Abstract:Objective:To analyze the stress distribution of bone around the dental implants with immediate loading with the method of three-dimensional finite element.Methods:Three-dimensional finite element model of thread dental implants with immediate loading was developed using CT scanning and USIS(Universal Surgical Integration System)software developed by ourselves.The value of Von Mises stress 、strain of implant-bone interface were calculated with the software of ANSYS,when the dental implants were loaded with vertical、 buccolingual and mesiodistal force of 150 N.Results:When the dental implants were loaded with vertical force,the Von Mises stress concentration appeared around the lingual cortical bone intergrated with the neck of implants,and Von Mises strain were well-distributed,.When the dental implants were loaded with buccolingual force,the Von Mises stress and strain concentration all appeared around the lingual cortical bone intergrated with the neck of implants,but the maximum value of Von Mises stress increased by 315%,the maximum value of strain increased by 298%,and the strain distribution weren' even.When the dental implants were loaded with mesiodistal force,the Von Mises stress concentration appeared around the distall cortical bone intergrated with the neck of implants,the maximum value of Von Mises stress increased by 272%,and Von Mises strain concentrated on on the mesial cancellous bone intergrated with the bottom of implants,the maximum value of Von Mises strain increased by 51%.Conclusion:Von Mises stress and strain didn't concentrated obviously and the distribution of them were even,when dental implants were loaded with immediate vertical force.However,when dental implants were loaded with immediate oblique force,the value of Von Mises stress and strain increased apparently and the distribution of them were quite uneven.

Abstract:Objective To analyze the biomechanical distribution of bone around the dental implant with immediate load and delayed load.Methods Three-dimensional finite element models of whole mandible with dental implants for immediate load and delayed load were established by means of CT scanning and USIS(Universal Surgical Integration System)software developed by ourselves.The distribution of stress and strain on the bone around dental implants and displacement of implants were calculated and analyzed with the method of finite element.Results The values of Von Mises stress on the bone around dental implants with immediate load were a bit less than those with delayed load,and both stresses were concentrated on the cortical bone,which were on the neck of dental implants.And the values of Von Mises strain on the bone around dental implants with immediate load were more than those with delayed load apparently,which were concentrated on the cancellous bone,at the bottom of dental implants and in the area of screw.Displacement of dental implants with immediate load was a bit more than those with delayed load.When the implants were loaded with buccolingual force,Von Mises stress,strain and displacement of dental implants with immediate load and delayed load increased respectively.Conclusion Biomechanical characteristics of bone around the dental implant with immediate load were similar to those with delayed load.It is certain that the technique of dental implant with immediate load is available.
Alveolar Bone Stress Around Implants With Different Abutment Angulation: An FE-Analysis of Anterior Maxilla

Abstract:Objectives:To comparatively assess the masticatory stress distribution in bone around implants placed in the anterior maxilla with three different labial inclinations. Materials and Methods:Three-dimensional finite element models were fabricated for three situations in anterior maxilla: (1) a fixture in contact with buccal cortical plate restored by straight abutment, (2) a fixture inclined at 15 degrees, and (3) 20 degrees labially restored with corresponding angled abutment. A palatal bite force of 146 N was applied to a point 3 mm below the incisal edge. Stress distribution around the bone-fixture interface was determined using ANSYS software. Results:The maximum compressive stress, concentrated in the labial crestal cortical bone, was measured to be 62, 108, and 122 MPa for 0-, 15-, and 20-degree labially inclined fixtures, respectively. The maximum tensile stress, concentrated in the palatal crestal cortical bone, was measured to be 60, 108, and 120 MPa for 0-, 15-, and 20-degree labially inclined fixtures, respectively. Conclusions:While all compressive stress values were under the cortical yield strength of 169 MPa, tensile stress values partially surpassed the yield strength (104 MPa) especially when a 20-degree inclination was followed for fixture placement.

