Abstract:Many modern and future technological applications involve ultrathin polymer films with a thickness below the 100-nanometer scale, where statistical bulk averaging is jeopardized and interfacial constraints dictate transport properties. In such confined polymeric systems, transport properties strongly depend on molecular relaxation and structural phases that deviate from the bulk. This is particularly relevant in applications involving nano-electromechanical systems (NEMS). In this paper, we address the correspondence between bulk deviating local glass transition values with the non-monotonic plastic deformation properties in ultrathin polystyrene films. Polystyrene serves as a model material in a NEMS application designed to circumvent the superparamagnetic limit associated with magnetic data storage. The application involves data bit writing via an ultrahigh density thermomechanical indentation process. An elaborate frictionvelocity analysis is introduced as a material characterization tool. It provides fundamental insight into the glass forming process, and consequently, the glass transition value in ultrathin spin coated polymer films. The glass transition value is thereby discussed as a phenomenological limit, not unlike an asymptote, to a diverging size of cooperative rearranging regions upon cooling. Unexpected large cooperative clusters up to 40 nm were observed – a dimension that is noticeable at the 100-nanometer length scale. In the light of MD simulations and their good correspondence with the presented intrinsic friction analysis , the importance of angular and torsional intramolecular motions are particularly emphasized for nanotechnological applications.
Microbiological characterization of neuropathic diabetic foot infection: a retrospective study at a Portuguese tertiary hospital

Abstract:Diabetic foot infection imposes a significant burden and is the major cause of nontraumatic limb amputation. Adequate patient management with effective antibiotic therapy is crucial.This retrospective cohort study aimed to characterize the microbiology and resistance patterns of moderate to severe neuropathic diabetic foot infection in patients hospitalized at a tertiary referral hospital between January 2020 and June 2023. Deep tissue specimens from ulcers were collected for culture.Sixty inpatients were included (62% male, mean age 59.1 ± 11.5 years). Osteomyelitis was present in 90% of the patients. Among 102 microorganisms (average of 1.91 ± 1.25 pathogens per patient), 60.8% were gram-positive bacteria, 31.4% were gram-negative, 3.92% were anaerobic bacteria, and 3.92% were fungi. Staphylococcus aureus (19%) and Enterococcus faecium (17%) were the most common. Pseudomonas aeruginosa (8%) and bacteria of the Enterobacterales family (24%) accounted for all the isolated gram-negative bacteria. Sixteen percent of Staphylococcus aureus and 67% of coagulase-negative Staphylococci were resistant to methicillin. Resistance to ampicillin was found in 11% of Enterococci. All Pseudomonas aeruginosa isolates were sensitive to piperacillin-tazobactam, ceftazidime, or cefepime. Among the Enterobacterales, resistance rates were 35% for piperacillin-tazobactam, 38% for ceftazidime, 21% for cefepime, and 13% for carbapenems.Although the prevalence of methicillin-resistant staphylococci was lower than that in other studies, carbapenem resistance among gram-negative bacteria warrants attention. This study highlights the importance of understanding local epidemiology for effective diabetic foot infection management and resistance mitigation.

Abstract:This study identifies the risk factors, microbiological properties, antimicrobial susceptibility patterns, mortality, and clinical complications associated with organisms causing diabetic foot infections (DFIs) with or without antibiotic treatment using data from a retrospective multicenter surveillance. Specimens collected from different hospitals were cultured and the extended-spectrum β-lactamase (ESBL) excretion was estimated. The antibacterial susceptibility pattern and biofilm formation were completed along with the recommended standard methods. Overall, 792 diabetic foot patients (DFPs) were enrolled and a total of 1803 causative organisms were isolated. Polymicrobial infection was identified in 48.5% of the patients. The isolated Gram-positive pathogens (46.7%) were higher than Gram-negative (38.6%) or anaerobes (7.9%). The predominant pathogens were S. aureus (22.2%), methicillin-resistant S. aureus (7.7%), Enterococcus spp. (12.8%), Pseudomonas aeruginosa (9.4%), E. coli (7.9%), Klebsiella spp. (7.5%), Proteus mirabilis (8.9%), coagulase negative staphylococci (CoNS) (6.6%), anaerobic organisms (5.9%), and fungi (2.3%). Vancomycin and clindamycin exhibited no activity against Gram-positive bacteria. However, meropenem and imipenem displayed high activity against the Gram-negative isolates. Out of the 765 tested strains, 251 showed moderate (15.8%) to high (34%) level biofilm-producing phenotype. DFIs were widespread among the diabetic patients with different microbial etiology and the major organisms were aerobic organisms. Our findings may provide an insight into the development of appropriate therapeutic strategies for the management of DFIs.

