Abstract:Today, many hospitals have a running enterprise picture archiving and communication system (PACS) and their administrators should have the tools to measure the system activity and, in particular, how much it is used. The information would be valuable for decision-makers to address asset management and the development of policies for its correct utilization and eventually start training initiatives to get the best in resource utilization and operators’ satisfaction. On the economic side, a quantitative method to measure the usage of the workstations would be desirable to better redistribute existing resources and plan the purchase of new ones. The paper exploits in an unconventional way the potential of the IHE Audit Trail and Node Authentication (ATNA) profile: it uses the data generated in order to safeguard the security of patient data and to retrieve information about the workload of each PACS workstation. The method uses the traces recorded, according to the profile, for each access to image data and to calculate how much each station is used. The results, constituted by measures of the frequency of PACS station usage suitably classified and presented according to a convenient format for decision-makers, are encouraging. In the time of the spending review, the careful management of available resources is the top priority for a healthcare organization. Thanks to our work, a common medium such as the ATNA profile appears a very useful resource for purposes other than those for which it was born. This avoids additional investments in management tools and allows optimization of resources at no cost.

Abstract:The enormous number of studies performed at the Nuclear Medicine Department of University of Arkansas for Medical Sciences (UAMS) generates a huge amount PET/CT images daily. A DICOM workstation had been used as "mini-PACS" to route all studies, which is historically proven to be slow due to various reasons. However, replacing the workstation with a commercial PACS server is not only cost inefficient; and more often, the PACS vendors are reluctant to take responsibility for the final integration of these components. Therefore, in this paper, we propose an alternative imaging archiving and communication system called Enterprise Imaging Repository (EIR). EIR consists of two distinguished components: an image processing daemon and a user friendly web interface. EIR not only reduces the overall waiting time of transferring a study from the modalities to radiologists' workstations, but also provides a more preferable presentation.

Abstract:Computer aided diagnosis/detection (CAD) goes beyond subjective visual assessment of clinical images providing quantitative computer analysis of the image content, and can greatly improve clinical diagnostic outcome. Many CAD applications, including commercial and research CAD, have been developed with no ability to integrate the CAD results with a clinical picture archiving and communication system (PACS). This has hindered the extensive use of CAD for maximum benefit within a clinical environment. In this paper, we present a CAD-PACS integration toolkit that integrates CAD results with a clinical PACS. The toolkit is a software package with two versions: DICOM (digital imaging and communications in medicine)-SC (secondary capture) and DICOM-IHE (Integrating the Healthcare Enterprise). The former uses the DICOM secondary capture object model to convert the screen shot of the CAD results to a DICOM image file for PACS workstations to display, while the latter converts the CAD results to a DICOM structured report (SR) based on IHE Workflow Profiles. The DICOM-SC method is simple and easy to be implemented without ability for further data mining of CAD results, while the DICOM-IHE can be used for data mining of CAD results in the future but more complicated to implement than the DICOM-SC method.
Automating Object-Oriented Integration and Visualization of Multidisciplinary Biomedical Data in Radiology Workflow: Compartmental PACS Model

Abstract:This paper is aimed to present a novel compartmental PACS model for the automatic object-oriented integration and visualization of heterogeneous data for the multidisciplinary biomedical studies in the midst of the imaging services in radiology department. The generic PACS is conceptually partitioned into two compartments: service and integration. The service compartment supports the routine imaging service by connecting the imaging modalities to the web client workstations through a single fault-tolerant hardware. The integration compartment is synthesized by open source and open standard software tools to perform the long-term archiving of imaging cases and to integrate the images with the related data from other clinical disciplines. The prototype of the compartmental PACS model has been successfully implemented in the Department of Health Technology and Informatics (HTI) at the Hong Kong Polytechnic University (PolyU). Multi-disciplinary study on a cardiovascular case is considered as an example in this paper to demonstrate the seamless integration and web-based visualization of heterogeneous data from radiology, ophthalmology, cardiology and hematology. The implementation of the compartmental PACS model demonstrates a deliberative system design for allocating mission-critical components at the clinical frontline of imaging services and long-term archiving components at the backend. The integrative feature of object-oriented long-term archive addresses the comprehensive needs of patient-centered multidisciplinary biomedical studies.

