Abstract:A large body of empirical research shows that psychosocial risk factors (PSRFs) such as low socio-economic status, social isolation, stress, type-D personality, depression and anxiety increase the risk of incident coronary heart disease (CHD) and also contribute to poorer health-related quality of life (HRQoL) and prognosis in patients with established CHD. PSRFs may also act as barriers to lifestyle changes and treatment adherence and may moderate the effects of cardiac rehabilitation (CR). Furthermore, there appears to be a bidirectional interaction between PSRFs and the cardiovascular system. Stress, anxiety and depression affect the cardiovascular system through immune, neuroendocrine and behavioural pathways. In turn, CHD and its associated treatments may lead to distress in patients, including anxiety and depression. In clinical practice, PSRFs can be assessed with single-item screening questions, standardised questionnaires, or structured clinical interviews. Psychotherapy and medication can be considered to alleviate any PSRF-related symptoms and to enhance HRQoL, but the evidence for a definite beneficial effect on cardiac endpoints is inconclusive. A multimodal behavioural intervention, integrating counselling for PSRFs and coping with illness should be included within comprehensive CR. Patients with clinically significant symptoms of distress should be referred for psychological counselling or psychologically focused interventions and/or psychopharmacological treatment. To conclude, the success of CR may critically depend on the interdependence of the body and mind and this interaction needs to be reflected through the assessment and management of PSRFs in line with robust scientific evidence, by trained staff, integrated within the core CR team.

Psychosocial predictors for cardiac rehabilitation participation for patients with coronary artery disease

Abstract:Aims: Cardiac rehabilitation (CR) is a class Ia recommendation by international guidelines. However, the participation remains disappointingly low. Multiple barriers were already identified. Nevertheless, the relationship with psychosocial factors remains unsettled. The aim of this study is to analyse the role of psychosocial factors in CR participation. Methods and results: A prospective study on 165 patients with a percutaneous coronary intervention. The main outcome was participation in a centre-based CR programme. The independent relationship between participation and optimism, type D personality or distressed personality, health literacy, depression, social support and self-efficacy. A total number of 165 patients were included in the study of which 81 (49.1%) participated in a centre-based CR programme. Participants were predominantly male. Solely, patients participating in CR were significantly younger (P = 0.005). Participants of CR had significantly higher levels of social support especially from their significant other (P = 0.020) and family (P = 0.050). Furthermore, the health literacy score was significantly higher in the participants group (P = 0.030). A multiple logistic regression demonstrated that higher age, higher social support, not working, being obese and being able to drive a car were significant statistical predictors for CR participation. Conclusions: This study is one of the first to demonstrate the role of social support and health literacy in the participation of CR. Other well-known barriers for CR participation such as age, transport and scheduling issues were identified as barriers in this study.

Abstract:OBJECTIVES To obtain experiences and expert opinion on treatment of SSc patients with TNF-α antagonists. METHODS An investigation was carried out among the EUSTAR centres into their expertise on use of TNF-α antagonists. Assessment forms on the frequency of TNF-α inhibitor use were distributed to EULAR Scleroderma Trials and Research Group (EUSTAR) centres. Afterwards, a three round Delphi exercise was performed to obtain expert consensus on the use of TNF-α inhibitors in SSc. RESULTS Seventy-nine centres returned information on use of TNF-α antagonists in SSc patients. A total of 65 patients were treated with TNF-α inhibitors in 14 different centres. Forty-eight of the 65 patients treated with TNF-α inhibitors improved. Improvement was mainly seen in patients with arthritis, whereas the effects on fibrosis varied. In the first round of the subsequent Delphi approach, 71 out of 79 experts stated that they would use TNF-α antagonists in SSc. Arthritis was suggested as an indication for TNF-α antagonists by 75% of the experts. However, after the third stage of the Delphi exercise, the acceptance for the off-label use of TNF-α antagonists decreased and 59% recommended that TNF-α antagonists should not be used or only used in clinical trials in SSc patients, while 38% of the experts suggested the use of TNF-α antagonists for arthritis associated with SSc. CONCLUSIONS Most of the experts do not recommend the routine use of TNF-α antagonists in systemic sclerosis. Arthritis might be a potential indication in SSc, although controlled clinical trials with TNF-α antagonists are needed before general recommendations can be given.

