Abstract:Objective To analyze the biomechanical distribution of bone around the dental implant with immediate load and delayed load.Methods Three-dimensional finite element models of whole mandible with dental implants for immediate load and delayed load were established by means of CT scanning and USIS(Universal Surgical Integration System)software developed by ourselves.The distribution of stress and strain on the bone around dental implants and displacement of implants were calculated and analyzed with the method of finite element.Results The values of Von Mises stress on the bone around dental implants with immediate load were a bit less than those with delayed load,and both stresses were concentrated on the cortical bone,which were on the neck of dental implants.And the values of Von Mises strain on the bone around dental implants with immediate load were more than those with delayed load apparently,which were concentrated on the cancellous bone,at the bottom of dental implants and in the area of screw.Displacement of dental implants with immediate load was a bit more than those with delayed load.When the implants were loaded with buccolingual force,Von Mises stress,strain and displacement of dental implants with immediate load and delayed load increased respectively.Conclusion Biomechanical characteristics of bone around the dental implant with immediate load were similar to those with delayed load.It is certain that the technique of dental implant with immediate load is available.

Abstract:Objective:To analyze the stress distribution of bone around the dental implants with immediate loading with the method of three-dimensional finite element.Methods:Three-dimensional finite element model of thread dental implants with immediate loading was developed using CT scanning and USIS(Universal Surgical Integration System)software developed by ourselves.The value of Von Mises stress 、strain of implant-bone interface were calculated with the software of ANSYS,when the dental implants were loaded with vertical、 buccolingual and mesiodistal force of 150 N.Results:When the dental implants were loaded with vertical force,the Von Mises stress concentration appeared around the lingual cortical bone intergrated with the neck of implants,and Von Mises strain were well-distributed,.When the dental implants were loaded with buccolingual force,the Von Mises stress and strain concentration all appeared around the lingual cortical bone intergrated with the neck of implants,but the maximum value of Von Mises stress increased by 315%,the maximum value of strain increased by 298%,and the strain distribution weren' even.When the dental implants were loaded with mesiodistal force,the Von Mises stress concentration appeared around the distall cortical bone intergrated with the neck of implants,the maximum value of Von Mises stress increased by 272%,and Von Mises strain concentrated on on the mesial cancellous bone intergrated with the bottom of implants,the maximum value of Von Mises strain increased by 51%.Conclusion:Von Mises stress and strain didn't concentrated obviously and the distribution of them were even,when dental implants were loaded with immediate vertical force.However,when dental implants were loaded with immediate oblique force,the value of Von Mises stress and strain increased apparently and the distribution of them were quite uneven.
Influence of various implanting angles of micro-implants on peak stress in peri-implant surface

Abstract:BACKGROUND:Skeletal anchorage with mini-implants has greatly broadened the treatment possibilities in orthodontics over the last few years.To reduce implant failure rates,it is advisable to obtain adequate stress distribution around implant.OBJECTIVE:To evaluate the stress distribution on bone adjacent to a mini-implant under the different implanting direction and angle.MATERIALS:Jaw data were obtained from the scanning of Dong.Implant model was established according to screwed mini-implant,titanium,Aarhus Screw Syst 1.6,Medicon,Germany.METHODS:Three-dimensional finite element mandible model was established based on the CT scanning data.The mini-implant was implanted in the region between first mola and second premolar at 30°,60° and 90° tilted mesiolly,and 30° and 60° tilted occlusally,respectively.In addition,a force load of 1.96 N was applied horizontally to the head of implant.MAIN OUTCOME MEASURES:The maximum first(pulling) and third(pressing) stress,minimum stress,Von-mises stress and maximum strain were calculated.RESULTS:The peak pulling and pressing stress and Von-mises stress at 30° tilted mesiolly were significantly smaller than the other models.The Von-mises stress mainly located at the neck of implant-bone interface,and decreased rapidly in the lamina cortex.The Von-mises stress in cancellous bone was small,and the side adjacent to loading was pressed,while the other side was pulled.CONCLUSION:Implanting angle shows much more influence on the stress distribution.The stress is primarily located at the neck of implant under different loads.Decreased implanting angle and reduced angle between load line and implant long axis can reduce peak stress.
[Stress Distribution in Press-Fit Orthodontic Microimplant Bone Interface].

