Abstract:The capillary electrophoretic behavior of 44 aromatic organic ions was investigated. The observed ionic radii ( r _obs0) for the aromatic organic ions were obtained from the electrophoretic mobilities of sodium tetraborate (pH 9.2), potassium tetraborate (pH 9.2), ammonium borate (pH 9.2), and trisodium phosphate (pH 11.7) buffers with zero ionic strength. The linear relationships between the r _obs0 values and the ionic radii ( r _calc), calculated by either the AM1 or PM3 method, were determined for benzyltrialkylammonium and aromatic sulfonate ions. However, the r _obs0 values were constant for the aromatic carboxylate ions in buffers, in spite of the different r _calc values. This indicates that aromatic carboxylate ions, such as benzenecarboxylate, pyridinecarboxylate, naphthalenecarboxylate, and anthracenecarboxylate ions, migrate as planar ions in buffers, whereas aromatic sulfonate ions could migrate as approximately spherical ions.

Effect of reciprocal headgear forces on the calvarium: A finite element study

Abstract:Introduction: Orthopedic appliances continue to be used for various sagittal, vertical, and transverse corrections of the maxilla. Although cranial growth can continue to adulthood, no study has drawn attention to the effects of headgear forces on the calvarium, in which anchorage is taken. This study focused on the different biomechanical responses on the calvarium of young children wearing a high-pull headgear of varying forces, using a 3-dimensional finite element analysis and the possible implications of such changes on the human skull. Methods: A 3-dimensional finite element model of a 9-year-old child was designed from the computed tomography scan. The material properties of the juvenile skull were assigned. Varying orthopedic forces (400, 500, and 600 g of force) were applied, and the magnitude of displacement and stresses generated on the cranial bones and sutures were interpreted using ANSYS software (version 12.1; Canonsburg, Pa). Results: Maximum displacement was found for the parietal bone in the inferior direction; parietal and temporal bones in the transverse direction; and frontal, parietal, and temporal bones in the sagittal direction. The least displacement was noted for the occipital bone in all the 3-dimensions. The maximum stresses were concentrated over the region of the lateral margins of the piriform aperture and the medial walls of the orbit. Higher stress values were also found on the parietal bone adjacent to the sagittal suture. The highest value of stresses among the sutures of the craniofacial complex was found to be in the region of the frontonasal, frontomaxillary, and nasomaxillary sutures. Conclusions: The effects of displacement and stresses obtained from this study suggest a restriction to the growth of the cranial vault and its normal mobility, in turn altering the intracranial tension and causing altered cranial morphology in young, growing children undergoing high-pull headgear therapy. The human cranial system is dynamic throughout life and may be restricted or altered by hereditary or environmental factors.

Abstract:Objective: To test the hypothesis that there are significant differences in skeletal and/or dental changes between Class II subjects treated with headgear (HG) compared with those treated with HG plus maxillary acrylic biteplate (BP) discluding teeth. Setting and sample population: Secondary analysis performed in Department of Orthodontics at the University of Washington. Fifty pre-adolescent Class II subjects were treated with HG as part of a randomized clinical trial (RCT) at the University of North Carolina/Chapel Hill, and 81 similar subjects were treated with HG plus a flatplane maxillary anterior BP for occlusal separation and anterior labial bow at the University of Florida as part of a separate RCT. Material and methods: This retrospective cohort study examined anteroposterior (AP) and vertical cephalometric changes in two cohorts of Class II subjects. Pre- and post-treatment cephalometric radiographs for each group were obtained from the two centers and measured for dental and skeletal changes. These data were adjusted for differences in magnification and compared using ancova, controlling for important cohort and protocol differences between the two centers. Results: Overbite and maxillary incisor inclinations were reduced significantly more in the HG/BP group. All other vertical and AP changes were not statistically significantly different between the groups. Conclusion: The maxillary anterior BP with labial bow is an effective appliance for reducing overbite and retracting incisors but provides no additional AP dental or skeletal benefit over HG treatment.
Headgear therapy in children with Class II malocclusion and the role of compliance on treatment outcome: A nine‐month randomized controlled trial

