Abstract:This study aimed to evaluate the effect of direct restorations using unidirectional glass fiber orientations and a short-fiber-reinforced composite (SFRC) on the fracture resistance of endodontically treated premolars with mesio-occluso-distal cavities. Ninety double-rooted premolars were selected. Fifteen teeth were left intact/as a control group. The endodontic treatment and cavity preparations of seventy-five teeth were performed and divided into five experimental groups: Resin composite (RC), modified transfixed technique + RC, circumferential technique + RC, cavity floor technique + RC, and SFRC + RC. All teeth were fractured under oblique static loading at a 30° angle using a universal testing machine. The fracture patterns were observed and classified. Data were analyzed with one-way analysis of variance, Pearson chi-square, and Tukey HSD post hoc tests (p = 0.05). The highest fracture strength values were obtained in intact teeth (599.336 N), followed by modified transfixed + RC treated teeth (496.58 N), SFRC + RC treated teeth (469.62 N), RC (443.51 N), circumferential + RC treated teeth (442.835 N), and cavity floor + RC treated teeth (404.623 N) (p 0.05). Unrepairable fractures were observed at low rates (20%) in the modified transfixed + RC and SFRC + RC teeth, and at higher rates in RC (73.3%), cavity floor + RC (60%), and circumferential + RC (80%) teeth. The application of an SFRC or the modified transfixed technique yielded an improved fracture strength and the fracture pattern of ETPs being restored with a universal injectable composite.

Effects of the lining material, thickness and coverage on residual stress of class II molar restorations by multilayer technique

Abstract:Objective: This study aims to analyse the influence of different lining material, thickness and coverage on residual stress of class II molar restoration by the multilayer technique through the three-dimensional (3D) finite element (FE) method. The objective is to reveal the correlations between the base layer configurations and the residual stress distributions of the tooth. Methods: A 3D reconstructed model of an extracted first molar was built through micro-CT images and a class II mesio-occlusal-distal cavity was prepared using computer-aided engineering (CAE) software. A bilayer technique was then applied, and 3D FE analyses were performed under polymerization contraction loading. Glass ionomer composites (Vitrebond and Ionosit) and flowable composite resin (Luxa Flow and Z350 Flowable Restorative) were used as lining materials. Moreover, several sensitivity analyses with dozens of hypothetical lining materials were conducted to provide definitive results. The thickness of the base layers was set to be distinct (0.5 mm or 1 mm) so as to reveal its effect on the stress alleviation. Various lining strategies with the liner covering different areas of the cavity walls (Closed Sandwich, Open Sandwich and Special Open Sandwich) were adopted to determine the effects of the base layers. Results: Adoption of an appropriate liner could effectively reduce the polymerization shrinkage stress. The mechanical properties of the base layer can affect the residual stresses, basically a lower Young's modulus and lower Poisson's ratio (of lining material) result in better stress mitigation, therefore reducing the stress transmitted to the dentin. Increase of the lining thickness within a certain range could lead to a decrease in the probability of stress concentration formation. Lining strategies had the strongest influence on the stress distributions. Different lining coverage could cause various stress responses, and covering all cavity walls with the lining material had the optimal performance among all the simulations in this study. Significance: The multilayer technique is an effective way to prolong the service life of resin composites restorations. A thorough evaluation of the sandwich technique through the FE method can provide a better understanding of the stress distributions of the restoration, and reveal its internal mechanisms.
Fracture resistance and marginal gap formation of post-core restorations: influence of different fiber-reinforced composites