Abstract:Bacterium 11 differs from Bacteroides amylogens (Doetsch et al. 1957) in some of its fermentation reactions, and the fact that it does not form an iodophilic polysaccharide excludes it from classification with this organism. Bryant and his coworkers have described a number of Gramnegative rod-like bacteria isolated from bovinerumen contents, and bacterium 11 could be classified as a strain of Butyrivibrio fibrisolvens (Bryant & Small, 1956a). The fact that it rapidly ferments xylan precludes it from inclusion with the species Succinivibrio dextrinsolvens (Bryant & Small, 1956b) and Bacteroides succinogenes (Hungate, 1950), and suggests its inclusion with Butyrivibrio fibrisolvens. Although the type species of B. fibrisolvens was motile, it was suggested that the 'less actively cellulolytic rod' of Hungate was a Butyrivibrio species and this was non-motile (Bryant & Small, 1956a). The fermentation reactions of B. fibrisolvens strains are variable, but are generally similar to those of bacterium 11, and all are non-iodophilic. Bacterium 11 differs from Bacteroides amylogenes in the products of fermentation of xylose, especially in the formation of lactic acid. However, a lactic acid fermentation of xylose is known, for instance with Lactobacillus pentosus (Gest & Lampen, 1952). Although lactic acid is not a final product of the fermentation of xylose by mixed rumen organisms (Doetsch, Robinson, Brown & Shaw, 1953), the work of Pazur, Shuey & Georgi (1958) suggests that it might be an intermediate product.
Clinical and microbiological characteristics of diabetic foot infection by carbapenem-resistant bacteria in a hospital in southern Brazil

Abstract:Carbapenem-resistant bacteria are a major cause of diabetic foot (DF) infection. They are resistant to almost all available antimicrobial agents and lead to increased hospital costs. The clinical and microbiological characteristics of DF infection due to these microorganisms and the association of DF with outcomes such as mortality and amputation are not yet well established. This was a retrospective cohort study involving 60 vascular surgery patients who were admitted to Nossa Senhora da Conceição Hospital due to DF infection caused by carbapenem-resistant bacteria and who underwent debridement or amputation, and a control group with 60 randomly selected patients. The analyzed parameters were mortality and amputation rates and patients’ epidemiological and microbiological profiles. A significant association was observed between bacterial resistance and previous amputation, previous hospitalizations, and DF ulcer classification into ischemic and neuroischemic; however, there were no differences in mortality and amputation rates between the groups, even after using the Charlson comorbidity index. Although DF infections by carbapenem-resistant bacteria were not statistically associated with worse outcomes, their study is important given that there are few therapeutic options for treating these specific infections.

