Abstract:Claridge, C. A. (Merck Sharp & Dohme Research Laboratories, Rahway, N.J.) and David Hendlin. Oxidation of glycerol by Streptococcus faecalis. J. Bacteriol. 84:1181–1186. 1962.—The nature of the factors in yeast autolysate essential for the oxidation of glycerol by Streptococcus faecalis F24 was examined. Two factors appear to be involved in the oxidation of glycerol. One factor was shown to be an inducer of the enzyme system required for glycerol oxidation; the other was shown to be α-lipoic acid. Minute quantities of glucose will “spark” growth of S. faecalis in a medium containing glycerol and acetate as carbon sources, probably by supplying sufficient energy for induction of the glycerol-oxidation system.
Prevalence and Clinico-pathologic Profile of Biopsied Gingival Lesions from 2 Thai Dental Schools

Abstract:Background: Gingiva can be afflicted by several pathological entities apart from gingivitis and periodontitis. There have been limited number of studies on gingival lesions, especially from Thailand. Aim: To analyze the prevalence and distribution of biopsied gingival lesions from Chulalongkorn and Rangsit Dental Schools, Thailand. Material and methods: Biopsy records of the participating institutions from 1995 to 2020 were reviewed for gingival lesions. The demographic data, site of lesions and diagnoses were collected. Data were analyzed by descriptive statistics. Results: From a total of 16,207 biopsies, 1,589 cases (10.2%) were diagnosed in the category of gingival lesions. The mean age ± SD of the patients was 42.3 ± 18.6 years with the highest prevalence being in the fourth decade of life (17.4%). A male-to-female ratio was 0.48:1. The most common location was the posterior mandible (27.4%). Regarding the type of gingival lesions, non-neoplastic lesions (88.5%) outnumbered neoplastic lesions (11.5%). The most common lesion was pyogenic granuloma, followed by irritation fibroma and peripheral ossifying fibroma. Among the neoplastic lesions, squamous cell carcinoma was the most prevalent lesion followed by papilloma and lymphoma. Conclusions: Gingival lesions mostly occur in the fourth decade of life and have a predilection for female patients. The majority of the lesions are located in the posterior mandible. Non-neoplastic lesion, especially the reactive subtype, is the most prevalent group and pyogenic granuloma is the most common gingival lesion. Data from this study represent biopsied gingival lesions from Thailand which may be different from those of other countries. A Clinicopathological Study of 22 Cases of Oral Melanotic Macule

Abstract:Oral melanotic macules, or melanosis, are uncommon oral mucosal lesions. Twenty-two cases of oral melanotic macule diagnosed and treated in our institute during the period from January 1989 to June 2004 were analyzed in this study. There were 8 male and 14 female patients. The mean age of patients at the time of diagnosis was 41.2 (range, 5~80) years. The most-common sites for melanotic macule were the buccal mucosa (7 cases) and lower lip (6 cases). Multiple lesions were noted in 5 cases. The clinical appearance of the lesion was frequently a black macule (19 cases). The greatest dimension of the lesion was 1.5cm with a mean of 0.5cm. Of the 22 cases, the clinical diagnosis was oral melanosis in 12, oral melanotic macule in 6, intramucosal nevus in 2, and others in 2. Excisional biopsy was the treatment of choice in 21 cases and incisional biopsy in 1 case. Recurrence of the lesion was found in 3 cases. We found that buccal mucosal lesions had a significantly lower mean density of pigmented basal epithelial cells/mm of the basement membrane (14±26 cells/mm) than of the lower lip (89±62 cells/mm, p=0.014) or gingival lesions (76±62 cells/mm, p=0.041). In contrast, buccal mucosal lesions had a significantly higher mean density of melanophages/mm of basement membrane(52±29 cells/mm) than gingival lesions (14±2 cells/mm, p=0.031). However, there was no significant difference ii the mean density of melanophages/mm of the basement membrane between buccal mucosal lesions and lower lip lesions (28±16 cells/mm, p=0.1). We concluded that oral melanotic macules in this study occurred most often in the fifth decade. The most-commonly affected sites were the buccal mucosa and lower lip. The black pigmentation of these buccal mucosal melanotic macules might have been due to an increase in the number of melanophages in the subepithelial connective tissue, and the black pigmentation of lower lip or gingival melanotic macule might have been due to an increase in the number of melanin-pigmented basal epithelial cells.