Abstract:Objectives: The aim was to explore the fracture behavior and marginal gap within the root canal of endodontically treated (ET) premolars restored with different fiber-reinforced post-core composites (FRCs). Further aim was to evaluate the composite curing at different depths in the canal. Materials and methods: Eighty-seven intact upper premolars were collected and randomly divided into six groups. After endodontic procedure, standard MOD cavities were prepared and restored with their respective fiber-reinforced post-core materials: group 1: prefabricated unidirectional FRC-post + conventional composite core; group 2: prefabricated unidirectional FRC-post + short fiber composite (SFRC) core; group 3: individually formed unidirectional FRC-post + conventional composite core; group 4: randomly oriented SFRC directly layered as post and core; group 5: individually formed unidirectional FRC + randomly oriented SFRC as post and core. After restorations were completed, teeth (n = 3/group) were sectioned and then stained. Specimens were viewed under a stereo microscope and the percentage of microgaps within the root canal was calculated. Fracture load was measured using universal testing machine. Results: SFRC application in the root canal (groups 4 and 5) showed significantly higher fracture load (876.7 N) compared to the other tested groups (512-613 N) (p < 0.05). Post/core restorations made from prefabricated FRC-post (group 1) exhibited the highest number of microgaps (35.1%) at the examined interphase in the root canal. Conclusions: The restoration of ET premolars with the use of SFRC as post-core material displayed promising performance in matter of microgap and load-bearing capacity. Clinical significance: Fracture resistance of ET premolar restored by bilayered composite restoration that includes both SFRC as post-core material and surface conventional resin seems to be beneficial.
Effects of 3 different residual root treatments after post-and-core restoration: An in vitro fracture resistance experiment and finite element analysis

Abstract:Statement of problem: Crown-lengthening procedures (CLPs) and orthodontic eruption procedures (OEPs) improve the biomechanical properties of residual root restoration. However, their use is limited by clinic time, cost, and crown-root ratio. An inner shoulder retention form (ISRF) overcomes these limits; however, whether ISRF meets the biomechanical requirements is unclear. Purpose: The purpose of this in vitro and finite element analysis (FEA) study was to evaluate the effects of 3 residual root treatments (ISRF, CLP, and OEP) on premolar fracture resistance and stress distribution after post-and-core restoration. Material and methods: Thirty-two extracted mandibular second premolars were screened and randomly divided into 4 groups (n=8): a 2-mm ferrule restoration group (NPR2; control group) and 3 experimental groups (0.5×0.5-mm ISRF [ISRF0.5], 2-mm CLP [CLP2], and 2-mm OEP [OEP2]). After removal of the crown, endodontic treatment, and embedding and restoration, the specimens were loaded on a universal tester (crosshead speed of 0.1 mm per minute) at 30 degrees to the long axis of the tooth until fracture. Fracture resistance and pattern were analyzed. Four groups were added in FEA: 1-mm ferrule restoration group (NPR1), 1.0×1.0-mm ISRF (ISRF1), 1-mm CLP (CLP1), and 1-mm OEP (OEP1). The maximum principal stress peak (σmax), maximum displacement, and stress distribution were evaluated. Results: Mean fracture load ±standard deviation was 796.23 ±155.61 N (NPR2), 650.32 ±150.43 N (ISRF0.5), 385.38 ±149.92 N (CLP2), and 542.93 ±79.34 N (OEP2); these differences were statistically significant (F=12.724; P<.001). The main fracture patterns were decementation from the lingual crown margin and oblique root fracture in ferrule restoration groups and post-and-core decementation and oblique fracture starting from the middle proximal surface in the ISRF groups without ferrules. In FEA, the results of NPR2, ISRF0.5, CLP2, and OEP2 were consistent with the in vitro experiments and provided stress distribution and displacement information. Conclusions: Teeth with crown dentin ferrules showed higher fracture resistance and lower stress concentration. For residual roots, ISRF and OEP showed no differences, but CLP had lower fracture resistance and higher stress concentration.
Effect of surface treatments on fracture resistance of root filled teeth with bonded fibre posts

