Abstract:Topic: 16. Myeloproliferative neoplasms - Clinical Background: Currently, the only marketed Janus kinase (JAK) inhibitor for the treatment of myelofibrosis in China is Ruxolitinib, which demonstrated symptomatic and splenic benefit despite of exacerbated cytopenias. However, it is reported that at least 50% of patients treated with ruxolitinib could develop resistance or intolerance during the long-term follow up, for whom the life expectancy is largely reduced. There are limited therapeutic options available after ruxolitinib failure. Jaktinib, an oral novel JAK and AVCR1 inhibitor showed promising results in patients with intermediate or high risk myelofibrosis, including primary myelofibrosis, post-polycythemia vera myelofibrosis and post-essential thrombocythemia myelofibrosis. Aims: We conducted a phase 2 study to evaluate jaktinib in patients who were refractory or relapsed to prior ruxolitinib therapy. Methods: Patients who had primary, post-polycythemia vera or post-essential thrombocythemia myelofibrosis and were previously treated with ruxolitinib for at least 3 months and with either inadequate efficacy response (defined as <10% spleen volume reduction [SVR] by MRI/CT or <30% decrease in spleen size by palpation from baseline) or spleen regrowth to <10% SVR or <30% decrease in spleen size from baseline following an initial response were recruited to receive jaktinib 100 mg bid. The primary endpoint was the proportion of patients with SVR ≥35% from baseline (SVR35) at week 24, measured by MRI/CT images and assessed by Independent Review Committee. Secondary endpoints included the best spleen response rate (defined as achieving SVR35 at any time), the proportion of patients with ≥50% reduction in Myeloproliterative Neoplasm Symptom Assessment Form Total Symptom Score (MPN-SAF TSS), improvement of anemia, safety, etc. Results: A total of 34 patients were enrolled. The median duration of previous ruxolitinib treatment was 18.6 months. The median baseline TSS was 19.5 (range 1-55), hemoglobin was 101.5 g/L (range 61-149), and platelet count was 159×109/L (range 66-951). Patient baseline characteristics are shown in the Table 1.At week 24, 11 (32.4%) patients achieved SVR35. The best spleen response rate was 38.2% (13/34). The median time to achieve the first SVR35 was 8.1 weeks, and the median duration of response was not reached. At week 24, 13 (46.4%) patients achieved a ≥50% improvement in TSS from baseline. Of 16 transfusion-independent patients with hemoglobin ≤100 g/L at baseline, eight (50.0%) had a ≥20 g/L hemoglobin increase by week 24. Of 6 patients who required red blood cell transfusion at baseline, two (33.3%) had a 50% decrease in red blood cell infusion by week 24. The median exposure time of jaktinib was 231 days (range 96-371). Twenty three (67.6%) patients had one or more grade ≥3 treatment emergent adverse events (TEAEs). The most common grade ≥3 TEAEs were anemia (32.4%, n=11), thrombocytopenia (32.4%, n=11), leukocytosis (20.6%, n=7) and leukopenia (11.8%, n=4). A dose reduction or temporary interruption due to TEAEs were reported in 9 (26.5%) patients. Treatment discontinuation due to TEAEs occurred in 2 (5.9%) patients. Summary/Conclusion: Jaktinib is effective and well tolerated in myelofibrosis patients who were refractory/relapsed to ruxolitinib. Jaktinib not only substantially improves splenomegaly, but also reduces myelofibrosis-related symptoms. Safety and Efficacy of Jaktinib in the Treatment of Janus Kinase Inhibitor‐naïve Patients with Myelofibrosis: Results of a Phase II Trial

Abstract:Myelofibrosis (MF) is associated with several constitutional symptoms. Currently, there are few therapeutic options for MF. Jaktinib, a novel, small-molecule inhibitor of JAK, is currently being studied for its potential to treat MF. This phase 2 trial investigated efficacy and safety of jaktinib in the treatment of MF patients. The primary end point was the proportion of patients with >= 35% reduction in spleen volume (SVR35, proportion of patients with >= 35% reduction in spleen volume) at week 24. The secondary end points included improvement of anemia, rates of symptom response, and safety profile. Between January 8, 2019 and August 29, 2020, 118 patients were recruited and treated with either jaktinib 100 mg BID or 200 mg QD. At week 24, 54.8% (34/62) of patients in the 100 mg BID group and 31.3% (15/48) in the 200 mg QD group achieved SVR35 (p = .0199). Jaktinib treatment increased hemoglobin level to >= 20 g/L in 35.6% (21/59) of patients with hemoglobin <= 100 g/L at baseline. The proportion of patients who achieved a >= 50% improvement in total symptom score at week 24 was 69.6% (39/56) in the BID group and 57.5% (23/40) in the QD group. The most common >= grade 3 hematological treatment-emergent adverse events (TEAEs; >= 10%) were anemia (100 mg BID: 24.2%, 200 mg QD: 28.8%), thrombocytopenia (16.7%, 11.5%), and neutropenia (3.0%, 11.5%). All non-hematological TEAEs were mild. These results indicate that jaktinib can shrink the spleen, improve anemia, and other clinical symptoms with good tolerability.

