Abstract:High-reliability organizations (HROs) including commercial airlines, the NASA Space Shuttle engineering team, US Naval aircraft carrier and nuclear submarine crews, and US nuclear power plants are relatively safe. However, these organizations experience system breakdowns often with catastrophic outcomes. This study focuses on risk information management strategies employed by a nuclear energy power plant located within 10 miles of a population center. The evacuation planning zone (EPZ) includes a hospital, several schools, and a public university. The nuclear plant provides written preparedness and evacuation information for all residents within the EPZ in the event of a radiological emergency. Focusing on the campus community within the EPZ, this study investigates individual awareness regarding the potential of a radiological event, the emergency information booklet, and the information provided within the booklet. We use descriptive statistics, frequency distribution, and cross tabulations (contingency tables) to establish awareness levels. Our study determines those participants who read the emergency instructions booklet are prepared to make an informed decision in the event of a radiological incident. We find college affiliation, educational level, university role, and age significantly related to emergency instruction booklet utilization. We also find gender is significantly linked to overall risk perception regarding a radiological event. Findings support previous research regarding women's higher levels of risk aversion and pessimism involving dangerous new risky technologies and activities like nuclear energy plants. These findings support modifying policy to ensure nuclear facilities assess the efficacy of their warning systems in alerting the public. Furthermore, our findings provide guidance regarding the evaluation of the effectiveness of emergency instruction booklet distribution.
A Risk Management Perspective on Climate Change: Lessons Learned from the Nuclear Industry

Abstract:Despite the widely acknowledged role of the anthropogenic drivers of climate change, there has been little success in developing a clear overview of the strengths and weaknesses of counter-measures or developing a consensus on their application. Problems with conventional approaches arise from the strongly coupled, multidisciplinary issues involved and the long timescales (centuries or more) over which some key processes operate. Here we outline an alternative approach based on experience gained in risk assessment for an area with similar challenges-the geological disposal of radioactive waste. Utilization of such risk assessment approaches and tools to facilitate a holistic, top-down synthesis of the interactions between the key features, events and processes driving climate change and constraining responses to it, are illustrated. We especially focus on visual presentations that encourage dialog between both specialists and non-technical stakeholders. These can thus form a basis to assist balancing responses in terms of energy policy, modified socio-economic boundary conditions and environmental management.
Safety in unpredictable complex systems – a framework for the analysis of safety derived from the nuclear power industry

Abstract:The increasing complexity and power of our technologies compels us to find new ways in which to conceptualise, understand and maintain their safety in the long term. Some complex technological industries have performed better than others in terms of applying sustained and systematic approaches to the maintenance of safety. The United States nuclear power industry can be seen as an ideal test-bed for the development of safety initiatives, being responsible for the control of potentially unpredictable technology that involves extraordinary forces and costs. This paper describes and formalises a framework for better understanding the safety of complex socio-technological systems, based on key events in the development of safety in the United States nuclear power industry. The framework comprises two components: (1) a state-space approach for better conceptualising system failures, the benefits of incident reporting and remedial safety initiatives; and (2) a set of milestones that can be used to assess the development of safety in socio-technological industries. Healthcare and the United States nuclear power industry both represent complex socio-technological systems with similar technical characteristics. However, safety strategies in healthcare have not kept pace with the increasing complexity of clinical practice, and there have been international calls for improvements in patient safety. The framework is applied to the analysis of safety in healthcare, demonstrating its utility as an alternative safety analogy in healthcare. Use of the framework indicates substantial scope for improvements in healthcare safety through major evidence-based system redesign. By lowering the threshold for the reporting of incident data to include accident precursors, it is possible to identify problem areas before patient harm occurs.

Abstract:This study aims to present a more balanced perspective of risk acceptability through integrating economic individualism and social constructivism and examining the impact of trust in the national and local government on citizens' attitudes toward risk and nuclear power plants, respectively. The data were gathered from surveys of local residents engaged in fierce debates on whether they agreed to accept a nuclear power plant in their local area, the city of Samcheok in South Korea. To capture the causal links among the determinants of risk acceptability of a nuclear power plant and their relationships, the authors utilized a structural equation model. Demonstrating that a shared worry about nuclear stigmatization has both a positive impact on risk perception but also a negative impact on trust in local government, our findings emphasize that risk studies of nuclear energy should pay attention to the social, cultural, and historical contexts of nuclear energy as well as its economic aspects when examining the determinants of risk acceptability. This study also enriches our understanding that national and local governments can play different roles in promoting citizens' risk acceptability of a nuclear power plant. Thus, this study contributes towards presenting a better specified causal model of risk acceptability and offers practical implications of how to promote the public's risk acceptability of nuclear power facilities.

