Abstract:Introduction: To investigate the ablation efficiency of super-pulse thulium fiber laser (SPTFL) with different laser settings and fiber usage. Materials and Methods: SPTFL machine was attached with different fibers. Artificial stones were fixed in water, whereas laser fiber was driven on a platform for ablation. Pulse energy, frequency, fiber-moving speed, fiber-to-stone distance, and fiber size were adjusted in each trial. The cross-sectional area of craters on the lateral stone surface was measured for comparison of ablation rate, combined with fiber-moving speed. Results: There was a trend that the ablation rate increased as pulse energy or frequency increased. When pulse energy was set as 0.2 J and frequency was increased from 50 to 150 Hz, the cross-sectional area of the crater was enlarged from 0.21 to 0.37 mm(2) (p < 0.05); when the frequency was set as 100 Hz and pulse energy was increased from 0.1 to 0.3 J, the crater was enlarged from 0.10 to 0.45 mm(2) (p < 0.05). Furthermore, energy demonstrated greater impact on ablation rate and the crater was enlarged from 0.20 mm(2) in the 0.1 J x 300 Hz group to 0.44 mm(2) in the 0.3 J x 100 Hz group (p < 0.05). Then fiber was set at different moving speeds with the same laser setting; the ablation rate of 3 mm/second group was 3.64 times higher than 0.5 mm/second group (p < 0.05). Ablation diminished as fiber-to-stone distance grew. A 200 mu m fiber produced thinner and deeper fissure than 272 and 550 mu m fibers, and the ablation rate was the highest for the 200 mu m fiber. Conclusion: Pulse energy is a more important factor in influencing ablation efficiency compared with frequency. Closer fiber-to-stone distance, faster fiber movement, and smaller fiber size increase ablation efficiency.
Safety of a Novel Thulium Fiber Laser for Lithotripsy: an in Vitro Study on the Thermal Effect and Its Impact Factor.

Abstract:Introduction: To investigate the thermal effect on the water by a novel thulium fiber laser (TFL) designed for lithotripsy and evaluate the safety of this laser for clinical use. Materials and Methods: An in vitro experimental setup was constructed. A test tube filled with saline was immersed in an electric water bath, and a TFL fiber and a thermal probe were inserted into it. Saline was irrigated into the tube and pumped out synchronously at the same speed by two pumps, respectively, to maintain convection when needed. Then, continuous TFL firing of different power settings was imposed to saline in the tube for 60 seconds, on the conditions of different irrigation rates. The temperature was recorded every 5 seconds during the whole trial, and each trial was repeated five times. Safety threshold of temperature increase (STTI) was determined comparing with the deemed safe temperature of 43 degrees C in vivo. Results: On condition of 0 mL/min irrigation rate, STTI was 6.5 degrees C, and water temperature increase (WTI) caused by >= 15 W settings surpassed STTI after 20 seconds of laser firing; on condition of 15 mL/min irrigation rate, only WTI caused by the highest 30 W power setting surpassed STTI after 45 seconds of laser firing. When irrigation rate was added up to 25 and 50 mL/min, WTIs caused by all power settings were below STTIs in a 60-second experiment. High frequency and low pulse energy combinations caused a slightly higher WTI compared with low frequency and high pulse energy, given a constant power and irrigation rate. Conclusion: Power setting and irrigation rate collaboratively play a critical role in WTI during TFL lithotripsy, and it is safe to use TFL referring to the thermal effect as long as there is moderate irrigation, while TFL power should be lowered enough when irrigation is ceased.
A Fiber-Optic Thermal Source For Laser Surgery Applications

Abstract:A novel fiber-optic thermal source, consisting of a Nd3+ doped fiber tip and a pure YAG crystal fiber that grown by LHPG(Laser heated pedestal growth) method, had been studied for laser surgery applications. Pumping by a CuBr laser, the thermal source generated temperature as high as 520 degreesC with stability of better than 4% (absolute temperature), the thermal response of the thermal source is about 3 seconds, and the temperature can be controlled by the input laser power. Experimental results show that, the fiber-optic thermal source is promising for laser surgery applications, and is expected to avoid overheating that likely to happen in direct laser tissue interactions.
In vitro investigation of stone ablation efficiency, char formation, spark generation, and damage mechanism produced by thulium fiber laser

