Abstract:Observation and characterization of any changes in anatomical structures of ocular components remain as a conventional technique for diagnosis, staging, therapeutic treatments, and post-treatment monitoring of any ophthalmic disorders. The existing technologies fail to provide imaging of all of the various components of the eye simultaneously at one scanning time, i.e., one can recover vital patho-physiological information (structure and bio-molecular content) of the different ocular tissue sections only one after another. This article addresses the longstanding technological challenge by use of an emerging imaging modality [photoacoustic imaging (PAI)] in which we integrated a synthetic aperture reconstruction technique (SAFT). Experimental results-with experiments being conducted in excised tissues (goat eye)-demonstrated that we can simultaneously image the entire structure of the eye (∼2.5 cm) depicting clearly the distinctive ocular structures (cornea, aqueous humor, iris, pupil, eye lens, vitreous humor, and retina). This study uniquely opens an avenue for promising ophthalmic (clinical) applications of high clinical impact.

Abstract:Ultrasound A-scan is an important tool for quantitative assessment of ocular lesions. However, its usability is limited by the difficulty of accurately localizing the ultrasound probe to a lesion of interest. In this study, a transparent LiNbO3 single crystal ultrasound transducer was fabricated, and integrated with a widefield fundus camera to guide the ultrasound local position. The electrical impedance, phase spectrum, pulse-echo performance, and optical transmission spectrum of the ultrasound transducer were validated. The novel fundus camera-guided ultrasound probe was tested for in vivo measurement of rat eyes. Anterior and posterior segments of the rat eye could be unambiguously differentiated with the fundus photography-guided ultrasound measurement. A model eye was also used to verify the imaging performance of the prototype device in the human eye. The prototype shows the potential of being used in the clinic to accurately measure the thickness and echogenicity of ocular lesions in vivo.

Abstract:Previously, we described a large Dutch family with hereditary vascular retinopathy (HVR), Raynaud's phenomenon and migraine. A locus for HVR was mapped on chromosome 3p21.1-p21.3, but the gene has not yet been identified. The fact that all three disorders share a vascular aetiology prompted us to study whether the HVR haplotype also contributed to Raynaud's phenomenon and migraine in this family. Whereas the parent-child transmission disequilibrium test (TDT) did not reach significance, the sibling TDT revealed that the HVR haplotype harbours a susceptibility factor for Raynaud's phenomenon and migraine. Identification of the HVR gene will improve the understanding of the pathophysiology of HVR, Raynaud's phenomenon and migraine.

Abstract:Photoacoustic microscopy (PAM) is an emerging imaging technology that can non-invasively visualize ocular structures in animal eyes. This report describes an integrated multimodality imaging system that combines PAM, optical coherence tomography (OCT), and fluorescence microscopy (FM) to evaluate angiogenesis in larger animal eyes. High-resolution in vivo imaging was performed in live rabbit eyes with vascular endothelial growth factor (VEGF)-induced retinal neovascularization (RNV). The results demonstrate that our multimodality imaging system can non-invasively visualize RNV in both albino and pigmented rabbits to determine retinal pathology using PAM and OCT and verify the leakage of neovascularization using FM and fluorescein dye. This work presents high-resolution visualization of angiogenesis in rabbits using a multimodality PAM, OCT, and FM system and may represent a major step toward the clinical translation of the technology.
Preclinical imaging of iridocorneal angle and fundus using a modified integrated flexible handheld probe

Abstract:A flexible handheld imaging probe consisting of a [Formula: see text] charge-coupled device camera, light-emitting diode light sources, and near-infrared laser source is designed and developed. The imaging probe is designed with specifications to capture the iridocorneal angle images and posterior segment images. Light propagation from the anterior chamber of the eye to the exterior is considered analytically using Snell's law. Imaging of the iridocorneal angle region and fundus is performed on ex vivo porcine samples and subsequently on small laboratory animals, such as the New Zealand white rabbit and nonhuman primate, in vivo. The integrated flexible handheld probe demonstrates high repeatability in iridocorneal angle and fundus documentation. The proposed concept and methodology are expected to find potential application in the diagnosis, prognosis, and management of glaucoma.