Abstract:Background: Ruxolitinib and hydroxyurea are recommended by Chinese myelofibrosis guideline for splenomegaly. There is an unmet need for new treatments. Jaktinib, a novel JAK and AVCR1 inhibitor, showed promising activity on splenomegaly, anemia, and myelofibrosis symptoms in a phase 2 study (NCT03886415). Aims: This randomized double-blind phase 3 study was aimed to assess the efficacy and safety of jaktinib compared to hydroxyurea in patients with intermediate-2 or high risk myelofibrosis. Methods: Patients aged ≥18 with primary, post-polycythemia vera or post-essential thrombocythemia myelofibrosis, dynamic international prognostic scoring system (DIPSS) Int-2 or high risk, and no prior or ≤10 days' treatment with a JAK inhibitor were enrolled and randomly assigned (2:1) to receive jaktinib 100 mg bid plus hydroxyurea placebo or hydroxyurea 0.5 g bid plus jaktinib placebo, stratified by DIPSS risk status (Int-2 or high risk). One interim analysis was pre-specified to be conducted when the 70 patients completed the 24-week treatment or met the criteria for the treatment termination before week 24. The primary endpoint was the proportion of patients with a spleen volume reduction of ≥35% from baseline (SVR35) at week 24, measured by MRI/CT images and assessed by Independent Review Committee. Secondary endpoints included the best spleen response rate (defined as achieving SVR35 at any time), the proportion of patients with a ≥50% reduction in Total Symptom Score (TSS50), improvement of anemia, safety, etc. Results: This interim analysis (data cut-off: April 8, 2022) included 47 patients receiving jaktinib and 23 receiving hydroxyurea. 66.0% (jaktinib) and 69.6% (hydroxyurea) had a baseline hemoglobin <100 g/L. 59.6% (jaktinib) and 69.6% (hydroxyurea) were JAK2V617F positive. Patient baseline characteristics were well balanced between the two arms (Table 1). The SVR35 rates at week 24 were 72.3% for jaktinib vs. 17.4% for hydroxyurea (p≤0.0001). Jaktinib showed a consistent spleen response benefit over hydroxyurea across all subgroups analyzed (Figure 1). The best spleen response rates were 80.9% of jaktinib-treat patients vs. 26.1% of hydroxyurea-treated patients (p≤0.0001). The median maximum percentage change from baseline in spleen volume were -46.59% vs. -18.50%. The TSS50 rates at week 24 were 63.8% for jaktinib vs. 43.5% for hydroxyurea (p=0.1163). 5 of 7 jaktinib-treated and two of 5 hydroxyurea-treated patients who required red blood cell transfusion at baseline achieved a ≥50% decrease in red blood cell transfusion by week 24. One of four jaktinib-treated and 0 of three hydroxyurea-treated patients who were transfusion-dependent at baseline changed to transfusion-independent. In transfusion-independent patients with baseline hemoglobin ≤100 g/L, 39.3% for jaktinib and 15.4% for hydroxyurea had a ≥20 g/L hemoglobin increase. The most common grade ≥3 hematological treatment-emergent adverse events (TEAEs) were anemia (25.5% [jaktinib] vs. 43.5% [hydroxyurea]), thrombocytopenia (17.0% vs. 39.1%), leukopenia (2.1% vs. 21.7%), neutropenia (2.1% vs. 21.7%) and decreased lymphocyte count (2.1% vs. 13.0%). Most common non-hematological TEAEs were upper respiratory tract infection (21.3% vs. 21.7%), elevated bilirubin (12.8% vs. 26.1%), fever (12.8% vs. 21.7%) and diarrhea (10.6% vs. 21.7%), predominantly of grade 1 or 2. TEAEs leading to treatment discontinuation occurred in 8.5% of jaktinib and 17.4% of hydroxyurea.Summary/Conclusion: Jaktinib demonstrated significant clinical benefits over hydroxyurea in myelofibrosis patients for spleen response with improved symptom response and less cytopenias. Aims: We conducted a single-arm phase 2b study to evaluate jaktinib in MF patients who were intolerant to ruxolitinib. Here we present the results of the cohort of patients who received jaktinib 100 mg twice a day. Methods: Patients who had primary, post-polycythemia vera or post-essential thrombocythemia MF and were previously treated with ruxolitinib for ≥28 days who either required red blood cell (RBC) transfusions or required more frequent transfusions on ruxolitinib, or had grade 3/4 anemia, thrombocytopenia or hematoma when receiving ruxolitinib, and with palpable spleen ≥5 cm were included. Patients were excluded from the study if they had received ruxolitinib ≥20 mg Bid for at