Abstract:Ecological Momentary Assessment (EMA) and retrospective hearing questionnaires like the Speech Spatial Qualities of Hearing Scale (SSQ) provide insights into real-world hearing-aid use and outcomes that are unavailable from each in isolation ( Wu et al. 2020 ). Specifically, EMA responses provide in-situ information about real-life listening experiences, and immediate opinions about outcome and listening intent at a specific moment in time ( Shiffman et al. 2008 ; Wu et al. 2015 ; Timmer et al. 2018 ; Holube et al. 2020 ), while retrospective questionnaires provide insights into the way a user recalls hearing-aid function over time ( Robinson & Clore 2002 ) over a wide variety of listening situations ( Gatehouse & Noble 2004 ). Each approach has advantages and disadvantages (see later) but likewise, each can play a different role in understanding opinions about a hearing aid and assist in the choice of hearing-aid settings or model for a particular individual ( Lelic et al. 2024 ). For clinical purposes, it is therefore valuable to learn what combination of information from retrospective questionnaires and EMAs provide optimal clinical insights, face validity, and generalizability of outcomes so that time spent on clinical investigations and patient burden can be minimized. In addition to this, today's technology allows for seamless recording of objective environmental data, such as ambient acoustic characteristics, which can potentially help contextualize EMA outcomes ( Williger & Lang 2015 ; Christensen et al. 2021 ; Yellamsetty et al. 2021 ; Hart et al. 2022 ). However, EMA can be problematic because the process is invasive and burdensome to participants, necessitates a mobile phone, and can lead to biased sampling of situations due to a lack of willingness or it being inappropriate to respond to EMAs in certain situations ( Schinkel-Bielefeld et al. 2020 ). Retrospective questionnaires, on the other hand are less invasive and burdensome, but they rely on recollection and thus encounter recency biases and recall issues ( Bradburn et al. 1987 ; Stone & Shiffman 2002 ).

Abstract:Ecological momentary assessment (EMA) is a method to assess the test participants' sensations, perceptions, and experiences in everyday life. It consists of repeated real-time questionnaires in the respondent's natural environment ( Stone & Shiffman 1994 ; Shiffman et al. 2008 ). Compared with laboratory experiments, it is less controlled but also less artificial and more representative of the participants' life. While it has been applied in other areas for decades, EMA has only recently become popular in audiology (see Holube et al. 2020 , for an overview). One potential use of EMA in audiology is the comparison of two technologies such as two hearing aids or two hearing programs ( Jensen et al. 2019 ; Wu et al. 2019 ; Schinkel-Bielefeld 2020 ; Andersson et al. 2021 ; Glista et al. 2021 ). For example, Wu et al. (2019 ) compared basic and high-end hearing aids in daily life using EMA, Jensen et al. (2019 ) compared two different gain prescriptions, whereas Schinkel-Bielefeld (2020 ) and Andersson et al. (2021 ) compared hearing programs with different directional characteristics for noise reduction (NR).

Abstract:Background: Recently, we developed a hearing-related lifestyle questionnaire (HEARLI-Q) which asks respondents to rate their hearing aid (HA) satisfaction in 23 everyday listening situations. It is unknown how HA satisfaction on the retrospective HEARLI-Q scale compares to HA satisfaction measured on the same scale implemented in Ecological Momentary Assessments (EMA). Purpose: To learn how retrospective (HEARLI-Q) and in-situ (EMA) assessments can complement each other. Research Design: An observational study. Study Sample: Twenty-one experienced HA users. Data Collection and Analysis: The participants first filled out the HEARLI-Q questionnaire, followed by a one-week EMA trial using their own hearing aids. HA satisfaction ratings were compared between the two questionnaires and the underlying drivers of discrepancies in HA satisfaction ratings were evaluated. Results: HA satisfaction scores were significantly higher in EMA for speech communication with one or several people. Hearing difficulty in these situations was rated higher in HEARLI-Q than in EMA, but occurrence of those difficult listening situations was also rated to be lower. When comparing only the situations that occur on daily or weekly basis, the two questionnaires had similar HA satisfaction ratings. Conclusions: Lower occurrence of difficult listening situations seems to be the key driver of discrepancies in HA satisfaction ratings between EMA and HEARLI-Q. The advantage of EMA is that it provides insight into an individual’s day-to-day life and is not prone to memory bias. HEARLI-Q, on the other hand, can capture situations which occur infrequently or are inconvenient to report in the moment. Administering HEARLI-Q and EMA in combination could give a more holistic view of HA satisfaction.

