Abstract:During the past 70 years, patient safety science has evolved through four organizational frameworks known as Safety-0, Safety -1, Safety-2, and Safety-3. Their evolution reflects the realization over time that blaming people, chasing errors, fixing one-offs, and regulation would not create the desired patient safety. In Safety-0, the oldest framework, harm events arise from clinician failure; event prevention relies on better staffing, education, and basic standards. In Safety-1, used by hospitals, harm events arise from individual and/or system failures. Safety is improved through analytics, workplace culture, high reliability principles, technology, and quality improvement. Safety-2 emphasizes clinicians' adaptability to prevent harm events in an everchanging environment, using resilience engineering principles. Safety-3, used by aviation, adds system design and control elements to Safety-1 and Safety-2, deploying human factors, design-thinking, and operational control or feedback to prevent and respond to harm events. Safety-3 represents a potential way for anesthesia and perioperative care to become safer.
Exploring a model nuclear planning and response program: Evaluating public awareness of written risk and emergency

Abstract:High-reliability organizations (HROs) including commercial airlines, the NASA Space Shuttle engineering team, US Naval aircraft carrier and nuclear submarine crews, and US nuclear power plants are relatively safe. However, these organizations experience system breakdowns often with catastrophic outcomes. This study focuses on risk information management strategies employed by a nuclear energy power plant located within 10 miles of a population center. The evacuation planning zone (EPZ) includes a hospital, several schools, and a public university. The nuclear plant provides written preparedness and evacuation information for all residents within the EPZ in the event of a radiological emergency. Focusing on the campus community within the EPZ, this study investigates individual awareness regarding the potential of a radiological event, the emergency information booklet, and the information provided within the booklet. We use descriptive statistics, frequency distribution, and cross tabulations (contingency tables) to establish awareness levels. Our study determines those participants who read the emergency instructions booklet are prepared to make an informed decision in the event of a radiological incident. We find college affiliation, educational level, university role, and age significantly related to emergency instruction booklet utilization. We also find gender is significantly linked to overall risk perception regarding a radiological event. Findings support previous research regarding women's higher levels of risk aversion and pessimism involving dangerous new risky technologies and activities like nuclear energy plants. These findings support modifying policy to ensure nuclear facilities assess the efficacy of their warning systems in alerting the public. Furthermore, our findings provide guidance regarding the evaluation of the effectiveness of emergency instruction booklet distribution.

Abstract:Medical diagnostics has been revolutionized with the discovery of X-rays. The number of radiological procedures performed globally has increased rapidly during past decades and according to UNSCEAR 2021 report, 4.2 billion medical radiological procedures are performed annually. Ionizing radiations if not used appropriately and carefully can lead to health hazards for health professionals involved in the radiation work as well as patients undergoing the radiological procedure. However, the awareness about the ionising radiation and radiation safety is very limited among the healthcare professionals. This educational paper will provide basic insights in radiation protection training, and key topics to cover during education and training for nurses working in various medical radiological procedures of patients. Nurses need to understand radiation protection principles and ways to minimize their exposure of radiation during radiological procedures. They should be able to practically apply radiation protection principles in healthcare settings with enhanced patient and occupational safety without Radiophobia. Applying radiation safety culture in nursing practice improves quality and safety in patient care.

Abstract:As the decision‐making basis for "safety" in risk management and risk assessment activities, acceptable risk has always been an important topic of risk‐related research. Based on the records in the Science Citation Index Expanded database and Social Sciences Citation Index database via the Web of Science Core Collection, 1124 articles or reviews related to acceptable risk in engineering and operations research and management science were retrieved. These documents, published between 1961 and 2021, covered 3056 authors, 75 countries/territories, 1296 institutions, and 323 journals. In this study, bibliometric data such as annual growth trends were analyzed by using descriptive statistics. Through the co‐occurrence maps created by CiteSpace and Gephi, the most productive and influential countries/territories, institutions, and authors as well as their cooperation networks were identified. Further analysis was conducted to determine the core publications and publication sources in this field through co‐citation analysis. Insights into focus areas and research topics over time were obtained through keyword co‐occurrence analysis. This study provides a macroscopic overview of acceptable risk research and may help researchers better understand this research field and predict its dynamic directions.

