Abstract:Purpose: The treatment of pelvic and acetabular fractures remains technically demanding, and traditional surgical navigation systems suffer from the hand-eye mis-coordination. This paper describes a multi-view interactive virtual-physical registration method to enhance the surgeon's depth perception and a mixed reality (MR)-based surgical navigation system for pelvic and acetabular fracture fixation. Methods: First, the pelvic structure is reconstructed by segmentation in a preoperative CT scan, and an insertion path for the percutaneous LC-II screw is computed. A custom hand-held registration cube is used for virtual-physical registration. Three strategies are proposed to improve the surgeon's depth perception: vertices alignment, tremble compensation and multi-view averaging. During navigation, distance and angular deviation visual cues are updated to help the surgeon with the guide wire insertion. The methods have been integrated into an MR module in a surgical navigation system. Results: Phantom experiments were conducted. Ablation experimental results demonstrated the effectiveness of each strategy in the virtual-physical registration method. The proposed method achieved the best accuracy in comparison with related works. For percutaneous guide wire placement, our system achieved a mean bony entry point error of 2.76 ± 1.31 mm, a mean bony exit point error of 4.13 ± 1.74 mm, and a mean angular deviation of 3.04 ± 1.22°. Conclusions: The proposed method can improve the virtual-physical fusion accuracy. The developed MR-based surgical navigation system has clinical application potential. Cadaver and clinical experiments will be conducted in future.

Partial-to-Full Registration based on Gradient-SDF for Computer-Assisted Orthopedic Surgery

Abstract:In computer-assisted orthopedic surgery (CAOS), accurate pre-operative to intra-operative bone registration is an essential and critical requirement for providing navigational guidance. This registration process is challenging since the intra-operative 3D points are sparse, only partially overlapped with the pre-operative model, and disturbed by noise and outliers. The commonly used method in current state-of-the-art orthopedic robotic system is bony landmarks based registration, but it is very time-consuming for the surgeons. To address these issues, we propose a novel partial-to-full registration framework based on gradient-SDF for CAOS. The simulation experiments using bone models from publicly available datasets and the phantom experiments performed under both optical tracking and electromagnetic tracking systems demonstrate that the proposed method can provide more accurate results than standard benchmarks and be robust to 90% outliers. Importantly, our method achieves convergence in less than 1 second in real scenarios and mean target registration error values as low as 2.198 mm for the entire bone model. Finally, it only requires random acquisition of points for registration by moving a surgical probe over the bone surface without correspondence with any specific bony landmarks, thus showing significant potential clinical value.

Abstract:Most image-guided interventions rely on surgical tracking and image/model to patient registration to establish a spatial relationship between the patient and the pre- and intraprocedural images, by using surgical tracking and localization systems. In this work, we characterize the tracking, registration and navigation accuracy using two different surgical localization systems - the NDI Polaris Spectra optical tracking system and the NDI Aurora electromagnetic tracking system - in the context of an image-guided renal intervention, using a 3D printed life-size model of a patient-specific kidney phantom generated from a CT image. Our results reported a 0.05 mm fiducial localization error, 0.70 mm fiducial registration error, and 0.78 mm target registration error, and 0.63 mm overall navigation error using the optical tracking, and 0.12 mm fiducial localization error, 0.78 mm fiducial registration error, 0.93 mm target registration error and 0.89 mm overall navigation error using electromagnetic tracking. Additionally, our study also showed similarity between the overall navigation accuracy using optical (0.63 mm RMS error) or electromagnetic tracking (0.89 mm RMS error) and the overall navigation accuracy achieved using direct visualization of the surgical scene (0.68 mm and 1.06 RMS error respectively), which serves as a baseline control metric.