Abstract:Objective: To determine the prevalence of methicillin-resistant Staphylococcus aureus (MRSA) in diabetic foot wounds and analyze antibiotic resistance patterns. Introduction: The global incidence of diabetes mellitus, particularly type II, has significantly increased, leading to complications such as diabetic foot ulcers. These ulcers often become infected, with MRSA being a common and challenging pathogen. Understanding the prevalence and resistance patterns of MRSA in diabetic foot infections is crucial for effective treatment. Methodology: Diabetic foot wound samples were collected from 65 patients in the Valley of Toluca, Mexico. Samples were cultured and analyzed using chromogenic agar, BHI, salt and mannitol, calf blood, EMB, and MacConkey agars. Strain identification and antibiotic sensitivity testing were performed using the Vytek automated system. Oxacillin and cefoxitin were used to detect methicillin resistance. Statistical analyses, including Kolmogorov-Smirnov, Shapiro-Wilk tests, and Spearman correlation, were conducted to evaluate relationships between clinical factors and antibiotic resistance. Results: Of the 65 samples, 56.9% were from men and 43.1% from women, with 97.1% having type II diabetes. MRSA prevalence was 63%. Resistance rates were highest for ampicillin (100%), clindamycin (89%), erythromycin (87%), and gentamicin (73%). Statistical analysis showed no significant correlation between oxacillin resistance and glycemic control, erythromycin resistance (MLS resistance), hypertension, or gender. Conclusion: The high prevalence of MRSA in diabetic foot wounds underscores the need for targeted infection control and appropriate antibiotic therapy. The lack of correlation between resistance and clinical factors suggests a multifactorial nature of antibiotic resistance, necessitating broader clinical and microbiological considerations for future studies.
[Results of preoperative chemoradiotherapy for T4 rectal cancer].

Abstract:We reviewed clinical records of 11 cases with preoperative chemoradiotherapy to evaluate the clinical effectiveness of chemoradiotherapy for T4 rectal cancer. The preoperative radiotherapy consisted of 40-50 Gy delivered in fractions of 1 .8- 2.0 Gy per day, five days per week. A treatment of 5-fluorouracil, 500 mg/body per day intravenously, or oral UFT-E (300 mg/m²) with l-leucovorin (75 mg) per day, or oral S-1 (80 mg/m²) per day five days per week, was given during radiotherapy. One patient died due to pelvic abscess in 63 days after chemoradiotherapy. Invasive findings to the adjacent organs identified by CT and MRI disappeared in 6 cases with complete or partial response 1 month after chemoradiotherapy. Curative surgery was performed in 7 patients. Although the adjacent organs were also removed during surgery in 7 patients, there was no histological invasion to the adjacent organs in 4 patients, and one patient had histological complete disappearance of tumor. Although complications after surgery were found in all of the patients, they were improved by conservative treatment. Two of 7 patients with curative surgery had recurrence, but the rest of them survived without recurrence. Preoperative chemoradiotherapy was expected to be an effective treatment to improve the resection rate and prognosis for T4 rectal cancer. However, it was thought that it was necessary to be careful about severe toxicity, such as pelvic abscess.

Abstract:This paper focuses on the design of fast algorithms for rotating images and preserving high quality. The basis for the approach is a decomposition of a rotation into a sequence of one-dimensional translations. As the accuracy of these operations is critical, we introduce a general theoretical framework that addresses their design and performance. We also investigate the issue of optimality and present an improved least-square formulation of the problem. This approach leads to a separable three-pass implementation of a rotation using one-dimensional convolutions only. We provide explicit filter formulas for several continuous signal models including spline and bandlimited representations. Finally, we present rotation experiments and compare the currently standard techniques with the various versions of our algorithm. Our results indicate that the present algorithm in its higher-order versions outperforms all standard high-accuracy methods of which we are aware, both in terms of speed and quality. Its computational complexity increases linearly with the order of accuracy. The best-quality results are obtained with the sine-based algorithm, which can be implemented using simple one-dimensional FFTs.