Abstract:To investigate stone ablation characteristics of thulium fiber laser (TFL), BegoStone phantoms were spot-treated in water at various fiber tip-to-stone standoff distances (SDs, 0.5 ~ 2 mm) over a broad range of pulse energy (Ep, 0.2 ~ 2 J), frequency (F, 5 ~ 150 Hz), and power (P, 10 ~ 30 W) settings. In general, the ablation speed (mm3/s) in BegoStone decreased with SD and increased with Ep, reaching a peak around 0.8 ~ 1.0 J. Additional experiments with calcium phosphate (CaP), uric acid (UA), and calcium oxalate monohydrate (COM) stones were conducted under two distinctly different settings: 0.2 J/100 Hz and 0.8 J/12 Hz. The concomitant bubble dynamics, spark generation and pressure transients were analyzed. Higher ablation speeds were consistently produced at 0.8 J/12 Hz than at 0.2 J/100 Hz, with CaP stones most difficult yet COM and UA stones easier to ablate. Charring was mostly observed in CaP stones at 0.2 J/100 Hz, accompanied by strong spark-generation, explosive combustion, and diminished pressure transients, but not at 0.8 J/12 Hz. By treating stones in parallel fiber orientation and leveraging the proximity effect of a ureteroscope, the contribution of bubble collapse to stone ablation was found to be substantial (16% ~ 59%) at 0.8 J/12 Hz, but not at 0.2 J/100 Hz. Overall, TFL ablation efficiency is significantly better at high Ep/low F setting, attributable to increased cavitation damage with less char formation.

Abstract:Purpose: To investigate temperature changes around the fibres of the super pulse thulium fibre laser (SP-TFL) during in vitro lithotripsy. Materials and methods: Stones were placed in the in vitro model. The laser was continuously excited for 180 s; the probe was positioned 5 mm around the fibre; the laser power was set at 10, 15, 20, 25, and 30 W; and the irrigation rate was set at 0, 15, 25, 35 ml/min, using a domestic SP-TFL. The temperature variations around the fibre under different power settings and different irrigation rates were compared. Results: Without irrigation, the temperature around the fibre rapidly reached the safety threshold of 43℃ at 24 s. At irrigation rate 15 ml/min and laser power <15 W, the temperature around the fibre was <43℃. Once the laser power increased to ≥20 W, the temperature around the fibre increased to >43℃ at its lowest plateau. At irrigation rate 25 ml/min and laser power ≤25 W, the temperature around the fibre was <43℃. At irrigation rate 35 ml/min and laser power <30 W, the fibre temperature was <43℃. When laser power was ≥30 W, the fibre temperature was >43℃. Conclusion: In extracorporeal ureteroscope SP-TFL lithotripsy, when the laser power is ≤15 W, ≤25 W, and ≤30 W, the irrigation rate should be maintained at ≥15 ml/min, ≥25 ml/min, and ≥35 ml/min, respectively.

Abstract:Introduction: Several preclinical studies about a novel pulsed-thulium:yttrium-aluminum-garnet (p-Tm:YAG) device have been published, demonstrating its possible clinical relevance. Methods: We systematically reviewed the reality and expectations for this new p-Tm:YAG technology. A PubMed, Scopus and Embase search were performed. All relevant studies and data identified in the bibliographic search were selected, categorized, and summarized. Results: Tm:YAG is a solid state diode-pumped laser that emits at a wavelength of 2013 nm, in the infrared spectrum. Despite being close to the Ho:YAG emission wavelength (2120 nm), Tm:YAG is much closer to the water absorption peak and has higher absorption coefficient in liquid water. At present, there very few evaluations of the commercially available p-Tm:YAG devices. There is a lack of information on how the technical aspects, functionality and pulse mechanism can be maximized for clinical utility. Available preclinical studies suggest that p-Tm:YAG laser may potentially increase the ablated stone weight as compared to Ho:YAG under specific condition and similar laser parameters, showing lower retropulsion as well. Regarding laser safety, a preclinical study observed similar absolute temperature and cumulative equivalent minutes at 43° C as compared to Ho:YAG. Finally, laser-associated soft-tissue damage was assessed at histological level, showing similar extent of alterations due to coagulation and necrosis when compared with the other clinically relevant lasers. Conclusions: The p-Tm:YAG appears to be a potential alternative to the Ho:YAG and TFL according to these preliminary laboratory data. Due to its novelty, further studies are needed to broaden our understanding of its functioning and clinical applicability.
In Vitro Comparison of Pulsed-Thulium:YAG, Holmium:YAG, and Thulium Fiber Laser

