Abstract:We present 16 patients, who received a totally implantable venous port catheter insertion for chemotherapy and later suffered broken catheter falling off into the heart or vena cava system, and provide an upgrade of some percutaneous techniques for retrieval. From January 2010 to December 2015, chest radiographs showed in 16 patients with port catheter after tumor resection that the catheter part of the port had dislodged and migrated to the right atrium or inferior vena cava. Although no significant complications occurred, all patients underwent a transluminal method to retrieve the foreign body. The vascular tools used in these procedures included loop snare, basket, soft-Uv catheter, and guidewires. All the procedures were performed with local anesthesia, approaching from the right femoral vein. All 16 retrievals were successfully accomplished. It took 15 to 55 minutes to performing these operations. Stone basket was used in four patients and loop snares were used in the other 12 patients (3 gooseneck and 9 EN snares). In two patients a soft-Uv catheter was used combined with guidewires and snares. All patients received surgery at the puncture site to take out the ports when the catheter fragment was removed. We pulled out the catheters after they were folded in two by the snares, along with the sheath as a unit. N o severe femoral vein injury and bleeding occurred in these patients. A catheter broken or separated from its port in application of the chemotherapy pump insertion should be cautioned, and intravascular retrieval may be the best to deal with it.Fig 2View Large Image Figure ViewerDownload Hi-res image Download (PPT)

Abstract:The peripherally inserted central catheter (PICC) is a non-tunneled central venous catheter placed in the upper limb venous system, mainly in the basilic vein, and the tip terminates in the superior vena cava (SVC). A PICC is a preferred modality of central venous access in oncology, as it is associated with minimal discomfort and can be kept in situ for up to one year. Despite multiple advantages, it is also associated with complications. Fracture and migration are rare but potentially serious complications that can lead to arrhythmias, cardiac perforation, cardiac tamponade, pulmonary embolism, and sepsis. The migrated PICC fragment can be retrieved using percutaneous techniques, which have a high success rate of excess, with minimum complications. In our patient of adenocarcinoma gastroesophageal junction, the fractured and migrated PICC to pulmonary arteries was retrieved using the balloon catheter method. With more and more cancer patients using PICCs for chemotherapy administration, healthcare workers must be aware of the standard and sporadic complications of PICCs. Care of the PICC is crucial, and any lapse may lead to fracture and embolization, which is a potentially life-threatening complication. This case highlights the importance of healthcare persons being aware of the possibility of catheter embolization and methods to prevent and mitigate this phenomenon.

Abstract:Totally implantable intravenous port catheters (TIVPs) and venous peripherally inserted central catheters (PICCs) are used to deliver chemotherapy in patients with breast cancer. [ 1 ] TIVP is a safe and effective chemotherapy route for patients with cancer. [ 2 ] The incidence of central venous stenosis is greater in patients with breast cancer after placement of TIVPs via the left internal jugular vein (IJV). The possibility of left innominate vein stenosis should be considered when a left TIVP needs to be placed in patients with right-sided breast cancer. [ 3 ] Transfemoral PICC is a medium-to-long-term intravenous infusion with the catheter tip in the inferior vena cava (IVC) for patients without the option of a superior vena cava infusion. [ 4 ] Intracavitary electrocardiography (IC-ECG) accurately locates the tip of the femoral vein catheter, minimizing the incidence of catheter-related complications and the time and cost of localization. [ 5 ] We successfully established venous access for a patient with breast cancer and a history of left IJV access infusion port placement and ipsilateral innominate vein stenosis after placement. Re-treatment using a PICC ectopic to the internal thoracic vein via the left upper limb and a femoral vein tunneled PICC combined with IC-ECG positioning technology enabled the patient to complete the treatment cycle successfully.

