Abstract:Background: Ultrasound biomicroscopy (UBM) is a high-resolution ultrasound technique that allows noninvasive, in vivo imaging of the ocular anterior segment structures. Before interpreting the UBM images of the diseased eyes, it is essential to understand the structures seen in the UBM image of the normal eye. Purpose: This video is a compilation of short video clips that gives description of identification of the anterior segment structures in the axial scan, a cross-sectional view through the anterior chamber angle region of a normal subject in a radial scan, and identification of ciliary processes in the transverse scan. Synopsis: UBM provides two-dimensional, grayscale images of the various anterior segment structures and allows all these structures to be imaged simultaneously, in their normal state, as they occur in the living eye. The real-time image is displayed on a video monitor and can be recorded for qualitative and quantitative analysis. Role of B-scan ultrasonography in the localization of intraocular foreign bodies in the anterior segment: a report of three cases

Abstract:Background The accurate localization of intraocular foreign bodies (IOFBs) is very important for the management of ocular trauma patients. B-scan ultrasonography is usually used to detect IOFBs in the posterior segment. Here, we report three cases with IOFBs in the anterior segment near the posterior lens capsule, which were accurately localized by B-scan ultrasonography under dynamic transversal scanning. Case presentation All three patients had a history of ocular trauma, and their clinical symptoms were compatible with the persistence of IOFBs. It was difficult to get a direct visualization of IOFBs with slit-lamp biomicroscopy because of opacities of the cornea and traumatic cataract. A computed tomography scan detected IOFBs in the anterior segment, but could not determine the exact location. Ultrasound biomicroscopy was performed but failed to show any IOFBs owing to the limited depth of penetration. B-scan ultrasonography was further applied but also failed to show any intraocular foreign bodies using axial scanning, a routine procedure of B-scan ultrasonography examination. However, using dynamic transversal scanning of B-scan ultrasonography, the accurate location of IOFBs was eventually shown to be embedded in the posterior lens cortex in case 1, adjacent to the posterior lens capsule in case 2, and located in the anterior vitreous close to the posterior lens capsule in case 3. Different surgical procedures were designed according to localization by B-scan ultrasonography, and all IOFBs were successfully removed. Conclusion B-scan ultrasonography is a simple and effective imaging modality in the localization of IOFBs in traumatic cataract. Transversal scanning is more suitable than axial scanning to detect IOFBs in the anterior segment near the posterior lens capsule.
Three-Dimensional In Vivo Imaging Of Human Anterior Eye Segment With Optical Coherence Tomography

Abstract:Objects: A novel computer method for three-dimensional in vivo imaging of human anterior eye segment is described with optical coherence tomography. Methods: A laser beam was projected at an oblique angle on the cornea and scanned to acquire 18 optical section images at 10-degree intervals across the corneal area. Tissue's edges were detected using Canny detector, and Track Points arithmetic was designed to extract the single-pixel edges, which were smoothed by curve fitness. According to the characters of rotating image, which appeared displaced because of the motion blur of patients, the classical hat-head method was improved to build 3D model for OCT images sequence. Three B-Spine interpolation and curve fitness would be used to obtain the outlines if they were lost in original OCT images. The optical section images were segmented to construct a series of anterior eye. Imaging was performed in normal subject and patients. Results: A series of digital 3D model of anterior eye was reconstructed, which allowed enhanced visualization of the micro-structures, including the section of anterior eye segment at any rotating degree, the contours of corneal thickness and curvature, and the depths and angles of anterior chamber. Conclusion: A method for in vivo three-dimensional anterior eye imaging was developed, which has potential as a tool for objective, noninvasive and real-time evaluation of diseases.

