Abstract:The aim of this study is to evaluate morphological features of corneal flap/cap and the correlations with corneal higher-order aberrations (HOAs) changes after femtosecond laser in situ keratomileusis (FS-LASIK) and small incision lenticule extraction (SMILE). This was a retrospective study. Pre- and postoperative (1 and 3 months) corneal HOAs were assessed with Pentacam HR. The corneal flap/cap thickness at 32 points (± 1.5 mm, ± 2 mm, ± 2.5 mm, and ± 3 mm from the corneal vertex on meridian 0°/45°/90°/135°) were measured using anterior segment optical coherence tomography at 3 months postoperatively. Morphological features of corneal flap/cap including predictability (P), uniformity (U), and symmetry (S) were calculated and used for correlation analysis with corneal HOAs changes. Eighty-six eyes (44 patients) and ninety-six eyes (50 patients) were involved in FS-LASIK and SMILE groups, respectively. Significant thicker corneal flap/cap than the predicted was observed at each measuring point and meridian in both groups (difference > 2.225 μm, the within-subject standard deviation over 6-mm optical zone). There was no statistically significant difference in predictability of corneal flap/cap thickness, while U6 mm (P < .0001), U0 (P < .001), U45 (P = .002), U90 (P < .0001), U135 (P = .004), S6 mm (P < .0001), S0 (P < .001), and S90 (P < .0001) over 6 mm zone were less in SMILE than in FS-LASIK. The changes of corneal tHOAs, Z (3, − 1), Z (3, 1), and SA were significantly correlated with morphological features of corneal flap/cap. Both FS-LASIK and SMILE had good predictability in flap or cap thickness, while the uniformity and symmetry of SMILE cap were better than FS-LASIK flap. The quality of flap/cap was closely associated with the changes of corneal HOAs.

Abstract:PURPOSE: To evaluate and compare the thickness and the morphology of femtosecond and mechanical microkeratome LASIK flaps using anterior segment optical coherence tomography (AS-OCT). METHODS: Bilateral LASIK was performed in 132 eyes from 61 myopic patients. Flaps were created in 72 eyes using the WaveLight FS200 femtosecond laser (WaveLight GmbH, Erlangen, Germany) and in 50 eyes using the Moria microkeratome (Moria SA, Antony, France). AS-OCT was used 1 week postoperatively to evaluate the thickness of 17 points across each flap, which were 0, 2, and 3.5 mm to the corneal vertex on the horizontal, vertical, 45 degrees, and 135 degrees meridians. RESULTS: The mean central flap thickness was 105.53 +/- 5.86 mu m in the WaveLight group and 132.96 +/- 13.91 mu m in the Moria group (P < .001). The difference between the achieved and the intended flap thickness (accuracy) was 6.17 +/- 3.98 and 23.60 +/- 12.64 mu m, respectively (P < .001). The standard deviation within individual flap (uniformity) was smaller in the WaveLight group. The symmetry and regularity were also better in the WaveLight group. Flap morphology showed a more regular planar shape in the WaveLight group and a meniscus shape in the Moria group. CONCLUSIONS: AS-OCT showed that the flaps created by the WaveLight femtosecond laser were more accurate, reproducible, and uniform than those created by the Moria microkeratome.
A Comparative Study of the Effect of Femtosecond Laser-Assisted Cataract Surgery on Corneal Astigmatism in Post-LASIK Eyes and Virgin Eyes