least three months and were refractory or relapsed to ruxolitinib. The primary endpoint was the proportion of patients with spleen volume reduction of ≥35% from baseline (SVR35) at week 24, assessed by Independent Review Committee on the basis of MRI/CT images. Secondary endpoints included the best spleen response rate (defined as achieving SVR35 at any time), the proportion of patients with ≥50% reduction in MPN Symptom Assessment Form Total Symptom Score (MPN-SAF TSS), anemia response, safety, etc. Results: In the full analysis set for efficacy, a total of 44 patients received jaktinib 100 mg twice daily and had completed the 24-week treatments and evaluations or terminated prior to week 24. In addition, one patient was excluded from the efficacy analysis due to incorrect diagnosis of MF but included in the safety analysis. The median baseline TSS was 9.5, hemoglobin was 80 g/L, and platelet count was 132.5×109/L. Patient baseline characteristics are summarized in the Table 1. While on ruxolitinib, 21 patients (47.7%) required RBC transfusion and all patients experienced grade 3/4 anemia, thrombocytopenia or both.The SVR35 rate at week 24 was 43.2% (19/44). Over the study phase, the best spleen response rate was 54.5% (24/44). The median time to achieve the first SVR35 was 12.1 weeks. Spleen response was maintained in 80.4% patients for at least 24 weeks and median duration was not reached. At week 24, 61.8% of the evaluable patients achieved a ≥50% improvement in TSS from baseline. Of 31 transfusion-independent patients with hemoglobin ≤100 g/L at baseline, 13 (41.9%) had an increase of hemoglobin ≥20 g/L by week 24. Of 19 patients who required RBC transfusion at baseline, 11 (57.9%) had a 50% decrease in RBC infusion frequency by week 24. Of nine transfusion-dependent patients at baseline, one became transfusion-independent by week 24. In the safety set (n=45), the median jaktinib exposure time was 280 days. The most common grade ≥3 treatment-related adverse events (TEAEs) were anemia (31.1%, n=14), thrombocytopenia (22.2%, n=10), pneumonia (17.8%, n=8), neutropenia (15.6%, n=7) and leukopenia (15.6%, n=7). Treatment discontinuation due to TEAEs occurred in five patients (11.1%). A dose reduction or temporary interruption due to TEAEs were reported in 12 patients (26.7%). Summary/Conclusion: Jaktinib demonstrated promising therapeutic activities in MF patients intolerant to ruxolitinib. Jaktinib 100 mg bid taken orally, not only substantially reduced splenomegaly, but also ameliorated MF-related symptom burdens and anemia. Abstract:Janus kinase 2 inhibitors (JAKi) are now part of the therapeutic armamentarium for primary and secondary myelofibrosis (MF). Patients with MF endure shortened survival and poor quality of life (QoL). Allogeneic stem cell transplant is currently the only treatment modality in MF with the potential to cure the disease or prolong survival. By contrast, current drug therapy in MF targets QoL and does not modify the natural history of the disease. The discovery of JAK2 and other JAK-STAT activating mutations (i.e., CALR and MPL) in myeloproliferative neoplasms, including MF, has facilitated the development of several JAKi that are not necessarily specific to the oncogenic mutations themselves but proved effective in countering JAK-STAT signaling, resulting in suppression of inflammatory cytokines and myeloproliferation. This non-specific activity resulted in clinically favorable effects on constitutional symptoms and splenomegaly and, consequently, FDA approval of three small molecule JAKi: ruxolitinib, fedratinib, and pacritinib. A fourth JAKi, momelotinib, is poised for FDA approval soon and has been shown to provide additional benefit in alleviating transfusiondependent anemia in MF. The salutary effect of momelotinib on anemia has been attributed to inhibition of activin A receptor, type 1 (ACVR1) and recent information suggests similar effect from pacritinib. ACRV1 mediates SMAD2/3 signalling that contributes to upregulation of hepcidin production and iron-restricted erythropoiesis. Therapeutic targeting of ACRV1 raises therapeutic prospects in other myeloid neoplasms associated with ineffective erythropoiesis, such as myelodysplastic syndromes with ring sideroblasts or SF3B1 mutation, especially those with co-expression of JAK2 mutation and thrombocytosis.