Abstract:Assessment, Ahead of Print. Although ecological momentary assessment (EMA) provides an opportunity for the examination of intervention mechanisms in real time, there are few validated tools to assess key treatment mechanisms in EMA studies. Our focus in this study is a potentially critical treatment mechanism, improvement in self-efficacy for managing negative emotions. We examined the psychometric properties of the Patient-Reported Outcomes Measurement Information System (PROMIS) self-efficacy for managing negative emotions scale measured via EMA. Participants (n = 145 college students) in a clinical trial of Dialectical Behavior Therapy skills completed four daily EMAs for 6 weeks (13,153 total responses). Results indicated (a) excellent internal consistency and good factor structure, (b) sufficient variability at both the between- and the within-persons levels, and (c) strong construct and predictive validity. This study supports the validity of an EMA measure of self-efficacy for managing negative emotions that can be used in real time, both in intervention studies and in observational research.

Feasibility of the HEAR-aware App for Hearing Loss Self-Management: A Nonrandomized Intervention Study to Examine Intervention Acceptability and the Stages-of-Change Concept

Abstract:Objectives: The HEAR-aware project targets adults ≥50 years who were recently diagnosed with hearing loss and declined hearing aids, but were open for support via a smartphone app on different target behaviors (TBs). The HEAR-aware app, based on Ecological Momentary Assessment and Ecological Momentary Intervention (EMA, EMI), contains educational materials ("snippets") tailored partly to the user's experienced listening situations. The app aims to increase adults' TB-specific readiness to take action on hearing problems. The present study focused on examining feasibility regarding three novel aspects: (1) the app's acceptability, mainly regarding its EMA and EMI elements (compliance, usability, usefulness, satisfaction), (2) psychometric properties of 10 new TB-specific stages-of-change (SoC) measures (test-retest reliability, construct validity), and (3) the potential of tailoring snippets on a person's SoC. Design: A nonrandomized intervention study including four measurements with 2-week intervals (T0-T3). (1) The intervention period lasted 4 weeks. App usage data were collected throughout (T1-T3). Usability, usefulness, and satisfaction were measured at T3 (n = 26). (2) Reliability concerned T0 and T1 data, in between which no intervention occurred. Intraclass correlation coefficients (ICCs) were calculated (n = 29). Construct validity was examined by calculating correlations between the different TB-specific scales (at T0), and also between each of them and self-reported hearing disability (n = 29). (3) Person-tailoring by SoC was examined using T0 and T1 data. Linear mixed models were applied to test whether users rated snippets corresponding to their SoC as more interesting and useful than noncorresponding snippets (n = 25). Results: (1) The percentage of participants that complied with the intended usage varied across the five predefined compliance criteria (lowest: 8%; highest: 85%). Median snippet satisfaction scores were reasonably positive (3.5 to 4.0 of 5). Usability was good (System Usability Score, mean = 72.4, SD = 14.3) and usefulness satisfactory (Intrinsic Motivation Inventory, mean = 4.4, SD = 1.4), but showed large variance. (2) The 10 TB-specific scales showed fair-to-excellent reliabilities (range ICCs = 0.51 to 0.80). Correlations between the TB-specific scales ranged between -0.17 ( p > 0.05) and 0.74 ( p < 0.001), supporting only partly overlap between their underlying constructs. Only the correlation between TB-specific readiness for hearing aid uptake and self-reported hearing disability was significant. (3) Correspondence of a snippet's SoC with the person's SoC significantly related to "interesting" ratings ( p = 0.006). Unexpectedly, for snippets with a lower SoC than the participant's, further deviation of the snippet's SoC from the participant's SoC, increased the participant's interest in the snippet. The relationship with "usefulness" was borderline significant. Conclusions: (1) Overall usability, usefulness, and satisfaction scores indicated sufficient app acceptability. The high variance and fairly low compliance showed room for improving the app's EMA/EMI parts for part of the participants. (2) The 10 new TB-specific SoC measures showed sufficient reliability, supporting that they measured different types of readiness to take action on hearing problems (construct validity). (3) The unexpected findings regarding tailoring educational app materials to individuals' SoC deserve further study.
Building an ecological momentary assessment smartphone app for 4- to 10-year-old children: A pilot study