Abstract:Purpose:To evaluate and compare the effect of femtosecond laser-assisted cataract surgery on corneal astigmatism in post-LASIK eyes and virgin eyes. Patients and Methods:Patients who underwent femtosecond laser-assisted cataract surgery were included in the study and categorized into two groups: Group A, consisting of patients with post-LASIK eyes, and Group B, consisting of patients with virgin eyes. Visual acuity, corneal astigmatism, and surgically induced astigmatism (SIA) were evaluated. Additionally, the correlation between SIA and preoperative corneal astigmatism, mean corneal curvature, and central corneal thickness was also analyzed. Results:A total of 168 eyes were enrolled in this study, with 62 eyes in Group A and 106 eyes in Group B. Significant differences in corneal astigmatism and SIA were observed between the two groups in the early postoperative period following cataract surgery (P<0.05). However, there was no significant difference at 6 months postoperatively (P>0.05). Corneal astigmatism demonstrated an against-The-rule shift in both groups postoperatively. No significant correlation was identified between SIA and preoperative corneal astigmatism, corneal curvature or corneal thickness. Additionally, there was no significant difference observed between the two groups in terms of uncorrected distance visual acuity (UDVA) at 6 months postoperatively. Conclusion:The effect of femtosecond laser-assisted cataract surgery on corneal astigmatism in post-LASIK eyes and virgin eyes was different in the early postoperative period. However, there was no significant difference at 6 months postoperatively. The post-LASIK eyes exhibited a delayed recovery compared to the virgin eyes.
[Analysis of Flap Thickness by Anterior Segment Optical Coherence Tomography in Different Flap Preparation Styles of Excimer Laser Surgery].

Abstract:OBJECTIVETo assess and compare the accuracy and regularity and short-term change of corneal flap in different flap preparation styles.METHODSEighty eyes of 40 cases were treated with laser-assisted subepithelial keratectomy. One hundred and fifty-five eyes of 78 cases were treated with laser in situ keratomileusis with 90 microm microkeratome flap preparation and eighty-two eyes of 42 cases were treated with laser in situ keratomileusis with 100 microm femtosecond laser flap preparation. Anterior segment optical coherence tomography was used to assess the thickness of epithelial or stromal flaps created with M2 microkeratome or VisuMax femtosecond laser by the same investigator 1 hour, 1 day, 1 week, 1 month and 3 months postoperatively, respectively. The thickness of twenty-five measuring points across each flap in LASIK, which were 1.5 mm, 2.5 mm and 3.5 mm away from vertex of cornea on the horizontal, vertical, 45 degrees and 135 degrees meridian respectively and the vertex of cornea itself, was evaluated. The results were analyzed with t-test or ANOVA by SPSS software.RESULTSThe thickness of epithelial flaps was (57.91 + or - 6.76), (54.94 + or - 4.65), (54.96 + or - 2.70), (55.26 + or - 4.27) and (53.86 + or - 5.56) microm at 1 hour, 1 day, 1 week, 1 month and 3 months respectively, with statistical significant difference between 1 hour and the other time points (F = 20.25, P < 0.05). The average thickness of stromal flaps on the vertex of cornea in microkeratome group was (129.16 + or - 12.10) microm, while (112.69 + or - 5.12) microm in the femtosecond laser group, which means the accuracy of flap preparation with femtosecond laser is better than with microkeratome. The absolute difference between actual and average thickness was (13.16 + or - 4.78) and (5.26 + or - 1.21) microm in the microkeratome group and femtosecond laser group, which means the reproducibility of flap preparation with femtosecond laser is better than with microkeratome as well. Flap morphology showed the feature of thin central area and thick peripheral area in microkeratome flap and uniformity in femtosecond laser flap, which illustrated the better regularity of femtosecond laser flap preparation.CONCLUSIONSAnterior segment optical coherence tomography is a safe, quick and reliable facility to examine and assess flap morphology. Our results showed the accuracy and regularity of femtosecond laser flap preparation outweighed microkeratome flap preparation.
Small Incision Lenticule Extraction and Femtosecond-Assisted Laser in Situ Keratomileusis in Patients with Deep Corneal Opacity: Case Series.

