Abstract:Although experiential avoidance is emphasized as an underlying factor for persevering worry, limited empirical attempt exists to integrate this concept with cognitive models explaining pathological worry. Therefore, the current study aims to investigate the mediator role of different experiential avoidance strategies in the relationship between metacognitions and worry-related symptoms. The study was conducted with a community sample consisting of 563 volunteer participants (405 females, 158 males) whose ages ranged between 18 and 40 (M = 25.56, SD = 4.72). Online data collection of self-report measures including demographic information form, Metacognitions Questionnaire-30, Multidimensional Experiential Avoidance Questionnaire, and Penn State Worry Questionnaire was applied. The results of the hierarchical regression analysis revealed that experiential avoidance predicted worry beyond metacognition. In addition, particular avoidant strategies, aversion/avoidance, procrastination, distraction, and distress endurance explained the associations between positive and negative metacognitive beliefs and worry. Current findings point to different avoidant strategies that might play a role in the intensification and maintenance of worry and suggest the benefits of incorporating acceptance and commitment-based interventions with metacognitive therapy for better therapeutic outcomes.

Abstract:The present paper reports two questionnaire studies and an experimental investigation of worrying. Worry correlated more highly than general tension reports with a variety of affect scales and was characterized by: (a) feelings of anxiety, tension and apprehension; (b) moderate awareness of somatic cues including muscle tension and upset stomach; and (c) concerns over future rather than past or present situations. Self-labeled worriers were most distinguished from non-worriers by their reported uncontrollability of cognitive intrusions once worrying was initiated. In the experimental study, worriers and non-worriers were assessed for their ability to focus their attention on a monotonous (breathing) stimulus before and after 0, 15 or 30 min of worrying. Worriers reported significantly higher anxiety. depression and hostility, a lower frequency of focused attention and a greater frequency of negative thought intrusions than did non-worriers prior to the worry period. For both worriers and non-worriers. 15 min of worry resulted in an apparent incubation of negative cognitive intrusions on the subsequent attention-focusing task.

Abstract:Purpose: Worry is an intuitive sense that goes beyond logical reasoning and is valuable in situations where patients' conditions are rapidly changing or when objective data may not fully capture the complexity of a patient's situation. Nurse anesthetists' subjective reasons for worry are quite vague as they are valued inconsistently and not accurately expressed. This study aimed to identify factors playing a role in the emergence of worry during anesthesia practice to clarify its concept. Design: Mixed-methods design consisting of quantitative online surveys followed by qualitative focus group interviews including Dutch nurse anesthetists. Methods: Both quantitative and qualitative thematic analyses were performed, followed by data and methodological triangulation to enhance the validity and credibility of findings and mitigate the presence of bias. Findings: Surveys (N = 102) were analyzed, and 14 nurse anesthetists participated in the focus group interviews. A total of 89% of the survey respondents reported that at least once have had the feeling of worry, of which 92% use worry during clinical anesthesia practice. Worry was mentioned to be a vital element during anesthesia practice that makes it possible to take precautionary actions to change the anesthetic care plan in a changing situation or patient deterioration. Conclusions: While a clear definition of worry could not be given, it is a valuable element of anesthesia practice as it serves as a catalyst for critical thinking, problem-solving, clinical reasoning, and decision-making. Use of the feeling of worry alongside technological systems to make an informed decision is crucial. Technology has significantly improved the ability of health care providers to detect and respond to patient deterioration promptly, but it is crucial for nurse anesthetists to use their feeling of worry or intuition alongside technological systems and evidence-based practice to ensure quick assessments or judgments based on experience, knowledge, and observations in clinical practice.

Abstract:BackgroundAlthough worry is well-known to share a robust relationship with psychopathology, whether this association is higher in generalized anxiety disorder (GAD) versus other anxiety disorders and depressive disorder, and clinical versus nonclinical populations is unclear.MethodsWe conducted a meta-analysis in which the association between worry and various mental health indicators was quantitatively summarized using a random-effects model; additionally, several moderators were tested.ResultsResults synthesizing data from 120 correlational studies (138 independent samples) conducted over the past 35 years indicate a strong association between worry and poor mental health (r = .53) but also high heterogeneity. This association was not systematically impacted by the study population or whether individuals had a diagnosis of GAD or major depressive disorder (MDD). Participants’ age and how worry and mental health were measured moderated this association.ConclusionsThe main findings suggest that (1) worry is strongly associated with poor mental health, (2) worry is associated with poor mental health even outside of GAD and clinical populations included in the current meta-analysis, and (3) there are differences in this association across the lifespan. Overall, the results from this meta-analysis support earlier calls for intense worry to be assessed and treated, regardless of whether the individual has a diagnosis of GAD or MDD.