Abstract:Ecological momentary assessment (EMA) minimizes recall burden and maximizes ecological validity and has emerged as a valuable tool to characterize individual differences, assess contextual associations, and document temporal associations. However, EMA has yet to be reliably utilized in young children, in part due to concerns about responder reliability and limited compliance. The present study addressed these concerns by building a developmentally appropriate EMA smartphone app and testing the app for feasibility and usability with young children ages 4–10 (N = 20; m age = 7.7, SD = 2.0). To pilot test the app, children completed an 11-item survey about their mood and behavior twice a day for 14 days. Parents also completed brief surveys twice a day to allow for parent-child comparisons of responses. Finally, at the end of the two weeks, parents provided user feedback on the smartphone app. Results indicated a high response rate (nearly 90%) across child surveys and high agreement between parents and children ranging from 0.89–0.97. Overall, findings suggest that this developmentally appropriate EMA smartphone app is a reliable and valid tool for collecting in-the-moment data from young children outside of a laboratory setting.

Abstract:Knowledge about people’s auditory reality (AR), i.e., the variety of listening demands and environments experienced in everyday life, is crucial when developing and evaluating hearing-aid function. We have previously investigated AR of older people with impaired hearing and generally seen an “easy” AR (many reports of passive listening and focused listening to TV, mainly in quiet). In the current study, we included participants with both normal and impaired hearing to investigate if participants with impaired hearing are avoiding difficult situations. The EMA study ran for a week with six daily prompts. In addition to the EMA survey items from our previous study, sound levels were recorded, and avoidance was investigated in an exit interview and by analyzing differences in AR between the groups. Preliminary analyses show that there were differences between the two groups in terms of where they were when they responded. The differences in listening task were small, but the participants with impaired hearing rated the reported situations to be more difficult than the normal-hearing participants. Our new results will be presented together with a discussion about the consequences of our auditory reality data on the way we evaluate hearing-aid function.

Abstract:Objective: Ecological momentary assessment (EMA) involves repeated collection of real-time self-report data, often multiple times per day, nearly always delivered electronically by smartphone. While EMA has shown promise for researching internal states, behaviors, and experiences in multiple populations, concerns remain regarding its feasibility in samples with cognitive impairments, like those associated with chronic moderate-to-severe traumatic brain injury (TBI). Methods: This study examines adherence to a 7-week high-frequency (5x daily) EMA protocol in individuals with moderate-to-severe TBI, considering changes in response rate over time, as well as individual participant characteristics (memory function, education, injury severity, and age). Results: In the sample of 39 participants, the average overall response rate was 65% (range: 5%–100%). Linear mixed-effects modeling revealed a small but statistically significant linear decay in response rate over 7 weeks of participation. Individual trajectories were variable, as evidenced by the significant effect of random slope. A better response rate was positively associated with greater educational attainment and better episodic memory function (statistical trend), whereas the effects of age and injury severity were not significant. Conclusions: These findings shed light on the potential of EMA in TBI studies but underscore the need for tailored strategies to address individual barriers to adherence.