Abstract:Ultrasound biomicroscopy (UBM) is an invaluable investigation for imaging anterior segment structures. Although it is operator‐dependent and time consuming, unlike optical‐based imaging techniques, it is able to image structures posterior to the iris, such as the zonules, ciliary body and part of the pars plana. It is especially useful in advanced cataracts, traumatic cataracts, subluxed lenses, posterior polar cataracts, and congenital and developmental anomalies affecting the anterior segment. It provides diagnostic information in eyes with complex cataracts or intraocular lens (IOL)‐related pathology, and aids in surgical planning in order to minimise complications. In this review, we describe the UBM features of various lenticular pathologies and demonstrate its application in the diagnosis and surgical management of lens and IOL‐related pathologies.

Abstract:Ocular trauma often involves intraocular foreign bodies (IOFBs) that pose challenges in accurate diagnosis due to their size, shape, and material composition. In this study, we proposed a novel whole-eye 3D ophthalmic ultrasound B-scan (3D-UBS) system for automating image acquisition and improved 3D visualization, thereby improving sensitivity for detecting IOFBs. 3D-UBS utilizes 14 MHz Clarius L20 probe, a motorized translation stage, and a surgical microscope for precise placement and movement. The system's 3D point spread function (PSF) is 0.377 × 0.550 × 0.894 mm 3 characterized by the full-width at half-maximum intensity values in the axial, lateral and elevation directions. Digital phantom and ex vivo ocular models were prepared using four types of IOFBs (i.e., plastic, wood, metal, and glass). Ex vivo models were imaged with both 3D-UBS and clinical computed tomography (CT). Image preprocessing was performed on 3D-UBS images to remove uneven illumination and speckle noise. Multiplanar reformatting in 3D-UBS provides optimal plane selection after acquisition, reducing the need for a trained ultrasonographer. 3D-UBS outperforms CT in contrast for wood and plastic, with mean contrast improvement of 2.43 and 1.84 times, respectively. 3D-UBS was able to identify wood and plastic IOFBs larger than 250 μm and 300 in diameter, respectively. CT, with its wider PSF, was only able to detect wood and plastic IOFBs larger than 600 and 550 μm, respectively. Although contrast was higher in CT for metal and glass IOFBs, 3D-UBS provided sufficient contrast to identify those. 3D-UBS provides an easy-to-use, non-expert imaging approach for identifying small IOFBs of different materials and related ocular injuries at the point of care.

Abstract:Developments in optical coherence tomography (OCT) have expanded its clinical applications for ultra-high resolution imaging of the anterior segment of the human eye. This review presents the latest advances for imaging the anterior segment of the eye using ultra-high resolution OCT (UHR-OCT). Unique applications of UHR-OCT technology in clinical and basic scientific laboratory research are discussed and a summary of the results is provided. The authors focused on the use of UHR-OCT for imaging of tear dynamics, contact lens interactions with the corneal surface, and in vivo histological diagnosis of disorders of the cornea, as well as the future direction in this field.

Abstract:To measure by ultralong scan depth optical coherence tomography (UL-OCT) dimensional changes in the anterior segment of human eyes during accommodation.Evaluation of diagnostic test or technology.Forty-one right eyes of healthy subjects with a mean age of 34 years (range, 22-41 years) and a mean refraction of -2.5 ± 2.6 diopters were imaged in 2 repeated measurements at minimal and maximal accommodations.A specially adapted and designed UL-OCT instrument was used to image from the front surface of the cornea to the back surface of the crystalline lens. Custom software corrected the optical distortion of the images and yielded the biometric measurements. The coefficient of repeatability and the intraclass correlation coefficient were calculated to evaluate the repeatability and reliability.Anterior segment parameters and associated repeatability and reliability upon accommodation. The dimensional results included central corneal thickness (CCT), anterior chamber depth and width (ACD, ACW), pupil diameter (PD), lens thickness (LT), anterior segment length (ASL = ACD + LT), lens central position (LCP = ACD + 1/2LT), and horizontal radii of the lens anterior and posterior surface curvatures (LAC, LPC).Repeated measurements of each variable within each accommodative state did not differ significantly (P>0.05). The coefficients of repeatability (CORs) and intraclass correlation coefficients for CCT, ACW, ACD, LT, LCP, and ASL were excellent (1.2%- 3.59% and 0.998-0.877, respectively). They were higher for PD (18.90%-21.63% and 0.880-0.874, respectively) and moderate for LAC and LPC (34.86%-42.72% and 0.669-0.251, respectively) in the 2 accommodative states. Compared with minimal accommodation, PD, ACD, LAC, LPC, and LCP decreased and LT and ASL increased significantly at maximal accommodation (P<0.05), whereas CCT and ACW did not change (P>0.05).The UL-OCT measured changes in anterior segment dimensions during accommodation with good repeatability and reliability. During accommodation, the back surface of the lens became steeper as the lens moved forward.The authors have no proprietary or commercial interest in any of the materials discussed in this article.