Abstract:Objectives To evaluate three‐dimensional movements of maxillary teeth during headgear treatment in Class II growing children, using digital analytical tools, and to determine the effects of compliance on these movements. Materials and Methods A 9‐month parallel‐group randomized controlled trial was carried out on 40 children with Class II malocclusion, aged 8‐12 years, half assigned to receive a cervical headgear and half to a no‐treatment group, using block randomization. Subjects in the treatment group were instructed to wear the headgear for 12 hours daily and monitored using an electronic module. After 9 months, the following dental outcomes were measured: first maxillary molar distalisation, rotation, tip and torque, arch depth, and interpremolar and intermolar distances. Caregivers and participants were not blinded to group assignments, but those assessing outcomes were. Linear regression models were used to detect differences between groups and correlation coefficients to find correlations between compliance and dental outcomes. Results All 40 included patients were analysed. A significant difference in molar distalisation was observed between the treatment (1.2 mm) and control groups (−0.2 mm). Arch depth change was also increased to a larger extent in the treatment groups (1.3 mm vs 0.1 mm), as was the interpremolar distance (1.9 mm vs 0.4 mm). In contrast, no significant differences in molar rotation or torque change were observed. With regard to compliance, average compliance was 55%. A significant correlation was found between molar distalisation and compliance in the treatment group. Conclusions Headgear therapy has significant effects on molar distalisation, arch depth, and arch width. Compliance has a significant positive effect on molar distalisation.

Initial and late treatment effects of headgear-Herbst appliance with mandibular step-by-step advancement

Abstract:The aim of this study was to investigate the effects of the headgear-Herbst appliance with mandibular step-by-step advancement followed by retention with the headgear activator. The sample comprised 22 consecutively treated patients (13.2 +/- 1.5 years old) and 31 matched controls (12.6 +/- 1.3 years old). Lateral cephalograms were obtained at the beginning of treatment, after 6 months (at completion of the initial phase), after 12 months of active treatment (at the end of the late phase), and after 18 months (at the end of retention). The results showed that the skeletal effects over 12 months of treatment were restrained maxillary growth, enhanced mandibular growth, and reduced increase in lower facial height. The restraint effect on the maxilla was significant in both the initial and late phases; the enhanced growth of the mandible was significant during the initial phase only; and the height of the lower face was affected during the late phase only. During retention with the headgear activator, the jaw base relationship was maintained, and the effect on the lower facial height was reinforced. A small relapse of the overjet and molar relationship was the result of dental changes only. During 18 months of active treatment and retention, the overjet correction was the result of 70% skeletal changes. The headgear-Herbst appliance is an effective orthopedic device, and the effect on the maxilla seems to increase with the length of treatment, but the pattern was reversed for the mandible.
Presynaptic Alpha-Synuclein Aggregation in a Mouse Model of Parkinson's Disease

Abstract:Parkinson's disease and dementia with Lewy bodies are associated with abnormal neuronal aggregation of α-synuclein. However, the mechanisms of aggregation and their relationship to disease are poorly understood. We developed an in vivo multiphoton imaging paradigm to study α-synuclein aggregation in mouse cortex with subcellular resolution. We used a green fluorescent protein-tagged human α-synuclein mouse line that has moderate overexpression levels mimicking human disease. Fluorescence recovery after photobleaching (FRAP) of labeled protein demonstrated that somatic α-synuclein existed primarily in an unbound, soluble pool. In contrast, α-synuclein in presynaptic terminals was in at least three different pools: (1) as unbound, soluble protein; (2) bound to presynaptic vesicles; and (3) as microaggregates. Serial imaging of microaggregates over 1 week demonstrated a heterogeneous population with differing α-synuclein exchange rates. The microaggregate species were resistant to proteinase K, phosphorylated at serine-129, oxidized, and associated with a decrease in the presynaptic vesicle protein synapsin and glutamate immunogold labeling. Multiphoton FRAP provided the specific binding constants for α-synuclein's binding to synaptic vesicles and its effective diffusion coefficient in the soma and axon, setting the stage for future studies targeting synuclein modifications and their effects. Our in vivo results suggest that, under moderate overexpression conditions, α-synuclein aggregates are selectively found in presynaptic terminals.

