Abstract:We compared the mechanical properties of morselized cancellous bone grafts of two sizes: 7-10 mm bone and small slurry bone (about 2 mm). The in vitro test was designed to simulate the hammer and impactor system for impaction bone grafting used in hip arthroplasty clinical practice. The 7-10 mm bone grafts showed higher height, elastic modulus, and massive extrusion strength than those of the small slurry bone grafts. No difference was found in yield strength. The bone mineral density of the 7-10 mm grafts continued to increase during impaction and became higher than that of the small slurry bone grafts after 10 impactions. Our results demonstrated that the small slurry bone grafts exhibit worse mechanical properties as compared with the 7-10 mm bone grafts, which implies that the use of this material in reconstruction of a bone defect in the acetabulum should be limited. Optimized reamer geometry for controlled reaming of the proximal femur

Abstract:Preparation of the femoral proximal medullary cavity by reaming is essential for intramedullary nail osteosynthesis and hip revision arthroplasty. The use of reamers sometimes exerts high torsional forces on the bone. Design and direction of rotation of the reamer are potential influencing factors. The aim of this biomechanical study is to evaluate the best combination of a right- or left-cutting reamer with a clockwise- or counterclockwise-rotating insert in terms of preparation and safety. Right- and left-cutting reamers with conical design were each introduced into five synthetic femurs in both clockwise and counterclockwise rotation with constant feed force. A specially constructed test system was used for this series of tests, with which the respective intramedullary channel were reamed step by step. This was then used to determine the required torque. In addition, the feed rate measurement was analyzed using a modified digital caliper. The feed rates of the reamers with rotation in the same direction as the cutting direction were significantly increased compared to rotation in the opposite cutting direction (CCRLC vs. CCRRC 76.8 ± 9.0 mm/s vs. 25.2 ± 8.3 mm/s and CRRC vs. CRLC 54.3 ± 12.3 mm/s vs. 19.3 ± 0.6 mm/s; p < 0.01). In contrast, the mean torque during the reaming process was identical in all four groups. When preparing the proximal femoral medullary cavity, especially in cases with fragile bone structure, the available reamers should be introduced in opposite rotation to the cutting direction to achieve a more controllable feed of the reamer. Left-cutting reamers represent an alternative, using them in the usual clockwise-rotating technique to reduce the risk of complications during reaming.

3D Printing and Computer-Aided Design for Precision Osteotomy-Aided Modules in Bone Biomechanical Study

Abstract:Precise and shape-matching osteotomy models are determinants of the experimental homogeneity in the assessment of orthopedic biomechanical properties. At present, however, publications on detailed description of osteotomy in bone biomechanical study are scanty. The purposes of this study were to design a new method of osteotomy-aided module production for bone biomechanical study with the help of three-dimensional (3D) printing and computer-aided design (CAD) and to test the accuracy of osteotomy. Fourteen fourth-generation composite femurs were analyzed. The composite bone was scanned using computed tomography (CT) scanner and loaded in Mimics for reconstruction and, then, imported into 3-Matic software to design intertrochanteric region, distal femur, and rotation control lever models. 3D printer was used to print each component. After assembling Sawbones and osteotomy modules, a horizontal band-saw was used to create fracture models. The volume and mass of intermediate fragments were calculated and analyzed. Satisfactory osteotomies of all composite Sawbones were achieved. The mean volume and mass of intermediate fragments were 21.0 ± 1.5 mm3 and 19.0 ± 1.2 g, respectively. Range of deviation from average of volumes was -1.9 - 2.8 mm3 and most of these deviations fall within the range of -1.4 - 2.1 mm3. Range of deviation from average of mass was -2.0 - 1.6 g and most of these deviations fall within the range of -1.4 - 1.6 g. One-dimensional histogram of deviation from average shows the precise and stable osteotomy performed based on the modules accordingly. A new method of osteotomy-aided module production for bone biomechanical study with the help of 3D printing and CAD was designed and the accuracy of osteotomy was verified. This method is expected to achieve homogeneity and standardization of osteotomy in bone biomechanical study.