Abstract:Journal of Mechanics in Medicine and Biology, Ahead of Print. In oral and maxillofacial surgery, computer-assisted navigation technologies have been widely used to achieve intraoperative positioning. The traditional methods mainly rely on the experience of doctors and the difference between the locations of key points in the surgical area and the preoperative planning, which have certain limitations. In this paper, a new intraoperative surgical navigation framework based on mobile laser scanner is proposed, which ensures that surgery is performed accurately according to the preoperative planning. The framework mainly includes two parts. First, the real-time surface reconstruction of the anatomy should be realized during the operation. Second, the acquired image is matched to the planned image in real time. Although the most common method of surface reconstruction is to render the volume directly from raw data or render the surface from the segmented data using computed tomography/magnetic resonance (CT/MR) data, this method is too complicated for performing the real-time operation during surgery. Furthermore, a new surface registration technique is proposed for image-guided oral and maxillofacial surgery based on the point sets. To improve the registration accuracy and robustness, the point sets are modeled by Mixed Student's t-Distribution model. In the experiments, the point sets of CT data are from 10 patients with craniomaxillofacial diseases and the surface point set is from the LRS. The TRE of 10 data was less than 1[math]mm. Compared with the paired-point registration method and Iterative Closest Point algorithm, the results demonstrated better performance of the proposed method, the surgical situation can be displayed in real time during the surgical process, and any differences from the surgical plan can also be reflected.

Abstract:Purpose This work proposes a robot‐assisted augmented reality (AR) surgical navigation system for mandibular reconstruction. The system accurately superimposes the preoperative osteotomy plan of the mandible and fibula into a real scene. It assists the doctor in osteotomy quickly and safely under the guidance of the robotic arm. Methods The proposed system mainly consists of two modules: the AR guidance module of the mandible and fibula and the robot navigation module. In the AR guidance module, we propose an AR calibration method based on the spatial registration of the image tracking marker to superimpose the virtual models of the mandible and fibula into the real scene. In the robot navigation module, the posture of the robotic arm is first calibrated under the tracking of the optical tracking system. The robotic arm can then be positioned at the planned osteotomy after the registration of the computed tomography image and the patient position. The combined guidance of AR and robotic arm can enhance the safety and precision of the surgery. Results The effectiveness of the proposed system was quantitatively assessed on cadavers. In the AR guidance module, osteotomies of the mandible and fibula achieved mean errors of 1.61 ± 0.62 and 1.08 ± 0.28 mm, respectively. The mean reconstruction error of the mandible was 1.36 ± 0.22 mm. In the AR‐robot guidance module, the mean osteotomy errors of the mandible and fibula were 1.47 ± 0.46 and 0.98 ± 0.24 mm, respectively. The mean reconstruction error of the mandible was 1.20 ± 0.36 mm. Conclusions The cadaveric experiments of 12 fibulas and six mandibles demonstrate the proposed system's effectiveness and potential clinical value in reconstructing the mandibular defect with a free fibular flap.

Improving registration accuracy during total hip arthroplasty: a cadaver study of a new, 3-D mini-optical navigation system

Abstract:Introduction: Maintaining accuracy of component placement is an important step in ensuring the long-term stability of components during total hip arthroplasty (THA). Computer-assisted navigation has improved accuracy but errors associated with the registration process are known to impact the accuracy of final measurements. The purpose of this cadaver study was to determine the registration error associated with a novel mini-navigation system. Methods: 3 board-certified orthopaedic surgeons performed 4 THA procedures each via the posterolateral approach on 6 cadavers (12 hips) using the mini-navigation tool. Pre- and post-operative radiographs and post-operative computed tomography (CT) images were obtained. Image analysis was performed by 2 radiologists not involved in the surgical procedures. During registration, surgeons aligned the alignment rod with the anterior pelvic plane (APP) to provide a reference plane for comparison with traditional navigation. Cup position from the device was compared with measurements gathered from post-op imaging. Results: The mean difference between CT and device measurements for inclination was -1.7° (standard deviation [SD] 4.9°), while the mean absolute difference was 4.2° (SD 3.2°). The mean difference between anteversion angles calculated from CT scans and from the device was -3.5° (SD 4.5°), with an absolute difference of 4.0° (SD 4.0°). 100% (12/12) of inclination measurements and 92% (11/12) of anteversion measurements fell within both the clinical and statistical limits of agreement when analyzed via the Bland-Altman technique. Conclusions: This study demonstrates that the registration error associated with this new mini-navigation system compares favourably with the known registration error associated with traditional navigation systems.