Abstract:AIM To report the safety, efficacy, and accuracy of small-incision lenticule extraction (SMILE) or femtosecond-assisted laser in situ keratomileusis (FS-LASIK) for the correction of myopia or myopic astigmatism in patients with deep corneal opacity denoted by anterior segment optical coherence tomography (AS-OCT). METHODS Four patients with monocular corneal opacity (3 due to mechanical injury, 1 due to a firecracker wound) were recruited and treated with refractive surgery (3 for SMILE, 1 for FS-LASIK combined with limbal relaxing incision (LRI). Preoperative ocular manifestations, surgical details, postoperative visual outcomes, corneal opacity parameters, and corneal topography were analyzed. RESULTS Preoperatively, spherical diopter ranged from -3.0 D to -4.75 D with cylinder ranging from -0.75 to -5.0 D, and corrected distance visual acuity (CDVA) ranging from 20/25 to 20/20. One eye's corneal opacity was located in the central zone and three were in the mid-peripheral optical zone. Three patients underwent uneventful SMILE in both eyes, whilst one patient underwent FS-LASIK for high astigmatism in both eyes and LRI in the right eye. CDVA of the eye with corneal opacity ranged from 20/22 to 20/20 one to six weeks postoperatively. Two patients achieved better CDVA and no patients lost Snellen lines. The postoperative diopter was within ±0.75 D for all eyes. Significant edema existed above the corneal opacity in one eye and dissipated soon. No eccentric corneal topography or morphological anomaly of the corneal cap or flap was observed. CONCLUSION The cases demonstrate that SMILE or FS-LASIK is safe and effective to treat myopic astigmatism combined with deep corneal opacity lesions after comprehensive preoperative evaluation and appropriate candidate selection. FS-LASIK combined with LRI is also sufficient for correcting high astigmatism due to corneal scarring.
Comparison of corneal flap morphology in laser in situ keratomileusis with the WaveLight femtosecond laser versus the Moria mechanical microkeratome

Abstract:Objective To evaluate and compare the thickness and morphology of laser in situ keratomileusis (LASIK) flaps created by either the WaveLight FS-200 femtosecond laser or the Moria microkeratome M2 90 using anterior segment optical coherence tomography (AS-OCT).Methods This was a prospective case-control study.Bilateral LASIK was performed on 61 myopic patients.Flaps were created using the WaveLight FS-200 femtosecond laser in 72 eyes and the Moria M2 90 microkeratome in 50 eyes.The thickness at 17 points across each flap,0 mm,±2 mm,and ±3.5 mm to the corneal vertex on the horizontal,vertical,45° and 135° meridians,were evaluated using a Visante AS-OCT at 1 week postoperatively.An independent t test was used.Results The central flap thickness of the WaveLight group was 105.53±5.86 μm,while it was 132.96±13.91 μm in the Moria group (t=-13.16,P＜0.01).The difference between the achieved and the intended flap thickness (accuracy) was 6.17±3.98 μm and 23.60±12.64 μm,respectively (t=-15.20,P＜0.01).The standard deviation of thickness in the 17 points within an individual flap (uniformity) was 4.07±0.94 μm in the WaveLight group and 8.48±2.35 μm in the Moria group (t=-12.60,P＜0.01).The index S (Symmetry) of Moria group (33.81±11.28 μm) was significantly smaller in the WaveLight group (68.46±26.66 μm) (t=-8.67,P＜0.01).Conclusion AS-OCT showed that the flaps created with the WaveLight FS-200 femtosecond laser were more accurate,reproducible,and uniform than those created with the Moria M2 90 microkeratome.
Anterior Segment Optical Coherence Tomography Measurement of Flap Thickness after Myopic LASIK Using the Moria One Use-Plus Microkeratome

Abstract:PURPOSE: To analyze the accuracy and consistency of corneal flap thickness created in myopic LASIK using the Mona One Use-Plus microkeratome compared with the Mona M2 Single Use 90-mu m microkeratome.METHODS: Bilateral LASIK was performed in 68 myopic patients. Flaps were created using the One Use-Plus microkeratome in 82 eyes (41 patients) and the M2 90-mu m microkeratome in 54 eyes (27 patients). Flap complications and visual outcomes were evaluated. Horizontal "High Res. Corneal" scan pattern of anterior segment optical coherence tomography (AS-OCT) was applied to measure flap thickness at five locations (0, +/- 2, and +/- 3.5 mm from the corneal vertex) on the first postoperative day.RESULTS: No significant differences were noted in flap complications and visual outcomes between groups. The central flap thickness was dramatically thinner in the One Use-Plus group (114.7 +/- 10.1 mu m and 109.4 +/- 11.0 mu m for right and left eyes, respectively) than in the M2 group (155.6 +/- 14.8 mu m and 151.6 +/- 12.5 mu m for right and left eyes, respectively) (P<.001) The One Use-Plus did not show a markedly better uniformity than the M2; the variation was mainly observed in the periphery. Multiple linear regression showed that for the One Use-Plus, the steeper the preoperative keratometry, the thicker the flap thickness, and for the M2, the thicker the preoperative pachymetry, the thicker the flap (P<.1).CONCLUSIONS: The One Use-Plus and M2 microkeratomes have similar safety and efficacy. The flap created by the One Use-Plus was much thinner than the flap created with the M2; however, the One Use-Plus can not realize a fully planar-shaped flap. Evaluation of the change in retinal thickness after femtosecond laser-assisted laser in situ keratomileusis and photorefractive keratectomy