Abstract:A 56-year-old female presented to the dermatology clinic with pigmentation on the lips, gums and tongue for 5 years. The amout and range of lesions gradually increased. The patient denied having digestive tract symptoms. The family history was unremarkable. Physical examination showed brown to black spotted patches on the lips, gums and buccal mucosa (Fig. A, B). Light brown stripes appear on multiple nails (Fig. C). Except for the mucocutaneous lesions, no other significant signs were found. No abnormality was detected in gastrointestinal endoscopy. The feces examination did not find red blood cells. No endocrine abnormal was found. Dermoscopy demonstrated brownish uniform pigmentation on the pale red background of the lips. Brown longitudinal stripes with uniform color were visible on the nails. Histopathology of a biopsy taken from the lip showed hyperpigmentation in the basal layer of the epidermis (Fig. 1). A diagnosis of Laugier-Hunziker (LH) syndrome was made. LH syndrome is a rare clinical syndrome, often manifested as pigmentation in the lips and buccal mucosa, hands, feet and nails. The diagnosis of the disease is exclusive. It is necessary to distinguish it from other diseases with mucocutaneous pigmentation, including Peutz- Jeghers syndrome, Addison syndrome, and melanoma. Unlike Peutz- Jeghers syndrome, it often occurs in middle age and has no polyps in the gastrointestinal tract. Peutz- Jeghers syndrome often occurs at childhood and accompanied with multiple polyps in the gastrointestinal tract, although they have similar mucocutaneous appearance. Addison's disease often manifests as diffuse skin and mucous membrane pigmentation, especially in the exposed parts of the face and limbs. Abstract:Background/aim: Oral adverse events caused by anticancer drugs are diverse, but few reports have examined pigmentation of the oral mucosa. The aim of this study was to clarify the prevalence of oral mucosal pigmentation caused by anticancer drugs. Patients and methods: This single-centre retrospective study investigated patients who underwent oral examination in our hospital during cancer chemotherapy for 3 years from April 1, 2019 to March 31, 2021. Inclusion criteria were patients who could be followed-up for ≥3 months after completing chemotherapy with drugs that caused pigmentation. The primary predictive variable was the cancer chemotherapeutic agent used. The primary outcome variable was pigmentation of the oral mucosa. Collected data were statistically analysed using the χ2 test or Fisher's exact test, with the level of significance set at p<0.05. Results: A total of 388 patients were enrolled in the study. Eleven patients (2.8%) showed oral mucosal pigmentation. Drugs causing pigmentation [deposition rate (number of patients with deposits/users)] were TS-1 (combination of tegafur, gimeracil, and oteracil potassium) [12.2% (5/41)], paclitaxel [4.0% (2/50)], gemcitabine [5.0% (1/20)], cyclophosphamide [2.3% (1/42)], carboplatin [1.6% (1/64)], fluorouracil [2.3% (1/43)], and capecitabine [3.4% (1/29)]. Conclusion: Oral pigmentation due to cancer chemotherapy was found in 2.8% of patients. TS-1, carboplatin, cyclophosphamide, capecitabine, fluorouracil, gemcitabine, and paclitaxel caused pigmentation of the oral mucosa. Among these, TS-1 was the most likely to cause pigmentation, affecting 12.2% of users.

Abstract:The purpose of the present study was to evaluate the prevalence of oral mucosal lesions in Manipal, Karnataka State, India. A total of 1190 subjects who visited the department of oral medicine and radiology for diagnosis of various oral complaints over a period of 3 months were interviewed and clinically examined for oral mucosal lesions. The result showed the presence of one or more mucosal lesions in (41.2%) of the population. Fordyce's condition was observed most frequently (6.55%) followed by frictional keratosis (5.79%), fissured tongue (5.71%), leukoedema (3.78%), smoker's palate (2.77%), recurrent aphthae, oral submucous fibrosis (2.01%), oral malignancies (1.76%), leukoplakia (1.59%), median rhomboid glossitis (1.50%), candidiasis (1.3%), lichen planus (1.20%), varices (1.17%), traumatic ulcer and oral hairy leukoplakia (1.008%), denture stomatitis, geographic tongue, betel chewer's mucosa and irritational fibroma (0.84%), herpes labialis, angular cheilitis (0.58%), and mucocele (0.16%). Mucosal lesions like tobacco-related lesions (leukoplakia, smoker's palate, oral submucous fibrosis, and oral malignancies) were more prevalent among men than among women. Denture stomatitis, herpes labialis, and angular cheilitis occurred more frequently in the female population.
Oral pigmentation in physiologic conditions, post-inflammatory affections and systemic diseases