Abstract:Objective: To characterize the pulse characteristics and risk of fiber fracture (ROF) of the pulsed-Thulium:YAG (p-Tm:YAG) laser and to compare its ablation volumes (AVs) against Holmium:Yttrium-Aluminium-Garnet (Ho:YAG) laser and Thulium fiber laser (TFL). Materials and Methods: p-Tm:YAG (100 W-Thulio, Dornier-Medtech©, Germany) was characterized using single-use 272 μm core-diameter-fibers. p-Tm:YAG characterization included pulse shape, duration, and peak power (PP) studies. ROF was assessed after 5 minutes of continuous laser activation (CLA) at five decreasing fiber bend radii (1, 0.9, 0.75, 0.6, and 0.45 cm). p-Tm:YAG, Ho:YAG (120 W-Cyber-Ho, Quanta®, USA), and TFL (60 W-TFLDrive, Coloplast®, Denmark) AVs were compared using a 20-mm linear CLA at 2 mm/second velocity in contact with 20 mm3 hard stone phantoms (HSP) and soft stone phantoms (SSP) (15:3 and 15:5 water to powder ratio, respectively) fully submerged in saline at 0.5 J-20 Hz or 1 J-10 Hz. After CLA, phantoms underwent three-dimensional (3D) micro-scanning (CT) and subsequent 3D segmentation to estimate the AVs, using 3DSlicer©. Each experiment was performed in triplicate. Results: p-Tm:YAG presents a uniform pulse profile in all of the available preset modes. PP ranged from 564 to 2199 W depending on pulse mode. No laser fiber fracture occurred at any bend radius. p-Tm:YAG achieved similar mean AVs to TFL and Ho:YAG for HSP (8.96 ± 3.1 vs 9.78 ± 1.1 vs 8.8 ± 2.8 mm3, p = 0.67) but TFL was associated with higher AVs compared with p-Tm:YAG and Ho:YAG (12.86 ± 1.85 vs 10.12 ± 1.89 vs 7.56 ± 2.21 mm3, p = 0.002) against SSP. AVs for HSP increased with pulse energy for p-Tm:YAG and Ho:YAG and (11.56 ± 1.8 vs 6.36 ± 0.84 mm3 and 11.27 ± 1.98 vs 6.34 ± 0.55 mm3, p = 0.03 and p = 0.02), whereas AVs for SSP were similar across laser settings for all laser sources. AVs with TFL were similar across laser settings for both phantom types. Conclusion: p-Tm:YAG combines intermediate PP between Ho:YAG and TFL, a uniform pulse profile, no ROF with increasing deflection and effective ablation rates. Further clinical studies are needed to confirm these in vitro results.