Abstract:Revision total hip arthroplasty is often complicated by femoral or acetabular loss of bone stock and periprosthetic fracture.Restoration of bone stock to either provide stability for revision components or to provide bone stock for future revision procedures is often a critical element in revision joint arthroplasty.In these cases,bone grafting is usually needed.The various commonly-used grafts have their limitations.Recent researches have centered on the use of osteoinductive materials such as recombinant human bone morphogenetic proteins(rhBMPs)in revision total hip arthroplasty.They are used to treat cavity or segmental defects or to augment the healing in incorporation of allografts.Although these studies showed that bone morphogenetic proteins had their great advantages as compared with other bone grafts,more researches are needed.This article will summarize the results of these studies.
A cortical bone milling force model based on orthogonal cutting distribution method

Abstract:In orthopedic surgery, the bone milling force has attracted attention owing to its significant influence on bone cracks and the breaking of tools. It is necessary to build a milling force model to improve the process of bone milling. This paper proposes a cortical bone milling force model based on the orthogonal cutting distribution method (OCDM), explaining the effect of anisotropic bone materials on milling force. According to the model, the bone milling force could be represented by the equivalent effect of a transient cutting force in a rotating period, and the transient milling force could be calculated by the transient milling force coefficients, cutting thickness, and cutting width. Based on the OCDM, the change in transient cutting force coefficients during slotting can be described by using a quadratic polynomial. Subsequently, the force model is updated for robotic bone milling, considering the low stiffness of the robot arm. Next, an experimental platform for robotic bone milling is built to simulate the milling process in clinical operation, and the machining signal is employed to calculate the milling force. Finally, according to the experimental result, the rationality of the force model is verified by the contrast between the measured and predicted forces. The milling force model can satisfy the accuracy requirement for predicting the milling force in the different processing directions, and it could promote the development of force control in orthopedic surgery.

Abstract:Abstract The ultrasonic bone scalpel is a cutting-edge surgical instrument that leverages ultrasonic mechanical effects to remove bone tissue with precision, safety, and efficacy. While the adoption of the instrument has surged, the intricate relationship between its mechanical effects and the efficiency of bone removal remains ambiguous. This study aimed to elucidate the quantitative interplay between the mechanical parameters of the instrument and its bone removal performance. To achieve this, we engineered a testing apparatus capable of consistently regulating the scalpel’s cutting force, traversal speed, and depth of cut. The bone removal proficiency of the instrument was then assessed under varying conditions: cutting forces, ultrasonic power outputs, tip traversal speeds, and bone densities. Comprehensive data analysis unveiled a distinct quantitative correlation between the aforementioned parameters and bone removal rates. Furthermore, we identified optimal parameter combinations that maximize cutting efficiency. These insights are pivotal for the design, fabrication, evaluation, and clinical application of ultrasonic bone scalpels, offering a robust reference for future advancements in the field.

Abstract:Cutting surgery is a commonly used method to treat femoral necrosis,which includes advanced mechanical-assisted surgical methodologies such as drilling automation or minimally invasive process,which tend to progressively replace the traditional,manhanded methods;drill bit design optimization and analysis of the cutting properties of bone are of great interest regarding the conception of such innovative mechanism.A new system able to enlarge holes aimed at practice minimally invasive surgery has been analyzed,and an experiment platform has been used to reproduce design parameters of this machine,to achieve the relationship between rotation speed,feed rate and axial force,thus providing a base to optimally design the machine.

Abstract:Motivation: The easily displaced structure and the depth interference caused by the mixed bone layer make the depth-of-cut control of complex bone a key task for the automatic removal of hard tissue in orthopedics. Method: First, mappings for tool current and depth of cut are pre-established individually on cancellous bone and cortical bone, thereby obtaining two calibrated mapping functions. When the cutting area is with mixed bone tissues, the depth of cut is further predicted according to these two mappings and bone tissue proportion. Next, to obtain accurate bone tissue proportion, a vibration harmonic feature extraction method was introduced to construct a robust vector, and mainstream regression models were trained and tested. The effectiveness of the proposed feature extraction method is proved by comparative experiments, and the best regression model for the bone tissue proportion recognition task is selected. Finally, to avoid excessive instantaneous cutting energy during the robotic bone removal process, the controller parameter is optimized in real-time according to the bone tissue proportion. The accuracy and robustness of depth control when cutting complex bone tissue were tested by automatically cutting porcine scapula samples. Results: Experiment results show that when desired depths change within [0.5 1] mm, the depth of cut can be maintained at the set value, the maximum average depth deviation is less than 0.05 mm, and the maximum absolute depth deviation is less than 0.15 mm. Conclusion: this article enables surgical robots to achieve automatic cutting on complex bones based on sensing signal feedback.