Assessment of stress shielding around a dental implant for variation of implant stiffness and parafunctional loading using finite element analysis

Abstract:Purpose: The aim of this study was to evaluate the mechanical stimuli transfer at the bone-implant interface via stress and strain energy density transfer parameters. This study also aimed to investigate the effect of different implant stiffness and parafunctional loading values on the defined mechanical stimuli transfer from the implant to the surrounding bone. Methods: A three-dimensional finite element model of two-piece threaded dental implant with internal hexagonal connection and mandibular bone block was constructed. Response surface method through face-centred central composite design was applied to examine the influence of two independent factors variables using three levels. The analysis model was fitted to a second-order polynomial equation to determine the response values. Results: The results showed that the implant stiffness was more effective than the horizontal load value in increasing the stress and strain energy density transfers. The interaction between both factors was significant in decreasing the likelihood of bone resorption. Decreasing the implant stiffness and horizontal load value led to the increased stress transfer and unexpected decrease in the strain energy density, except at the minimum level of the horizontal load. The increase in the implant stiffness and horizontal load value (up to medium level) have increased the strain energy transfer to the bone. Conclusions: The stress and strain energy density were transferred distinctively at the bone-implant interface. The role of both implant stiffness and parafunctional loading is important and should be highlighted in the preoperative treatment planning and design of dental implant.

[Three-Dimensional Finite Element Analysis of Influence of Occlusal Surface Height on Stress Distribution Around Posterior Implant-Supported Single Crown].

Abstract:OBJECTIVE:To evaluate the effect of the occlusal surface height of a mandibular posterior implant-supported single crown on stress in bone tissues.METHODS:Three-dimensional finite element models of the implant-supported single crown replacing the missing right lower second premolar, mesial and distal natural teeth, periodontal membrane, alveolar bone, loaded rigid body and analog food of almond were established. Using the Federation Dentaire International (FDI) system, the first premolar, the second premolar and the first molar were represented with 44, 45, 46. Three occlusal surface heights of the crown were studied: (1) normal height; (2) 15 μm reduction in height; (3) 30 μm reduction in height. The models were loaded by independent loading with maximal occlusal force(44 by 280 N, 45 by 360 N, and 46 by 480 N) and average occlusal force(44 by 140 N, 45 by 180 N, and 46 by 240 N)on the single crown; combined loading (maximal occlusal force transformed into uniform load of 3.7 MPa on top of rigid body, in contact with points on the occlusal surface), and analog almond-like food loading (average occlusal force transformed into uniform load of 1.67 MPa in simulated food chewing, in contact with points on the occlusal surface).RESULTS:For maximal biting force under independent loading, Von Mises stress peak values in bone tissues around 44, 45, and 46 were 82.57 MPa, 45.26 MPa and 27.79 MPa; For average biting force, peak values were 41.28 MPa, 22.63 MPa and 13.89 MPa. Under combined loading, compared with the normal occlusal surface height group, Von Mises stress peak values decreased 4.6 MPa, by 0.84%; increased 7.52 MPa, by 20.04%, and decreased 1.8 MPa, by 5.84%, for 45, 46, and 44 in the 30 μm infra-occlusion group, respectively. Under food loading, Von Mises stress peak values decreased 0.34 MPa, by 1.62%; increased 1.11 MPa, by 2.66%; and increased 0.06 MPa, by 0.54%, and for 45, 46, and 44 in the 30 μm infra-occlusion group, respectively.CONCLUSION:Within the limitation of this study, within 30 μm reduction of the occlusal surface height of implant-supported single crown, no significant difference of the peak values was observed.

Abstract:BACKGROUND Research on resolving implant fracture is still gaining attention as it can be a serious treatment failure outcome. The implant fracture is likely to occur due to increased stress in implant body associated with peri-implant bone resorption. OBJECTIVE This study aimed to investigate the relationship between degree of peri-implant bone resorption and stress distribution in implant body by using finite element analysis (FEA). METHODS The magnitude and direction of loads on implants at the mandibular molar region were measured with 3D piezoelectric force transducers in a patient during maximal voluntary clenching, grinding, and tapping to obtain input for the FEA as the loading condition. Simplified finite element bone models were generated to simulate the six patterns of peri-implant bone resorption progression which the bone levels from implant platform were set at 0, 1, 2, 3, 4, and 5 mm. Three types of implants with different diameters (3.5, 4.0, and 5.0 mm), corresponding abutments and screws were created. FEA using the in vivo measured load was conducted to investigate how peri-implant bone loss affects stress distribution in the implant body. RESULTS Loading condition affected stress distribution in the implant body. As bone resorption increased, von Mises stress in the implant body became larger, which was marked in smaller diameter implants, and this change was amply demonstrated when the bone resorption increased from 0 to 2 mm. CONCLUSION The FEA demonstrated that as peri-implant bone loss increased, the magnitude of stress in the implant increased, especially in small diameter implants.