Abstract:Objective : To evaluate using an inanimate model the thermal injury and laser efficiency on high frequency, high energy and its combination in hands of junior and experienced urologists during holmium YAG (Ho:YAG) and Thulium fiber laser (TFL) lithotripsy. Methods : Cyber: Ho 150 WTM and Fiber Dust TFL (Quanta System) with 200μm core-diameter laser fibers (LF) were used in a saline in vitro ureteral model. Each participant (5 junior -5 experienced urologist) performed 32 sessions of 5 minutes lasering (125 mm3 phantom BegoStonesTM), comparing four modes (3J/5Hz (1.5W), 0.3J/20Hz (6W), 1.2 J/5Hz (6W), and 1.2J/20Hz (24W)). Transparent tip and cleaved LF, and digital and fiberoptic ureteroscopes were also compared. Ureteral damage was classified in a scale (0-5) according to the burns and holes seen in the ureteral model's surface. Results : High-power setting (24W) was associated with higher delivered energy and higher ablation rates in both lasers (p<0.001). For the same power setting (6W), there were no differences in delivered energy nor stone ablation rates. Regardless the settings, higher ablation rate was observed with TFL than with Ho:YAG ((0.5Δmg/s ± 0.33 vs 0.39 Δmg/s ± 0.31, p:0.002) laser. Higher mean ablation rate was found with cleaved tip vs transparent tip (p:0.03) in TFL. For both lasers, higher ureteral damage was observed in the 24W group (p: 0.006) and in the junior urologists (p: 0.03). Between 6W groups, different type of lesions were found and junior urologist have more lesions when high frequency was used, for both Ho:YAG (p=0.05) and TFL (p=0.04). Conclusion : High-power settings have more stone ablation rate and may reduce the operative time; however, more ureteral thermic-related damage is produced. When comparing the same power, higher energy or frequency does not modify the ablation rate. Nonetheless, more ureteral thermic-related thermal damage is observed in high frequency settings in unexperienced hands.

Abstract:Objectives: The super-pulsed thulium fiber laser (SP TFL) is a new alternative to high-power holmium laser for intracorporeal lithotripsy. The SP TFL has shown advantages in dusting regimes, but benefits in fragmentation regimes are less understood. The second-generation SP TFL introduces an advanced fragmentation pulse (AFP) sequence to maximize SP TFL's efficiency in fragmentation. This study evaluates safety and efficacy of the SP TFL fragmentation mode in ureteroscopy (URS) and mini percutaneous nephrolithotomy (mini-PCNL). Materials and Methods: The study was conducted in two phases. Safety of a new AFP was compared ex vivo to standard SP TFL fragmentation settings by measuring the dimensions of wounds created in porcine kidney after laser exposure for 0.5, 1.0, and 2.0 s. The resulting wounds were evaluated histologically using nitro blue tetrazolium chloride (NBTC) stain. In the clinical phase, the second-generation SP TFL was used to fragment and extract ureteral and renal stones in 40 patients using ureteroscopic and percutaneous approaches. The stone size, volume, density, laser-on-time, and total energy were recorded for each patient. In addition, the chemical composition, ablation rate, and ablation efficiency were assessed for each stone treatment. Results: The ex vivo mucosa damage profiles caused by AFP were similar to those caused by regular pulses. In clinical phase, the median volume and density for ureteral stones were 0.4 cm3 and 1029 Hounsfield units (HU), for renal stones 1.3 cm3 and 1113 HU, respectively. Different stone types were crushed into fragments suitable for extraction. The mean AFP energy was 3 J and the average power for ureteral stones was 10.5 W, whereas for renal stones it was 28.5 J and 31 W, respectively. The overall complication rate was low in both groups. Conclusion: ST PFL with AFP capability facilitates effective fragmentation of ureteral and renal stones of any composition during URS and mini-PCNL with minimal complication rates.
Exploring optimal settings for safe and effective thulium fibre laser lithotripsy in a kidney model

