Abstract:Objective To approach washing quality for medical instrument,and study preferred cleaning method of medical instrument.Methods To adopt test paper of occult blood for washing quality of medical instrument in spot.Results Medical instrument with soaking in enzymatic detergent plus washing of ultrasonic washing unit,eligible rate of occult blood test is 91.60%;medical instrument with soaking in hydrogen peroxide plus washing of automatic rinser,eligible rate of occult blood test is 89.47%;medical instrument with soaking in enzymatic detergent plus manual washing,eligible rate of occult blood test is 98.44%.It's showed medical instrument with dry bloodstain to amply soaking in enzymatic detergent plus washing of ultrasonic washing unit,washing quality eligible rate is 89.47%;to amply soaking in 30 g/L hydrogen peroxide plus washing of automatic rinser,washing quality eligible rate is 100%.Conclusions The washing quality is better when cleaning of contaminated medical instruments by soaking in enzymatic detergent plus ultrasonic washing unit,and good at cleaning of medical instruments with dry bloodstain by soaking in hydrogen peroxide plus automatic rinser.
Understanding The Factors Involved In Determining The Bioburdens Of Surgical Masks

Abstract:Background: Surgical site infection (SSI) continues to be one of the most common postoperative complications. In our previous study, surgical mask (SM) bioburden was identified to be a potential source of SSI. In the present study, we investigated the factors involved in SM bioburden.Methods: Bioburdens of the disposable SM (A: medical mask; B: medical surgical mask) and newly laundered cloth SM (C) were tested by immediately making an impression of the external surface of the mask on sterile culture media. SM microstructure was observed using a scanning electron microscope (SEM). Filtering efficiency and airflow resistance were evaluated with TSI Automated Filter Tester 8130 (TSI Incorporated) according to GB/19083-2010. Whether speaking during operation and washing the face pre-operatively affect SM bioburdens was also evaluated. Surgical procedures were performed in a dynamic operation room. Fifty cases of mask use were enrolled in this study.Results: The bioburden of mask A was the highest. The bioburden of mask B was the lowest. Mask C possessed the lowest filtering efficiency and the highest airflow resistance. SM bioburden was higher in the speaking group. SM bioburden showed no significant difference after washing the face, despite the finding that washing could significantly reduce facial bioburden.Conclusions: Multiple factors influence SM bioburdens. Mask B showed the lowest bioburden and best protection effects. Mask C is not recommended to be used, especially considering that surgeons do not wash the cloth masks daily. Unnecessary talking during operation is not recommended, and washing the face before surgery is not strictly necessary.

Abstract:BACKGROUND: This study aimed to examine the body contamination rates and environmental contamination levels during the removal of 3 types of personal protective clothing (PPC) by the individual accustomed removal method (IARM) and gown removal methods recommended by the Centers for Disease Control and Prevention (CDC).METHODS: Fifty participants performed IARM and CDC-recommended gown removal methods to remove 3 types of PPC (ie, cotton gown, water resistant gown, and plastic apron) in random order at 2 separate sessions after applying Glo Germ simulated germ lotion on the gown's surface. A video demonstrating the CDC-recommended gown removal method was shown between the 2 sessions. After PPC removal, fluorescent stains were counted by an ultraviolet scan under dim light.RESULTS: Following IARM, contaminants were splashed in the surroundings, particularly on the front part of the subject. The plastic apron and cotton gown obtained the highest and lowest contaminative hazards, respectively, to the hands, shoes, and environment. Females, nurses, and senior staff had serious hand or shoe contamination. The CDC removal method more significantly reduced body and environmental contamination of small fluorescent stains (<1 cm(2)), but not of large patches (>1 cm(2)), than IARM.CONCLUSION: The effect of gown removal, PPC type, discarding PPC location, training of infection control measures, hand hygiene, and special work shoes should be considered daily.
Source strength as a measurement to define the ability of clean air suits to reduce airborne contamination in operating rooms

