Minute quantities of glucose will “spark” growth of S. faecalis in a medium containing glycerol and acetate as carbon sources, probably by supplying sufficient energy for induction of the glycerol-oxidation system.

Abstract:Melanocytes are found throughout the oral mucosa but usually go unnoticed because of their relatively low level of pigment production. When focally or generally active in pigment production or proliferation they may be responsible for several affections in the oral mucosae ranging from physiologic pigmentation, systemic diseases to malignant neoplasms. The diagnosis of oral pigmentations (OP) is usually challenging for the physician, but a careful examination of the oral cavity may reveal the first manifestation of underlying systemic diseases. Therefore, a full medical history (including drug assumption and smoking) together with a general dermatological examination are mandatory and represent the first approach to OPs. When the diagnosis cannot be reached clinically with enough certainty, a biopsy for histological examination is needed, also in order to exclude possible life threatening conditions such as melanoma. Dermoscopy is another reliable diagnostic tool to make a differential diagnosis between melanocytic lesions and other conditions and then to manage the follow-up of patients. Few papers on the subject have been published in the dermatological literature and the oral cavity is often poorly investigated during routine dermatological examinations. We therefore decided to perform a review of benign OPs, classifying them into diffuse (physiological/racial pigmentations, smoker's melanosis, drug-induced hyperpigmentation, post-inflammatory hyperpigmentation, black hairy tongue, OPs associated to systemic diseases) and localized (amalgam tattoo, melanocytic nevi, melanoacanthoma, melanosis) lesions.

Abstract:Background Several lesions can affect the oral and maxillofacial region, with trends influenced by lifestyle and age. Aim To investigate the occurrence of oral and maxillofacial lesions in paediatric patients assisted in an oral medicine diagnostic service in Brazil over 20 years. Design A retrospective cross‐sectional study was performed in an oral medicine diagnostic service (1999–2019). Data included sex, age, anatomic site, and diagnosis from clinical records of children (0–9 years) and adolescents (10–19 years). Lesions were diagnosed through clinical examination. Biopsy and histopathologic examination were performed when necessary. Results Among 10 530 records, 702 (6.67%) were from patients aged 0–19 years diagnosed with oral or maxillofacial lesions. A high frequency of females was observed (58.1%). Reactive/inflammatory lesions were the most common group of lesions (45.2%), followed by abnormalities of teeth/developmental defects (17.8%) and pulpal and periapical diseases (8.5%). Mucocele/ranula was the most common lesion in children (29.3%) and adolescents (25.3%). Lips were the most affected anatomic sites. Conclusion Most lesions were benign, and mucocele/ranula was the most frequent. Our results did not differ from those of histopathological‐based studies, but they provided information on the occurrence of oral and maxillofacial lesions to dentists and paediatricians.

Abstract:A fifty-six-year-old female presented to the service in June 2021, complaining of the appearance of a large, dark mass that had been present for about 5 months. She also reported experiencing pain and weight loss. She has been a smoker since the age of 15 and is also an alcoholic. Upon clinical examination, an exophytic, lobulated, pigmented, and expansive nodule was observed. This nodule bled when touched and had a firm consistency. It affected the right hard and soft palate, crossed the midline, and extended to the upper attached gingiva on the same side.

Abstract:Background To investigate the incidence and demographic profile of hamartomatous, choristomatous, and teratoid lesions in a Brazilian population over a 49 years-period. Methods A retrospective cross-sectional study was performed, and data regarding demographic and clinical aspects were obtained from the medical records of a [removed for blind peer review] referral center (1970-2019). The collected data were submitted to descriptive analysis and Pearson's chi-square, Fisher's exact, and Kruskal–Wallis tests ( p ≤ 0.05). Results In a total of 16,412 medical records analyzed, 300 (1.83%) were hamartomatous, 2 (0.01%) choristomatous, and 1 (0.01%) teratoid lesions. Hamartomas were most diagnosed in females and adults. Statistical significance was observed between hamartoma and age group ( p < 0.001). Odontoma was the most frequent hamartomatous lesion. In choristomatous and teratoid lesions, there was no occurrence in males. The jaws were the most affected anatomical site by hamartoma. Choristomas were observed on the mandible and tongue, while a teratoid lesion was seen on the floor of the mouth. Conclusions Low occurrence of choristomatous and teratoid lesions over hamartomatous lesions and a heterogeneous occurrence profile regarding sex, age group, and anatomic site were observed. Hamartomas are relatively common and benign conditions that may cause damage and require special care during dental treatment. Thus, the dentist needs to be able to identify and treat them appropriately. Likewise, although choristomas and teratomas present no risk to patients and have a lower occurrence than hamartomas, they also require treatment.