Abstract:Morphological and hemodynamic changes in the ocular vasculature are important signs of various ocular diseases. The evaluation of the ocular microvasculature with high resolution is valuable in comprehensive diagnoses. However, it is difficult for current optical imaging techniques to visualize the posterior segment and retrobulbar microvasculature due to the limited penetration depth of light, particularly when the refractive medium is opaque. Thus, we have developed a 3D ultrasound localization microscopy (ULM) imaging method to visualize the ocular microvasculature in rabbits with micron-scale resolution. We used a 32 × 32 matrix array transducer (center frequency: 8 MHz) with a compounding plane wave sequence and microbubbles. Block-wise singular value decomposition spatiotemporal clutter filtering and block-matching 3D denoising were implemented to extract the flowing microbubble signals at different imaging depths with high signal-to-noise ratios. The center points of microbubbles were localized and tracked in 3D space to achieve the micro-angiography. The in vivo results demonstrate the ability of 3D ULM to visualize the microvasculature of the eye in rabbits, where vessels down to 54 μm were successfully revealed. Moreover, the microvascular maps indicated the morphological abnormalities in the eye with retinal detachment. This efficient modality shows potential for use in the diagnosis of ocular diseases.

The Correlation of Anterior Segment Structures in Primary Congenital Glaucoma by Ultrasound Biomicroscopy with Disease Severity and Surgical Outcomes.

Abstract:PURPOSE:To evaluate the anterior segment structures using ultrasound biomicroscopy (UBM) in primary congenital glaucoma (PCG) and explore their correlation with disease severity and surgical outcomes. METHODS:Clinical information of PCG patients who underwent UBM prior to their first glaucoma surgeries from September 2014 to March 2021 were reviewed. The study included 214 UBM images of 154 PCG eyes and 60 fellow unaffected eyes. Anterior segment characteristics were analyzed. UBM parameters, including the iris thickness (IT) at variant distances from the pupil edge and iris root, anterior chamber depth (ACD), and pupil diameter (PD), were compared between two groups and their relationship with clinical factors and surgical outcomes were analyzed in PCG eyes. RESULTS:PCG eyes had unclear scleral spur, thin iris, wide anterior chamber angle, deep anterior chamber, rarefied ciliary body, elongated ciliary processes, and abnormal anterior iris insertion. ITs were thinner, ACD was deeper, and PD was larger in PCG eyes than fellow unaffected eyes (all P < 0.001). In PCG eyes, thinner ITs correlated with bilateral involvement and earlier age at presentation, and larger PD correlated with earlier age at presentation (P = 0.030) and higher intraocular pressure (P < 0.001). Thinner IT2 (P = 0.046) and larger PD (P = 0.049) were identified as risk factors for surgical failure. CONCLUSION:UBM is a powerful technique to exam anterior segment structures in PCG. The anatomical features are associated with disease severity and surgical outcomes, providing essential clinical insights.