Abstract:The bcr-abl negative myeloproliferative neoplasms (MPN) include polycythemia vera (PV),essential thrombocythemia (ET) and primary myelofibrosis (PMF).Along with the detecting of JAK2 V617F gene mutation in MPN patients,a series of small molecules drugs that targeted to the mutation have been investigated in clinical trials.Ruxolitinib is the inhibitor of JAK1/2,which has been proved by USA Food and Drug Administration (FDA) and the European Medicines Agency (EMA) to treat patients with intermediate-2 or high-risk PMF or post-PV MF or post-ET MF and PV patients who are intolerant or resistant to hydroxycarbamide,attributed to a number of clinical trials,especially in COMFORT-Ⅰ and COMFORT-Ⅱ.As a result,the MPN patients have new hope for the treatment.
Myelofibrosis Drug Development through the Decade: Novel JAK Inhibitors

Abstract:Abstract Introduction Myelofibrosis (MF), a BCR-ABL negative myeloproliferative neoplasm (MPN), has an annual incidence of 1 in 100000 for the primary MF and 0.3-0.7 in 100000 for secondary MF in the USA. MF patients have a median survival of 6.5 years. The primary mutation, JAK2V617F, occurs in 40-60% of MF cases. Ruxolotinib, a JAK inhibitor, has been the mainstay in treating high risk, debilitating MF but largely clinical needs are unmet. Methods A comprehensive literature research was performed using PubMed, Cochrane, EMBASE, Web of Science and Clinicaltrials.gov. We included all trials that were under development in phase I/II/III trials. Our search identified 1642 full-length manuscripts or abstracts with published results in the last decade ( Jan 2007 till Dec 2017) were screened for relevant studies. After screening by 2 independent reviewers, 212 articles were finalized for our final analyses. We have reviewed the mechanism of action, safety and efficacy of 2nd generation JAK inhibitors in this review. Results JAK1 inhibitor: Itacitinib reduced total symptom score (TSS) ≥ 50% in 15/42 (36%) patients. Mild gastrointestinal (GI) disturbances and some grade 3-4 myelosuppression (anemia: 33%, thrombocytopenia: 29%) were reported. JAK2 inhibitors: In PERSIST-1, pacritinib when compared to best available therapy (BAT) showed SVR ≥ 35% in 19.1% vs. 4.7% patients, with lower rates of myelosuppression (thrombocytopenia: 17%, anemia: 11%). In PERSIST-2, a phase III trial of pacritinb vs. BAT in patients with baseline cytopenias, similar efficacy was demonstrated (SVR ≥ 35%: 18% vs. 3%). Increasing rates of heart failure and intracranial hemorrhages led to a temporary hold which was lifted in August 2017. Lestaurtinib showed CI in 7 (44%) patients in a phase I trial (n=16) and 6 (27%) patients in a phase II trial (n=22). Most notable toxicities were G 1/2 GI disturbances, anemia occurred in 14% and thrombocytopenia in 23% of patients. In a phase III trial (n=193), fedratinib showed a SVR ≥ 35% and a TSS ≥ 50% in 40% and 36% patients, respectively. However, incidence of significant neurotoxicity and Wernicke's encephalopathy led to its suspension. Similarly, a trial of XL019 was terminated due to emergence of central and peripheral neurotoxicity. In a phase I trial (n=48), NS-018 exhibited a spleen length reduction (SLR) ≥ 50% in 20 (56%) patients along with prompt improvement in bone marrow fibrosis (37%). Anemia and thrombocytopenia were reported in 15% and 27% of patients, respectively. Dizziness (23%) and nausea (19%) were also reported. Gandotinib demonstrated SLR ≥50% in 62% patients, in a phase I trial (n=38). G1 diarrhea (55.3%) and nausea (42.1%) were the most common toxicities. JAK 1/2 inhibitors: SIMPLIFY-1 (S1), a phase III clinical trial (n=432) of momelotinib vs. ruxolotinib in JAK inhibitor-naïve patients, demonstrated non-inferiority for momelotinib, in spleen volume reduction (SVR) ≥ 35% (26.5% vs. 29%; p=0.01). However, SIMPLIFY-2 trial (S2), that compared these two drugs in JAK inhibitor exposed patients did not achieve similar