Abstract:Background/aim: This study examined the effect of several dentin surface treatments on fracture resistance of root filled teeth containing bonded fibre posts. Material and methods: The roots of 84 single-canal premolars were instrumented, filled with gutta-percha and randomly divided into seven groups. The coronal 9 mm of gutta-percha was removed to create a post space. In the experimental groups, canals were irrigated with distilled water, 37% phosphoric acid, 5% sodium hypochlorite, 17% EDTA, 5% sodium hypochlorite followed by 17% EDTA or 5% sodium hypochlorite followed by 10% ascorbic acid. Fibre posts were cemented using Panavia F2.0 resin cement, and the specimens were mounted in acrylic resin blocks. Following 1-week storage in an incubator, specimens were thermocycled and their fracture resistance was determined using a universal testing machine. Data were analysed using the Kruskal-Wallis test. Results: Mean fracture resistance values ranged from 465.8 N in specimens treated with sodium hypochlorite and ascorbic acid to 739.1 N for those treated with sodium hypochlorite only. However, differences in fracture resistance were not statistically significant (P = 0.114). Conclusions: None of the surface treatments improved vertical fracture resistance of root filled teeth containing bonded fibre posts.

Abstract:We describe the construction of expression vectors based on three of the most frequently used gene fusion affinity tags [glutathione S-transferase (GST), maltose binding protein (MBP), and the His6 peptide]. The polylinkers of pGEX4T1, pMal-c2, and a pET vector were replaced with the polylinker isolated from the baculovirus expression plasmid pFastBac. Once appropriate restriction sites have been introduced into a gene, it can be fused to all three affinity tags with little effort, allowing expression-screening experiments to be performed efficiently. We discuss the development and use of these vectors with respect to overcoming purification problems encountered for the RhoA GDP/GTP nucleotide dissociation inhibitor (RhoGDI) and their advantages over commercially available expression vectors.
In Vitro Effect of the Length of Relocated Cervical Margin with Casting Post and Core, Prefabricated Fiber Post, and Polyethylene Fiber with a Composite Core on Fracture Resistance and Marginal Integrity

Abstract:Objective: This study aimed to assess the effect of length of the relocated cervical margin with casting post and core (CP), prefabricated fiber post and composite core (PFP), and polyethylene fiber-reinforced composite (PEFRC) on fracture resistance and marginal integrity. Materials and methods: In this in vitro study, 70 sound human maxillary premolars were divided into seven groups according to the type of post and core system and length of the relocated cervical margin (n = 10): control (no preparation), PFP-3, PEFRC-3, CP-3 with 3 mm of cervical margin relocation (CMR), PFP-6, PEFRC-6, and CP-6 (with 6 mm of CMR). The samples were restored with zirconia crowns (except the control group). Epoxy resin replicas were fabricated before and after thermomechanical loading. Marginal integrity was assessed at the luting cement-core, core-tooth, and luting cement-enamel interfaces under a scanning electron microscope (SEM) (×200). Fracture resistance and failure mode were subsequently assessed. Data were analyzed by independent t-test, paired t-test, ANOVA, Tukey-Games Howell, Mann-Whitney, Kruskal-Wallis, Wilcoxon, Mann-Whitney with Bonferroni correction, and Fisher-Freeman-Halton tests (α = 0.05). Results: The marginal integrity of the groups with 3 mm of CMR followed the following order: PEFRC > PFP > CP at all interfaces (P < 0.05). In 6-mm CMR groups, this order was CP < PFP = PEFRC at the luting cement-core and (CP < PEFRC) = PFP at the core-tooth interface. No significant difference was found in fracture resistance when comparing the 3-mm CMR groups with each other (P > 0.05). PFP-6 showed higher FR than CP-6 (P < 0.001). PEFRC-6 had no significant difference with PFP-6 and CP-6 (P > 0.05). In each post and core system, 3-mm CMR groups showed higher marginal integrity and fracture resistance (P < 0.05). Conclusion: Increasing the length of the relocated cervical margin decreased the marginal integrity and fracture resistance of all three systems of CP, PFP, and PEFRC.
Comparative Evaluation of Fracture Resistance of Composite Core Buildup Materials: An In Vitro Study

