Abstract:Background: Powered instrumentation is often used for bone preparation and/or removal in many orthopaedic procedures but does risk thermogenesis. This study compares biomechanical properties of a fluted burr and a novel fluteless resurfacing tool. Methods: Twenty cadaveric metatarsals were tested with four predetermined cutting forces to evaluate heat generation and cutting rate for the fluted burr and fluteless resurfacing tool over 40 s or until a depth of 4 mm was reached. Cutting rate was calculated from displacement transducer data. Heat generation was measured by thermocouples placed in the bone adjacent to the burring site. Assuming a body temperature of 37 °C, a 10 °C increase in heat was used as the threshold of inducing osteonecrosis. Findings: At 1.0 N and 1.7 N, the thermal osteonecrosis threshold was reached at comparable times between burrs, while the bone removed by the resurfacing tool was on average five times greater than fluted burr at 1.0 N and over twice as great at 1.7 N. Statistical analysis of these common cutting forces showed the resurfacing tool had significantly higher cutting rates (P < 0.01). As a result, the fluted burr produced higher temperatures for the same amount of bone removal (P < 0.01). Interpretation: In a cadaveric study, the fluteless resurfacing tool demonstrated higher bone cutting rates and lower heat generation for the same amount of bone removed than a traditional fluted burr.

Abstract:The paper describes a new technique of precise drop dispensing of deoxyribonucleic acid functionalized single walled carbon nanotube (DFC) solution by microfluidic cantilevers for use as resistance-type humidity sensors. Electrodes of different gap lengths L = 10, 15, 20, and 25 μm were accurately bridged by DFC drops of desired diameter by regulating the relative humidity (RH) of the chamber, contact time tc of the microcantilever type surface patterning tool touching the substrate and the UV/ozone exposure time tUV of the substrate. Sensor performance, including sensitivity and dynamic response characteristics, is investigated in detail for L = 25 μm in the RH range of 20%-90%. The phenomenon of electric field assisted desorption of water molecules from the sensor surface is explained from the decrease in recovery time with the bias voltage. The device shows excellent dynamic repeatability and fairly good environmental stability over a period of one month.
Temperature variations during bone removal procedures similar to third molar extraction using different instruments

Abstract:Bone removal is commonly used in the extraction of third molars and the heat generated during the process can interfere with bone repair. The aim of this study was to evaluate the temperature variation presented in bone removal performed with a high speed turbine (400,000rpm), implant motor with straight piece (100,000rpm), low speed micromotor (20,000rpm) and piezoelectric saw (30kHz) in pig mandibles. For this, bone removal was carried out around 20 posterior teeth, under constant saline solution irrigation with syringe and needle. In addition, the time required to perform bone removal was recorded. The results indicated a mean temperature variation and standard deviation of 0,96°C (0,6°C) for the high speed turbine, 1,38°C (0,5°C) with the implant motor, 2,22°C (0,7°C) for the low speed micromotor and 2,90°C (1,3°C) for the piezoeletric. The conventional variance was calculated discounting the variation of time used for bone removal around the teeth. There was a statistically significant difference in temperature variation between the high speed turbine vs micromotor (p=0,00) and the high speed vs piezoeletric (p=0,04). We conclude that there is a statistically significant difference in temperature variation between the instruments used in oral and maxillofacial surgery, with higher rotation speeds resulting in lowest temperature variations and a reduced surgical time.

Abstract:Working on bone is a major aspect of orthopaedic surgery. Despite its well-known appreciable thermal effects on the edges of the bone cut, the oscillating bone saw blade the oscillating saw remains the standard instrument both for cutting long bones and creating a bed for an endoprosthesis. The application of abrasive water jets offers the possibility of achieving an extremely precise curved cut in bone with no accompanying thermal effect. The thermographically measured absolute temperature increase at the cut edges seen with the water jet was 13 K maximum. The small process forces permit the application in automated handling systems.