Abstract:PURPOSE:The goal of this study is to analyse the stress distribution in the press-fit microimplant-bone interface and its indications for immediate loading of orthodontic microimplant.METHODS:Three-dimensional finite element models were created of a 20 mm section of posterior mandible simplified in isosceles trapezoid shape, 30 mm in height, 10mm in upper side width, 14 mm in lower side width,with a single microimplant, 1.2 mm in diameter, 6 mm in length embedded in the bone. The cortical bone thickness was assumed as 1.6 mm. Cortical and cancellous bone were modeled as transversely isotropic and linearly elastic materials. Titanium was modeled as isotropic and linearly elastic material. Perfect bonding was assumed at microimplant- bone interfaces. ANSYS 9.0 finite element analysis software was used to generate the simplified finite element models of the local mandible-implant complex. 0 mm, 0.05 mm and 0.1 mm press-fit were arbitrarily set to the implant-bone interface to mimic the situation of immediate placement of microimplant. Stresses in the microimplant-bone interface were calculated under these "press-fit".RESULTS:Stresses distributed mainly in the cortical bone interface. At Omm press-fit, the stress was 0 MPa. For 0.05mm press-fit, the stress was 1648 MPa in mesio-distal direction, 1782MPa in occluso-gingival direction;and for 0.1 mm, it reached 2012MPa in mesio-distal direction, 2110MPa in occluso-gingival direction. As the "press-fit" increased, the stresses increased accordingly.CONCLUSION:Values of initial stress in the microimplant-bone interface due to press-fit generated by immediately placed microimplant were very high in these limited and simplified three dimensional finite element models. It reminded us that the initial stress be taken into consideration when immediate loading of the microimplant is planned. Supported by Research Fund of Health Bureau of Zhejiang Province (2005B104).
Model-based biomechanical dental implant optimization in bone-implant system

Abstract:Purpose: The purpose of this study was to identify the important parameters for a model-based design optimization method through comparison of different biomechanical environments and various dental implant models. Materials and Methods: Finite element analysis (FEA) was applied to model a mandibular bone implant system. Essential parameters, such as, cortical bone properties, boundary conditions, and loading directions, were investigated and altered. The effects of altering these parameters in bone-implant FEA models were evaluated based on differences in stresses. Results: The results of this study showed that under different oblique loads, cortical bone properties and constraints the differences in the stress level and distribution between the different implant designs could be clearly identified. Key parameters for a model-based design optimization method were identified effectively by using FEA to compare the stress situation in different implant systems with various biomedical environments. Conclusion: Without knowing which parameters play substantial roles, design optimization of a dental implant system remains a challenge, especially when comparison of different implants are involved and bone quality has to be taken into account. This study shows that an oblique load, excellent cortical bone property, and minimal support constraints are critical parameters affecting the stress distribution in the bone-implant model. Treatment with dental implants is considered an effective and safe method. However, in some cases, the use of dental implants had some failures. The success of dental implants is influenced by several biomechanical factors such as loading type, used material properties, shape and geometry of implants, quality and quantity of bone around implants, surgical method, lack of rapid and proper implant surface's integration with the jaw bone, etc. The main purpose of functional design is to investigate and control the stress distribution on dental implants to optimize their performance. Finite element analysis allows researchers to predict the stress distribution in the bone implant without the risk and cost of implant placement. In this study, the stresses created in the 3A.P.H.5 dental implant's titanium fixture and screw due to the change in abutment angles tolerance have been investigated. The results show that although the fixture and the screw's load and conditions are the same in different cases, the change of the abutment angle and the change in the stress amount also made a difference in the location of maximum stress. The 21-degree abutment puts the fixture in a more critical condition and increases the chance of early plasticization compared to other states. The results also showed that increasing the abutment angle to 24 degrees reduces the stress in the screw, but decreasing the angle to 21 degrees leads to increased screw stress and brings it closer to the fracture.