Abstract:Introduction and purpose: We present the needs, design, development, implementation, and accessibility of a crafted experimental PACS (ePACS) system to securely store images, ensuring efficiency and ease of use for AI processing, specifically tailored for research scenarios, including phantoms, animal and human studies and quality assurance (QA) exams. The ePACS system plays a crucial role in any medical imaging departments that handle non-care profile studies, such as protocol adjustments and dummy runs. By effectively segregating non-care profile studies from the healthcare assistance, the ePACS usefully prevents errors both in clinical practice and storage security. Methods and results: The developed ePACS system considers the best practices for management, maintenance, access, long-term storage and backups, regulatory audits, and economic aspects. Moreover, key aspects of the ePACS system include the design of data flows with a focus on incorporating data security and privacy, access control and levels based on user profiles, internal data management policies, standardized architecture, infrastructure and application monitorization and traceability, and periodic backup policies. A new tool called DicomStudiesQA has been developed to standardize the analysis of DICOM studies. The tool automatically identifies, extracts, and renames series using a consistent nomenclature. It also detects corrupted images and merges separated dynamic series that were initially split, allowing for streamlined post-processing. Discussion and conclusions: The developed ePACS system encompasses a successful implementation, both in hospital and research environments, showcasing its transformative nature and the challenging yet crucial transfer of knowledge to industry. This underscores the practicality and real-world applicability of our innovative approach, highlighting the significant impact it has on the field of experimental radiology.
Implementation and Evaluation of PACS Workflow Monitoring Programme

Abstract:Objective This article, by implementation and evaluation of a monitoring programme in PACS system, brings forward and discusses the issues of database security, which have to pay much attention to when integrating a PACS. Methods Our PACS system has been mostly integrated by a controller server (Fire V480, SUN Corp.) and a SCSI RAID (redundant array of inexpensi\e disks. RAID) of 800GB. The different imaging modalities have been connected to the PACS server by using a star topology. Image display workstations have been connected to the PACS server by a bus topology through switch equipments. A special web server has been linked to display workstations (personal computers) as our RIS system, whose database is directly controlled by the PACS server. Security monitoring for database utilizes images workflow and runs monitoring programme-'PACSMON', which can monitor the workflow and the use of images. Security monitoring for text files (diagnostic reports) has been performanced by monitoring the database flow of RIS system. Results After 10 months running of our PACS system, the PACSMON has perfectly showed acts of querying and retrieving of images on the whole. and monitored working status of any display workstations on real time. Especially for the acts of querying and retrieving, the PACSMON can accurately show the location of display workstations and the quantity. frequency of querying and retrieving, and has effectively controlled the system to be attacked and the images to be stolen. Conclusions The operating management of PACS must resolve some following problems:1.the security of system running; 2. the security of system database; 3.the security of images and files content; 4.the lose of database transmitted.
A Data Ingestion Procedure towards a Medical Images Repository

Abstract:This article presents an ingestion procedure towards an interoperable repository called ALPACS (Anonymized Local Picture Archiving and Communication System). ALPACS provides services to clinical and hospital users, who can access the repository data through an Artificial Intelligence (AI) application called PROXIMITY. This article shows the automated procedure for data ingestion from the medical imaging provider to the ALPACS repository. The data ingestion procedure was successfully applied by the data provider (Hospital Clínico de la Universidad de Chile, HCUCH) using a pseudo-anonymization algorithm at the source, thereby ensuring that the privacy of patients' sensitive data is respected. Data transfer was carried out using international communication standards for health systems, which allows for replication of the procedure by other institutions that provide medical images. Objectives: This article aims to create a repository of 33,000 medical CT images and 33,000 diagnostic reports with international standards (HL7 HAPI FHIR, DICOM, SNOMED). This goal requires devising a data ingestion procedure that can be replicated by other provider institutions, guaranteeing data privacy by implementing a pseudo-anonymization algorithm at the source, and generating labels from annotations via NLP. Methodology: Our approach involves hybrid on-premise/cloud deployment of PACS and FHIR services, including transfer services for anonymized data to populate the repository through a structured ingestion procedure. We used NLP over the diagnostic reports to generate annotations, which were then used to train ML algorithms for content-based similar exam recovery. Outcomes: We successfully implemented ALPACS and PROXIMITY 2.0, ingesting almost 19,000 thorax CT exams to date along with their corresponding reports.