Abstract:Introduction Central venous catheterization is a common tool widely used in medical treatment for long-term intravenous administration of medication (e.g. chemotherapy or antibiotics) or parenteral nutrition. Here, we present a case of a missing peripherally inserted central catheter (PICC) which was detected on computed tomography (CT) scan thorax. Case Report A 50-year-old female admitted for follow-up for adenocarcinoma rectum was found to have a missing PICC line and advised CT thorax for evaluation. The imaging findings revealed coiled PICC line in the right atrium extending to inferior vena cava and into right ventricle and further extending and coiled PICC is seen in the main, right, and left pulmonary arteries. Through emergency interventional procedure, the PICC line was successfully retrieved percutaneously via the right femoral vein. Conclusion PICC fracture is less common and always seen without significant discomfort if not found timely, and it may lead to serious complications, such as pulmonary embolism, and even death. Early detection and removal may help to prevent signiﬁcant mortality.

Abstract:BACKGROUND Peripherally inserted central catheters (PICCs) are an essential infusion route for oncology patients receiving intravenous treatments, but lower extremity venipuncture is the preferred technique for patients with superior vena cava syndrome (SVCS). We report the case of a patient with a lower extremity PICC ectopic to the ascending lumbar vein, to indicate and verify PICC catheterisation in the lower extremity is safe and feasible. And hope to provide different perspectives for clinical PICC venipuncture to get the attention of peers. CASE SUMMARY On 24 August 2022, a 58-year-old male was admitted to our department due to an intermittent cough persisting for over a month, which worsened 10 d prior. Imaging and laboratory investigations suggested the patient with pulmonary malignancy and SVCS. Chemotherapy was not an absolute contraindication in this patient. Lower extremity venipuncture is the preferred technique because administering upper extremity venous transfusion to patients with SVCS can exacerbate oedema in the head, neck, and upper extremities. The patient and his family were informed about the procedure, and informed consent was obtained. After successful puncture and prompt treatment, the patient was discharged, experiencing some relief from symptoms. CONCLUSION Inferior vena cava catheterisation is rare and important for cancer patients with SVCS, particularly in complex situations involving ectopic placement.

Abstract:Abstract Objective With the increasing use of a peripherally inserted central catheter (PICC) in clinical application, the breaking of the PICC is increasing as well, which would turn into the emboli and drift to the heart and pulmonary artery, causing severe consequences. However, few cases have been reported on the rescue of patients with a broken PICC. Patient concerns A 33-year-old man, diagnosed with chronic hepatitis B-related decompensated cirrhosis and cryptococcal meningitis, was treated with amphotericin B combined with flucytosine and fluconazole by means of PICC catheterization. The patient was discharged with a catheter; when he returned for re-examination, a 50cm length of PICC broke and slipped into the vein after his sudden dysphoria. First aid was immediately administered, and then the intervention therapy to extricate the tube, including pulmonary artery angiography and intravascular removal of foreign matter, was performed based on the consensus of the in-hospital vein treatment group. At last, the broken PICC fragment was successfully taken out of the vessel. Re-examination after surgery showed that he recovered well. Conclusions Once the catheter is broken, the X-ray examination should be performed at the first instance and re-examined frequently. Moreover, the involvement of a multidisciplinary team should be formed to decide the appropriate method of treatment to ensure a successful rescue.
The Incidence, Risk Factors, and Patterns of Peripherally Inserted Central Catheter-Related Venous Thrombosis in Cancer Patients Followed Up by Ultrasound

Abstract:PURPOSE:A peripherally inserted central catheter (PICC) is associated with venous thromboembolism (VTE) especially in patients suffering from cancer. We analyzed the incidence, risk factors, and patterns of PICC-related VTE in cancer patients.PATIENTS AND METHODS:Patients with cancer who underwent PICC placement were evaluated retrospectively. Routine, prospective ultrasound post-PICC placement was used for asymptomatic and symptomatic patients to identify VTE. Multivariable logistic regression models with odds ratios (ORs) were used to examine VTE risk factors.RESULTS:Of 2353 PICCs placed, 165 patients (7.01%) developed PICC-related VTE with a median thrombosis time of 12 days. After adjustment of multivariable analysis, patients with PICC-related VTE were more likely to have a ratio of PICC diameter:vein diameter >0.35 (adjusted OR, 1.689; 95% CI, 1.023-2.789) and high level of triglycerides (1.561; 1.096-2.223). The prevalence of A (adjusted OR, 1.680; 95% CI, 1.009-2.798), B (1.835; 1.137-2.961), and AB (3.275; 1.840-5.829) blood group was significantly higher than that of the O blood group in VTE patients. Venous recanalization was observed in 44.8% (74/165) patients after anticoagulation therapy, and more often in patients with combined deep VTE than in patients with isolated superficial VTE (OR, 17.942; 95% CI, 5.427-59.316). The recanalization time was 20±5 (range, 10-31) days.CONCLUSION:The non-O blood group, larger ratio of PICC diameter:vein diameter, and high level of triglycerides were significantly associated with PICC-related VTE. Almost half of cases of PICC-related deep VTE could be reversed by anticoagulation treatment.