Abstract:Aim This study aimed to compare the fracture resistance of different materials used in composite core buildups, including conventional filler composite, nanofiller composite, and short fiber-reinforced composite (SFRC). Methods This in vitro study was conducted on 30 freshly extracted premolars. The teeth were treated using a uniform endodontic procedure, and Fiber Posts (REFORPOST, Angelus) were placed. The teeth were then divided into three groups and restored using different materials. Group 1 was restored using SFRC (everX Posterior, GC, Europe), Group 2 using microfiller composite (Te-Econom Flow, Ivoclar Vivadent), and Group 3 using nanofiller composite (Tetric N-Flow, Ivoclar Vivadent). The restoration materials were then light-cured for 40 seconds. The teeth were placed in a Universal Testing Machine (Instron) and a load was applied with a stainless-steel ball (4 mm diameter) until the tooth fractured. The fracture load for each tooth was recorded, and after the mechanical test, the experimental groups were examined for failure modes. Statistical analysis was performed using SPSS version 21.0 software. A one-way ANOVA test was conducted to compare more than two groups, followed by Tukey's test for post hoc pairwise comparison. Results The mean fracture resistance of the microfiller composite (346.94±44.63) was the lowest among the three groups. When analyzed using Tukey's test at p<0.05, fracture resistance was significantly higher in the SFRC, followed by nanofillers and microfiller composites. Conclusion Due to the increasing demand for aesthetic restorations in recent years, composites have become important in modern restorative dentistry. The development and implementation of composite dental restorative materials rely on a comprehensive understanding of each composite component and consideration of methods for modifying each component. As a result, the findings of this study will be beneficial in determining which material to use based on specific cases.

Abstract:This study compared the effectiveness of 0.5% ropivacaine and 0.5% bupivacaine for brachial plexus block. Forty-eight patients received a subclavian perivascular brachial plexus block for upper-extremity surgery. One group (n = 24) received ropivacaine 0.5% (175 mg) and a second group (n = 24) received bupivacaine 0.5% (175 mg), both without epinephrine. Onset times for analgesia and anesthesia in each of the C5 through T1 brachial plexus dermatomes did not differ significantly between groups. Duration of analgesia and anesthesia was long (mean duration of analgesia, 13-14 h; mean duration of anesthesia, 9-11 h) and also did not differ significantly between groups. Motor block was profound, with shoulder paralysis as well as hand paresis developing in all of the patients in both groups. Two patients in each group required supplemental blocks before surgery. Ropivacaine 0.5% and bupivacaine 0.5% appeared equally effective in providing brachial plexus anesthesia.
Fracture Resistance in Fibre-Reinforced Resin Composite Restorations in Deciduous and Permanent Molars: An Ex Vivo Study

Abstract:Aim: The aim of this study was to compare the fracture resistance levels of restored deciduous teeth and permanent molars restored with different materials, including ultra-polyethylene fibre tape (Ribbond-Ultra), fibre-reinforced resin composite EverX posterior, fibre-reinforced flowable resin composite EverX Flow and bulk-fill flow restorative material (Tetric N-flow) in the posterior region. Methods: We tested sixty-four caries-free human mandibular molars (32 deciduous molars and 32 permanent molars). Deep and wide Class I cavities were prepared in each tooth. The teeth were embedded in poly(methyl methacrylate) resin up to the cementoenamel junction, leaving the crown completely exposed. Oral temperature fluctuations were then simulated using two thousand thermocycling cycles, each lasting 30 seconds in the hot phase (approximately 35 °C). The loading rate for our force-fracture tests was set at 0.5 mm/min. Direct restorations were made using a spherical tip and applying the force perpendicular to the sample surface. Visual inspection of the fractured specimens, in combination with adhesive scanning electron microscopy (SEM) and finite element analysis (FEA), provided detailed insights into the failure modes and stress distribution at the restoration-tooth interface. Results: Teeth restored with fibre-reinforced composite (EverX Posterior) had the highest fracture resistance, followed by fibre-reinforced flowable resin composite (EverX Flow). Teeth restored with the ultra- polyethylene fibre tape (Ribbond-Ultra), followed by the flowable bulk-fill composite (Tetric N-flow) had the lowest resistance. Statistical analysis revealed significant differences between the groups, except for EverX Posterior and EverX Flow. A predictive algorithm was also proposed for the likelihood of restoration failure. Conclusion: Modern fibre-reinforced resin composites, such as EverX Posterior, effectively reinforce teeth against fractures, with success in both restored deciduous and permanent molars. Meanwhile, the use of polyethylene fibre tapes is less effective, and they involve a time-consuming application.
Mechanical properties and fracture behavior of flowable fiber reinforced composite restorations