Mechanistic models to predict thrust force and torque in bone drilling: An in-vitro study validated with robot-assisted surgical drilling parameters

Abstract:Compression plates are widely used in orthopaedic surgeries for internal fixation of fractured femurs. To fix the plate and thus to provide compression to a fracture, the self-tapping bone screws are tightened through predrilled pilot holes of smaller diameter. Preliminary investigation showed that the holes drilled with the inappropriate cutting parameters cause mechanical and thermal damages to the local host bone, which further lead to loosening of internal fixations. In this paper, the mechanistic models to predict the thrust forces and torques during bone drilling were developed, using a 3.20 mm diameter drill bit. As a procedure, the cutting action was investigated at three different regions of the drill point, namely cutting lips, secondary cutting edges and indentation zone. The models employed the analytical approach to account for the drill-bit geometry and cutting parameters, and an empirical approach to account for the material and friction properties. To complete the procedure, calibration experiments were conducted on bovine cortical femurs with two different spindle speeds (1000 and 3000 r/min) and feeds (0.03 and 0.06 mm/rev), and then the specific normal and friction coefficients were determined. The developed mechanistic models were validated with different ranges of parameters (500–3500 r/min speeds, and 0.02–0.07 mm/rev feeds) those commonly involved in manual and robot-assisted surgery. The validation study revealed that the thrust forces predicted using the mechanistic models showed a maximum error of only 5.80%. However, the torques predicted from the mechanistic model found with more error than the thrust forces. The predominant reasons for this under-prediction might because of the extrapolation used to determine the specific cutting pressures, slip-line field applied to the indentation zone instead of compressive fracture, and chip clogging involved during the bone drilling as demonstrated in earlier studies. Despite the deviations, the developed mechanistic models satisfactorily follow the trends of the thrust forces and torques experienced during bone drilling. The outcomes can be used to practice the bone drilling procedure and monitor the effect of process parameters on thrust forces and torques in the in-silico environment before performing actual surgery.

Abstract:With the development of artificial intelligence technologies, spine-surgery robots have gradually been applied in clinical practice, and they have exhibited favorable development prospects. Force perception technology can be used to obtain the milling force, quantify the tactile sensation of a surgeon, and provide feedback or suggestions to the surgeon and robot for safe milling. In this study, a robotic system is proposed to measure the vertebral lamina milling force by using an ultrasonic bone scalpel to realize a safe milling strategy. The developed bone recognition model based on the backpropagation neural network is suitable for robot-assisted vertebral lamina milling using the milling delamination and recognition algorithm analysis. The model uses the characteristic milling force, milling speed, milling depth, and ultrasonic scalpel power as inputs to determine whether milling has reached the inner cortical bone to recognize and judge bone layers. The verification experiment on live animals showed that this model could accurately determine a safe milling endpoint. In general, this recognition model can significantly improve the safety and reliability of robot-assisted laminectomy and has significant translational prospects.

Drilling Speed and Bone Temperature of a Robot-assisted Ultrasonic Osteotome Applied to Vertebral Cancellous Bone