Abstract:Mechanical and thermal damage due to cutting force and temperature rise during bone drilling plays a significant role in implant failure during osteosynthesis. High cutting force results in hole inaccuracy and weakens the stability between bone and implant. In comparison, temperature above 47 °C during drilling of bone results in the permanent death of regenerative bony cells consequences of thermal osteonecrosis. Microcracks at the drill site result in bone delamination and affect the holding strength at the bone-screw interface, resulting in implant failure. Accumulation of bone chips produced during drilling may cause chip clogging. The cutting force, microcracks, bone chip accumulation, and temperature rise are significant reasons for implant instability and revision surgery. Therefore, the present work uses the rotary ultrasonic bone drilling (RUBD) technique of the pig femur bone to monitor cutting force, temperature, microcracks and bone debris generated during the drilling. The results were also compared with conventional bone drilling (CBD). The twisted surgical drill bit was used during CBD and the diamond-impregnated tool was used during RUBD with the same diameters of 3.5 mm. The histopathological analysis was conducted at the drilled site to observe the tissue damage and the effect of heat generation on bone morphology. The output parameters were correlated, and the influence of cutting force on temperature rise, microcracks, and chip morphology was observed. This study showed that the increase in cutting force from 35.63 to 57.66 N in RUBD, and 54.83 to 78.7 N in CBD, resulted in a decrease in temperature rise by 51.5 and 32.7%, respectively. In contrast, the intensity of microcracks and chips size increased with the increase in cutting force for both RUBD and CBD techniques. Moreover, the RUBD technique reduces cutting force, temperature rise, microcracks and bone chip morphology as compared to CBD.

Abstract:This study investigates neurosurgical bone grinding with varying parameters on skull bone using a miniature grinding burr. Three process parameters, namely, rotational speed, feed rate, and depth of cut, have been investigated at three different levels in the terms of tangential force, thrust force, and torque generated during grinding. The results revealed that as the rotational speed is increased, the cutting forces and torque showed a decreasing trend. Nevertheless, the increase in feed rate and depth of cut leads to the escalation in response characteristics. The best parametric combination for minimum cutting forces and torque is as follows: rotational speed = 55,000 r/min, feed rate = 20 mm/min, and depth of cut = 0.50 mm. Morphological analysis reveals cracks in the bone’s surface at a higher feed rate. Furthermore, delamination and cutting streaks are also visible on the surface of the bone after grinding. Energy-dispersive spectroscopy and elemental mapping of the tool after bone grinding indicate the accumulation of the bone chips in the successive diamond abrasives. The outcomes of the study will be beneficial for the neurosurgeons in understanding the effect of various process parameters on cutting force, toque, microcracks, and bone’s regeneration ability during surgical bone grinding.

Abstract:This study investigates the effect of process parameters on neurosurgical bone grinding performance using a miniature surgical diamond wheel. Bone grinding is an important procedure in the expanded endonasal approach for removing the cranial bone and access to the skull base tumor via nasal corridor. Heat and force are generated during the grinding process, which may cause thermal and mechanical damage to the adjacent tissues. This study investigates the effect of grinding process parameters (including the depth of cut, feed rate, and spindle speed) on the bone grinding performance using temperature and force measurement sensors in order to optimize the grinding process. An orthogonal experimental design with a standard orthogonal array, L-9 (3(3)), is selected with each parameter in three levels. The experimental results have been statistically analyzed using the range and variance analysis methods in order to determine the importance order of the process parameters. The results indicate that the effect of the cutting depth on the grinding temperature and normal force is the largest, while the effect of the spindle speed on the tangential force is the largest. A high spindle speed would make the temperature rise to a certain extent; however, it significantly reduces the grinding force. At a certain spindle speed, a lower depth of cut and feed rate help to reduce the grinding temperature and force.
Cutting Behavior of Cortical Bone in Different Bone Osteon Cutting Angles and Depths of Cut

Abstract:Cortical bone is semi-brittle and anisotropic, that brings a challenge to suppress vibration and avoid undesired fracture in precise cutting process in surgeries. In this paper, a novel analytical model is proposed to represent cortical bone cutting processes. The model is utilized to predict the chip formations, material removal behavior and cracks propagation under varying bone osteon cutting angles and depths. Series of orthogonal cutting experiments were conducted on cortical bone to investigate the impact of bone osteon cutting angle and depth of cut on cutting force, crack initialization and propagation. The observed chip morphology highly agreed with the prediction of chip formation based on the analytical model. The curly, serrated, grainy and powdery chips formed when the cutting angle was set as 0°, 60°, 90°, and 120°, respectively. Cortical bone were removed dominantly by shearing at a small depth of cut from 10 to 50 μm, and by a mixture of pealing, shearing, fracture and crushing at a large depth of cut over 100 μm at different bone osteon angles. Moreover, its fracture toughness was calculated based on measured cutting force. It is found that the fluctuation of cutting force is suppressed and the bone material becomes easy to remove, which attributes to lower fracture toughness at bone osteon cutting angle 0°. When the cutting direction develops a certain angle to bone osteon, the fracture toughness increases then the crack propagation is inhibited to some extent and the fluctuation of cutting force comparatively decreases. There is a theoretical and practical significance for tools design and operational parameters choice in surgeries.