Abstract:OBJECTIVE: The aim was to evaluate the effect of short glass-fiber/filler particles proportion on fracture toughness (FT) and flexural strength (FS) of an experimental flowable fiber-reinforced composite (Exp-SFRC) with two methacrylate resin formulations. In addition, we wanted to investigate how the fracture-behavior of composite restorations affected by FT values of SFRC-substructure.METHODS: Exp-SFRC was prepared by mixing 50wt% of dimethacrylate based resin matrix (bisGMA or UDMA based) to 50wt% of various weight fractions of glass-fiber/particulate filler (0:50, 10:40, 20:30, 30:20, 40:10, 50:0wt%, respectively). FT and FS were determined for each experimental material following standards. Specimens (n=8) were dry stored (37°C for 2 days) before they were tested. Four groups of posterior composite crowns (n=6) composed of different Exp-SFRCs as substructure and surface layer of commercial particulate filler composite were fabricated. Crowns were statically loaded until fracture. Failure modes were visually examined. The results were statistically analysed using ANOVA followed by post hoc Tukey's test.RESULTS: ANOVA revealed that ratio of glass-fiber/particulate filler had significant effect (p<0.05) on tested mechanical properties of the Exp-SFRC with both monomer systems. Exp-SFRC (50wt%) had significantly higher FT (2.6MPam1/2) and FS (175.5MPa) (p<0.05) compared to non-reinforced material (1.3MPam1/2, 123MPa). Failure mode analysis of crown restorations revealed that FT value of the substructure directly influenced the failure mode.SIGNIFICANCE: This study shows that short glass-fibers can significantly reinforce flowable composite resin and the FT value of SFRC-substructure has prior importance, as it influences the crack arresting mechanism.

Abstract:Purpose To evaluate the impact of various preparation designs and the material type on fracture resistance of minimally invasive posterior indirect adhesive restorations after aging using a digital standardization method. Materials and Methods One‐hundred sixty human maxillary premolars free from caries were assigned into 16 groups (n = 10): bevel design on enamel substrate with mesial box only (VEM), butt joint design on enamel substrate with mesial box only (BEM), bevel design on enamel substrate with mesial and distal box (VED), butt joint design on enamel substrate with mesial and distal box (BED), bevel design on dentin substrate with mesial box only (VDM), butt joint design on dentin substrate with mesial box only (BDM), bevel design on dentin substrate with mesial and distal box (VDD), and butt joint design on dentin substrate with mesial and distal box (BDD). Each group was restored with pressable lithium disilicate (LS2) or disperse‐filled polymer composite (DPC) materials. Adhesive resin cement was used to bond the restorations. Abstract:Aims: To compare and evaluate the bonding ability of bulk-fill resin composite (RC) to four different liners: Biodentine (BD), TheraCal (TLC) - a novel resin-modified calcium silicate cement, resin-modified glass ionomer cement (RMGIC) and conventional glass ionomer cement (GIC) using a universal silane-containing adhesive and characterizing their failure modes.Materials and Methods: Forty extracted intact human premolars with occlusal cavity (4-mm diameter and 2-mm height) were mounted in acrylic blocks and divided into four groups of (n=10 samples) each based on the liner used as group I; (BD), group II; (TLC), group III; (RMGIC) and Group IV; (GIC). Bulk-fill composite buildup of 3 mm diameter and 5 mm height was then bonded to each sample using universal adhesive. Shear bond strength (SBS) analysis was performed using materials testing machine at a cross-head speed of 0.5 mm/min. Statistical Analysis: Statistical analysis was performed with one-way analysis of variance (ANOVA) and Tukey’s post hoc test for numerical data while chi square test for categorical one. Results: One-way analysis with ANOVA revealed significant difference in bond strength values between the different groups (p<0.001). The observed modes of failure were predominantly cohesive in Biodentine, TheraCal and GIC groups while RMGIC showed majority of mixed and minority adhesive failures.Conclusions: Biodentine demonstrated lower bond strength values when immediately bonded to bulk-fill resin composite compared with RMGIC, TheraCal and GIC groups