Abstract:Peripherally inserted central catheter (PICC), is a peripheral vein puncture, catheter tip enters into central vein. Drugs are directly infused in central vein for avoiding the damage on blood vessels caused by long-term infusion or the infusions from hypertonic and irritating drugs in order to alleviate the pain caused by repeated puncture and improve the quality of life among patients, [ 1 ] which is now widely used in the chemotherapy of tumors or other diseases, [ 2 , 3 ] the parenteral nutritional pathway for adults postoperatively, [ 4 ] and the creation of nutritional pathway for preterm infants, [ 5 ] and so on. Although there are many advantages of PICC, such as avoiding the stimulation of blood vessels caused by drugs for patients who receiving oncology chemotherapy [ 6 ] and reducing the incidence of drug extravasation and the rate of unplanned extubation, etc. [ 7 ] However, some studies have confirmed that the incidence of its related complications is 16.35%. [ 8 ] During PICC intubation, due to improper operation and other reasons, patients may experience catheter separation and secondary dislodgement, resulting in unplanned extubation. The incidence of PICC-related complications not only increases the physical burden of patients, but also increases the cost of treatment. It has been found in studies that the incidence of these problems is related to various factors such as the placement position of catheter, sealing method and catheter material, etc. [ 8–10 ] Therefore, it is extremely important to improve puncture methods for effectively reducing complications and upgrading puncture effects.

Abstract:Objective: We herein demonstrate the efficacy of PICC placement through a superficial femoral vein in patients with superior vena cava syndrome using ultrasound guidance and electrocardiographic localization. The treatment of PICC disconnection was also discussed. Methods: The study enrolled 51 patients with superior vena cava syndrome. Ultrasound-guided technology and ECG positioning technology are employed to help these patients in catheterization. The puncture time, the number of punctures, and catheter tip position were recorded. The patient was followed up for at least 2 years. The complications and treatment during follow-up were recorded. Result: The average puncture time was 32.13 ± 3.91 min. A total of 49 patients were successfully punctured once, while 2 patients failed in the first puncture. The main reason for puncture failure is that the inability of a guide wire to pass through. After the nurse removed the needle and pressed the puncture point until no rebleeding occurred, the puncture above the original puncture point was successful. X-ray examination revealed that the catheter tip was located in the inferior vena cava, above the diaphragm, near the right atrium. The success rate of catheterization was 100%. The visual analog scale (VAS) score was (2.44 ± 0.73) at the time of puncture, which was tolerable during the operation, and the patient did not complain of obvious pain following the operation. One patient developed complications of broken tube half a year after the puncture. Interventional physicians utilized angiography to locate the broken catheter. Conclusion: It is safe and feasible to place PICC through a superficial femoral vein under ultrasound combined with ECG positioning technology in patients with superior vena cava syndrome.

