Abstract:Purpose: In clinical work, accurately measuring the volume and the size of breast cancer is significant to develop a treatment plan. However, it is time-consuming, and inter- and intra-observer variations among radiologists exist. The purpose of this study was to assess the performance of a Res-UNet convolutional neural network based on automatic segmentation for size and volumetric measurement of mass enhancement breast cancer on magnetic resonance imaging (MRI). Materials and methods: A total of 1,000 female breast cancer patients who underwent preoperative 1.5-T dynamic contrast-enhanced MRI prior to treatment were selected from January 2015 to October 2021 and randomly divided into a training cohort (n = 800) and a testing cohort (n = 200). Compared with the masks named ground truth delineated manually by radiologists, the model performance on segmentation was evaluated with dice similarity coefficient (DSC) and intraclass correlation coefficient (ICC). The performance of tumor (T) stage classification was evaluated with accuracy, sensitivity, and specificity. Results: In the test cohort, the DSC of automatic segmentation reached 0.89. Excellent concordance (ICC > 0.95) of the maximal and minimal diameter and good concordance (ICC > 0.80) of volumetric measurement were shown between the model and the radiologists. The trained model took approximately 10-15 s to provide automatic segmentation and classified the T stage with an overall accuracy of 0.93, sensitivity of 0.94, 0.94, and 0.75, and specificity of 0.95, 0.92, and 0.99, respectively, in T1, T2, and T3. Conclusions: Our model demonstrated good performance and reliability for automatic segmentation for size and volumetric measurement of breast cancer, which can be time-saving and effective in clinical decision-making.
Quantitative Analysis of Bladder Wall Thickness for Magnetic Resonance Cystoscopy

Abstract:Objective: To find an effective way for quantitative evaluation on wall thickness variation of human bladder with/without bladder tumor, a novel pipeline of thickness measurement and analysis for magnetic resonance (MR) cystography is proposed. Methods: After the acquisition of volumetric bladder images with a high-resolution T2-weighted 3-D sequence, the inner and outer borders of the bladder wall were segmented simultaneously by a coupled directional level-set method. Then, the bladder wall thickness (BWT) was estimated using the Laplacian method. To reducing the influence of individual variation and urine filling on wall thickness, a thickness normalization using Z-score is performed. Finally, a parametric surface mapping strategy was applied to map thickness distribution onto a unified sphere surface, for quantitative intra- and intersubject comparison between bladders of different shapes. Results: The proposed pipeline was tested with a database composed of MR bladder images acquired from 20 volunteers and 20 patients with bladder cancer. The results indicate that the thickness normalization step using Z-score makes the quantitative comparison of wall thickness quite possible and there is a significant difference on BWT between patients and volunteers. Using the proposed pipeline, we established a thickness template for a normal bladder wall based on dataset of all volunteers. Conclusion: As a first attempt to establish a general pipeline for bladder wall analysis, the presented work provides an effective way to achieve the goal of evaluating the entire bladder wall for detection and diagnosis of abnormality. In addition, it can be easily extended to quantitative analyses of other bladder features, such as, intensity-based or texture features.
Cross-Field Strength and Multi-Vendor Validation of MagDensity for MRI-based Quantitative Breast Density Analysis

Abstract:Purpose: Breast density (BD) is a significant risk factor for breast cancer, yet current assessment methods lack automation, quantification, and cross-platform consistency. This study aims to evaluate MagDensity, a novel magnetic resonance imaging (MRI)-based quantitative BD measure, for its validity and reliability across different imaging platforms. Methods: Ten healthy volunteers participated in this prospective study, undergoing fat-water MRI scans on three scanners: 3T Siemens Prisma, 3T Siemens Biograph mMR, and 1.5T GE Signa. Great effort was made to schedule all scans within a narrow three-hour window on the same day to minimize any potential intraday variations, highlighting the logistical challenges involved. BD was assessed using the MagDensity technique, which included combining magnitude and phase images, applying a fat-water separation technique, employing an automated whole-breast segmentation algorithm, and quantifying the volumetric water fraction. The agreement between measures was analyzed using mean differences, two-tailed t-tests, Pearson's correlation coefficients, and Bland-Altman plots. Results: No statistically significant differences in BD measurements by MagDensity within the same field strength and vendor (3T Siemens), with high correlation (Pearson’s r > 0.99) and negligible mean differences (< 0.2%). Cross-platform comparison between the 3T Siemens and the 1.5T GE scanners showed mean differences of < 5%. After linear calibration, these variations were reduced to insignificant levels, yielding a strong correlation (Pearson’s r > 0.97) and mean differences within ±0.2%. Conclusion: MagDensity, an MRI-based BD measure, exhibits robustness and reliability across diverse scanner models, vendors, and field strengths, marking a promising advancement towards standardizing BD measurements across multiple MRI platforms. It provides a valuable tool for monitoring subtle longitudinal changes in BD, which is vital for breast cancer prevention and personalized treatment strategies.