Abstract:Background Co-morbid depression and anxiety symptoms are frequently under-recognised and under-treated in heart disease and this negatively impacts self-management. Aims The purpose of this study was to determine the prevalence, correlates and predictors of depression and anxiety in cardiac rehabilitation programmes, the impact of cardiac rehabilitation on moderate depression, anxiety and stress symptoms, and the relationship between moderate depression, anxiety and stress symptoms and cardiac rehabilitation adherence. Methods This was a retrospective cohort study of 5908 patients entering cardiac rehabilitation programmes from 2006–2017, across two Sydney metropolitan teaching hospitals. Variables included demographics, diagnoses, cardiovascular risk factors, medication use, participation rates, health status (Medical Outcomes Study Short Form-36) and psychological health (Depression Anxiety Stress Scales) subscale scores. Results Moderate depression, anxiety or stress symptoms were prevalent in 18%, 28% and 13% of adults entering cardiac rehabilitation programmes, respectively. Adults with moderate depression (24% vs 13%), anxiety (32% vs 23%) or stress (18% vs 10%) symptoms were significantly less likely to adhere to cardiac rehabilitation compared with those with normal-mild symptoms ( p < 0.001). Anxiety (odds ratio 4.395, 95% confidence interval 3.363–5.744, p < 0.001) and stress (odds ratio 4.527, 95% confidence interval 3.315–6.181, p < 0.001) were the strongest predictors of depression. Depression (odds ratio 3.167, 95% confidence interval 2.411–4.161) and stress (odds ratio 5.577, 95% confidence interval 4.006–7.765, p < 0.001) increased the risk of anxiety on entry by more than three times, above socio-demographic factors, cardiovascular risk factors, diagnoses and quality of life. Conclusion Monitoring depression and anxiety symptoms on entry and during cardiac rehabilitation can assist to improve adherence and may identify the need for additional psychological health support. Exploring the relevance and use of adjunct psychological support strategies within cardiac rehabilitation programmes is warranted.

Abstract:Background: It is known that psychosocial distress affects the morbidity and mortality of patients with cardiovascular disease of every age. The aim of this study was to produce novel information on how psychological distress can influence cardiovascular performance in patients after cardiac surgery undergoing multidisciplinary cardiac rehabilitation. Methods: Patients (n = 57) admitted after cardiac surgery for valvular or coronary disease underwent, within 5 days of admission, the Symptom Checklist-90-Revised (SCL-90-R) self-report questionnaire to measure psychiatric symptoms and the 12-item General Health Questionnaire (GHQ-12) to assess the level of psychological distress. The Positive Symptom Distress Index (PSDI) was measured to indicate the amplitude of symptom distress. Cardiovascular performance was assessed by a 6 min walking test (6MWT) at admission and discharge, and oxygen consumption (VO2 max) was derived. Results: Within the SCL-90-R score, somatic symptoms (47.4%), depressive and anxiety symptoms (36.8% and 33.3%, respectively), symptoms of phobic anxiety (21.1%), and psychoticism (24.6%) were over-represented. As for the GHQ-12, 75.4% of the sample reported an abnormally negative perception of their health status. An inverse correlation was shown between the variation in 6MWT and SCL depression (p = 0.048), PSDI (p = 0.022), and the GHQ-12 (p = 0.040). Similarly, an inverse correlation was shown between the variation in the VO2 max, GHQ-12 (p = 0.041), and the PSDI (p = 0.023). Conclusions: Post-cardiac surgery cardiac rehabilitation was associated with increased symptoms of psychological discomfort, as compared with the general population. The amplitude of psychological distress, depression, and hostility are associated with limited improvement in performance. These data strengthen the need for psychological support during cardiac rehabilitation programs.

