Abstract:Objective: Markerless patient registration is a new procedure that may reduce logistical efforts and possibly also the radiation load on the patients prior to a computer-assisted intervention. Congruent surfaces, such as bone surfaces or skin surfaces, represented in a data set and in the surgical site, can be overlapped using surface-matching. Previous studies describing this kind of markerless registration, however, show inaccuracies of up to 10 mm during computer-assisted navigation. Furthermore, these systems use less than 1000 surface points of the soft tissue surface in order to establish a correlation between the patient and the data set. Previous papers did not answer the question whether it is this scanning resolution that induces these inaccuracies in registration or rather intraoperative skin deformations. Study design: In the present study therefore a new navigation system (SSN++) was used which is able to register up to 180,000 surface points of the surgical site. SSN++ is an infrared navigation system enlarged by a Minolta VI 900 3D volume digitizer. Three different kinds of laser scan-resolution were used for data correlation. An additional congruence analysis was performed in order to assess the geometry of the matched skin surfaces. 22 patients suffering from different cranial diseases (tumors, bony malformations, foreign bodies) were prepared for a computer-assisted intervention. Intraoral titanium-markers, rigidly fixed on the patients by a maxillary splint, were placed as targets while the CT data sets were made. These targets were - after markerless laser scan registration of the patients - supposed to serve for validating the new high-resolution navigation system SSN++. Results: The accuracy of markerless laser scan registration depends on the intraoperative laser scan's resolution. A high accuracy of the data correlation can be achieved if the number of the laser scan cloud points is about the same as the number of voxels of the corresponding surface on the CT data set. A reduction of the laser scan cloud points to less than 10 % compared to the number of voxels of the CT surface, however, leads to a significant loss of accuracy after markerless patient registration. Conclusion: The markerless laser scan registration of the surgical site may achieve the same accuracy as a patient registration made by rigidly fixed titanium screws (mean accuracy: 1.2 mm) as long as a high-resolution laser scan is being used.

Abstract:PURPOSE: Despite the high clinical accuracy of dynamic navigation, inherent sources of error exist. The purpose of this study was to improve the accuracy of dynamic navigated surgical procedures in the edentulous maxilla by identifying the optimal configuration of intra-oral points that results in the lowest possible registration error for direct clinical implementation.MATERIALS AND METHODS: Six different 4-area configurations were tested by 3 operators against positive and negative controls (8-areas and 3-areas, respectively) using a skull model. The two dynamic navigation systems (X-Guide® and NaviDent®) and the two registration methods (bone surface tracing and fiducial markers) produced four registration groups. The accuracy of the registration was checked at the frontal process of the zygoma. Intra- and inter- operator reliability for each registration group were reported. Multiple comparisons were conducted to find the best configuration with the minimum registration error.RESULTS: Ranking revealed one configuration in the tracing groups (Conf.3) and two configurations in the fiducial groups (Conf.3 and Conf.5) that had the best accuracy. When the inferior surfaces of the zygomatic buttress were excluded, fiducial registration produced better accuracy with both systems (p 0.006 and <0.0001). However, tracing 1 cm areas at these surfaces bilaterally resulted in similar registration accuracy as placing fiducial markers there (p 0.430 and 0.237). NaviDent® performed generally better (p 0.049, 0.001 and 0.002) albeit having a wider margin of uncertainty in the obtained values. Changing the distribution of the 4 tracing areas or fiducial markers had a less pronounced effect with X-Guide® than with the NaviDent® system.CONCLUSION: For edentulous maxillary surgeries, 4 fiducial markers placed according to configuration 3 or 5 result in the lowest registration error. Where implants are being placed bilaterally, an additional 2 sites may reduce the error further. For bilateral zygomatic implant placement, it is optimal to place 2 fiducials on the inferior surfaces of the maxillary tuberosities, other 2 on their buccal surfaces, and 2 on the anterior labial surface of the alveolar bone. Utilising the inferior zygomatic buttress is recommended over the inferior maxillary tuberosities in other types of maxillary surgeries.