Abstract:Objective To Compare different methods of quantitative breast density measurement.Methods The study included sixty patients who underwent both mammography and breast MRI. The breast density was computed automatically on digital mammograms with R2 workstation. Two experienced radiologists read the mammograms and assessed the breast density with Wolfe and ACR classification respectively. Fuzzy C-means clustering algorithm (FCM) was used to assess breast density on MRI. Each assessment method was repeated after 2 weeks. Spearman and Pearson correlations of inter- and intrareader and intermodality were computed for density estimates. Results Inter- and intrareader correlation of Wolfe classification were 0. 74 and 0. 65, and they were 0. 74 and 0. 82 for ACR classification respectively.Correlation between Wolfe and ACR classification was 0. 77. High interreader correlation of 0. 98 and intrareader correlation of 0. 96 was observed with MR FCM measurement. And the correlation between digital mammograms and MRI was high in the assessment of breast density (r = 0. 81, P ＜ 0. 01). Conclusion High correlation of breast density estimates on digital mammograms and MRI FCM suggested the former could be used as a simple and accurate method.
A publicly available deep learning model and dataset for segmentation of breast, fibroglandular tissue, and vessels in breast MRI

Abstract:Breast density, or the amount of fibroglandular tissue (FGT) relative to the overall breast volume, increases the risk of developing breast cancer. Although previous studies have utilized deep learning to assess breast density, the limited public availability of data and quantitative tools hinders the development of better assessment tools. Our objective was to (1) create and share a large dataset of pixel-wise annotations according to well-defined criteria, and (2) develop, evaluate, and share an automated segmentation method for breast, FGT, and blood vessels using convolutional neural networks. We used the Duke Breast Cancer MRI dataset to randomly select 100 MRI studies and manually annotated the breast, FGT, and blood vessels for each study. Model performance was evaluated using the Dice similarity coefficient (DSC). The model achieved DSC values of 0.92 for breast, 0.86 for FGT, and 0.65 for blood vessels on the test set. The correlation between our model's predicted breast density and the manually generated masks was 0.95. The correlation between the predicted breast density and qualitative radiologist assessment was 0.75. Our automated models can accurately segment breast, FGT, and blood vessels using pre-contrast breast MRI data. The data and the models were made publicly available.

Improved accuracy of breast volume calculation from 3D surface imaging data using statistical shape models

Abstract:Background Three-dimensional (3D) scanning is an established method of breast volume estimation. However, this method can never be entirely precise, since the thoracic wall cannot be imaged by the surface scanner. Current methods rely on interpolation of the posterior breast border from the surrounding thoracic wall. Here, we present a novel method to calculate the posterior border and increase the accuracy of the measurement. Methods Using principal component analysis, computed tomography images were used to build a statistical shape model (SSM) of the thoracic wall. The model was fitted to 3D images and the missing thoracic wall curvature interpolated (indirect volumetry). The calculations were evaluated by ordinary least squares regression between the preoperative and postoperative volume differences and the resection weights in breast reduction surgery (N = 36). Also, an SSM of the breast was developed, allowing direct volumetry. Magnetic-resonance images (MRI) and 3D scans were acquired from 5 patients in order to validate the direct 3D volumetry. Results Volumetry based on a SSM exhibited a higher determination coefficient (R 2 = 0,737) than the interpolation method (R 2 = 0,404). The methods were not equivalent (p = 0.75), suggesting that the methods significantly differ. There was no influence of BMI on the correlation in either method. The MRI volumetry had a strong correlation with the 3D volumetry (R 2 = 0,978). Conclusion The SSM-based method of posterior breast border calculation is reliable and superior to the currently used method of interpolation. It should serve as a basis of software applications aiming at calculation of breast volume from 3D surface scanning data.