Materials and Methods. Forty freshly extracted maxillary premolar teeth with comparable sizes were used in this in vitro study. Each tooth received mesio-occluso-distal cavity preparation (3 mm width and 6 mm depth) followed by endodontic treatment. Canals were instrumented with RACE EVO rotary files (FKG, Dentaire, Switzerland) up to MAF 25/.06. Canals were obturated using a single cone technique, then the teeth were divided arbitrarily into five groups (n = 8): Group A: direct composite resin only using a centripetal technique, Group B: direct composite resin with glass fiber post, Group C: direct composite resin with short fiber-reinforced composite (everX Flow), Group D: direct composite resin with leno wave ultra high molecular weight polyethylene (LWUHMWPE) fibers placed on cavity floor, and Group E: direct composite resin with LWUHMWPE fibers placed circumferentially around the cavity walls (wallpapering technique). The teeth were then stored in distilled water at 37°C for 24 hr. The fracture resistance of each sample was measured using a universal testing machine in Newton (N). The data were analyzed using one-way analysis of variance (ANOVA) and the Bonferroni test with a significance level of 0.05. Results. Group E recorded the maximum mean of fracture load (2,139.375 N), while Group A recorded the minimum mean of fracture load (689.6250 N). The one-way ANOVA test revealed a significant difference between the groups. The Bonferroni test showed a significant difference between each two groups, with the exception of those between Groups B and C and between Groups D and E, where there were no statistically significant differences ( p > 0.05 ). Conclusion. Restoration of endodontically treated teeth using the wallpapering technique recorded the highest mean of fracture resistance with a repairable mode of fracture.
Impaired amyloid β-degrading enzymes in brain of streptozotocin-induced diabetic rats.

Abstract:Enzymes that degrade the amyloid β-peptide (Aβ) are important regulators of cerebral Aβ levels. High level of Aβ was found in the brain of diabetic patients and diabetic animals. Aim of the study was to investigate whether activities of Aβ-degrading enzymes neprilysin (NEP), endothelin-converting enzyme 1 (ECE-1) and insulin-degrading enzyme (IDE) were impaired in the brain of diabetic rats. Diabetes was induced in rats by ip administration of 65 mg/kg streptozotocin. The temporal cortex and hippocampus were obtained for activity and mRNA level assays of the three enzymes on the 35th day after induction. ECE-1 activity was significantly decreased both in the hippocampus and cortex of diabetic rats, while for IDE significantly lower activity occurred only in the cortex. NEP activity was slightly decreased in both brain regions. The hippocampus of diabetic rats showed significant decrease in mRNA levels of NEP and ECE-1 and moderate increase in IDE mRNA level. The cortex of diabetic rats showed slight decrease in mRNA levels of the three enzymes. The results indicated that the three Aβ-degrading enzymes were damaged to different extents in the brain of diabetic rats, and impairment of ECE-1 and IDE partly contributed to the elevated Aβ(1-40) levels in brain of diabetic rats.
Post-Fatigue Fracture and Marginal Behavior of Endodontically Treated Teeth: Partial Crown vs. Full Crown vs. Endocrown vs. Fiber-Reinforced Resin Composite