Abstract:High costs for installing, maintaining, and updating a standard picture archiving and communication system (PACS) can be prohibitive for small/medium-sized veterinary facilities. The aims of this prospective, exploratory study were to describe the design, implementation, and author experiences for 1 year's use of a low-cost PACS based on network-attached storage. The system described here was easily installed and resiliently stored redundant copies of data. It excellently balanced data recovery, system speed, security, and available memory for storage. A virtual private network also allowed off-site data review. This system can also be used for future off-site backup of data in the cloud.
Queue monitor toolkit for tracking of PACS clinical workflow

Abstract:Expectation of rapid image retrieval and distribution from PACS contributes to increased information technology (IT) infrastructure investments and continuing demands upon PACS administrators to respond to "slow" system calls. Studies show that it is important for computer users to be able to check on the progress of their task via progress indicators (e.g., time left to download file) to know that the computer is still working. By analogy, the ability to provide predicted delivery times to a PACS user may curb user expectations for "fast" response especially during peak hours. Allowing for some periods of slow response means PACS infirastructure do not have to be overbuilt and also reduce time spent by PACS administrators fielding user inquiries on image status. For this condition, a queryable PACS queue monitor is the cornerstone for providing a progress indicator to the user. The typical PACS server holds image file information and destination workstation information in a queue until the RetrieveSend process can send the image. We developed an agent that queries the contents of the PACS RetrieveSend queue in real-time and coded an algorithm to predict delivery time. Delivery time can be predicted from the number and types of images in progress and the download time of prior images that accounts for network load and performance at that time of day. We have developed a PACS queue monitor prototype that is being tested on clinical data using the PACS Simulator at the Imaging Processing and Informatics (IPI) Laboratory of the University of Southern California (USC).
The IAEA remote and automated quality control methodology for radiography and mammography

Abstract:Radiography remains the most widely used imaging modality throughout the world. Additionally, while it has been demonstrated that a quality control (QC) program, especially in mammography, improves image quality, weekly technologist QC testing might be lacking even where there is clinical qualified medical physicist (CQMP) support. Therefore, the International Atomic Energy Agency (IAEA) developed simple QC phantoms that can easily be used on a regular basis (daily/weekly) for radiography and mammography. These are simple in design and use materials that are easily accessible in most parts of the world. A software application is also developed that automatically analyzes images and Digital Imaging and Communications in Medicine (DICOM) header information. It exports data to a comma-separated values (CSV) file that is read by a Microsoft Excel® spreadsheet for documentation and graphical analysis. The phantom and the software were tested in four institutions (in Costa Rica and the United States of America) both on computed radiography and direct digital mammography and radiography systems. Data were collected over a 3-year period. No corrective actions were taken on the data, but service was performed on two of the units. Results demonstrated noise that could be attributed to suboptimal placement of the phantom and incorrect data being put into the DICOM header. Preliminary evaluation of the IAEA methodology has demonstrated that it can provide meaningful QC data that are sensitive to changes in the imaging systems. Care must be taken at implementation to properly train personnel and ensure that the image data, including the DICOM header, are being correctly transmitted. The methodology gives the opportunity for a single CQMP to provide QC services even to remote sites where travel is prohibitive, and it is feasible and easy to implement.

Abstract:PurposeContent-based image retrieval (CBIR) bears great potential for computer-aided diagnosis (CAD). However, current CBIR systems are not able to integrate with clinical workflow and PACS generally. One essential factor in this setting is scheduling. Applied and proved with modalities and the acquisition of images for a long time, we now establish scheduling with CBIR.MethodsOur workflow is based on the IHE integration profile ‘Post-Processing Workflow’ (PPW) and the use of a DICOM work list.ResultsWe configured dcm4chee PACS and its including IHE actors for the application of CBIR. In order to achieve a convenient interface for integrating arbitrary CBIR systems, we realized an adapter between the CBIR system and PACS. Our system architecture constitutes modular components communicating over standard protocols.ConclusionThe proposed workflow management system offers the possibility to embed CBIR conveniently into PACS environments. We achieve a chain of references that fills the information gap between acquisition and post-processing. Our approach takes into account the tight and solid organization of scheduled and performed tasks in clinical settings.