Abstract:The process to arrive at the radiation protection practices of today to protect workers, patients, and the public, including sensitive populations, has been a long and deliberative one. This paper presents an overview of the US Environmental Protection Agency's (US EPA) responsibility in protecting human health and the environment from unnecessary exposure to radiation. The origins of this responsibility can be traced back to early efforts, a century ago, to protect workers from x rays and radium. The system of radiation protection we employ today is robust and informed by the latest scientific consensus. It has helped reduce or eliminate unnecessary exposures to workers, patients, and the public while enabling the safe and beneficial uses of radiation and radioactive material in diverse areas such as energy, medicine, research, and space exploration. Periodic reviews and analyses of research on health effects of radiation by scientific bodies such as the National Academy of Sciences, National Council on Radiation Protection and Measurements, United Nations Scientific Committee on the Effects of Atomic Radiation, and the International Commission on Radiological Protection continue to inform radiation protection practices while new scientific information is gathered. As a public health agency, US EPA is keenly interested in research findings that can better elucidate the effects of exposure to low doses and low dose rates of radiation as applicable to protection of diverse populations from various sources of exposure. Professional organizations such as the Health Physics Society can provide radiation protection practitioners with continuing education programs on the state of the science and describe the key underpinnings of the system of radiological protection. Such efforts will help equip and prepare radiation protection professionals to more effectively communicate radiation health information with their stakeholders.

Abstract:This study sheds new light on nuclear risk governance from a sociological perspective by analyzing cases of post-Fukushima controversies on nuclear safety and nuclear emergency preparedness in Japan. By critically analyzing how the three risk-related concepts and methodologies, namely, probabilistic risk assessment, safety goals, and the System for Prediction of Environmental Emergency Dose Information, have been interpreted, implemented, and/or abandoned before and after the Fukushima accident, this study identifies three common features that characterize Japan's nuclear risk governance: avoiding critical conflicts, proclivity toward automated decision making, and strategic overlooking of "uncomfortable knowledge." These features all involve ignorance of the dynamic nature of safety where addressing uncertainties, heterogeneous knowledge, and incommensurable values can be key for continuously reviewing the existing edifice of safety. By elucidating why such ignorance persists in Japan despite the post-accidental drastic reform, the authors both articulate the deep-rooted structure that underlies it and reflects the societal and historical context, and eventually conceptualize this ignorance as "structural ignorance" of expertise in nuclear safety controversies and policy processes. The results also provide direction for further research to solve this structural problem.

Abstract:People's perceptions concerning radiation inform decision making in relation to nuclear power production. There have been extensive studies of people's perceptions of radiation including in relation to the Fukushima and Chernobyl nuclear events. This paper reports on a social science literature review study and is concerned with public responses to low dose radiation (LDR) and nuclear facilities, particularly in the marine environment. This review confirms that people with little knowledge about radiation have the highest risk perceptions in relation to nuclear power; conversely people with a broader knowledge have lower risk perceptions and are more favourable to nuclear power. Education can increase positive perceptions, but the influence of underlying experience of feeling, emotion, attachment, or mood may dampen this effect. This paper reports studies which document anxiety within the Japanese community in relation to the Fukushima nuclear accident. Literature concerning specific radionuclides released is described. While there are uncertainties about the presence of health impacts following LDR exposures, public attitudes are not in line. There is a need for better dialogue between nuclear professionals and scientists to achieve better nuclear education and public communication outcomes; more specific communication surrounding multi-isotopes, and multi-elements of nuclear power plant disaster releases is warranted. Trusted communicators should include nuclear and social scientists as well as regulators. Given the importance of decarbonization in the context of climate change, this is increasingly important.

DOE safety goals comparison using NUREG-1150 PRA (probabilistic risk assessment) methodology

Abstract:A full-scope Level 3 probabilistic risk assessment (PRA) including external events has been performed for N Reactor, a US Department of Energy (DOE) Category A production reactor. This four-year, multi-million dollar task was a joint effort by Westinghouse Hanford Company, Science Applications International Corporation (SAIC), and Sandia National Laboratories (SNL). Technical lead in external events and NUREG-1150 methodology was provided by SNL. SAIC led the effort in the Level 1 analysis for the internally initiated events. Westinghouse Hanford supported the task in many key areas, such as data collection and interpretation, accident progression, system interaction, human factor analyses, expert elicitation, peers review, etc. The main objective of this Level 3 PRA are to assess the risks to the public and onsite workers posed by the operation of N Reactor, to identify modifications to the plant that could reduce the overall risk, and to compare those risks to the proposed DOE and Nuclear Regulatory Commission (NRC) quantitative safety goals. This paper presents the methodology adopted by Westinghouse Hanford and SNL for estimating individual health risks, and the comparison of the N Reactor results and DOE quantitative nuclear safety guidelines. Public-Private Partnering in Nuclear Reactor Development - Historical Review and Implications for Today