Abstract:In computer-assisted surgery, navigation based on pre-operative images and intra-operative tracking requires fusion of data from different coordinate systems. Intra-operative registration is used to determine the spatial relationship between these coordinate systems. Feature-based registration methods rely on reference structures localized both in the pre-operative images and in the surgical site. Optically tracked A-Mode ultrasound (US) allows for non-invasive and cost-efficient digitization of bone surface points needed as input data for registration algorithms. It is especially attractive in combination with surface-based registration algorithms, such as the Iterative Closest Point algorithm and its variants, because they automatically localize the corresponding points, which are covered by soft tissue and, hence, not visible in the site. However, as transcutaneous palpation relies on some assumptions, e.g., regarding the average speed of sound of the scanned soft tissue, that are only partly justified in practice, errors occur that make transcutaneous palpation less reliable than direct palpation. Furthermore, optical tracking causes errors that have to be considered, especially if a so-called dynamic reference base is attached to the patient. The present work investigates how to reduce the effect of important error sources in A-Mode US-based registration. The major contributions are techniques for application-specific error modeling and new methods for surface-based registration with anisotropic weighting. This includes a Newton-like optimization scheme for point-to-point registration and a modified kd-tree-based algorithm for closest point computation. Various combinations of registration algorithms and modeling techniques are tested in a simulation study addressing periacetabular osteotomy, and it is clearly demonstrated that standard methods are not recommendable. On the contrary, the new algorithms allow for a substantial increase in registration accuracy and encourage further research in that field.
A Small-Scaled Intraoperative 3D Visualization Navigation System for Femoral Head Repair Surgery

Abstract:Osteonecrosis of the femoral head is a common disabling disease in orthopaedic field. The current repair method is positioning a localization needle with the assist of fluocsopy, which is lack of stability, efficiency, and objective evaluation criteria. This surgery has the following features: (1) The incision of minimally invasive surgery for femoral head repair is very small. (2) The anatomical marks suitable for point-registration navigation and positioning are not obvious. (3) It is not easy to fix the marking points on the femur. Thus, conventional standardized surgical navigation system is not suitable for this kind of surgery with non-standardized procedures. An assisted navigation system for femoral head repair surgery is presented in this paper, with accurate and reliable effect, small scale and convenient manipulation. First of all, point cloud selected form operation window is used to perform registration and realize the precise positioning of femoral head. Secondly, a pose estimation algorithm based on 3D-3D point pairs is proposed and realized the real-time pose tracking of the needle. Finally, the real-time display of the CT model of the femoral head and the relative position of the needle is realized through the mixed reality technology, and guide the surgeons to accurately insert the needle into the necrotic lesions. In the navigation experiment of human femoral head specimen, the navigation error of lesion center is less than 2mm. Under the premise of not changing doctors' surgical operation, the navigation system expands surgeons' vision, and the navigation accuracy meets the accuracy requirements of femoral head repair surgery, so it has certain clinical application value.
2D/3D registration based on biplanar X-ray and CT images for surgical navigation