responses with momelotinib (6.7% vs. 5.8%; p=0.90). Interestingly, momelotinib excelled at achieving transfusion independency in both trials (S1: 66.5% vs. 49.3%; p=0.001, S2: 43.3% vs 21.2%; p=0.001). Grade ≥ 3 infections and peripheral neuropathy were the major toxicities noted. These trials were suspended after 89% of patients failed to achieve the primary endpoint of SVR. AZD1480 demonstrated clinical improvement (CI) in four (11%) patients in a phase I trial (n=35). Most common adverse events included grade (G) 1-2 dizziness and anemia. Conclusion Novel JAK pathway inhibitors have shown promising efficacy in MF but safety concerns regarding the hematological (cytopenias) and non-hematological adverse effects needs to be addressed until their use in clinical practice is established. Momelotinib success in achieving anemia related endpoints is note-worthy and should be further explored in this regard. A phase II study [NCT03165734] evaluating pacritinib monotherapy as a second line treatment in patients with baseline thrombocytopenia is ongoing. Abstract:Dysregulated JAK/STAT hyperactivity is essential to the pathogenesis of myelofibrosis, and JAK inhibitors are the first-line treatment option for many patients. There are four FDA-approved JAK inhibitors for patients with myelofibrosis. Single-agent JAK inhibition can improve splenomegaly, symptom burden, cytopenias, and possibly survival in patients with myelofibrosis. Despite their efficacy, JAK inhibitors produce variable or short-lived responses, in part due to the large network of cooperating signaling pathways and downstream targets of JAK/STAT, which mediates upfront or acquired resistance to JAK inhibitors. Synergistic inhibition of JAK/STAT accessory pathways can increase the rates and duration of response for patients with myelofibrosis. Two recently reported, placebo-controlled phase III trials of novel agents added to JAK inhibition met their primary endpoint, and additional late-stage studies are ongoing. This paper will review role of dysregulated JAK/STAT signaling, biological plausible additional therapeutic targets and the recent advancements in combination strategies with JAK inhibitors for myelofibrosis.

Comprehensive profiling of clinical JAK inhibitors in myeloproliferative neoplasms

Abstract:Small molecule inhibitors targeting JAK2 provide symptomatic benefits for myeloproliferative neoplasm (MPN) patients and are among first-line therapeutic agents. However, despite all having potent capacity to suppress JAK-STAT signaling, they demonstrate distinct clinical profiles suggesting contributory effects in targeting other ancillary pathways. Here, we performed comprehensive profiling on four JAK2 inhibitors either FDA-approved (ruxolitinib, fedratinib, and pacritinib) or undergoing phase 3 studies (momelotinib) to better outline mechanistic and therapeutic efficacy. Across JAK2-mutant in vitro models, all four inhibitors demonstrated similar anti-proliferative phenotypes, whereas pacritinib yielded greatest potency on suppressing colony formation in primary samples, while momelotinib exhibited unique erythroid colony formation sparing. All inhibitors reduced leukemic engraftment, disease burden, and extended survival across patient-derived xenograft (PDX) models, with strongest effects elicited by pacritinib. Through RNA-sequencing and gene set enrichment analyses, differential suppressive degrees of JAK-STAT and inflammatory response signatures were revealed, which we validated with signaling and cytokine suspension mass cytometry across primary samples. Lastly, we assessed the capacity of JAK2 inhibitors to modulate iron regulation, uncovering potent suppression of hepcidin and SMAD signaling by pacritinib. These comparative findings provide insight into the differential and beneficial effects of ancillary targeting beyond JAK2 and may help guide the use of specific inhibitors in personalized therapy.