Abstract:Background: Surgical site infections after total hip and knee replacement are linked to the quality of the operating room (OR) air. Applying tight occlusive clothing, effective ventilation and correct working methods are key concepts to obtain low bacterial concentrations in the OR air. The dry penetration test referred to in European standard EN 13795-2:2019 is a screening method for materials used in surgical clothing. Source strength, defined as the dispersal of bacteria-carrying particles from persons during activity, is a functional test of clothing systems and has been calculated in a dispersal chamber and in ORs. Results from both tests can be used when comparing surgical clothing systems. Aim: This study relates results of dry penetration tests to source strength values for five surgical clothing systems available on the Swedish market. Methods: Experimental data are reported on the function of these products, expressed as source strength calculated from results in a dispersal chamber and in ORs during orthopaedic operations. Findings: All materials tested with dry penetration ≤50 colony-forming units (cfu) had source strength values <3 cfu/s for one person in the dispersal chamber, whereas the material of one product when laundered >50 times had source strength in the dispersal chamber of up to 8 cfu/s. Conclusion: The dry penetration test could predict the performance of clean air suits of the same design, but more studies are needed to obtain a more valid correlation. Requirements of source strength should be included in standards.
Full-scale experimental and numerical study of bioaerosol characteristics against cross-infection in a two-bed hospital ward

Abstract:The transmission and deposition of pathogenic bioaerosols and the subsequent contamination of the air and surfaces is well recognized as a potential route of hospital cross-infection. A full-scale experiment using Bacillus subtilis and computational fluid dynamics were utilized to model the bioaerosol characteristics in a two-bed hospital ward with a constant air change rate (12 ACH). The results indicated that the bioaerosol removal efficiency of unilateral downward ventilation was 50% higher than that of bilateral downward ventilation. Additionally, health care workers (HCWs) and nearby patients had lower breathing zone concentrations in the ward with unilateral downward ventilation. Furthermore, a partition played a positive role in protecting patients by reducing the amount of bioaerosol exposure. However, no obvious protective effect was observed with respect to the HCWs. Only 10% of the bioaerosol was deposited on the surfaces in the ward with unilateral downward ventilation, while up to 35% of the bioaerosol was deposited on the surfaces in the ward with bilateral downward ventilation during the 900 s. The main deposition locations of the bioaerosols were near the wall on the same side of the room as the patient's head in all cases. This study could provide scientific evidence for controlling cross-infection in hospital wards, as well as several guidelines for the disinfection of hospital wards.

Abstract:With the wide application of electronic surgical equipment, the generation of surgical smoke is inevitable. Surgical smoke will not only obstruct the operator's visibility, but also release toxic and harmful substances into the air, which will make the operator a headache and cause inflammation of the eyes and mucous membranes. In order to evaluate whether the existing operating room ventilation system can give priority to control the diffusion of surgical smoke, it is necessary to understand the generation, release, transmission and elimination performance of surgical smoke. This paper studied the rule of the influence of surgical smoke in different areas of the operating room through experiments, and verified by the three-area heterogeneous model. At a static state, the cleanliness level of the operating area and the surrounding area is ISO4 and ISO6. When there is staff activity, the operating room is in a dynamic state, and the operating area and the surrounding area are ISO7. When there is surgical smoke, the cleanliness level of the operating area is reduced to ISO8, and the surrounding area has little influence. This indicates that personnel activities and surgical smoke in the operating room have a great impact on the operating room environment.

