Abstract:As health-care consumers face increasing responsibility for medical care costs, health systems have launched innovative programmes to help patients successfully engage in the financial process from pre-service to final billing. Both national and regional organisations are employing automation, optimised workflows and dedicated patient teams to improve transparency and communicate more personally with patients as they make important financial decisions. This article explores how to leverage processes, technology and automation to give patients the full financial picture, access to affordability tools and the ability to self-serve. A host of important steps were used to measure patient preferences with the desired outcome of enabling patients to make self-service payments. Other target outcomes included moving a significant proportion of patients onto self-service payment plans, with the goal of reducing accounts receivable (A/R) days and collection costs while improving patient satisfaction. Also envisioned were digital patient engagement tools that would allow patients to view their entire financial picture, understand costs, self-enrol in payment plans and access critical information, providing convenience and ease of access and making it easier for patients to financially engage while improving the overall patient experience.

Abstract:Caritas Norwood Hospital and its affiliates went from $17 million in the red to a $4 million profit after implementing comprehensive financial and operational strategies, which included reengineering the revenue cycle, outsourcing some services, and focusing on the hospital's core business. External pressures were out of the hospital's control, but a comprehensive analysis identified many internal problems that the hospital had to resolve. The new management team also uncovered many undisclosed problems when it took the reins.
Identification and proposed control of helicopter transmission noise at the source

Abstract:Helicopter cabin interiors require noise treatment which is expensive and adds weight. The gears inside the main power transmission are major sources of cabin noise. Work conducted by the NASA Lewis Research Center in measuring cabin interior noise and in relating the noise spectrum to the gear vibration of the Army OH-58 helicopter is described. Flight test data indicate that the planetary gear train is a major source of cabin noise and that other low frequency sources are present that could dominate the cabin noise. Companion vibration measurements were made in a transmission test stand, revealing that the single largest contributor to the transmission vibration was the spiral bevel gear mesh. The current understanding of the nature and causes of gear and transmission noise is discussed. It is believed that the kinematical errors of the gear mesh have a strong influence on that noise. The completed NASA/Army sponsored research that applies to transmission noise reduction is summarized. The continuing research program is also reviewed.

Automated charge capture at the point of care increases revenue

Abstract:Group practices can reduce lost charges, denied claims, and time-to-billing rates by using automated charge capture at the point of service. One orthopedic department had been experiencing many process-related problems associated with physician charge capture. To improve revenues, the physicians began using hand-held devices to input data during patients' office visits. A 12-month analysis of charge data from the orthopedic physicians' outpatient visits using a paper-based charge process showed that 824 outpatient charges had been missed or lost, representing 6 percent of the department's annual outpatient activity and more than $54,000 in projected additional revenue. Another analysis conducted after the automated charge-capture process at the point of care was implemented showed that the orthopedic department had not missed any charges.

Abstract:Objective: This study aimed to understand how different communication strategies influence patients' behaviour in paying unpaid hospital bills. The research focused on a healthcare system where patients have to pay a significant portion of their medical costs out-of-pocket. Methods: The research was conducted in collaboration with a debt collection agency in Latvia. The field experiment involved 9196 individuals with unpaid hospital bills. These individuals received randomly assigned reminders through mobile text messages and e-mails. The study compared the effectiveness of personalized messages, which included the recipient's name, with generic reminders and messages that appealed to social norms or public good contributions. Results: The findings revealed that personalized messages, specifically those that included the recipient's name, significantly improved payment rates compared with generic reminders. Conversely, messages that used social norms or public good appeals did not have a significant impact on payment rates. Conclusions: The study highlights the importance of personalized communication strategies in improving hospital debt collection. Even simple, cost-effective modifications in communication, like adding the recipient's name, can significantly enhance payment compliance. This approach not only keep the financial books of healthcare providers balanced but also suggests that personalized strategies can be extended to other areas of healthcare management. However, while these findings are promising, they indicate that more personalized and nuanced communication strategies are needed to address the broader issue of unpaid hospital bills effectively.

Abstract:Lutheran Medical Center (LMC) was a failing hospital in a depressed Brooklyn neighborhood until administrators and board members reassessed their purpose and goals. Now LMC is a community-oriented organization that offers acute hospital care for an urban population in excess of 350,000 people, provides cost-effective healthcare to more than 50,000 ambulatory patients a year, and has helped to revitalize its neighborhood. LMC's surprising range of accomplishments and activities include the following: Providing staff members to serve as resources and support staff for community groups; Organizing a neighborhood redevelopment committee, obtaining and administering grants, providing office space, and acting in an advisory capacity until the committee was able to separately incorporate; Creating an educational program for undereducated and underemployed area residents; Establishing a school health program to handle unmet routine healthcare needs of area children; Renovating an abandoned factory on the worst block in the community and relocating the hospital to that site; Founding the country's largest federally supported neighborhood health center; Signing a contract with New York state to provide healthcare for 10 percent less than the average cost of care for Medicaid patients.

