Abstract:HPLC method is the standard method for the separation and quantification of impurities from quinolone antibiotics. However, due to the large differences in the UV absorption of the impurities in quinolone antibiotics, quantitative analysis without the availability of corresponding reference substances currently poses a challenge. A sensitive and direct method using high performance liquid chromatography coupled with diode array detector and charged aerosol detector (HPLC-DAD-CAD) was developed for the analysis of impurities in quinolone antibiotics. The chromatographic conditions were optimized for good separation and output signal of CAD detector by response surface method (RSM). The systematic variation of CAD parameter settings, such as nebulization temperature, filter constant and power function value (PFV), were used to study the effect of on the detector response of signal-to-noise ratios (S/N) and linearity for ofloxacin, ciprofloxacin and their impurities. In the method validation, good linearity of each component was obtained with coefficient of determination (r) greater than 0.999 in the range of 0.5-300 mu g mL-1. The average recoveries of each component were 99.02-102.39 % by DAD, were 98.22-101.91 % by CAD, RSDs were less than 2.5 % for intra-day and inter-day precision by DADCAD, with good precision and accuracy. The correction factor experimental results showed that the developed method provided a uniform response to the impurities with differences chromophores and could unbiasedly and directly detect the impurities in quinolone antibiotics. The method is first reported application of HPLC-DADCAD method for the analysis of impurities in quinolone antibiotics and it can be used for quality control of quinolone antibiotics.
Simultaneous determination of serum concentration of amoxicillin and clavulanate by HPLC with fluorescence detection

Abstract:OBJECTIVE: To establish a HPLC method for the simultaneous determination of amoxicillin and clavulanate in human serum. METHODS: Serum samples were pretreated with benzaldehyde, then separated on the Discovery C 18 column (5 μm, 150 mm × 4.6 mm) at 29°C with MeOH-H 2O = 55:45 as the mobile phase at a flow rate of 1.0 mL·min -1. The fluorescence detection wavelengths were: λex = 386 nm and λem = 460 nm. RESULTS: Clavulanate and amoxicillin were eluted at about 5.10 min and 6.20 min, respectively. The calibration curves were linear (r = 0.9999) from 0.062 to 4.968 mg·L -1 for clavulanate and from 0.089 to 35.718 mg·L -1 I amoxicillin, respectively. CONCLUSION: The method is sensitive, accurate, reproducible, and suitable for the pharmacokinetic studies of amoxicillin sodium and clavulanate potassium formulations.
Development and Validation of A New Ultra-Performance Liquid Chromatographic Method for Vancomycin Assay in Serum and Its Application to Therapeutic Drug Monitoring

Abstract:Objective:The aim of this study was to develop and validate an ultra-performance liquid chromatographic (UPLC) method with photodiode array detector for the measurement of vancomycin in human serum samples for therapeutic drug monitoring or other applications.Methods:The method included the extraction of vancomycin in serum by deproteinization with acetonitrile. The analyses were carried out using an ACQUITY UPLC BEH C-18 column (2.1 x 50 mm, 1.7 m) using acetonitrile and 0.005 M KH2PO4 buffer (pH 2.5) as the mobile phase at a flow rate of 0.3 mL/min, with photodiode array detection at 230 nm. The method was validated for extraction recovery, inter- and intraday precision (relative standard deviation, RSD%), and accuracy and stability of vancomycin in serum. Both the established UPLC method and fluorescence polarization immunoassay (FPIA) were used to measure the prepared quality control (QC) samples (1.0, 7.0, 35.0, 75.0 mg/L) to validate the accuracy of UPLC. Furthermore, both methods were subsequently used to assay the vancomycin concentration in 172 clinical serum samples collected from patients receiving vancomycin in the hospitals localized in Shanghai (China) and 32 control samples from United Kingdom National External Quality Assessment Service (UK NEQAS).Results:The retention time of vancomycin was 2.6 minutes. The calibration curve for UPLC was linear over the range 1.0-100.0 mg/L (R-2 > 0.999). The method was fully validated in terms of recovery, selectivity, accuracy, precision, and various conditions. The absolute difference% and RSD% of the prepared QC samples assayed by UPLC were all better than the results by FPIA. A paired t test of the results of the prepared QC samples indicated that the results of all the QC samples had significant difference (P < 0.05), except for the 7.0 mg/L QC samples, which suggested that UPLC was more accurate for the samples containing low or high concentration of vancomycin. A correlation with the Deming model provided a good linear relation between the results of the 2 methods applied to 172 samples, with equation of UPLC = 0.99 x FPIA - 0.19 (R-2= 0.923), and the agreement of the 2 methods was illustrated using Bland-Altman plot with a mean difference (UPLC - FPIA) of -0.428 mg/L and 95% confidence interval of -8.33 to 7.47 mg/L, respectively. A Student t test comparing results obtained by the UPLC method and group mean results of control samples from UK NEQAS were not significant (P = 0.057).Conclusions:A short analysis time, small amount of serum needed, high specificity, and accuracy make the UPLC method developed in this study appropriate and practical for vancomycin therapeutic drug monitoring and could be applied to other nonserum applications or where requiring superior validation parameters such as for pharmacokinetic/pharmacodynamic studies.