Abstract:To investigate the change in individual retinal layer thickness by spectral-domain optical coherence tomography (SD-OCT) in eyes underwent femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK) and photorefractive keratectomy (PRK).In patients who underwent PRK and FS-LASIK, changes in the thickness of all retinal layers in the foveal, parafoveal, and perifoveal regions were evaluated by SD-OCT automated segmentation analysis at pre-operatively and different time points post-operatively.Seventy-one eyes of 71 patients (38 patients in PRK, 33 patients in LASIK) were included. In the pre-operative period, mean spherical equivalent (SE), mean keratometry, axial length, and segmentation values of the retinal layers were similar (P> .05). In the PRK group, the pre-operative measurements of individual retinal layers did not show a statistically significant difference compared to the post-operative measurements on the 1st day, 1st week, and 1st month. In the FS-LASIK group, the mean inner nuclear layer (INL) thickness one day after the surgery was significantly higher than that before surgery in the foveal (21.22 ± 4.66 µm vs 19.03 ± 4.50 µm, P= .013) and parafoveal regions (41.98 ± 3.70 µm vs 40.56 ± 3.49 µm, P= .044).The findings of our study suggest that the increase of INL thickness may be due to temporary structural and circulatory changes of the retina that may occur in the suction phase in the FS-LASIK procedure.

Evaluating Macular Retinal Thickness and Retinal Nerve Fiber Layer Thickness in the Early Stage after Femtosecond Laser-Assisted Laser in Situ Keratomileusis

Abstract:Objective To evaluate the changes of retinal nerve fiber layer and macular parameters after femtosecond laser-assisted laser in situ keratomileusis (fs-LASIK) assessed by spectral-domain optical coherence tomography (SD-OCT).Methods This prospective,consecutive study included 17 patients (33 eyes) with high (greater than-6.00 D) myopia treated with fs-LASIK.The macular thickness and volume and RNFLT measurements were performed using RTVue-100 OCT preoperatively and then on 1 day,1 week and 1 month postoperation.The measurements before and after fs-LASIK were compared using one-way analysis of variance (ANOVA).Results None of the RNFL parameters of whole,temporal,upper,nasal and below were observed to have a statistically significant change after fs-LASIK surgery (F=0.003,0.503,0.285,0.164,0.277,P＞0.05).Also,there was no statistically significant difference in macular thickness (F=0.491,0.281,0.234,0.341,0.614,0.192,P＞0.05) and volum (F=0.484,0.968,0.200,0.314,0.622,0.206,P＞0.05) of central fovea,parafovea,temporal,upper,nasal and below between pre-and postoperatively.Conclusion Femtosecond laser-assisted laser in situ keratomileusis has no significant effect on retinal nerve fiber layer and macular parameters determined by SD-OCT.
Evolution of corneal thickness and optical density after laser in situ keratomileusis versus small incision lenticule extraction for myopia correction