Abstract:Abstract The dominant nuclear reactor technologies that comprise the current global operating fleet were developed and deployed over a relatively short, mid-twentieth century, period spanning the 1950s and 60s. Four of these technologies were deployed at fleet scales and commercially exported. The historical record indicates a remarkably consistent process of phased technology development that enabled the commercialization of designs that would define the global nuclear marketplace, beginning with research and development (R&D) and advancing through test reactors, small and large demonstration reactors, and first commercial-scale units. Following proof-of-principle R&D, historical commercialization lead times (from decision to construction of a demonstration reactor to first commercial launch) ranged from 12 to16 years for these four commercial technologies. Key factors contributing to successful commercialization included durable government support for early R&D and varying degrees of public-private partnering through commercial launch. This partnering included arrangements for technical support, siting, facility ownership, nuclear material provision, and cost sharing. The policy environment was characterized by unambiguous government support; stabile, effective and informed government program management and oversight; and flexibility in the public-private partnership arrangements to promote technology development and demonstration. Government advocacy was structured to support progressively increasing industry independence and self-sufficiency. This experience is documented and analyzed in this paper to provide salient lessons and example program elements for contemporary efforts to stimulate development and commercialization of a new generation of advanced nuclear technologies through collaboration and public-private partnerships.
Resilient Design in Nuclear Energy: Critical Lessons from a Cross-Disciplinary Review of the Fukushima Dai-ichi Nuclear Accident

Abstract:Nuclear energy has been gaining momentum recently as one of the solutions to tackle climate change. However, significant environmental and health-risk concerns remain associated with potential accidents. Despite significant preventive efforts, we must acknowledge that accidents may happen and, therefore, develop strategies and technologies for mitigating their consequences. In this paper, we review the Fukushima Dai-ichi Nuclear Power Plant accident, synthesize the time series and accident progressions across relevant disciplines, including in-plant physics and engineering systems, operators' actions, emergency responses, meteorology, radionuclide release and transport, land contamination, and health impacts. In light of the latest observations and simulation studies, we identify three key factors that exacerbated the consequences of the accident: (1) the failure of Unit 2 containment venting, (2) the insufficient integration of radiation measurements and meteorology data in the evacuation strategy, and (3) the limited risk assessment and emergency preparedness. We propose new research and development directions to improve the resilience of nuclear power plants, including (1) meteorology-informed proactive venting, (2) machine learning-enabled adaptive evacuation zones, and (3) comprehensive risk-informed emergency planning while leveraging the experience from responses to other disasters.

Calculating dependent surveillance update rates by modeling the time-dependence of information value

Abstract:The time-dependence of information has historically been managed though manual procedures, typically developed heuristically, specifying how frequently or under what conditions aging information should be refreshed. In the design of Automatic Dependent Surveillance (ADS) applications, when aircraft should automatically broadcast state information (e.g., position, velocity, or intent) must be addressed more rigorously. To answer questions about how frequently or under what conditions aging information should be updated, when the cost of seeking information must be accounted, a novel model of time-dependent information value is developed for a class of proceduralized decision problems, common in aviation. The model combines elements of classic, time-invariant information value theory, which is incapable of modeling the relationship between information value and information age, with estimation techniques. The time-dependent information value model is applied to investigate the rate at which aircraft should broadcast state information in an ADS environment. Results are presented for aircraft flying on parallel trajectories as well as on crossing trajectories. Results also provide general insight into the time-dependence of information value and its implications on information management processes.

Abstract:A comprehensive understanding of the “safety” of nuclear reactors is essential for effective and efficient safety management by licensees and regulation by authorities. Nuclear reactors are designed subject to incomplete knowledge of factors that affect their safety. The idea of defence-in-depth has evolved to combat the threat of the unknown; it is implemented by means of technical artefacts, leading to a complex set of technical safety requirements to prevent accidental radioactive releases. Nuclear power plants have thus become systems of technical systems. Similarly, significant human and organizational aspects are involved in nuclear power plant construction and operation; a nuclear power plant is an organization of organizations. Earlier studies have identified the need for holistic understanding of safety and accounting for the technical and organizational aspects simultaneously (Harvey and Stanton, 2014). This paper seeks to clarify the concept of defence-in-depth using the Overall Safety Concept (ORSAC) developed at LUT (Hyvärinen et al., 2016), and the sociotechnical systems view in the nuclear power industry context, extending defence-in-depth thinking to the organizational context in one transparent framework. We show how organizational and technical aspects affect each other in the operation of nuclear power plants. This paper paves the way for systematic modelling of how technical and organizational aspects affect each other.