Understanding the experience of rumination and worry: A descriptive qualitative survey study

Abstract:Objectives: Rumination and worry have been implicated in the onset, severity, maintenance and relapse risk of depression and anxiety disorders. Despite this, little research has examined individuals' personal experiences of these processes. This study investigates how individuals experience these processes, which will provide insight into these common features of mental disorders and inform the development of an online intervention specifically targeting rumination and worry. Design: An online qualitative survey was conducted to gain insight into people's personal definitions, experiences with and understandings of rumination and worry. Methods: Participants answered open- and close-ended questions about their personal understanding of rumination and worry, typical thought content, triggers, frequency, duration and coping strategies. Participant responses were coded into themes. Participants also completed self-report questionnaires of depression, anxiety and stress and repetitive negative thinking. Results: Two hundred and seven adults completed the online survey (76% female; mean age = 28.2 years, range = 17-71), 51% of whom reported previously experiencing depression and anxiety. All participants were familiar with the concept of worry, whereas 28% of participants indicated they had never heard of rumination. Participants reported most commonly ruminating and/or worrying about personal relationships, past mistakes, negative experiences and conversations/social interactions. The most commonly reported triggers for rumination and/or worry were social situations/interpersonal interactions (25%) and negative events/experiences (24%). Distraction was the most common coping strategy (48%); however, 21% reported being unable to stop themselves from ruminating and/or worrying. Conclusions: The results provide a unique insight into the personal experiences and understandings of rumination and worry of potential end users of treatment programs targeting these processes.
Self‐guided imagery rescripting for worry images: A preliminary experimental investigation

Abstract:Background Mental images of feared events are overactive and intrusive in generalized anxiety disorder (GAD). Imagery rescripting involves integration of positive or neutral imagery and corrective information into images to facilitate emotional processing, reduce imagery intrusions, and re‐structure underlying schema. Yet only one known study has applied the technique to treatment of worry. The present study aimed first to examine the relationship between trait worry and properties of future‐oriented worry images, and second to examine the efficacy of a self‐guided imagery rescripting intervention in improving individuals' response to their worries. Methods Participants recruited through Amazon Mechanical Turk (N = 365) identified their major worry and wrote the script of a worst‐case scenario mental image. Participants were randomized to three conditions: re‐writing the same worry image script (exposure), or writing scripts of either one or three positive alternative future‐oriented images (rescripting conditions). Results In preliminary analyses, trait worry negatively predicted participants' ratings of worry images, including valence and ability to cope, and positively predicted distress, anticipated cost, and belief in their negative meaning. In experimental analyses, linear mixed‐effects models revealed anxious response and cognitive appraisal of the threat were significantly lower among participants allocated to rescripting relative to exposure. There was no effect of rescripting type. Conclusions This investigation demonstrated the impact of a future‐oriented imagery rescripting task on anxiety and cognitive biases associated with real worries in an unselected sample. Results may contribute to the development of imagery rescripting interventions for GAD.

Abstract:Abstract Background Previous research has suggested that worry is negatively associated with working memory performance. However, it is unclear whether these findings would replicate across different worry levels and in individuals with anxiety and depressive disorders (i.e. clinical statuses). Method One-hundred-thirty-eight participants performed a two-block working memory task (150 trials per block). Based on participants` current clinical status, four groups were considered (generalised anxiety disorder group: n = 36; clinical group with another anxiety or mood disorders: n = 33; subclinical group: n = 27; control group: n = 42). Trait worry levels were collected from all of the participants. Working memory performance was measured as accuracy and reaction time. Results During the first block, higher worry scores were significantly associated with longer reaction times. Moreover, the generalised anxiety disorder group, clinical group, and subclinical groups demonstrated significantly longer reaction times compared to the control group in Block 1, when age was controlled for. From Block 1 to Block 2, all of the participants demonstrated a significant decrease in accuracy and reaction time, regardless of worry level or clinical status. Conclusion The results indicate that higher worry levels negatively impact WM processing efficiency. Moreover, when age was controlled for, we found participants` clinical status to be linked with WM impairments. The results highlight the relevance of investigating the impact of different worry levels on cognitive processes across clinical and non-clinical populations.