Intraoral Scanning Enables Virtual-Splint-Based Non-Invasive Registration Protocol for Maxillofacial Surgical Navigation

Abstract:Background/Objectives: Surgical navigation has advanced maxillofacial surgery since the 1990s, bringing benefits for various indications. Traditional registration methods use fiducial markers that are either invasively bone-anchored or attached to a dental vacuum splint and offer high accuracy but necessitate additional imaging with increased radiation exposure. We propose a novel, non-invasive registration protocol using a CAD/CAM dental splint based on high-resolution intraoral scans. Methods: The effectiveness of this method was experimentally evaluated with an ex vivo 3D-printed skull measuring the target registration error (TRE). Surgical application is demonstrated in two clinical cases. Results: In the ex vivo model, the new CAD/CAM-splint-based method achieved a mean TRE across the whole facial skull of 0.97 ± 0.29 mm, which was comparable to traditional techniques like using bone-anchored screws (1.02 ± 0.23 mm) and dental vacuum splints (1.01 ± 0.33 mm), while dental anatomical landmarks showed a lower accuracy with a mean TRE of 1.84 ± 0.44 mm. Multifactorial ANOVA confirmed significant differences in TRE based on the registration method and the navigated level of the facial skull (p < 0.001). In clinical applications, the presented method demonstrated high accuracy for both midfacial and mandibular surgeries. Conclusions: Our results suggest that this non-invasive CAD/CAM-splint-based method is a viable alternative to traditional fiducial marker techniques, with the potential for broad application in maxillofacial surgery. This approach retains high accuracy while eliminating the need for supplementary imaging and reduces patient radiation exposure. Further clinical trials are necessary to confirm these findings and optimize splint design for enhanced navigational accuracy.

Abstract:ABSTRACT The present study tested in vitro susceptibility of Candida bloodstream isolates to fluconazole to determine if the ratio of the fluconazole area under the concentration-time curve (AUC) or weight-normalized daily dose (dosewn) to MIC correlated with mortality. Fluconazole susceptibility and outcome data were determined for 77 patients with a positive Candida blood culture between 2002 and 2005. The most commonly isolated Candida species were C. albicans (64%), C. glabrata (14%), C. parapsilosis (8%), C. tropicalis (6%), and C. lusitaniae (4%). Only two isolates were classified as fluconazole resistant by the CLSI M27-A2 method. Fluconazole MICs were highest against C. glabrata relative to other Candida species. Overall the crude mortality assessed at hospital discharge was 19.4% (n = 15). Mortality rates by species were as follows: C. albicans, 16.3%; C. glabrata, 36.4%; C. parapsilosis, 0%; C. tropicalis, 0%; C. lusitaniae, 33.3%. A mortality rate of 50% was noted among patients infected with nonsusceptible isolates (MIC ≥ 16 μg/ml) compared to 18% for patients infected with susceptible (MIC ≤ 8 μg/ml) isolates (P = 0.17). The fluconazole dosewn/MIC (24-h) values were significantly higher for the 62 survivors (13.3 ± 10.5 [mean ± standard deviation]) compared to the 15 nonsurvivors (7.0 ± 8.0) (P = 0.03). The fluconazole AUC/MIC (24 h) values also trended higher for survivors (775 ± 739) compared to nonsurvivors (589 ± 715) (P = 0.09). These data support the dose-dependent properties of fluconazole. Underdosing fluconazole against less-susceptible Candida isolates has the potential to increase the risk of mortality associated with candidemia.
A Robust Automated Markerless Registration Framework for Neurosurgery Navigation.

Abstract:Background The registration of a pre-operative image with the intra-operative patient is a crucial aspect for the success of navigation in neurosurgery.Methods First, the intra-operative face is reconstructed, using a structured light technique, while the pre-operative face is segmented from head CT/MRI images. In order to perform neurosurgery navigation, a markerless surface registration method is designed by aligning the intra-operative face to the pre-operative face. We propose an efficient and robust registration approach based on the scale invariant feature transform (SIFT), and compare it with iterative closest point (ICP) and coherent point drift (CPD) through a new evaluation standard.Results Our registration method was validated by studies of 10 volunteers and one synthetic model. The average symmetrical surface distances (ASDs) for ICP, CPD and our registration method were 2.24 +/- 0.53, 2.18 +/- 0.41 and 2.30 +/- 0.69 mm, respectively. The average running times of ICP, CPD and our registration method were 343.46, 3847.56 and 0.58 s, respectively.Conclusion Our system can quickly reconstruct the intra-operative face, and then efficiently and accurately align it to the pre-operative image, meeting the registration requirements in neurosurgery navigation. The accuracy and learning curve of active and passive dynamic navigation-guided dental implant surgery: An in vitro study