Abstract:Abstract Funding Acknowledgements Type of funding sources: None. Introduction Cardiac Rehabilitation Programs (CRP) still do not reach all patients with this indication. In the context of COVID-19 pandemic, the new home-CRP model has been introduced. Purpose To evaluate the results in terms of improvement of psychosocial outcomes and changes in employment situation with home-CRP. Methods Cross-Sectional Observational study that included patients of both sexes, ≥ 18 years, after acute coronary syndrome or after coronary revascularization, included in home CRP. Information was collected from the computerized clinical history of the patients (diseased vessels, degree and type of revascularization, risk stratification). Data was collected at the beginning and in the end after 2 months of the home CRP: smoking habit, weight, abdominal perimeter, anxiety/ depression (measured by HADS: Hospital Anxiety and Depression scale) and employment status. Results A total of 173 patients has been studied. Mean age was 51.6 (SD: 7.94), 15.6% women. Indication for home CRP was non ST elevation myocardial infarction (NSTEMI) in 45.7%, 31.8% had STEMI, 17.9% after elective coronary interventions ,3.5% after cardiac arrest, and 1.2% corresponded to other causes. Patients were at low risk in 50%, 48% moderate and 2% high risk. With 1, 2, or 3 vessels disease in 38.1%, 28.3%, and 33.5% respectively. Coronary revascularization was obtained by percutaneous interventions in 80.9%, by surgery in 14.5% and 4.6% of patients received only medical treatment. At baseline, 33.5% obese and 55% smoked at index admission. After two months of home PRHC we obtained a reduction of 85.5% in smoking (p<0.001), a loss of 4.6 kg of weight in obese people (p=0.125), reduction of anxiety (p= 0.0012) and depression (p=0.0013) amongst those who had a HADS scale higher than 10, and improvement in employment status with return to work in 61,2% (p<0.001). Conclusions The present study indicates that home- cardiac rehabilitation programs reduce anxiety, depression and improve return to work after acute coronary syndrome.

Abstract:Introduction Advanced heart failure therapies, heart transplantation (HT) and left ventricular assist device (LVAD) are limited due to scarcity of donors, expenses, and expansive lifetime care. Psychosocial determinants are persistently emphasized, but subjectively considered during the selection process. Described is our center's proof of concept utilizing a new behavioral/palliative consult system. Hypothesis The delivery of validated psychosocial measures, yields an objective risk score potentiating patient stratification and improving clinical outcomes. Methods HT or LVAD patients were assessed by Patient Health Questionnaire 9-Item (PHQ-9), Generalized Anxiety Disorder 7-Item (GAD-7), and the Millon Behavioral Medicine Diagnostic (MBMD) prior to selection. Associations with post-HT/LVAD readmission-free 1-year survival were assessed using univariable analysis. Risk factors with p<0.20 were included in a cumulative risk score. The cumulative sum of dichotomous risk factors was used to categorize patients into 3 risk groups (G1:0, G2: 1-5, G3: 5+). Kaplan Meier curve was used to assess the readmission-free 1-year survival of each risk groups. The hazards ratio of the risk groups was evaluated using a log rank test. Results Twenty-four patients were included (median age of 59, 46% female). The readmission and death rate within 1-year was 54%. Demographic, socioeconomic factors, and substance use rates did not differ significantly. Risk G3 patients with moderate anxiety, moderate depression, underestimating HT/LVAD care, and higher MBMD scores (anxiety, depression, emotional lability, inhibited, dejected, oppositional, denigrated, isolation, interventional fragility, medication abuse, and psychological referral) had a higher risk of death and/or readmission. Readmission-free survival decreased with increasing risk scores (p=0.002). The figure. Conclusions An objective risk score utilizing psychosocial determinants is feasible,. Aiding in patient selection and identifying at-risk patients may provide opportunities to improve outcomes.

