highly aligned in radial and circumferential orientations. This unique multi-layer fiber ultrastructure makes TM’s mechanical behavior display both anisotropy and nonlinearity, which is important in sound transmission. However, the constitutive model of TM which includes both features has not been proposed. In this study, we develop a fiber-reinforced mesoscale constitutive model of TM which captures both anisotropic and nonlinear elastic mechanical behaviors. The TM is considered a continuum fiber-reinforced composite with two families of collagen fibers. Its overall properties are built up by integrating its heterogeneous material properties through the thickness. The homogenized mechanical properties are assumed to be uniformly distributed through TM’s thickness and superposed by three uncoupled elastic contributions of radial collagen fibers, circumferential collagen fibers, and an equivalent isotropic matrix. The model is calibrated using literature data through the inverse method. Simulation results indicate that specific collagen fibers alignment is responsible for the significant spatial and directional variation of deformation of the TM strip. With the appropriate strength criteria related to fiber deformation, the anisotropic localized failure mode of the TM strip observed in the experiment can be captured. The nonlinear nature and rotation of collagen fiber bundles are the origin of the nonlinear mechanical behavior of TM strips under uniaxial loading. The mesoscale constitutive model offers a different perspective on TM’s anisotropic and nonlinear elastic mechanical behavior. This research improves our understanding of the mechanical behavior of the TM and could help biomimetic graft development.

Abstract:Due to ethical issues and the nature of the ear, it is difficult to directly perform experimental measurements on living body elements of the human ear. Therefore, a numerical model has been developed to effectively assess the effect of the replacement of artificial ossicles on hearing in the inner ear. A healthy volunteer's right ear was scanned to obtain CT data, which were digitalized through the use of a self-compiling program and coalescent Patran-Nastran software to establish a 3D numerical model of the whole ear, and a frequency response of a healthy human ear was analyzed. The vibration characteristics of the basilar membrane (BM) after total ossicular replacement prosthesis (TORP) implantation were then analyzed. The results show that although the sound conduction function of the middle ear was restored after replacement of the TORP, the sensory sound function of the inner ear was affected. In the low frequency and medium frequency range, hearing loss was 5.2~10.7%. Meanwhile, in the middle–high frequency range, the replacement of a middle ear TORP in response to high sound pressure produced a high acoustic stimulation effect in the inner ear, making the inner ear structures susceptible to fatigue and more prone to fatigue damage compared to the structures in healthy individuals. This developed model is able to assess the effects of surgical operation on the entire hearing system.