Abstract:Purpose To compare the thickness and stromal optical density (OD) evolution of the flap and cap after femtosecond (FS-LASIK; Visumax) and small incision lenticule extraction (SMILE), respectively, for myopia correction. Methods A prospective study comprising 78 eyes (n=39 per group) was performed. Anterior segment optical coherence tomography (MS39) images were collected at each postoperative visit (1 day, 1 week and 1 and 3 months) and flap/cap thicknesses were measured. Using ImageJ software, three regions were defined to measure OD (in grayscale units: flap/cap stroma, residual stromal bed (RSB) and all stroma (including flap/cap-stroma, surgical interface and RSB). Results Mean central thickness significantly increased during the 3-month follow-up in both LASIK (5.43±4.23 μm; p=0.0118) and SMILE (2.76±5.61 μm; p=0.0118), with differences between both techniques statistically significant (p=0.020). All OD values showed a statistically significant reduction during SMILE follow-up: cap 48.96±5.55 versus 44.95±5.41 (p=0.19×10 −6 ), RSB 50.26±7.06 versus 45.42±7.53 (p=0.00005), total stroma 50.34±6.49 versus 45.46±6.96 (p=0.5×10 −5 ) at 1 day and 3 months, respectively, whereas no statistically significant changes were found during LASIK follow-up. No significant differences were observed in OD values between both techniques at any time point, although SMILE showed a tendency for higher OD values than LASIK within the first postoperative month. Visumax presented a tendency for thicker caps than target (11.48±7.85 μm), but not for flaps (2.73±8.93 μm) (p=0.00003). Conclusions Both LASIK flaps and SMILE caps show a significant postoperative rethickening. SMILE corneas present higher optical densities than LASIK corneas in the early postoperative period, with a significant decrease thereafter and up to 3 months. These findings correlate with the delayed visual recovery observed after SMILE.

Effect of Flap Thickness on Higher Order Wavefront Aberrations Induced by LASIK: a Bilateral Study.

Abstract:PURPOSE: To investigate the effect of flap thickness on wavefront aberrations induced by LASIK.METHODS: LASIK was performed on 56 eyes of 28 patients with refraction errors that were well matched between the right and left eyes. For each patient, a planned 160-mu m flap was created for one randomly selected eye using the Moria M2 microkeratome (130-mu m head; thick flap group), and a 110-mu m flap was created for the contralateral eye with the same microkeratome (90-mu m head; thin flap group). Flap thickness was measured using ultrasonic pachymetry Wavefront aberrations were measured in the anterior cornea using the Orbscan II (Bausch & Lomb) and in the whole eye using the Wavefront Supported Custom Ablation (WASCA) aberrometer (Carl Zeiss Meditec) preoperatively and at 1 month and 1 year postoperatively.RESULTS: Mean flap thickness was 155 +/- 13 mu m in the thick flap group and 112 +/- 11 mu m in the thin flap group. Mean root-mean-square of higher order wavefront aberrations were changed for the different test times in the cornea (F=29.9, P<.0001) and the whole eye (F=48.0, P<.0001). There was no significant difference between the two flap groups for the cornea (F=0.76) or for the whole eye (F=0.07). Similar results were observed for higher order Zernike aberrations such as spherical aberration and comas.CONCLUSIONS: The results suggest higher order wave-front aberrations induced by LASIK are independent of flap thickness. Complications in visual outcome for patients with different flap thickness should be attributed to factors other than aberrations.
Corneal sensitivity and ocular surface health in patients undergoing femtosecond LASIK retreatment for residual refraction

Abstract:Purpose: To assess corneal sensitivity and the ocular surface in patients undergoing primary femtosecond laser in situ keratomileusis (FS-LASIK) and those undergoing FS-LASIK retreatment under the same flap due to residual refractive error. Setting: Ekol Eye Hospital, Izmir, Turkey. Design: Prospective case series. Methods: 19 patients with previous FS-LASIK who had myopic and astigmatic refractive error were included in the study group, and 19 age-matched and sex-matched patients undergoing FS-LASIK for the first time as the control group. Corneal sensitivity, Schirmer test, tear film break-up time (TBUT), Oxford grading scheme for ocular surface staining, and Ocular Surface Disease Index (OSDI) were measured preoperatively and at postoperative 1 week and 1, 3, and 6 months. Results: The mean refractive correction in the study and control groups, respectively, was 2.18 ± 0.78 diopters (D) (range: 1.00 to 3.50) and 2.76 ± 1.20 D (range: 1.00 to 4.50; P = .07). Corneal esthesiometry results in the study and control groups, respectively, were 6.10 ± 12.55 vs 9.90 ± 11.50 mm at 1 week ( P = .001), 41.95 ± 6.98 vs 45.09 ± 5.88 mm at 1 month ( P = .004), 56.09 ± 3.37 vs 56.19 ± 2.52 mm at 3 months ( P = .8), and 58.60 ± 2.01 vs 58.80 ± 1.39 mm at 6 months ( P = .5). Significant difference between the 2 groups in Schirmer test score that disappeared at postoperative 3 months and in TBUT and ocular surface staining that disappeared at 6 months, whereas the statistically significant difference in OSDI score persisted at 6 months ( P = .03) was detected. Conclusions: Corneal surface sensitivity and ocular surface health are more impaired in patients undergoing FS-LASIK retreatment due to residual refractive error.