Abstract:Purpose: Noise reduction technologies in hearing aids provide benefits under controlled conditions. However, differences in their real-life effectiveness are not established. We propose that a deep neural network (DNN)–based noise reduction system trained on naturalistic sound environments will provide different real-life benefits compared to traditional systems. Method: Real-life listening experiences collected with Ecological Momentary Assessments (EMAs) of participants who used two premium models of hearing aid are compared. One hearing aid model (HA1) used traditional noise reduction; the other hearing aid model (HA2) used DNN-based noise reduction. Participants reported listening experiences several times a day while ambient SPL, SNR, and hearing aid volume adjustments were recorded. Forty experienced hearing aid users completed a total of 3,614 EMAs and recorded 6,812 hr of sound data across two 14-day wear periods. Results: Linear mixed-effects analysis document that participants' assessments of ambient noisiness were positively associated with SPL and negatively associated with SNR but are not otherwise affected by hearing aid model. Likewise, mean satisfaction with the two models did not differ. However, individual satisfaction ratings for HA1 were dependent on ambient SNR, which was not the case for HA2. Conclusions: Hearing aids with DNN-based noise reduction resulted in consistent sound satisfaction regardless of the level of background noise compared to hearing aids implementing noise reduction based on traditional statistical models. While the two hearing aid models also differed on other parameters (e.g., shape), these differences are unlikely to explain the difference in how background noise impacts sound satisfaction with the aids. Abstract:Background: Ecological momentary assessment (EMA) is a measurement methodology that involves the repeated collection of real-time data on participants' behavior and experience in their natural environment. While EMA allows researchers to gain valuable insights into dynamic behavioral processes, the need for frequent self-reporting can be burdensome and disruptive. Compliance with EMA protocols is important for accurate, unbiased sampling; yet, there is no "gold standard" for EMA study design to promote compliance. Objective: The purpose of this study was to use a factorial design to identify optimal study design factors, or combinations of factors, for achieving the highest completion rates for smartphone-based EMAs. Methods: Participants recruited from across the United States were randomized to 1 of 2 levels on each of 5 design factors in a 2×2×2×2×2 design (32 conditions): factor 1-number of questions per EMA survey (15 vs 25); factor 2-number of EMAs per day (2 vs 4); factor 3-EMA prompting schedule (random vs fixed times); factor 4-payment type (US $1 paid per EMA vs payment based on the percentage of EMAs completed); and factor 5-EMA response scale type (ie, slider-type response scale vs Likert-type response scale; this is the only within-person factor; each participant was randomized to complete slider- or Likert-type questions for the first 14 days or second 14 days of the study period). All participants were asked to complete prompted EMAs for 28 days. The effect of each factor on EMA completion was examined, as well as the effects of factor interactions on EMA completion. Finally, relations between demographic and socioenvironmental factors and EMA completion were examined. Results: Participants (N=411) were aged 48.4 (SD 12.1) years; 75.7% (311/411) were female, 72.5% (298/411) were White, 18.0% (74/411) were Black or African American, 2.7% (11/411) were Asian, 1.5% (6/411) were American Indian or Alaska Native, 5.4% (22/411) belonged to more than one race, and 9.6% (38/396) were Hispanic/Latino. On average, participants completed 83.8% (28,948/34,552) of scheduled EMAs, and 96.6% (397/411) of participants completed the follow-up survey. Results indicated that there were no significant main effects of the design factors on compliance and no significant interactions. Analyses also indicated that older adults, those without a history of substance use problems, and those without current depression tended to complete more EMAs than their counterparts. No other demographic or socioenvironmental factors were related to EMA completion rates. Finally, the app was well liked (ie, system usability scale score=82.7), and there was a statistically significant positive association between liking the app and EMA compliance. Conclusions: Study results have broad implications for developing best practices guidelines for future studies that use EMA methodologies. Abstract:Background: Hearing loss is a major public health challenge. Audiology services need to utilise a range of rehabilitative services and maximise innovative practice afforded by technology to actively promote personalized, participatory, preventative and predictive care if they are to cope with the social and economic burden placed on the population by the rapidly rising prevalence of hearing loss. Digital interventions and teleaudiology could be a key part of providing high quality, cost-effective, patient-centred management. There is currently very limited evidence that assesses the hearing impaired patient perspective on the acceptance and usability of this type of technology. Aim: This study aims to identify patient perceptions of the use of a hearing support system including a mobile smartphone app when used with Bluetooth-connected hearing aids across the everyday life of users, as part of the EVOTION project. Methods: We applied a questionnaire to 564 participants in three countries across Europe and analysed the following topics: connectivity, hearing aid controls, instructional videos, audiological tests and auditory training. Key findings: Older users were just as satisfied as younger users when operating this type of technology. Technical problems such as Bluetooth connectivity need to be minimised as this issue is highly critical for user satisfaction, engagement and uptake. A system that promotes user-controllability of hearing aids that is more accessible and easier to use is highly valued. Participants are happy to utilise monitoring tests and auditory training on a mobile phone out of the clinic but in order to have value the test battery needs to be relevant and tailored to each user, easy to understand and use. Such functions can elicit a negative as well as positive experience for each user. Conclusion: Older and younger adults can utilise an eHealth mobile app to complement their rehabilitation and health care. If the technology works well, is tailored to the individual and in-depth personalised guidance and support is provided, it could assist maximisation of hearing aid uptake, promotion of self-management and improving outcomes.
Acceptability and feasibility of ecological momentary assessment with augmentation of passive sensor data in young adults at high risk for suicide