A HIPAA-Compliant Architecture for Securing Clinical Images

Abstract:The Health Insurance Portability and Accountability Act (HIPAA, instituted April 2003) Security Standards mandate health institutions to protect health information against unauthorized use or disclosure. One approach to addressing this mandate is by utilizing user access control and generating audit trails of the various authorized as well as unauthorized user access of health data. Although most current clinical image systems [e.g., picture archiving and communication system (PACS)] have components that generate log files for application debugging purposes, there is a lack of methodology to obtain and synthesize the pertinent data from the large volumes of log data generated by these multiple components within a PACS. We have designed a HIPAA-compliant architecture specifically for tracking and auditing the image workflow of clinical imaging systems such as PACS. As an initial first step, we developed HIPAA-compliant auditing system (H-CAS) based on parts of this HIPAA-compliant architecture. H-CAS was implemented within a test-bed PACS simulator located in the Image Processing and Informatics lab at the University of Southern California. Evaluation scenarios were developed where different user types performed legal and illegal access of PACS image data within each of the different components in the PACS simulator. Results were based on whether the scenarios of unauthorized access were correctly identified and documented as well as on normal operational activity. Integration and implementation pitfalls were also noted and included.
PACS-CAD toolkit for integrating an independent CAD workstation to diagnostic workflow

Abstract:Computer Aided Diagnosis (CAD) coupled with physician's knowledge can improve accuracy of clinical decision. However, many developed CAD software have no features to integrate its results with a picture archive and communication system (PACS). This obstacle hinders the extensive use of independent CAD results within a more streamlined diagnosis workflow. In this paper, we demonstrate a universal PACS-CAD toolkit that can seamlessly integrate independent CAD results with a clinical PACS. The PACS-CAD toolkit consisted of two versions, a DICOM Secondary Capture (DICOM-SC) version and a DICONI-IHE version to accommodate various PACS. The DICOM-SC version toolkit installed on a CAD workstation converts the screen shot of CAD results to a DICOM image file for storing in a PACS server and displaying on PACS workstations. The DICONI-IHE version toolkit follows DICOM and IHE standards using DICOM Structured Report and Post-Processing Workflow Profiles; thus, results from various CAD software can be integrated into diagnosis workflow of a PACS having DICOM and IHE-compliance and, most importantly, these quantified CAD results can be directly queried for and retrieved from within PACS for future data mining applications. The successful implementation of this toolkit can greatly ease the extensive use of various CAD results in the clinical diagnosis workflow.
Managed Pacs Operation with an Automatic Monitoring Tool

Abstract:Huadong hospital in Shanghai with 800 beds provides health care services for inpatients and outpatients, as well as special senior and VIP patients. In order to move to digital imaging based radiology practice, and also provide better intra-hospital consultation services for senior and VIP patients, we started to implement PACS for hospital wide services from 1999, and also designed and developed an automatic monitoring system (AMS) to monitor and control PACS operation and dataflow to decrease the total cost of ownership for PACS operation. We installed the AMS on top of the Huadong Hospital PACS in the May of 2001. The installation was painless, did not interrupt the normal PACS operation, and took only one month. The PACS administrators with the AMS can now monitor and control the entire PACS operation in real time, and also track patient and image data flow automatically. These features make administrators take proper action even before user's complaint if any failure happened in any PACS component or process, they reduce the size of the management team, and decrease total cost of PACS ownership.
A PACS archive architecture supported on cloud services

Abstract:Purpose: Diagnostic imaging procedures have continuously increased over the last decade and this trend may continue in coming years, creating a great impact on storage and retrieval capabilities of current PACS. Moreover, many smaller centers do not have financial resources or requirements that justify the acquisition of a traditional infrastructure. Alternative solutions, such as cloud computing, may help address this emerging need. Methods: A tremendous amount of ubiquitous computational power, such as that provided by Google and Amazon, are used every day as a normal commodity. Taking advantage of this new paradigm, an architecture for a Cloud-based PACS archive that provides data privacy, integrity, and availability is proposed. The solution is independent from the cloud provider and the core modules were successfully instantiated in examples of two cloud computing providers. Operational metrics for several medical imaging modalities were tabulated and compared for Google Storage, Amazon S3, and LAN PACS. Results: A PACS-as-a-Service archive that provides storage of medical studies using the Cloud was developed. The results show that the solution is robust and that it is possible to store, query, and retrieve all desired studies in a similar way as in a local PACS approach. Conclusion: Cloud computing is an emerging solution that promises high scalability of infrastructures, software, and applications, according to a "pay-as-you-go" business model. The presented architecture uses the cloud to setup medical data repositories and can have a significant impact on healthcare institutions by reducing IT infrastructures.
Evaluation of network attached storage technique in PACS clinical application