Abstract:Background and objectives: Image-based 2D/3D registration is a crucial technology for fluoroscopy-guided surgical interventions. However, traditional registration methods relying on a single X-ray image into surgical navigation systems. This study proposes a novel 2D/3D registration approach utilizing biplanar X-ray images combined with computed tomography (CT) to significantly reduce registration and navigation errors. The method is successfully implemented in a surgical navigation system, enhancing its precision and reliability. Methods: First, we simultaneously register the frontal and lateral X-ray images with the CT image, enabling mutual complementation and more precise localization. Additionally, we introduce a novel similarity measure for image comparison, providing a more robust cost function for the optimization algorithm. Furthermore, a multi-resolution strategy is employed to enhance registration efficiency. Lastly, we propose a more accurate coordinate transformation method, based on projection and 3D reconstruction, to improve the precision of surgical navigation systems. Results: We conducted registration and navigation experiments using pelvic, spinal, and femur phantoms. The navigation results demonstrated that the feature registration errors (FREs) in the three experiments were 0.505±0.063 mm, 0.515±0.055 mm, and 0.577±0.056 mm, respectively. Compared to the point-to-point (PTP) registration method based on anatomical landmarks, our method reduced registration errors by 31.3%, 23.9%, and 26.3%, respectively. Conclusion: The results demonstrate that our method significantly reduces registration and navigation errors, highlighting its potential for application across various anatomical sites. Abstract:In order to use pre-operative images during an intervention for navigation, they must be registered to the patient's co-ordinate system in the operating theatre or to an intra-operative image. For the registration to be valid in the case of patient movements, the registration must be updated or the patient movement must be tracked. One problem in this area is the registration of intra-operatively acquired X-ray fluoroscopies with 3D CT images obtained before the intervention as well as motion tracking for this setup. The result can be used to support the placement of pedicle screws in spine surgery or aortic endoprostheses in transfemoral endovascular aneurysm management (TEAM). The different approaches to 2D/3D registration are discussed and a novel voxel-based method is presented: using a small part of the CT image covering only the vertebra of interest, pseudo-projections are computed and the resulting vertebra template is compared to the X-ray projection using a new similarity measure which is called pattern intensity. Application, performance and registration accuracy are discussed and demonstrated by application to images of a TEAM procedure and of a spine phantom.
ArthroNavi framework: stereo endoscope-guided instrument localization for arthroscopic minimally invasive surgeries

Abstract:Significance: As an example of a minimally invasive arthroscopic surgical procedure, arthroscopic osteochondral autograft transplantation (OAT) is a common option for repairing focal cartilage defects in the knee joints. Arthroscopic OAT offers considerable benefits to patients, such as less post-operative pain and shorter hospital stays. However, performing OAT arthroscopically is an extremely demanding task because the osteochondral graft harvester must remain perpendicular to the cartilage surface to avoid differences in angulation. Aim: We present a practical ArthroNavi framework for instrument pose localization by combining a self-developed stereo endoscopy with electromagnetic computation, which equips surgeons with surgical navigation assistance that eases the operational constraints of arthroscopic OAT surgery. Approach: A prototype of a stereo endoscope specifically fit for a texture-less scene is introduced extensively. Then, the proposed framework employs the semi-global matching algorithm integrating the matching cubes method for real-time processing of the 3D point cloud. To address issues regarding initialization and occlusion, a displaying method based on patient tracking coordinates is proposed for intra-operative robust navigation. A geometrical constraint method that utilizes the 3D point cloud is used to compute a pose for the instrument. Finally, a hemisphere tabulation method is presented for pose accuracy evaluation. Results: Experimental results show that our endoscope achieves 3D shape measurement with an accuracy of <730 μm. The mean error of pose localization is 15.4 deg (range of 10.3 deg to 21.3 deg; standard deviation of 3.08 deg) in our ArthroNavi method, which is within the same order of magnitude as that achieved by experienced surgeons using a freehand technique. Conclusions: The effectiveness of the proposed ArthroNavi has been validated on a phantom femur. The potential contribution of this framework may provide a new computer-aided option for arthroscopic OAT surgery.

Abstract:In surgical navigation, finding correspondence between preoperative plan and intraoperative anatomy, the so-called registration task, is imperative. One promising approach is to intraoperatively digitize anatomy and register it with the preoperative plan. State-of-the-art commercial navigation systems implement such approaches for pedicle screw placement in spinal fusion surgery. Although these systems improve surgical accuracy, they are not gold standard in clinical practice. Besides economical reasons, this may be due to their difficult integration into clinical workflows and unintuitive navigation feedback. Augmented Reality has the potential to overcome these limitations. Consequently, we propose a surgical navigation approach comprising intraoperative surface digitization for registration and intuitive holographic navigation for pedicle screw placement that runs entirely on the Microsoft HoloLens. Preliminary results from phantom experiments suggest that the method may meet clinical accuracy requirements.