The effects of maxillary protraction and its long-term stability--a clinical trial in Chinese adolescents

Abstract:The purpose of this study was to evaluate the effect of reverse pull headgear (RPHG) in the treatment of Class III malocclusions in the late mixed and early permanent dentition and its long-term stability at the time when facial growth was close to completion. The treatment group comprised 22 subjects (12 males and 10 females, mean age: 11.38 ± 0.69 years). The control group included 17 subjects (7 males and 10 females, mean age: 11.54 ± 1.07 years). The mean RPHG treatment time was 1.5 ± 0.95 years and the observation time for the control group was 1.75 ± 0.83 years. For each subject, lateral cephalograms were obtained before (T1) and after (T2) treatment or observation. These cephalograms were traced and analyzed and the differences between T1 and T2 values were examined with paired t-tests. Of the 22 treated cases, 10 patients were followed up until at the mean age of 16.18 years (T3). Since there was no relapse in anterior crossbite, the long-term effects of RPHG were evaluated by measuring the maxillary and mandibular skeletal changes. The follow-up patients were divided into two groups based on the change in ANB: a stable group (decrease in ANB < 2 degrees) and an unstable group (decrease in ANB > 2 degrees). The skeletal effects of RPHG in treating Class III anomalies just before or at the beginning of the pubertal growth spurt include protraction of the maxilla and dentition and inhibition of forward growth of the mandible. With regard to the long-term change, a slight alteration in the position of the maxilla and in the position and growth direction of the mandible resulted in a slight decrease in ANB in the stable group. The slight retrusion in the maxilla, combined with the significant protrusion in the mandible and the more horizontal mandibular growth direction, resulted in a decrease in ANB in the unstable group. This indicated that the maxilla remained relatively stable and that the unstable factor was continuing mandibular growth during the pubertal and post-pubertal period. For patients with an excessive mandible, orthopaedic therapy should start at the beginning of pubertal growth and orthodontic fixed appliance should follow immediately after RPHG so that mandibular growth in the sagittal direction during puberty or even after pubertal growth may be effectively inhibited.
Treatment Effects of Simple Fixed Appliance and Reverse Headgear in Correction of Anterior Crossbites

Abstract:The aim of this study was to compare the skeletal and dental changes contributing to the correction of Class III malocclusion using a 2 x 4 appliance and reverse headgear in the mixed dentition. Seventeen consecutive patients (mean age, 9.7 years) with pseudo-Class III malocclusions and an anterior functional shift and straight or concave facial profile were treated with a simple fixed appliance. Another 20 consecutive patients (mean age, 8.5 years) with Class III incisor relationship and straight or concave facial profiles, were treated with reverse headgear. Lateral cephalometric films taken at the beginning of treatment, the end of the treatment, and 1 year after the active treatment, were analyzed with the modified Pancherz analysis. After active treatment, the overjet correction, 5.2 mm and 6.5 mm on average, respectively, were achieved using the 2 x 4 and reverse headgear. The overjet correction by the 2 x 4 appliance was due to dental changes only. In the reverse headgear group, 60% of the overjet correction was due to dental changes and 40% due to skeletal changes. During the 12 months follow-up period, the overjet was unchanged in the 2 x 4 group (1.6 mm) and decreased in the reverse headgear group, the difference being statistically significant (P < .05). The change of jaw relationship was similar between the 2 x 4 and reverse headgear groups. During the follow-up period, a decrease in overjet in the reverse headgear group was mainly due to forward growth of the mandible and proclination of lower incisors. The overjet in the 2 x 4 group was unchanged due to dental compensation (1.6 mm). To conclude there was a similar amount of overjet correction in the 2 x 4 and reverse headgear groups. Overjet correction by the simple fixed appliance was produced by dental changes whereas in the reverse headgear group, it was produced by both dental and skeletal changes.
Comparison of treatment outcomes between skeletal anchorage and extraoral anchorage in adults with maxillary dentoalveolar protrusion