Automated 3D ultrasound image segmentation for assistant diagnosis of breast cancer

Abstract:Segmentation of an ultrasound image into functional tissues is of great importance to clinical diagnosis of breast cancer. However, many studies are found to segment only the mass of interest and not all major tissues. Differences and inconsistencies in ultrasound interpretation call for an automated segmentation method to make results operator-independent. Furthermore, manual segmentation of entire three-dimensional (3D) ultrasound volumes is time-consuming, resource-intensive, and clinically impractical. Here, we propose an automated algorithm to segment 3D ultrasound volumes into three major tissue types: cyst/mass, fatty tissue, and fibro-glandular tissue. To test its efficacy and consistency, the proposed automated method was employed on a database of 21 cases of whole breast ultrasound. Experimental results show that our proposed method not only distinguishes fat and non-fat tissues correctly, but performs well in classifying cyst/mass. Comparison of density assessment between the automated method and manual segmentation demonstrates good consistency with an accuracy of 85.7%. Quantitative comparison of corresponding tissue volumes, which uses overlap ratio, gives an average similarity of 74.54%, consistent with values seen in MRI brain segmentations. Thus, our proposed method exhibits great potential as an automated approach to segment 3D whole breast ultrasound volumes into functionally distinct tissues that may help to correct ultrasound speed of sound aberrations and assist in density based prognosis of breast cancer.
Fully automatic quantification of fibroglandular tissue and background parenchymal enhancement with accurate implementation for axial and sagittal breast MRI protocols

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Purpose</h3><p>To propose and evaluate a fully automated technique for quantification of fibroglandular tissue (FGT) and background parenchymal enhancement (BPE) in breast MRI.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>We propose a fully‐automated method, where after preprocessing, FGT is segmented in T1‐weighted, nonfat‐saturated MRI. Incorporating an anatomy‐driven prior probability for FGT and robust texture descriptors against intensity variations, our method effectively addresses major image processing challenges, including wide variations in breast anatomy and FGT appearance among individuals. Our frame‐work then propagates this segmentation to dynamic contrast enhanced (DCE)‐MRI to quantify BPE within the segmented FGT regions. Axial and sagittal image data from 40 cancer‐unaffected women were used to evaluate our proposed method versus a manually‐annotated reference standard.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>High spatial correspondence was observed between the automatic and manual FGT segmentation (mean Dice similarity coefficient 81.14%). The FGT and BPE quantifications (denoted FGT% and BPE%) indicated high correlation (Pearson's <i>r</i> = 0<i>:</i>99 for both) between automatic and manual segmentations. Furthermore, the differences between the FGT% and BPE% quantified using automatic and manual segmentations were low (mean differences: ‐0<i>:</i>66±2<i>:</i>91% for FGT% and ‐0<i>:</i>17±1<i>:</i>03% for BPE%). When correlated with qualitative clinical BI‐RADS ratings, the correlation coefficient for FGT% was still high (Spearman's <i>ρ</i> = 0<i>:</i>92), whereas that for BPE was lower (<i>ρ</i> = 0<i>:</i>65). Our proposed approach also performed significantly better than a previously validated method for sagittal breast MRI. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Our method demonstrated accurate fully‐automated quantification of FGT and BPE in both sagittal and axial breast MRI. Our results also suggested the complexity of BPE assessment, demonstrating relatively low correlation between segmentation and clinical rating.</p></section>

Interpretable deep learning regression for breast density estimation on MRI

Abstract:Breast density, which is the ratio between fibroglandular tissue (FGT) and total breast volume, can be assessed qualitatively by radiologists and quantitatively by computer algorithms. These algorithms often rely on segmentation of breast and FGT volume. In this study, we propose a method to directly assess breast density on MRI, and provide interpretations of these assessments. We assessed breast density in 506 patients with breast cancer using a regression convolutional neural network (CNN). The input for the CNN were slices of breast MRI of 128 x 128 voxels, and the output was a continuous density value between 0 (fatty breast) and 1 (dense breast). We used 350 patients to train the CNN, 75 for validation, and 81 for independent testing. We investigated why the CNN came to its predicted density using Deep SHapley Additive exPlanations (SHAP). The density predicted by the CNN on the testing set was significantly correlated with the ground truth densities (N = 81 patients, Spearman's rho = 0.86, P < 0.001). When inspecting what the CNN based its predictions on, we found that voxels in FGT commonly had positive SHAP-values, voxels in fatty tissue commonly had negative SHAP-values, and voxels in non-breast tissue commonly had SHAP-values near zero. This means that the prediction of density is based on the structures we expect it to be based on, namely FGT and fatty tissue. To conclude, we presented an interpretable deep learning regression method for breast density estimation on MRI with promising results.