Worrying About Finances During COVID-19: Resiliency Enhances the Effect of Worrying on Both Proactive Behavior and Stress

Abstract:Amidst the COVID-19 pandemic and resulting economic instability, many people are contending with financial insecurity. Guided by Conservation of Resources Theory (Hobfoll, American Psychologist 44:513-524, 1989; Hobfoll et al., Annual Review of Organizational Psychology and Organizational Behavior 5:103-128, 2018), the current research explores the consequences of experiencing financial insecurity during a pandemic, with a focus on individuals who report relatively higher rates of financial insecurity, performance challenges, and stress during such experiences: working parents (American Psychological Association, 2022). This research also examines the role that personal resources, in the form of trait resiliency, play in the relationships between financial insecurity and behavioral and psychological outcomes including worrying, proactive behaviors, and stress. In a study of 636 working parents and their children, we find that financial insecurity heightens worrying, underscoring the threatening nature of the loss or anticipated loss of material resources. Worrying, in turn, promotes proactive behaviors at work-an effect that is more pronounced among high-resiliency individuals. However, worrying is also associated with elevated stress among high-resiliency individuals, providing support for a trait activation perspective (rather than buffering hypotheses) on ongoing, uncontrollable adversities. Taken together, our results help to (1) illuminate the impact of financial insecurity on work and well-being, (2) reveal a mechanism (i.e., worrying) that helps explain the links between financial insecurity and work and personal outcomes, and (3) expand our knowledge of the implications trait resiliency has for both psychological and behavioral reactions to ongoing crises.

Abstract:Individuals higher in trait worry exhibit increased activation in Broca’s area during inhibitory processing tasks. To identify whether such activity represents an adaptive mechanism supporting top-down control, we investigated functional and effective connectivity of Broca’s area during a task of inhibitory control. Functional MRI data obtained from 106 participants performing an emotion-word Stroop task were examined using psychophysiological interaction and Granger causality (GC) analyses. Findings revealed greater directed connectivity from Broca’s area to amygdala in the presence of emotional distraction. Furthermore, a predictive relationship was observed between worry and the asymmetry in effective connectivity; worriers exhibited greater directed connectivity from Broca’s area to amygdala. When performing the task, worriers with greater GC directional asymmetry were more accurate than worriers with less asymmetry. Present findings indicate that individuals with elevated trait worry use a mechanism of top-down control in which communication from Broca’s area to amygdala fosters successful compensation for interference effects.

Abstract:Ample work has already been conducted on worry and rumination as negative thought processes involved in the etiology of most of the anxiety and mood related disorders. However, minimal effort has been exerted to investigate whether one type of negative thought process can make way for another type of negative thought process, and if so, how it subsequently results in experiencing a host of symptoms reflective of one or the other type of psychological distress. Therefore, the present study was taken up to investigate whether rumination mediates the relationship between worry and generalized anxiety disorder (GAD), and between worry and obsessive compulsive disorder (OCD) in two clinical groups. Self-report questionnaires tapping worry, rumination, generalized anxiety disorder (GAD), and obsessive compulsive disorder (OCD) were administered to a clinical sample of 60 patients aged 30-40. Worry, rumination, generalized anxiety disorder (GAD), and obsessive compulsive disorder (OCD) correlated substantially with each other, however, rumination did not mediate the relationship between worry and generalized anxiety disorder (GAD) and between worry and obsessive compulsive disorder (OCD). We also analyzed differences of outcome variables within two clinical groups. These results showed that worry and rumination were significantly different between GAD and OCD groups.