Abstract:Introduction: The goal of this retrospective cephalometric study was to compare orthodontic outcomes in patients with maxillary dentoalveolar protrusion malocclusion treated with extraoral headgear or mini-implants for maximum anchorage. Materials: Forty-seven subjects with Angle Class II malocclusion or Class I bimaxillary dentoalveolar protrusion were treated by retracting the maxillary dentoalveolar process by using the extraction space of the bilateral maxillary first premolars. Two anchorage systems were used. Group 1 (n = 22) received traditional anchorage preparation with a transpalatal arch and headgear; group 2 (n = 25) received mini-implants (miniplates, miniscrews, or microscrews) for bony anchorage. Pretreatment and posttreatment lateral cephalograms were superimposed to compare the following parameters between groups: (1) amount of maxillary central incisor retraction, (2) reduction in maxillary central incisor angulation, (3) anchorage loss of the maxillary first molar, (4) movements of the maxillary central incisor and first molar in the vertical direction, and (5) changes in skeletal measurements representing the anteroposterior and vertical jaw relationships. Results: The skeletal anchorage group had greater anterior tooth retraction (8.17 vs 6.73 mm) and less maxillary molar mesialization (0.88 vs 2.07 mm) than did the headgear group, with a shorter treatment duration (29.81 vs 32.29 months). Translational movement of the incisors was more common than tipping movement, and intrusion of the maxillary dentition was greater, in patients receiving miniplates than in those receiving screw-type bony anchorage, resulting in counterclockwise rotation of the mandible and a statistically significant decrease in the mandibular plane angle. Cephalometric analysis of skeletal measurements in patients with low to average mandibular plane angles showed no significant difference between groups, although greater maxillary incisor retraction and less mesial movement of the first molar were noted in the mini-implant group. In patients with a high mandibular plane angle, those receiving skeletal anchorage had genuine intrusion of the maxillary first molar and reduction in the mandibular plane angle, whereas those receiving headgear anchorage had extrusion of the maxillary first molar and an increase of mandibular plane angle. In contrast to the posterior movement in the headgear group, anterior movement of Point A was noted in the mini-implant group. Conclusions: In both the anteroposterior and vertical directions, skeletal anchorage achieved better control than did the traditional headgear appliance during the treatment of maxillary dentoalveolar protrusion. Greater retraction of the maxillary incisor, less anchorage loss of the maxillary first molar, and the possibility of counterclockwise mandibular rotation all facilitated the correction of the Class II malocclusion.

Abstract:SAR polarimetry has been expected to be a potential candidate of a tool to measure physical parameters such as biomass and soil moisture. However, it still has some difficulties to even simply relate the parameters to a radar backscattering cross section. In this paper, a multi-incidence angle approach is applied to understand the radar backscatter from the vegetated field. We theoretically developed a boreal forest model based on Discrete scatterer model, and utilized it to simulate a backscatter in terms of incidence angles in some cases. Analyzing the simulation results, we revealed that a trunk in the boreal forest works as a radio wave absorber. In addition, increase and decrease of the backscatter is strongly influenced by the dominant scattering mechanism as expected. This theoretical study will be validated by real data which will be obtained by real airborne and spaceborne SAR sensors.
Dental arch effects after early and later timed cervical headgear treatment-a randomized controlled trial

Abstract:Background: Cervical headgear (CH) is a commonly used orthodontic appliance and its dentoalveolar changes are known. However, the effects related to gender and timing have gained less attention. Objectives: To examine dimensions of dental arches among children with Class II occlusion without posterior mandibular rotation according to timing of Kloehn-type CH treatment. Trial design: Prospective, randomized, parallel-group controlled trial. Methods: Sixty-seven seven-year-old children with a Class II occlusion were included in the study. The children were randomized into two equal-size groups in 1:1 ratio by sealed-envelope randomization. The early group (EG, n = 33) was treated between T0 and T1 (26 months), right after eruption of the first maxillary molars. The late group (LG, n = 34) was treated between T1 and T2 (24 months). The children were treated with CH until normal Class I occlusion on first molars was achieved. Impressions for dental casts were taken from all participants at T0, T1, and T2. Blinding was applicable for outcome assessors. Changes in dental cast measurements were compared between the groups and genders using t-test, Mann-Whitney U-test, and repeated measures analysis of variance. Results: Of the children, 56 completed the study. The maxillary arch length and the transversal changes between the upper canines and upper first molars were significantly increased in EG at T0-T1 (P < 0.001). At T2, the transversal dimension between the upper first molars was larger (P < 0.05), and in the lower arch the mandibular arch length (P < 0.05) and the transversal dimension between the lower first molars (P < 0.01) were increased in EG males compared to LG males. No harms were encountered. Conclusions: The male gender benefits most from early timing of the CH treatment, showing larger dimensions at the end of the follow-up. The results clearly indicated a wider and longer upper dental arch and spontaneous expansion of the lower dental arch after treatment. Clinical registration: NCT02010346.
Effects of headgear Herbst and mandibular step-by-step advancement versus conventional Herbst appliance and maximal jumping of the mandible