Abstract:Purpose: In this retrospective study, the efficacy of the FEMTO LDV Z8 Femtosecond Laser-Assisted Cataract Surgery (Femto Z8 FLACS) and the Femtosecond laser Arcuate Keratotomy (FSAK) in decreasing the corneal astigmatism was investigated. Methods: During FLACS, FSAK was positioned with the help of the FEMTO LDV Z8 laser at a diameter of 8.5 mm and an 80% depth. Before and 3 months after surgery, the astigmatism of the cornea was measured with the aid of Pentacam. The variables influencing the efficacy of FSAK were assessed using the multiple regression analysis technique. Vector analyses were carried out. To determine the net corneal alterations, the with-the-wound and against-the-wound variations were computed along the FSAKs' meridian. Results: This study investigated 80 eyes from 62 participants. The average keratometric astigmatism value was 0.92 ± 0.44 diopters (D). The average keratometric astigmatism decreased to 0.61 ± 0.45 D 3 months following FSAK compared to preoperative corneal astigmatism ( p < 0.05). Additionally, there was a considerable decline in the percentage of eyes with ±0.5 D and ±1.0 D astigmatism, which reduced 3 months after surgery by 58% and 85%, respectively ( p < 0.05). Conclusion: The FEMTO LDV Z8 laser can create an effective and precise arcuate keratotomy with an excellent safety profile, rapid recovery, and vision stability.

Abstract:Purpose: To evaluate the effect of femtosecond laser-assisted in situ keratomileusis (FS-LASIK) on retinal fovea thickness, volume, and retinal nerve fibre layer (RNFL) thickness. Methods: Thirty-seven eyes (37 patients) undergoing FS-LASIK were included in this prospective study. Optical coherence tomography (OCT) was performed 1 day before, 1 h and 1 day after FS-LASIK surgery. Result: Eighteen male and nineteen females were enrolled. Mean patient age was 22.94 ± 4.22 years. One hour postoperatively, macula fovea thicknesses, macula fovea volume, macula parafovea thickness, macula parafovea volume, macula perifovea thickness, macula perifove volume, temporal RNFL thickness, and superior RNFL thickness measures showed significant decrease ( t = 6.171, 6.032, and 9.837, 9.700, 2.532, 4.393, 4.926, 2.265; p = 0.000, 0.000, 0.000, 0.000, 0.016, 0.000, 0.000, and 0.011). Day 1 post-operation, macula fovea thicknesses, macula fovea volume, macula parafovea thickness, macula parafovea volume, and inferior RNFL thickness measures showed significant change compared to preoperative measures ( t = 3.620, 3.220, 2.901, 2.910, 3.632; p = 0.001, 0.003, 0.006, 0.006, and 0.001). Conclusion: Our data suggest there are alterations in retinal foveal and RNFL measurements by OCT 1 h and 1 day after FS-LASIK surgery.

Ocular Aberrations and Corneal Thickness-Intraocular Pressure Relationship One Year after Laser in situ Keratomileusis (LASIK) Enhancement with An Aspheric Ablation Profile

