Abstract:Background: This study compared the cardiorespiratory response of trained swimmers to 400-m unimpeded front crawl swimming (SW), treadmill running (TR) and ergometer cycling (EC) maximal exercise tests, and evaluated the validity and specificity of a method to measure maximal aerobic power in swimming. Methods: Two series of experiments were conducted. In series A (n=15), comparisons were made between VO2peak and other cardiorespiratory variables in three maximal tests: after 400-m SW, and during incremental TR and EC. In series B, VO2 peak and related variables were measured after SW and during EC (n=33). Results: No significant differences were observed between VO2peak and VE in the three modes of exercise, although SW values tended to be higher. After SW, maximal ventilatory response was characterized by higher tidal volumes (VT) and lower respiratory rates (fR) as compared with TR and EC. The highest heart rate values (fH) were also observed in TR, followed by EC and SW. In series B, no significant differences were observed either in peak VO2 or VE, but fH was also lower in SW. Conclusions: A maximal 400-m unimpeded freestyle SW test yields essentially equal or nonsignificantly higher peak VO2 and VE values than during maximal TR or EC tests in trained swimmers. The specific maximal cardiorespiratory response to the SW test is characterized by higher VT, lower fR, and lower fH. Breath-by-breath measurements during the immediate recovery after a 400-m voluntary maximal swim is proposed as a valid and specific test for directly measuring maximal metabolic parameters and evaluating specific maximal aerobic power in swimming.

Abstract:This study aimed to determine the relationship between three testing procedures during different intensity interval efforts in swimming. Twelve national-level swimmers of both genders executed, on different occasions and after a standardized warm-up, a swimming protocol consisting of either a submaximal (Submax: 8 efforts of 50 m) or a maximal interval (Max: 4 efforts of 15 m), followed by two series of four maximal 25 m efforts. Near-infrared spectroscopy in terms of muscle oxygen saturation (SmO 2 ), heart rate (HR), and blood lactate concentration (BLa) were analyzed at three testing points: after the Submax or the Max protocol (TP 1 ), after the 1st 4 × 25-m (TP 2 ), and after the 2nd maximal 4 × 25-m set (TP 3 ). BLa and HR showed significant changes during all testing points in both protocols ( P ≤ 0.01; ES range : 0.45–1.40). SmO 2 was different only between TP 1 and TP 3 in both protocols ( P ≤ 0.05–0.01; ES range: 0.36–1.20). A large correlation during the Max protocol between SmO 2 and HR ( r : 0.931; P ≤ 0.01), and also between SmO 2 and BLa was obtained at TP 1 ( r : 0.722; P ≤ 0.05). A range of moderate-to-large correlations was revealed for SmO 2 /HR, and BLa/HR for TP 2 and TP 3 after both protocols ( r range: 0.595–0.728; P ≤ 0.05) were executed. SmO 2 is a novel parameter that can be used when aiming for a comprehensive evaluation of competitive swimmers' acute responses to sprint interval swimming, in conjunction with HR and BLa.
Phosphorylation of protein S by platelet kinases enhances its activated protein C cofactor activity