Abstract:The aims of this study were to compare dental and skeletal treatment changes in Class II division 1 malocclusions with two modes of maxillary control and two modes of bite-jumping. The subjects comprised Chinese children with severe Class II division 1 malocclusions, i.e. 21 consecutive subjects (13.4 +/- 1.4 years) treated with a headgear Herbst appliance and step-by-step advancement (HHSSA) of the mandible, and 24 consecutive subjects (13.2 +/- 1.4 years) treated with a 'conventional' Herbst appliance with maximal jumping (HMJ) of the mandible. Lateral cephalograms obtained at the start and end of treatment were analysed. The results showed that the improvement of the sagittal jaw relationship was significantly larger (2.9 mm; P < 0.001) in the HHSSA group than in the HMJ group due to the increased effect on the maxilla (-1.5 mm, P < 0.001) and the mandible (+1.4 mm, NS). There was no significant difference in the change in lower anterior face height, being 2.7 and 3.1 mm, respectively. The mandibular plane angle decreased significantly in the HHSSA group (-0.7 degree; P < 0.05) and increased insignificantly in the HMJ group (0.4 degree, NS), the difference being statistically significant (P < 0.01). The maxillary molars moved significantly more distally (1.1 mm, P < 0.05) and were intruded in the HHSSA group (-1.0 mm, P < 0.001) compared with a small extrusion in the HMJ group (+0.3 mm, NS), the difference being statistically significant (P < 0.001). There was no significant difference in the effect on the mandibular teeth. Treatment with HHSSA seems to result in a greater effect on the sagittal jaw relationship, improved vertical control and more maxillary molar movement. Mandibular anchorage loss was not reduced with step-by-step advancement of the mandible.
The effect of early intervention on skeletal pattern in Class II malocclusion: A randomized clinical trial

Abstract:Early treatment for Class II malocclusion is frequently undertaken with the objective of correcting skeletal disproportion by altering the growth pattern. Because the majority of previous studies of growth modification for Class II malocclusion have been based on retrospective record reviews, the efficacy of such an approach has not been well established. In this controlled clinical trial, patients in the mixed dentition with overjet > or = 7 mm were randomly assigned to either early treatment with headgear, or modified bionator, or to observation. All patients were observed for 15 months with no other appliances used during this phase of the trial. The three groups, who were equivalent initially, experienced statistically significant differences (p < 0.01) in skeletal change. There was considerable variation in the pattern of change within all three groups, with about 80% of the treated children responding favorably. Although patients in both early treatment groups had approximately the same reduction in Class II severity, as reflected by change in the ANB angle, the mechanism of this change was different. The headgear group showed restricted forward movement of the maxilla, and the functional appliance group showed a greater increase in mandibular length. The permanence of these skeletal changes and their impact on the subsequent treatment remains to be evaluated.

Abstract:Anterior cutaneous nerve entrapment syndrome (ACNES) is one the most common cause of chronic abdominal wall pain. The syndrome is mostly misdiagnosed, treated wrongly and inadequately. If diagnosed correctly by history, examination and a positive carnett test, the suffering of the patient can be relieved by addressing the cause i.e. local anaesthetic with steroid injection at the entrapment site. Conventionally, the injection is done by landmark technique. In this report, we have described 2 patients who were diagnosed with ACNES who were offered ultrasound guided transverses abdominis plane (TAP) injection who got significant pain relief for a long duration of time.