Abstract:PURPOSE:The goal of this study is to analyse the stress distribution in the press-fit microimplant-bone interface and its indications for immediate loading of orthodontic microimplant.METHODS:Three-dimensional finite element models were created of a 20 mm section of posterior mandible simplified in isosceles trapezoid shape, 30 mm in height, 10mm in upper side width, 14 mm in lower side width,with a single microimplant, 1.2 mm in diameter, 6 mm in length embedded in the bone. The cortical bone thickness was assumed as 1.6 mm. Cortical and cancellous bone were modeled as transversely isotropic and linearly elastic materials. Titanium was modeled as isotropic and linearly elastic material. Perfect bonding was assumed at microimplant- bone interfaces. ANSYS 9.0 finite element analysis software was used to generate the simplified finite element models of the local mandible-implant complex. 0 mm, 0.05 mm and 0.1 mm press-fit were arbitrarily set to the implant-bone interface to mimic the situation of immediate placement of microimplant. Stresses in the microimplant-bone interface were calculated under these "press-fit".RESULTS:Stresses distributed mainly in the cortical bone interface. At Omm press-fit, the stress was 0 MPa. For 0.05mm press-fit, the stress was 1648 MPa in mesio-distal direction, 1782MPa in occluso-gingival direction;and for 0.1 mm, it reached 2012MPa in mesio-distal direction, 2110MPa in occluso-gingival direction. As the "press-fit" increased, the stresses increased accordingly.CONCLUSION:Values of initial stress in the microimplant-bone interface due to press-fit generated by immediately placed microimplant were very high in these limited and simplified three dimensional finite element models. It reminded us that the initial stress be taken into consideration when immediate loading of the microimplant is planned. Supported by Research Fund of Health Bureau of Zhejiang Province (2005B104).
[Three-Dimensional Finite Element Analysis of Influence of Occlusal Surface Height on Stress Distribution Around Posterior Implant-Supported Single Crown].

Abstract:OBJECTIVE:To evaluate the effect of the occlusal surface height of a mandibular posterior implant-supported single crown on stress in bone tissues.METHODS:Three-dimensional finite element models of the implant-supported single crown replacing the missing right lower second premolar, mesial and distal natural teeth, periodontal membrane, alveolar bone, loaded rigid body and analog food of almond were established. Using the Federation Dentaire International (FDI) system, the first premolar, the second premolar and the first molar were represented with 44, 45, 46. Three occlusal surface heights of the crown were studied: (1) normal height; (2) 15 μm reduction in height; (3) 30 μm reduction in height. The models were loaded by independent loading with maximal occlusal force(44 by 280 N, 45 by 360 N, and 46 by 480 N) and average occlusal force(44 by 140 N, 45 by 180 N, and 46 by 240 N)on the single crown; combined loading (maximal occlusal force transformed into uniform load of 3.7 MPa on top of rigid body, in contact with points on the occlusal surface), and analog almond-like food loading (average occlusal force transformed into uniform load of 1.67 MPa in simulated food chewing, in contact with points on the occlusal surface).RESULTS:For maximal biting force under independent loading, Von Mises stress peak values in bone tissues around 44, 45, and 46 were 82.57 MPa, 45.26 MPa and 27.79 MPa; For average biting force, peak values were 41.28 MPa, 22.63 MPa and 13.89 MPa. Under combined loading, compared with the normal occlusal surface height group, Von Mises stress peak values decreased 4.6 MPa, by 0.84%; increased 7.52 MPa, by 20.04%, and decreased 1.8 MPa, by 5.84%, for 45, 46, and 44 in the 30 μm infra-occlusion group, respectively. Under food loading, Von Mises stress peak values decreased 0.34 MPa, by 1.62%; increased 1.11 MPa, by 2.66%; and increased 0.06 MPa, by 0.54%, and for 45, 46, and 44 in the 30 μm infra-occlusion group, respectively.CONCLUSION:Within the limitation of this study, within 30 μm reduction of the occlusal surface height of implant-supported single crown, no significant difference of the peak values was observed.