Abstract:Protein S (PS) is a multifunctional plasma protein of the hemostatic and inflammatory pathways, although mechanisms for its regulation are poorly understood. Since certain plasma proteins are regulated through extracellular phosphorylation, we investigated whether the anticoagulant activity of PS is regulated through phosphorylation by platelet‐secreted kinases. PS was phosphorylated on exposure to activated platelets or their releasates, as judged by immunoblotting for phospho‐amino acids and PS. PS phosphorylation was reduced by specific inhibitors of casein kinase 1 (CK1) and casein kinase 2 (CK2) (10 μM D4476, 100 μM CK2‐inhibitory peptide YNLKSKSSEDIDESS). Involvement of CKs in PS phosphorylation was confirmed using purified CK1/CK2. Phosphorylation of PS by purified CK1 did not affect its activated protein C (APC) cofactor activity in activated partial thromboplastin time assays in PS‐depleted plasma. However, phosphorylation of PS by CK2 or by CK1/CK2 increased PS cofactor activity ~1.5‐fold (158.7±4.8%, P<0.01) or ~2‐fold (191.5±6.4%, P<0.0001), respectively. The APC cofactor activity of PS in PS‐depleted plasma exposed to platelet‐secreted kinases was enhanced, while CK2 but not CK1 inhibitors reduced APC cofactor activity. Mass spectrometry revealed a phosphorylated CK2 site at Thr37 within the N‐terminal Gla‐domain. Abstract:Objective This study aims to identify the optimal method for determining V̇O 2max in competitive swimmers in terms of validity and test–retest reliability. Design Controlled experiment. Methods Twenty competitive swimmers performed four maximal incremental exercise tests: cycling, arm cranking, ergometer swimming, and tethered swimming. Gas analysis was conducted to estimate V̇O 2max . Validity was assessed in terms of the amount of variance explained by the estimated V̇O 2max of the performance on a 1500-m time trial. Test–retest reliability was evaluated using the intraclass correlation coefficient (ICC). Results V̇O 2max obtained from tethered swimming, ergometer swimming, and cycling explained a similar amount of variance of the 1500-m performance (R 2 = 0.64, 0.64, 0.65, respectively). However, ergometer swimming yielded significantly lower V̇O 2max estimates (40.54 ± 6.55 ml/kg/min) than tethered swimming (54.40 ± 6.21 ml/kg/min) and cycling (54.39 ± 5.63 ml/kg/min). Arm cranking resulted in both a lower explained variance (R 2 = 0.41) and a significantly lower V̇O 2max (43.14 ± 7.81 ml/kg/min). Tethered swimming showed a good reliability (ICC = 0.81). Conclusions Bicycle and tethered swimming tests demonstrated high validity with comparable V̇O 2max estimates, explaining a large proportion of differences in endurance performance. Choosing between these two methods involves a trade-off between a higher practical applicability and reliability of the bicycle test and the more sport-specific nature of the tethered swimming test.

Acute Effects of a Practical Blood Flow Restriction Device During Swimming Exercise

Abstract:Purpose: The present study aimed to analyze: 1) the reliability of the tissue saturation index (TSI) and ratings of perceived discomfort (RPD) responses wearing a neoprene practical cuff (PrC), comparing with the responses from traditional (TrC) pneumatic cuffs (study I); 2) the effects of PrC on metabolic (blood lactate concentration, BLC), perceptual (rate of perceived effort, RPE) and kinematic responses at sub-maximal swimming velocities (study II). Methods: Study I; 1) PrC test-retest at rest and during swimming ergometer exercise; 2) BFR at rest with TrC inflated to different percentages of the minimum arterial occlusion pressure (MAOP; 60, 80, 100, 120 and 140%). Test-retest reliability of TSI and RPD was assessed by the intraclass correlation coefficient (ICC) and comparisons among conditions were analyzed by one-way repeated-measures ANOVA. Study II; 1) 50, 200 and 400 m swimming performances; 2) sub-maximal incremental swimming protocol with and without PrC. Two-way repeated measures ANOVA was used to compare all variables during sub-maximal velocities. Results: TSI (ICC = 0.81; 95%CI 0.62-0.91) and RPD (ICC = 0.97; 95%CI 0.94-0.99) were reliable under restricted exercise using PrC. TSI during restricted exercise was lower (p <.001) compared to unrestricted exercise (6.8 ± 6.1% vs. 21.6 ± 8.2% of physiological normalization). PrC showed higher BLC only at or above 91% of critical velocity (p < .03), while stroke rate and RPE were higher (p < .005), and stroke length was lower (p < .03) during all swimming velocities. Conclusion: This easy-to-handle and affordable practical BFR device increased physiological stress at sub-maximal efforts which could be an additional training tool for swimmers.
Comparison of cardiopulmonary responses to two types of dry-land upper-body exercise testing modes in competitive swimmers