Abstract:Diabetic foot ulcers (DFUs) are a serious and common problem in patients with diabetes mellitus and constitute one of the major causes of lower extremity amputation. The microbiological profile of DFUs depends on the acute or chronic character of the wound. Aerobic gram-positive cocci are the predominant organisms isolated from DFUs. Diabetic foot biopsies from patients admitted to the Angiology and Vascular Surgery Hospital of the Northeast, in Reynosa, Tamaulipas from December 2011 to April 2016 were analyzed. The samples were processed using standard microbiology techniques. Antimicrobial susceptibility testing was carried out according to the protocol established by the Clinical & Laboratory Standards Institute (CLSI). We obtained 246 bacterial isolates, based on the results of phenotypic resistance. The least effective antibiotics for gram-positive bacteria were penicillin and dicloxacillin; for gram-negative bacteria, cefalotin and penicillin were the least effective. Levofloxacin, cefalotin, and amikacin were the most effective antibiotics for gram-positive and negative bacteria, respectively. Enterobacter genus was significantly associated with muscle biopsies ( P = .011) and samples without growth were significantly associated with specimens of pyogenic origin ( P = .000). In 215 DFU samples, we found that Staphylococcus aureus was the most commonly isolated pathogen followed by Enterobacter sp. This is consistent with previous reports. Enterobacter species may play an important role in the colonization/infection of certain tissues; however, further studies are needed in this regard.
Insight into multidrug-resistant microorganisms from microbial infected diabetic foot ulcers

Abstract:Aim: This study was undertaken to inspect the preponderance of multidrug-resistant (MDR) microorganisms in microbial infected diabetic foot ulcers (DFUs) in north Egypt. Moreover, their later impact on the patients and previous antibiotic therapies were taken into consideration. Methods: To accomplish this goal, twenty-two of diabetic foot patients with purulent wounds were enrolled in this prospective study. These wounds were swabbed and the antibiotics susceptibility patterns of most virulent bacteria and yeast were studied. Furthermore, bacterial and yeast strains were identified using 16S rRNA and 16S rRNA nucleotide sequences, respectively, and following their phenotypic characteristics employing the VITEK 2 system. Results: Microbial profiles showed a predominance of monomicrobial infections (77.3%), while polymicrobial infections were found in 22.7%. A total of 24 bacterial isolates (15 Gram-positive and 9 Gram-negative) and four yeast isolates were perceived. Four bacteria were selected based on their resistance toward more than six of empirical antibiotics. They were identified and deposited in GenBank as Acinetobacter baumanni MT3 (KY421195), Staphylococcus aureus MT1 (KY421197), Klebsiella pneumonia MT2 (KY421196), and Staphylococcus aureus MT4 (KY421198). On the other hand, one strain belonged to yeast was opted and identified as Candida albicans MT5 (MG851796). Conclusion: These findings might effectively help to avert the severe complications of diabetic foot infections (DFIs) besides our endeavours to find new antimicrobial wound dressings.

Abstract:Diabetes mellitus is a major chronic disease that continues to increase significantly. One of the most important and costly complications of diabetes is foot ulceration that may be colonized by pathogenic and antimicrobial resistant bacteria, which may express several virulence factors that could impair treatment success. These bacterial communities can be organized in polymicrobial biofilms, which may be responsible for diabetic foot ulcer (DFU) chronicity. We evaluated the influence of polymicrobial communities in the ability of DFU isolates to produce biofilm, using a microtiter plate assay and a multiplex fluorescent in situ hybridization, at three time points (24, 48, 72 h), after evaluating biofilm formation by 95 DFU isolates belonging to several bacterial genera (Staphylococcus, Corynebacterium, Enterococcus, Pseudomonas and Acinetobacter). All isolates were biofilm-positive at 24 h, and the amount of biofilm produced increased with incubation time. Pseudomonas presented the higher biofilm production, followed by Corynebacterium, Acinetobacter, Staphylococcus and Enterococcus. Significant differences were found in biofilm formation between the three time points. Polymicrobial communities produced higher biofilm values than individual species. Pseudomonas + Enterococcus, Acinetobacter + Staphylococcus and Corynebacterium + Staphylococcus produced higher biofilm than the ones formed by E. faecalis + Staphylococcus and E. faecalis + Corynebacterium. Synergy between bacteria present in dual or multispecies biofilms has been described, and this work represents the first report on time course of biofilm formation by polymicrobial communities from DFUs including several species. The biological behavior of different bacterial species in polymicrobial biofilms has important clinical implications for the successful treatment of these infections.