Abstract:Oral melanotic macules, or melanosis, are uncommon oral mucosal lesions. Twenty-two cases of oral melanotic macule diagnosed and treated in our institute during the period from January 1989 to June 2004 were analyzed in this study. There were 8 male and 14 female patients. The mean age of patients at the time of diagnosis was 41.2 (range, 5~80) years. The most-common sites for melanotic macule were the buccal mucosa (7 cases) and lower lip (6 cases). Multiple lesions were noted in 5 cases. The clinical appearance of the lesion was frequently a black macule (19 cases). The greatest dimension of the lesion was 1.5cm with a mean of 0.5cm. Of the 22 cases, the clinical diagnosis was oral melanosis in 12, oral melanotic macule in 6, intramucosal nevus in 2, and others in 2. Excisional biopsy was the treatment of choice in 21 cases and incisional biopsy in 1 case. Recurrence of the lesion was found in 3 cases. We found that buccal mucosal lesions had a significantly lower mean density of pigmented basal epithelial cells/mm of the basement membrane (14±26 cells/mm) than of the lower lip (89±62 cells/mm, p=0.014) or gingival lesions (76±62 cells/mm, p=0.041). In contrast, buccal mucosal lesions had a significantly higher mean density of melanophages/mm of basement membrane(52±29 cells/mm) than gingival lesions (14±2 cells/mm, p=0.031). However, there was no significant difference ii the mean density of melanophages/mm of the basement membrane between buccal mucosal lesions and lower lip lesions (28±16 cells/mm, p=0.1). We concluded that oral melanotic macules in this study occurred most often in the fifth decade. The most-commonly affected sites were the buccal mucosa and lower lip. The black pigmentation of these buccal mucosal melanotic macules might have been due to an increase in the number of melanophages in the subepithelial connective tissue, and the black pigmentation of lower lip or gingival melanotic macule might have been due to an increase in the number of melanin-pigmented basal epithelial cells.

Abstract:Diagnosing solitary pink skin lesions poses a significant challenge due to the scarcity of specific clinical and dermoscopic criteria. Several benign lesions, such as cherry angioma, clear cell acanthoma, dermal nevus, keloid, hypertrophic scar, and Spitz nevus, often exhibit similar clinical and dermoscopic features. This similarity extends to some malignant lesions, including basal cell carcinoma, actinic keratosis, and amelanotic melanoma, making differentiation difficult. Recent studies highlight the enhanced diagnostic accuracy of reflectance confocal microscopy (RCM), which offers increased sensitivity and specificity compared to dermoscopy alone for diagnosing skin cancer. This study aims to summarize the application of dermoscopy and RCM in distinguishing between benign and malignant pinkish–reddish skin lesions. The integration of RCM with traditional dermoscopic techniques improves the ability to accurately identify and differentiate these lesions. However, it is crucial to note that for any suspicious lesions, a final diagnosis must be confirmed through surgical excision and histopathological evaluation. This comprehensive approach ensures accurate diagnosis and appropriate treatment, highlighting the importance of combining advanced imaging techniques in clinical practice.

Abstract:The objective of this study is to present a case of a patient diagnosed with oral lichen planus (OLP), proliferative verrucous leukoplakia (PVL), and lip cancer. The patient is a 71-year-old male who initially sought a stomatology service due to an ulcerated lesion on the lower lip. During clinical examination, a whitish plaque that could not be removed by scraping was observed on the tongue, and a 2 cm ulcer was noticed on the lower lip. Histopathological analysis confirmed the presence of an OLP lesion on the tongue and a diagnosis of squamous cell carcinoma in the lip.