Abstract:Epidemic prevention policies in hospitals address issues such as, indoor air quality control, cleanliness of medical staff clothing and employee hand-washing procedures. Our hospital employed Bio-Kil to treat air-conditioning filters and nursing staff uniforms. We also assessed the efficacy of different detergents. Using Bio-Kil technology, the mean bacterial count in the air was reduced from 108.8 CFU/h/plate (n=420) to 68.6 CFU/h/plate (n=630). On the lower hems of the Bio-Kil-treated gowns, the mean bacterial count was 1,201 CFU/100 cm(2), markedly lower than the bacterial count of 7,753 CFU/100 cm(2), found on the parts of the gowns not treated with Bio-Kil (p=0.0401). On the cuffs of sleeves treated with Bio-Kil, the mean count was 1,165 CFU/100 cm(2), markedly lower than that of 2,131 CFU/100 cm(2), found on the cuffs not treated with Bio-Kil (p=0.0073). With regard to the mean bacterial eradication rates of antimicrobial solutions, Steridal Solution, 75% alcohol and Bio-Kil (3rd generation) were shown to be the most effective, with rates exceeding 80%. Hibiscrub with paper towels and Fresh Protect Skin were the second most effective. Rio-Kul (1st generation), tap water with paper towels, liquid hand soap with paper towels and ozone water were the least effective. One important observation was that hand-washing without the use of paper towels increased the bacterial count by as much as 84%. Bio-Kil is effective in reducing bacterial counts in the air, on nursing staff uniforms and is an effective detergent.

Abstract:Background The role of environment in infection prevention and control is being increasingly acknowledged. However, gaps remain between what is promoted as best practice in the literature and what is occurring in healthcare settings. In part, this is due to a lack of generally accepted scientific standards, further confounding the ability to demonstrate an undisputed role for the healthcare environment in healthcare-acquired infections (HAIs). Evaluating environmental cleanliness in a standardised format is required, in order to enable a framework for performance management and provide a method by which interventions can be evaluated. Standardised assessment would provide reliable data to support quality-improvement activities and to ensure that healthcare staff have relevant and useful information to inform and adapt practice. Methods This integrative literature reviewdescribes approaches to assessing environmental cleanliness. A search of the published literature was undertaken, in combination with a targeted review of the grey literature. Results Four methods for assessing environmental cleanliness were identified: visual inspection, fluorescent gel marker, adenosine triphosphate (ATP) and microbial cultures. Advantages and disadvantages for each are explored. Conclusion Methods that evaluate cleaning performance are useful in assessing adherence to cleaning protocols, whereas methods that sample bio-burden provide a more relevant indication of infection risk. Fast, reproducible, costeffective and reliable methods are needed for routine environmental cleaning evaluation in order to predict timely clinical risk.

Abstract:Disposable materials enjoy annually steady growth in volume in the field of medical applications as surgical gowns, isolation gowns, and medical drapes which impede cross contamination from reusable protective apparel in a hospital. There are four levels of liquid barrier performance according to the American National Standard/Association for the Advancement of Medical Instrumentation (ANSI/AAMI) PB70:2012 in terms of liquid impact penetration, hydrostatic pressure or hydrohead, and blood penetration. Different areas in a surgical gown require different levels of liquid barrier performance.Laminates of spunbond (SB), meltblown (MB), and spunbond (SB), called SMS fabric, and laminates of a porous membrane with a nonwoven fabric, usually with the SB layer for strength and ease of handling, are widely used as medical protective clothing. SMS materials are usually made to achieve up to Level 3, which requires a hydrostatic pressure of greater than 50 cm tested according to ANSI/AAMI PB70:2012. Above that level, Level 4 requires the liquid barrier to resist penetration by blood, in which case a laminate with a porous membrane is necessary.In this paper, the pore size, calculated from the fluid flow through a fibrous media, was used to estimate the hydrostatic pressure. The hydrostatic pressure was then used to correlate with the resistance of the media to blood penetration. The agreement of experimental and calculated data indicated that these theories were successful not only in predicting the pore size and the hydrostatic pressure of a fibrous material but also in correlating the hydrostatic pressure and blood penetration. The fiber fineness required to resist the penetration of blood was also calculated.
Analysis of Air Purification Methods in Operating Rooms of Chinese Hospitals.