Abstract:Objective:To observe the changes in the anterior chamber before and after cataract surgery by ultrasound biomicroscopy (UBM) in eyes with primary angle closure glaucoma. Methods: Ultrasound biomicroscopy was used for anterior chamber imaging in 78 eyes of 50 patients with primary angle closure glaucoma before and 1 month after cataract phacoemulsification and intraocular lens implantation. And the intraocular pressures were recorded at the same time. Results: UBM allowed the imaging of the entire anterior eye segment. In the images, the differences between iris thickness (IT), iris zonule distance (IZD) and iris lens angle (θ2) before and after the operation were statistically insignificant. After the operation, marked increases were observed in the anterior chamber depth (ACD), angle opening distance at 500 μm from the scleral spur (AOD500), trabecular iris angle (θ1) and trabecular ciliary process distance (TCPD), with statistically very significant differences from preoperation (P < 0.01). Scleral ciliary body angle (θ3) was also increased (P < 0.05). The intraocular pressures were normal after the withdrawal of medication postoperatively. Visual acuity was improved in all the patients after the operation. Conclusion: Primary angle closure glaucoma can be successfully managed by cataract phacoemulsification, which, at the early stage of the disease, can deepen the anterior chamber, widen the chamber angle and reopen the trabeculae. UBM is very helpful to the operation by providing rapid images of the anterior eye segment.

Abstract:ABSTRACT We cloned a gene, sugE, from the chromosome of Enterobacter cloacae ATCC 13047. Analysis of the susceptibilities of the sugE-containing strain (Escherichia coli KAM32/pSUGE28) and sugE-deficient E. cloacae (EcΔsugE) showed that SugE confers resistance to cetyltrimethylammonium bromide, cetylpyridinium chloride, tetraphenylphosphonium, benzalkonium chloride, ethidium bromide, and sodium dodecyl sulfate. We also investigated expression of sugE. We confirm here that SugE from E. cloacae is an SMR family transporter as determined by observing its energy-dependent drug efflux activity.
Integrating a Fundus Camera with High-Frequency Ultrasound for Precise Ocular Lesion Assessment

Abstract:Ultrasound A-scan is an important tool for quantitative assessment of ocular lesions. However, its usability is limited by the difficulty of accurately localizing the ultrasound probe to a lesion of interest. In this study, a transparent LiNbO3 single crystal ultrasound transducer was fabricated, and integrated with a widefield fundus camera to guide the ultrasound local position. The electrical impedance, phase spectrum, pulse-echo performance, and optical transmission spectrum of the ultrasound transducer were validated. The novel fundus camera-guided ultrasound probe was tested for in vivo measurement of rat eyes. Anterior and posterior segments of the rat eye could be unambiguously differentiated with the fundus photography-guided ultrasound measurement. A model eye was also used to verify the imaging performance of the prototype device in the human eye. The prototype shows the potential of being used in the clinic to accurately measure the thickness and echogenicity of ocular lesions in vivo.
Accuracy and Feasibility of Three-Dimensional Ultrasound Testing in Eye Clinic and Emergency Department Patients with Vision Complaints