Abstract:Myelofibrosis (MF) is a chronic myeloid neoplasm characterized by myeloproliferation, bone marrow fibrosis, splenomegaly, and constitutional symptoms related to pro-inflammatory cytokine signaling. Biologically, MF is characterized by constitutive activation of JAK-STAT signaling; accordingly, JAK inhibitors have been rationally developed to treat MF. Following the initial approval of ruxolitinib in 2011, three additional agents have been approved: fedratinib, pacritinib, and momelotinib. As these therapies are non-curative, allogeneic stem cell transplantation remains a key treatment modality and patients with MF who are deemed candidates should be referred to a transplant center. This potentially curative but toxic approach is typically reserved for patients with higher-risk disease, and JAK inhibitors are recommended in the pre-transplant setting. JAK inhibitors have proven effective at managing splenomegaly and constitutional symptoms and should be started early in the disease course in patients presenting with these clinical manifestations; asymptomatic patients may initially be followed with close surveillance. Drug-related myelosuppression has been a challenge with initial JAK inhibitors, particularly in patients presenting with a cytopenic phenotype. However, newer agents, namely pacritinib and momelotinib, have proven more effective in this setting and are approved for patients with significant thrombocytopenia and anemia, respectively. Resistance or disease progression is clinically challenging and may be defined by several possible events, such as increasing splenomegaly or progression to accelerated or blast phase disease. However, with multiple JAK inhibitors now approved, sequencing of these agents appears poised to improve outcomes. Additionally, novel JAK inhibitors and JAK inhibitor-based combinations are in clinical development.

Abstract:The overactivation of Janus kinases 2 (JAK2) by gain-of-function mutations in the JAK2, Myeloproliferative leukemia virus oncogene, or Calreticulin genes are the most important factor in the development of Philadelphia-negative myeloproliferative neoplasms (MPNs). The discovery of the JAK2V617F mutation is a significant breakthrough in understanding the pathogenesis of MPNs, and inhibition of JAK2 abnormal activation has become one of the most effective strategies against MPNs. Currently, three JAK2 inhibitors for treating MPNs have been approved, and several are being evaluated in clinical trials. However, persistent challenges in terms of drug resistance and off-target effects remain unresolved. In this review, we introduce and classify the available JAK2 inhibitors in terms of their mechanisms and clinical considerations. Additionally, through an analysis of target points, binding modes, and structure–activity inhibitor relationships, we propose strategies such as combination therapy and allosteric inhibitors to overcome specific challenges. This review offers valuable insights into current trends and future directions for optimal management of MPNs using JAK2 inhibitors.

Abstract:Post-myeloproliferative neoplasm secondary Acute Myeloid Leukemia (Post-MPN sAML) is an aggressive and lethal hematologic malignancy arising from myeloproliferative neoplasms (MPN). Hyperactivation of JAK/STAT signaling was identified in 50-90% of MPNs - underscoring its role in driving MPN pathogenesis and transformation to post-MPN sAML. Ruxolitinib, a Type-I JAK2 inhibitor approved for the treatment of myelofibrosis, was tested in a Phase II clinical trial in post-MPN sAML. The trial revealed that ruxolitinib improved the quality of life in patients but was not disease-modifying. Thus, there is an urgent need to find synergistic drug combinations capable of complementing the therapeutic activity of JAK2 inhibitors in post-MPN sAML. Here, we demonstrate that combination treatment of ruxolitinib and CBP30, a bromodomain inhibitor of histone acetyltransferases (HAT) CBP and p300, enhances the therapeutic activity of ruxolitinib in post-MPN sAML. Genome-wide CRISPR loss-of-function screens in JAK2-mutated HEL cells determined that knockout of CBP sensitizes leukemic cells to JAK2 inhibitors. Consistently, functional validation through CRISPR/Cas9 demonstrated that genetic deletion of CBP but not of p300 renders HEL cells sensitive to ruxolitinib treatment. Pharmacologic inhibition of JAK/STAT signaling and HAT activity through ruxolitinib and CBP30 treatment in post-MPN sAML lines HEL and SET2 decreases