Abstract:Study design: An experimental investigation of a robot-assisted ultrasonic osteotome applied to vertebral cancellous bone. Objective: The aim of this study was to investigate the effect of various ultrasonic parameter settings on temperature in the drilling site and penetration time and determine the most suitable parameters for efficient and safe robot-based ultrasonically assisted bone drilling in spinal surgery. Summary of background data: A robot-assisted ultrasonic osteotome device may be safe and effective for spinal drilling. Methods: Sixty specimens of bovine vertebral cancellous were randomly assigned to one of six groups, which varied by mode of ultrasonic vibration (L-T and L) and feed rate (one percent [0.8 mm/s], two percent [1.6 mm/s], and three pecent [2.4 mm/s]). Maximum temperature in the drilling site and penetration time was recorded. Results: Maximum temperature in the drilling site decreased as output power increased for L-T and L modes, was significantly lower for L-T compared to L mode at each feed rate and power setting, was significantly different at feed rates of 1.6 mm/s versus 0.8 mm/s and 2.4 mm/s versus 0.8 mm/s for L-T mode at an output power of 60 W and 84 W, but was not influenced by feed rate for L mode. Penetration time did not significantly improve as output power increased for both L-T and L modes, was significantly decreased with increased feed rates, but was not significantly different between L-T and L modes. Conclusion: The optimal parameters for applying a robot-assisted ultrasonic osteotome to vertebral cancellous bone are L-T mode, maximum output power of 120 W, and maximum feed rate of 2.4 mm/s.Level of Evidence: 4.
Influence of cutting force on temperature, microcracks and chip morphology during rotary ultrasonic bone drilling: An in-vitro study

Abstract:Mechanical and thermal damage due to cutting force and temperature rise during bone drilling plays a significant role in implant failure during osteosynthesis. High cutting force results in hole inaccuracy and weakens the stability between bone and implant. In comparison, temperature above 47 °C during drilling of bone results in the permanent death of regenerative bony cells consequences of thermal osteonecrosis. Microcracks at the drill site result in bone delamination and affect the holding strength at the bone-screw interface, resulting in implant failure. Accumulation of bone chips produced during drilling may cause chip clogging. The cutting force, microcracks, bone chip accumulation, and temperature rise are significant reasons for implant instability and revision surgery. Therefore, the present work uses the rotary ultrasonic bone drilling (RUBD) technique of the pig femur bone to monitor cutting force, temperature, microcracks and bone debris generated during the drilling. The results were also compared with conventional bone drilling (CBD). The twisted surgical drill bit was used during CBD and the diamond-impregnated tool was used during RUBD with the same diameters of 3.5 mm. The histopathological analysis was conducted at the drilled site to observe the tissue damage and the effect of heat generation on bone morphology. The output parameters were correlated, and the influence of cutting force on temperature rise, microcracks, and chip morphology was observed. This study showed that the increase in cutting force from 35.63 to 57.66 N in RUBD, and 54.83 to 78.7 N in CBD, resulted in a decrease in temperature rise by 51.5 and 32.7%, respectively. In contrast, the intensity of microcracks and chips size increased with the increase in cutting force for both RUBD and CBD techniques. Moreover, the RUBD technique reduces cutting force, temperature rise, microcracks and bone chip morphology as compared to CBD.

Study DesignA biomechanical study.ObjectiveWe aimed to assess temperature fluctuations when applying the ultrasonic osteotome during laminectomy and identify the most appropriate cutting method for robot-assisted laminectomy to mitigate the risk of heat-related injuries.MethodsUtilizing a robotic system for laminectomy, the study implemented the ultrasonic bone scalpel to cut both artificial polyurethane bones and animal spinal bones. The research focused on comparing and analyzing the maximum temperature of the inner surface of four types of artificial bones through three different cutting techniques: vertical constant cutting at speeds of .5 mm/s and 1 mm/s, as well as robot-assisted vertical reciprocating cutting. After the initial results, two optimal vertical cutting approaches were selected for subsequent trials, evaluating the effectiveness and temperature impact of various ultrasonic osteotome modes on 10 isolated spinal bones from pigs.ResultsWhen cutting polyurethane bones, reciprocating cutting demonstrated the lowest maximum temperature in contrast to constant speed cutting at .5 mm/s and 1 mm/s. In the animal bone trial, direct cutting registered an average maximum temperature of 43.25°C with an average cutting duration of 688.3 s, while reciprocating cutting recorded an average maximum temperature of 34.20°C with an average cutting time of 713.0 s.ConclusionsThe reciprocating cutting strategy utilized in robot-assisted ultrasonic osteotome is effective in reducing heat generation and maintaining high cutting efficiency.