Abstract:A rapid and simple ultra high performance liquid chromatography with tandem mass spectrometry method was developed and validated for the simultaneous separation and determination of vancomycin and its crystalline degradation products in human serum. Vancomycin and two isomers of the degradants were extracted from human serum with a protein precipitation method. The compounds were separated on an Acquity BEH C18 column (2.1 x 50 mm, 1.7 mu m) eluted with a gradient mixture of acetonitrile and 0.1% formic acid as the mobile phase. Norvancomycin was used as the internal standard. The linear ranges of vancomycin and two degradant isomers were 1.057-105.7, 0.1437-14.37, and 0.2540-25.40 mu g/mL, respectively. The established methods were validated and successfully applied to a therapeutic drug monitoring study of vancomycin in patients with renal insufficiency.

Abstract:Antimicrobial resistance (AMR) is a major threat to global health due to the wide use of antibacterial drugs. Multiple studies show that the pharmacokinetic/pharmacodynamic (PK/PD) studies of antibiotics are an approach to prevent/delay AMR. The pharmacokinetic parameters of antibiotics are the basis of PK/PD studies, and therapeutic drug monitoring (TDM) is the key method to obtain pharmacokinetic information. We developed an ultra-performance liquid chromatography–tandem mass spectrometry to determine 18 antibacterial drugs (piperacillin, cefazolin, cefuroxime, cefoperazone, ceftriaxone, cefepime, aztreonam, meropenem, imipenem, levofloxacin, moxifloxacin, azithromycin, clindamycin, tigecycline, linezolid, vancomycin, voriconazole and caspofungin) in human plasma for practical clinical usage. Samples were prepared using protein precipitation with methanol. Chromatographic separation was accomplished in 6 min on a BEH C 18 column (2.1 × 100 mm, 1.7 μm) using a gradient elution of acetonitrile and 0.1% formic acid in water at a flow rate of 0.3 ml/min. The electrospray ionization source interface was operated in the positive and negative ionization modes. Inter- and intra-day precision, accuracy, recovery, matrix effect, and stability were validated according to the Food and Drug Administration guidance. The correlation coefficients of calibration curves were all greater than 0.99. The accuracies of the 18 antibacterial drugs ranged from 89.1% to 112.4%. The intra-day precision of the analytes ranged from 1.4% to 9.3% and the inter-day precision from 2.1% to 7.2%. The matrix effects ranged from 93.1% to 105.8% and the extraction recoveries ranged between 90.1% and 109.2%. The stabilities of the 18 antibacterial drugs in plasma were evaluated by analyzing three different concentrations following storage at three storage conditions. All samples displayed variations less than 15.0%. The validated method was successfully applied to routine clinical TDM for 231 samples.