Abstract:The increasing complexity and power of our technologies compels us to find new ways in which to conceptualise, understand and maintain their safety in the long term. Some complex technological industries have performed better than others in terms of applying sustained and systematic approaches to the maintenance of safety. The United States nuclear power industry can be seen as an ideal test-bed for the development of safety initiatives, being responsible for the control of potentially unpredictable technology that involves extraordinary forces and costs. This paper describes and formalises a framework for better understanding the safety of complex socio-technological systems, based on key events in the development of safety in the United States nuclear power industry. The framework comprises two components: (1) a state-space approach for better conceptualising system failures, the benefits of incident reporting and remedial safety initiatives; and (2) a set of milestones that can be used to assess the development of safety in socio-technological industries. Healthcare and the United States nuclear power industry both represent complex socio-technological systems with similar technical characteristics. However, safety strategies in healthcare have not kept pace with the increasing complexity of clinical practice, and there have been international calls for improvements in patient safety. The framework is applied to the analysis of safety in healthcare, demonstrating its utility as an alternative safety analogy in healthcare. Use of the framework indicates substantial scope for improvements in healthcare safety through major evidence-based system redesign. By lowering the threshold for the reporting of incident data to include accident precursors, it is possible to identify problem areas before patient harm occurs.
Signal subspace approach for speech enhancement in nonstationary noises

Abstract:In this paper, we propose a speech enhancement algorithm that is based on signal subspace approach (SSA) combined with RL noise estimation for nonstationary noises. A signal/noise Karhunen-Loeve transform (SNK) is used to implement signal subspace decomposition for noisy speech signal. RL noise estimation is used for adaptive estimates over time for nonstationary noises. Since the noise covariance matrix for SSA can be recursively updated by RL noise estimation, it results that the proposed algorithm can efficiently suppress the noise while reducing speech distortion. Objective experimental measurements indicated that the proposed algorithm was preferred over other SSA methods when tested with sentences corrupted by nonstationary noises.

Abstract:Although debate continues over estimates of the amount of preventable medical harm that occurs in health care, there seems to be a consensus that health care is not as safe and reliable as it might be. It is often assumed that copying and adapting the success stories of nonmedical industries, such as civil aviation and nuclear power, will make medicine as safe as these industries. However, the solution is not that simple. This article explains why a benchmarking approach to safety in high-risk industries is needed to help translate lessons so that they are usable and long lasting in health care. The most important difference among industries lies not so much in the pertinent safety toolkit, which is similar for most industries, but in an industry's willingness to abandon historical and cultural precedents and beliefs that are linked to performance and autonomy, in a constant drive toward a culture of safety. Five successive systemic barriers currently prevent health care from becoming an ultrasafe industrial system: the need to limit the discretion of workers, the need to reduce worker autonomy, the need to make the transition from a craftsmanship mindset to that of equivalent actors, the need for system-level (senior leadership) arbitration to optimize safety strategies, and the need for simplification. Finally, health care must overcome 3 unique problems: a wide range of risk among medical specialties, difficulty in defining medical error, and various structural constraints (such as public demand, teaching role, and chronic shortage of staff). Without such a framework to guide development, ongoing efforts to improve safety by adopting the safety strategies of other industries may yield reduced dividends. Rapid progress is possible only if the health care industry is willing to address these structural constraints needed to overcome the 5 barriers to ultrasafe performance.

Abstract:Molecular dynamics simulations are carried out to investigate the dynamic behavior of the slip length in thin polymer films confined between atomically smooth thermal surfaces. For weak wall-fluid interactions, the shear rate dependence of the slip length acquires a distinct local minimum followed by a rapid growth at higher shear rates. With increasing fluid density, the position of the local minimum is shifted to lower shear rates. We found that the ratio of the shear viscosity to the slip length, which defines the friction coefficient at the liquid/solid interface, undergoes a transition from a nearly constant value to power law decay as a function of the slip velocity. In a wide range of shear rates and fluid densities, the friction coefficient is determined by the product of the value of the surface-induced peak in the structure factor and the contact density of the first fluid layer near the solid wall.

Abstract:The increasing complexity of radiation therapy treatment presents new potentials for error and suboptimal care. High-performing programs thus not only require adherence to, but also ongoing improvement of, key safety and quality practices. In this article, we review these practices including standardization, risk analysis, peer review, and maintenance of strong safety culture, while also describing recent innovations and promising future directions. We specifically highlight the growing role of artificial intelligence in radiation oncology, both as a tool to deliver safe, high-quality care and as a potential new source of safety challenges.