proliferation and induces apoptosis and cell cycle arrest. Mechanistically, the synergism between ruxolitinib and CBP30 elicits a unique transcriptome profile in HEL with significant downregulation of E2F and MYC targets. In addition, epigenetic profiling revealed that the synergism induces global changes in H3K27ac signals with downregulation and upregulation of H3K27ac signals in predicted STAT5/BCL3 and GATA-bounded regions, respectively. Remarkably, combination treatment of ruxolitinib and CBP30 in HEL luciferase NSG mouse xenografts showed significant leukemia regression and decrease in spleen size compared to mice treated with vehicle or single agents. Overall, these results substantiate the potential of targeting JAK/STAT signaling and histone acetyltransferase activity in the treatment of post-MPN sAML. New Concepts of Treatment for Patients with Myelofibrosis

Abstract:Seven years after the approval of the Janus kinase 1/2 (JAK1/2) inhibitor ruxolitinib, it remains the only drug licensed for the treatment of myelofibrosis. Patients who discontinue ruxolitinib have a dismal outcome, and this is, therefore, an area of significant unmet need. Given the central role that JAK-signal transducer and activator of transcription (STAT) activation plays in disease pathogenesis, there have been many other JAK inhibitors tested, but most have been abandoned, for a variety of reasons. The JAK2-selective inhibitor fedratinib has recently been resurrected, and there has been a resurgence of interest in the failed JAK1/2 inhibitor momelotinib, which possibly improves anemia. Pacritinib, a non-myelosuppressive JAK2-selective inhibitor, is currently in a dose-ranging study mandated by regulatory authorities. A plethora of other targeted agents, most backed by preclinical data, are in various stages of investigation. These include epigenetic and immune therapies, agents targeting cellular survival, metabolic and apoptotic pathways, the cell cycle, DNA repair, and protein folding and degradation, among others. However, at this time, none of these is close to registration or even in a pivotal trial, illustrating the difficulties in recapitulating the clinical disease in preclinical models. Most current clinical trials are testing the addition of a novel agent to ruxolitinib, either in the frontline setting or in the context of an insufficient response to ruxolitinib, or attempting to study new drugs in the second-line, "ruxolitinib failure" setting. Emerging data supports the addition of azacitidine to ruxolitinib in some patients. Other strategies have focused on improving cytopenias, through amelioration of bone marrow fibrosis or other mechanisms. This is important, because cytopenias are the commonest reason for ruxolitinib interruption and/or dose reduction, and dose optimization of ruxolitinib is tied to its survival benefit. The activin receptor ligand trap, sotatercept, and the anti-fibrotic agent, PRM-151, have shown promise in this regard.

Abstract:INTRODUCTION: Myelofibrosis is a clonal hematologic malignancy with clinical manifestations that include cytopenias, debilitating constitutional symptoms, splenomegaly, bone marrow fibrosis and a propensity toward leukemic progression. While allogeneic hematopoietic stem cell transplantation can be curative, this therapy is not available for the majority of patients. Ruxolitinib and fedratinib are approved JAK2 inhibitors that have produced meaningful benefits in terms of spleen reduction and symptom improvement, but there remain several unmet needs.AREAS COVERED: We discuss novel therapies based upon published data from phase II or III clinical trials. Specifically, we cover novel JAK inhibitors (momelotinib and pacritinib), and agents that target bromodomain and extra-terminal domain (pelabresib), the antiapoptotic proteins BCL-2/BCL-xL (navitoclax), MDM2 (navtemadlin), phosphatidylinositol 3-kinase (parsaclisib), or telomerase (imetelstat).EXPERT OPINION: Patients with disease related cytopenias are ineligible for currently approved JAK2 inhibitors. However, momelotinib and pacritinib may be able to fill this void. Novel therapies are being evaluated in the upfront setting to improve the depth and duration of responses with ruxolitinib. Future evaluation of agents must be judged on their potential to modify disease progression, which current JAK2 inhibitors lack. Combination therapy, possibly with an immunotherapeutic agent might serve as key components of future myelofibrosis treatment options.

