Abstract:Our aim in this study was to investigate the changes of serum high-sensitive C-reactive protein (hs-CRP) and uric acid (UA), and evaluate the synergistic effect of amlodipine and atorvastatin on blood pressure and left ventricular remodeling in hypertensive patients with primary hypercholesterolemia. One hundred and twenty-six hypertensive patients with hypercholesterolemia were randomized into amlodipine group (10 mg/day, group A, n = 65) and amlodipine (10 mg/day) plus atorvastatin group (20 mg/day, group B, n = 61), treated for 4 months continuously. Serum concentrations of total cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, triglycerides, hs-CRP, and UA were determined, and blood pressure of both groups was examined before and after treatment. Left ventricular posterior wall thickness and interventricular spectum thickness were measured by echocardiography, and left ventricular mass index (LVMI) was calculated. After 4-months of treatment with atorvastatin, serum concentrations of total cholesterol, low-density lipoprotein cholesterol, triglycerides, hs-CRP, and UA were significantly decreased in group B ( P < 0.05, P < 0.01), while serum concentrations of high-density lipoprotein cholesterol was elevated ( P < 0.05). Meanwhile, systolic blood pressure and diastolic blood pressure were reduced in both groups ( P < 0.05), and blood pressure in group B was markedly lower than that in group A after treatment ( P < 0.05). Compared with that before treatment, LVMI in both groups decreased ( P < 0.05), to a significantly lower degree in group B than in group A ( P < 0.05). Atorvastatin can decrease serum concentrations of hs-CRP and UA. The amlodipine-atorvastatin combination markedly reduces blood pressure and reverses left ventricular hypertrophy more than amlodipine monotherapy. The positive effect suggests that in hypertensive and hypercholesterolemic patients, the combination of amlodipine and atorvastatin could be the treatment of choice.

Abstract:OBJECTIVE:To compare the efficacy of atenolol and diltiazem-SR and the effects on the quality of life in hypertensive patients. METHODS:Seventy-three patients with mild to moderate hypertension (DBP 90 - 109 mmHg) were allocated randomly to be administered with atenolol 25 mg/d (group A, n=37) and diltiazem-SR 90 mg/d (group B, n=36) for eight weeks. The changes of heart rate, office blood pressure(OBP), ambulatory blood pressure(ABP) and the quality of life were compared before and after treatment. RESULTS:Heart rate, OBP and ABP decreased after treatment in both groups. The effective rate of blood pressure was 88.2% in group A and 93.8% in group B. Twenty four hour mean daytime and nighttime BP,daytime and nighttime BP loads declined in both groups (P<0.05 - 0.01). The quality of life was significantly increased in group B (P<0.05). Side effects were 21.6% in group A and 11.1% in group B, respectively (P>0.05). CONCLUSION:Atenolol and diltiazem-SR are more effective and tolerant in the treatment of the hypertension. Diltiazem improves the quality of life better than atenolol.

