Abstract:PRESENTATION ABSTRACT The midfoot joint complex (MFJC), composed of all joints connecting the rearfoot and forefoot, plays an important role in the mechanical efficiency of the lower limb in weight-bearing activities. When the forefoot is on the ground, rearfoot eversion is accompanied by motions at the midfoot joints, which permit the metatarsal heads, as a unit, to invert relative to the rearfoot and stay horizontally supported. Thus, the soft tissues that resist this collective inversion of the midfoot joints may also resist weight-bearing rearfoot eversion. This reasoning was called “torsion spring theory” by Souza et al. (2014). Therefore, it is important to have an instrument to accurately measure the passive mechanical properties of the MFJC, and then to investigate their effects on the functionality of the lower limbs. The first step was to design, build and test the Foot Torsimeter, an instrument to measure the passive torque and angle of the forefoot motion over the rearfoot. Through these measures, other variables can be calculated, as passive stiffness. Magalhaes et al. (2020) showed good to excellent reliability with low measurement error in healthy adult individuals for the variables angle, passive torques and stiffnesses during inversion and eversion. Hence, clinicians and researchers may benefit from the reliable and stable measures provided by the Foot Torsimeter when assessing patients and planning interventions. The next step was to verify the association between these measures and clinical tests. Then, other studies were conducted to investigate whether hip passive stiffness, midfoot passive stiffness and forefoot-shank alignment are related to each other. The results showed that individuals with reduced MFJC passive stiffness also have reduced hip passive stiffness (Fajardo et al., 2021) and increased forefoot-shank alignment (Paes et al., 2019). Abstract:Background: Several tools exist to measure tightness of the gastrocnemius muscles; however, few of them are reliable enough to be used routinely in the clinic. The primary objective of this study was to evaluate the intra- and inter-rater reliability of a new equinometer. The secondary objective was to determine the load to apply on the plantar surface of the metatarsal heads to achieve the highest reliability when measuring gastrocnemius tightness. Methods: The equinometer consisted of a goniometer and an electronic dynamometer, hooked up to a computer. Three raters carried out three trials of passive dorsiflexion by applying controlled pressure to the metatarsal heads of both ankles in 29 healthy subjects under two experimental conditions: knee extended (KE) and knee flexed at 30 degrees (KF). The equinometer continuously recorded the ankle dorsiflexion values (in °) corresponding to each 1 N interval of plantar pressure between 4 N and 20 N. The intra- and inter-rater reliability of the ankle dorsiflexion were evaluated through the intra-class correlation (ICC) coefficients in each of the pressure intervals. Results: The intra-rater ICC in KE and KF was between 0.84 and 0.98. The inter-rater ICC in KE and KF was between 0.59 and 0.92. The pressure interval between 14 N and 15 N had the highest intra-rater (ICC = 1) and inter-rater reliability (0.87≤ICC≤0.99). A more refined analysis of this interval found that a load of 14.5 N yielded the best reliability. Conclusions: This compact equinometer has excellent intra-rater reliability and moderate to good inter-rater reliability. Since this reliability is optimal in the 14-15 N range, this load should be used going forward in clinical practice, especially when aiming to define a pathological threshold for tightness of the gastrocnemius muscles.
An instrumented tissue tester for measuring soft tissue property under the metatarsal heads in relation to metatarsophalangeal joint angle.