Clinical outcomes associated with depression, anxiety and social support among cardiac rehabilitation attendees

Abstract:Objective: To utilize existing medical record information in order to examine the relationship between depression, anxiety, levels of social support and clinical outcomes in cardiac rehabilitation attendees. Method: In a tertiary care centre 389 records for cardiac rehabilitation outpatients were analysed. Hospital Anxiety and Depression Scales scores collected at week 4 of their cardiac rehabilitation programme were linked with medical record information on demographic, lifestyle, medical and other coronary heart disease risk factors, as well as follow-up cardiovascular events and readmissions for an average of 2.6 years. Variables of interest were anxiety and depression scores and proxy measures of social support. Clinical outcome measures were number of hospital admissions, length of stay and mortality. Results: Multiple variable analysis revealed higher anxiety scores were associated with more hospital admissions and higher depression scores were associated with longer length of stay after controlling for other established prognostic risk factors. Depression, anxiety and social support were not associated with mortality, however there was a low mortality rate in the present study. Conclusion: A comprehensive summary of the characteristics of cardiac rehabilitation patients was gathered through the linkage and examination of existing clinical databases. The results of this study provide some support for a prognostic role of depressive and anxiety symptoms and highlight the need for screening for psychological distress and provision of appropriate interventions when indicated.
The Cardiac Rehabilitation Inventory: A New Method of Tailoring Patient Support

Abstract:Background:Uptake and adherence to cardiac rehabilitation (CR) are low, and a contributing factor to this may be the practical difficulties of providing a tailored CR environment suited to individual preferences and needs. Objective:The aim of this study was to develop and test a short questionnaire that CR practitioners can use to understand individual patient need and tailor support accordingly. Methods:A conceptual framework of engagement in CR was derived from a comprehensive literature review and the content analysis of semistructured interviews with 15 CR patients. The conceptual framework was used to construct the first version of the Cardiac Rehabilitation Inventory (CRI), which comprised 42 items. Responses on the CRI were measured using a 5-point Likert scale. The CRI was administered to 380 phase III and IV CR patients, and factor analysis (FA) was used to identify salient CR engagement factors. Results:The simplest structure found using FA was three 6-item subscales that all had good levels of internal consistency (Cronbach’s &agr;) and were labeled (a) outcome anxiety, &agr; = .726; (2) process anxiety, &agr; = .724; and (3) autonomy, &agr; = .653. The 3-factor CRI model was verified using confirmatory FA (CMin/df = 3.2, root-mean-square error of approximation = 0.073). Attenders were found to have higher levels of outcome anxiety than nonattenders (P < .001), and precontemplator nonattenders were found to have lower autonomy compared with attenders (P < .001). Standard multiple regression analysis indicated outcome anxiety was a strong predictor of CR intentions (r2 = 0.716), followed by autonomy (r2 = 0.110) and process anxiety (r2 = 0.031). Conclusions:The CRI is a reliable method of measuring CR outcome anxiety, process anxiety, and autonomy. These CRI measurements provide rehabilitation practitioners with valuable information that can help provide individual tailored support.