Abstract:Robotic drilling through direct cochlear access for cochlear implantation surgery is currently a prominent area. However, the safety of drilling remains an issue. In this study, a 3D finite element model of temporal bone drilling is established for the first time to predict the drilling force. The self‐developed cochlear implant surgical robot validates the findings through experimental drilling. Robotic drilling through direct cochlear access for cochlear implantation (CI) is currently a prominent area in the field of CI surgery. However, the safety of drilling remains an issue due to the limited space within the facial recess. Estimation and control of drilling force are critical to prevent drill‐bit breakthrough, excessive heat generation, and mechanical damage to the facial nerve. The bone drilling process presents a favorable application scenario for finite element (FE) analysis. In this study, a 3D FE model of temporal bone drilling is established for the first time to predict the drilling force. The self‐developed CI surgical robot validates these findings through experimental drilling. The experimental results show that the drilling process can be divided into three stages, and the corresponding force curve can be divided into six zones. The overall trend of temporal bone drilling is two force peaks and one trough in the middle. The drilling force is positively correlated with the feed rate. The FE results found to be in good agreement with the experimental results. Numerical simulation can be utilized as an invaluable tool for assisting in predicting the position of drilling tools and optimizing drilling parameters in the future.

Accuracy of a New Robotically Assisted Technique for Total Knee Arthroplasty: A Cadaveric Study

Abstract:Background: Although the utility of robotic surgery has already been proven in cadaveric studies, it is our hypothesis that this newly designed robotically assisted system will achieve a high level of accuracy for bone resection. Therefore, we aimed to analyze in a cadaveric study the accuracy to achieve targeted angles and resection thickness. Methods: For this study, 15 frozen cadaveric specimens (30 knees) were used. In this study, Zimmer Biomet (Warsaw, IN) knees, navigation system, and robot (ROSA Knee System; Zimmer Biomet) were used. Eight trained, board-certified orthopedic surgeons performed robotically assisted total knee arthroplasty implantation using the same robotic protocol with 3 different implant designs. The target angles obtained from the intraoperative planning were then compared to the angles of the bone cuts performed using the robotic system and measured with the computer-assisted system considered to be the gold standard. For each bone cut the resection thickness was measured 3 times by 2 different observers and compared to the values for the planned resections. Results: All angle mean differences were below 1° and standard deviations below 1°. For all 6 angles, the mean differences between the target angle and the measured values were not significantly different from 0 except for the femoral flexion angle which had a mean difference of 0.95°. The mean hip-knee-ankle axis difference was -0.03° ± 0.87°. All resection mean differences were below 0.7 mm and standard deviations below 1.1mm. Conclusion: Despite the fact that this study was funded by Zimmer Biomet and only used Zimmer Biomet implants, robot, and navigation tools, the results of our in vitro study demonstrated that surgeons using this new surgical robot in total knee arthroplasty can perform highly accurate bone cuts to achieve the planned angles and resection thickness as measured using conventional navigation.

Abstract:Micro-grinding with a spherical grinding head has been deemed an indispensable method in high-risk surgeries, such as neurosurgery and spine surgery, where bone grinding has long been plagued by the technical bottleneck of mechanical stress-induced crack damage. In response to this challenge, the ultrasound-assisted biological bone micro-grinding novel process with a spherical grinding head has been proposed by researchers. Force modeling is a prerequisite for process parameter determination in orthopedic surgery, and the difficulty in establishing and accurately predicting bone micro-grinding force prediction models is due to the geometric distribution of abrasive grains and the dynamic changes in geometry and kinematics during the cutting process. In addressing these critical needs and technical problems, the shape and protrusion heights of the wear particle of the spherical grinding head were first studied, and the gradual rule of the contact arc length under the action of high-speed rotating ultrasonic vibration was proposed. Second, the mathematical model of the maximum thickness of undeformed chips under ultrasonic vibration of the spherical grinding head was established. Results showed that ultrasonic vibration can reduce the maximum thickness of undeformed chips and increase the range of ductile and bone meal removals, revealing the mechanism of reducing grinding force. Further, the dynamic grinding behavior of different layers of abrasive particles under different instantaneous interaction states was studied. Finally, a prediction model of micro-grinding force was established in accordance with the relationship between grinding force and cutting depth, revealing the mechanism of micro-grinding force transfer under ultrasonic vibration. The theoretical model's average deviations are 10.37% in x -axis direction, 6.85% in y -axis direction, and 7.81% in z -axis direction compared with the experimental results. This study provides theoretical guidance and technical support for clinical bone micro-grinding.