Intraoral Scanning Enables Virtual-Splint-Based Non-Invasive Registration Protocol for Maxillofacial Surgical Navigation

Abstract:Background/Objectives: Surgical navigation has advanced maxillofacial surgery since the 1990s, bringing benefits for various indications. Traditional registration methods use fiducial markers that are either invasively bone-anchored or attached to a dental vacuum splint and offer high accuracy but necessitate additional imaging with increased radiation exposure. We propose a novel, non-invasive registration protocol using a CAD/CAM dental splint based on high-resolution intraoral scans. Methods: The effectiveness of this method was experimentally evaluated with an ex vivo 3D-printed skull measuring the target registration error (TRE). Surgical application is demonstrated in two clinical cases. Results: In the ex vivo model, the new CAD/CAM-splint-based method achieved a mean TRE across the whole facial skull of 0.97 ± 0.29 mm, which was comparable to traditional techniques like using bone-anchored screws (1.02 ± 0.23 mm) and dental vacuum splints (1.01 ± 0.33 mm), while dental anatomical landmarks showed a lower accuracy with a mean TRE of 1.84 ± 0.44 mm. Multifactorial ANOVA confirmed significant differences in TRE based on the registration method and the navigated level of the facial skull (p < 0.001). In clinical applications, the presented method demonstrated high accuracy for both midfacial and mandibular surgeries. Conclusions: Our results suggest that this non-invasive CAD/CAM-splint-based method is a viable alternative to traditional fiducial marker techniques, with the potential for broad application in maxillofacial surgery. This approach retains high accuracy while eliminating the need for supplementary imaging and reduces patient radiation exposure. Further clinical trials are necessary to confirm these findings and optimize splint design for enhanced navigational accuracy.

Abstract:Background: This study introduced an Augmented Reality (AR) navigation system to address limitations in conventional high tibial osteotomy (HTO). The objective was to enhance precision and efficiency in HTO procedures, overcoming challenges such as inconsistent postoperative alignment and potential neurovascular damage. Methods: The AR-MR (Mixed Reality) navigation system, comprising HoloLens, Unity Engine, and Vuforia software, was employed for pre-clinical trials using tibial sawbone models. CT images generated 3D anatomical models, projected via HoloLens, allowing surgeons to interact through intuitive hand gestures. The critical procedure of target tracking, essential for aligning virtual and real objects, was facilitated by Vuforia's feature detection algorithm. Results: In trials, the AR-MR system demonstrated significant reductions in both preoperative planning and intraoperative times compared to conventional navigation and metal 3D-printed surgical guides. The AR system, while exhibiting lower accuracy, exhibited efficiency, making it a promising option for HTO procedures. The preoperative planning time for the AR system was notably shorter (4 min) compared to conventional navigation (30.5 min) and metal guides (75.5 min). Intraoperative time for AR lasted 8.5 min, considerably faster than that of conventional navigation (31.5 min) and metal guides (10.5 min). Conclusions: The AR navigation system presents a transformative approach to HTO, offering a trade-off between accuracy and efficiency. Ongoing improvements, such as the incorporation of two-stage registration and pointing devices, could further enhance precision. While the system may be less accurate, its efficiency renders it a potential breakthrough in orthopedic surgery, particularly for reducing unnecessary harm and streamlining surgical procedures.
Robust tracking of deformable anatomical structures with severe occlusions using deformable geometrical primitives