Abstract:Objective: To evaluate the effect of an aspheric LASIK refractive enhancement performed by relifting the original flap on the higher order aberrations (HOA) of the eye and the relationship between central corneal thickness (CCT) and intra-ocular pressure (IOP). Methods: Consecutive case series of 30 eyes requiring refractive enhancement from 1804 uncomplicated primary LASIK procedures performed in 2012. Flap relift and wavefront optimized ablation (Wavelight Allegretto Eye Q 400 Hz excimer laser) were performed in all cases. Pre-and post-operative examination included Shack-Hartmann aberrometry for 3 mm and 5 mm pupil sizes, CCT and IOP. Postoperative results at 1 year were compared to baseline values. Results: Changes in average values of coma, spherical aberration (SA), trefoil, CCT and IOP were not significant. Linear regression revealed significant associations between change (y) in trefoil and pre-enhancement value of trefoil for 3 mm pupil (0.828x-0.045, r=0.722, p<0.001), coma and pre-enhancement value of coma for 5 mm pupil (y=0.281x-0.030, r=0.501, n=30, p=0.048), SA for 3 mm pupil compared with change in CT (y=0.0080-0.0009x, r=-0.378, p=0.0392), and SA for 5 mm pupil compared with change in CCT (y=0.0035x-0.0541, r=0.524, p=0.0029). Also there was a significant association between IOP and CCT both pre-(IOP=0.0313CT-3.3, r=0.740, p<0.0001) and post-enhancement (IOP=0.0243CT-0.018, r=0.675, p<0.0001). Conclusion: LASIK enhancement with an aspheric ablation profile did not significantly impact on the average values for HOAs, IOP or CCT. For individual cases, the likely shift in magnitude of coma, trefoil and SA can be estimated and, the change in SA is correlated with changes in CCT. The change in the abscissa value linking IOP with CCT may be an indication of changes in biomechanical properties of the cornea in the central region.

Abstract:Objective To investigate the predictability of ultra-thin corneal flap thickness created with VisuMax femtosecond laser in high myopia and to identify the factors affecting flap thickness.Methods A prospective clinical study involved 21 eyes of 11 high myopia patients who underwent femtosecond laser in situ keratomileusis (LASIK).The VisuMax femtosecond laser was used to create ultra-thin corneal flaps with an intended central flap thickness of 85 μm. The preoperative central corneal thickness (CCT) and residual stromal bed thickness (RST) after corneal flap lift were measured by ultrasound pachymetry.The actual flap thickness was calculated as the difference between preoperative CCT and intraoperative RST after flap lift.Pearson correlation analysis was used to compare actual flap thickness and data that included preoperative intraocular pressure,spherical equivalent refractive error (SE),keratometry,horizontal corneal diameter and preoperative CCT.Results The mean intraoperative corneal flap thickness was (96.0±10.5)μm for all eyes.Corneal flap thickness was not significantly correlated to preoperative SE,intraocular pressure,keratometry or horizontal corneal diameter,but was significantly correlated to preoperative CCT (r=0.512,P=O.018).Conclusion The thickness of ultra-thin corneal flaps created with VisuMax femtosecond laser in high myopia is essentially predictable.The intended 85 μm corneal flap thickness yields an actual flap thickness that is significantly correlated to preoperative CCT.

Abstract:Optical coherence tomography (OCT) has gained momentum on segmenting anterior corneal substructures, such as treatment-induced flaps and lenticule cuts created by femtosecond lasers. However, recent semi-automated methods rely on manual markers, which can introduce bias and variability. In this work, we present an automated algorithm designed to overcome the limitations in the geometric quantification of intrastromal cuts produced by two different femtosecond lasers, using a unique imaging system. Our analysis, conducted on peri-operative segmentations of goat eyes, aims to demonstrate the method's potential for contributing to ongoing efforts and enhancing clinical outcomes in refractive surgery treatment.

LASIK flap thickness profile and reproducibility of the standard vs zero compression Hansatome microkeratomes: three-dimensional display with Artemis VHF digital ultrasound