Abstract:Objectives: To explore the optimal laser settings and treatment strategies for thulium fibre laser (TFL) lithotripsy, namely, those with the highest treatment efficiency, lowest thermal injury risk, and shortest procedure time. Materials and methods: An in vitro kidney model was used to assess the efficacy of TFL lithotripsy in the upper calyx. Stone ablation experiments were performed on BegoStone phantoms at different combinations of pulse energy (EP ) and frequency (F) to determine the optimal settings. Temperature changes and thermal injury risks were monitored using embedded thermocouples. Experiments were also performed on calcium oxalate monohydrate (COM) stones to validate the optimal settings. Results: High EP /low F settings demonstrated superior treatment efficiency compared to low EP /high F settings using the same power. Specifically, 0.8 J/12 Hz was the optimal setting, resulting in a twofold increase in treatment efficiency, a 39% reduction in energy expenditure per unit of ablated stone mass, a 35% reduction in residual fragments, and a 36% reduction in total procedure time compared to the 0.2 J/50 Hz setting for COM stones. Thermal injury risk assessment indicated that 10 W power settings with high EP /low F combinations remained below the threshold for tissue injury, while higher power settings (>10 W) consistently exceeded the safety threshold. Conclusions: Our findings suggest that high EP /low F settings, such as 0.8 J/12 Hz, are optimal for TFL lithotripsy in the treatment of COM stones. These settings demonstrated significantly improved treatment efficiency with reduced residual fragments compared to conventional settings while keeping the thermal dose below the injury threshold. This study highlights the importance of using the high EP /low F combination with low power settings, which maximizes treatment efficiency and minimizes potential thermal injury. Further studies are warranted to determine the optimal settings for TFL for treating kidney stones with different compositions.
In Pursuit of the Optimal Dusting Settings with the Thulium Fiber Laser: An In Vitro Assessment

Abstract:Objective: Low energy and high frequency settings are used in stone dusting for holmium lasers. Such settings may not be optimal for thulium fiber laser (TFL). With the seemingly endless combination of settings, we aim to provide guidance to the practicing urologists and assess the efficiency of the TFL platform in an automated in vitro "dusting model." Materials/Methods: Three experimental setups were designed to investigate stone dusting produced by an IPG Photonics TLR-50 W TFL system using 200 μm fiber and soft BegoStone phantoms. The most popular 10 and 20 W dusting settings among endourologist familiar with TFL were evaluated. We directly compared short pulse (SP) vs long pulse (LP) mode using various combinations of pulse energy (Ep) and pulse frequency (F). Thereafter, we tested the 10 and 20 W settings and compared them among each other to elucidate the most efficient settings at each power. Treatments were performed under the same total laser energy delivered to the stone at four different standoff distances (SDs) with a clinically relevant scanning speed of either 1 or 2 mm/sec. Ablation volumes were quantified by optical coherence tomography to assess stone dusting efficiency. Fragment size after ablation at different pulse energies was evaluated by sieving and evaluating under a microscope after treatment. Results: Overall, SP provided greater ablation volume when compared with LP. Our dusting efficiency model demonstrated that the maximum stone ablation was achieved at the combination of high energy/low frequency settings (p < 0.005) and at a SD of 0.2 mm. At all tested pulse energies, no stone phantoms were broken into fragments >1 mm. Conclusions: During stone dusting with TFL, SP offers superior ablation to LP settings. Optimal dusting at clinically relevant scanning speeds of 1 and 2 mm/sec occurs at high energy/low frequency settings. Thulium lithotripsy with high Ep does not result in increased fragment size.

Abstract:Effects of improved beam quality of a low diffraction laser beam on laser material removal processes are experimentally investigated in a polymeric material. The experimental results are in agreement with theoretical predictions. The results show that the low diffraction beam has marked advantages over the Gaussian beam in ablation-dominated material removal processes in terms of larger depth and smaller taper at the same average power level.
Preclinical comparison of superpulse thulium fiber laser and a holmium:YAG laser for lithotripsy

Does the Novel Thulium Fiber Laser Have a Higher Risk of Urothelial Thermal Injury than the Conventional Holmium Laser in an In Vitro Study?