Abstract:This research demonstrates the current use of air purification methods in the operating rooms (ORs) in China. 154 hospitals from 6 provinces were included in this survey to reflect the air purification methods of ORs in 2017. Air cleaning technology (ACT) is used in 124 (80.52%) hospitals. We find that the rates of using grade I, III, or IV clean operating room (COR) in tertiary hospitals are all higher than in lower level hospitals; the rate of using ACT in the ORs is higher, too. In addition, general hospitals have higher rate in using ACT in the ORs than specialized hospitals. The highest rate of using ACT in the ORs is in the eastern region of China. The number of hospitals using ACT, ultraviolet light disinfection, and air sterilizers (such as circulating air UV sterilizer) increased yearly. All grades of CORs can be maintained as required by more than 90% hospitals except grade II COR. In this research, we found air purification methods, especially the ACT, are widely used in hospitals' ORs. However, finding the way to select and use different air purification methods correctly is an urgent problem to be solved next.
Status of bacterial contamination of bedside curtains in some hospitals of Shanghai

Abstract:OBJECTIVE To understand the status of bacterial contamination of bedside curtains of ICUs and common wards in some general hospitals of Shanghai so as to reduce the rate of bacterial contamination of bedside curtains .METHODS A total of 120 bedside curtains specimens were collected from 8 hospitals by using standard cotton swab wiping method and were spread on the blood and MRSA agar plates ,then the samples were uniformly submitted to the clinical microbiological laboratory of Zhongshan Hospital ,the growth of pathogens on the plates was observed after 48 hours of culture in 35℃condition ,the bacterial colony counts on the plates were recorded , and the suspected pathogens or opportunistic pathogens were identified by using VITEK MS .RESULTS Of the 120 bedside curtains samples ,48 were collected from the ICUs ,and 72 were collected from the common wards .The qualified rate (<10CFU/100 cm2 )of the collected samples was 45 .8% in the ICUs ,6 .9% in the common wards (P<0 .01);the rate of severe contamination (≥50 CFU/100 cm2 ) was 12 .5% in the ICUs ,44 .4% in the com‐mon wards (P<0 .01) .The rate of bacterial contamination (≥10 CFU/100 cm2 )of the bedside curtains of the ICUs that were cleaned within one week was 25 .0% ,significantly lower than 68 .8% of the bedside curtains of the ICUs that were not cleaned for more than one week ,and there was statistically significant difference (P<0 .05) . Totally 342 strains of pathogens have been isolated from the bedside curtains samples ,of which 40 .3% (138 strains) were the coagulase‐negative Staphylococcus (CNS) ,followed by the Micrococcus luteus/lira micrococcus ( 71 strains ,20 .8% ) and bacillus ( 48 strains ,14 .0% ) .CONCLUSIONS The rate of bacterial contamination of the bedside curtains is significantly lower in the ICUs than in the common wards .The CNS and gram‐positive bacilli are most common among the bacteria causing the contamination , and regular cleaning can significantly reduce the contamination rate .

Abstract:Due to the serious global harm caused by the outbreak of various viral infectious diseases, how to improve indoor air quality and contain the spread of infectious bioaerosols has become a popular research subject. Negative pressure isolation ward is a key place to prevent the spread of aerosol particles. However, there is still limited knowledge available regarding airflow patterns and bioaerosol diffusion behavior in the ward, which is not conducive to reducing the risk of cross-infection between health care workers (HCWs) and patients. In addition, ventilation layout and patient posture have important effects on aerosol distribution. In this study, the spatial and temporal characteristics as well as dispersion patterns of bioaerosols under different ventilation patterns in the ward were investigated using the computational fluid dynamics (CFD) technique. It is concluded that changes in the location of droplet release source due to different body positions of the patient have a significant effect on the bioaerosol distribution. After optimizing the layout arrangements of exhaust air, the aerosol concentration in the ward with the patient in both supine and sitting positions is significantly reduced with particle removal efficiencies exceeding 95%, that is, the ventilation performance is improved. Meanwhile, the proportion of aerosol deposition on all surfaces of the ward is decreased, especially the deposition on both the patient's body and the bed is less than 1%, implying that the risk of HCWs being infected through direct contact is reduced.