Abstract:AIMS AND OBJECTIVES:To investigate the incidence and risk factors for peripherally inserted central venous catheters-related upper extremity venous thrombosis in patients with cancer. BACKGROUND:With the widespread use of peripherally inserted central venous catheters, peripherally inserted central venous catheters-related upper extremity venous thrombosis in patients with cancer leads to increasing morbidity and mortality. It is very important to further explore the incidence and risk factors for peripherally inserted central venous catheters-related venous thrombosis. DESIGN AND METHODS:Consecutive patients with cancer who were scheduled to receive peripherally inserted central venous catheters, between September 2009 and May 2012, were prospectively studied in our centre. They were investigated for venous thrombosis by Doppler sonography three times a day within 30 days after catheter insertion. Univariable and multivariable logistic regressions' analyses were performed to identify the risk factors for peripherally inserted central venous catheters-related thrombosis. RESULTS:A total of 89 patients with cancer were studied in our research. Of these, 81 patients were followed up within one month. The mean interval between catheter insertion and the onset of thrombosis was 12.45 ± 6.17 days. The multivariable analyses showed that chemotherapy history, less activities and diabetes were the key risk factors for thrombosis. CONCLUSIONS:Peripherally inserted central venous catheters-related upper extremity venous thrombosis had high incidence rate, and most cases had no significant symptoms. The history of chemotherapy, less activities and diabetes were found to be the key risk factors. It should be routinely scanned in high-risk patients every 3-5 days after catheter insertion, which would then find blood clots in time and reduce the incidence of pulmonary embolism. RELEVANCE TO CLINICAL PRACTICE:Risk factors associated with peripherally inserted central venous catheters-related upper extremity venous thrombosis are of critical importance in improving the quality of patients' life. It is very important to grasp the indications to reduce the incidence rate of peripherally inserted central venous catheters-related upper extremity venous thrombosis.

Abstract:Peripherally inserted central catheter (PICC) is a widely used technique in oncology chemotherapy, characterized by safety, reliability, and long dwell time. However, a catheter can break due to various causes. When an acute rupture occurs, it always lead to sever complications which may threaten patients' safety and potentially result in medical disputes. In this study, we collected and analyzed 3 cases of PICC line breakage causing drug leakage in our hospital from 2018 to 2023. All these 3 cases were postoperative breast cancer patients accepting chemotherapy, with 2 cases involving external partial breakage and 1 case involving internal partial breakage. Due to timely and appropriate management, no acute rupture occurred. We propose some ideas such as selecting high-quality catheter materials and avoiding over extension or repeated bending are crucial in preventing PICC line breakage. In addition, we also recommend establishing a standardized and scientific management pattern of PICC to ensure the safety and effectiveness of its clinical application, including comprehensive assessment, "four-element" intervention, and continuous evaluation.

Abstract:Abstract Aims Peripherally inserted central catheters(PICC) lines are becoming increasingly popular in solid cancer patients for the administration of chemotherapy. This study aims looking at the incidence of PICC line related and distant thromboembolism associated with these catheters and exploring risk factors. Methods Records were reviewed for 158 patients who underwent PICC line insertion over the two years period in the medical oncology unit, Milton Keynes University Hospital. The Incidence PICC line related Deep Venous Thrombosis (DVT) which is defined as upper extremity DVT at the site of PICC line insertion was documented after checking reports of ultrasound Doppler of all symptomatic patients to confirm the presence of thrombo-embolism and Computed Tomography(CT)scan or Computed Tomography Pulmonary Angiography (CTPA) to confirm the presence Pulmonary Embolism(PE). Results 23(13%) symptomatic patients with confirmed diagnosis by ultrasound Doppler were found to have PICC line related DVT and similar number of patients developed distant VTE, namely PE and lower limbs DVT. Average time to thrombo-embolism from the insertion of PICC line was 13 days and 51 days in distant VTE. Statistically significant results have been identified in the term of risk factors leading to VTE events during the period of PICC line insertion. Conclusions VTE is a common complication in medical oncology patients who underwent insertion PICC line insertion for chemotherapy. Risk of distant VTE is high as well as the PICC line related DVT and the risk of the PICC line related DVT is higher in the first two weeks after PICC insertion. We concluded that high BMI,high PLTs count and Fluropyrimidine containing chemotherapy are all significant risk factors for VTE events recorded while smoking and high BMI are significantly contributing to the high rate of the PICC line related DVT.