Abstract:The evaluation and monitoring of cells health in the human retina is crucial and follow time course of retinal diseases, detect lesions before irreversible visual loss and to evaluate treatment effects. Towards this goal, a major challenge is to image and quantify retinal cells in human eyes in a non-invasive manner. Despite the phenomenal advances in Optical Coherence Tomography (OCT) and Adaptive Optics systems, in vivo imaging of many of these cells is limited by the fact that cell contrast in reflection is extremely low. Here, we report on a major advance by proposing and demonstrating a radically different method compared with OCT to visualize retinal cells with high contrast, resolution and an acquisition time suitable for clinical use. The method uses a transscleral illumination which provides a high numerical aperture in a dark field configuration. The light backscattered by the Retinal Pigment Epithelium (RPE) and the choroid provides a forward illumination for the upper layer of the retina, thus providing a transmission illumination condition. By collecting the scattered light through the pupil, the partially coherent illumination produces dark field images, which are combined to reconstruct a quasi-quantitative phase image with twice the numerical aperture given by the eye's pupil. The retrieved quasi-quantitative phase images (QPI) of cells in ex vivo human and pig's eyes are validated with those taken with a standard QPI system. We then apply the technique in vivo on human eye, without pupil dilation, we demonstrated the feasibility to image retinal cells up to the retinal pigment epithelium within a few seconds.

Abstract:Objects: A novel computer method for three-dimensional in vivo imaging of human anterior eye segment is described with optical coherence tomography. Methods: A laser beam was projected at an oblique angle on the cornea and scanned to acquire 18 optical section images at 10-degree intervals across the corneal area. Tissue's edges were detected using Canny detector, and Track Points arithmetic was designed to extract the single-pixel edges, which were smoothed by curve fitness. According to the characters of rotating image, which appeared displaced because of the motion blur of patients, the classical hat-head method was improved to build 3D model for OCT images sequence. Three B-Spine interpolation and curve fitness would be used to obtain the outlines if they were lost in original OCT images. The optical section images were segmented to construct a series of anterior eye. Imaging was performed in normal subject and patients. Results: A series of digital 3D model of anterior eye was reconstructed, which allowed enhanced visualization of the micro-structures, including the section of anterior eye segment at any rotating degree, the contours of corneal thickness and curvature, and the depths and angles of anterior chamber. Conclusion: A method for in vivo three-dimensional anterior eye imaging was developed, which has potential as a tool for objective, noninvasive and real-time evaluation of diseases.

Abstract:The goal of this study is to further develop a multi-modality eye imaging system and evaluate its feasibility of acquiring images of different modalities simultaneously. An integrated multimodality imaging system combining spectral-domain optical coherence tomography (SD-OCT), photoacoustic microscopy (PAM), and fluorescence microscopy (FM) was developed, and its performance for eye imaging was validated on multiple clinically-relevant retinal disease models in vivo in rabbits. OCT imaging allows for visualization of the different anatomic retinal layers with high axial resolution. PAM can be used to image vasculature, angiogenesis, and hemorrhages. The leakage of neovascularization can be verified with FM and fluorescein dye. Simultaneous imaging with OCT, PAM, and FM ensures co-registration of the three modalities without being affected by motion artifacts caused by breathing, body or eye movements, and heartbeat. This simultaneous multi-modality eye imaging system could be a new tool for applications both in ophthalmology and other fields.

Abstract:Morphological and hemodynamic changes in the ocular vasculature are important signs of various ocular diseases. The evaluation of the ocular microvasculature with high resolution is valuable in comprehensive diagnoses. However, it is difficult for current optical imaging techniques to visualize the posterior segment and retrobulbar microvasculature due to the limited penetration depth of light, particularly when the refractive medium is opaque. Thus, we have developed a 3D ultrasound localization microscopy (ULM) imaging method to visualize the ocular microvasculature in rabbits with micron-scale resolution. We used a 32 × 32 matrix array transducer (center frequency: 8 MHz) with a compounding plane wave sequence and microbubbles. Block-wise singular value decomposition spatiotemporal clutter filtering and block-matching 3D denoising were implemented to extract the flowing microbubble signals at different imaging depths with high signal-to-noise ratios. The center points of microbubbles were localized and tracked in 3D space to achieve the micro-angiography. The in vivo results demonstrate the ability of 3D ULM to visualize the microvasculature of the eye in rabbits, where vessels down to 54 μm were successfully revealed. Moreover, the microvascular maps indicated the morphological abnormalities in the eye with retinal detachment. This efficient modality shows potential for use in the diagnosis of ocular diseases.