Abstract:OBJECTIVE:To observe the effect of mini-implant lengths on stress distribution in peri-implant surface.METHODS:The 3D finite element analysis mandible and mini-implant models with diameter of 1.6 mm, lengths of 6, 8, 10 and 12 mm were established. The mini-implants were inserted into designed site of mandibular vertically, respectively. A force of 1.96 N were applied mesioly and 45 degrees tilted mesio-vertically in models. The stress distribution under every condition was recorded and analyzed.RESULTS:When load was applied mesially, the maximum stress varied from 3.500 to 3.765 MPa, the maximum displacement varied from 1.266 to 1.288 microm. When load was applied 45 degrees tilted mesio-vertically, the maximum stress varied from 4.075 to 4.510 MPa, the maximum displacement varied from 1.668 to 1.694 microm. All of the maximum stress and displacement of loading mesially were lower than loading mesio-vertically.CONCLUSION:The change of the mini-implant length within 6-12 mm don't show much influence on the stress distribution. The loading type is an important factor influencing stress and displacement of peri-implant bone.
Three-dimensional finite element analysis of upper anterior teeth retraction and intrusion using clear aligners and mini-implants

Abstract:Objectives: This study aimed to investigate the differences in treatment outcomes of upper anterior teeth retraction and intrusion with clear aligners and different types of elastics on mini-implants via the 3D finite element method. Methods: Cone-beam computed tomography (CBCT) was conducted to construct a 3D finite element model of maxillary bone, dentition, and clear aligners using Mimics, Geomagic, Solidworks, and Ansys. Four model groups were developed. Group 1 was the control group. Group 2 implanted a mini-implant between the upper second premolar and the upper first molar with a force of 0.98 N applied to the canine cut along the retracting direction for each side. Group 3 was based on Group 2, which implanted an additional mini-implant between the upper central incisors with a force of 0.98 N applied to the upper anterior teeth cuts. Group 4 was also based on Group 2, which implanted 2 additional mini-implants between the upper central and lateral incisors with a force of 0.56 N applied to the upper anterior teeth cuts for both sides. Anterior teeth torque changes, displacement patterns, and stress distributions were analyzed. Results: Among all the experimental groups, the anterior teeth demonstrated intrusion and distal inclination, accompanied with varied degrees of torque loss. In Group 3, the maxillary and lateral incisors had the smallest sagittal coronal displacement. In Group 4, the intrusion values of the maxillary and lateral incisors were the largest among all the experimental groups. The maximum stress concentration in Group 2 was the most significant among all the experimental groups. Conclusions: Implanting the mini-implant between the central incisors for traction is conducive to torque control. By contrast, implanting the mini-implants between the central incisors and lateral incisors combined with vertical traction is conducive to simple intrusion, avoiding the "bowing effect" to a certain extent.
Influence of sagittal root positions on the stress distribution around custom-made root-analogue implants: a three-dimensional finite element analysis

Abstract:Abstract Background Stress concentration may cause bone resorption even lead to the failure of implantation. This study was designed to investigate whether a certain sagittal root position could cause stress concentration around maxillary anterior custom-made root-analogue implants via three-dimensional finite element analysis. Methods The von Mises stresses in the bone around implants in different groups were compared by finite element analysis. Six models were constructed and divided into two groups through Geomagic Studio 2012 software. The smooth group included models of unthreaded custom-made implants in Class I, II or III sagittal root positions. The threaded group included models of reverse buttress-threaded implants in the three positions. The von Mises stress distributions and the range of the stresses under vertical and oblique loads of 100 N were analyzed through ANSYS 16.0 software. Results Stress concentrations around the labial lamella area were more prominent in the Class I position than in the Class II and Class III positions under oblique loading. Under vertical loading, the most obvious stress concentration areas were the labial lamella and palatal apical areas in the Class I and Class III positions, respectively. Stress was relatively distributed in the labial and palatal lamellae in the Class II position. The maximum von Mises stresses in the bone around the custom-made root-analogue implants in this study were lower than around traditional implants reported in the literature. The maximum von Mises stresses in this study were all less than 25 MPa in cortical bone and less than 6 MPa in cancellous bone. Additionally, compared to the smooth group, the threaded group showed lower von Mises stress concentration in the bone around the implants. Conclusions The sagittal root position affected the von Mises stress distribution around custom-made root-analogue implants. There was no certain sagittal root position that could cause excessive stress concentration around the custom-made root-analogue implants. Among the three sagittal root positions, the Class II position would be the most appropriate site for custom-made root-analogue implants.