Abstract:Introduction and Objective: The novel thulium fiber laser (TFL) has been shown to break stones more rapidly than the holmium:YAG laser (HL). However, some evidence suggests that the TFL generates more heat. The purpose of this study is to compare ureteral temperatures generated by these lasers during ureteroscopic laser lithotripsy in a benchtop model. Methods: A 1-cm BegoStone was manually impacted in the proximal ureter of a 3D printed kidney-ureter model and submerged in 35.5°C saline. Lithotripsy was performed using a 7.6 French flexible ureteroscope and a 200µm laser fiber without a ureteral access sheath. The Dornier 30W HL, Olympus 100W HL, and Olympus 60W TFL were compared. A needle thermocouple to measure temperature was inserted 2 mm from the laser tip. Irrigation was maintained at 35cc/min at room temperature using the Thermedx FluidSmart System. Intraluminal temperature was continuously recorded for 60 seconds of laser activation. 5 trials were performed for each of 4 different power settings: 3.6, 10, 20, and 30 Watts. ANOVA and Mann-Whitney U tests were performed with p<0.05 considered significant. Results: Intraureteral fluid temperature increased as laser power settings increased for all lasers (p<0.05). The TFL generated higher average ureteral fluid temperatures than the Dornier and Empower HL at all power settings tested (p<0.001). The maximum temperature for the TFL was higher than the Dornier and Empower HL at all power settings tested (p<0.001), except at 20W with the Empower HL. At 30W, the TFL exceeded 43°C, the threshold for tissue damage. Conclusions: The TFL generated more heat at all settings tested. Supraphysiologic ureteral temperatures may be generated with extended use at high energy settings and low irrigation rates. Understanding the heat generation properties of both lasers could help improve the safety of ureteroscopic laser lithotripsy.

Abstract:The burst‐mode femtosecond laser has the potential to be a novel thrombus removal technique. This paper proposed to investigate the mechanism of thrombus ablation in vitro by burst‐mode femtosecond laser. A simulation model of the interaction between femtosecond laser and thrombus was established. An in vitro thrombus model was prepared. Combined the high‐speed galvanometer and femtosecond laser, the ablation experiments in vitro were performed. The experimental results showed that the ablative threshold was 0.27 times and the efficiency was about 1.4 times of burst‐mode femtosecond laser as those of traditional mode femtosecond laser. These phenomena were related to the residual temperature and free electrons on the thrombus surface, which confirmed to the simulating results and had relationship with incubation effects. The high ablative efficiency and safety of burst‐mode femtosecond laser for thrombus ablation was verified, which may help to achieve the femtosecond pulse output through flexible fiber easily and stably. The burst‐mode femtosecond laser represents an important technological advancement of the method in terms of endovascular treatment with femtosecond laser. This article is protected by copyright. Abstract:ObjectiveTo investigate the effects of laser temporal pulse shaping of the super pulse Thulium fiber laser (SPTFL) and to compare them in controlled in vitro conditions with various Ho:YAG pulse delivery modes. Materials and methodsSPTFL (Urolase SP, IRE‐Polus, Fryazino, Russia) with an emission wavelength of 1.94 μm and a Ho:YAG laser (P120H, Lumenis, Yokneam, Israel) with Moses technology were compared. Pulse shape, stone retropulsion, and ablation efficiency were evaluated using Begostones and compared for each laser modality: short (SP), long (LP), and Moses pulse (MP) for Ho:YAG, regular pulse (RP) and dual pulse (DP) for SPTFL. ResultsHo:YAG SP mode exhibited an asymmetrical pulse shape with steep leading slope and much more gradual trailing slope, without any flat section. Pulses generated by SPTFL were significantly longer and therefore had lower peak power (Ppeak) than those generated by Ho:YAG laser at equivalent energy settings. Retropulsion for Ho:YAG LP and MP modes was similar and lower than that of SP, but higher than for SPTFL (all p ≤0.02), with an average stone displacement about 4 times and 2 times lower for SPTFL as compared to Ho:YAG. Comparison of ablation volumes indicated that SPTFL induced significantly higher (2‐fold) ablation than Ho:YAG laser. ConclusionsMagnitude and initial velocity of stone retropulsion decrease with longer pulse duration and lower pulse peak power, but without sacrificing ablation efficiency. These observations are manifest when comparing Ho:YAG laser with SPTFL. The novel SPTFL provides greater versatility and control of pulse parameters than Ho:YAG laser. Further clinical investigation of practical benefits achievable with pulse‐shaping SPTFL modes is warranted.