Abstract:Objective Information technologies were pushing the development of PACS clinical applications.Network attached storage technique is good at its performance,with successful application in IT industries.This article is to evaluate the application of Network Attached Storage technique in PACS.Methods Using a NAS as long-term archiving system storage unit,on which 15.5 terabytes available for images;both PACS long-term archiving system and NAS unit are configured to be connected at 2 Gbps through Ethernet switches via CIFS.Through single DICOM association,100 studies of four different modalities are retrieved from PACS long-term archiving system.Through multiple DICOM association,five group of which,images are retrieved from PACS long-term archiving system.With start time,end time,image count and study size logged,calculate the velocity of retrieving a study or an image.Make a comparison of single and multiple DICOM associations.Results Centricity Enterprise Archive manages to send one copy of digital radiography study to hosts within 1-3 seconds and manages to send one copy of CT or MRI study at the rate of 30-40 images per seconds throughout single DICOM association.In case of multiply DICOM associations,the rate does not differ much from that of increasing the number of associations.Conclusion NAS-based PACS long-term archive has a fast and steady performance.It could be a perfect match of the PACS clinical applications.
Towards quality assurance: implementation of a POCUS image archiving system in a high-volume community emergency department

Abstract:BackgroundQuality assurance review is an integral part of point-of-care ultrasound (POCUS) programs but may not be routine practice in community hospitals. Lack of image acquisition and documentation can result in suboptimal patient care. In cases with an adverse outcome and no record of images, there is no mechanism for quality improvement.ObjectivesOur goal was to implement a system of POCUS image archiving in a community hospital. Our SMART (Specific, Measurable, Actionable, Realistic, Timely) aim was to have > 50% of emergency department (ED) POCUS users archiving scans, and > 80% of all billed POCUS scans archived, measuring improvements bi-weekly over a period of 9 months.MethodsThe study was conducted at a single-community ED between August 2020 and April 2021. The POCUS archiving workflow was developed and refined through multiple plan-do-study-act (PDSA) cycles. Surveys, stakeholder meetings, audits, and feedback were used to generate and re-evaluate the interventions. These included introduction of QPathE© software, streamlining of the workflow process, strategic machine placement, POCUS rounds, use of a website for POCUS workflow instructions, and dissemination of audit results. Scans were tracked biweekly, and indexed by the number of scans billed. The primary outcome measure was the number of POCUS scans archived per 100 scans billed.ResultsOver a 9-month period, spanning 72,986 ED visits, 550 scans were archived. The percentage of POCUS users who changed practice to consistently archiving scans was 51%. The rate of POCUS scans archived per 100 scans billed was > 80%, compared to no archiving at baseline.ConclusionWe were able to transition from a system with entirely unarchived POCUS scanning, to one with > 80% of scans archived over a period of 9 months. This is the first published paper documenting implementation of a POCUS image archiving system in a Canadian Community ED.

Here, we present a novel approach, Artificial Intelligence (AI)-Based QUality Assurance by Restricted Investigation of Unequal Scores (AQUARIUS), for re-defining radiology QA, which reduces human effort by up to several orders of magnitude over existing approaches. AQUARIUS typically includes automatic comparison of AI-based image analysis with natural language processing (NLP) on radiology reports. Only the usually small subset of cases with discordant reads is subsequently reviewed by human experts. To demonstrate the clinical applicability of AQUARIUS, we performed a clinical QA study on Intracranial Hemorrhage (ICH) detection in 1936 head CT scans from a large academic hospital. Immediately following image acquisition, scans were automatically analyzed for ICH using a commercially available software (Aidoc, Tel Aviv, Israel). Cases rated positive for ICH by AI (ICH-AI+) were automatically flagged in radiologists' reading worklists, where flagging was randomly switched off with probability 50%. Using AQUARIUS with NLP on final radiology reports and targeted expert neuroradiology review of only 29 discordantly classified cases reduced the human QA effort by 98.5%, where we found a total of six non-reported true ICH+ cases, with radiologists' missed ICH detection rates of 0.52% and 2.5% for flagged and non-flagged cases, respectively. We conclude that AQUARIUS, by combining AI-based image analysis with NLP-based pre-selection of cases for targeted human expert review, can efficiently identify missed findings in radiology studies and significantly expedite radiology QA programs in a hybrid human-machine interoperability approach.