Abstract:OBJECTIVE To establish a method for the simultaneous determination of paracetamol and caffeine in human plasma by HPLC. METHODS Theophylline was used as the internal standard. A Zorbax-Sb-C_ 18 analytical column ( 4.6 mm×250 mm,5 μm) was used and the column temperature was 40 ℃. The mobile phase was a mixture of acetonitrile-methanol-buffer (7∶7∶86, the buffer consisted of formic acid 0.2 % and ammonium formate 18 mmol·L~ -1) and the flow rate was 1.0 mL·min~ -1 . The injection volume was 20 μL. The UV detection wavelenght was 273 nm. RESULTS The linear range of paracetamol and caffeine in plasma were 0.125- 8.0 mg·L~ -1 and 0.0125- 0.8 mg·L~ -1, respectively. The method recoveries were 95.9 %- 108.7 % and 92.6 %- 108.1 %, the RSDs of intra-day were less than 3.0 % and 10.0 %, the RSDs of inter-day were no more than 12.0 % and 5.0 %, respectively. CONCLUSION The method is convenient, specific, accurate and sensitive enough to be applied to the pharmacokinetic studies of paracetamol and caffeine in human plasma .

Abstract:In this study, a specific and rapid high-performance liquid chromatography (HPLC) method has been developed and validated for the simultaneous determination of amoxicillin, lansoprazole, and levofloxacin in pharmaceuticals. Paracetamol was used as internal standard (IS) in the measurements. UV–Vis absorption spectra of the analytes and the IS were taken for the determination of suitable absorption wavelength of UV–Vis detector (diode array detector, DAD) in the HPLC instrument. A reverse-phase C18 column was used in the separation and determination of amoxicillin, lansoprazole, and levofloxacin together with the IS. The pharmaceutical analytes were quantified by the UV–Vis diode array detector in the HPLC using MeOH-0.01 M CH3COONH4 (70:30) as the mobile phase. The linear calibration curves of them were measured in the ranges of 15–40 mg/L, 2.5–15.0 mg/L, and 7.5–20.0 mg/L for amoxicillin, lansoprazole, and levofloxacin, respectively. Excellent calibration correlations (R2: 0.9942, 0.9997, and 0.9974) were obtained. The percentage recoveries of the amoxicillin, lansoprazole, and levofloxacin in commercial pharmaceuticals were obtained as 105.5%, 98.57%, and 102.5%, respectively. The results showed that amoxicillin, lansoprazole, and levofloxacin together with paracetamol IS could be separated and determined simultaneously with low LOD and LOQ values using the proposed HPLC method.

Tumorous deformity of mitral valve leaflet after chordal rupture in a child

Abstract:A case with tumorous deformity of the posterior mitral valve leaflet after spontaneous chordal rupture in a child is described. A partial rupture in the chordae tendineae of the posterior mitral leaflet was found by echocardiography in a 9‐year‐old Japanese boy. Tumorous bulging was gradually developed in the leaflet and was surgically excised 5 years later. Multiple nodular tumors were found on the atrial surface of the posterior mitral leaflet. Histological examination revealed that the tumorous bulging consisted of myxomatous materials in which collagen fibrils and very fine elastic fibers were distributed loosely and irregularly. Normal‐looking endothelial cells covered the luminal surface of the bulging lesion. Vimentin‐positive spindle‐shaped mesenchymal cells were scattered in the bulge area. The labeling index of proliferating cell nuclear antigen (PCNA) in these cells was 29.3%. These spindle cells were positive for matrix metalloproteinase (MMP)‐1 in the entire bulge area. The cells and matrix were positive for MMP‐2 and tissue inhibitor of MMP (TIMP)‐1 in the basal area of bulging, but were weakly positive or negative at the surface area. Reactivity for TIMP‐2 in the cells in the bulge area was obviously weaker than that in the cells at the spongiosa of the anterior mitral leaflet, which was obtained from the patient at the valve replacement operation 9 months after the initial operation. These findings indicated that the tumorous deformity of the mitral valve was formed by the overgrowth of valve tissue under the stimulation of mitral regurgitation in this child, and the imbalance of MMP and TIMP might play an important role in the bulge formation.
Comparison of EMIT and 2D-HPLC-UV in the Quantification of Van-comycin in Human Plasma