Abstract:Objectives: International guidelines recommended combination of different classes of antihypertensive agents to achieve the targeted blood pressure (BP) in initial treatment for hypertension. However, the optimal combinations are not clarified. The present study was to evaluate the treatment efficacy with the combination of a dihydropyridine calcium-channel blocker amlodipine with diuretics or angiotensin II receptor blocker on reducing the rate of cardiovascular events. Methods: In a randomized, open-label, blinded-endpoint trial, hypertensive patients at high risk for cardiovascular events from 180 clinical centers in China were assigned to receive treatment with either amlodipine plus amiloride/hydrochlorothiazide (n = 6776) or amlodipine plus telmisartan (n = 6766). The primary outcome was the composite of non-fatal stroke, non-fatal myocardial infarction or death from cardiovascular causes. Results: The median follow-up was 41 months. No between-group difference was observed in mean BP lowering. However, the primary-outcome events were reduced by 24.1% in the amlodipine-diuretics group compared with the amlodipine-telmisartan group (hazard ratio [HR], 0.76; p = 0.005). Of note, compared with amlodipine-telmisartan regiments, amlodipine-diuretics treatment significantly lowered the risk of death from cardiovascular causes (HR, 0.64; p = 0.03) and fatal and nonfatal stroke (HR, 0.78; p = 0.03). Both treatments were safe and well tolerated. Conclusion: The amlodipine-diuretics combination was more efficacious in reducing cardiovascular events than the amlodipine-telmisartan combination in hypertensive patients with high risk for such events, independent of blood pressure reduction. Abstract:Inadequate 24-hour blood pressure control could cause complications, especially in high-risk hypertensive individuals. Hence, it is recommended that the targeted blood pressure levels be maintained round the clock. This prospective, observational study assessed the 24-hour ambulatory blood pressure (ABP) control with 8-week amlodipine therapy 5/10 mg/day in patients with hypertension. Adult patients (62 m, 42 f) with mild-to-moderate essential hypertension received oral amlodipine 5 mg/day (n = 68) or 10 mg/day (n = 36) for 8 weeks. The study was approved by the independent ethics committee and informed consent was obtained from all participants. Primary outcome was change at 8 weeks from baseline in mean 24-hr ABP (systolic and diastolic), and secondary outcomes were change from baseline in mean morning, daytime, night-time, and last 6-hr dosing interval ABP. A repeat measures analysis of covariance (ANCOVA) was done for changes in ABP over 8 weeks, with age, gender, BMI and associated comorbidities as covariates. Nocturnal dippers were assessed as those greater than or equal to 10% reduction in ABP at 8 weeks. Sub-groups were created based on age, duration of hypertension, occupation, baseline BMI and amlodipine dose. Safety was evaluated based on clinical adverse events and 12-lead electrocardiogram. Of the 104 patients from 2 study centers, 49 completed 8-weeks therapy and were included for per-protocol analysis. Significant reductions in the mean 24-hr blood pressure were seen for both SBP and DBP (figure 1). The mean 24-hr. reduction was -10.82 and -6.25 mmHg with 5 mg amlodipine and -19.81 and -11.19 mmHg with 10 mg amlodipine for SBP and DBP respectively (p < 0.0001). The mean morning, daytime, night-time, and last 6-hr reductions in SBP were -14.06, -18.63, -20.42, and -13.51 respectively (p < 0.0001), and DBP reductions were -7.78, -10.27, -11.07, and -6.44 respectively (p < 0.0001). Significant reduction in ABP was seen in all sub-groups (p < 0.0001). Greater reduction in SBP and DBP were observed in patients with stage-2 hypertension. At baseline, there were 30 (61.2%) non-dippers which decreased to 18 (36.7%) at 8 weeks. At the end of the study, 40% of non-dippers were converted to dippers by Amlodipine, whereas none of the dippers were converted to non-dippers. No adverse effects were reported. Amlodipine significantly reduces the 24-hr. mean morning, daytime, night-time, and last 6-hr. ABP and converted non dippers to dippers in adult patients with mild to moderate essential hypertension. The Study highlights the consistent effects of amlodipine on both quantity and quality of 24 hours BP control which is important for associated CV risk reduction.