Abstract:Identification of the localized mechanical response of the plantar soft tissue pads underneath the metatarsal heads (i.e., sub-MTH pad) to external loading is key to understand and predict how it functions in a gait cycle. The mechanical response depends on various parameters, such as the external load (direction and rate), the sub-MTH tissue properties (anisotropy and viscoelasticity), and the configuration of the metatarsophalangeal (MTP) joint overlying the tissue. In this study, an instrument-driven tissue tester that incorporates a portable motorized indentor within a special foot positioning apparatus was developed for realistic in vivo mechanical characterization (i.e. tissue stiffness and force relaxation behavior) of the local sub-MTH pad with the MTP joint configured at various dorsiflexion angles associated with gait. The tester yields consistent results for tests on the 2nd sub-MTH pad. Measurement errors for the initial stiffness (for indentation depths ≤1mm), end-point stiffness, and percentage force relaxation were less than 0.084N/mm, 0.133N/mm, and 0.127%, respectively, across all test configurations. The end-point tissue stiffness, which increased by 104.2% due to a 50° MTP joint dorsiflexion, also agreed with a previous investigation. In vivo tissue's force relaxation was shown to be pronounced (avg.=8.1%), even for a short holding-time interval. The proposed technique to facilitate study of the dependence of the local sub-MTH pad and tissue response on the MTP joint angle might be preferable to methods that focus solely on measurement of tissue property because under physiologic conditions the sub-MTH pad elasticity may vary in gait, to adapt to drastically changing mechanical demands in the sub-MTH region of the terminal stance-phase, where MTP joint dorsiflexion occurs.

Abstract:Objective To evaluate isokinetic dynamometer reliability for isometric assessment of plantar flexor (PF) strength. Design Cross-sectional. Setting Testing by the same physiotherapist twice during a first session (repeatability) and once during a second session (reproducibility). Participants Twenty-two healthy subjects (44 ankles, 11 men/11 women). Main Outcome Measures Isometric PF peak torque, with and without body mass normalization, at 0° and +20° of plantar flexion. Measurement reliability was evaluated using intraclass correlation coefficient (ICC), standard error of measurement (SEM) and minimal detectable change (MDC). Results Without normalization, measurement repeatability was excellent at 0° of plantar flexion (ICC, 0.94; SEM, 6.6%; MDC, 18.4%) compared with good repeatability at +20° (ICC, 0.85; SEM, 11.1%; MDC 30.6%). Measurement repeatability following normalization was good at 0° (ICC, 0.88; SEM, 5.2%; MDC, 14.4%) and +20° (ICC, 0.79; SEM, 10.2%; MDC, 28.1%). While reproducibility was good at 0° with normalization (ICC, 0.84; SEM, 5.9%; MDC, 16.3%) or excellent without (ICC 0.92; SEM 7.5%; MDC, 20.8%), it was moderate at +20° with normalization (ICC 0.71; SEM 11.3%; MDC, 31.3%) or good without (ICC 0.78; SEM 13.0%; MDC, 36.1%). Conclusion The reliability of PF maximal isometric strength is good/excellent at 0° of plantar flexion but moderate/good at +20°.

Abstract:Background Composite finger flexion (CFF) is proposed to be a convenient alternative to total active motion (TAM) and total passive motion (TPM). Passive CFF (PCFF) may be useful for early monitoring in post-operative rehabilitation of traumatic hand injuries. Purpose To determine whether active and passive CFF are reliable, valid, and responsive measures of hand motion and of higher utility to the tester. Study Design Cross-sectional observational clinical measurement study. Methods Fifty hand injury patients were recruited from a hospital-based out-patient clinic. TAM, TPM, repeated measures of active CFF (ACFF) and PCFF, self-reported stiffness, patient reported wrist/hand evaluation (PRWHE) scores, and grip strength were recorded. Intraclass correlation coefficients (ICCs) and standard error of measurement were calculated for inter-rater and test-retest reliability. Criterion and construct validity were assessed using correlation coefficients. Responsiveness was explored by calculating correlation coefficients of change scores, effect sizes, and standardized response means. Time taken to measure CFF and TAM/TPM was recorded to consider utility. Results The average age of participants was 47 years and 36% were female. Inter-rater and test-retest reliability estimates for ACFF and PCFF were excellent (ICCs = 0.95-98). Standard error of measurement values ranged from 0.21 to 0.33. The correlation coefficient for criterion validity between ACFF and TAM was −0.69; PCFF and TPM was −0.65; and ACFF and PCFF was 0.83. For construct validity, ACFF and TAM were similarly correlated with PRWHE. Correlations between changes in stiffness with ACFF and PCFF were 0.43 and 0.26, respectively. Effect sizes of ACFF and PCFF were small at 0.1 and 0.2. Time taken to measure CFF was much shorter than TAM/TPM. Conclusions The results of this study support the use of active and passive CFF as a reliable, valid, and efficient tool in the clinical setting. Further study is required to verify the responsiveness of CFF.