Abstract:Objectives: Infrared dynamic navigation systems can be categorized into active and passive based on whether the surgical instruments can emit or only reflect light. This in vitro study aimed to compare the accuracy of implant placement and the learning curve of both active and passive dynamic navigation systems, using different registration methods.Methods: Implants (n = 704) were placed in 64 sets of models and divided into active (Yizhime, DCARER, Suzhou, China) and passive (Iris-Clinic, EPED, Kaohsiung, China) dynamic navigation groups. Both marker point-based registration (M-PBR) and feature point-based registration (F-PBR) were employed for the two groups. Based on preoperative and postoperative cone-beam computed tomography imaging, the coronal, midpoint, apical, and angular deviations were analyzed from 2D and 3D views. The operation time was recorded for each group.Results: The active dynamic navigation group exhibited significantly higher accuracy than the passive dynamic navigation group (angular deviation, 4.13 ± 2.39° versus 4.62 ± 3.32°; coronal global deviation, 1.48 ± 0.60 versus 1.86 ± 1.12 mm; apical global deviation, 1.75 ± 0.81 versus 2.20 ± 1.68 mm, respectively). Significant interaction effects were observed for both registration methods and four quadrants with different dynamic navigation systems. Learning curves for the two dynamic navigation groups approached each other after 12 procedures, and finally converged after 27 procedures.Conclusions: The accuracy of active dynamic navigation system was superior to that of passive dynamic navigation system. Different combinations of dynamic navigation systems, registration methods, and implanted quadrants displayed various interactions.Clinical significance: Our findings could provide guidance for surgeons in choosing an appropriate navigation system in various implant surgeries. Furthermore, the time required by surgeons to master the technique was calculated. Nevertheless, there are certain limitations in this in vitro study, and therefore further research is required.

Abstract:Objective: The precision of neuronavigation systems relies on the correct registration of the patient's position in space and aligning it with radiological 3D imaging data. Registration is usually performed by the acquisition of anatomical landmarks or surface matching based on facial features. Another possibility is automatic image registration using intraoperative imaging. This could provide better accuracy, especially in rotated or prone positions where the other methods may be difficult to perform. The aim of this study was to validate automatic image registration (AIR) using intraoperative cone-beam computed tomography (CBCT) for cranial neurosurgical procedures and compare the registration accuracy to the traditional surface matching (SM) registration method based on preoperative MRI. The preservation of navigation accuracy throughout the surgery was also investigated. Methods: Adult patients undergoing intracranial tumor surgery were enrolled after consent. A standard SM registration was performed, and reference points were acquired. An AIR was then performed, and the same reference points were acquired again. Accuracy was calculated based on the referenced and acquired coordinates of the points for each registration method. The reference points were acquired before and after draping and at the end of the procedure to assess the persistency of accuracy. Results: In total, 22 patients were included. The mean accuracy was 6.6 ± 3.1 mm for SM registration and 1.0 ± 0.3 mm for AIR. The AIR was superior to the SM registration (p < 0.0001), with a mean improvement in accuracy of 5.58 mm (3.71–7.44 mm 99% CI). The mean accuracy for the AIR registration pre-drape was 1.0 ± 0.3 mm. The corresponding accuracies post-drape and post-resection were 2.9 ± 4.6 mm and 4.1 ± 4.9 mm, respectively. Although a loss of accuracy was identified between the preoperative and end-of-procedure measurements, there was no statistically significant decline during surgery. Conclusions: AIR for cranial neuronavigation consistently delivered greater accuracy than SM and should be considered the new gold standard for patient registration in cranial neuronavigation. If intraoperative imaging is a limited resource, AIR should be prioritized in rotated or prone position procedures, where the benefits are the greatest.