Abstract:Purpose: The mammalian ocular lens is an avascular multicellular organ that grows continuously throughout life. Traditionally, its cellular organization is investigated using dissected lenses, which eliminates in vivo environmental and structural support. Therefore, in vivo optical imaging methods for studying lenses in their native context in live animals are urgently needed. Methods: Here, we demonstrated that two-photon fluorescence microscopy can visualize lens cells in vivo. To maintain subcellular resolution at depth, we used adaptive optics to correct aberrations owing to ocular and lens tissues, which led to substantial signal and resolution improvements. Results: Imaging lens cells up to 980 µm deep, we observed novel cellular organizations including suture-associated voids, enlarged vacuoles, and large cavities, contrary to the conventional view of a highly ordered organization. We tracked these features longitudinally over weeks and observed the incorporation of new cells during growth. Conclusions: Taken together, noninvasive longitudinal in vivo imaging of lens morphology using adaptive optics two-photon fluorescence microscopy will allow us to observe the development or alterations of lens cellular organization in living animals directly.

Abstract:Non-invasive imaging plays a crucial role in diagnosing and studying eye diseases. However, existing photoacoustic ophthalmoscopy (PAOM) techniques in mice have limitations due to handling restrictions, suboptimal optical properties, limited availability of light sources and permissible light fluence at the retina. This study introduces an innovative approach that utilizes Rose Bengal, a contrast agent, to enhance PAOM contrast. This enables visualization of deeper structures like the choroidal microvasculature and sclera in the mouse eye using visible light. The integration of near-infrared-II optical coherence tomography (NIR-II OCT) provides additional tissue contrast and insights into potential NIR-II PAOM capabilities. To optimize imaging, we developed a cost-effective 3D printable mouse eye phantom and a fully 3D printable tip/tilt mouse platform. This solution elevates PAOM to a user-friendly technology, which can be used to address pressing research questions concerning several ocular diseases such as myopia, glaucoma and/or age-related macular degeneration in the future.

Abstract:We have developed a prototype ocular imaging system that integrates optical-resolution photoacoustic microscopy and high-frequency ultrasound imaging. The system can perform high-resolution ocular imaging from the anterior region down to the fundus. It has successfully imaged murine eyes in vivo, including iris, lens, retina, and retinal pigment epithelium. Abstract:To view the individual cells and ambient microvasculature simultaneously will be helpful to study tumor angiogenesis and microenvironments. To achieve this, two molecular contrast mechanisms were exploited simultaneously by integrating two imaging modalities, confocal fluorescence microscopy (CFM) and photoacoustic microscopy (PAM). These share the same scanning optical path and laser source. The induced photoacoustic (PA) signal was detected by a highly sensitive needle hydrophone; while the back-traveling fluorescent photons emitted from the same sample were collected by an avalanche photodetector. Experiments on ex vivo rat bladders were conducted. The CFM image depicted the shape and size of the individual cells successfully. Besides large polygonal umbrella cells, some intracellular components can also be discerned. With the CFM image presenting morphologic cellular information in the bladder wall, the PAM image provides the complementary information, based on the endogenous optical absorption contrast, of the microvascular distribution inside the bladder wall, from large vessels to capillaries. Such multimodal imaging provides the opportunity to realize both histological assay and characterization of microvasculature using one imaging setup. This approach offers the possibility of comprehensive diagnosis of cancer in vivo.