Abstract:This study aims to identify optimal parameters for using Thulium fiber lasers (TFL) in ureteral stone lithotripsy to ensure laser safety and maximize efficacy. Our goal is to improve the outcomes of single-use semi-rigid ureteroscopy for treating stones located in the proximal ureter. A clinically relevant thermal testing device was designed to investigate heating effects during TFL stone fragmentation. The device was utilized to identify safe power thresholds for TFL at various irrigation rates. Three other devices were used to assess varying pulse energy effects on stone fragmentation efficiency, dusting, retropulsion, and depth of tissue vaporization. Comparative experiments in fresh porcine renal units were performed to validate the efficacy and safety of optimal TFL parameters for semi-rigid ureteroscopy in proximal ureteral stone procedures. Our study found that the improved device generated a higher thermal effect. Furthermore, the safe power threshold for laser lithotripsy increased as the irrigation rate was raised. At an irrigation rate of 40 ml/min, it is safe to use an average power of less than 30 watts. Although increasing pulse energy has a progressively lower effect on fragmentation and dust removal efficiency, it did lead to a linear increase in stone displacement and tissue vaporization depth. Thermal testing showed 20 W (53.87 ± 2.67 °C) indicating potential urothelial damage. In our study of laser lithotripsy for proximal ureteral stones, the group treated with 0.3 J pulses had several advantages compared to the 0.8 J group: Fewer large fragments (> 4 mm): 0 vs. 1.67 fragments (1-2.25), p = 0.002, a lower number of collateral tissue injuries: 0.50 (0-1.25) vs. 2.67 (2–4), p = 0.011, and lower stone retropulsion grading: 0.83 (0.75-1) vs. 1.67 (1–2), p = 0.046. There was no significant difference in operating time between the groups (443.33 ± 78.30 s vs. 463.17 ± 75.15 s, p = 0.664). These findings suggest that TFL irradiation generates a greater thermal effect compared to non-irradiated stones. Furthermore, the thermal effect during laser lithotripsy is influenced by both power and irrigation flow rate. Our study suggests that using a power below 15 W with an irrigation flow rate of 20 ml/min is safe. Moreover, a pulse energy of 0.3 J appears to be optimal for achieving the best overall stone fragmentation effect.

Abstract:Objective: To evaluate the ablation speed (AS), laser efficiency and direct thermal lesions during urinary stone lithotripsy with the current available laser technologies: Holmium:YAG (Ho:YAG), pulsed-Thulium:YAG (p-Tm:YAG) and thulium fiber laser (TFL) in vitro using different laser settings. Materials and Methods: Ho:YAG, p-Tm:YAG, and TFL laser system were used in an in vitro ureteral model with a volume of 125 mm3 Begostone. The following parameters were tested across all laser devices: 0.6J/10 Hz (6 W), 0.6 J/20 Hz (12 W), 1.5 J/10 Hz (15 W), and 1.5 J/20 Hz (30 W), employing short pulse width for all lasers and long pulse width for Ho:YAG and p-Tm:YAG. Ten participants conducted the experimental setup during 3-minutes laser on time, combining the laser technology, settings, and pulse widths, with a total of 20 different combinations. The efficiency, AS and ureteral damage resulting from each intervention were analyzed. Results: p-Tm:YAG and TFL demonstrated significantly higher efficiency compared with Ho:YAG (0.049 ± 0.02 Δgr/KJ and 0.042 ± 0.01 Δgr/KJ vs 0.029 ± 0.01 Δgr/KJ; p < 0.05). In all laser sources, as the power increases, the AS also increases (p < 0.05). Furthermore, only at high-energy settings (1.5 J) higher frequency led to increase AS (p < 0.05). Both, p-Tm:YAG and TFL exhibited higher AS compared to Ho:YAG (0.64 ± 0.33 Δgr/s and 0.62 ± 0.31 Δgr/s vs 0.44 ± 0.22 Δgr/s; p < 0.05). Regarding ureteral injuries, as the power increases, there is a higher chance of ureteral damage (p = 0.031). No differences were observed between laser technologies (p = 0.828). Conclusions: Both, p-Tm:YAG and TFL exhibited superior performances during laser lithotripsy compared with Ho:YAG, as they demonstrated higher efficiency and ablation speed. Thermal damage did not appear to be associated with specific laser equipment, but higher grades of lesions are described by increasing power.