Abstract:There exist great individual differences in the concentration of vancomycin, so it is necessary to monitor its concentration in clinic. An enzyme multiplied immunoassay technique (EMIT) and a two-di-mensional liquid chromatography coupled with ultraviolet detection method (2D-HPLC-UV) were established to measure the concentration of vancomycin in human plasma. The linear range, accuracy and precision of the two methods satisfied the requirements for bioanalysis. Sixty plasma samples from patients intravenously infused with vancomycin were detected by EMIT and 2D-HPLC-UV, respectively. There were no significant differences in the plasma concentrations of vancomycin detected by EMIT and 2D-HPLC-UV. A good cor-relation was obtained between the two methods. The regression equation was Y2D-HPLC-UV=0.9670×XEMIT-0.7029. Bland-Altman plots analysis showed the two methods were consistent. Both EMIT and 2D-HPLC-UV were effective and reliable in the quantification of vancomycin. The monitoring purpose, laboratory conditions and staffing should be taken into consideration when a method was chosen for monitoring the concentration of vancomycin.

Abstract:A rapid and specific liquid chromatography/tandem mass spectrometry method was described for the simultaneous determination of hydrochlorothiazide and valsartan in human plasma. After extracted from plasma using methanol, hydrochlorothiazide, valsartan and hydroflumethiazide, irbesartan, used as the internal standard, respectively, were chromatographically analyzed on a Phenomenex Kromasil C-8 column with water and methanol ( 27: 73, v/v) as the mobile phase. Selected reaction monitoring was specific for mass detection employing negative electrospray ionization. The calibration standards were linear over the concentration range (3.13-800 ng/ml for hydrochlorothiazide and 11.72-3000 ng/ml for valsartan). The method was found to be suitable for application to a pharmacokinetic study after oral administration of dispersible tablet containing 12.5mg hydrochlorothiazide and 80 mg valsartan to 20 healthy volunteers.

Abstract:Aim: This study aimed to develop and validate a method for better therapeutic monitoring of vancomycin serum concentration. Methods & results: An ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) method was developed and validated to minimize the interference of crystalline degradation product and matrix. It was compared with chemiluminescence microparticle immunoassay (CMIA)and ultra-performance liquid chromatography with ultraviolet detection (UPLC-UV) in the performance of testing normal, on-dialysis and hemolytic serum samples. For on-dialysis samples, a moderate correlation (r=0.534) was observed between UPLC-UV and UPLC-MS/MS. In testing hemolytic samples, ten (10/85, 11.8%) samples were overestimated by CMIA method. Conclusion: Vancomycin concentration determined by CMIA, UPLC-UV was more affected by various panels of serum samples than UPLC-MS/MS assay, suggesting that UPLC-MS/MS is a more reliable and promising tool for clinical vancomycin therapeutic drug monitoring.

Abstract:Abstract Objectives Cefiderocol and ceftobiprole are new generation cephalosporin antibiotics that exhibit high inter-individual plasma concentration variability that potentially impact their efficacy or toxicity. The aim of this study was to develop and validate a selective, simple, and fast UPLC-MS/MS method for simultaneous quantification of cefiderocol and ceftobiprole in human plasma to enable their therapeutic drug monitoring (TDM) and support PK and PK/PD studies, in particular in critically ill patients. Methods After a simple and fast single-step protein precipitation, cefiderocol and ceftobiprole were separated on a Waters Acquity UPLC BEH C18 column by linear gradient elution; with subsequent detection by Shimadzu MS 8060 triple quadrupole tandem mass spectrometer in a positive ionization mode. Results Analysis time was 5 min per run. The analytical performance of the method in terms of specificity, sensitivity, linearity, precision, accuracy, matrix effect (ME), extraction recovery (ER), limit of quantification, dilution integrity, and stability of analytes under different conditions met all criteria for a bioanalytical method for the quantification of drugs. The calibration curves were linear over the range of 1–200 mg/L for cefiderocol and 0.5–100 mg/L for ceftobiprole with a linear regression coefficient above 0.995 for both. Conclusions A simple, fast, and selective liquid chroma-tography-tandem mass spectrometry method was developed and validated for the simultaneous quantification of cefiderocol and ceftobiprole. This new method was successfully applied to the measurement of plasma concentration of cefiderocol and ceftobiprole in critically ill patients and showed good performance for their therapeutic monitoring and optimizing antibiotic therapy.