Abstract:Melanocytes are found throughout the oral mucosa but usually go unnoticed because of their relatively low level of pigment production. When focally or generally active in pigment production or proliferation they may be responsible for several affections in the oral mucosae ranging from physiologic pigmentation, systemic diseases to malignant neoplasms. The diagnosis of oral pigmentations (OP) is usually challenging for the physician, but a careful examination of the oral cavity may reveal the first manifestation of underlying systemic diseases. Therefore, a full medical history (including drug assumption and smoking) together with a general dermatological examination are mandatory and represent the first approach to OPs. When the diagnosis cannot be reached clinically with enough certainty, a biopsy for histological examination is needed, also in order to exclude possible life threatening conditions such as melanoma. Dermoscopy is another reliable diagnostic tool to make a differential diagnosis between melanocytic lesions and other conditions and then to manage the follow-up of patients. Few papers on the subject have been published in the dermatological literature and the oral cavity is often poorly investigated during routine dermatological examinations. We therefore decided to perform a review of benign OPs, classifying them into diffuse (physiological/racial pigmentations, smoker's melanosis, drug-induced hyperpigmentation, post-inflammatory hyperpigmentation, black hairy tongue, OPs associated to systemic diseases) and localized (amalgam tattoo, melanocytic nevi, melanoacanthoma, melanosis) lesions.

Abstract:Introduction: Oral mucosal lesions though mostly benign, may impair the quality of life of patients. Some may even progress to malignancies. Many physicians, including dermatologists, tend to skip oral examinations, missing many important diagnoses. Understanding the frequency and types of oral mucosal lesions in dermatological settings can help in early diagnosis, referral and adequate treatment. This study was done to determine demographic characteristics and clinical presentations of patients with oral mucosal lesions presenting to the out-patient department (OPD) of dermatology in Nepal Armed Police Force (APF) Hospital, Kathmandu. Methods: This cross-sectional descriptive study was conducted after obtaining the ethical approval from the Institutional Review Committee of Nepal APF Hospital. Retrospective data of 264 patients presenting with oral mucosal lesions to the dermatology OPD were collected from 1st January 2021 to 31st December 2023 by using a pre-formed proforma. Data was entered in SPSS software and descriptive statistics were computed. Results: Out of 13,832 cases, oral mucosal lesion was seen in 264 (1.90%) cases among which 153 (57.96%) cases were males with male female ratio of 1.37:1. Most common age group affected was 31-45 years 96 (36.36%). Buccal mucosa 86 (32.57%) was the commonest site involved followed by tongue 73 (27.65%). Aphthous ulcer 82 (31.06%) was the commonest lesion found followed by oral candidiasis 25 (9.46%) and oral lichen planus 24 (9.09%). Conclusions: Aphthous ulcer was the commonest oral mucosal lesion seen in patients visiting dermatology outpatient department of Nepal APF Hospital, with buccal mucosa being the commonest site affected.

Abstract:Background and objective: The most frequently seen intra-oral soft tissue is the gingiva. Most often, it is seen as coral-pink tissue that surrounds the neck of the teeth. Gingiva that encircles the tooth necks and covers the alveolar processes of the jaws is an intra-oral tissue that exhibits biomimetic features. The wide range of colors of the gingiva depends on the configuration of gingival vascularity, the degree of epithelial cornification, level of melanogenesis, and the depth of epithelialization. However, the color of the gingiva varies depending on the degree of melanin pigmentation. The current study aimed to identify the different distribution patterns of gingival color and determine the correlation between skin color, gender, and geographical area of origin. Materials and methods: A total of 839 subjects were involved in the study where the gingival color and skin tone were measured using the Dummett-Gupta Oral pigmentation Index (DOPI) combined with VITA VMK MASTER and skin shade method developed by Revlon (USA) and L'Oreal (France) for makeup foundation shades. One investigator was calibrated for the examination of the colors after being tested for normal color vision and color aptitude using the line test. Results: A significant association was found between skin color and gingival pigmentation (χ2 value (6) = 114.48; P = 0.001). It was also found that females (67.1%) significantly had darker gingiva than males (58.3%). The study statistics display that location of the individual was also statistically associated with melanin pigmentation of the gingiva (χ2 value (57) = 559.33; P = 0.001). Conclusion: The study concluded that gender, skin color, and individual location are significantly associated with gingival melanin pigmentation.