Abstract:PURPOSE: To compare MRI sequences for breast density measurements on a 3T MRI system using IDEAL (Iterative Decomposition of water and fat with Echo Asymmetry and Least squares estimation) as possible physiology-like reference.MATERIALS AND METHODS: MRI examination was performed in 48 consecutive patients (mean age 41, years; range, 35-67 years) on a 3.0T scanner and 46 were included. All (fertile) women, were examined between days 5 and 15 of their menstrual cycle. MRI protocol included: T1-turbo spin-echo (T1-tSE), T2-turbo spin-echo (T2-tSE), VIBRANT (Volume Imaging for Breast Assessment) before and after injection of contrast media and IDEAL. Breast density was calculated with semi-automated software. Statistical analysis was performed with non-parametric tests.RESULTS: Mean percentage of breast density calculated in each sequence was: T1-tSE = 56%; T2-tSE = 52%; IDEAL FatOnly = 55%; IDEAL WaterOnly = 53%, VIBRANT = 55%. Significant differences were observed between T2-tSE and both T1-tSE (p<0.001), VIBRANT sequences (p = 0.009), T1-tSE and both IDEAL WaterOnly (p = 0.007) and IDEAL FatOnly (p = 0.047). Breast density percentage showed a positive linear correlation among different sequences: r ≥ 0.93.CONCLUSIONS: Differences exist between MRI sequences used to assess breast density percentage. T1-weighted sequences values were similar to IDEAL sequences.

Abstract:Mammographic density is a strong risk factor for breast cancer (BC) and is reported clinically as part of Breast Imaging Reporting and Data System (BI-RADS) results issued by radiologists. Automated assessment of density is needed that can be used for both full-field digital mammography (FFDM) and digital breast tomosynthesis (DBT) as both types of exams are acquired in standard clinical practice. We trained a deep learning model to automate the estimation of BI-RADS density from a prospective Washington University (WashU) clinic-based cohort of 9,714 women, entering into the cohort in 2013 with follow-up through, October 31, 2020. The cohort included 27% non-Hispanic Black women. The trained algorithm was assessed in an external validation cohort that included 18,360 women screened at Emory from January 1, 2013 and followed through December 31, 2020 that included 42% non-Hispanic Black women. Our model-estimated BI-RADS density demonstrated substantial agreement with the density as assessed by radiologist. In the external validation, the agreement with radiologists for category B 81% and C 77% for FFDM and B 83% and C 74% for DBT show important distinction for separation of women with dense breast. We obtained a Cohen's κ of 0.72 (95% CI, 0.71, 0.73) in FFDM and 0.71 (95% CI 0.69, 0.73) in DBT. We provided a consistent and fully automated BI-RADS estimation for both FFDM and DBT using a deep learning model. The software can be easily implemented anywhere for clinical use and risk prediction.