Materials

Abstract:Objectives: To investigate in vitro post-fatigue fracture behavior of endodontically treated molars having been differently restored. Methods: A total of 120 extracted human molars were used. A total of 120 specimens in 14 test groups and one control group (n = 8) were root canal treated. After postendodontic sealing and build-up (AdheSE Universal, SDR), additional MOD preparations were cut. Postendodontic restorations were: Direct restorations (Tetric EvoCeram BulkFill bonded with AdheSE Universal and EverX Posterior/Essentia bonded with G-Premio Bond; as filling (F) or direct partial crown (DPC) after reducing the cusps 2 mm; indirect adhesive restorations (partial crown/PC vs. full crown/FC): e.max CAD, Celtra Duo, both luted with Variolink Esthetic; indirect zirconia restorations (partial crown/PC vs. full crown/FC), luted with RelyX Unicem 2; indirect non-bonded cast gold restorations (partial crown/PC vs. full crown/FC; Degunorm), luted with Ketac Cem. Before and after 300,000 thermocycles (5/55 °C) and 1.2 Mio. A total of 100 N load cycles, replicas were analyzed under a SEM for marginal quality in enamel and dentin (where applicable) and finally, specimens were loaded until fracture. Results: In direct groups, there was no difference between RC and FRC in fracture strength (p > 0.05); however, direct partial crowns showed higher post-fatigue fracture resistance. Regarding marginal quality, intracoronal FRC restorations exhibited more gap-free margins in enamel than RC. In the indirect groups, there was no significant difference between partial and full crowns in any of the adhesively luted ceramic groups regarding post-fatigue fracture resistance. Zirconia partial crowns exhibited significantly lower marginal quality in enamel. Indirect groups performed significantly better than direct groups in fracture resistance. Within the indirect restorations, both cast gold groups and zirconia full crowns exhibited the highest fracture resistance being superior to control teeth. Significances: Within the limits of this in vitro investigation, it can be concluded that any kind of indirect restoration with cusp replacement is suitable for ETT restoration when a certain cavity extension is exceeded. All indirect restorations, i.e., endocrowns, partial crowns, and full crowns showed a promising performance after in vitro fatigue-loading.

Abstract:To evaluate the impact of using ion-releasing liners on the 3-year clinical performance of posterior resin composite restorations after selective caries excavation with polymer burs. 20 patients were enrolled in this trial. Each patient had two deep carious lesions, one on each side of the mouth. After selective caries removal using polymer bur (PolyBur P1, Komet, Brasseler GmbH Co. KG, Lemgo, Germany), cavities were lined with bioactive ionic resin composite (Activa Bioactive Base/Liner, Pulpdent, Watertown, MA, USA) or resin-modified glass ionomer liner (Riva Light Cure, SDI, Bayswater, Victoria, Australia). All cavities were then restored with nanofilled resin composite (Filtek Z350XT, 3M Oral Care, St. Paul, MN, USA). All the tested materials were placed according to the manufacturers' instructions. Clinical evaluation was accomplished using World Dental Federation (FDI) criteria at baseline and after 6 months, 1, 2, and 3 years. Data were analyzed using Mann–whitney U and Friedman tests (p 0.05). Resin composite restorations showed acceptable clinical performance over 3 years either lined with bioactive ionic or resin-modified glass ionomer liners after selective caries excavation preserving pulp vitality. After the 3-year follow-up period, Activa Bioactive and Riva Light Cure liners were clinically effective and they exhibited with the overlying composite restorations successful clinical performance.

Abstract:To investigate the influence of post length and amount of remaining root tissue on the fracture resistance of roots restored with fibre posts relined with resin composite. Ninety upper canine teeth were divided into nine groups (n = 10). The post spaces were prepared resulting in different lengths, as follows: group 2/3, preparations with lengths of 10 mm; group 1/2, preparations with lengths of 7·5 mm; and group 1/3, preparations with lengths of 5 mm. Each group was divided into 3 subgroups according to amount of remaining root tooth tissue (2, 1 mm or 0·5 mm of thick root). Fibre posts relined with resin composite were cemented, and all teeth were restored with metal crowns. The samples were submitted to the fracture resistance test in a universal testing machine, at an angle of 135° and speed of 0·5 mm min(-1) . Failure modes were observed and the data of fracture resistance were submitted to the anova and Tukey's (α = 0·05). No statistically significant difference in fracture resistance was found among different post lengths (P > 0·05). Remaining dentin thickness of 2 and 1 mm did not differ statistically in fracture resistance (P > 0·05), which was higher than of 0·5 mm dentin thickness (P < 0·05). A prevalence of repairable failure was observed in all groups. It can be concluded that the length of fibre post relined with resin composite did not influence fracture resistance, but thickness was an important factor for the restoration of endodontically treated teeth.