Abstract:Healthcare is transitioning into a new era-an era of accountability. This era demands heightened awareness of the quality, cost, and safety of healthcare, with value (quality/cost) and safety being the watchwords of accountability. Many factors are driving this transition, and it is affecting all healthcare disciplines, including radiation oncology. The transition is accompanied by the transformation of healthcare from a craft-based culture to an information-age culture in which patient needs and information are given top priority. These changes call for new measures to quantify and document the value and safety of procedures in radiation oncology.

Abstract:This paper discusses four topics relating to safety culture, three theoretical and one practical. The first considers why it is that an unsafe culture is more likely to be involved in the causation of organizational rather than individual accidents. It is the pervasive nature of culture that makes it uniquely suitable for creating and sustaining the co-linear gaps in defences-in-depth through which an accident trajectory has to pass. The second topic relates to pathological adaptations, and discusses two examples: the Royal Navy of the mid-nineteenth century and the Chernobyl reactor complex. The third issue deals with recurrent accident patterns and considers the role of cultural drivers in creating typical accidents. The final topic is concerned with the practical question of whether a safety culture can be engineered. It is argued that a safe culture is an informed culture and this, in turn, depends upon creating an effective reporting culture that is underpinned by a just culture in which the line between acceptable and unacceptable behaviour is clearly drawn and understood.

Abstract:Safety climate and safety culture are important research domains in risk and safety science, and various industry and service sectors show significant interest in, and commitment to, applying its concepts, theories, and methods to enhance organizational safety performance. Despite the large body of literature on these topics, there are disagreements about the scope and focus of these concepts, and there is a lack of systematic understanding of their development patterns and the knowledge domains on which these are built. This article presents a comparative analysis of the literature focusing on safety climate and safety culture, using various scientometric analysis approaches and tools. General development patterns are identified, including the publication trends, in terms of temporal and geographical activity, the science domains in which safety culture and safety climate research occurs, and the scientific domains and articles that have primarily influenced their respective development. It is found that the safety culture and safety climate domains show strong similarities, e.g., in dominant application domains and frequently occurring terms. However, safety culture research attracts comparatively more attention from other scientific domains, and the research domains rely on partially different knowledge bases. In particular, while measurement plays a role in both domains, the results suggest that safety climate research focuses comparatively more on the development and validation of questionnaires and surveys in particular organizational contexts, whereas safety culture research appears to relate these measurements to wider organizational features and management mechanisms. Finally, various directions for future research are identified based on the obtained results.

Abstract:Background: This study aims to analyze radiation safety management and regulatory perceptions, focusing on companies that must report radiation sources. The intent is to reduce the gap between regulation measures and addressing real concerns while improving practical safety management measures and regulations for all stakeholders.Materials and Methods: Radiation safety officers at a total of 244 reporting companies using radiation generators (79.8%) and sealed radioisotopes (15.1%) were surveyed using a questionnaire.Results and Discussion: The perception that regulation is stronger than the actual risk of the radiation source used was 3.47 points (out of 5 points), indicating a score above average. The most important factors and considerations were education and training (48%) as a human factor, safety devices of the radiation source (71.3%) as a hazardous material factor, the use of radiation (50.8%) as an organizational environment, and the radiation effect of nearby facilities (67.2%) as a physical environment. Radiation safety management educational experience (F=5.030, p<0.01), the group with high subjective knowledge (t=6.017, p<0.001), and the group with high objective knowledge (t=1.989, p<0.05) was found to be better at radiation safety management.Conclusion: It is necessary to standardize the educational experience regarding radiation safety management because each staff member has individual differences in educational experience. It is necessary to provide more information on how to solve radiation accidents via educational content. Applying radiation safety regulations based on the factors that significantly affect radiation safety management shown in this survey will help improve safety.