Abstract:Background: Current hypertension guidelines recommend combination of an angiotensin-converting enzyme inhibitor or angiotensin-receptor blocker with a calcium-channel blocker or thiazide diuretic as initial antihypertensive therapy in patients with monotherapy uncontrolled hypertension. However, to what extent these two different combinations are comparable in blood pressure (BP)-lowering efficacy and safety remains under investigation, especially in the Chinese population. We investigated the BP-lowering efficacy and safety of the amlodipine/benazepril and benazepril/hydrochlorothiazide dual therapies in Chinese patients. Methods: In a multi-center, randomized, actively controlled, parallel-group trial, we enrolled patients with stage 1 or 2 hypertension from July 2018 to June 2021 in 20 hospitals and community health centers across China. Of the 894 screened patients, 560 eligible patients were randomly assigned to amlodipine/benazepril 5/10 mg (n = 282) or benazepril/hydrochlorothiazide 10/12.5 mg (n = 278), with 213 and 212 patients, respectively, who completed the study and had a valid repeat ambulatory BP recording during follow-up and were included in the efficacy analysis. The primary outcome was the change from baseline to 24 weeks of treatment in 24-h ambulatory systolic BP. Adverse events including symptoms and clinically significant changes in physical examinations and laboratory findings were recorded for safety analysis. Results: In the efficacy analysis (n = 425), the primary outcome, 24-h ambulatory systolic BP reduction, was - 13.8 ± 1.2 mmHg in the amlodipine/benazepril group and - 12.3 ± 1.2 mmHg in the benazepril/hydrochlorothiazide group, with a between-group difference of - 1.51 (p = 0.36) mmHg. The between-group differences for major secondary outcomes were - 1.47 (p = 0.18) in 24-h diastolic BP, - 2.86 (p = 0.13) and - 2.74 (p = 0.03) in daytime systolic and diastolic BP, and - 0.45 (p = 0.82) and - 0.93 (p = 0.44) in nighttime systolic and diastolic BP. In the safety analysis (n = 560), the incidence rate of dry cough was significantly lower in the amlodipine/benazepril group than in the benazepril/hydrochlorothiazide group (5.3% vs 10.1%, p = 0.04). Conclusions: The amlodipine/benazepril and benazepril/hydrochlorothiazide dual therapies were comparable in ambulatory systolic BP lowering. The former combination, compared with the latter, had a greater BP-lowering effect in the daytime and a lower incidence rate of dry cough. Trial registration: ClinicalTrials.gov, NCT03682692. The Effectiveness and Safety of L-amlodipine Besylate for Blood Pressure Control in Patients with Mild to Moderate Essential Hypertension

Abstract:OBJECTIVE:To evaluate the effectiveness and safety of L-amlodipine besylate for blood pressure control in patients with mild to moderate essential hypertension.METHOD:A total of 1051 mild to moderate essential hypertensives were recruited from 22 centers and randomized into three groups and were given low-dose L-amlodipine besylate (2.5 mg/d), high-dose L-amlodipine besylate (5.0 mg/d), and amlodipine maleate (5.0 mg/d) on the principle of open-label and parallel control. The study drugs were administered for 8 weeks.RESULTS:After 8 weeks treatments, the response rate of the three groups were 72.4%, 85.6%, and 76.2%, respectively. The rate difference between high-dose L-amlodipine besylate group and the other two groups were statistically significant (P < 0.01), while the rate difference between low-dose L-amlodipine besylate group and amlodipine maleate group was similar (P = 0.28). The event rates of the three groups were similar (4.3%, 4.6%, and 5.1%, respectively, P = 0.84).CONCLUSION:High-dose L-amlodipine besylate is superior to the other 2 groups on blood pressure control. The efficacy profiles of amlodipine maleate and low-dose L-amlodipine besylate are equivalent. Safety profiles of these three groups are comparable.

Abstract:Objectives: To investigate the efficacy and safety of the single-pill combination of telmisartan 80 mg plus amlodipine 5 mg (T80/A5 SPC) versus amlodpine 5 mg (A5) in hypertensive Asian patients who were uncontrolled on amlodipine 5 mg (A5) monotherapy. Design and Methods: After a 6-weekopen-label run-in period with amlodipine 5 mg monotherapy, patients, who failed to respond adequately to A5 (defined as seated DBP ≥90 mmHg), underwent double-blind randomization with 160 and 164 patients assigned to receive either T80/A5 SPC or A5 monotherapy daily. The primary endpoint was change from baseline in mean seated trough DBP after 8 weeks of randomized treatment. Treatment groups were compared using an Analysis of Covariance (ANCOVA) model including treatment, country and the baseline measurement as a covariate. Results: In the full population (n = 314), seated trough mean ± SE BP reductions (mmHg) with T80/A5 vs A5 from baseline to Week 8 were –16.2 ± 1.3 vs –11.7 ± 1.3 for SBP (p < 0.001) and −12.4 ± 0.95 vs –10.2 ± 0.93 for DBP (p = 0.007). T80/A5 provided DBP goal attainment (<140/90 mmHg) in 64.5% vs 45.3% with A5 alone (p = 0.007) and DBP response rate (<90 mmHg or ≥10 mmHg reduction) was 80.0% vs 63.5% (p = 0.0017). The incidence of related AEs (including peripheral edema) was 1.9% with T80/A5 and 2.4% with A5. Conclusions: In Asian patients T80/A5 SPC provided superior BP reductions and goal rate achivement vs A5 monotherapy after 8 weeks of treatment, T80/A5 was well-tolerated and had a safety profile comparable to A5 monotherapy.