Abstract:Interpersonal problems are common in individuals with generalized anxiety disorder (GAD) and improved interpersonal functioning is an important goal in treatment. In the metacognitive model of worry and generalized anxiety disorder, emotional distress and psychological dysfunction are related to two distinct types of worry. Particular emphasis is put on meta-worry which refers to "worry about worrying" or appraisal of worrying as threatening in itself. Meta-worry impairs effective mental regulation and might therefore be an important predisposition to self-regulate via others and thereof interpersonal problems. In the current study, we tested the prediction that meta-worry is uniquely associated with interpersonal problems in two samples. The first consisted of 135 individuals with analogue GAD and the second of 56 individuals with a diagnosis of GAD. Using hierarchical linear regressions controlling for anxiety, depression and worry, meta-worry showed a unique relationship with interpersonal problems in both samples. These findings indicate that meta-worry may be relevant to formulate and treat interpersonal problems in individuals with GAD.

Abstract:W orry, characterized by repetitive and involuntary aversive thoughts about perceived concerns, is a central mental health complaint known to negatively impact quality of life of individuals facing perceived threats to health or well-being. Although empirical focus on worry has escalated over the past three decades, there has been less attention to current research trends and knowledge development related to this important phenomenon in nursing. This article presents a biobehavioral conceptual framework of worry for nursing application that is derived from a concept synthesis of biological, behavioral, clinical, and cognitive psychology sources. Databases including CINAHL, EBM Review, Health & Wellness Resource Center, PsycINFO, and PubMed were systematically reviewed for the years 2000–2012. The adapted conceptual framework describes relationships among perceived threat; process, structure, and content worry attributes; and outcomes of worry. The framework may be applied in clinical environments as a resource to better understand and care for patients facing perceived threats to health and well-being. The framework contributes a foundation to build empirical knowledge for nursing practice, theory, and research that has potential to improve patient well-being and health-related quality of life outcomes.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Rumination and worry are repetitive negative thinking (RNT) tendencies that contribute to the development and maintenance of internalizing psychopathologies. Accruing data suggest rumination and worry represent overlapping and unique transdiagnostic cognitive processes. Yet, prior neuroimaging research has mostly focused on rumination in depression, which points to involvement of resting‐state brain activity in default mode, executive, salience, and/or affective networks.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>The current study examined relations between brain activity during rest and RNT in a transdiagnostic sample. Resting‐state fMRI data was analyzed in 80 unmedicated patients with internalizing conditions. Regression analysis, controlling for anxiety and depression symptoms, was performed with seed regions implicated in default mode, executive, salience, and affective networks. Rumination and worry were assessed with standard self‐report measures.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Whole‐brain regression results showed more rumination and worry jointly corresponded with greater positive resting‐state functional connectivity (rsFC) between the amygdala and prefrontal regions (i.e., middle frontal gyrus, inferior frontal gyrus). Conversely, more worry (controlling for rumination) corresponded with greater negative rsFC between amygdala and precuneus. No significant results were observed for rumination alone (controlling for worry).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Findings indicate the affective network plays a role in RNT, and distinct patterns of connectivity between amygdala and regions implicated in the executive and default mode networks were observed across patients with internalizing conditions. Results suggest different mechanisms contribute to RNT as a unitary construct and worry as a unique construct.</p></section>

Abstract:Repetitive Negative Thinking (RNT) encompasses both worry and rumination and is a transdiagnostic process. A systematic review and meta-analysis were co-produced with individuals with lived experience of anxiety and depression. The objective was to examine the efficacy of RNT-specific psychological interventions. Inclusion criteria were mean age or range 10–24 years, and including a measure of worry, rumination, RNT, anxiety, depression, psychosis, self-harm, or suicide. The Cochrane Risk of Bias measure was used. PsycInfo, Medline/PubMed, Scopus and Embase databases were searched on 24 April 2023. There were 16 studies with 20 RNT interventions, N = 1,183 participants, 79% female, mean pooled age = 20.08 years. There was a small effect of RNT interventions on anxiety symptoms ( g = −0.43 to −0.47, 95% CI = −0.23 to −0.70). There were small-to-medium effects on depressive symptoms ( g = −0.40 to −0.52, 95% CI = −0.19 to −0.84), and RNT (g = −0.59 to −0.71, 95% CI = −0.41 to −1.01). Most studies indicated some concerns for risk of bias. Transdiagnostic interventions that target RNT are efficacious in reducing symptoms of anxiety, depression, and RNT in young people. Further research is required to address younger age groups, psychosis, and different cultural contexts.