Abstract:Labial and oral melanotic macules are commonly encountered in a broad range of conditions ranging from physiologic pigmentation to a sign of an underlying life-threatening disease. Although Laugier-Hunziker syndrome (LHS) shares some features of labial and oral pigmentation with a variety of conditions, it is a benign and acquired condition, frequently associated with longitudinal melanonychia. Herein, the demographic, clinical, dermoscopic, and pathological aspects of LHS were reviewed comprehensively. The important differential diagnoses of mucocutaneous and nail pigmentation are provided. An accurate diagnosis is crucial to design a reasonable medical strategy, including management options, malignant transformation surveillance, and psychological support. It is important that clinicians conduct long-term follow-up and surveillance due to the potential risks of malignant transformation and local severe complications in some conditions.

Abstract:Introduction: Oral cavity lesions comprise a wide spectrum of diseases that varies from non-neoplastic to neoplastic. The clinical evaluation alone is insufficient for proper diagnosis in most cases. So, histopathological examination is the gold standard method for diagnosis and management of patients accordingly. Objective: The present study was done to evaluate the histopathological spectrum of oral cavity lesions and compare them in relation to age, sex, site, clinical features, risk factors, and clinical diagnoses. Methods: This prospective cross-sectional study enrolled 127 cases of oral biopsies which were received at the Department of Pathology, Tribhuvan University and Teaching Hospital, Kathmandu Nepal, from May 2018 to April 2019 for histopathological examination. Specimens were fixed in 10% formalin and subjected for tissue processing and Hematoxylin and Eosin stained sections. Data entry and analysis were done by using SPSS 24 version where frequency and percentile were calculated. Results: Total cases were 127 with slight female predilection and the age group of 50-60 years (mean age of 44.24 years) were commonly affected. The tongue being the most common site, frequently lesions presented as swelling. Most of the lesions were non-neoplastic comprising 45% whereas malignant lesions comprised 23.6%. Smoking increased the risk of malignancy by 2 fold. The most common benign lesions were squamous papilloma & fibroepithelial polyp whereas the malignant lesion was squamous cell carcinoma. Sixty percent of clinical diagnoses didn’t show correlation. Conclusions: Oral cavity lesions have a wide spectrum of distribution in age, sex, site, and clinical presentation. Initially, oral lesions may present with subtle symptoms which may cause underdiagnosis. Thus, histopathological diagnosis is a must to rule out malignancy. Abstract:Abstract Amelanotic melanoma (AM) is a subtype of melanoma where the lesion demonstrates little to no pigmentation. This can lead to delays in referral due to misdiagnosis, with studies showing a higher mortality rate compared with pigmented melanoma. However, there are few reports in the current literature regarding the characteristics associated with AM. In a 10-year period at a single centre in the north-west of England, patients with a histological diagnosis of AM were enrolled and data were collated retrospectively to determine the characteristics of AM lesions. Of the 16 patients identified, 69% were male, and the mean age at diagnosis was 78 years. The most common location for AM was the head (38%), which also demonstrated a higher mitotic rate (10.7 mm2) compared with the average (7.3 mm2). More than half of the lesions (56%) had been present for > 1 year. With a misdiagnosis rate of 88%, the likelihood of delays was evident. Dermoscopic findings were variable, but conspicuous vessels were noted in 63% of lesions. The most common vessel patterns were linear and irregular vessels, each identified in 19% of cases, with 13% of cases showing arborizing vessels. Our case series has demonstrated that AM is typically diagnosed in the older population, with a greater incidence rate noted in men. The age at diagnosis could be secondary to a delay in referral as AM is commonly not identified, with a misdiagnosis rate of 88% in our series. Lesions were most commonly located on the head region, whereas the back and legs were less commonly affected. This is contrary to melanoma, where the legs are most common for women and the back the most common for men. We suspect that a prolonged exposure of the head region to ultraviolet light is the reason for the highest distribution rate noted in the head. Furthermore, AM of the head was noted to have a greater mitotic rate than the average, potentially indicating more aggressive and lethal disease. The gold standard in clinical assessment is dermoscopic examination, with particular emphasis on identifying abnormal vessel features, a finding amplified due to a lack of pigmentation. We have demonstrated that AM can have a significant misdiagnosis rate with the possibility for a more aggressive cancer to form, potentially leading to unfavourable patient outcomes. We have identified key features of suspicious lesions that require assessment, notably abnormal vessels. Practitioners in primary and secondary care should be aware of and vigilant in identifying AM lesions. All suspicious lesions should undergo histopathological analysis to determine their exact nature.