Abstract:Ecological Momentary Assessment (EMA) and wearable sensor data have the potential to enhance prediction of suicide risk in real-world conditions. However, the feasibility of this methodology with high-risk populations, including over extended periods, warrants closer attention. This study examined the feasibility and acceptability of concurrent EMA and wearable sensor monitoring in young adults after emergency department (ED) care for suicide risk-related concerns. For 2 months after ED discharge, 106 participants (ages 18-25; 81.1% female) took part in EMA surveys (4x per day) and passive sensor (Fitbit) monitoring and completed an end-of-study phone interview. Overall adherence to EMA (62.1%) and wearable sensor (53.6%) was moderate and comparable to briefer protocols. Relative to EMAs (81%), fewer participants completed the full 8 weeks of Fitbit (63%). While lower initial hopelessness was linked to reduced EMA adherence, previous-day suicidal ideation predicted lower Fitbit adherence on the next day. Self-endorsed barriers to EMA and wearable sensor adherence were also examined. Participants tended to report positive experience with the protocol, with majority indicating EMAs were minimally burdensome, reporting that the Fitbit was generally comfortable, and expressing interest in participating in a similar study again. Findings provide support for the feasibility and acceptability of concurrent intensive self-report and wearable sensor data during a high-risk period. Implications and future directions are discussed.

Abstract:Recently the use of mobile technologies in Ecological Momentary Assessments (EMA) and Interventions (EMI) has made it easier to collect data suitable for intra-individual variability studies in the medical field. Nevertheless, especially when self-reports are used during the data collection process, there are difficulties in balancing data quality and the burden placed on the subjects. In this paper, we address this problem for a specific EMA setting which aims to submit a demanding task to subjects at high/low values of a self-reported variable. We adopt a dynamic approach inspired by control chart methods and design optimization techniques to obtain an EMA triggering mechanism for data collection which takes into account both the individual variability of the self-reported variable and of the adherence rate. We test the algorithm in both a simulation setting and with real, large-scale data from a tinnitus longitudinal study. A Wilcoxon-Mann-Whitney Rank Sum Test shows that the algorithm tends to have both a higher F1 score and utility than a random schedule and a rule-based algorithm with static thresholds, which are the current state-of-the-art approaches. In conclusion, the algorithm is proven effective in balancing data quality and the burden placed on the participants, especially, as the analysis performed suggest, in studies where data collection is impacted by adherence.