Abstract:Two-photon excitation microscopy (TPM) and multiphoton fluorescence microscopy (MPM) are advanced forms of intravital high-resolution functional microscopy techniques that allow for the imaging of dynamic molecular processes and resolve features of the biological tissues of interest. Due to the cornea's optical properties and the uniquely accessible position of the globe, it is possible to image cells and tissues longitudinally to investigate ocular surface physiology and disease. MPM can also be used for the in vitro investigation of biological processes and drug kinetics in ocular tissues. In corneal immunology, performed via the use of TPM, cells thought to be intraepithelial dendritic cells are found to resemble tissue-resident memory T cells, and reporter mice with labeled plasmacytoid dendritic cells are imaged to understand the protective antiviral defenses of the eye. In mice with limbal progenitor cells labeled by reporters, the kinetics and localization of corneal epithelial replenishment are evaluated to advance stem cell biology. In studies of the conjunctiva and sclera, the use of such imaging together with second harmonic generation allows for the delineation of matrix wound healing, especially following glaucoma surgery. In conclusion, these imaging models play a pivotal role in the progress of ocular surface science and translational research.

Abstract:The corneo-scleral limbus contains several biological components, which are important constituents for understanding, diagnosing and managing several ocular pathologies, such as glaucoma and corneal abnormalities. An anterior segment optical coherence tomography (AS-OCT) system integrated with optical microangiography (OMAG) is used in this study to non-invasively visualize the three-dimensional microstructural and microvascular properties of the limbal region. Advantages include first the ability to correct optical distortion of microstructural images enabling quantification of relationships in the anterior chamber angle. Second, microvascular images enable the visualization of the microcirculation in the limbal area without the use of exogenous contrast agents. Third, by combining the microstructural and microvascular information, the aqueous outflow pathway can be identified. The proposed AS-OCT can serve as a useful tool for ophthalmological research to determine normal and pathologic changes in the outflow system. As a clinical tool it has the potential to detect early aqueous outflow system abnormalities that lead to the pressure elevation in glaucoma. Recent surgical innovations and their implementations also rely on an assessment of outflow system structure and function, which can be revealed by AS-OCT. (C) 2011 Optical Society of America

Abstract:Previously we demonstrated a combined scanning-point optical coherence tomography (OCT) and Scheimpflug Imaging (SI) system, to provide cross-section images of the corneal layers and the whole anterior segment of the eye simultaneously. However, to fully realise the benefits of this dual imaging modality concept towards clinical practice, it is required that the OCT part of the system can achieve ultra-high axial resolution (<3 m in air) and faster imaging speeds, without using prohibitively expensive components. To achieve this, a new imaging device integrating line-field OCT and SI, using a supercontinuum light source, is demonstrated. This line-field configuration enables both OCT and SI B-scan to be captured using the same light illumination in a single shot, which is the most significant improvement over single point scanning. This achieves an OCT axial resolution down to 2.1 mu m in air (1.6 mu m in corneal tissue) and an imaging speed up to 213 kA-Scans/s. Since the OCT and SI capture the identical corneal position and serve as cross-validation, this technique offers a great method for accurately determining the individual corneal thickness and refractive index, thereby minimizing individual variations, which can help determine the extent of cutting or correction needed in vision correction. Moreover, the precise measurement of corneal thickness provided by this technique allows for a better understanding of the biomechanical properties of the cornea. We demonstrate the efficacy of the proposed system by evaluating porcine and bovine eyes ex-vivo for determining the corneal refractive index and thickness.

Abstract:In the McCune‐Albright syndrome, fibrous dysplasia of bones and various forms of endocrine dysfunction are associated with multiple pigmented skin lesions. Examination of a 4‐year‐old female patient and comparison with photographs published in the literature revealed that the cutaneous pigmentation is arranged in a systematized pattern following the lines of Blaschko. Apparently, this pattern visualizes the dorso‐ventral outgrowth of two different populations of cells during early embryogenesis. As all cases of the syndrome are sporadic, it is postulated that the disease is caused by an autosomal “dominant” lethal gene, leading to loss of the zygote in utero. Cells bearing the mutation can only survive when they are intermingled with normal cells. The mosaic may arise either from a gametic half chromatid mutation, or from an early somatic mutation. This concept offers an explanation for the scattered asymmetric distribution of bone lesions, and for the observation that the endocrinopathy may be either of central or peripheral origin, according to the random distribution of the mutant population of cells.