Abstract:Introduction: The study indicated that XPD and XPG gene polymorphism is associated with the development of oral potentially malignant disorders (OPMDs) and oral squamous cell carcinoma. Xeroderma pigmentosa (XP) is a part of the complex DNA repair system. Xeroderma pigmentosum group G (XPG) and xeroderma pigmentosum group D (XPD) gene function in the nucleotide excision repair (NER) pathway. XPG and/or XPD gene alterations can cause defective DNA repair efficiency which ultimately leads to genomic instability and carcinogenesis. Thus, this study helps in early detection of OPMDs among individuals who have not yet developed any oral lesions and also helps in the management of oral squamous cell carcinoma as if XP gene polymorphism is known. Aims and Objectives: The aim of the study was to evaluate the expression of XPD and XPG gene polymorphism in oral squamous cell carcinoma cases. The study also had the objective to evaluate and compare the expression of XPG and XPD gene polymorphism in oral squamous cell carcinoma (OSCC) cases, tobacco chewers without any oral lesions, and normal healthy individuals without any habit. Materials and Method: A total of 150 subjects were included in the study and genotyped for the expression of XPD (AC) and XPG (GC) gene polymorphism using polymerase chain reaction (PCR) and agarose gel electrophoresis method. Results: XPD genotype for the study shows that most of the cases of OSCC show heterozygous (AC) genotype (64%), whereas in tobacco chewers without any oral lesions wild (AA) genotype (54%) is more common than other types. XPG genotype for the study shows that wild (GG) type is the most dominant genotype both in OSCC cases (78%) and tobacco chewers without any oral lesion (56%). Conclusion: The study shows the association of XPD and XPG gene polymorphism with the risk of developing OPMDs and oral cancer.
Analysis of Special Histopathological Characteristics of 1 011 Cases of Melanocytic Nevi

Abstract:Objective To analyze special histopathological characteristics of melanocytic nevi and their associations with age, gender, anatomical locations and pathological subtypes. Methods Clinical and histopathological data were collected from 1 011 patients with melanocytic nevi, who visited Beijing Hospital from January 2005 to January 2019, and analyzed retrospectively. Statistical analysis was carried out by using chi-square test for comparing enumeration data, and t test for comparing measurement data. Results Among the 1 011 patients with melanocytic nevi, the age at the clinic visit was 40.90 ± 19.19 years, and there were 289 males and 722 females. Lesional (biopsy) sites included the trunk (402 cases, 39.8%) , face and neck (268 cases, 26.5%) , extremities (138 cases, 13.6%) , hands and feet (133 cases, 13.2%) , scalp (53 cases, 5.2%) and vulva (17 cases, 1.7%) . Pathological subtypes included intradermal nevus (580 cases, 57.4%) , compound nevus (333 cases, 32.9%) and junctional nevus (98 cases, 9.7%) . Among special histopathological characteristics, neuralization and adipose cell hyperplasia were observed in 172 (17.0%) and 155 (15.3%) cases respectively, and the prevalence of neuralization and adipose cell hyperplasia was significantly higher in female patients than in male patients, higher in elderly patients than in young patients, and higher on the scalp than on the other sites (all P < 0.05) ; vascular proliferation was observed in 313 (31.0%) cases, and more commonly occurred on the scalp than on the other sites (P < 0.05) ; nevus cells distributed along the hair follicles/sebaceous glands were observed in 502 (49.7%) cases, and more commonly seen on the face and neck than on the other sites (P < 0.05) ; nevus cell lysis occurred in 203 (20.1%) cases, and fissures were observed in 384 (38.0%) . All the above histopathological characteristics were more frequently observed in the intradermal nevus subtype than in the compound nevus subtype (all P < 0.05) . Nevus cells distributed along the blood vessels were observed in 20 (2.0%) cases, and more commonly seen on the extremities than on the trunk, hands and feet (P < 0.05) , as well as in the compound nevus subtype than in the intradermal nevus subtype (P < 0.05) . Conclusions There are many special histopathological characteristics in melanocytic nevi, such as neuralization, adipose cell hyperplasia, vascular proliferation, and nevus cells distributed along the hair follicles/sebaceous glands, which are associated with patients′ age, gender, lesional locations and histopathological subtypes.