Study DesignA biomechanical study.ObjectiveWe aimed to assess temperature fluctuations when applying the ultrasonic osteotome during laminectomy and identify the most appropriate cutting method for robot-assisted laminectomy to mitigate the risk of heat-related injuries.MethodsUtilizing a robotic system for laminectomy, the study implemented the ultrasonic bone scalpel to cut both artificial polyurethane bones and animal spinal bones. The research focused on comparing and analyzing the maximum temperature of the inner surface of four types of artificial bones through three different cutting techniques: vertical constant cutting at speeds of .5 mm/s and 1 mm/s, as well as robot-assisted vertical reciprocating cutting. After the initial results, two optimal vertical cutting approaches were selected for subsequent trials, evaluating the effectiveness and temperature impact of various ultrasonic osteotome modes on 10 isolated spinal bones from pigs.ResultsWhen cutting polyurethane bones, reciprocating cutting demonstrated the lowest maximum temperature in contrast to constant speed cutting at .5 mm/s and 1 mm/s. In the animal bone trial, direct cutting registered an average maximum temperature of 43.25°C with an average cutting duration of 688.3 s, while reciprocating cutting recorded an average maximum temperature of 34.20°C with an average cutting time of 713.0 s.ConclusionsThe reciprocating cutting strategy utilized in robot-assisted ultrasonic osteotome is effective in reducing heat generation and maintaining high cutting efficiency.

Preliminary investigations of rotary ultrasonic neurosurgical bone grinding using Grey-Taguchi optimization methodology

Abstract:Purpose In present research work, the effects of rotational speed, feed rate and vibration amplitude have been investigated during novel method of ultrasonic-assisted bone grinding. During dissection of tumors, firstly a bone flap is removed near the target region to create passage for grinding burr. During abrasion, heat is produced, which sometimes increases the temperature to unsafe levels. So, efforts have been made to limit the temperature below the threshold levels of osteonecrosis during bone grinding. Design/methodology/approach The temperature produced during osteotomy has been measured using infrared thermography camera during the implementation of L18 Taguchi orthogonal array design. Subsequently, main effect plots and contour plots have been presented to analyze and visualize the effect of grinding parameters on temperature rise during bone grinding. Furthermore, the process parameters have been optimized for optimum results for response characteristics using Taguchi SN ratio-based optimization methodology. For multiobjective optimization, the process parameters are further optimized using grey relational analysis. Findings It is revealed that all three process parameters substantially affect the response characteristics. The proposed optimization methodology is successfully applied on the experimental findings and the optimum results for change in temperature are found to be rotational speed = 3,000 rpm, feed rate = 20 mm/min, amplitude = 10 μm and for standard deviation are 5,000 rpm, 60 mm/min, 10 μm. Research limitations/implications The present research findings cannot be generalized, and researchers are encouraged to further investigate the proposed rotary ultrasonic-assisted bone grinding at higher rotational speed up to 60k rpm on the skull bone. Originality/value The research on osteotomy is still at its initial phase, and continuous research is carried out for making patients’ life comfortable. In this direction, the authors have proposed a novel osteotomy method to limit the temperature below the threshold limit of osteonecrosis. The outcomes of the present study will be beneficial for the neurosurgeons working in this field.

Abstract:Bone cutting is an essential procedure of orthopedic surgery, while irreversible bone damage would be inevitably caused using the conventional cutting (CC) method. In this study, an ultrasonic vibration-assisted cutting (UVAC) method was applied in bone cutting to investigate the cutting performance, considering the cutting force and temperature rise, in comparison with CC. In addition, a finite element (FE) model was developed to investigate the cutting mechanism and the influence of a wide range of processing parameters on the performance of cutting bone. The results indicate that the proposed FE model shows good correlation with the experimental results for both cutting force and temperature. UVAC can significantly reduce the cutting force and increase the temperature in comparison with CC from the experimental and predicted results. The cutting force tends to decrease with the increasing vibrational parameters and decreasing cutting speed, while the temperature increases. The verified FE bone cutting model provides an efficient way to assist the optimization of the processing conditions in bone cutting operations.