Introduction Cardiovascular risk factors (CVRF) may contribute to the development and adverse outcome of coronary artery disease (CAD), thus the screening for CVRF in CAD patients undergoing cardiac rehabilitation is relevant. Purpose This study aimed assessing the cardiovascular risk factors in patients with CAD undergoing cardiac rehabilitation. Methods A cross-sectional study was performed. 201 patients with CAD undergoing cardiac rehabilitation were recruited. Anthropometric measurements and analytical tests were performed for each patient. Several variables related to cardiovascular risk factors were identified: weight, body mass index, abdominal circumference, blood pressure, smoking status, sedentary lifestyle, family history, basal level of glycosylated hemoglobin, total cholesterol, LDL-cholesterol, HDL-cholesterol and triglycerides. Results 82,5% of patients were men, mean age 58,24 ± 9,40 years. 46,2% had effort angina pectoris, 32,4% had infarction and 21,4% had unstable angina pectoris. Most of the sample had a moderate risk (51,5%), whereas 33,8% had low risk and 14,7% had high risk. 63,9% underwent percutaneous coronary intervention with stent, and 27% underwent coronary artery bypass grafting. The most prevalent risk factors were overweight (77,05%), dyslipidemia (64,3%), hypertension (58,10%), sedentary lifestyle (50%), family history (44,90%), smoking (22%) and diabetes mellitus (20,40%). Conclusions The prevalence of cardiovascular risk factors among patients with coronary artery disease undergoing cardiac rehabilitation is high. 51,5% had a moderate risk, whereas overweight and dyslipidemia are leading risk factors.

Abstract:PURPOSE: The purpose of this study was to evaluate the effects of optimism, depression, and neuroticism on adherence and outcomes among patients in cardiac rehabilitation (CR).METHODS: Participants included 46 patients (34 men) with coronary heart disease (CHD) who completed measures of psychological functioning (optimism, depression, neuroticism), physical functioning (exercise stress test, body mass index, symptom reports), and CHD knowledge during the first week (baseline) and last week of a 12-week CR program.RESULTS: Baseline depression scores accounted for 9.5% of the variance associated with change in aerobic capacity (maximum oxygen consumption), when controlling for relevant demographic variables and program attendance. However, optimism and neuroticism were not predictive of CR outcomes. Results also indicated that program dropouts were significantly younger and reported more psychological distress at baseline than did participants who completed the program.CONCLUSION: Depression appears to have a significant influence on adherence and improvement among patients in CR, but optimism and neuroticism were not significant predictors of change.

Abstract:BACKGROUND: Young adults with Fontan circulation (FC) face psychosocial, physical, and environmental challenges and are vulnerable to emotional distress. They need holistic intervention programs to face these problems and enhance their quality of life. This study developed and evaluated a cardiac and psychosocial rehabilitation (CPR) program for young adults with FC. METHODS AND RESULTS: Thirty-eight young adults with FC aged 18-23 years were prospectively recruited from an outpatient clinic and randomized into a CPR group (n = 12) that underwent eight weekly group-based sessions combining a psychosocial program based on cognitive behavioral therapy with home-based cardiac rehabilitation (CR), a CR group (n = 12) that received only home-based CR, and a usual care control group (n = 12). The study assessed CPR program effectiveness based on the Hospital Anxiety and Depression Scale, self-efficacy, quality of life, and exercise capacity testing at three time points: pre- intervention, post-intervention, and four weeks post-intervention. Participants' average ages were 19.75 (2.28), 19.65 (1.79), and 20.38 (1.85) years in the CPR, CR, and control groups, respectively; 50% (CPR group) to 53.8% (CR and control groups) of the participants were male. After eight weeks, depression and anxiety decreased, whereas self-efficacy and quality of life increased in the CPR group compared with the CR and control groups. The groups showed no differences in exercise capacity. CONCLUSION: CPR can be part of an intervention for alleviating psychological distress and improving self-efficacy in young adults with FC. Efforts should be made to increase the efficacy of home-based CR.