Abstract:Objective: The present observational study evaluated long-term management of hypertension in patients who received treatment with valsartan and amlodipine in a single-pill combination (Val/Aml SPC) in a real-world setting in China (Chinese Clinical Trial Registry number ChiCTR1900021324). Methods: This was a prospective, observational, multicenter, real-world registry study wherein patients with hypertension who had already received Val/Aml SPC (80/5 mg) for at least 4 weeks before study enrollment were observed for 1 year. Investigators recorded patient data every 3 months and essentially five times during the 1 year follow-up period. Effectiveness was assessed by the blood pressure (BP) control rate and average duration of treatment at the end of the study. Safety was monitored by the incidence of adverse events (AEs) and serious adverse events (SAEs). Results: Overall, 985 patients were enrolled (mean +/- standard deviation [SD] age: 60.3 +/- 11.5 years); of these, 894 were included in the full analysis set, 758 of whom completed the study. At baseline, BP was controlled (<140/90 mmHg) in 64.3% of patients on Val/Aml SPC for at least 4 weeks before enrollment. Office BP control rates significantly improved from baseline in 74.1% of patients at 1 year (p < .0001). Overall, 575 (87.0%) patients remained on Val/Aml SPC at 1 year (average exposure: 311.5 days). AEs were reported in 23.3% of patients. The majority of AEs were mild to moderate, and 0.6% of patients discontinued Val/Aml SPC because of SAEs. Conclusion: This study provides evidence that Val/Aml SPC effectively reduced BP over the long term among Chinese hypertensive patients, with a good adherence and tolerability profile, and that most hypertensive patients may benefit from this combination.
Profile on Circadian Blood Pressure and the Influencing Factors in Essential Hypertensive Patients after Treatment

Abstract:OBJECTIVE:To explore the circadian blood pressure (BP) profile and its influencing factors in essential hypertensive patients after treatment. METHODS:Cross-sectional surveillance was carried out in essential hypertensive subjects after treatment whose clinic blood pressure had been under control as 140/90 mm Hg (1 mm Hg = 0.133 kPa) for at least one month. All patients underwent a twenty-four-hour ambulatory blood pressure monitoring device (spacelabs 90207). The nocturnal fall of blood pressure (BP) was calculated from (daytime mean BP-night-time mean BP)/daytime BP, while 'daytime' values were recorded between 6 h and 22 h and 'night-time' values between 22 h and 6 h. Non-dippers were defined as those whose nocturnal decrease in mean systolic BP and/or mean diastolic BP was < 10% of the daytime BP. Binary logistic regression analysis was used to evaluate the correlation between circadian blood pressure profile and factors as gender, age, height, body mass index (BMI), family history of premature cardiovascular disease, women under age 65 or men under age 55, smoking habits, grade of hypertension, and strategy of antihypertensive drugs. RESULTS:208 treated essential hypertensive patients were enrolled in the study. 79 individuals were dippers and 129 were non-dippers. Data from logistic regression analysis showed that four factors as age, premature family history of cardiovascular disease, overweight or obesity, and strategy of antihypertensive drugs were significantly influencing the circadian blood pressure profile in treated hypertensive patients. The incidence of non-dippers in patients of 70 years of age or older and those between 60 and 69 were 3.3 and 2.3 times of those with less than 60 (P = 0.009 and 0.031, respectively). The prevalence of non-dippers in patients with a premature family history of cardiovascular disease was 3.7 times greater than those in subjects without a premature history of cardiovascular disease (P = 0.029). Similarly, the incidence of non-dippers in patients of overweight (24 = BMI < 28) and obesity (BMI >/= 28) were 3.0 and 4.8 times of those in subjects of normal weight (P = 0.003 and 0.009, respectively). Compared with patients treated with long-acting calcium channel blockers (CCBs), patients treated with angiotensin-converting enzyme (ACE) inhibitors or angiotensin II receptor blockers (ARBs) alone had less prevalence of nondippers (OR = 0.139, P = 0.010). Patients treated with joint antihypertensive scheme including ACE inhibitors or ARBs(but not including diuretics) had the tendency of lower incidence of abnormal circadian blood pressure rhythm (OR = 0.453, P = 0.118). Patients treated with joint antihypertensive scheme including diuretics (not including ACE inhibitors or ARBs) and with joint antihypertensive strategy including diuretics and ACE inhibitors or ARBs had lower incidence of nondippers (OR = 0.378 and 0.273, respectively; P = 0.030 and 0.011, respectively). CONCLUSIONS:Approximately 2/3 treated essential hypertensive patients had a non-dipper blood pressure profile. Age, premature family history of cardiovascular disease, overweight/obesity, and antihypertensive drugs strategy were correlated with circadian blood pressure profile. Compared with long-acting CCBs, diuretics, ACE inhibitors or ARBs might be helpful in keeping the circadian blood pressure rhythm at normal range.