Abstract:As advanced technology develops, it has been demonstrated by many accidents in the past that software (human) has a greater influence on safety than does hardware (equipment). Therefore, the software (human) of an organization should be focused on in order to enhance the safety in complex system industries. Safety culture plays an important role in the software of such organizations. Attempts to measure the worker safety culture awareness and to capture signs of their degradation are very difficult and highly uncertain. However, safety failures in the nuclear industry can lead to irreparable performance failures. Therefore, the industry has been continuing its efforts to measure the worker safety culture awareness and to improve weaknesses in organizations. It was confirmed in several studies that the probability of an accident is high at an organization with a low level of worker safety culture awareness (Vinodkumar and Bhasi, 2009; Morrow et al., 2014; Shirali et al., 2016). The previous paper suggested the method to quantitatively assess safety level of nuclear power plants. In this paper, the purpose is perform case studies in order to apply the suggested method to real nuclear power plant accidents. Weak safety culture traits through case studies assuming accident situation were analyzed and how the barriers of a nuclear power plant became vulnerable and how they contributed to the accident was identified. If this method proposed is applied to the real accident situation and extends to minor accidents at a nuclear power plant, workers can learn and understand how to behave in an accident situation and severe accidents can be proactively prevented. In addition to, the high frequency of weak safety culture traits analyzed, as a lagging indicator, could show which areas of a nuclear power plant are vulnerable, and their improvement will contribute to strengthen the safety of the nuclear power plant.

Abstract:Patient safety is currently a central issue in health care. Many principles of patient safety, such as a safety management system, have been copied from high-risk industries. However, without a fundamental understanding of the differences between health care and industry, most incentives and instruments will translate into bureaucracy, control and repression. The necessary risk reduction for the patient can only be achieved through changes in the culture and hierarchical structure within the health care system. This requires breaking through professional and departmental barriers and reshaping the traditional hierarchy.

Abstract:In this century society faces increasingly large-scale accidents and risks emerging from our own wondrous technologies. Two prominent organizational approaches to safety, Normal Accident Theory and High Reliability Organizations, have focused attention on a variety of industries that deal with hazardous situations, developed concepts to explicate organizational structure and culture, and debated whether accidents are inevitable in complex systems. We outline these approaches and identify some limitations, including narrow definitions, ambiguity about key concepts, confusion of reliability and safety, and overly pessimistic or optimistic conclusions. We believe that the debate between NAT and HRO can become a more productive three-way conversation by including a systems approach to safety emerging from engineering disciplines. The more comprehensive systems approach clarifies the strengths and weaknesses of NAT and HRO and offers a more powerful repertoire of analytic tools and intervention strategies to manage and control post modern risk in complex, high-tech, systems with their potential for catastrophic disruptions and losses.

Abstract:Many high-hazard industries around the world have explicitly recognized the critical role that human, management and organizational risk factors play in major accidents. The findings of accident investigations and risk assessments demonstrate a growing recognition that the cultural context of work practices may influence safety just as much as technology.The objective of this paper is to establish a relationship between the concepts of safety culture and organizational culture in a Nuclear Power Plant (NPP). This study permits the identification and quantification of the possible mechanisms for improving the safety culture in the NPP acting on organizational culture. It therefore provides a methodology to identify potential strategies for safety improvement.Probabilistic (Bayesian) Networks (BNs) have been used to determine the relationships between the organizational culture and safety culture in a quantitative form. To this aim, we considered data from a survey conducted of every employee at a Spanish NPP. The resulting data-driven models allow us to establish the probabilistic relationship among organizational culture factors, including the 12 OCI (Organizational Culture Inventory) scales, that have an influence on safety culture. The study yielded a ranking of organizational cultures that can be used to improve safety culture in a NPP.

Abstract:This paper describes an integrated effort to define and measure organizational factors related to nuclear power plant safety. The research began by reviewing previously conducted studies looking at nuclear power operations and operations in other high reliability industries for indications of common safety-related, performance dimensions. Having established a list of 20 common dimensions, the project went on to fully define these dimensions and develop methods for their assessment. The methods of assessment developed for this application were employee survey—a series of self report questions answered by employees, behavioral checklist—sets of statements about the plant and its operations that observers respond to by answering yes or no, structured interview—a set of questions and interviewer asks of an employee and built around the 20 dimensions identified, and behaviorally anchored rating scales—an extension of the methodology used for assessing the performance of individuals to the process of assessing a nuclear power plant. Each of the methodologies is described as it applies to the assessment within the nuclear power environment and examples of each method are presented. Pilot tests of the feasibility of using these assessment methods were conducted in two nuclear power plants and the results are encouraging both in terms of the immediate identification of potential safety issues and as valuable additions to probabilistic risk assessment. Implications of this work for the future assessment of organizational factors related to nuclear power safety are discussed.