Abstract:In this study we compared cardiopulmonary responses to upper-body exercise in 12 swimmers, using simulation of the front-crawl arm-pulling action on a computer-interfaced isokinetic swim bench and arm cranking on a modified cycle ergometer. Subjects adopted a prone posture; exercise was initially set at 20 W and subsequently increased by 10 W. min(-1). The tests were performed in a randomised order at the same time of day, within 72 h. The highest (peak) oxygen consumption (VO(2peak)), heart rate (HR(peak)), blood lactate ([la(-)](peak)) and exercise intensity (EI(peak)) were recorded at exhaustion. Mean (SEM) peak responses to simulated swimming were higher than those to arm cranking for VO(2peak) [2.9 (0.2) vs 2.4 (0.1) l x min(-1); P = 0.01], HR(peak) [174 (2) vs 161 (2) beats x min(-1); P = 0.03], and EI(peak) [122 (6) vs 102 (5) W; P = 0.02]. However, there were no significant differences in [la(-)](peak) [9.6 (0.6) vs 8.2 (0.6) mmol x l(-1); P = 0.08]. Thus simulated swimming is the preferred form of dry-land ergometry for the assessment of swimmers.
Assessing the cardiac autonomic response to bicycle exercise in Olympic athletes with different loads of endurance training: new insights from statistical indicators based on multilevel exploratory factor analysis

Abstract:Aim: The mechanisms governing the organism's response to exercise are complex and difficult to study. Spectral analysis of heart rate variability (HRV) could represent a convenient methodology for studying humans' autonomic nervous system (ANS). However, difficulties in interpreting the multitude of correlated HRV-derived indices, mainly when computed over different time segments, may represent a barrier to its usage. This preliminary investigation addressed to elite athletes proposes a novel method describing the cardiac autonomic response to exercise based on multilevel exploratory factor analysis (MEFA), which reduces the multitude of HRV-derived indices to fewer uncorrelated ANS indicators capable of accounting for their interrelationships and overcoming the above difficulties. Methods: The study involved 30 Italian Olympic athletes, divided into 15 cyclists (prevalent high-intensity endurance training) and 15 shooters (prevalent technical training with low-intensity endurance component). All athletes underwent a complete test of a dynamic protocol, constituted by a rest-stand test followed by a stepwise bicycle stress test subdivided into a single bout of progressive endurance (from aerobic to anaerobic) exercise and recovery. Then, by spectral analysis, values of 12 ANS proxies were computed at each time segment (9 epochs in all) of the complete test. Results: We obtained two global ANS indicators (amplitude and frequency), expressing the athletes' overall autonomic response to the complete test, and three dynamic ANS indicators (amplitude, signal self-similarity, and oscillatory), describing the principal dynamics over time of the variability of RR interval (RRV). Globally, cyclists have significantly higher amplitude levels (median ± MAD: cyclists 69.9 ± 20.5; shooters 37.2 ± 19.4) and lower frequency levels (median ± MAD: cyclists 37.4 ± 14.8; shooters 78.2 ± 10.2) than shooters, i.e., a parasympathetic predominance compared to shooters. Regarding the RRV dynamics, the signal self-similarity and oscillatory indicators have the strongest sensitivity in detecting the rest-stand change; the amplitude indicator is highly effective in detecting the athletes' autonomic changes in the exercise fraction; the amplitude and oscillatory indicators present significant differences between cyclists and shooters in specific test epochs. Conclusion: This MEFA application permits a more straightforward representation of the complexity characterizing ANS modulation during exercise, simplifying the interpretation of the HRV-derived indices and facilitating the possible real-life use of this non-invasive methodology.