Validation of automated registration of intraoral scan onto Cone Beam Computed Tomography for an efficient digital dental workflow

Abstract:Objective: This study aimed to validate a newly developed automated method (Virtual Patient Creator, Relu, Leuven, Belgium) for multimodal registration of intraoral scan (IOS) and Cone Beam Computed Tomography (CBCT). Methods: Time point-matched IOS and CBCT scans of forty patients with variable dental statuses (natural dentition, partial edentulism, presence of orthodontic brackets) were selected. Three operators registered IOS and CBCT scans using three state-of-the-art softwares for orthodontics and orthognathic surgery (IPS Case Designer, Proplan CMF and Dolphin Imaging). Automated registration was compared to expert-performed semi-automated registration. Time consumption, accuracy, and consistency of the proposed method were benchmarked to semi-automated registration using root mean squared error calculations. The robustness of the automated registration was evaluated in relationship to the dental status of the patients in the dataset. Results: On average, automatic registration was 7.3 times faster than semi-automatic registration performed by an expert operator. Automatic registration yielded reliable results with low deviation errors compared to the differently skilled operators and semi-automated software. Automated registration surpassed human variability as expressed in intra- and inter-operator inconsistencies. Neither orthodontic brackets nor edentulism impacted registration accuracy. Conclusions: The presented automated method for IOS and CBCT registration is faster, equally accurate, and more consistent than semi-automatic registration performed by an expert or an occasional operator. With similar results among cases with different dental statuses, the clinical feasibility of the method is ensured. Clinical significance: A validated automated registration method provides accurate and fast multimodal image integration without incorporating operator bias at the very start of the digital workflows for dentistry, periodontics, orthodontics and orthognathic surgery.

Abstract:The deviations between the preoperative (planned) and postoperative (actual) positions of dental implants have always been of a major concern in dental implant surgery. Dynamic computer-aided implant surgery (dCAIS) systems have been used to achieve optimal implant positioning. The method of registration is indeed an important factor that affects the implanting accuracy. Here, we propose a fast and concise registration method using a registration-and-fixation device as well as an adjustable handpiece for dynamic navigation in dental implant surgery. To the best of our knowledge, our work is the first study of such a registration method for dynamic navigation in a dental implant system.
Registration Sanity Check for AR-guided Surgical Interventions: Experience From Head and Face Surgery

Abstract:Achieving and maintaining proper image registration accuracy is an open challenge of image-guided surgery. This work explores and assesses the efficacy of a registration sanity check method for augmented reality-guided navigation (AR-RSC), based on the visual inspection of virtual 3D models of landmarks. We analyze the AR-RSC sensitivity and specificity by recruiting 36 subjects to assess the registration accuracy of a set of 114 AR images generated from camera images acquired during an AR-guided orthognathic intervention. Translational or rotational errors of known magnitude up to ±1.5 mm/±15.5°, were artificially added to the image set in order to simulate different registration errors. This study analyses the performance of AR-RSC when varying (1) the virtual models selected for misalignment evaluation (e. g., the model of brackets, incisor teeth, and gingival margins in our experiment), (2) the type (translation/rotation) of registration error, and (3) the level of user experience in using AR technologies. Results show that: 1) the sensitivity and specificity of the AR-RSC depends on the virtual models (globally, a median true positive rate of up to 79.2% was reached with brackets, and a median true negative rate of up to 64.3% with incisor teeth), 2) there are error components that are more difficult to identify visually, 3) the level of user experience does not affect the method. In conclusion, the proposed AR-RSC, tested also in the operating room, could represent an efficient method to monitor and optimize the registration accuracy during the intervention, but special attention should be paid to the selection of the AR data chosen for the visual inspection of the registration accuracy.