Abstract:Background The level of background parenchymal enhancement (BPE) at breast MRI provides predictive and prognostic information and can have diagnostic implications. However, there is a lack of standardization regarding BPE assessment. Purpose To investigate how well results of quantitative BPE assessment methods correlate among themselves and with assessments made by radiologists experienced in breast MRI. Materials and Methods In this pseudoprospective analysis of 5773 breast MRI examinations from 3207 patients (mean age, 60 years ± 10 [SD]), the level of BPE was prospectively categorized according to the Breast Imaging Reporting and Data System by radiologists experienced in breast MRI. For automated extraction of BPE, fibroglandular tissue (FGT) was segmented in an automated pipeline. Four different published methods for automated quantitative BPE extractions were used: two methods (A and B) based on enhancement intensity and two methods (C and D) based on the volume of enhanced FGT. The results from all methods were correlated, and agreement was investigated in comparison with the respective radiologist-based categorization. For surrogate validation of BPE assessment, how accurately the methods distinguished premenopausal women with (n = 50) versus without (n = 896) antihormonal treatment was determined. Results Intensity-based methods (A and B) exhibited a correlation with radiologist-based categorization of 0.56 ± 0.01 and 0.55 ± 0.01, respectively, and volume-based methods (C and D) had a correlation of 0.52 ± 0.01 and 0.50 ± 0.01 (P < .001). There were notable correlation differences (P < .001) between the BPE determined with the four methods. Among the four quantitation methods, method D offered the highest accuracy for distinguishing women with versus without antihormonal therapy (P = .01). Conclusion Results of different methods for quantitative BPE assessment agree only moderately among themselves or with visual categories reported by experienced radiologists; intensity-based methods correlate more closely with radiologists' ratings than volume-based methods. © RSNA, 2024 Supplemental material is available for this article. Abstract:Summary Background Assessment of breast volume is essential in preoperative planning of immediate breast reconstruction (IBR) surgery in order to achieve a satisfactory cosmetic outcome. This study introduced a breast volume measurement tool that can perform segmentation of magnetic resonance images (MRI) and calculate breast volume automatically. We compared the accuracy and reliability of this measurement method with four other conventional modalities. Methods Patients scheduled to undergo mastectomy with IBR between 2016 and 2021 were enrolled in the study. Five different breast volume assessments, including automatic segmentation of MRI, manual segmentation of MRI, 3D surface imaging, mammography, and the BREAST-V formula, were used to evaluate different breast volumes. The results were validated using water displacement volumes of the mastectomy specimens. Results In this pilot study, a total of 50 female patients met the inclusion criteria and contributed 54 breast specimens to volumetric analysis. There was a strong linear association between the MRI methods and the water displacement methods (automatic segmentation: r=0.911, p<0.001; manual segmentation: r=0.924, p<0.001), followed by 3D surfacing imaging (r=0.858, p<0.001), mammography (r=0.841, p<0.001), and the Breast-V formula method (r=0.838, p<0.001). Breast volumes measured by automatic and manual segmentation of MRI had lower mean relative errors (30.3%+/-22.0 and 28.9%+/-19.8 respectively) than 3D surface imaging (38.9%+/-31.2), Breast-V formula (44.8%+/-25.8) and mammography (60.3%+/-37.6). Conclusion Breast volume assessment using the MRI methods had better accuracy and reliability than the other methods in our study. Breast volume measurement using automatic segmentation of MRI could be more efficient.

Abstract:The evaluation of mammographic breast density, a critical indicator of breast cancer risk, is traditionally performed by radiologists via visual inspection of mammography images, utilizing the Breast Imaging-Reporting and Data System (BI-RADS) breast density categories. However, this method is subject to substantial interobserver variability, leading to inconsistencies and potential inaccuracies in density assessment and subsequent risk estimations. To address this, we present a deep learning-based automatic detection algorithm (DLAD) designed for the automated evaluation of breast density. Our multicentric, multi-reader study leverages a diverse dataset of 122 full-field digital mammography studies (488 images in CC and MLO projections) sourced from three institutions. We invited two experienced radiologists to conduct a retrospective analysis, establishing a ground truth for 72 mammography studies (BI-RADS class A: 18, BI-RADS class B: 43, BI-RADS class C: 7, BI-RADS class D: 4). The efficacy of the DLAD was then compared to the performance of five independent radiologists with varying levels of experience. The DLAD showed robust performance, achieving an accuracy of 0.819 (95% CI: 0.736–0.903), along with an F1 score of 0.798 (0.594–0.905), precision of 0.806 (0.596–0.896), recall of 0.830 (0.650–0.946), and a Cohen's Kappa (κ) of 0.708 (0.562–0.841). The algorithm achieved robust performance that matches and in four cases exceeds that of individual radiologists. The statistical analysis did not reveal a significant difference in accuracy between DLAD and the radiologists, underscoring the model's competitive diagnostic alignment with professional radiologist assessments. These results demonstrate that the deep learning-based automatic detection algorithm can enhance the accuracy and consistency of breast density assessments, offering a reliable tool for improving breast cancer screening outcomes.