Spirometry and oxidative stress after rebreather diving in warm water

Abstract:Introduction: Hyperbaric oxygen (HBO₂) therapy and use of enriched air can result in oxidative injury affecting the brain, lungs and eyes. HBO₂ exposure during diving can lead to a decrease in respiratory parameters. However, the possible effects of acute exposure to oxygen-enriched diving on subsequent spirometric performance and oxidative state in humans have not been recently described recently. We aim to investigate possible effects of acute (i) hyperbaric and (ii) hyperbaric hyperoxic exposure using scuba or closed-circuit rebreather (CCR) on subsequent spirometry and to assess the role of oxidative state after hyperoxic diving. Methods: Spirometry and urine samples were obtained from six well-trained divers (males, mean ± SD, age: 43.33 ± 9.16 years; weight: 79.00 ± 4.90 kg; height: 1.77 ± 0.07 meters) before (CTRL) and after a dive breathing air, and after a dive using CCR (PO₂ 1.4). In the crossover design (two dives separated by six hours) each subject performed a 20-minute session of light underwater exercise at a depth of 15 meters in warm water (31-32°C). We measured urinary 8-isoprostane and 8-OH-2-deoxyguanosine evaluating lipid and DNA oxidative damages. Results: Different breathing conditions (air vs. CCR) did not significantly affect spirometry. A significant increase of 8-OH-dG (1.85 ± 0.66 vs. 4.35 ± 2.12; P ⟨ 0.05) and 8-isoprostane (1.35 ± 0.20 vs. 2.59 ± 0.61; P ⟨ 0.05) levels after CCR dive with respect to the CTRL was observed. Subjects did not have any ill effects during diving. Conclusions: Subjects using CCR showed elevated oxidative stress, but this did not correlate with a reduction in pulmonary function.
Inspiratory muscles pre-activation in young swimmers submitted to a tethered swimming test: effects on mechanical, physiological, and skin temperature parameters

Abstract:Inspiratory muscles pre-activation (IM PA ) has been studied to improve subsequent performance in swimming. However, the effects of IM PA on various parameters in swimmers are still unknown. Therefore, this study aimed to investigate the effects of IM PA on the mechanical parameters, physiological responses, and their possible correlations with swimming performance. A total of 14 young swimmers (aged 16 ± 0 years) underwent a 30-s all-out tethered swimming test, preceded or not by IM PA, a load of 40% of the maximal inspiratory pressure (MIP), and with a volume of 2 sets of 15 repetitions. The mechanical (strength, impulse, and fatigue index) and physiological parameters (skin temperature and lactatemia) and the assessment of perceived exertion and dyspnea were monitored in both protocols. The IM PA used did not increase the swimming force, and skin temperature, decrease blood lactate concentration, or subjective perception of exertion and dyspnea after the high-intensity tethered swimming exercises. Positive correlations were found between mean force and blood lactate (without IM PA : r = 0.62, P = 0.02; with IM PA : r = 0.65, P = 0.01). The impulse was positively correlated with blood lactate (without IM PA : r = 0.71, P < 0.01; with IM PA : r = 0.56, P = 0.03). Our results suggest that new IM PA protocols, possibly with increased volume, should be developed in order to improve the performance of young swimmers.