Abstract:Background: Three-dimensional motion analysis using optoelectronic cameras and force platforms is typically used to quantify gait disorders. However, these systems have various limitations, particularly when assessing patients in an ecological environment. To address these limitations, several wearable devices have been developed. However, few studies have reported metrological information regarding their repeatability and sensitivity to change. Methods: A healthy adult performed 6 min walking tests with FeetMe® system insoles under different walking conditions overground and on a treadmill. The standard error of measurement (SEM), the minimum detectable differences (MDDs), and the effect size (ES) were calculated for spatio-temporal parameters, and the ground reaction force was calculated from the 16,000 steps recorded. Results: SEM values were below 3.9% for the ground reaction force and below 6.8% for spatio-temporal parameters. ES values were predominantly high, with 72.9% of cases between overground and treadmill conditions with induced asymmetry, and 64.5% of cases between treadmill conditions with and without induced asymmetry exhibiting an ES greater than 1.2. The minimum detectable differences ranged from 4.5% to 10.7% for ground reaction forces and 2.1% to 18.9% for spatio-temporal parameters. Conclusion: Our study demonstrated that the FeetMe® system is a reliable solution. The sensitivity to change showed that these instrumented insoles can effectively reflect patient asymmetry and progress.

Reliability testing of an IMU-based 2-segment foot model for clinical gait analysis

Abstract:Background: The one of the most commonly used reference system for clinical gait analysis is an optical motion capture system (OMC) using a multi-segment foot model. A time- and cost-efficient alternative could be an inertial measurement unit (IMU)-based systems. However, these are limited to a single segment approach for the foot and ankle. Therefore, the current setup was modified to be based on a 2-segment foot model, allowing for a separate analysis of the hind- and midfoot. The study aimed to evaluate the reliability (inter-rater, intra-rater, and test-retest reliability) of an IMU-based 2-segment foot model. Material and methods: Twelve healthy subjects were recruited to test the inter-rater, intra-rater, and test-retest reliability of the new IMU based 2-segment foot model. Gait analysis was performed on a treadmill at a constant speed of 4 km/h. Kinematic data of the tibia/hindfoot, tibia/forefoot and hindfoot/forefoot over 100 % gait cycle were analyzed. The reliability was tested by using statistical parametric mapping (SPM) and the intraclass correlation coefficient (ICC). Results: The SPM showed no significant difference for inter-, intra-rater, and test-retest reliability, but for a small segment of tibia/forefoot dorsiflexion test-retest reliability (2.1° difference). The single standard deviation measurement error for the sagittal and transverse plane was <5° and worse for the frontal plane. Conclusion: The new 2-segment foot model revealed a high inter-rater, intra-rater, and test-retest reliability. It is suitable for use in adult clinical practice. Still, comparative data to the OMC system using a multi-segment foot model are missing.
Validity and reproducibility of foot motion analysis using a stretch strain sensor

Abstract:Background: Multi-segment foot analysis is traditionally challenging to perform while subjects are wearing footwear or a foot orthosis and is difficult to apply in the clinical setting. A recently developed stretch strain sensor (STR), that is thin and highly flexible, may solve this limitation because it does not require observation using a camera and is highly portable. Research question: This study aimed to examine the reproducibility and validity of foot motion analysis using the STR during walking and running by comparing it with a conventional motion capture system. Methods: Twenty-one healthy participants were examined in this study. The STR was placed on the participant's foot in one of two locations in separate experiments (spring ligament; SL and navicular drop; ND methods). Foot kinematic data during walking and running were simultaneously recorded using the STR and a three-dimensional motion capture system. Intra-class correlation (ICC) was used to assess test-retest reproducibility of the STR method. Cross-correlation coefficient evaluated the similarity of the pattern of the signals between the two systems. Pearson and Spearman correlation analysis was used to evaluate the relationships between the STR measurement and angular excursion of the forefoot or hindfoot. Results: The ICCs of the SL method were 0.95 and 0.96, and those of the ND method were 0.93 and 0.71 during walking and running, respectively. In the SL method, the pattern of the signals between the STR and forefoot frontal motion was strongly correlated. The STR measurement was significantly correlated with forefoot eversion excursion (walking: r=-0.67, running: r=-0.64, p < 0.01 each). In the ND method, the STR signal was not associated with forefoot and hindfoot kinematics. Significance: Our results showed that the STR has acceptable reproducibility and validity of foot motion analysis. This system may enable measurement of foot motion while subjects are wearing shoes and outside the laboratory.