Abstract:Data on the occurrence and clinicopathological characteristics of actinic cheilitis (AC) and lip squamous cell carcinoma (LSCC) are well studied; however, they are based on studies limited to a single centre. Herein, we described the frequency of AC and LSCC submitted to microscopic examination from representative geographic regions of Brazil. A retrospective multicentre study was performed on biopsies obtained from 1953 to 2018 at 10 Brazilian oral and maxillofacial pathology centres. A total of 198,709 biopsy specimens were surveyed. Sociodemographic data and clinicopathologic characteristics were analysed. A total of 2017 cases of ACs (1.0%) and 850 cases of LSCCs (0.4%) were recorded. A strong fair-skinned (> 87%) male (> 70%) predilection was observed in both conditions. The mean age was 54.8 ± 18.7 for individuals with AC and 57.8 ± 19.0 for individuals with LSCC. The most commonly affected site was the lower lip (> 90%). This is a large multicentre study of AC and LSCC from Brazil. The frequency and clinicopathological features of AC and LSCC were similar to those described worldwide. This study provides robust and representative epidemiological data of these conditions for the scientific community.

Abstract:Introduction: Pigmentation of lip and/or genitalia is mainly due to the development of benign melanotic macules, with a less occurrence of melanocytic and other non-melanocytic lesions. Mucosal melanoma has worse prognosis compared with cutaneous counterpart, hence identification of atypical features for an early diagnosis is crucial. Objectives: The aim of this study was to report further data of confocal features characterizing pigmented mucosal lesions of genital area and of the lips and test the diagnostic role of the reflectance confocal microscopy (RCM)lip score. Methods: Clinical, dermoscopic and RCM images of histologically proven pigmented lesions, involving the genital area (vulva or glans penis) and lip, were retrospectively reviewed. RCM images were evaluated for malignant criteria, and statistical analysis was conducted for categorical variables. Results: Seventy pigmented lesions were included in the study and divided into two groups based on the body area location: lip (17) and genital area (53). Architectural disarray (P = 0.002), dendritic (P = 0.031) and roundish cells in epidermis (P < 0.0001), interpapillary dendritic cells (P = 0.039) and junctional atypical cells (P = 0.002) were associated to genital melanoma. Melanoma involving the lip was characterized by roundish cells in epidermis, a criterion found in one labial benign lesion, only (P = 0.005). Main limitations of the study are the inclusion of low melanomas and the presence of epidermal dendritic cells in melanosis and melanoma, as a confusing factor in imaging. Conclusions : Dermatologists should consider confocal microscopy as an adjunctive tool to dermoscopy in the differential diagnosis of pigmented mucosal lesions, especially in presence of clinical and dermoscopic findings suspicious for malignancy.

Abstract:Lupus, Ahead of Print. BackgroundSystemic lupus erythematosus (SLE) is a multifactorial autoimmune disease that may affect the oral mucosa. The variable spectrum of oral lesions observed in SLE can pose challenges in diagnosis, particularly when the lesions occur in isolation. The aim of this study was to describe the oral lesions occurring in patients with SLE from Latin America.MethodsThis collaborative record-based study involving 11 oral and maxillofacial pathology and medicine services across Venezuela, Argentina, Chile, Brazil, and Mexico describes the clinicopathological profile of SLE-related oral lesions.ResultsSeventy patients with SLE and oral lesions were included in the study. The majority were females (75.7%; female/male ratio: 3.1:1) and white (62.1%), with a mean age of 38.4 years (range: 11–77 years). The most common site of oral lesions was the hard/soft palate (32.0%). Clinically, oral lesions predominantly presented as ulcers (26.6%), erosions (26.6%), and white lesions (23.4%). Isolated oral lesions occurred in 65.2% of individuals, while cutaneous manifestations occurred in 80.3%. The main clinical diagnostic hypothesis in 71.4% of cases was an immune-mediated disease. Oral biopsies followed by histopathological analysis were performed in 50 cases.ConclusionOral lesions of SLE exhibit a variety of clinical and histopathological features. A key point in diagnosis is that unusual oral changes without an obvious local cause may indicate a possible systemic condition presenting with oral lesions. A multidisciplinary approach, which includes regular oral examination, is warranted to identify oral lesions and provide treatment.