Abstract:Background: This prospective clinical study aimed to investigate a possible deviation between the digitally planned implant position and the position achieved using dynamic navigation. The aim of the study was to establish clinical effectiveness and precision of implantation using dynamic navigation. Methods: Twenty consecutive patients received an implant (iSy-Implantat, Camlog, Wimsheim, Germany). One screw implant was placed in one jaw with remaining dentition of at least six teeth. The workflow was fully digital. Digital implant planning was conducted using cone-beam computed tomography (CBCT) and an intraoral scan of the actual condition. Twenty implants were subsequently placed using a dynamic computer-assisted procedure. The clinical situation of the implant position was recorded using an intraoral scan. Using these data, models were produced via 3D printing, and CBCTs of these models were made using laboratory analogs. Deviations of the achieved implant position from the planned position were determined using evaluation software. Results: The evaluation of 20 implants resulted in a mean angle deviation of 2.7° (95% CI 2.2–3.3°). The 3D deviation at the implant shoulder was 1.83 mm (95% CI 1.34–2.33 mm). No significant differences were found for any of the parameters between the implantation in the upper or lower jaw and an open or flapless procedure (p-value < 0.05). Conclusion: The clinical trial showed that sufficiently precise implantation was possible with the dynamic navigation system used here. Dynamic navigation can improve the quality of implant positioning. In particular, the procedure allows safe positioning of the implants in minimally invasive procedures, which usually cannot be performed freehand in this form. A clinical benefit and effectiveness can be determined from the results.

Abstract:Current-transformer (CT) saturation causes severe distortion in the measured current waveform which may lead to maloperation of the protective devices. This paper proposes a low-cost, power-electronic device to prevent the CT from saturation. The proposed compensator is inserted in series with the relay in the CT secondary circuit and acts as a controlled voltage source (CVS). The proposed CVS generates a time-varying voltage to cancel the voltage developed across the CT burden; therefore, the CT magnetic flux remains almost constant and undistorted during the power system transients. It will be shown that this device can precisely compensate fault current, inrush current, and other probable transient currents despite its simplicity. The proposed device can be employed to compensate the already in-service CTs connected to nondigital and digital relays. Comprehensive computer simulations are carried out to validate the effectiveness of the proposed compensator. The performance of a sample compensator of this type is validated through special high-current laboratory experiments (carried out over several hundred amperes up to 1.6 kA), and the obtained results illustrate the capability of the proposed compensator to prevent CT saturation.

Abstract:Journal of Mechanics in Medicine and Biology, Ahead of Print. In oral and maxillofacial surgery, computer-assisted navigation technologies have been widely used to achieve intraoperative positioning. The traditional methods mainly rely on the experience of doctors and the difference between the locations of key points in the surgical area and the preoperative planning, which have certain limitations. In this paper, a new intraoperative surgical navigation framework based on mobile laser scanner is proposed, which ensures that surgery is performed accurately according to the preoperative planning. The framework mainly includes two parts. First, the real-time surface reconstruction of the anatomy should be realized during the operation. Second, the acquired image is matched to the planned image in real time. Although the most common method of surface reconstruction is to render the volume directly from raw data or render the surface from the segmented data using computed tomography/magnetic resonance (CT/MR) data, this method is too complicated for performing the real-time operation during surgery. Furthermore, a new surface registration technique is proposed for image-guided oral and maxillofacial surgery based on the point sets. To improve the registration accuracy and robustness, the point sets are modeled by Mixed Student's t-Distribution model. In the experiments, the point sets of CT data are from 10 patients with craniomaxillofacial diseases and the surface point set is from the LRS. The TRE of 10 data was less than 1[math]mm. Compared with the paired-point registration method and Iterative Closest Point algorithm, the results demonstrated better performance of the proposed method, the surgical situation can be displayed in real time during the surgical process, and any differences from the surgical plan can also be reflected.