Deep-LIBRA: Artificial intelligence method for robust quantification of breast density with independent validation in breast cancer risk assessment

Abstract:Breast density is an important risk factor for breast cancer that also affects the specificity and sensitivity of screening mammography. Current federal legislation mandates reporting of breast density for all women undergoing breast screening. Clinically, breast density is assessed visually using the American College of Radiology Breast Imaging Reporting And Data System (BI-RADS) scale. Here, we introduce an artificial intelligence (AI) method to estimate breast percentage density (PD) from digital mammograms. Our method leverages deep learning (DL) using two convolutional neural network architectures to accurately segment the breast area. A machine-learning algorithm combining superpixel generation, texture feature analysis, and support vector machine is then applied to differentiate dense from non-dense tissue regions, from which PD is estimated. Our method has been trained and validated on a multi-ethnic, multi-institutional dataset of 15,661 images (4,437 women), and then tested on an independent dataset of 6,368 digital mammograms (1,702 women; cases=414) for both PD estimation and discrimination of breast cancer. On the independent dataset, PD estimates from Deep-LIBRA and an expert reader were strongly correlated (Spearman correlation coefficient = 0.90). Moreover, Deep-LIBRA yielded a higher breast cancer discrimination performance (area under the ROC curve, AUC = 0.611 [95% confidence interval (CI): 0.583, 0.639]) compared to four other widely-used research and commercial PD assessment methods (AUCs = 0.528 to 0.588). Our results suggest a strong agreement of PD estimates between Deep-LIBRA and gold-standard assessment by an expert reader, as well as improved performance in breast cancer risk assessment over state-of-the-art open-source and commercial methods.

Abstract:Objectives Background parenchymal enhancement (BPE) on dynamic contrast-enhanced MRI (DCE-MRI) as rated by radiologists is subject to inter- and intrareader variability. We aim to automate BPE category from DCE-MRI. Methods This study represents a secondary analysis of the Dense Tissue and Early Breast Neoplasm Screening trial. 4553 women with extremely dense breasts who received supplemental breast MRI screening in eight hospitals were included. Minimal, mild, moderate and marked BPE rated by radiologists were used as reference. Fifteen quantitative MRI features of the fibroglandular tissue were extracted to predict BPE using Random Forest, Naïve Bayes, and KNN classifiers. Majority voting was used to combine the predictions. Internal-external validation was used for training and validation. The inverse-variance weighted mean accuracy was used to express mean performance across the eight hospitals. Cox regression was used to verify non inferiority of the association between automated rating and breast cancer occurrence compared to the association for manual rating. Results The accuracy of majority voting ranged between 0.56 and 0.84 across the eight hospitals. The weighted mean prediction accuracy for the four BPE categories was 0.76. The hazard ratio (HR) of BPE for breast cancer occurrence was comparable between automated rating and manual rating (HR = 2.12 versus HR = 1.97, P = 0.65 for mild/moderate/marked BPE relative to minimal BPE). Conclusion It is feasible to rate BPE automatically in DCE-MRI of women with extremely dense breasts without compromising the underlying association between BPE and breast cancer occurrence. The accuracy for minimal BPE is superior to that for other BPE categories.

Abstract:In light of the global increase in breast cancer cases and the crucial importance of the density of fibroglandular tissue (FGT) in assessing risk and predicting the course of the disease, the accurate measurement of FGT emerges as a significant challenge in diagnostic imaging. The current study focuses on the automatic segmentation of breast glandular tissue in MRI scans using a deep learning model. The aim is to establish a solid foundation for the development of methods for the precise quantification of fibroglandular tissue. For this purpose, the publicly available 'Duke Breast Cancer MRI' dataset was systematically processed to train a deep neural network model utilizing the nnU-Net ('no-new-Net') framework, which was then subjected to a quantitative evaluation. The results show the following macro-averaged metrics with standard deviation: Dice Similarity Coefficient 0.827 ± 0.152, accuracy 0.997 ± 0.003, sensitivity 0.825 ± 0.158, and specificity 0.999 ± 0.001. The effectiveness of our model in segmenting FGT is underscored by the high values of the Dice coefficient, Accuracy, Sensitivity, and Specificity, which reflect the precision and reliability of our results. The findings of this study lay a solid foundation for developing automated methods to quantify FGT. Our research efforts, especially driven by clinical studies at the University Hospital Augsburg, are focused on further exploring and validating these potentials.