Abstract:Abstract Background Cardiac rehabilitation (CR) participation after percutaneous coronary intervention (PCI) for coronary heart disease lowers the disease burden and risk of recurrent cardiac events. Examining psychological factors may improve post-PCI health behavior adherence. Purpose To determine whether psychological factors are associated with post-PCI health behavior adherence, and the role of CR participation. Methods Data from 1,682 patients (22.1% female, Mage = 64.0, SDage = 10.5 years) from the THORESCI cohort were included. Adjusted mixed models were used to examine associations between psychological factors and the 1-year course of health behaviors, using interactions to test for moderation by CR participation. Results Psychological factors were associated with the trajectories of adherence to medical advice, exercise, and diet. The strongest association found was between optimism and the trajectory of dietary adherence (B: = −0.09, p = .026). Patients with high optimism levels had a worse trajectory of dietary adherence compared to patients with low to middle optimism levels. Participation in CR buffered the associations of high anxiety, pessimism, and low to middle resilience, but strengthened the associations of high stress in the past year with the probability of smoking. Conclusions Psychological factors are associated with post-PCI health behavior adherence, but the pattern of associations is complex. Patients with high levels of anxiety, pessimism, and low to middle resilience levels may disproportionately benefit from CR. Cardiac rehabilitation programs could consider this to improve post-PCI health behavior adherence. Clinical Trials Registration # NCT02621216.

Abstract:Objective To test the measurement properties of the revised version of the English Cardiac Rehabilitation Barriers Scale (CRBS-R), suitable for hybrid delivery, structural validity, internal reliability, as well as face, cross-cultural, construct and criterion validity were assessed. Design Cross-sectional study, where participants completed an online survey via Qualtrics (2023–2024). Setting Multicentre, with cardiac rehabilitation (CR) programmes recruiting patients globally; most patients stemmed from a hybrid programme in Iran and supervised programme in Brazil. Participants Participants include inpatients or outpatients with a cardiovascular diagnosis or procedure that is indicated for participation in CR. Measures In addition to sociodemographic and CR use items, the 21-item CRBS-R was administered. It assesses multilevel barriers and was revised based on a literature review. Responses range from 1 to 5, with higher scores indicating greater barriers. Results 235 patients participated from all 6 WHO regions. Items were rated as highly applicable, and open-ended responses revealing no key barriers were omitted, supporting face and cross-cultural validity. Cronbach's α for the total CRBS-R was 0.82. Principal components analysis resulted in the extraction of 4 components, which collectively accounted for 60.5% of the variance and were all internally consistent. Construct validity was supported by associations of total CRBS scores with work status (p=0.04), exercise history (p=0.01) and social support (p=0.03). Total CRBS-R scores were significantly lower in patients who were referred and enrolled versus those who were not (both p≤0.01), confirming criterion validity. Conclusions The CRBS-R is a reliable and valid scale comprising four subscales, applicable to hybrid CR across diverse settings. It can serve as a valuable tool to support identification of patient's CR barriers, to optimise secondary prevention utilisation globally.