Abstract:Introduction/Aims Limitations exist in evaluating mechanical detection thresholds (MDTs) due to a lack of dependable electronic instruments designed to assess Aβ fibers and measure MDTs across different body areas. This study aims to evaluate the test–retest and inter‐rater reliability of the cutaneous mechanical stimulator (CMS), an electronic tactile stimulator, in quantifying MDTs. Methods Using a test–retest design, participants underwent assessments of MDTs using Semmes–Weinstein monofilaments (SWM) and the CMS. This study included 27 healthy volunteers (mean age 24.07 ± 3.76 years). Two raters assessed MDTs using SWM and the CMS at two stimulation sites (the left hand and foot) in two experimental sessions approximately 2 weeks apart. Results MDTs using SWM and the CMS showed excellent reliability on the hand (intraclass correlation coefficient [ICC] = .84) and foot (ICC = .90). A comparison of results obtained at the two sessions showed that MDTs on the hand displayed good reliability for both SWM (ICC = .63) and the CMS (ICC = .73), whereas MDTs on the foot displayed fair reliability for SWM (ICC = .50) and the CMS (ICC = .42). MDTs exhibited good inter‐rater reliability with SWM (ICC = .66) and excellent inter‐rater reliability with the CMS (ICC = .82) on the hand, as well as showing fair inter‐rater reliability with SWM (ICC = .53) and good inter‐rater reliability with the CMS (ICC = .60) on the foot. Discussion The CMS showed superior inter‐rater reliability, indicating its potential as a valuable tool for assessing tactile sensitivity in research and clinical settings.

Abstract:Background: The subtalar joint axis (STJA) occupies a key role in the dynamics of the lower limb kinetic chain, and its location has a wide interindividual variability. It has been suggested that considering the STJA location when designing foot orthoses may help to apply the required mechanical dose. However, the evidence is more anecdotal than empirical. Objective: This study aimed to evaluate the reliability of the STJA digitization, a procedure combining the clinical determination of the functional STJA location and its subsequent 3-dimensional (3D) scanning. Study Design: Two examiners identified the posterior and anterior exit points of the functional STJA on the skin of 15 healthy participants using a clinical method in a repeated-measure design. Methods: A handheld 3D scanner was used to scan the feet and the skin markers. The 3D coordinates of the skin markers were subsequently quantified and (1) STJA digitization intratester within-session, (2) STJA digitization intratester between-session, and (3) STJA digitization intertester between-session reliabilities were evaluated. Results: When pooling all skin marker 3D coordinates, intraclass correlation coefficients (ICCs) for the STJA intratester within-session reliability ranged from 0.74 to 0.98. ICCs for the STJA digitization intratester between-session reliability ranged from 0.58 to 0.94. ICCs for the STJA digitization intertester reliability ranged from 0.56 to 0.81. Standard error of measurement for the mediolateral position of the talus marker (anterior exit point of the STJA) was substantially higher than that for the other coordinates. Conclusions: Overall, the STJA digitization demonstrated a good intratester between-session reliability and may be used in a computer-aided design and computer-aided manufacturing workflow to create foot orthoses. However, further efforts should be considered to improve the scanning process and intertester reliability.