Abstract:Purpose: To measure the mean flap thickness profile and calculate accuracy and reproducibility for standard and zero compression Hansatome microkeratomes (Bausch & Lomb). Methods: Retrospective study of 99 myopic eyes (51 patients) after LASIK using the standard or zero compression Hansatome microkeratomes (160-μm head). Reinstein Flap Thickness was calculated as preoperative epithelial thickness plus postoperative stromal component of the flap as measured by Artemis VHF digital ultrasound (ArcScan Inc). Mean and standard deviation flap thickness were calculated and plotted for a 10-mm diameter. Accuracy was calculated as the difference between the mean and intended flap thickness. Reproducibility was evaluated as the flap thickness standard deviation between eyes. Multivariate regression was performed for flap thickness against surgery order, atmospheric/vacuum pressure difference, spherical equivalent refraction, pachymetry, and keratometry. Results: For both Hansatome models, the mean flap thickness profile showed a thinnest region inferotemporally and thicker in the periphery of the inferotemporal edge and nasally. Mean central flap thickness was 140.3±17.5 μm (accuracy: -19.7 μm) and 119.0±12.7 μm (accuracy: -41.0 μm) for standard and zero compression Hansatomes, respectively. Eyes treated second were thinner (15.4 μm and 12.6 μm thinner for standard and zero compression Hansatomes, respectively). Multivariate regression improved the reproducibility to 14.8 μm (P<.05; surgery order, pachymetry) and 9.4 μm (P<.05; surgery order, atmospheric/vacuum pressure difference) for standard and zero compression Hansatomes, respectively. Conclusions: The flap thickness profiles were similar for standard and zero compression Hansatomes characterized by a thinnest region inferotemporally, rather than a meniscus shape as has been suggested previously. The zero compression Hansatome created thinner, more reproducible flaps. Surgery order and atmospheric/vacuum pressure difference might be used to obtain thinner flaps using a Hansatome.

Abstract:This study assessed the clinical safety and efficacy of elliptical thin-flap LASIK with a low–pulse energy femtosecond laser in 3 thickness subgroups (85, 90, and 100 μm). A total of 80 patients who underwent bilateral LASIK surgery at Taiwan between April and September 2019 were retrospectively enrolled. Elliptical corneal flaps with wide temporal hinges and inverted-angled side cuts were created. Target flap thickness was calculated on the basis of residual stromal bed thickness and percent tissue altered. Before flap creation, an optical coherence tomography image for visualization of the precut flap position was obtained with the built-in camera. At postoperative month 1, the overall mean logMAR uncorrected distance visual acuity (UDVA) was − 0.04 ± 0.07 (20/18 Snellen), with 96% of eyes achieving UDVA of 20/20 or better. Postoperative mean manifest spherical equivalent was − 0.37 ± 0.42 D at 1 month. The visual and refractive outcomes in each of the 3 subgroups were similar. The achieved flap thicknesses were found to be highly predictable and consistent in the respective thickness subgroups. Optical coherence tomography–guided thin-flap LASIK performed with the low–pulse energy femtosecond laser was found to be safe with no intraoperative or postoperative complications.

Femtosecond laser versus mechanical microkeratome: a retrospective comparison of visual outcomes at 3 months

Abstract:Purpose: To compare the visual outcomes of LASIK procedures in which flaps were created with a femtosecond laser (IntraLase FS 60Hz, Abbott Medical Optics [AMO]) to procedures in which flaps were created with a mechanical microkeratome (Moria Evo3 One Use-Plus, Moria SA). Methods: A retrospective analysis was performed on 2000 eyes treated in 2008 for low myopia and astigmatism (sphere < -3.00 diopters [D]; cylinder < or = -0.75 D). The first 1000 consecutive eyes that had LASIK flaps created with a femtosecond laser were compared with the first 1000 consecutive eyes that had flaps created with a mechanical microkeratome. All eyes received wavefront-guided LASIK treatments performed with a VISX S4 IR Advanced CustomVue excimer laser (AMO). Refractive predictability, change in mean spherical equivalent refraction, postoperative uncorrected visual acuity (UCVA), and loss of best spectacle-corrected visual acuity (BSCVA) were compared at 1 day, 1 week, 1 month, and 3 months following surgery. Results: The refractive accuracy was the same for both groups. At all time points measured, the percentage of eyes that achieved a postoperative UCVA of 20/20 or better was significantly higher in the femtosecond laser group than in the mechanical keratome group. Also, a higher percentage of eyes in the femtosecond laser group achieved a postoperative UCVA of 20/16 at 3 months. Finally, a lower percentage of eyes in the femtosecond laser group lost two or more lines of BSCVA at 1 week and 1 month postoperative. Conclusions: Creating LASIK flaps with the femtosecond laser resulted in faster visual recovery and better UCVA.