Adherence to treatment was high: 91% of patients at month 6 reported that they never missed one dose, and 93.4% of patients at month 6 and 95.3% of patients at study end maintained Val/Aml FDC treatment. A majority of AEs was mild to moderate and 0.6% of patients discontinued Val/Aml FDC because of SAEs. Conclusion: Val/Aml FDC provided long-term sustained BP control with good tolerability and good long-term compliance in this 1-year, real-world registry study in patients with hypertension from China.

Methods In this multinational study, 654 adults (82% Chinese) with stage 1 or 2 hypertension not adequately controlled with 4 weeks of valsartan monotherapy were randomized to receive double-blind valsartan 160 mg or amlodipine/valsartan 5/160 mg SPC once daily for 8 weeks. The primary efficacy variable was the change from baseline to endpoint in trough mean sitting diastolic blood pressure (MSDBP). Results Overall, the mean age of participants was 53.0 years and mean baseline blood pressure (BP) was 146.6/96.4 mmHg. Amlodipine/valsartan SPC reduced MSDBP to a greater extent than valsartan monotherapy: least-squared mean (LSM) changes (standard error [SE]) from baseline to endpoint were −10.3 (0.39) mmHg vs−6.6 (0.40) mmHg (p < 0.0001). The LSM changes (SE) from baseline to endpoint in mean sitting systolic blood pressure (MSSBP) were also greater with combination therapy than valsartan monotherapy: −14.9 (0.61) mmHg vs−7.0 (0.61) mmHg (p < 0.0001). A greater proportion of patients achieved MSSBP/MSDBP<140/90 mmHg with amlodipine/valsartan SPC vs valsartan monotherapy (61.3% vs 39.3%; p < 0.0001). Both treatments were well tolerated, with few adverse events reported (8.8% combination therapy, 11.7% monotherapy). Conclusion Amlodipine/valsartan 5/160 mg SPC was safe and effective therapy for lowering BP in predominantly Chinese adults with stage 1 or 2 hypertension not adequately controlled with valsartan 160 mg monotherapy.

Abstract:Objective To study the effect and safety of fixed compound amlodipine and atorvastatin in treatment of patients with hypertension accompanying hyperhpidemia.Methods Two hundred and seventeen patients with mild-moderate primary hypertension accompanying hyperhpidemia were randomly divided into experimental group(n = 108) and control group(n= 109).Patients in experimental group were given per os fixed compound amlodipine and atorvastatin at its initial dose and those in control group were given per os 5 mg amlodipine and 10 mg atorvastatin,once a day for 4 weeks.The patients were treated with the adjusted dose of the drugs according'to their diastolic pressure for another 8 weeks to observe their clinical effect and adverse events.Results The SBP and DBP,and the LDL-CTG and TC levels were significantly lower in two groups after treatment than before treatment(P0.01).No significant difference was found in adverse events between the two groups(P 0.05) and no severe adverse events occurred.Conclusion Fixed compound amlodipine and atorvastatin can effectively decrease hypertension and down-regulate lipid level in patients with mild-moderate primary hypertension accompanying hyperhpidemia with a fair safety and tolerance and can thus be used in treatment of such patients.