Abstract:Aim: Using finite element analysis, determine the influence of short dental implant material on surrounding bone stresses. Material and Methods: One simplified model was created for a short implant of 4.8×4.8×4 mm placed vertically in simplified bone geometry to support dummy crown fixed by 50micron resin cement layer. Three materials were tested as an implant material, Zirconia, Titanium, and 30% CFR-PEEK. Components of the 3D model were prepared on engineering CAD/CAM software accumulated under ANSYS modeling for finite element analysis. The model was subjected to two loading cases as; 100 N compressive load and 50 N Oblique (45°), both at the central fossa. Results: Under the applied loads, all values of total deformations and Von Mises stresses that developed during the current investigation were within physiological limits. Under both loading cases, changing the implant material from Zirconia to titanium to Polyether ether ketone (PEEK) decreased Von Mises stress values in the implant, cortical, and cancellous bone. The cement layer, abutment, and connecting screws all showed signs of growth. Conclusion: Zirconia and Titanium can replace each other as short implant material. In addition, 30% CFR-PEEK can also be used as short implant material with minor acceptable stress differences.

Effect of Implant-Abutment Connection and Abutment Angulations on Peri-implant Stress Levels: A Finite Element Analysis Study

Abstract:Introduction: Placement of an implant in the maxillary anterior region is challenging due to the angulation of bone in this area. Angled abutments may be used to achieve proper restorative contours. The present study was undertaken to examine and compare the stress levels of implants in the maxillary anterior region having different types of internal connections and different abutment angulations. Material and methods: Implants with three types of abutment connections, internal conical, Morse taper, and internal hex, were modeled using SolidWorks software. Three abutment angulations of 0, 15, and 30 degrees were used for each type of implant. A 100 N axial load was applied to the implants, and the stresses on the implant, abutment, and bone were analyzed by finite element analysis. Results: Among the straight abutments, the most stress was in model 3A (62.60 MPa). The stress value among angled abutments was highest with 30-degree angled abutments. The value was highest in model 3C (94.83 MPa). Internal hex connection showed the highest stress levels in all degrees of angulation of the abutment, and Morse taper connection showed the least amount of stress in all three abutment angles. The most stress concentration was seen in the cortical bone on the buccal surface in the implant-abutment junction. Conclusion: The Morse taper design of implant exhibited the least-highest stress levels on the alveolar bone. The stress levels increased with the increasing angulation of the implant or implant-abutment connection.
Effect of crown stiffness and prosthetic screw absence on the stress distribution in implant-supported restoration: A 3D finite element analysis

Abstract:This in-silico investigation evaluated the mechanical impact of Morse tape implant-abutment interface and retention system (with and without screw) and restorative materials (composite block and monolithic zirconia) by means of a three-dimensional finite element analysis (3D-FEA). Four 3D models were designed for the lower first molar. A dental implant (4.5 × 10 mm B&B Dental Implant Company) was digitized (micro CT) and exported to computer-aided design (CAD) software. Non-uniform rational B-spline surfaces were reconstructed, generating a 3D volumetric model. Four different models were generated with the same Morse-type connection, but with a different locking system (with and without active screw) and a different crown material made of composite block and zirconia. The D2 bone type, which contains cortical and trabecular tissues, was designed using data from the database. The implants were juxtaposed inside the model after Boolean subtraction. Implant placement depth was simulated for the implant model precisely at crestal bone level. Each acquired model was then imported into the finite element analysis (FEA) software as STEP files. The Von Mises equivalent strains were calculated for the peri-implant bone and the Von Mises stress for the prosthetic structures. The highest strain values in bone tissue occurred in the peri-implant bone interface and were comparable in the four implant models (8.2918e-004–8.6622e-004 mm/mm). The stress peak in the zirconia crown (64.4 MPa) was higher than in the composite crown (52.2 MPa) regardless of the presence of the prosthetic screw. The abutment showed the lowest stress peaks (99.71–92.28 MPa) when the screw was present (126.63–114.25 MPa). Based on this linear analysis, it is suggested that the absence of prosthetic screw increases the stress inside the abutment and implant, without effect on the crown and around the bone tissue. Stiffer crowns concentrate more stress on its structure, reducing the amount of stress on the abutment.