Abstract:Dalbavancin is a novel semisynthetic glycopeptide antibiotic that comprises multiple homologs and isomers of similar polarities. However, pharmacokinetic studies have only analyzed the primary components of dalbavancin, namely B(0)and B-1. In this study, an ultra-high-performance liquid chromatography-tandem mass spectrometry (UHPLC-MS/MS) method was developed to simultaneously determinate and investigate the five homologous components of dalbavancin, namely, A(0), A(1), B-0, B-1, and B-2, in rat plasma. In this method, methanol was used to precipitate plasma, and a triple-bonded alkyl chromatographic column was used for molecule separation, using 0.1% formic acid-acetonitrile as the mobile phase for gradient elution. Targeted homologs were analyzed by a triple quadrupole mass spectrometer using positive electrospray ionization in multiple reaction monitoring mode. The linearity range was 50-2500 ng/mL with a high correlation coefficient (r(2)> 0.998). This method was successfully applied in the pharmacokinetic analysis of dalbavancin hydrochloride to investigate dalbavancin components in rats.

Abstract:To minimize confusion in the expression of measurement results of stable isotope and gas-ratio measurements, recommendations based on publications of the Commission on Isotopic Abundances and Atomic Weights of the International Union of Pure and Applied Chemistry (IUPAC) are presented. Whenever feasible, entries are consistent with the Système International d'Unités, the SI (known in English as the International System of Units), and the third edition of the International Vocabulary of Basic and General Terms in Metrology (VIM, 3rd edition). The recommendations presented herein are approved by the Commission on Isotopic Abundances and Atomic Weights and are designed to clarify expression of quantities related to measurement of isotope and gas ratios to ensure that quantity equations instead of numerical value equations are used for quantity definitions. Examples of column headings consistent with quantity calculus (also called the algebra of quantities) and examples of various deprecated usages connected with the terms recommended are presented.

Abstract:The highly overlapping chromatograms and spectra of the major metabolites of asprin, salicylic acid (SA) and gentisic acid (GA) in the presence of uncalibrated and interfering pyrocatechuic acid (PA) were resolved using HPLC-DAD combined with SWATLD algorithm. The elution time was set from 0.6628 min to 0.9495 min and the detection wavelength was chosen from 268 nm to 332 nm. Both SA and GA were determined simultaneously in aqueous solution with their recoveries were (100.5 ± 6.7) % and (109.2 ± 5.3) %, respectively. The satisfying results indicate this method can be easily performed and applied to solving second-order calibration problems quickly and accurately.

METHOD The plasma samples were extracted with 1.25 mol×L -1 perchloric acid. The analysis involved a 150mm×4.6mm ID column packed with C 18( 5μm ). The mobile phase consisted of 1.50 mmol·L -1 perchloride acid solution-acetonitrile( 85∶15 ) with a flow rate of 1.20 mL·min -1. The UV detector were set at 254nm.RESULTS A good linearity was obtained in the rang of 0.10 ～ 6.00 mg·L -1( r=0.9999 ). The recovery was between 98.0% ～ 110.0%. The relative standard deviation of within-day and between-day were less than 13% ( n=6 ). The minimal detectable concentration in plasma was 0.10 mg·L -1. CONCLUSION The established method is sensitive, accurate and simple for the determination of cefpodoxime levels in human plasma. It is suitable for the pharmacokinetics and bioavailability study of cefpodoxime proxetil.

Abstract:A simultaneous rapid, sensitive supercritical fluid chromatography and tandem mass spectrometry method for the separation and quantification of valsartan (VAL) and hydrochlorothiazide (HCT) is developed and validated. Separation was done in a Nucleosil (250 mm × 4.6 mm) 10 mm, Nucleosil-C 18 column with acetonitrile modified supercritical fluid carbon dioxide (15 mL min –1 ) as mobile phase. The column temperature was 45°C and the outlet pressure was set at 17 MPa. The analytes were quantitated in negative ionization by multiple reaction monitoring (MRM) with a mass spectrometer, the mass ionizations m/z 434.2 – 350.2 and m/z 295.9 – 204.9 used to measure VAL and HCT respectively. The assay exhibited a linear dynamic range of 3.2 – 3200 ng/mL for VAL and 0.8 – 800 ng/ml for HCT. The method was validated and successfully used for simultaneous determination of drugs in tablets.