Abstract:Background: Ocular emergencies comprise 2-3% of emergency department (ED) visits, with retinal detachment requiring emergency surgery. Two-dimensional ultrasound is a rapid bedside tool but is highly operator dependent. Objective: We determined three-dimensional ultrasound (3DUS) feasibility, acceptability, and usability in eye pathology detection using the ophthalmologist examination as reference standard. Methods: We performed a prospective, blinded cohort study of a 3DUS-enabling device in 30 eye clinic and ED patients with visual symptoms and calculated 3DUS performance characteristics. Two expert readers interpreted the 3DUS images for pathology. All participants completed surveys. Results: 3DUS sensitivity was 0.81, specificity 0.73, positive predictive value 0.54, negative predictive value 0.91, and likelihood ratio (LR)+/LR- 3.03 and 0.26, respectively. Novice and expert sonographers had "substantial" agreement in correct diagnosis of abnormal vs. normal (κ = 0.68, 95% confidence interval 0.48-0.88). Most patients indicated that 3DUS is fast, comfortable, helps them understand their problem, and improves provider interaction/care, and all sonographers agreed; 4/5 sonographers felt confident performing ultrasound. Expert readers correctly identified an abnormal eye in 83/120 scans (76%) and correct diagnosis in 72/120 scans (65%), with no statistical difference between novice (79%; 69%) and expert (72%; 61%) sonographers (p = 0.39, p = 0.55), suggesting reduced operator dependence. Reader diagnosis confidence and image quality varied widely. Image acquisition times were fast for novice (mean 225 ± 83 s) and expert (201 ± 51) sonographers, with fast expert reader interpretation times (225 ± 136). Conclusions: A 3DUS-enabling device demonstrates a sensitivity of 0.81 and specificity of 0.73 for disease detection, fast image acquisition, and may reduce operator dependence for detecting emergent retinal pathologies. Further technological development is needed to improve diagnostic accuracy in identifying and characterizing retinal pathology.

Abstract:Ocular health is currently a major concern for astronauts on current and future long-duration spaceflight missions. Spaceflight-associated neuro-ocular syndrome (SANS) is a collection of ophthalmic and neurologic findings that is one potential physiologic barrier to interplanetary spaceflight. Since its initial report in 2011, our understanding of SANS has advanced considerably, with a primary focus on posterior ocular imaging including fundus photography and optical coherence tomography. However, there may be changes to the anterior segment that have not been identified. Additional concerns to ocular health in space include corneal damage and radiation-induced cataract formation. Given these concerns, precision anterior segment imaging of the eye would be a valuable addition to future long-duration spaceflights. The purpose of this paper is to review ultrasound biomicroscopy (UBM) and its potential as a noninvasive, efficient imaging modality for spaceflight. The analysis of UBM for spaceflight is not well defined in the literature, and such technology may help to provide further insights into the overall anatomical changes in the eye in microgravity.

Abstract:The effective treatment of primary angle-closure glaucoma (PACG) is surgery.The anterior segments of the eye can be analyzed and quantitatively measured with the ultrasound biomicroscope (UBM).Thus,the pathogenesis of PACG can be evaluated precisely,such as pupillary block angle-closure glaucoma,non-pupillary angle block glaucoma or combined mechanisms.After surgery,the peripheral iridectomy,the shape of the filtering bleb as well as the postoperative complications such as ciliary body detachment,choroidal detachment and delayed suprachoroidal hemorrhage,can be also observed with UBM.
Automatic Classification of Anterior Chamber Angle Based on Ultrasound Biomicroscopy Images

Abstract:Introduction: Evaluating the anterior chamber angle (ACA) is important for the early diagnosis and treatment of primary angle-closure glaucoma. The assessment of ultrasound biomicroscopy (UBM) images usually requires well-trained ophthalmologists and screening for patients with narrow ACA is usually time- and labor-intensive. Therefore, the automatic assessment of UBM could be cost-effective and valuable in daily practice. Objective: The objective of this study is to develop an automatic method for localizing and classifying ACA based on UBM images. Methods: UBM images were collected and a coarse-to-fine method was used to localize the apex of the angle recess. By analyzing the grayscale features around the angle recess, closed angles were identified, and the rest were then classified as open or narrow angles, based on the degree of ACA. Using manual classification as the reference standard, the overall accuracy (OAcc), sensitivity (Sen), specificity (Spe), and balanced accuracy of the automatic classification method were evaluated. Results: A total of 540 UBM images from 290 participants were analyzed. Using these UBM images and the proposed method, the ACA was classified as open, narrow, or closed. During processing, the method localized the angle recess with 95% accuracy. The OAcc of the ACA classification was 77.8%, and the Spe and Sen of our method were 85.8 and 81.7% for angle closure; 88.9 and 75.6% for open angles; 91.9 and 76.1% for narrow angles, respectively. Conclusions: Our method of automatic angle localization and classification based on UBM images is feasible and reliable. Abstract:The rapid advancement of anterior segment imaging technologies in recent years has allowed for precise visualization and accurate assessment of the anterior segment structures. Therefore, these devices have become necessary for better diagnosis and treatment of anterior segment disorders, i.e., corneal diseases, cataract, glaucoma, and disorders of the eyelid and lacrimal system.