Abstract:Despite substantial advancements in ensuring safety, high-risk and highly regulated organizations have not been immune to major life-threatening accidents. Growing evidence indicates that these accidents are not solely attributable to the limitations of technical barriers, but rather to the complex interactions among technical, human, and organizational factors. While both the literature and regulatory frameworks have acknowledged the role of leadership in addressing this complexity, traditional views of leadership for safety tend to focus on individual abilities to achieve safety objectives, overlooking its processual and organizationally embedded nature needed to deal with complexity. To address this gap, we draw upon Complexity Leadership Theory, which helps to embrace tensions stemming from complexity. Through a qualitative study based on an international and interdisciplinary workshop on leadership for safety in the nuclear sector, we develop a conceptual framework of organizationally embedded and dynamic tensions that leaders must engage with to enhance safety. Our results contribute to leadership for safety by explaining the embeddedness of this complex process and providing a more nuanced analysis of tensions at multiple levels and their interrelations. We also enrich the Complexity Leadership Theory by going beyond portraying different logics, mechanisms, or processes as irreconcilable polar extremes. By delving into dimensions of complexity related to leadership for safety, our framework provides new insights that can inform more nuanced safety policies and regulations in high-risk and highly regulated environments.

A social license for nuclear technologies

Abstract:Nuclear energy technologies have the potential to help mitigate climate change. However, these technologies face many challenges, including high costs, societal concern and opposition, and health, safety, environmental and proliferation risks. Many companies and academic research groups are pursuing advanced designs, both fission and fusion-based, to address both costs and these risks. This Chapter complements these efforts by analyzing how nuclear technologies can address societal concerns through the acquisition of a social license, a nebulous concept that represents "society's consent" and that has been used to facilitate and improve a wide range of publicly and privately funded projects and activities subject to a range of regulatory oversight, including large industrial facilities, controversial genetic engineering research, and environmental management. Suggestions for public engagement and consent-based siting, two aspects of a social license, have been made before. This chapter modernizes these suggestions by briefly reviewing the social license and engagement literature. The Chapter discusses, in the context of how to acquire a social license, the role of government regulation, the role of project proponents and government actors, and the role of four key principles, including engendering trust, transparency, meaningful public engagement, and protection of health, safety and the environment. Further, the Chapter uses the social license concept to explain why some nuclear waste repositories have succeeded while others languish and provides concrete recommendations for the deployment of new nuclear waste repositories and advanced power plants, both fission and fusion-based.

Abstract:The integration of data science, analytics, and model-based reasoning provides a mechanism for enhanced understanding of systems and improved decision-making, but its potential has not been thoroughly explored for improving the safety and operational efficiency of nuclear power reactors. Nuclear power owners, operators, regulators, and researchers have made significant investments in probabilistic risk assessments, numerical models, computational simulations, and development of databases that capture industry-wide component performance and operating experience. The nuclear industry is relatively unique in the size, variety, scope, technical sophistication, and quality of available data and models that capture system performance under normal operations and a wide-range of adverse event conditions. However, to date, these resources have been used in a largely static and siloed manner. Data science, analytics, and model-based reasoning provide a mechanism for fusing diverse data sources and models to develop new insights on a variety of topics. Of particular interest to the nuclear industry is the ability to leverage these resources to enhance the safety and operational efficiency of nuclear power reactors.In this paper, we present a challenge to the nuclear energy community to better leverage the existing investments in data and models to enhance decision-making. In particular, we propose that integration of recent advances in data science, analytics, and model-based reasoning provides a valuable opportunity for the nuclear industry to build upon their existing investments by accessing the power of modern data integration and risk assessment tools. We begin by describing common data and model resources available in nuclear power operations and safety analysis and offer commentary on the potential power of using Bayesian networks as a structured framework for data fusion and model integration. Then we present two example problem structures for modeling risk-informed operational decisions using heterogeneous data and models to provide simple illustrations of the means by which information streams can be leveraged in real-time to provide online assessment of risk and to increase diagnostic capabilities. Illustrative model formulations are presented for decisions under adverse events and normal operational contexts. We conclude by identifying research activities that will enable the transformation of decision-making by applying new computational and modeling tools to existing data and models.