Abstract:Objective: To analyze the effect of sanitary wipes and medical alcohol + gauze disinfection methods on the management and use of medical equipment in hospitals. Methods: From February 2023 to August 2023, 80 anorectal surgery medical instruments and equipment were randomly sampled as observation samples, and they were grouped using the random number table method (40 instruments in each group). One group was managed with sanitary wipes and medical alcohol + gauze disinfection method (observation group), and the other group was managed with medical alcohol + gauze disinfection method (control group). The degree of contamination on the surface of instruments and equipment and the staff satisfaction of both groups was noted and compared. Results: After cleaning and disinfection, the relative luminescence luminosity (RLU) value of the surface contamination of the sample instruments and equipment decreased significantly (P < 0.05); and after cleaning and disinfection, the relative luminescence luminosity (RLU) of the observation group’s instruments and equipment decreased significantly, and the value was lower than that of the control group, (P < 0.05); the results of the nursing job satisfaction questionnaire showed that the scores of each survey item in the observation group were higher than those in the control group, (P < 0.05). Conclusion: Integrating sanitary wipes and medical alcohol + gauze disinfection method into the management and use of medical equipment not only improves the disinfection of medical instruments but also improves the job satisfaction of nursing staff. Therefore, this method should be popularized.
Effect of worker's respirator and hat wearing method on ICU air quality

Abstract:Objective To investigate the effect of employers' respirator and hat wearing method on class 100 000 clean ICU air quality,in order to provide basis for hospital infection management in ICU.Methods For the control group,the first day (d1) and third day (d3),every employer was demanded to wear a respirator and a hat before entering the ICU.For the observation group,the second day (d2) and forth day (d4),employers were demanded not to wear any respirator or hat.During all four days,they must wear respirator and hat before any nursing operation.Air quality was sampled by class 100 000 clean air using flat panel natural settlement method,dust planktonic bacteria method and particle counting method.The amount of dust,plankton bacterium and descending bacteria were monitored for 4 days and compared between two groups.Results The difference of 0.5 μm dust particles in ICU between the control group and the observation group were statistically significant ( F =40.95,P ＜ 0.05 ).As to 5.0 μm dust particles,there was no significant difference between two groups (F =2.86,P ＞ 0.05 ).0.5 μm dust particles in ICU was lower at d1 and d3,and the difference was statistically significant ( F =40.95,P ＜ 0.05).The number of 0.5 μm dust particles had significant difference between different periods of a day:the morning time segmcnt＞ evening ＞ afternoon ( F =80.72,P ＜0.05 ).The number of 0.5 μm dust particles in the observation group was higher than that in the control group,and the difference was statistically significant ( F =68.84,P ＜ 0.05 ).The number of 5.0 μm dust particles had significant difference between different time periods of a day:the morning time segment ＞afternoon ＞ evening ( F =98.17,P ＜ 0.01 ).The number of dust particles at hall was larger than that at single and double rooms (P ＜0.01 ),but the number had no difference between single room and double room.More subsidence bacteria and floatingbacteria was detected during dl and d3 in the control group,and the difference was statistically significant ( P ＜ 0.01 ).Besides,the difference of the number of subsidence bacteria and floating bacteria in ICU between different time periods was also statistically significant ( P ＜ 0.05 ).Conclusions The number of 0.5 μm dust particles in ICU is smaller when workers wear hats and respirators,while the number of 5.0 μm dust particles remains the same.Under the other conditions remain unchanged,the mainly cause of number increase of dusts,planktonic bacteria and sedimentation bacteria in the morning rounds is the increasing number of working staff.ICU staff has no apparent effect on dynamic class 100 000 clean ICU air quality whether they wear respirators or hats.This kind of behavior is not the key management elements in clean ICU environment.
Survey on air purification methods applied in different types of hospitals