Abstract:Background: PICC-ports may be defined as totally implantable central venous devices inserted in the upper limb using the current state-of-the-art techniques of PICC insertion (ultrasound-guided venipuncture of deep veins of the arm, micro-puncture kits, proper location of the tip preferably by intracavitary ECG), with placement of the reservoir at the middle third of the arm. A previous report on breast cancer patients demonstrated the safety and efficacy of these devices, with a very low failure rate. Methods: This retrospective multicenter cohort study—developed by GAVeCeLT (the Italian Group of Long-Term Venous Access Devices)—investigated the outcomes of PICC-ports in a large cohort of unselected patients. The study included 4480 adult patients who underwent PICC-port insertion in five Italian centers, during a period of 60 months. The primary outcome was device failure, defined as any serious adverse event (SAE) requiring removal. The secondary outcome was the incidence of temporary adverse events (TAE) not requiring removal. Results: The median follow-up was 15.5 months. Device failure occurred in 52 cases (1.2%), the main causes being local infection ( n = 7; 0.16%) and CRBSI ( n = 19; 0.42%). Symptomatic catheter-related thrombosis occurred in 93 cases (2.1%), but removal was required only in one case (0.02%). Early/immediate and late TAE occurred in 904 cases (20.2%) and in 176 cases (3.9%), respectively. Conclusions: PICC-ports are safe venous access devices that should be considered as an alternative option to traditional arm-ports and chest-ports when planning chemotherapy or other long-term intermittent intravenous treatments.

Abstract:An integrated microchip platform with automated analysis capability for DNA melting curves is developed for Single Nucleotide Polymorphism (SNP) genotyping applications. The microchip contains fluidic channels where genomic DNA samples are encapsulated into a series of droplets and transported through a detection region with a stable temperature gradient. As the temperature is elevated from 60 C to 85 C, the DNA strains denature and the associated fluorescence signals decay with this relationship being acquired as the melting curve. The droplets serve as discrete reactors to conduct DNA melting curve analysis in the liquid phase, eliminating the need for immobilization of reagents. They provide the advantage of signal homogeneity, which reduces the signal fluctuations, and thus the signalto-noise ratio is improved. In addition, a droplet detection and tracking software system which can identify the droplets, records the fluorescence intensities, plots the melting curves, and finds the melting temperatures, is developed making automated SNP genotyping possible. The platform has been verified with genomic DNA from Landrace sows and shown successful SNP discrimination from homozygotes and heterozygotes which demonstrates its potential to conduct on-site SNP genotyping for disease research, medical diagnostics, agriculture, and farm animal reproduction.

Abstract:Abstract Purpose So far there is little evidence on peripherally inserted central venous catheter (PICC) in radiation oncology patients maintaining the access during the periods of ambulatory and hospital treatment. Methods A total of 522 PICC placements in 484 patients were performed between 11/2011 and 07/2016 at the Department of Radiation Oncology and analysed retrospectively for complications and treatment- and patient-related factors during ambulatory and hospital inpatient use. On initial hospitalization, all patients received a multimodal radio-oncological treatment consisting of radiation and intravenous therapy administered via the PICC. Results A total of 18,292 catheter days were documented. Median follow-up from catheter insertion to their removal was 37 days (1–97). The overall complication rate was 4.1 per 1000 catheter days ( n = 75, 14.4%). Complications were similar between the cohort of outpatient 3.6 per 1000 catheter days and the cohort of inpatient 4.8 per 1000 catheter days (OR 0.976; 95% CI [0.598; 1.619]; p = 0.924). Severe bloodstream infections occurred at a rate of 0.60 per 1000 catheter days ( n = 11, 2.1%), deep vein thrombosis at a rate of 0.82 per 1.000 catheter days ( n = 15, 2.9%) and local inflammation at a rate of 1.26 per 1.000 catheter days ( n = 23, 4.4%). Only immunotherapy could be identified as an independent risk factor for complications (OR 5.6; 95% CI [2.4; 13.1]; p < 0.001). Conclusion Using PICC in outpatients is not associated with an elevated risk of complications. Particular attention should be payed to early identification of PICC associated bloodstream infections. Immunotherapy is an independent risk factor for local skin complication.