Abstract:Background and objective: Surgical robotics tends to develop cognitive control architectures to provide certain degree of autonomy to improve patient safety and surgery outcomes, while decreasing the required surgeons' cognitive load dedicated to low level decisions. Cognition needs workspace perception, which is an essential step towards automatic decision-making and task planning capabilities. Robust and accurate detection and tracking in minimally invasive surgery suffers from limited visibility, occlusions, anatomy deformations and camera movements. Method: This paper develops a robust methodology to detect and track anatomical structures in real time to be used in automatic control of robotic systems and augmented reality. The work focuses on the experimental validation in highly challenging surgery: fetoscopic repair of Open Spina Bifida. The proposed method is based on two sequential steps: first, selection of relevant points (contour) using a Convolutional Neural Network and, second, reconstruction of the anatomical shape by means of deformable geometric primitives. Results: The methodology performance was validated with different scenarios. Synthetic scenario tests, designed for extreme validation conditions, demonstrate the safety margin offered by the methodology with respect to the nominal conditions during surgery. Real scenario experiments have demonstrated the validity of the method in terms of accuracy, robustness and computational efficiency. Conclusions: This paper presents a robust anatomical structure detection in present of abrupt camera movements, severe occlusions and deformations. Even though the paper focuses on a case study, Open Spina Bifida, the methodology is applicable in all anatomies which contours can be approximated by geometric primitives. The methodology is designed to provide effective inputs to cognitive robotic control and augmented reality systems that require accurate tracking of sensitive anatomies.
Closing the Calibration Loop: An Inside-out-tracking Paradigm for Augmented Reality in Orthopedic Surgery

Abstract:In percutaneous orthopedic interventions the surgeon attempts to reduce and fixate fractures in bony structures. The complexity of these interventions arises when the surgeon performs the challenging task of navigating surgical tools percutaneously only under the guidance of 2D interventional X-ray imaging. Moreover, the intra-operatively acquired data is only visualized indirectly on external displays. In this work, we propose a flexible Augmented Reality (AR) paradigm using optical see-through head mounted displays. The key technical contribution of this work includes the marker-less and dynamic tracking concept which closes the calibration loop between patient, C-arm and the surgeon. This calibration is enabled using Simultaneous Localization and Mapping of the environment of the operating theater. In return, the proposed solution provides in situ visualization of pre- and intra-operative 3D medical data directly at the surgical site. We demonstrate pre-clinical evaluation of a prototype system, and report errors for calibration and target registration. Finally, we demonstrate the usefulness of the proposed inside-out tracking system in achieving "bull's eye" view for C-arm-guided punctures. This AR solution provides an intuitive visualization of the anatomy and can simplify the hand-eye coordination for the orthopedic surgeon.
Fusion of Multimodality Image and Point Cloud for Spatial Surface Registration for Knee Arthroplasty

Abstract:Background Image-guided computer-aided navigation system is an indispensable part of computer assisted orthopaedic surgery. However, the location and number of fiducial markers, the time required to localise fiducial markers in existing systems affect their effectiveness. Method The study proposed that spatial surface registration between the point cloud on the surface of the fusion model based on preoperative knee MRI and CT images and the point cloud on the cartilage surface captured by intraoperative laser scanner could solve the above limitations. Results The experimental results show that the registration error of the method is less than 2 mm, but the total time from scanning the point cloud on patient's cartilage surface to registering it with the point cloud in preoperative image space is less than 2 min. Conclusion The method achieves the registration accuracy similar to existing methods without selecting anatomical corresponding points, which is of great help to the clinic.
Fiducial-Free 2D/3D Registration for Robot-Assisted Femoroplasty

Abstract:Femoroplasty is a proposed alternative therapeutic method for preventing osteoporotic hip fractures in the elderly. Previously developed navigation system for femoroplasty required the attachment of an external X-ray fiducial to the femur. We propose a fiducial-free 2D/3D registration pipeline using fluoroscopic images for robot-assisted femoroplasty. Intraoperative fluoroscopic images are taken from multiple views to perform registration of the femur and drilling/injection device. The proposed method was tested through comprehensive simulation and cadaveric studies. Performance was evaluated on the registration error of the femur and the drilling/injection device. In simulations, the proposed approach achieved a mean accuracy of 1.26±0.74 mm for the relative planned injection entry point; 0.63±0.21° and 0.17±0.19° for the femur injection path direction and device guide direction, respectively. In the cadaver studies, a mean error of 2.64 ± 1.10 mm was achieved between the planned entry point and the device guide tip. The biomechanical analysis showed that even with a 4 mm translational deviation from the optimal injection path, the yield load prior to fracture increased by 40.7%. This result suggests that the fiducial-less 2D/3D registration is sufficiently accurate to guide robot assisted femoroplasty.