Abstract:Submitted for the MAR17 Meeting of The American Physical Society Prediction of Robust Non-centrosymmetric Topological Dirac Semi-metallic State in Ternary Half-Heusler Compounds KOUSHIK PAL, UMESH WAGHMARE, Jawaharlal Nehru Ctr Adv Sci, MATERIALS THEORY GROUP TEAM — Topological Dirac semi-metal (TDSM), a novel quantum state of matter with exotic transport, magnetic, chiral and superconducting properties, has been a subject of intense research in recent years. TDSM is a 3-dimensional analogue of graphene, and is also interesting as a parent to other topological states. Although half-Heusler (HH) compounds were shown to exhibit rich topological phases, a robust TDSM phase in them is yet to be discovered. Here, we present a generic topological phase diagram of a large family of HH compounds with strained structures maintaining a three-fold symmetry, and discover their highly robust non-centrosymmetric TDSM state. We show that its symmetry permits a direct control of valley current with parallel electric field. Using an existing, stable half-Heusler LiMgBi as a model system in first-principles theoretical analysis we show that topological semi-metal, topological Dirac semi-metal, normal and topological insulating states are common to strained structure of these HHs which can be realized experimentally through epitaxially grown hetero-structures. Uncovering many HHs exhibiting a variety of topological states, our work opens up exciting phenomena involving chirality, polarity, valley and topology that have the potential for novel technologies. Comparative efficacy and hematologic safety of different dosages of JAK inhibitors in the treatment of myelofibrosis: a network meta-analysis

Abstract:Background: Myelofibrosis (MF) is a myeloproliferative neoplasm characterized by bone marrow fibrosis associated with substantial morbidity and mortality. The therapeutic landscape for MF has advanced with the development of Janus kinase inhibitors (JAKis) like ruxolitinib (RUX), fedratinib (FED), pacritinib (PAC), and momelotinib (MMB), aiming to alleviate symptoms and enhance patient comfort. Methods: A network meta-analysis was conducted to assess the efficacy and safety of eleven JAKi treatment regimens across nine randomized controlled trials (RCTs) with a total of 2340 participants. Outcomes were evaluated in terms of spleen volume reduction (SVR), total symptom score reduction (TSSR), hematological safety profiles, and overall survival (OS). Results: RUX and MMB were superior in achieving SVR and TSSR, with significant dose-response relationships observed. PAC and MMB were associated with a decreased risk of grade 3/4 anemia and thrombocytopenia compared to other JAKis. However, no substantial benefits in OS were observed with newer JAKis compared to RUX. The poorer OS outcomes with certain PAC dosages were likely influenced by baseline patient characteristics, particularly severe cytopenias. Conclusion: The introduction of JAKis significantly changed the treatment of MF. This meta-analysis reaffirms the core role of RUX and positions MMB as a potentially powerful alternative for treating symptoms and reducing spleen size. Meanwhile, MMB and PAC have a positive effect on anemia in MF while FED is more tolerable for patients with thrombocytopenia. However, it should be noted that these results are influenced by baseline patient characteristics, particularly cytopenias, which affects both management and overall survival. Therefore, there is an urgent need for personalized dosing strategies to optimize the balance between efficacy and safety, with careful consideration of patient-specific factors. Abstract:The JAK2 V617F mutation is the most common driver gene in myeloproliferative neoplasm (MPN), which means that the JAK/STAT signaling pathway is persistently activated independent of cytokines, and plays an important part in the onset and development of MPN. The JAK inhibitors, although widely used in the clinical practice, are unable to eradicate MPN. Therefore, the unavoidable long‐term treatment poses a serious burden for patients with MPN. It is established that the JAK2 V617F mutation, in addition its role in the JAK/STAT pathway, can promote cell proliferation, differentiation, anti‐apoptosis, DNA damage accumulation, and other key biologic processes through multiple pathways. Other than that, the JAK2 V617F mutation affects the cardiovascular system through multiple mechanisms. Although JAK inhibitors cannot eradicate MPN cells, the combined use of JAK inhibitors and other drugs may have surprising effects. This requires an in‐depth understanding of the mechanism of action of the JAK2 V617F mutation. In this review, the authors explored the role of the JAK2 V617F mutation in MPN from multiple aspects, including the mechanisms of non‐JAK/STAT pathways, the regulation of cellular methylation, the induction of cellular DNA damage accumulation, and effects on the cardiovascular system, with the objective of providing valuable insights into multidrug combination therapy for MPN.

Abstract:Philadelphia chromosome (Ph)-negative myeloproliferative neoplasms (MPNs) are characterized by stem cell-derived, unrestrained clonal myeloproliferation. The World Health Organization classification system, proposed in 2008, identifies 7 distinct categories of Ph-negative MPNs including essential thrombocythemia (ET); polycythemia vera (PV); primary myelofibrosis (PMF); mastocytosis; chronic eosinophilic leukemia; chronic neutrophilic leukemia; and MPN, unclassifiable. For many years, the treatment of ET, PV, and PMF, the most frequently diagnosed Ph-negative MPNs, has been largely supportive. In recent years, that paradigm has been challenged because of the discovery of a recurrent point mutation in the Janus kinase 2 (JAK2) gene (JAK2(V617F)). This mutation can be detected in the vast majority of patients with PV and approximately half of patients with ET or PMF and serves as both a diagnostic marker as well as representing a putative molecular target for drug development. Several putative targeted agents with significant in vitro JAK2 inhibitory activity and various degrees of JAK2 specificity are currently undergoing clinical evaluation. Furthermore, other investigational non-tyrosine kinase inhibitor approaches such as immunomodulatory agents and pegylated interferon- have also shown promising results in MPNs.