Abstract:Objective To compare the efficacies of upper arm placement of peripherally inserted central venous catheters using ultrasound guidance and modified Seldinger technique with traditional method.Methods A multicenter,randomized,and blind controlled trial was conducted in 150 patients from five cancer hospitals.The patients were randomly assigned to control group and experiment group.The success rate of PICC placement,the complications of insertion sites,mechanical phlebitis,and the comfort of insertion arms were recorded and compared immediately and on the second and seventh days with PICC-insertion,respectively.Results The successful rate of PICC placement was 100% in both groups.No significant differences were found on the incidence of hemorrhage of insertion sites and the position of PICC tips during the process of PICC-insertion between the two groups(P>0.05).There were no significant differences on the incidence of hemorrhage of insertion sites and finger swelling on the second day with PICC(P>0.05).But the incidence of mechanical phlebitis in the experiment group was significantly lower than that of control group(P<0.05).Significant differences were found on the incidence of hemorrhage of insertion sites,finger swelling,mechanical phlebitis and the degree of comfort between two groups on the seventh day with PICC(P<0.05 or 0.01).Conclusion The method of PICC insertion utilizing ultrasound guidance and modified Seldinger technique can reduce the incidence of hemorrhage of insertion sites,finger swelling and mechanical phlebitis while improve the comfort of the inserted arms in patients.

Abstract:Abstract Peripherally inserted central catheters (PICCs) are widely used medical devices to provide medium to long–term infusion treatment and therapy. PICC placement is contraindicated in cases of deep–vein thrombosis, end–stage renal disease or if deep venous access is compromised. Among the possible complications there are some related to the interventional procedure (e.g. accidental puncture of an arterial vessel, injury to peripheral nerves, misplacement, arrhythmias, haematoma) and others related to the post–implantation phase (e.g. thrombosis, infection, gas emboli, phlebitis). One of the possible intra–procedural complications is the occasional finding of a persistent left superior vena cava (PLSVC). PLSVC is an anatomical variant present in at least 0.5% of the general population and its occurrence during interventional procedures is not uncommon. Clinical case description An 82–year–old woman was admitted to our Institute following a diagnosis of herpetic encephalitis (HSV1). She had a history of hypothyroidism undergoing substitution treatment. Cause the poor peripheral venous patrimony and the need of infusion therapy with acyclovir, levetiracetam and ceftriaxone, the indication for PICC implantation was given. After antibiotic prophylaxis and sterile field preparation, the procedure was performed with ultrasound–guided Seldinger technique by puncturing the left basilic vein. A successive X–ray showed the PICC running unusually parallel to the right sternum (Fig. 1). The echoscopy with bubble test allowed to visualize the agitated saline solution in the right atrium. In view of the suspected venous abnormality, the PICC was removed and a single–lumen MiniMidline 4F of 11 cm length was placed in the right basilic vein (4 mm). A subsequent chest CT scan visualised the PLSVC and its intrathoracic course, the presence of the right superior vena cava was also observed (Fig. 2). The PLSVC is an anatomical variant encountered during interventional procedures involving peripherally inserted central venous catheters. It is important to recognise this variant to avoid possible procedural and/or post–procedural related surgical complications.

Abstract:Objective:The aim of this study was to investigate the clinical value and the advantages of the improved peripherally inserted central catheter (PICC) guided by color Doppler ultrasound.Methods:Forty-two patients with malignant tumor history were included in this study.They all needed PICC and were not suitable for blind puncture because the superficial veins were not palpated.The improved PICC guided by color Doppler ultrasound were used.The improvements included the use of puncture frame on the probe and the substitution of one-off anesthesia needles for puncture needles in Seldinger package.The puncture success rate and the complication rate were analyzed and compared with 23 patients using standard PICC.Results:Both the improved PICC and the standard PICC had a 100 ％ puncture success rate.One-time success rate of the improved PICC and standard PICC were 97.6 ％ (41/42) and 91.3 ％ (21/23).And the difference was not statistically significant (P＞0.05).Two patients with improved PICC had complications,including one case of local edema and one case of catheter heterotopia; six patients with standard PICC had complications,including two cases of local edema,one case of catheter heterotopia,one case of phlebitis and two cases of local infection.And the difference of the complication rate was statistically significant (P=0.019).Conclusions:The improved PICC guided by color Doppler ultrasound had higher success rate and lower complication rate compared with standard PICC and it was worthy of clinical promotion.