Fully automated 2D-3D registration and verification

Abstract:Clinical application of 2D-3D registration technology often requires a significant amount of human interaction during initialisation and result verification. This is one of the main barriers to more widespread clinical use of this technology. We propose novel techniques for automated initial pose estimation of the 3D data and verification of the registration result, and show how these techniques can be combined to enable fully automated 2D-3D registration, particularly in the case of a vertebra based system. The initialisation method is based on preoperative computation of 2D templates over a wide range of 3D poses. These templates are used to apply the Generalised Hough Transform to the intraoperative 2D image and the sought 3D pose is selected with the combined use of the generated accumulator arrays and a Gradient Difference Similarity Measure. On the verification side, two algorithms are proposed: one using normalised features based on the similarity value and the other based on the pose agreement between multiple vertebra based registrations. The proposed methods are employed here for CT to fluoroscopy registration and are trained and tested with data from 31 clinical procedures with 417 low dose, i.e. low quality, high noise interventional fluoroscopy images. When similarity value based verification is used, the fully automated system achieves a 95.73% correct registration rate, whereas a no registration result is produced for the remaining 4.27% of cases (i.e. incorrect registration rate is 0%). The system also automatically detects input images outside its operating range.
Deformable 3D/3D CT-to-digital-tomosynthesis image registration in image-guided bronchoscopy interventions

Abstract:Traditional navigational bronchoscopy procedures rely on preprocedural computed tomography (CT) and intraoperative chest radiography and cone-beam CT (CBCT) to biopsy peripheral lung lesions. This navigational approach is challenging due to the projective nature of radiography, and the high radiation dose, long imaging time, and large footprints of CBCT. Digital tomosynthesis (DTS) is considered an attractive alternative combining the advantages of radiography and CBCT. Only the depth resolution cannot match a full CBCT image due to the limited angle acquisition. To address this issue, preoperative CT is a good auxiliary in guiding bronchoscopy interventions. Nevertheless, CT-to-body divergence caused by anatomic changes and respiratory motion, hinders the effective use of CT imaging. To mitigate CT-to-body divergence, we propose a novel deformable 3D/3D CT-to-DTS registration algorithm employing a multistage, multiresolution approach and using affine and elastic B-spline transformation models with bone and lung mask images. A multiresolution strategy with a Gaussian image pyramid and a multigrid strategy within the B-spline model are applied. The normalized correlation coefficient is included in the cost function for the affine model and a multimetric weighted cost function is used for the B-spline model, with weights determined heuristically. Tested on simulated and real patient bronchoscopy data, the algorithm yields promising results. Assessed qualitatively by visual inspection and quantitatively by computing the Dice coefficient (DC) and the average symmetric surface distance (ASSD), the algorithm achieves mean DC of 0.82±0.05 and 0.74±0.05 , and mean ASSD of 0.65±0.29mm and 0.93±0.43mm for simulated and real data, respectively. This algorithm lays the groundwork for CT-aided intraoperative DTS imaging in image-guided bronchoscopy interventions with future studies focusing on automated metric weight setting.

Abstract:Annals of Otology, Rhinology &Laryngology, Ahead of Print. Introduction:Although surgical navigation is commonly used in rhinologic surgery, data on real world performance are sparse because of difficulties in collecting measurements for target registration error (TRE). Despite publications showing submillimeter TRE, surgeons do report TRE of >3 mm. We describe a novel method for assessing TRE during surgery and report findings with this technique.Methods:The TruDi navigation system (Acclarent, Irving, CA) was registered using a contour-based protocol. The surgeon estimated target registration error (e-TRE) at up to 8 points (anatomic regions of interest [ROI]) during endoscopic sinus surgery (ESS). System logs were used to simulate the localization for quantitative assessment of TRE (q-TRE).Results:We performed 98 localizations in 20 patients. The ROI in the sinuses were ethmoid (33 sites), maxillary (28 sites), frontal (17 sites), and sphenoid (22 sites). For localizations, mean qTRE and eTRE were 0.93 and 0.84 mm (P = .56). Notably, 80% of qTRE and 81% of eTRE were 1 mm or less. Mean qTRE and eTRE were less for attending-performed registrations at the maxillary, frontal and sphenoid.Conclusion:Surgical navigation accuracy, as measured by qTRE and eTRE, approaches 1 mm or better at all sinus sites in a real-world setting for 80% of localizations. The qTRE method provides a unique approach for assessing TRE. Surgeons underestimate TRE (overstate navigation accuracy), but this difference does not seem to be statistically significant. Registration performed by trainees yields higher TRE than registration performed by attendings. These data may be used to guide navigation optimization.