Abstract:In the January 23,1995, issue of theARCHIVES, Becker and Ansell 1 present a synopsis of the third American College of Chest Physicians Consensus Conference on Antithrombotic Therapy. In the clinical guidelines presented, they recommended the routine use of subcutaneous heparin therapy for the prevention of venous thromboembolism in patients with ischemic stroke. For the management of acute cardioembolic stroke, they recommend intravenous heparin. However, this advice is not supported by the available evidence. Abstract:In 2008, Mayo Clinic in Jacksonville, Florida, developed the preoperative evaluation (POE) clinic under the department of anesthesiology to provide preoperative history and physical examination, and medical optimization. Over time, the POE clinic expanded to accommodate more than 90% of surgical patients, outgrowing the initial practice model. The increased patient volume with shortened turnaround times bottlenecked patient access. A multidisciplinary quality improvement team used Define, Measure, Analyze, Improve, and Control methodology to understand the issues, test potential solutions, and develop sustainable processes. With progressive Plan-Do-Study-Act cycles, it moved from small internal tests of change to implementation throughout the institution. Patient access improved by 14% ( P < .001) and triage efficiency by 30% ( P < .001). These elements led to a 14% improvement in operating margin and a 24% improvement in staff satisfaction. Sustainability was ensured with an accessible dashboard of performance indicators.

Abstract:Over the decades, hospitals have slowly evolved from humanitarian foundations to business ventures and the impact of this dramatic shift can be seen in modern day healthcare. In the United States, hospitals typically establish arbitrary pricing methods, instruct staff to offer the highest priced services, and demand intense labor from residents without offering a fair compensation. These changes in vision and action have resulted in an ample number of Americans in tremendous medical debt. Incorporating evidence from reviews, personal anecdotes, and research journals, this manuscript demonstrates that hospitals are run with the intention of generating the most profit and revenue rather than with the noble purpose of philanthropy from which the idea of hospitals were first conceived. It argues that hospitals have long benefitted from generous tax breaks, there is a need for an intervention of checks and balances from policymakers, and for more reasonable compensations for hospital staff's strenuous labor.

Abstract:Recent U.S. policies require hospitals and health insurers to post payer-specific negotiated prices for services. Few studies, if any, have thoroughly investigated the accuracy of these payer-specific prices for individual hospitals. Our aim was to evaluate the concordance of prices using three datasets: 1) hospital price transparency, 2) payer price transparency data, and 3) Electronic Remittance Advice (ERA) claims data, reflecting actual payments. Evaluating transthoracic echocardiograms at 22 top-rated hospitals, we found high variability in prices and low concordance between datasets, as less than 30% of claim payments had a concordant price in the price transparency datasets. Deriving accurate price estimates for patients requires precise matching of the insurance plan, place of service, and fee type, all of which should be included in standardized reporting. This process highlights the complexity of insurance contracting, and it is not surprising that there is some disagreement in prices. Regardless, we are hopeful that highlighting variation and complexity in prices is a step towards a better system of healthcare payments in the United States.

Abstract:Introduction: The University of Florida Equal Access Clinic Network (EACN) is the largest student-run free clinic (SRFC) network in Florida. This student-driven, continuous quality improvement (CQI) project is intended to decrease total patient visit length at Eastside clinic, one of EACN's primary care sites. The original median visit length of 126.25 minutes represented a significant time burden for patients, especially those with limited transportation or inflexible schedules. Methods: Over six months, four Plan-Do-Study-Act (PDSA) cycles were implemented. PDSA cycle 1 increased personnel and space for taking vitals. PDSA cycle 2 reduced redundancy in the intake process. PDSA cycle 3 triaged patients to match patient complexity with student experience level. PDSA cycle 4 introduced "nudge" interventions to reinforce clinic flow. Total patient visit length and time spent at each step of clinic flow were recorded anonymously for each patient visit. The median visit length per week was tracked on a run chart. Results: From PDSA cycle 1 through PDSA cycle 4, the median visit length decreased from 126 minutes to 114 minutes. This shift was primarily driven by a decrease in the length of patient intake from a median of 19 minutes to 9 minutes. The run chart did not show clear trends until PDSA cycle 4, which demonstrated a strong downward trend. Conclusion: This study demonstrated the ability of a student-driven CQI model to decrease patient visit length in an SRFC setting. Similar models could be used to address this and other contributors to patient experience across SRFCs nationwide.