Abstract:Purpose: The primary objective of this study was to investigate the biomechanical effects and stresses on bone, periodontal ligament (PDL), cementum and displacement along X-, Y- and Z-axis during true intrusion of incisors using mini-implants with Finite Element Analysis; the secondary objective of the study was to find out the best method for anterior intrusion in clinical practice to treat anterior deep bite malocclusions. Materials and methods: A 3D finite element method was used to simulate true anterior intrusion with sliding mechanics using mini-implants. Two groups were modelled with mini-implants placed distal to lateral incisors for Model 1, and below the anterior nasal spine (ANS) for Model 2, to achieve intrusion. von Mises stress, principal stress on PDL and alveolar bone, displacements in all 3 planes were determined. Results: Amongst the modalities compared in the present study, the stresses on bone and PDL were showing best behavior for mini-implants placed distal to lateral incisors (Model 1). The teeth showed controlled tooth movement in Model 1. Conclusion: Maximum stress was found in the cortical bone and in the PDL. Nature of the stress changed from compressive to tensile from cervical area to root apex, concentrating mainly at the apical area. Amongst the modalities compared, the best controlled tooth movements for anterior intrusion to treat anterior deep bite malocclusions, was for mini-implants placed distal to lateral incisors (Model 1).

Abstract:Introduction The aim of this study is to evaluate stress and displacement effects of maxillary posterior intrusion mechanics with mini-implant anchorage by using finite element method. Materials and Methods A computer stimulation of three-dimensional model maxilla with all teeth, PDL, bone, mini-implants, brackets, arch wire, force element, and transpalatal arch was constructed on the basis of average anatomic morphology. Finite element analysis was done to evaluate the amount of stress and its distribution during orthodontic intrusive force. Results Increased Von Mises stress values were observed in mesio-cervical region of first molar. The middle third of second premolar and second molar and regions adjacent to force application sites also showed relatively high stress values. Minimum stress values were observed in apical region of first premolar as it is away from force application. Conclusion Using three mini-implant and transpalatal arches, this study demonstrates that significant amount of true intrusion of maxillary molars could be obtained with lesser concentration of stresses in the apical area recorded.

Abstract:Purpose. To investigate effects of number and location on patterns of von Mises stress distribution and volume average stress on abutment tooth, edentulous ridge, mini dental implant, and surrounding bone of mini dental implant-assisted mandibular Kennedy class I removable partial denture. Materials and Methods. Eight three-dimensional finite element models of mandibular Kennedy class I with different numbers and locations of mini dental implants were constructed. Mini dental implants were generated in the area of second premolar, first molar, and second molar, respectively. A static load of 400 N was applied on all models. The von Mises stress and volumetric average stress were calculated by three-dimensional finite element analysis. Result. The minimum volumetric average stress of abutment tooth was found in the model, where there was one mini dental implant at the second molar position and 2 mini dental implants at first molar and second molar positions. The model with three mini dental implants had reduced volumetric average stress of abutment tooth, which was not different from the model with two mini dental implants. However, the minimum volumetric average stress of mini dental implant and surrounding bone were found when three mini dental implants were applied, followed by two and one mini dental implants, respectively. Conclusion. Placing at least one mini dental implant at a second molar position can help reduce stress transferred to the abutment tooth. Stresses around each implant and surrounding bone reduced with increased numbers of mini dental implants.