Abstract:Chronic cerebral hypoperfusion (CCH) is a general pathophysiological condition occurring in vascular dementia (VaD) associated with negative impact on cognitive functions. Ryanodine as well as cysteinyl leukotriene-1 receptors (RyRs and CysLT1Rs) are extensively present in the central nervous system, where they participate in regulation of cognition, motivation, inflammation and neurodegeneration. The purpose of this study is to examine the role of ruthenium red; a selective RyR blocker as well as montelukast; a specific CysLT1 antagonist in CCH induced VaD in mice. Two vessel occlusion (2VO) or permanent ligation of bilateral common carotid arteries technique was used to induce CCH in mice. Animals with bilateral carotid arteries occlusion have revealed impaired learning and memory (Morris water maze), cholinergic dysfunction (increased acetylcholinesterase activity) as well as increased brain oxidative stress (reduction in brain superoxide dismutase, glutathione and catalase with an increase in thiobarbituric acid reactive substance level), with increased brain infarct size (2,3,5-triphenylterazolium chloride staining). While, administration of ruthenium red and montelukast considerably attenuated CCH induced cognitive impairments, cholinergic dysfunction, brain oxidative stress as well as brain damage. The results suggest that bilateral carotid arteries occlusion induced CCH has brought out VaD, which was attenuated by treatment with ruthenium red and montelukast. Therefore, modulation of RyRs as well as CysLT1 receptors may provide help in conditions involving CCH such as cognitive impairment and VaD.

Abstract:Breast cancer poses a threat to the lives of many women. Breast density is a closely related indicator of breast cancer risk. The aim of this paper is to propose a classification system for breast density, which can appropriately segment the glandular tissue from the whole breast and to achieve a better classification result. A new threshold method is applied to segment the breast glandular tissue. The gray level co-occurrence matrix (GLCM) is implemented to extract the texture features of the glandular tissue. Meanwhile, we obtain three statistical features (mean, skewness, kurtosis). In addition, the calculated breast density that is served as a new feature is added to the feature vectors. The mixed feature vectors are classified by Support Vector Machine (SVM) and Ultimate Learning Machine (ELM). Ten-fold cross-validation is used to verify the classifier performance. The system using the SVM achieves 96.19% accuracy for three density types in the MIAS database and achieves 96.35% accuracy of four density types in the DDSM database. The accuracy in the database mixed with the local database was 95.01% and there are three density types in the mixed database. The experimental results indicate that the system proposed has a better performance in breast density classification. The system proposed in this paper can be considered to help the physician to classify breast density.
Computer-aided assessment of breast density: comparison of supervised deep learning and feature-based statistical learning

Abstract:Breast density is one of the most significant factors that is associated with cancer risk. In this study, our purpose was to develop a supervised deep learning approach for automated estimation of percentage density (PD) on digital mammograms (DMs). The input 'for processing' DMs was first log-transformed, enhanced by a multi-resolution preprocessing scheme, and subsampled to a pixel size of 800 µm × 800 µm from 100 µm × 100 µm. A deep convolutional neural network (DCNN) was trained to estimate a probability map of breast density (PMD) by using a domain adaptation resampling method. The PD was estimated as the ratio of the dense area to the breast area based on the PMD. The DCNN approach was compared to a feature-based statistical learning approach. Gray level, texture and morphological features were extracted and a least absolute shrinkage and selection operator was used to combine the features into a feature-based PMD. With approval of the Institutional Review Board, we retrospectively collected a training set of 478 DMs and an independent test set of 183 DMs from patient files in our institution. Two experienced mammography quality standards act radiologists interactively segmented PD as the reference standard. Ten-fold cross-validation was used for model selection and evaluation with the training set. With cross-validation, DCNN obtained a Dice's coefficient (DC) of 0.79 ± 0.13 and Pearson's correlation (r) of 0.97, whereas feature-based learning obtained DC = 0.72 ± 0.18 and r = 0.85. For the independent test set, DCNN achieved DC = 0.76 ± 0.09 and r = 0.94, while feature-based learning achieved DC = 0.62 ± 0.21 and r = 0.75. Our DCNN approach was significantly better and more robust than the feature-based learning approach for automated PD estimation on DMs, demonstrating its potential use for automated density reporting as well as for model-based risk prediction.