Abstract:Background: Patients with mucosal lesions form a significant number of routine outpatients presenting to the dermatology department where diagnostic confirmation using histopathological examination of mucosal biopsy is neither feasible nor warranted in every patient.Objective: To study the dermoscopic features of various mucosal lesions affecting the oral cavity and to assess the reliability of mucoscopy vis-a-vis clinico-laboratory findings.Materials and Methods: An observational, cross-sectional, hospital-based study was conducted over a period of 2 years from March 2019 to February 2021 in the dermatology outpatient department. Patients presenting with oral mucosal lesions, with or without associated cutaneous involvement, were recruited for mucoscopic evaluation after taking an informed written consent. A detailed history and clinical examination, with emphasis on mucocutaneous examination, was performed and findings were recorded on a standard predesigned proforma. Mucoscopy of oral mucosa was carried out using a handheld dermoscope as well as Universal Serial Bus connected video-dermoscope in both nonpolarized and polarized modes. The different mucoscopic features were seen at these sites, compared with each other, analyzed and findings were recorded. A diagnosis was made on the basis of mucoscopic findings and correlated with clinical diagnosis. The data was analyzed using appropriate statistical tests.Results: The mean age of patients was 34.3 years and the mean lesional duration was 68.2 weeks. Oral lichen planus (18.66%) was the most common disorder studied, followed by recurrent apthous stomatitis (16.00%), pigmentary lesions (12.66%), vascular disorders (12.00%), mucocele (5.33%), pemphigus vulgaris (4.66%), and discoid lupus erythematosus (4.66%).Conclusion: Dermoscopy in oral lesions facilitates the visualization of the mucosal surface and provides quick confirmation of diagnosis in various mucosal disorders with advanced diagnostic accuracy. Mucoscopy was found helpful in differentiating the oral ulcers, which are a presenting feature of various serious mucocutaneous disorders. Mucoscopy could be a helpful aid in diagnosing pigmented skin lesions and alleviating the apprehension regarding oral melanoma and serve as a screening tool in case of squamous cell carcinoma lips.Limitations: Confirmatory histopathological analysis and correlation with mucoscopic findings could not be established in our study.

Abstract:OBJECTIVE: To determine the prevalence, epidemiological profile, and clinical characteristics of Oral or Oropharyngeal Mucosal Lesions (OOPML) in patients attended at the Otorhinolaryngology Service of the Evandro Chagas National Institute of Infectious Diseases (INI-FIOCRUZ) from 2005 to 2017.METHODS: Statistical analysis of descriptive data from medical records (gender, age, education level, skin color, origin, smoking, alcoholism, HIV co-infection, time of disease evolution, first symptom, and OOPML location) was performed.RESULTS: Of 7551 patients attended at the service, 620 (8.2%) were included in the study. OOPML were classified into developmental anomalies (n = 3), infectious diseases (non-granulomatous n = 220; granulomatous n = 155), autoimmune diseases (n = 24), neoplasms (benign n = 13; malignant, n = 103), and unclassified epithelial/soft tissue diseases (n = 102). OOPML of infectious diseases (60.5%) and neoplasms (18.7%) were the most frequent. The predominant demographics of patients with OOPML were: males (63.5%), white (53.5%), and those in the fifth to sixth decades of life (43.3%). Local pain (18.1%) and odynophagia (15%) were the most reported first symptoms, and the most frequent OOPML sites were the palatine tonsil (28.5%), hard palate (22.7%), and tongue (20.3%). The median evolution time was three months.CONCLUSIONS: Infectious OOPML were the most frequent, as expected in a reference center for infectious diseases, and thus, they are likely to be less frequent in general care and/or dental services. Underreporting of OOPML is possible, as oral/oropharyngeal examination is often not included in the routine medical examination. Oral cavity/oropharynx examination should be performed by specialists, such as dentists and otorhinolaryngologists, who have the expertise in identifying OOPML, even in incipient/asymptomatic cases. Given the numerous diseases in which OOPML can present, diagnosis could be facilitated by multidisciplinary teams, potentially enabling the early treatment of diseases, and thus, reduce morbidity and improve prognosis. The use of standardized medical records for oral/oropharyngeal systematic examination could provide relevant tools for differential diagnoses and information for new clinical-epidemiological studies.LEVEL OF EVIDENCE: Level 3.

Abstract:Oral malignant melanoma (OMM) represents an extremely rare neoplasm. Unlike cutaneous melanomas, the risk factors and pathogenesis of OMM are poorly understood. Here, we report a case of OMM occurring in a 69-year-old female patient. Ten years ago, the patient was referred for evaluation of an extensive, asymptomatic pigmented lesion on the left side of the hard palate, beneath a total prosthesis. An incisional biopsy was performed, and histopathological analysis revealed a variable proliferation of fusiform and pigmented epithelioid cells, exhibiting cellular pleomorphism and a high mitotic index.