Abstract:Objective: To compare the antihypertensive effect and safety of amlodipine and felodipine extended release (ER). Methods: Seventy-six patients with mild to moderate hypertension were randomized to two groups in a double-blind manner. One group was treated with amlodipine 5.0–10.0 mg/day (group 1), and the other group with felodipine ER 5.0–10.0 mg/day (group 2), for 12 weeks. Twenty-four-hour ambulatory blood pressure (ABP) monitoring at baseline and after treatment was used to evaluate drug efficacy and blood pressure (BP) changes during a ‘drug holiday’ (48 h). Results: Diastolic BP was reduced in the amlodipine group by 15.1 mm Hg (to a final BP of 84.8 mm Hg) and in the felodipine ER group by 15.3 mm Hg (to a final BP of 85.0 mm Hg). Efficacy was considered to be achieved in 83.8% of patients in group 1 and in 87.9% of patients in group 2 (not significant). The number of patients controlled at the starting dose was 28 in group 1 and 19 in group 2 (p < 0.01). In group 1, BP remained below 140/90 mm Hg 24–48 h after the missed dose, and 24-hour ABP was not significantly different from that before the missed doses. In group 2, BP was above 140/90 mm Hg after the missed dose, and both BP and 24-hour ABP were statistically significantly higher than those before the missed doses. Adverse reactions were infrequent in both groups. Conclusions: Both amlodipine and felodipine ER are effective, safe, reliable and well tolerated in the treatment of mild to moderate essential hypertension. BP was well controlled in group 1 24–48 h after the missed dose. The fluctuation of BP in group 2, however, was significantly worse than that in group 1.

Abstract:Objective: We aimed to characterize seasonal variation in US population-based blood pressure (BP) control and BP-related metrics and evaluate the association between outdoor temperature and BP control variation. Methods: We queried electronic health records (EHRs) from 26 health systems, representing 21 states, to summarize BP metrics by quarters of 12-month periods from January 2017 to March 2020. Patients with at least one ambulatory visit during the measurement period and a hypertension diagnosis during the first 6 months or prior to the measurement period were included. Changes in BP control, BP improvement, medication intensification, average SBP reduction after medication intensification across quarters and association with outdoor temperature were analyzed using weighted generalized linear models with repeated measures. Results: Among 1 818 041 people with hypertension, the majority were more than 65 years of age (52.2%), female (52.1%), white non-Hispanic (69.8%) and had stage 1/2 hypertension (64.8%). Overall, BP control and process metrics were highest in quarters 2 and 3, and lowest in quarters 1 and 4. Quarter 2 had the highest percentage of improved BP (31.95 ± 0.90%) and average SBP reduction after medication intensification (16 ± 0.23 mmHg). Quarter 3 had the highest percentage of BP controlled (62.25 ± 2.55%) and lowest with medication intensification (9.73 ± 0.60%). Results were largely consistent in adjusted models. Average temperature was associated with BP control metrics in unadjusted models, but associations were attenuated following adjustment. Conclusion: In this large, national, EHR-based study, BP control and BP-related process metrics improved during spring/summer months, but outdoor temperature was not associated with performance following adjustment for potential confounders.