Microbiological profile and clinical characteristics of diabetic foot infection in northern China: a retrospective multicentre survey in the Beijing area

Abstract:Purpose: We aimed to define the microbiological characteristics of diabetic foot infection in patients in the Beijing area and to explore the demographic and clinical factors correlated with pathogen distribution. Methodology: As part of a retrospective multicentre surveillance program conducted in eight hospitals in Beijing 2010-2014, we recruited all inpatients for whom bacterial culture had been performed. Demographic, clinical, laboratory and surgery data were obtained from medical records. Statistical analysis was performed to analyse data on microbiological and clinical characteristics.Results/Key findings. A total of 456 cases were included. The culture positivity was 95.4 %. Among all patients with positive cultures, 88 cases (20.2 %) had polymicrobial infections. Five hundred and fifty-one species were isolated from all specimens, including 39.6 % Gram-positive bacteria and 57.5 % Gram-negative bacteria. Enterobacteriaceae accounted for 41.0 % of all isolates. Staphylococcus aureus (17.1 %), Pseudomonas aeruginosa (13.1 %), Proteus spp. (9.8 %), Escherichia coli (9.3 %) and coagulase-negative Staphylococcus (8.3 %) were the most frequently isolated species. The rate of resistance to methicillin was 24.5 % for S. aureus. The susceptibility of P. aeruginosa to all antibiotics was over 60 %. The rate of extended-spectrum β-lactamase production among E. coli was 52.6 %. P. aeruginosa and Enterobacteriaceae show high sensitivity to piperacillin/tazobactam, carbapenems and amikacin. Multivariate analysis showed that patient age >60 years was independently associated with Gram-negative rods. Conclusions: Enterobacteriaceae were the most frequently isolated organisms in our area. Older patients were more likely to suffer from Gram-negative rod infections. Gram-negative rods show high sensitivity to piperacillin/tazobactam, carbapenems and amikacin.
The antimicrobial susceptibility patterns of diabetic foot ulcers in the South African public healthcare sector

Abstract:Background. Diabetic foot syndrome is defined as the presence of a diabetic foot ulcer (DFU) associated with neuropathy, peripheral artery disease and infection. While the use of antimicrobials in the treatment of DFU infection remains a mainstay, the choice of antimicrobial remains problematic owing to the presence of multidrug-resistant polymicrobial infections. In the South African public healthcare sector, the treatment of DFUs is based on the Standard Treatment Guidelines (STGs) and the Essential Drug List. These guidelines are developed using evidence-based medicine and are based on global susceptibility patterns rather than local susceptibility data, and may not provide the most appropriate treatment options. Objectives. To determine the antimicrobial susceptibility patterns of DFUs isolated from patients visiting selected Gauteng provincial public hospitals in order to determine a clinically effective treatment protocol for the management of these infections. Methods. Sample swabs were taken from 51 DFUs using the Levine method. Each sample swab was spread onto blood agar plates, and thereafter individual pathogens were isolated. The antimicrobial susceptibility patterns of all isolated pathogens were determined using zone of inhibition measurements. Pathogens were grouped according to macromorphological characteristics as well as susceptibility patterns, and a representative isolate from each group was then identified. Results. A total of 51 DFU ulcer swabs from 45 patients were included in the study. From the sample swabs, a total of 445 pathogens were isolated. The most effective antimicrobial was found to be gentamicin, followed by ciprofloxacin. Amoxicillin/clavulanic acid, the first-line treatment according to the STGs, was found to be ineffective for many of the isolated pathogens. The most commonly isolated pathogens were Proteus mirabilis, Enterococcus faecalis and Pseudomonas aeruginosa. Conclusion. These findings demonstrate the urgent need to reassess the STGs and base treatment plans on local epidemiological data. This study provides valuable data on common causative pathogens in DFU infections, as well as the resistance patterns of these pathogens, forming a baseline on which to base future DFU treatment plans.