Abstract:Background The use of eHealth technology in cardiac rehabilitation (CR) is a promising approach to enhance patient outcomes since adherence to healthy lifestyles and risk factor management during phase III CR maintenance is often poorly supported. However, patients’ needs and expectations have not been extensively analyzed to inform the design of such eHealth solutions. Objective The goal of this study was to provide a detailed patient perspective on the most important functionalities to include in an eHealth solution to assist them in phase III CR maintenance. Methods A guided survey as part of a Living Lab approach was conducted in Germany (n=49) and Spain (n=30) involving women (16/79, 20%) and men (63/79, 80%) with coronary artery disease (mean age 57 years, SD 9 years) participating in a structured center-based CR program. The survey covered patients’ perceived importance of different CR components in general, current usage of technology/technical devices, and helpfulness of the potential features of eHealth in CR. Questionnaires were used to identify personality traits (psychological flexibility, optimism/pessimism, positive/negative affect), potentially predisposing patients to acceptance of an app/monitoring devices. Results All the patients in this study owned a smartphone, while 30%-40% used smartwatches and fitness trackers. Patients expressed the need for an eHealth platform that is user-friendly, personalized, and easily accessible, and 71% (56/79) of the patients believed that technology could help them to maintain health goals after CR. Among the offered components, support for regular physical exercise, including updated schedules and progress documentation, was rated the highest. In addition, patients rated the availability of information on diagnosis, current medication, test results, and risk scores as (very) useful. Of note, for each item, except smoking cessation, 35%-50% of the patients indicated a high need for support to achieve their long-term health goals, suggesting the need for individualized care. No major differences were detected between Spanish and German patients (all P>.05) and only younger age (P=.03) but not sex, education level, or personality traits (all P>.05) were associated with the acceptance of eHealth components. Conclusions The patient perspectives collected in this study indicate high acceptance of personalized user-friendly eHealth platforms with remote monitoring to improve adherence to healthy lifestyles among patients with coronary artery disease during phase III CR maintenance. The identified patient needs comprise support in physical exercise, including regular updates on personalized training recommendations. Availability of diagnoses, laboratory results, and medications, as part of a mobile electronic health record were also rated as very useful. Abstract:Background: Frailty is an indicator of a decline in quality of life and functional capacity in cardiac rehabilitation (CR) patients. Currently, there is no standardized assessment tool for frailty used in CR. The aim of this study was to determine if the Clinical Frailty Scale (CFS) is feasible for assessing frailty in CR. Methods: Prospective, cross-sectional study within the framework of the ongoing multicenter prehabilitation study "PRECOVERY". Patients ≥75 years undergoing CR after cardiac procedure (n=122) were recruited in four German inpatient CR facilities. Assessments included: CFS, Katz-Index, hand grip strength (HGS), Short Physical Performance Battery (SPPB) and six-minute-walk test (6MWT). Outcomes were frailty (CFS≥4) and the correlation of frailty with assessments of functional capacity, activities of daily living and clinical parameters. Statistical analysis included descriptive statistics and correlations, using the spearman correlation coefficient and chi-square test to test for significance. Results: Data from 101 patients (79.9±4.0 years; 63% male) were analyzed. The mean CFS score was 3.2±1.4; 41.6% were defined as frail (CFS≥4). The mean time required to assess the CFS was 0.20 minutes. The findings show that CFS correlates significantly (p<0.001) with the following factors: Katz-Index, HGS, SPPB-Score and 6MWT (r≤-0.575). In addition, CFS correlated with small to moderate effects with co-morbidities (r=0.250), as-needed medications and need for nursing assistance (r≤0.248). Conclusions: The CFS assessment can be performed in under one minute and it correlates significantly with assessments of functional capacity, activities of daily living and clinical parameters in the CR setting. Abstract:Abstract Few studies have investigated psychosocial characteristics and lifestyle behaviors of participants at programs for secondary prevention of colorectal cancer (CRC). This study aimed, through a comprehensive psychosomatic assessment based on clinimetric principles, to evaluate psychosocial characteristics and lifestyle behaviors in participants at CRC secondary prevention program, and to investigate the associations between these variables and endoscopic outcomes. In this cross-sectional study, the first 150 consecutive asymptomatic participants at the CRC prevention program who resulted positive to fecal occult blood test (FOBT) and were thus referred to colonoscopy, underwent a psychosomatic assessment including psychiatric diagnoses (DSM-5), psychosomatic syndromes (DCPR-R), psychological distress, psychological well-being and lifestyle behaviors. Whereas only 5.3% of the sample showed at least one DSM-5 diagnosis, 51.3% showed at least one DCPR syndrome, such as allostatic overload, alexithymia, Type A behavior, and demoralization. Patients affected by psychosomatic syndromes presented with significantly higher psychological distress, lower psychological well-being and unhealthy lifestyle behaviors, such as tobacco smoking and unhealthy diet, in comparison with patients without DCPR syndromes. Among endoscopic outcomes, the presence of adenomas was significantly associated with DCPR irritable mood. In a clinical context of secondary prevention addressing asymptomatic patients with positive FOBT, a comprehensive psychosomatic assessment may provide relevant clinical information for those patients who present certain psychosomatic syndromes associated with high psychological distress, impaired psychological well-being, unhealthy lifestyle behaviors and colorectal precancerous lesions. The results of the present study indicate a road to the practice of “preventive” medicine at CRC screening program.