Abstract:Background: A majority of hypertensives require treatment with >= 2 antihypertensive therapies to achieve blood pressure (BP) goals. Single-pill combinations (SPC) may improve convenience and adherence to therapy and reduce health care resource use and costs. The antihypertensive effects of amlodipine and valsartan are well established. This study evaluated the efficacy and safety of amlodipine/valsartan 5/160 mg SPC for the treatment of hypertension in predominantly Chinese patients not adequately controlled on valsartan 160 mg alone.Methods: In this multicentre study (24 centres), adults with stage 1 or 2 hypertension not adequately controlled with valsartan monotherapy were randomised to receive double-blind amlodipine/valsartan 5/160 mg SPC or valsartan 160 mg once daily for 8 weeks.Results: The least-squaremean change (standard error) from baseline to endpoint in mean sitting diastolic blood pressure (MSDBP) at trough, the primary efficacy variable, was -10.3 (0.39) mm Hg with amlodipine/valsartan and -6.6 (0.40) mmHgwith valsartan (difference: -3.7 [0.54] mmHg, p<0.0001). The corresponding results for mean sitting systolic blood pressure (MSSBP) were -14.9 (0.61) mm Hg and -7.0 (0.61) mm Hg, respectively (difference: -7.9 [0.84] mm Hg, p<0.0001). A significantly greater proportion of patients achieved overall BP control (MSSBP/MSDBP<140/90 mm Hg) with combination therapy (61.3%) versus monotherapy (39.3%; p<0.0001). The acute effects of outdoor temperature on blood pressure in a panel of elderly hypertensive patients

Abstract:Higher level of blood pressure (BP) in winter than in summer has been observed, but the association between temperature and BP and its potential modifiers with adjustment of individual confounders and time trends was rarely explored. We aimed to investigate the association between outdoor temperature and BP and its potential modification factors in a longitudinal panel study in Shanghai, China. From January 2011 to December 2012, we scheduled 54 follow-ups for BP measurements per subject via home visit every other week for 50 elderly hypertensive patients. We applied linear mixed-effect models to analyze the association between temperature and BP after controlling for individual characteristics, antihypertensive medication, comorbidities, and time trends. We evaluated the potential effect modifiers by stratification analyses. For a 1 °C decrease in the average temperature on concurrent day and previous day, systolic BP increased by 0.19 mmHg (95 % confidence interval = 0.06, 0.31) and diastolic BP increased by 0.12 mmHg (95 % confidence interval = 0.03, 0.21). The effect of temperature on BP was stronger among those with older age, female sex, low socioeconomic status, and obese physique. The effect was weak and even null for those taking the angiotensin receptor blockers, angiotensin-converting enzyme inhibitor, or its combination with calcium antagonists. Further, the effect was almost restricted within those having chronic comorbidities. Our results demonstrated that an acute decrease in outdoor temperature was significantly associated with a rise in BP among elderly hypertensive patients, in Shanghai, China. Individual characteristics, antihypertensive medications, and comorbidities may modify this effect.
Effect of combining valsartan and amlodipine in the treatment of type 2 diabetes mellitus complicated by hypertension

Abstract:Purpose: To investigate the efficacy of combining valsartan and amlodipine on patients who were co-morbid with type 2 diabetes mellitus (T2DM) and hypertension. Methods: The medical records of 110 patients treated in North China Electric Power University Hospital, Beijing, China from July 2018 to October 2022 were retrospectively analyzed. Control group (CG; n = 50) was administered oral amlodipine (5 mg daily for 6 months), while the study group (SG; n = 60) received one tablet of co-formulated valsartan (80 mg)) and amlodipine (5 mg) daily for 6 months. All the patients, in addition to the above drugs, were also placed on insulin and atorvastatin calcium. After a 6-month treatment period, the therapeutic effect and incidence of adverse effects were compared between CG and SG. Blood glucose-related indices (glycosylated hemoglobin (HbA1c), 2 h postprandial blood glucose (2 h PG), and fasting blood glucose (FBG)) and blood pressure indices, viz, (diastolic blood pressure (DBP), systolic blood pressure (SBP)) were compared. Results: Compared to the pre-treatment values, both groups showed significant decrease in 2 h PG, FBG, HbA1c, SBP and DBP after treatment (p < 0.05), but the 2 h PG, FBG, HbA1c, SBP and DBP in SG were significantly lower in comparison to CG (p < 0.05). The overall response rate (ORR) of CG was significantly lower in comparison to SG (p < 0.05). In SG, there was significant reduction in the incidence of adverse effects compared to CG (p < 0.05). Conclusion: The combination of valsartan and amlodipine is effective in treating T2DM co-morbid with hypertension, and satisfactorily controls blood glucose level and blood pressure with a relatively high level of safety. Further experiments would be required to ascertain the reliability of these findings.