Abstract:The aim of this study was to evaluate the bone tissue effects under dynamic loading using finite element analysis (FEA) for four angled abutments with different deviated palatal lateral tilt angles. A three-dimensional model of the posterior maxillary region and an implant crown model were reconstructed and assembled with a three-dimensional model of the implant, angled abutment, and central screw to create a total of 10 three-dimensional finite element models tilted at 15 ∘ , 20 ∘ , 25 ∘ , and 30 ∘ in three groups, and the dynamic loads simulating oral mastication were loaded on the implant crown to analyze the equivalent stresses and strains in the peri-implant bone tissues. Under the dynamic loading, the cortical bone on the buccal side of the implant neck showed different degrees of stress concentration, and the cortical bone stress was much higher than the cancellous bone, and the strain concentration area of each model was located in the bone tissue around the implant neck and base. For the use of angular abutment, under the premise that the cortical bone stresses and strains of the 10 models meet the requirements for use, the peak stresses of 2.907 MPa, 3.018 MPa, and 2.164 MPa were achieved by using the 20 ∘ angular abutment to achieve the tilt angles of 20 ∘ , 25 ∘ , and 30 ∘ implantation, which is more advantageous compared with other models.

Abstract:OBJECTIVE:To analyze stress distribution in alveolar bone around implants of implant supported overdentures (ISO) with linear occlusion and with anatomic occlusion at lateral mandibular position, and to justify the possibility of decreased injurious force around implants in ISO with linear occlusion.METHODS:Computerized tomography scan and finite element analysis (FEA) were used to set up two 3-D FEA models of maxillae and mandible with severe residual ridge resorption. The mucosa, linear and anatomic occlusal ISO with bar attachments, and two implants inserted between mandibular foramina were also established in the models. With the condition of imitating the loading of masseter muscles, these models were loaded to simulate the stress distributions in alveolar bone around implants under ISO at lateral occlusion position.RESULTS:At lateral occlusion, the stress distributions in alveolar bone around implants under ISO with anatomic occlusion were mainly on the lingual and distal sides of the working side implants. However, stress distributions under ISO with linear occlusion were on the distal sides of bilateral implants. Both the stress peaks of ISOs with linear occlusion and with the anatomic one appeared in the working side. In anatomic occlusion model, sigma(z): -6.47 MPa and 6.81 MPa, sigma(1): -4.20 MPa and 7.20 MPa (negative value: compressive stress, positive value: tensile stress); in linear occlusion model, sigma(z): -4.86 MPa and 3.04 MPa, sigma(1): -3.48 MPa and 5.33 MPa.CONCLUSIONS:At lateral occlusion, when comparing the ISO with two different occlusion schemes, stress peak in alveolar bone around implants in the linear occlusion model was lower than that in the anatomic occlusion model at equal loading situation. Three-Dimensional Finite Element Analysis of Stress Distribution in Dental Implant Prosthesis and Surrounding Bone Using PEEK Abutments

Abstract:(1) Background: Polyetheretherketone (PEEK) has been used as an alternative to titanium in implant prosthetic systems, but its impact on stress distribution in implant systems needs to be investigated. This study aimed to compare the effect of polyetheretherketone (PEEK) and titanium abutments on implant prosthetic systems and the supporting bone using three-dimensional finite element analysis (FEA). (2) Methods: Three-dimensional finite element analysis was conducted using CATIA V5 and Abaqus V6.12 software to model mandibular first-molar implant systems with titanium and PEEK abutments. Under external loading conditions, finite element analysis was conducted for the stresses in the implant components and surrounding bones of each group. (3) Results: The implant fixture of the PEEK model exhibited the highest von Mises stress (VMS). The lowest VMS was observed in the abutment screw of the titanium model. Both implant systems demonstrated similar stress distributions and magnitudes in cortical and cancellous bones. (4) Conclusion: PEEK abutments show a similar stress distribution in the surrounding bone compared to titanium. However, PEEK absorbs the stresses within the implant system and exhibits the highest VMS values due to its low mechanical and physical properties. Therefore, PEEK abutments need improved mechanical properties for better clinical application.
A comparative finite element analysis of titanium, poly-ether-etherketone, and zirconia abutment on stress distribution around maxillary anterior implants