Abstract:Early adolescence is a critical period for the development of executive function (EF). EF deficits are associated with increased engagement in multiple health risk behaviors and may be influenced by momentary factors, such as state mindfulness and physical activity. Ecological momentary assessment (EMA) leverages the increasing ubiquity of smart-phones to assess moment-to-moment changes in neurocognition and behavior with minimal recall bias and high ecological validity. As such, EMA is a promising method for delivering performance-based EF tasks and assessing the degree to which EF is influenced by momentary variation in its putative antecedents (e.g. state mindfulness and PA). This study adapts the (1) State Mindfulness Attention Awareness Scale, (2) physical activity/sedentary behavior recall items, (3) complex symmetry span working memory task, and (4) the child flanker inhibitory control task into an EMA tool delivered via iPhone using Inquisit Web. This tool was piloted with a sample of 32 seventh graders over a 72 h period. Participants' posttest survey responses indicated that they found these study activities to be enjoyable, non-burdensome, not overly difficult, and preferable to pencil-and-paper instruments. Baseline correlations between flanker task performance and both BRIEF inhibitory control (σ = .23) and working memory subscales (σ = .34) were moderate. Correlations between symmetry span working memory task performance and BRIEF inhibitory control (σabsolute = .28; σpartial = .16) and working memory subscales (σabsolute = .19; σpartial = .15) were slightly lower, demonstrating associations consistent with previous studies. This study supports the feasibility and acceptability of administering two common performance-based EF tasks to adolescents via an EMA approach.

Abstract:In photosynthetic organisms the accumulation of harmful photodynamic chlorophyll precursors is prevented because of the tight regulation of the tetrapyrrole pathway. FLU is one of the regulatory factors involved in this process in land plants. We have examined the function of a Flu-like gene (FLP) from Chlamydomonas that gives rise to two FLP transcripts through alternative splicing. These transcripts are translated into a short and a long protein that differ by only 12 amino acids but that interact differently with glutamyl-tRNA reductase, an enzyme involved in an early step of the chlorophyll biosynthetic pathway. Expression of FLPs is light-regulated at the level of RNA accumulation and splicing and is altered by mutations affecting the pathway. The relative levels of the long and short forms of FLP can be correlated with the accumulation of specific porphyrin intermediates, some of which have been implicated in a signaling chain from the chloroplast to the nucleus. Reciprocally, reduction of the FLP proteins by RNA interference leads to the accumulation of several porphyrin intermediates and to photobleaching when cells are transferred from the dark to the light. Thus the FLP proteins act as regulators of chlorophyll synthesis, and their expression is controlled by light and plastid signals.
Patterns of Missing Data with Ecological Momentary Assessment: Feasibility Report from a Pilot Study of People Who Use Drugs.

Abstract:BACKGROUND Ecological momentary assessment (EMA) is a set of research methods that capture events, feelings, and behaviors as they unfold in their "real world" setting. Capturing data "in the moment" reduces important sources of measurement error but also generates challenges for non-compliance (i.e., missing data). To date, past EMA research has only examined overall rates of non-compliance. OBJECTIVE We identify four types of non-compliance among people who use drugs (PWUD) and examine the factors associated with the most common types. METHODS Data are from a recent pilot study of 28 Nebraskan PWUD who answered EMA questions for two weeks. We examine questions that were not answered because they were skipped, they expired, the phone was off, or the phone died after receiving them. RESULTS We find that the phone being off and questions expiring comprise 93% of our missing data. Generalized structural equation model results show that participant-level factors, including age (RRR=0.93, P=.005), gender (RRR=0.08, P=.006), homelessness (RRR=3.80, P=.04), personal device ownership (RRR=0.14, P=.008), and network size (RRR=0.57, P=.001), are important for predicting off missingness, while only question-level factors, including time of day (i.e., morning compared to noon/afternoon, RRR=0.55, P<.001) and day of week (i.e., Tuesday-Saturday compared to Sunday, RRR=0.70, P=.02, RRR=0.64, P=.005, RRR=0.58, P=.001, RRR=0.55, P<.001, RRR=0.66, P=.008) are important for predicting expired missingness. Week of study is important for both (i.e., week two compared to week one, RRR=1.21, P=.03 for off missingness, RRR=1.98, P<.001 for expired missingness). CONCLUSIONS We suggest a three-pronged strategy to preempt missing EMA data with high-risk populations: 1) provide additional resources for participants likely to experience phone charging problems (e.g., people experiencing homelessness); 2) ask questions when participants are not likely to experience competing demands (e.g., morning); and 3) incentivize continued compliance as the study progresses. Attending to these issues can help researchers ensure maximal data quality. CLINICALTRIAL