Abstract:Background and objective: Mini-implants have been developed and effectively used by clinicians as anchorage for orthodontic tooth movement. The objective of this study was to elucidate the stress response of orthodontic forces on the periodontal system, bone tissues, mini-implant and the bracket-enamel interface. Methods: Computer tomography images of a commercially available mini-implant, an orthodontic bracket bonded to a central incisor, and jawbone section models were used to reconstruct three dimensional computer models. These models were exported and meshed in an ABAQUSⓇ finite-element package. Material properties, multi-segment interactions, boundary and loading conditions were then applied to each component. Finite-element analyses were conducted to elucidate the effect of orthodontic force on the equivalent von Mises stress response within the simulated orthodontic system. Results: The highest stress values in the orthodontic system were predicted at the mini-implant neck, at the interface of the cortical bone, and gradually decreased in the internal apical direction of the miniscrew. On the alveolar bone, the maximum stress values were located in the alveolar cortical bone near the cervical areas of the mini-implant, which is in line with clinical findings of area where bone loss was found post orthodontic tooth treatment. Another peak of von Mises stress response was found in the enamel bracket junction with a maximum up to 186.05 MPa. To ensure good bonding between the enamel and bracket, it is vital to select carefully the type and amount of bonding materials used in the bracket-enamel interface to assure an appropriate load distribution between the teeth and alveolar bone. The results also revealed the significance of the periodontal ligaments, acting as an intermediate cushion element, in the load transfer mechanism. Conclusions: This study is sought to identify the stress response in a simulated orthodontic system to minimize the failure rate of mini-implants and bracket loss during orthodontic treatment.
Influence of Implant Number on the Biomechanical Behaviour of Mandibular Implant-Retained/supported Overdentures: A Three-Dimensional Finite Element Analysis

Abstract:OBJECTIVE:The aim of this study was to evaluate strain distribution in peri-implant bone, stress in the abutments and denture stability of mandibular overdentures anchored by different numbers of implants under different loading conditions, through three-dimensional finite element analysis (3D FEA).METHODS:Four 3D finite element models of mandibular overdentures were established, using between one and four Straumann implants with Locator attachments. Three types of load were applied to the overdenture in each model: 100N vertical and inclined loads on the left first molar and a 100N vertical load on the lower incisors. The biomechanical behaviours of peri-implant bone, implants, abutments and overdentures were recorded.RESULTS:Under vertical load on the lower incisors, the single-implant overdenture rotated over the implant from side to side, and no obvious increase of strain was found in peri-implant bone. Under the same loading conditions, the two-implant-retained overdenture showed more apparent rotation around the fulcrum line passing through the two implants, and the maximum equivalent stress in the abutments was higher than in the other models. In the three-implant-supported overdenture, no strain concentration was found in cortical bone around the middle implant under three loading conditions.CONCLUSIONS AND CLINICAL SIGNIFICANCE:Single-implant-retained mandibular overdentures do not show damaging strain concentration in the bone around the only implant and may be a cost-effective treatment option for edentulous patients. A third implant can be placed between the original two when patients rehabilitated by two-implant overdentures report constant and obvious denture rotation around the fulcrum line.
Finite element analysis of the stress state produced by an orthodontic skeletal anchorage system based on miniscrews

Abstract:Aims: The aim of this work is to produce detailed analysis and quantitative results of the stress state induced in an inferior first molar tooth by an orthodontic mesialization system based on a mini-screw, a bracket, and a rubber elastic string. Materials and Methods: The implant was modeled as a non-osteointegrated Ti-6Al-4V standard mini-screw, implanted in the anterior-buccal region of human jaw. 3D realistic models were constructed using data based on micro-computed tomography scans of human teeth as found in the literature. A couple of self-balanced forces of 4 N was applied and finite element method analysis was run and produced an approximate solution, which allowed to display stress distribution over the whole model. Materials' behavior was assumed elastic and linear. This approximation applies to the 1 st stage of the orthodontic treatments. Results: A detailed qualitative and quantitative analysis of the stress state is presented through images. Results in terms of stresses and displacements were studied to exclude any possibility of bracket debonding, bone failure, and loss of stability of the screw. Results were compared to the ultimate strengths of the bio-materials involved using engineering failure criteria. The actual stress state was found to be lower than the critical values. Conclusion: Results allows to qualitatively see the amount of bone, surronding the mini-screw, and the tooth, that undergoes stress. Furthermore, quantitative analysis of stress could exclude failure in alveolar bone and detachment at the enamel-bracket interface. The applied force and devices were proved to be safe to use for this kind of orthodontic facilities.