Abstract:Oscillating bone sawing produces periodic exchange of tool rake and flank faces with high oscillating frequency, resulting in greater cutting forces with negative rake angles at small depths of cut and different speeds. However, only some studies have focused on these conditions, and various contradictory conclusions have emerged. This work, for the first time, observed and analyzed the unique serrated chip formation mechanism in orthogonal bone cutting at small depths of cut under different cutting conditions (i.e., positive and negative rake angles, high and low cutting speeds) using SEM (scanning electron microscope) images of bone chips and cutting process data with synchronized force data. The influence of bone viscoelasticity on new surface formation was confirmed through cutting force and average segment width measurements at different speeds. A cutting model that considers the new surface formation was proposed to explain the increase in shear stress with decreasing depth of cut, and a segment width prediction model incorporating the energy of the new surface formation and fracture toughness was developed. The predicted results, including shear force and segment width, agree well with the experimental findings. This work provides insights into chip formation in brittle materials like cortical bone. It offers the potential for predicting cutting forces in bone sawing or micro-cutting, even with varying cutting speeds.

Ex-vivo parametric study of laser ablation-based drilling of cortical bone

Abstract:Frequently orthopedic surgeries require mechanical drilling processes especially for inserted biodegradable screws or removing small bone lesions. However mechanical drilling techniques induce large number of forces as well as have substantially lower material removal rates resulting in prolong healing times. This study focuses on analyzing the impact of quasi-continuous laser drilling on the bone's surface as well as optimizing the drilling conditions to achieve high material removal rates. An ex-vivo study was conducted on the cortical region of desiccated bovine bone. The laser-based drilling on the bovine bine specimens was conducted in an argon atmosphere using a number of laser pulses ranging from 100 to 15,000. The morphology of the resulting laser drilled cavities was characterized using Energy dispersive Spectroscopy (EDS) and the width and depth of the drills were measured using a laser based Profilometer. Data from the profilometer was then used to calculate material removal rates. At last, the material removal rates and laser processing parameters were used to develop a statistical model based on Design of Experiments (DOE) approach to predict the optimal laser drilling parameters. The main outcome of the study based on the laser drilled cavities was that as the number of laser pulses increases, the depth and diameter of the cavities progressively increase. However, the material removal rates revealed a decrease in value at a point between 4000 and 6000 laser pulses. Therefore, based on the sequential sum of square method, a polynomial curve to the 6th power was fit to the experimental data. The predicted equation of the curve had a p-value of 0.0010 indicating statistical significance and predicted the maximum material removal rate to be 32.10 mm3/s with 95%CI [28.3,35.9] which was associated with the optimum number of laser pulses of 4820. Whereas the experimental verification of bone drilling with 4820 laser pulses yielded a material removal rate of 33.37 mm3/s. Therefore, this study found that the carbonized layer formed due to laser processing had a decreased carbon content and helped in increasing the material removal rate. Then using the experimental data, a polymetric equation to the sixth power was developed which predicted the optimized material removal rate to occur at 4820 pulses.

Abstract:<p>In orthopedic surgery, thermal damage of bone affects the accuracy of the prosthesis fixation and delays the postoperative recovery time. In order to avoid thermal damage in high-speed bone machining, it is necessary to establish an effective relationship among machining parameters, tool parameters, and temperature distribution of bone materials. Based on the characteristics of bone material and high-speed processing, this study proposes a novel temperature diffusion mechanism model (TDMM) in high-speed bone milling. According to the model, heat during high-speed bone milling is mainly generated by the friction effect between the bone chip and the cutter tooth. Then the cutting edge temperature of the milling cutter is determined by the equivalent static heat source, and cutting edge is regarded as a moving heat source with constant temperature. Under the action of the moving heat source, the bone material temperature distribution was determined. Moreover, the degree of thermal damage of bone materials under different machining and tool parameters was evaluated. Next, a high-speed bone milling experiment platform was established, in order to obtain accurate cutting edge temperature and bone material temperature. According to a large number of experimental measurements, the theoretical prediction temperature values of the cutting edge and bone materials were found to be within the reasonable error range of experimental measurements. The proposed model was proved to reasonably predict the temperature of the cutting edge and bone materials in high-speed bone milling. Experimental results indicate that the processing methods, tool diameter, cooling method, and cutting layer have a noteworthy impact on bone material distribution in bone milling. Spray cooling method can reduce the bone material temperature significantly. And the bone material temperature outside the cutting layer can be greatly lower than that in the cutting layer. This study also indicates that according to the TDMM, it is convenient for surgeons to select the appropriate machining parameters, and for engineers to optimize tool design.</p>