Abstract:This work presents a protocol to prevent the transmission of multidrug-resistant infections. We focus on the Diabetic Foot Unit Podiatry Clinic Area attached to the University of Seville in particular. The most common complication for patients with diabetes is leg ulcers. Together with neuropathy, vasculopathy, and immunological response disorder, these individuals have a high predisposition to developing infections. Staphylococcus aureus is a highly prevalent microorganism in humans which, at times, may act as a pathogen. Due mainly to indiscriminate abuse of antibiotics, the methicillin-resistant strain known by its initials as MRSA is the most extended nosocomial infection globally and is a severe community and hospital healthcare problem. This paper describes compliance with new general recommendations on cleaning, hygiene, and decontamination, in addition to implementation of this specific protocol, after detection of cross infection (healthcare-related infection) in the studied unit in two patients with MRSA-infected ulcers. After an in-depth bibliographical review, strict hand hygiene measures and use of non-sterile gloves were used when treating all patients with a diabetic foot. Finally, we reflect on the need to educate healthcare personnel to guarantee correct prescription of selected antibiotics. The role of the podiatrist in the multidisciplinary team is highlighted not only in terms of management and treatment of lesions in diabetic patients, but also as a healthcare agent for the detection and prevention of MRSA together with other multidrug-resistant infections.

Abstract:Patients gain knowledge about their disease, medications, pain management, and available resources. Because of managed care, capitation, and the corresponding pressures to see as many patients as possible, the focus of care today is more often on cost reduction than on quality. Therefore, healthcare providers cannot always spend the amount of time necessary to address patients' educational needs. To meet these needs in an efficient way, our arthritis center developed an educational program for patients with systemic rheumatic diseases, .such as rheumatoid arthritis. The arthritis center is a busy ambulatory practice at a major teaching hospital in Boston. Approximately 19,500 individuals per year, including pediatric, adult, and geriatric patients, receive care for rheumatic diseases and musculoskeletal disorders at the center. The educational program provides patients with opportunities to ask questions and share information about how their disease is affecting them or about other health-related issues.

Abstract:Background and objectives: Diabetic foot ulcer (DFU) is one of the serious complications of diabetes, being related to frequent and long-term hospitalisation, reduced quality of life of the patient, amputations, a high rate of morbidity and mortality. The bacterial aetiology is complex, sometimes involving more than one pathogen, playing a major role in the infection prognosis and development of microbial resistance. This study evaluated the current state of the aetiology, clinical and pathological characteristics of DFU in a single diabetes centre in order to provide some specific measures to prevent it. Materials and Methods: This retrospective study was conducted on patients with diabetes mellitus (252 individuals diagnosed with DFU) between January 2018-December 2019. All participants were assessed based on their clinical characteristics, including complications of diabetes and pathological and microbiological evaluations. Results: The present research revealed that diabetic foot ulcer prevalence was higher in males than in females and higher in type 2 diabetic patients than in type 1 diabetic patients. The patients with diabetic foot ulcer were older, had a higher body mass index (BMI), longer diabetic duration and had more diabetic complications, such as retinopathy, diabetic polyneuropathy and diabetic kidney disease, than patients without diabetic foot ulceration. Conclusions: Taking into account all factors involved, including the aetiology and the antibiotic susceptibility pattern of these isolates, planning the suitable treatment options of patients is possible.

Abstract:The International Journal of Lower Extremity Wounds, Ahead of Print. This study presents a comprehensive investigation into the microbial ecology of diabetic foot infections (DFIs), using molecular-polymerase chain reaction (PCR) analysis to accurately identify the causative agents. One hundred DFI patients were recruited and classified using the Depth Extent Phase and Associated Etiology (DEPA) score according to their severity. Results revealed polymicrobial infections in 75% of cases, predominantly featuring Staphylococcus epidermidis (83%) and Staphylococcus aureus (63%). Importantly, 20% of samples exhibited facultative anaerobes Bacteroides fragilis or Clostridium perfringens, exclusively in high DEPA score ulcers. Candida albicans coinfection was identified in 19.2% of cases, underscoring the need for mycological evaluation. Empirical antimicrobial therapy regimens were tailored to DEPA severity, yet our findings highlighted a potential gap in methicillin-resistant Staphylococcus aureus (MRSA) coverage. Despite an 88% prevalence of methicillin-resistant Staphylococci, vancomycin usage was suboptimal. This raises concerns about the underestimation of MRSA risk and the need for tailored antibiotic guidelines. Our study demonstrates the efficacy of molecular-PCR analysis in identifying diverse microbial communities in DFIs, influencing targeted antibiotic choices. The results advocate for refined antimicrobial guidelines, considering regional variations in microbial patterns and judiciously addressing multidrug-resistant strains. This research contributes crucial insights for optimizing DFIs management and helps the physicians to have a fast decision in selection the suitable antibiotic for each patient and to decrease the risk of bacterial resistance from the improper use of broad-spectrum empirical therapies.