Abstract:Objective: To study the effects of implant size in single mandibular molar implant restoration on the stressdistribution of implant-bone interface. Methods: Three dimensional finite element method was employed to simulate theloss of the 1st molar restored by single implant and two implants. The effect of implant size on stress distribution at boneinterface was observed expelling the influence of the other factor. Results: There was a negative correlation betweenimplant diameter or length and stresses in bone. But influence of implant length was minor. Conclusion: If adequate bonequantity is provided, using wider implants to restore single mandibular molar should be recommended in clinical practice.

Abstract:To study the effects of different designs in single mandibular molar implant restoration on the stress distribution of implant-bone interface. Methods: Three-dimensional finite element method were used. Results: When wide-diameter implant or two-implant were used to replace single mandibular molar, the stress of implant-bone interface was much lower than that of standard-diameter implant restoration. Comparison between wide-diameter implant and two-implant replacement showed that the former created lower stress. The difference was more obvious under oblique loads. Conclusion: Using wide-diameter implant or two-implant to restore single mandibular molar should be recommended in clinical practice. If adequate bone quantity is provided, using wide-diameter implant should be better.

Abstract:Abstract Objective To assess the biomechanical effects of different prosthetic/implant configurations and load directions on 3-unit fixed prostheses supported by short dental implants in the posterior mandible using validated 3-D finite element (FE) models. Methods Models represented an atrophic mandible, missing the 2nd premolar, 1st and 2nd molars, and rehabilitated with either two short implants (implant length-IL = 8 mm and 4 mm) supporting a 3-unit dental bridge or three short implants (IL = 8 mm, 6 mm and 4 mm) supporting zirconia prosthesis in splinted or single crowns design. Load simulations were performed in ABAQUS (Dassault Systèmes, France) under axial and oblique (30°) force of 100 N to assess the global stiffness and forces within the implant prosthesis. Local stresses within implant/prosthesis system and strain energy density (SED) within surrounding bone were determined and compared between configurations. Results The global stiffness was around 1.5 times higher in splinted configurations vs. single crowns, whereby off-axis loading lead to a decrease of 39%. Splinted prostheses exhibited a better stress distribution than single crowns. Local stresses were larger and distributed over a larger area under oblique loads compared to axial load direction. The forces on each implant in the 2-implant-splinted configurations increased by 25% compared to splinted crowns on 3 implants. Loading of un-splinted configurations resulted in increased local SED magnitude. Conclusion Splinting of adjacent short implants in posterior mandible by the prosthetic restoration has a profound effect on the magnitude and distribution of the local stress peaks in peri-implant regions. Replacing each missing tooth with an implant is recommended, whenever bone supply and costs permit.

Abstract:BACKGROUND: Stress on the bone surrounding dental micro-implants affects implant success.PURPOSE: To compare the stress on the bone surrounding a micro-implant after application of a single force (SF) of 200 g or a composite force (CF) of 200 g and 6 N.mm torque.MATERIALS AND METHODS: Finite element models were developed for micro-implant diameters of 1.2, 1.6, and 2.0 mm, and lengths of 6, 8, 10, and 12 mm and either a SF or CF was applied. The maximum equivalent stress (Max EQS) of the bone surrounding the micro-implant was determined, and the relationships among type of force, diameter, and length were evaluated.RESULTS: The Max EQS of the CF exceeded that of the SF (P< 0.05). The effect of force on stress was related to implant diameter, but not to implant length. The larger CF led to greater instability of the micro-implant and the effect was most pronounced at an implant diameter of 1.2 mm. The use of implant diameters of 1.6 mm and 2.0 mm produced no significant difference in implant stability when either a CF or SF was applied.CONCLUSION: When considering the use of an implant to perform three-dimensional control on the teeth, the implant diameter chosen should be > 1.2 mm.

Abstract:OBJECTIVE:To study the influence of different shape of lower jaw on stress distribution at bone interface in the mandibular molar region of implant restoration models.METHODS:Six models with different lower jaw shapes were constructed by using three-dimensional finite element method. Implant-bone interface stresses in these models were studied under vertical and lateral loads.RESULTS:No matter in the condition of vertical loading of lateral loading, stress distribution patterns were similar in variant models and differences of stress values were not statistically significant. The maximal differences in Von-Mises stress, compressive stress and tensile stress among the six models were 6.4%, 2.8% and 6.2%respectively.CONCLUSION:It is rational to simplify the lower jaw shape in finite element analysis.