Abstract:The machining of hard tissues, such as cortical bone and cancellous bone in human bodies and enamel and dentin in teeth, is an essential process in orthopaedics, neurosurgery and dentistry. Owing to their natural high hardness and strength, as well as anisotropic composite structure and properties, the machining process is difficult and prone to generating large cutting forces and high temperatures, potentially leading to necrosis (i.e., cellular death) and microcracks and, therefore, tissue damage. Necrosis and microcracks affect the stability of implant fixation and post-surgery recovery. In this paper, studies in tissue structure and properties, cutting mechanisms, and thermal and mechanical damages formed in the machined surface and subsurface have been reviewed to facilitate the understanding of machining processes and mechanisms for hard tissue. Traditional (orthogonal cutting, drilling, milling and grinding) and non-traditional (vibration-assisted) cutting processes have been considered to explore the underlying mechanisms that control the material removal process and understand the damages that may be formed in hard tissues. The solution strategies developed for minimising the machining-induced damages to the tissues have also been evaluated to provide the knowledge needed for improving surgical tool design. Finally, the research gaps have been identified in this study and future research prospects in hard tissue machining have been proposed.

Abstract:Cortical bone is semi-brittle and anisotropic, that brings a challenge to suppress vibration and avoid undesired fracture in precise cutting process in surgeries. In this paper, a novel analytical model is proposed to represent cortical bone cutting processes. The model is utilized to predict the chip formations, material removal behavior and cracks propagation under varying bone osteon cutting angles and depths. Series of orthogonal cutting experiments were conducted on cortical bone to investigate the impact of bone osteon cutting angle and depth of cut on cutting force, crack initialization and propagation. The observed chip morphology highly agreed with the prediction of chip formation based on the analytical model. The curly, serrated, grainy and powdery chips formed when the cutting angle was set as 0°, 60°, 90°, and 120°, respectively. Cortical bone were removed dominantly by shearing at a small depth of cut from 10 to 50 μm, and by a mixture of pealing, shearing, fracture and crushing at a large depth of cut over 100 μm at different bone osteon angles. Moreover, its fracture toughness was calculated based on measured cutting force. It is found that the fluctuation of cutting force is suppressed and the bone material becomes easy to remove, which attributes to lower fracture toughness at bone osteon cutting angle 0°. When the cutting direction develops a certain angle to bone osteon, the fracture toughness increases then the crack propagation is inhibited to some extent and the fluctuation of cutting force comparatively decreases. There is a theoretical and practical significance for tools design and operational parameters choice in surgeries.
Investigation of an Indirect Measurement Method of Ultrasonic Bone Cutting Forces Based on Strain Sensors

Abstract:The measurement model of ultrasonic bone cutting force is an urgent problem to be solved in the application of ultrasonic osteotomy in robot assisted orthopedic surgery, which is of great significance for achieving bone layer perception and cutting force control during the surgical process. This article proposes an indirect measurement method of ultrasonic bone cutting force based on strain. Firstly, according to the axial vibration strain distribution curve of the ultrasonic osteotomy, the optimal measurement position for the strain gauge is selected as the high strain position (with a high signal-to-noise ratio), and the temperature compensation strategy of the dual strain gauge method is used to reduce or eliminate the thermal effects during the strain measurement. Then, the Pearson correlation coefficient between strain and cutting force in different cutting directions is calculated, and there is a strong positive correlation between strain and cutting force. Finally, the least squares method was used to obtain indirect measurement models of ultrasonic bone cutting force based on strain in different cutting directions. The experimental results showed that the waveform of cutting force measured indirectly based on strain was in good agreement with the waveform of cutting force measured directly by the force sensor. The error between the effective value of indirect measurement of cutting force and the effective value of direct measurement of cutting force did not exceed 13%, which verified the feasibility and effectiveness of this method. The indirect measurement models of ultrasonic bone cutting force based on strain will become a cost-effective choice with good accuracy and high sensitivity, providing a theoretical basis for orthopedic robots to perceive and control cutting force, and promoting the clinical application of ultrasonic osteotomy in orthopedic robots.