Abstract:Introduction: Diabetic foot infection (DFI) is one of the common diabetic complications. Pathogens causing DFI and their antibiotic susceptibility vary with location. Therefore, empirical antibiotic therapy should be based on the pathogens that are most likely to be present. Aim: To identify the frequent aerobic bacteria causing DFI with detection of their antibiotic susceptibility to help clinicians in our community choose the best empirical antibiotic for DFI. Methods: Swabs were collected from 104 diabetic foot ulcers (DFUs). Aerobic bacterial cultures were done followed by bacterial identification and antibiotic susceptibility testing on VITEK® 2 system. Extended-spectrum beta-lacatamase (ESBL) detection was performed phenotypically and confirmed by multiplex-PCR for bla CTX-M, bla TEM, and bla SHV genes. Results: Aerobic bacterial infection was detected in 82/104 (78.8%) of the DFUs. Gram-negative bacilli (GNB) were isolated more frequently (56.1%) than Gram-positive cocci (GPC) (43.9%). The most common single-isolated bacteria were K. pneumoniae (26.8%), S. aureus and coagulase negative staphylococci (22% for each). The only significant independent predictors of DFI with GNB or GPC were long DM duration and frequent hospitalizations, respectively. The most active antibiotics were amikacin, tigecycline and meropenem for GNB, and linezolid and vancomycin for staphylococci. Multidrug-resistance prevalence was 95.1%. ESBL was detected in 52.6% of Enterobacteriaceae; the bla CTX-M gene was the most common (90%), followed by bla TEM (65%) and bla SHV (35%). Peripheral neuropathy was the single independent predictor for DFI with ESBL producers (adjusted OR=15.5). Conclusions: There is a notable local pattern of DFI bacteriology in our community. Our findings could be valuable in developing the future empirical treatment guidelines for DFIs.

Abstract:Purpose: To determine the pattern and bacteriological characteristics of diabetic foot ulcers in patients attending a tertiary health care facility. Method: 160 Patients with Diabetes Mellitus foot syndrome were recruited, out of which 52 had diabetic foot ulcers. Relevant clinical, biochemical, and microbiological evaluations were carried out on the subjects. Data analysis was done using SPSS version 20. p value was set at <0.05. Results: 52 (32.5%) out of 160 subjects with Diabetes Mellitus Foot Syndrome (DMFS) had diabetic foot ulcers. Poor glycaemic control (mean HbA1c = 9.2 (2.7) %), and abuse of antibiotics (76.9%) characterized the subjects. Foot ulcers mainly involved the right lower limb and followed spontaneous blister formation (50%). Microbiological culture pattern was polymicrobial (71.2%); predominantly anaerobic organisms (53.3%). Gram positive and negative aerobic isolates yielded high sensitivity to common quinolones (76% - 87.8%). The gram positive and negative anaerobic isolates were highly sensitive to Clindamycin and Metronidazole respectively (80.2% - 97.8%). High sensitivity (>80%) yield for gram negative anaerobes was recorded for Imipinem and Ampicillin/Sulbactam. Conclusion: Diabetic foot ulcers (DFU) contribute about one-third of DMFS. The bacteriological isolates from these ulcers are mainly polymicrobial with high sensitivity to common antibiotics. The need for appropriate use of antibiotics should be advocated among the patients.