The purpose of this study is to characterize the patients who participated in the first three years of Hospital Garcia de Orta's cardiac reconditioning program in terms of key demographic variables and the prevalence of common risk factors.Material and methods: A transversal, descriptive study of patients admitted to CRP during the first three years of existence in Hospital Garcia de Orta. The following parameters were analyzed: age, gender, admission diagnosis and risk factors for CVD - hypertension, diabetes mellitus, dyslipidemia, increased body mass index, smoking and family history of CVD. This study follows all the principles of the Declaration of Helsinki.Results: 57 patients were admitted with a mean age of 53 [32–79] years old, predominantly male (74%). 55 were admitted after ACS (53 status after PCI and 2 status after CABG), 1 after valvular replacement surgery and 1 with heart failure class III (NYHA). The prevalence of hypertension was 56%; LDL ≥100 mg/dL 54%; HDL ≤45 mg/dL 52%; cholesterol ≥175 mg/dL 52%; triglycerides ≥150 mg/dL 39%; overweight 44%; obesity 19%; 53% were smokers; 16% with diabetes mellitus and family history of CVD in 63%.Discussion and conclusion: During this period only 57 patients were admitted, mainly due to a lack of staff, resources and infrastructures. Our CRP is expanding, foreseeing a greater admission of eligible patients in the future. In spite of the small sample size we observe a high prevalence of CV risk factors among our patient which is consistent with the data from literature. Thus referral to CRP is crucial as they include several strategies which aim to reduce CV risk factors and therefore reduce of cardiovascular disease morbi-mortality.

Abstract:Medical Decision Making, Ahead of Print. BackgroundContext-specific measures with adequate external validity are needed to appropriately determine psychosocial effects related to screening for cognitive impairment.MethodsTwo-hundred adults aged ≥65 y recently completing routine, standardized cognitive screening as part of their Medicare annual wellness visit were administered an adapted version of the Psychological Consequences of Screening Questionnaire (PCQ), composed of negative (PCQ-Neg) and positive (PCQ-Pos) scales. Measure distribution, acceptability, internal consistency, factor structure, and external validity (construct, discriminative, criterion) were analyzed.ResultsParticipants had a mean age of 73.3 y and were primarily female and socioeconomically advantaged. Most had a normal cognitive screening result (99.5%, n = 199). Overall PCQ scores were low (PCQ-Neg: [math]= 1.27, possible range 0–36; PCQ-Pos: [math] = 7.63, possible range 0–30). Both scales demonstrated floor effects. Acceptability was satisfactory, although the PCQ-Pos had slightly more item missingness. Both scales had Cronbach alphas >0.80 and a single-factor structure. Spearman correlations between the PCQ-Neg with general measures of psychological distress (Impacts of Events Scale–Revised, Perceived Stress Scale, Kessler Distress Scale) ranged from 0.26 to 0.37 (P's < 0.001); the correlation with the World Health Organization–Five Well-Being Index was −0.19 (P < 0.01). The PCQ-Neg discriminated between those with and without a self-reported subjective cognitive complaint ([math] = 2.73 v. 0.89, P < 0.001) and was associated with medical visit satisfaction (r = −0.24, P < 0.001) on the Patient Satisfaction Questionnaire. The PCQ-Pos predicted self-reported willingness to engage in future screening ([math] = 8.00 v. 3.00, P = 0.03).ConclusionsThe adapted PCQ-Neg is an overall valid measure of negative psychological consequences of cognitive screening; findings for the PCQ-Pos were more variable. Future studies should address measure performance among diverse samples and those with abnormal screening results.HighlightsThe PCQ scale is an overall valid measure of psychological dysfunction related to cognitive screening in older adults receiving normal screen results.PCQ scale performance should be further validated in diverse populations and those with abnormal cognitive screening results.The adapted PCQ may be useful to both health research and policy stakeholders seeking improved assessment of psychological impacts of cognitive screening.