Reliability and Validity of a Flume-Based Maximal Oxygen Uptake Swimming Test

Abstract:A mode-specific swimming protocol to assess maximal aerobic uptake (VO2maxsw) is vital to accurately evaluate swimming performance. A need exists for reliable and valid swimming protocols that assess VO2maxsw in a flume environment. The purpose was to assess: (a) reliability and (b) "performance" validity of a VO2maxsw flume protocol using the 457-m freestyle pool performance swim (PS) test as the criterion. Nineteen males (n = 9) and females (n = 10) (age, 28.5 ± 8.3 years.; height, 174.7 ± 8.2 cm; mass, 72.9 ± 12.5 kg; %body fat, 21.4 ± 5.9) performed two flume VO2maxsw tests (VO2maxswA and VO2maxswB) and one PS test [457 m (469.4 ± 94.7 s)]. For test-retest reliability (Trials A vs. B), moderately strong relationships were established for VO2maxsw (mL·kg-1·min-1)(r= 0.628, p = 0.002), O2pulse (mL O2·beat-1)(r = 0.502, p = 0.014), VEmax (L·min-1) (r = 0.671, p = 0.001), final test time (sec) (0.608, p = 0.004), and immediate post-test blood lactate (IPE (BLa)) (0.716, p = 0.001). For performance validity, moderately strong relationships (p < 0.05) were found between VO2maxswA (r =-0.648, p = 0.005), O2pulse (r= -0.623, p = 0.008), VEmax (r = -0.509 p = 0.037), and 457-m swim times. The swimming flume protocol examined is a reliable and valid assessment of VO2maxsw., and offers an alternative for military, open water, or those seeking complementary forms of training to improve swimming performance.
Upper limb kinematic differences between breathing and non-breathing conditions in front crawl sprint swimming

Abstract:The purpose of this study was to determine whether the breathing action in front crawl (FC) sprint swimming affects the ipsilateral upper limb kinematics relative to a non-breathing stroke cycle (SC). Ten male competitive swimmers performed two 25m FC sprints: one breathing to their preferred side (Br) and one not breathing (NBr). Both swim trials were performed through a 6.75m(3) calibrated space and recorded by six gen-locked JVC KY32 CCD cameras. A paired t-test was used to assess statistical differences between the trials, with a confidence level of p<0.05 accepted as significant. Swimmers were slower (3%) when breathing. Within the entry phase, swimmers had a slower COM horizontal velocity (3.3%), less shoulder flexion (8%), abduction (33%) and roll (4%) when breathing. The pull phase was longer in duration (14%) swimmers had a shallower hand path (11%), less shoulder abduction (11%), a slower hand vertical acceleration (30%) and slower centre of mass (COM) horizontal velocity (3%) when breathing. In the push phase, swimmers had a smaller elbow range of motion (ROM) (38%), faster backwards hand speed (25%) and faster hand vertical acceleration (33%) when breathing. Swimmers rolled their shoulders more (12%) in the recovery phase when breathing. This study confirms that swim performance is compromised by the inclusion of taking a breath in sprint FC swimming. It was proposed that swimmers aim to orient their ipsilateral shoulder into a stronger position by stretching and rolling the shoulders more in the entry phase whilst preparing to take a breath. Swimmers should focus on lengthening the push phase by extending the elbow more and not accelerating the hand too quickly upwards when preparing to inhale.
Underwater Near-Infrared Spectroscopy: Muscle Oxygen Changes in the Upper and Lower Extremities in Club Level Swimmers and Triathletes

Abstract:To date, measurements of oxygen status during swim exercise have focused upon systemic aerobic capacity. The development of a portable, waterproof NIRS device makes possible a local measurement of muscle hemodynamics and oxygenation that could provide a novel insight into the physiological changes that occur during swim exercise. The purpose of this study was to observe changes in muscle oxygenation in the vastus lateralis (VL) and latissimus dorsi (LD) of club level swimmers and triathletes. Ten subjects, five club level swimmers and five club level triathletes (three men and seven women) were used for assessment. Swim group; mean±SD=age 21.2±1.6 years; height 170.6±7.5 cm; weight 62.8±6.9 kg; vastus lateralis skin fold 13.8±5.6 mm; latissimus dorsi skin fold 12.6±3.7. Triathlete group; mean±SD=age 44.0±10.5 years; height 171.6±7.0 cm; weight 68.6±12.7 kg; vastus lateralis skin fold 11.8±3.5 mm; latissimus dorsi skin fold 11.2±3.1. All subjects completed a maximal 200 m freestyle swim, with the PortaMon, a portable NIR device, attached to the subject's dominant side musculature. ΔTSI% between the vastus lateralis and latissimus dorsi were analysed using either paired (2-tailed) t-tests or Wilcoxon signed rank test. The level of significance for analysis was set at p<0.05. No significant difference (p=0.686) was found in ΔTSI (%) between the VL and LD in club level swimmers. A significant difference (p=0.043) was found in ΔTSI (%) between the VL and LD in club level triathletes. Club level swimmers completed the 200 m freestyle swim significantly faster (p=0.04) than club level triathletes. Club level swimmers use both the upper and lower muscles to a similar extent during a maximal 200 m swim. Club level triathletes predominately use the upper body for propulsion during the same exercise. The data produced by NIRS in this study are the first of their kind and provide insight into muscle oxygenation changes during swim exercise which can indicate the contribution of one muscle compared to another. This also enables a greater understanding of the differences in swimming techniques seen between different cohorts of swimmers and potentially within individual swimmers.