Abstract:AIM:To evaluate the use of ultrasound biomicros-cope (UBM) in operative treatment of glaucoma com-bined with cataract,and to observe on the anterior chamber angle and its related structure in the patients with primary angle-closure glaucoma (PACG) after operation.·METHODS:Combined MMF,intraocular lens (IOL) implantation and trabeculectomy were performed in 30 patients (30 eyes) with cataract and PACG. The changes of anterior chamber angle,anterior chamber distance were determined quantitatively by using UBM before and post operation 1 month. Trabecular iris angle (TIA),angle opening distance (AOD500),trabecular ciliary processes distance (TCPD) and iris distance (ID1) were measured. At the same time,the intraocular pressure (IOP) was also recorded. ·RESULTS:The IOP were obviously decreased after operation. There were significant differences between the preoperative and postoperative ACD,AOD500,TIA,TCPD (P<0.05). There was no statistical difference between the preoperative and postoperative ID1. ·CONCLUSION:Combined glaucoma and cataract sur-gery can deepen the anterior chamber and anterior chamber angle,and reopen trabecular meshwork. It contributes to control intraocular pressure better and reduce postoperative complications such as shallow anterior chamber. A larger study with longer follow-up is needed to determine its long-term clinical outcomes.
In vivo three-dimensional confocal laser scanning microscopy of corneal surface and epithelium

Abstract:BACKGROUND/AIMS: To evaluate in vivo three-dimensional (3-D) confocal laser scanning microscopy (CLSM) as a technique for visualising the corneal surface and epithelium.METHODS: Ten human corneas (three from healthy volunteers, three with bullous keratopathy, three from patients following penetrating keratoplasty, and one with corneal erosion) were examined by 3-D CLSM. A novel polymethyl methacrylate (PMMA) contact cap was designed to minimise artefacts due to applanation pressure.RESULTS: 3-D reconstruction and different visualisation techniques (volume rendering, cross-section, en face view, oblique section and surface reconstruction) were performed to demonstrate alterations to corneal surface and epithelium. Image quality (cell identification, motion blur, absence of compression artefacts, imaging of superficial structures and of subepithelial nerve plexus) was considerably superior to that obtained using a conventional contact cap with a planar surface.CONCLUSIONS: 3-D CLSM permits in vivo visualisation and analysis of the corneal surface and of spatial arrangement at the cellular level in epithelium in normal and pathological corneas. The novel design of the contact cap minimises artefacts due to applanation pressure and improves the image quality of epithelial structures. The method provides a basis for further in vivo studies of alterations to corneal surface structure and its cellular arrangement.

Preclinical imaging of iridocorneal angle and fundus using a modified integrated flexible handheld probe

Abstract:A flexible handheld imaging probe consisting of a [Formula: see text] charge-coupled device camera, light-emitting diode light sources, and near-infrared laser source is designed and developed. The imaging probe is designed with specifications to capture the iridocorneal angle images and posterior segment images. Light propagation from the anterior chamber of the eye to the exterior is considered analytically using Snell's law. Imaging of the iridocorneal angle region and fundus is performed on ex vivo porcine samples and subsequently on small laboratory animals, such as the New Zealand white rabbit and nonhuman primate, in vivo. The integrated flexible handheld probe demonstrates high repeatability in iridocorneal angle and fundus documentation. The proposed concept and methodology are expected to find potential application in the diagnosis, prognosis, and management of glaucoma.