Abstract:Objective To understand the application of air purification methods in different hospitals.Methods A survey was conducted in hospitals of different levels and types in 7 provinces from 7 regions of China, to investigate the current application status of air purification methods in hospitals after the promulgation of Hospital Air Purification Management Code, including the application of different air purification methods, the combination of air purification methods, and the application of air purification methods in different regions.Results A total of 340 hospitals were surveyed. 38.53%（n=131） of which had negative pressure isolation wards, 81.47%（n=277） had clean operating rooms with cleaning technology, 97.35%（n=331） had air disinfectors. 70.00%（n=238） had two or more air purification methods simultaneously in a certain region. General intensive care unit, infectious disease department and fever clinic adopted three or more air purification methods, accounting for 55.93%, 43.01% and 38.49%, respectively. The most common combination was natural/mechanical ventilation+air disinfectors（83.19%, 198/238）. 20.59%（49/238） used ultraviolet lamps additionally for the disinfection in rooms with clean technology. Except for non-clean operating rooms, delivery rooms, catheter rooms and disinfection supply center, over 80% areas had ventilation systems.Conclusion The selection of air purification methods is diverse, and the architectural design pays more attention to ventilation, but the selection of air purification methods is still excessive and non-standardized.
The effect of surgical gowns made with barrier cloth on bacterial dispersal

Abstract:A dispersal chamber (body box) technique has been used to compare bacterial dispersal from the skin of subjects carrying out a stepping test under controlled conditions while wearing four differing garment systems namely: basic underwear, cotton 'blues' (standard pyjama style jacket and trousers for men or dress for women), ankle socks, boots for men and shoes for women, mask and theatre hat; the basic set covered with a cotton gown; the basic set covered by a gown with a front made from GORE-TEX fabric in which an expanded polytetrafluoroethylene membrane is sandwiched between layers of woven or knitted polyester; the basic set covered with a fully enclosed suit of the same fabric. A slit sampler was used to measure the number of bacteria liberated in a downward current of air. Six subjects (three female and three male) were studied. Males liberated more bacteria. Covering the 'blues' with a cotton gown increased the bacterial count; a gown of the new material reduced the increase by 50%, and the suit cut the dispersal to virtually zero. Preliminary work suggests that GORE-TEX garments survive laundering better than cotton, and may be cost-effective, but are not yet as comfortable. Research is presently in progress to improve this aspect.

Abstract:Two types of mobile screens producing ultraclean local laminar airflow were investigated as an addition to turbulent mixing operating room ventilation. The exploration was carried out numerically using computational fluid dynamics. Surface and volumetric particle counts were simulated on the operating and instrument tables with and without the additional mobile airflow screen. Three different source strengths (the mean bacteria-carrying particle value emitted from one person per second) due to staff clothing variety were considered. Model validation was performed through result comparisons with experimental data from the literature. Results confirmed that the mobile screen units reduced the airborne bacteria to an acceptable level for infection-prone surgeries. No significant particle concentration differences existed in the periphery of the operating room. Lower source strength resulting from a clothing system with high protective capacity reduced particle concentration.

Abstract:A clean air environment in the operating area is an important factor in reducing intraoperative infection, which is affected by medical staff and their dynamic behaviour. In this study, experimental and numerical simulation methods were used to investigate the cleaning performance of the operation area under actual working conditions, which can more intuitively simulate the movement of particles in the operating room. First, four air distribution cases are studied: empty state, static persons, arm waving, and medical staff walking. Second, the disturbance process, distribution, peak value, and uniformity of particulate matter concentration in these cases are discussed. The results show that the air distribution in the operation area is greatly affected by the dynamic disturbance, the cleanliness level deteriorates by 2–3 levels, and the cleanliness uniformity also deteriorates further. Therefore, it is important to reasonably evaluate the mobile operation shelter cleaning performance and infection risk under actual working conditions.