Abstract:Abstract: Introduction: The lack of knowledge of mechanical loads in orthotic joints can lead to oversized or breaking components. Previous studies suffer from small sample size and technical limitations. The goal of this study was to develop and validate a method that allows the direct measurement of moments in sagittal, frontal and transverse planes in knee and ankle joints of existing custom made orthoses. Methods: We developed a modular measurement system based on standard joint components, which were instrumented with strain gauges. To ensure sufficient signals and reduce cross talk an iterative approach based on FEM simulation was utilized. The system also contains inertial sensors for mobile gait analysis. Results: Instrumented joints show good results regarding linearity, hysteresis and cross talk. First pilot trials with post-polio and ICP patients demonstrated that joint loads depend on several factors and not solely on body weight. If combined with conventional gait analysis, measurement results can characterize the individual muscle situation of the patient. Conclusion: A novel method for obtaining data on loads in orthotic components was developed and validated. It provides the basis to develop safety testing standards and clinical guidelines, as well as allowing individual optimization of orthotic devices.

Abstract:Abstract Background Ankle sprain is the most common lower limb injury in physically active individuals. Loss of function, decreased postural control (PC), strength deficit, and reduced range of motion (ROM) are common after acute lateral ankle sprains. Some patients experienced long lasting symptoms, with recurrent sprains, and episodes of giving-way: a condition known as chronic ankle instability (CAI). Evaluating the function in patients with CAI in the clinical environment is important to identify the severity of the condition, in addition to allowing to assess the effectiveness of a given treatment. The aim of this study was to investigate the validation of the Foot and Ankle Ability Measure (FAAM) and the Foot and Ankle Outcome Score (FAOS) in terms of muscle strength, PC and ROM in adults with CAI. Methods This is a cross-sectional study. Individuals with CAI aged between 18 and 45 years were eligible. Individuals with CAI were identified using the Identification of Functional Ankle Instability (IdFAI). All patients filled in the FAAM and FAOS scores. Muscle strength was assessed by manual dynamometry, ROM by the Lunge test, PC by computerized posturography, modified Star Excursion Balance Test (mSEBT) and modified Balance Error Score System (mBESS). Results 50 participants were enrolled in the present study. The mean age of the patients was 27.2 ± 6.3 years, and the mean body mass index was 26.4 ± 4.8 kg/m 2 . 58% (29 of 50) were men and 42% (21 of 50) women. 18 individuals had unilateral (36%) and 32 bilateral (64%) CAI. The results of FAAM were associated with MCT, mSEBT, invertor muscles strength, plantar flexor muscles strength, dorsiflexor muscles strength, and external hip rotator muscles strength ( P < 0.05). The results of FAOS were associated with mSEBT, invertor muscles strength, plantar flexor muscles strength, dorsiflexor muscles strength, evertor muscles strength, and external hip rotator muscles strength, and mBEES ( P < 0.05). Conclusion Both the FAAM and FAOS demonstrated validity to evaluate postural control and muscle strength in patients with CAI, while no association was found in relation to ankle dorsiflexion.

Reliability of a portable device for quantifying tone and stiffness of quadriceps femoris and patellar tendon at different knee flexion angles

Abstract:The reliability of MyotonPRO that can monitor the mechanical properties of tissues is still unclear. This study aimed to analyze the within-day inter-operator and between-day intra-operator reliability of MyotonPRO for assessing tone and stiffness of quadriceps femoris and patellar tendon at different knee angles. The tone and stiffness of healthy participants (15 males and 15 females, aged 24.7±1.6 years) in the supine and resting position were measured using the MyotonPRO device. The measurements were quantified at 0°, 30°, 60°, and 90° of knee flexion. The intraclass correlation coefficient (ICC), standard error of measurement (SEM), and minimal detectable change (MDC) were calculated and a Bland–Altman analysis was conducted to estimate reliability. The results indicated excellent inter-operator reliability (ICC > 0.78) and good to excellent intra-operator reliability (ICC > 0.41). The inter-operator SEM measurements ranged between 0.1–0.9 Hz and 3.8–37.9 N/m, and intra-operator SEM ranged between 0.5–1.3 Hz and 7.9–52.0 N/m. The inter-operator MDC ranged between 0.3–2.5 Hz and 10.5–105.1 N/m, and intra-operator SEM ranged between 1.1–3.3 Hz and 21.9–144.1 N/m. The agreement of inter-operator was better than that of intra-operator. The study concluded that MyotonPRO is a reliable device to detect the tone and stiffness of quadriceps femoris and patellar tendon.