Abstract:A total of 14 insect juvenile hormone mimics was tested for activity in Amblyomma hebraeum by exposing newly engorged larvae to filter paper impregnated with hormone mimics. The most active compounds used in this assay were HS 103 (6-ethyl-3-pyridyl geranyl ether; EC50=0,0018 mg/cm2), ZR 512 (Ethyl 3,7,11-trimethyldodeca-2,4-dienoate; EC50=0,0022 mg/cm2), HS 2 (6-methyl-3-pyridyl geranyl ether; EC50=0,0035 mg/cm2), ZR 615 (N-ethyl 3,7,11-trimethyldodeca-2,4-dieneamide; EC50=0,0035 mg/cm2), ZR 777 (Prop-2-ynyl 3,7,11-trimethyldodeca-2,4-dienoate; EC50=0,0039 mg/cm2) and ZR 515 (Isopropyl-11-methoxy 3,7,11-trimethyldodeca-2,4-dienoate; EC50=0,0094 mg/cm2). Activity in this assay was similar to that reported in insects and was consistent with the susceptibility of these compounds to metabolic inactivation. The results suggest that ZR 615 may be of possible use in tick control.

Abstract:In orthopaedic surgery, bone cutting may occur serious mechanical damage due to large crack propagation, which impedes patient postoperative recovery. Ultrasonic vibration assisted cutting (UVAC) has emerged as a leading technique in bone cutting owing to its remarkable advantages, including minimal damage and cutting force. However, how to diminish its damage during bone UVAC remains challenging. Hence, this study established a sophisticated model to calculate the energy release rate for cortical bone UVAC and predicted the critical cutting depths of modes II and I brittle crack propagation. This superiority stems from the synergistic effects of periodic tool separation and the acoustic softening effect inherent in UVAC, which contribute to a lower average energy release rate and a subsequently reduced cutting force. Nevertheless, the high instantaneous energy release rate during UVAC can exceed the material's critical threshold for crack propagation, leading to the initiation of microcracks and thereby inducing material embrittlement behaviour. These insights shed light on the material removal mechanism of bone UVAC in medical orthopaedic surgery, thus enriching our understanding and potentially guiding further advancements in this field.

Analysis of machining process and thermal conditions during vibration-assisted cortical bone drilling based on generated bone chip morphologies

Abstract:When the temperature during bone drilling exceeds the safety threshold, the bone tissue surrounding the drilling site can be irreversibly damaged. To investigate the influence of vibration-assisted drilling (VAD) methods on the temperature increase during bone drilling and the causes for temperature increase, drilling experiments were performed on fresh bovine femur samples. The morphology and granularity distribution of the generated bone chips were innovatively used to directly compare the machining processes and thermal conditions of conventional drilling (CD), low-frequency vibration-assisted drilling (LFVAD), and ultrasonic vibration-assisted drilling (UVAD). The experimental results indicated that LFVAD produced the lowest temperature increase of 31.4°C, whereas UVAD produced the highest temperature increase of 44.1°C with the same drilling parameters. Additionally, the morphologies and granularity distributions of the bone chips significantly differed among these methods. We concluded that the smaller temperature increase in LFVAD was mainly attributed to the improved thermal conditions resulting from the periodic cutting/separation motion and the reliable geometric chip-breaking mechanism. In contrast, the unfavourable thermal conditions of UVAD were caused by the higher applied frequency, which created a significantly larger amount of friction heat. This was the main cause for the highest observed temperature increase, resulting in bone crushing processes that generated additional heat.