Abstract:Background: Thought processes such as worries are often described as difficult to control and predict suicidal thoughts and behaviors (STBs). Due to their uncontrollable nature, worries may lead to STBs as people attempt to escape their own internal thought processes. Objective: Examine the indirect role of perceived uncontrollability of one's thoughts in the relationship between worry and STBs. Design and method: We used stepwise mediation models to examine the proposed relationship in a sample of 145 undergraduates with lifetime suicidal ideation. STBs were categorized into (1) severity of lifetime suicidal ideation, (2) likelihood and (3) severity of recent suicidal ideation, and (4) lifetime suicide attempt. The likelihood and severity of recent ideation were separated due to the skewed nature of suicidal thoughts, with many people reporting zero ideation. Results: Worry was related to the severity of lifetime ideation, the likelihood of reporting recent ideation, and the severity of recent ideation through lower levels of perceived ability to control one's thoughts. Worry was not related to a lifetime suicide attempt. Conclusion: The perception that one's thoughts are uncontrollable may be a core feature of worry for the development and worsening of suicidal thinking. Interventions targeting internal perceptions of uncontrollability may be beneficial for suicidal ideation management.

Method: In total, 27 unmedicated patients with GAD and 56 healthy controls (HC) underwent a differential fear conditioning paradigm during functional magnetic resonance imaging. Results: Compared to HC, the GAD group demonstrated reduced vmPFC activation to safety signals and no safety-threat processing differentiation. This response was positively correlated with worry severity in GAD, whereas the same variables showed a negative and weak correlation in HC. Conclusions: Poor vmPFC safety-threat differentiation might characterise GAD, and its distinctive association with GAD worries suggests a neural-based qualitative difference between healthy and pathological worries.Declaration of interestNone.
The Relationship of Rumination, Worry and OCD Symptoms During Technology Supported Mindfulness Therapy for OCD

Abstract:Background: In this study, we re-examined data from a previous randomized controlled trial investigating 'technology supported mindfulness' (TSM)-an 8-week treatment intervention for individuals experiencing OCD. The current analysis involves an examination of the longitudinal relationships between rumination, worry and OCD symptom changes during mindfulness treatment, in comparison to a waitlist control. Methods: Participants experiencing OCD (n = 71) were randomly assigned to 8 weeks of (1) TSM or (2) waitlist control. We tested the extent to which rumination (using the Ruminative Response Scale) and worry (using the Penn State Worry Questionnaire) are associated with OCD symptom changes during the acute phase of treatment, concurrently (i.e., within the same longitudinal model). Results: Generalized linear model (GLM) results indicated a significant time (week 1 vs. week 8) by condition interaction involving decreased rumination in the TSM condition: F(1, 61) = 13.37, p = 0.001, partial η2 = 0.18 and observed power = 0.94. A second GLM demonstrated decreased worry in the TSM condition: F(1, 69) = 37.34, p = 0.001, partial η2 = 0.35 and observed power = 0.83. Longitudinal 'latent difference' structural equation analyses demonstrated a cross-lagged association between worry (but not rumination) and OCD symptom changes. Conclusions: Individuals in the TSM condition experienced greater reductions in rumination and worry during 8 weeks of TSM treatment compared to the waitlist control, and reduced worry predicted subsequent OCD symptom reduction.

Abstract:Studies investigating the reciprocal association between worry and sleep have yielded mixed findings and suffered from methodological limitations. The purpose of this study was to assess the reciprocal association between worry and various indices of sleep (i.e., sleep onset latency (SOL), total sleep duration (TSD), sleep quality (SQ), and morning lingering (ML)). In a longitudinal daily assessment study, participants ( N =182) completed a sleep diary and a measure of worrisome thinking three times per day for seven days. We used linear mixed effects models to examine worry predicting sleep, and Tobit regression models to examine sleep predicting worry, and controlled for previous day report of the outcome variable and person- and day- level effects. Results indicate that greater person-level worry, but not day-level worry, was significantly associated with longer SOL, worse SQ, and longer ML. Furthermore, greater person-level SOL, TSD, and ML was associated with greater worry, and greater person-level SQ was associated with less worry. Finally, we found that shorter day-level TSD and lower day-level SQ was associated with greater next-day worry. These findings support a reciprocal person-level association between worry and sleep (except for TSD), and a unidirectional day-level association between TSD and SQ as predictors of worry.