Abstract:Background: The aim of this study was to investigate the influence of abutment material, alveolar bone density, and occlusal forces on stress distribution around maxillary anterior implants. Materials and Methods: An in-vitro study was conducted. The maxillary anterior implant was modeled using a three-dimensional finite element model in D2 and D3 bones with three different abutment materials: titanium, zirconia, and poly-ether-ether ketone (PEEK). Von Mises stress was evaluated after the application of vertical and oblique loads of 100 N, 175 N, and 250 N. Statistical analysis was done by Friedman-Wilcoxon signed-rank test, Mann-Whitney U test, and Kruskal-Wallis test. The probability value <0.05 is considered a significant level. Results: Stress distribution around D3 bone was higher than D2 bone in all the abutment materials with greater values seen in oblique load than vertical load with insignificant difference ( P > 0.05). Statistically insignificant stress values were seen greater in PEEK than titanium or zirconia abutment ( P > 0.05). A statistically significant difference was observed between 100 N and 175 N of load ( P < 0.05). Conclusion: PEEK, zirconia, and titanium as abutment material in the anterior region showed similar properties. The stress on the bone was proportionately increased during the vertical and oblique loads suggesting the influence of mechanical load in crestal bone loss rather than the type of abutment material.
Mechano-adaptive Responses of Alveolar Bone to Implant Hyper-loading in a pre-clinical in vivo model

Abstract:Objectives: Oral implants transmit biting forces to peri-implant bone. In turn, those forces subject peri-implant bone to mechanical stresses and strains. Here, our objective was to understand how peri-implant bone responded to conditions of normal versus hyper-loading in a mouse model. Material and methods: Sixty-six mice were randomly assigned to 2 groups; both groups underwent bilateral maxillary first molar extraction followed by complete healing. Titanium alloy implants were placed in healed sites and positioned below the occlusal plane. After osseointegration, a composite crown was affixed to the implant so masticatory loading would ensue. In controls, the remaining dentition was left intact but in the hyper-loaded (test) group, the remaining molars were extracted. 3D finite element analysis (FEA) calculated peri-implant strains resulting from normal and hyper-loading. Peri-implant tissues were analyzed at multiple time points using micro-computed tomography (µCT) imaging, histology, enzymatic assays of bone remodeling, and vital dye labeling to evaluate bone accrual. Results: Compared to controls, hyper-loaded implants experienced a 3.6-fold increase in occlusal force, producing higher peri-implant strains. Bone formation and resorption were both significantly elevated around hyper-loaded implants, eventually culminating in a significant increase in peri-implant bone volume/total volume (BV/TV). In our mouse model, masticatory hyper-loading of an osseointegrated implant was associated with increased peri-implant strain, increased peri-implant bone remodeling, and a net gain in bone deposition. Conclusion: Hyper-loading results in bone strain with catabolic and anabolic bone responses, leading to a net gain in bone deposition.

Abstract:BACKGROUND: In the routine of dentistry, knowing the biomechanical properties of implant systems and their inherent stress distribution under force loading is an essential step to predict structural damage and biological responses. This study aimed to investigate stress distribution in zirconia and titanium implants and their biomechanical response in alveolar sockets of the anterior region of the maxilla through tridimensional finite element analysis.MATERIAL AND METHODS: From computed tomography scans of a reference patient, three models of the maxillary dental arch were designed with Rhinoceros 5.0 software (McNeel Europe™, Barcelona, Spain). In each model, a dental implant replaced the maxillary left central incisor. The implants consisted of M1) Zirconia Pure Ceramic Implant Monotype; M2) Zirconia Pure Ceramic ZLA; and M3) Titanium Bone Level - Roxolid SLA. Ceramic crowns were installed in all the implants. Implants and prostheses were loaded with 50N oblique and axial forces. Von-Mises and Mohr Coulomb criteria were used to assess stress distribution in the implant systems and perimplantar bone, respectively.RESULTS: Traction was detected in the cervical region of the palatal bone surface of all the models. Oppositely, compression was found in the cervical region of the vestibular bone surfaces.CONCLUSIONS: Zirconia Pure Ceramic Implant Monotype had the best response under oblique force loading. Ceramic implants may be an alternative to replace titanium implants in fresh alveolar sockets in the anterior region of the maxilla. <b>Key words:</b>Finite elements, implants, stress, ceramic, titanium.