Abstract:Objective To evaluate the differences in 24-hour ambulatory blood pressure (BP) in older patients with hypertension treated with the five major classes of antihypertensive drugs, as monotherapy or dual combination therapy, to improve daytime and nighttime BP control. Methods We enrolled 1920 Chinese community-dwelling outpatients aged >= 60 years and compared ambulatory BP values and ambulatory BP control (24-hour BP < 130/80 mmHg; daytime mean BP < 135/85 mmHg; and nighttime mean BP < 120/70 mmHg), as well as nighttime BP dip patterns for monotherapy and dual combination therapy groups. Results Patients' mean age was 71 years, and 59.5% of patients were women. Calcium channel blockers (CCBs) constituted the most common (60.3% of patients) monotherapy, and renin-angiotensin system (RAS) blockers combined with CCBs was the most common (56.5% of patients) dual combination therapy. Monotherapy with beta-blockers (BB) provided the best daytime BP control. The probabilities of having a nighttime dip pattern and nighttime BP control were higher in patients receiving diuretics compared with CCBs (OR = 0.52, P = 0.05 and OR = 0.41, P = 0.007, respectively). Patients receiving RAS/diuretic combination therapy had a higher probability of having controlled nighttime BP compared with those receiving RAS/CCB (OR = 0.45, P = 0.004). Compared with RAS/diuretic therapy, BB/CCB therapy had a higher probability of achieving daytime BP control (OR = 1.27, P = 0.45). Conclusions Antihypertensive monotherapy and dual combination drug therapy provided different ambulatory BP control and nighttime BP dip patterns. BB-based regimens provided lower daytime BP, whereas diuretic-based therapies provided lower nighttime BP, compared with other antihypertensive regimens.

Abstract:OBJECTIVES: The association of temperature maintenance with blood pressure (BP) has not been studied and the association of temperature with season-related BP was not being well understood. The present study aims to be further aware of season-related BP variation in Chinese adults.METHODS: We used decoded data from adults who visited the hospital for health examination from January 2008 to November 2013. Multivariate regression models were used to analyses the association between outdoor temperature and BP.RESULTS: The analysis included 438,811 adults. Larger change of BP accompanied with unit change of average outdoor temperature among outpatient than inpatient patients. The association of temperature with BP was stronger in normotensives than that in hypertensives. Compared with temperature variation, the impact of temperature maintenance on BP was smaller. These associations were stronger in men, older individuals.CONCLUSIONS: Temperature was strongly inversely associated with BP in Chinese adults. Temperature maintenance was mildly associated with BP compared to temperature variance. Seasonal temperature variation should be considered when screening for hypertension.

Abstract:Blood pressure (BP) is monitored and managed to prevent cardiovascular complications of hypertension, but BP variability (BPV) has not been sufficiently studied. This analysis assessed whether patients receiving amlodipine vs other antihypertensive agents had lower BPV after ≥12 weeks of treatment. Studies were included if individual subject data were available, had ≥1 active comparator, and treatment duration was ≥12 weeks. BPV was assessed using standard deviation (SD) and coefficient of variation (CV) of systolic BP across visits from 12 weeks. Individual trial and meta-analyses were performed for SD- and CV-based methodology. Five studies (47,558 BPV-evaluable patients) were included. Patient characteristics were largely consistent across the studies, but BP measurements varied from ∼4 months to ∼6 years. BPV with amlodipine was significantly (P < .0001) lower vs atenolol and lisinopril; significantly (P < .0001) lower than enalapril in one study and numerically, but not significantly lower in another; and similar to chlorthalidone and losartan. Meta-analysis revealed a treatment difference (standard error) for amlodipine vs all active comparators of −1.23 (0.46; P = .008) mm Hg using SD and −0.86 (0.31; P = .005) using CV. These findings suggest that amlodipine is effective for minimizing BPV. Future studies need to confirm a causal link between BPV and cerebrovascular/cardiovascular outcomes.