Capillary blood gases (pH, PCO<inf>2</inf>, PO<inf>2</inf>, HCO<inf>3</inf>, Hct, Hb) were collected at four (4) measurement time points: 1) at rest once; 2) at rest just after the 3rd repetition; 3) at finish; and 4) at 10 min of recovery. Heart rate (HR) during swimming, lactate acid (La) concentration and swimming time (t<inf>50</inf>) were measured. Results: Our study showed uncompensated metabolic acidosis due to increased lactic acidosis in both breathing conditions, more pronounced in apnea, and faster swimming times in apnea without different heart rate responses. Conclusions: Apnea during repetitive high intensity long distance interval 50 m freestyle swimming causes uncompensated metabolic acidosis but improves sprint performance.
Three political realities in expanding coverage for the working poor: one state's experience.

Abstract:In recent years, states have experimented with innovative approaches to expand coverage for underserved persons. Yet lack of health insurance among low income workers remains a problem. A 1997 survey reported that more than half of adults in low income, working families have been uninsured sometime in the previous two years. This group is likely to increase in the future as states implement time-limited welfare benefits and if employer-sponsored health coverage continues to decrease as in recent years. This paper describes the bipartisan effort of the Indiana Commission on Health Care for the Working Poor to design coverage expansions for uninsured, low income workers and families in Indiana. Initially, legislators challenged the state's insurance industry to develop a private health insurance plan that the state might subsidize to make it affordable to low income workers and/or their employers. However, based on findings from Indiana focus groups of working poor that the Commission conducted, the Commission recommended strategies to mobilize existing safety net providers into networks to provide comprehensive and coordinated care to low income workers and families in designated service areas. Specifically, the state would pay direct subsidies for primary and preventive care to community health centers that participate in networks with safety net hospitals. Also, the state would establish a state-sponsored stop loss fund to protect network hospitals against expenses incurred in the care of catastrophically ill network clients. These strategies reflect three realities that face states as they craft health coverage expansions for uninsured low income workers and families in today's health policy environment. First, in a conservative state, policy makers are unwilling to pay the cost of even limited coverage for low income workers and families. Second, the uninsured poor already obtain care from safety net providers that are programmatically constituted and ideologically committed to serve them. Third, this lack of demand for health coverage with minimum benefits threatens voluntary participation in a subsidized private program. The findings of the focus groups confirmed that, unless heavily subsidized, it is unlikely that low income workers are going to purchase even heavily subsidized health insurance to finance their health care. Given the constraints of the day, addressing the need for health coverage for the remaining uninsured calls for innovative thinking beyond conventional paradigms. Direct subsidies for community health centers and subsidized stop loss protection for safety net hospitals to facilitate the development of networks are promising strategies for extending coverage to uninsured, low income workers and families. While not a substitute for enrollment in a fully-paid health plan, these strategies do promise real improvement in access to high quality health care services for this group of vulnerable Americans.