Abstract:Objective: This study aimed: (i) to investigate the impact of fatigue, triggered by maximal isometric contraction exercises, on the active and passive stiffness of plantar flexors (PF), and (ii) to examine the relationship between changes in mechanical parameters and neuromuscular alterations after fatigue. Methods: A healthy cohort (n = 12; age = 27.3 ± 5.5 y; BMI = 24.4 ± 2.35 kg/m²) was instructed to perform 60 isometric contractions, each lasting 4 s with a 1-s rest interval, using an ergometer. Several measures were taken before and after the fatigue protocol. First, the stiffness of the PF-tendon complex (PFC) was quantified during passive ankle mobilization both during and after the fatigue protocol using the ergometer. Additionally, from shear wave elastography, the active and passive stiffness of the gastrocnemius medialis (GM) were measured during passive ankle mobilization and isometric maximal voluntary contraction (MVC), respectively. Finally, the peak torque and the rate of torque development (RFD) of PF were assessed during the MVC using the ergometer. Ankle muscle activities (surface electromyograph [SEMG]) were recorded during all evaluations using electromyography. Results: After the fatigue protocol, the results revealed a decline in active stiffness, peak torque of PF, RFD and SEMG activity of the GM (p < 0.001). Furthermore, significant correlation was identified between the decrease of the peak torque of PF and the active stiffness of the GM (r = 0.6; p < 0.05). A decrease in the PFC stiffness (p < 0.001) and a decrease in the shear modulus of the GM at 20° (p < 0.001) were also observed. Conclusion: Isometric fatiguing exercises modify the mechanical properties of both the contractile and elastic components. Notably, decreases in both passive and active stiffness may be critical for athletes, as these changes could potentially increase the risk of injury.

Abstract:Abstract Systemic and uncontrolled administration of erlotinib hydrochloride (ETB) is associated with severe toxicity. A novel targeted and extended release nanosponge (NS) was synthesized from glutathione (GHS) by a one-step reaction between β-cyclodextrin and pyromellitic dianhydride at room temperature for delivery of ETB in lung cancer. Characterization studies were performed using sophisticated instruments. In-vitro release study was performed in the presence of incremental concentrations of GHS which was analyzed using HPLC. Cell cytotoxicity study was evaluated on human lung cancer (A549) cell lines. In-vivo tumour inhibition and biodistribution of ETB-loaded GHS-NS (ETB-NS) were performed on BALB/c mice. NS obtained was spherical, size 212 ± 2.45 nm and high drug entrapment (92.34 ± 5.31%) (p < .001). In-vitro extended drug release (76.89 ± 0.1% release at 168 h), which was directly proportional to the concentration of GHS, demonstrated tumour targeting. There was enhanced in-vitro cytotoxicity and 97.5% inhibition in tumour growth on administering NS when compared to plain ETB (48% inhibition) indicating targeting of NS to the tumour site. Biodistribution study and in-vivo tumour growth inhibition study revealed drug release to the cancerous cell, thus preventing unnecessary drug exposure. ETB-NS exhibits extended drug release proportional to the external GSH concentration.

Abstract:BACKGROUND: Passive stiffness describes how easily a joint may move passively. To accurately measure wrist stiffness, an electro-oscillation device was developed. The objectives were to 1) ensuring that the measurement are free from intentional or reflex contraction, 2) analyzing how forearm anatomy affects the passive stiffness of the wrist and 3) determining the clinical practical relevance of the device.METHODS: In this prospective study, the device generated low amplitude sinusoidal motions in flexion and extension on the wrist to quantify elastic and viscous passive stiffness in voluntary orthopaedic patients. The first series of measurements was carried out in the state of voluntary relaxation, the second series of measurements was carried out after an axillary plexus anesthetic block. A matched group of healthy subjects were use for control.FINDINGS: The Electromechanical Oscillation methods effectively enable the measurement of passive joint stiffness since the stiffness values obtained show no statistically significant difference pre-post the anesthesia. The stiffness values are comparable to those of healthy subjects. The effect of forearm passive structure, estimated by the perimeter of the forearm, influences the passive stiffness of the wrist, mainly the viscous component.INTERPRETATION: The use of sinusoidal oscillation was well accepted by the participants, demonstrating its usefulness and applicability in a clinical setting. This work serves as a foundation for future investigations of orthopaedic and/or neurological pathological conditions characterized by abnormal passive joint stiffness of the wrist. It paves the way for its use as a diagnostic, prognostic, and monitoring tool in these pathologies.