Abstract:The internal threat to nuclear security is one of the most serious problems in the physical protection supervision of spent fuel reprocessing plants. Both insiders and nuclear security departments have obvious characteristics of situational decision making and even irrational decision making. Combined with Game theory and RDEU theory, the RDEU Game model of insiders and nuclear security departments was constructed to analyze the existence of equilibrium solutions of two-way strategies under different emotional states. From a dynamic point of view, the influence and change process of emotion on participants’ decision-making behavior were analyzed. Then, the model was numerically simulated to verify its accuracy and effectiveness, which showed that different emotional states and intensities would not only affect the final result of evolutionary equilibrium, but also change the evolution speed of the strategies. In addition, compared with insiders, the intensity of pessimism in the nuclear security department had a greater impact on the game equilibrium. Finally, we present some reasonable recommendations to prevent and protect nuclear security events at spent fuel reprocessing plants by strengthening the emotional supervision and guidance of insiders and the nuclear security department.

Abstract:Several advanced reactor designs are now under active development in the United States and elsewhere, promising sustainable solutions to the growing world energy needs. The designs currently being considered are quite diverse and different from the more established light water reactor technology that has dominated the operating commercial nuclear landscape. While advanced reactor concepts were first explored in the dawn of the nuclear age, they are now being reconsidered under the light of modern needs, and specifically, for their flexible operating conditions and inherent safety characteristics. In response, the U.S. Nuclear Regulatory Commission staff is moving forward with development of 10 CFR Part 53 rulemaking, which is a more risk-informed, technology-agnostic framework for licensing and regulating such new designs. The nuclear industry response to this regulatory initiative resulted in the technical report by the Nuclear Energy Institute, NEI 18-04 Revision 1, which provides an implementation roadmap of the risk-informed approach when defining the safety case for a new plant design. The implementation of this safety case may be a nontrivial exercise for an actual reactor design. This paper provides a demonstration of performing such an analysis for a representative advanced reactor. Public information from the General Atomics high-temperature gas reactor design was considered in this demonstration. The analysis workflow was facilitated with the FPoliSolutions' proprietary Risk-Informed System Engineering (RISE) digital platform, a product that was presented in previous publications. RISE is one application of FPoli's enterprise digital platform, which was created to facilitate orchestration of complex workflows leveraging recent technologies developed at national laboratories, such as Idaho National Laboratory's RAVEN and EMRALD frameworks. The analysis described in the paper includes the selection and classifications of events, the integration of probabilistic risk analysis artifacts, and event modeling simulations for consequence evaluations. The results are then used for system, structures, and components safety classification and a synthesis of the safety case for the design in line with the frequency-consequence targets presented in NEI 18-04. The purpose of the analysis, as framed in RISE, is to readily produce outputs and views that can aid users and regulators in making risk-informed decisions to demonstrate their plant safety case.

Abstract:This paper presents an analytical approach to design for safety that is based on 30years of experience in the field of Human-centered design. This field is often qualified as governing safety–critical systems where risk management is a crucial issue. We need to better understand what the main facets of safety are that should be taken into account during the design and development processes. There are many factors that contribute to design for safety. We propose some of these factors and an articulation of them from requirement gathering and synthesis to formative evaluations to summative evaluations. Among these factors, we analyze complexity, flexibility, stability, redundancy, support, training, experience and testing. However, we cannot design a safe and reliable product in one shot; design is incremental. A product and its various uses become progressively mature. When we deal with new products, issues come from the fact that practice features emerge from the use of the product and are difficult, even impossible, to predict ahead of time. The automation within is an important portion of this maturity, and must be understood well. This is why design for safety is not possible without anticipatory simulations and a period of tests in the real world, such as operational testing in nuclear power plants. In addition, designing for safety is not finished when the product is delivered; experience feedback, or human-in-the-loop simulation (HITLS) is an important part of the overall global design process. The AUTOS pyramid approach can assist in simplifying the understanding, and improving the design of a complex system by describing and relating Artifacts, Users, Tasks, Organizations, and Situations.

Abstract:Every day, health physicists and physicians are expected to communicate effectively with concerned people, but rarely (if ever) are they given training on how to effectively communicate. In an age of social media, this paper presents the relevance of teachings from an ancient Greek philosopher. Aristotle's Rhetoric is still considered one of the most influential works on persuasive messaging. He puts the onus of effective communications on the people with the "true" and "just" information to communicate that information clearly to the audience. By communicating with intention-using the persuasive appeals of ethos, pathos, logos, and storytelling-radiation professionals can speak to their expertise in radiation science, while adapting their instructions, presentations, and communication styles to meet the needs of each type of audience: from scientists to concerned citizens, from doctors to first responders, and beyond.