Abstract:Background The ankle dorsiflexion range of motion (ADF-ROM) during single support phase allows elastic energy storage in the calcaneal tendon, contributing to advance the body forward. Reduced ADF-ROM may influence lower limb kinetics and stiffness. Research question What is the influence of reduced passive ADF-ROM on lower limb internal moments and stiffness during gait? Methods Thirty-two participants, classified into two groups according to passive ADF-ROM (smaller than 10° and greater than 15°), were submitted to gait assessment at self-selected speed with a force platform and a three-dimensional motion analysis system. Statistical parametrical mapping (SPM) analyses were used to compare the lower limbs' internal moments between groups. Independent t-tests analyzed the differences between groups on lower limb stiffness during gait. Results The lower ADF-ROM group had greater knee flexor moment (terminal stance and push-off), greater ankle abductor (i.e., shank internal rotator) moment in terminal stance and greater knee internal rotator moment in mid to terminal stance. The lower ADF-ROM group also had higher lower limb stiffness during gait. Significance Individuals with reduced passive ADF-ROM had greater lower limb stiffness and adopted a gait pattern with increased knee and ankle moments, suggesting increased loading at these joints.

Abstract:Background. Clinicians employ foot morphology assessment to evaluate the functionality of the method and anticipate possible injuries. This study aims to correlate static foot posture and the dynamic barefoot evaluation in a sample of healthy adult participants. Methods. The foot posture was evaluated using the Foot Posture Index-6 (FPI-6) and the dynamics were evaluated through baropodometric examination. Two operators independently assessed the participants' foot posture through FPI-6, and then a dynamic evaluation was performed by asking them to walk 8 times across a platform. One hundred participants (mean age: 32.15 ± 7.49) were enrolled. Results. The inter-rater agreement between the two assessors was found to be excellent. The majority of the feet belonged to the 0 < FPI < 4 class (32%), followed by the 4 < FPI 8 ranges (19.5%). Our "area of contact" analysis showed a significant poor correlation between FPI and total foot, midfoot, and the second metatarsophalangeal joint (MTPJ) (−0.3 < r < 0). Regarding "force" parameters, the analysis showed a poor correlation between the midfoot, hallux, and the second toe (−0.2 < r < 2); finally the "pressure" analysis showed a poor correlation between FPI, the fourth MTPJ, and the second toe (−0.2 < rs < 0.3) and a moderate correlation between the hallux (r = 0.374) and the fifth MTPJ (r = 0.427). Conclusions. This study emphasizes the constrained correlation between static foot posture observation and dynamic barefoot examination.

Abstract:Individuals with chronic ankle instability (CAI) exhibit a variety of sensorimotor deficits which contribute to the long-term health risks and lower overall health-related quality of life. Recent literature finds abnormal cutaneous reflex characteristics and perceptions of instability during gait among those with CAI. These may serve as important patient-specific outcome measures in diagnosing and monitoring the condition, however, the test–retest reliability of these measurements is still undetermined. Therefore, the purpose of this study is to assess reliability of cutaneous reflex amplitudes and variability and perceived instability following perturbation in those with CAI and healthy controls during gait. Subjects walked on a treadmill while receiving random, non-noxious sural nerve stimulations throughout the stance phases of gait. Muscle activity was measured via electromyography for the peroneus longus, and medial and lateral gastrocnemius. Subjects reported their perceived instability following each stimulation. Cutaneous reflex amplitudes and perceived instability throughout the stance phases of gait can be reliably measured within the same day and over a 1-week period while reflex variability may be a less reliable measure. Our findings support the use of these variables as clinical outcome measures to identify and monitor neuromuscular recovery.
