Abstract:Previous deterministic finger biomechanical models predicted that the flexor digitorum superficialis (FDS) was silent and the flexor digitorum profundus (FDP) was the only active flexor during finger flexion. Experimental studies in vivo, however, recorded activities of both flexors. In this study, in an attempt to elucidate the roles of the flexors, a probabilistic biodynamic model of the index finger was constructed to estimate the muscle-tendon forces during an experimentally measured index finger flexion movement. A Monte-Carlo simulation was performed with four model parameters, including moment arms, physiological cross sectional areas (PCSA), passive torques, and anthropometric measures as independent random variables. The muscle-tendon forces at each time point were determined using a nonlinear optimization technique. The model predicted that both FDS and FDP contributed to sustaining the movement and the FDS was not necessarily silent. The two distinct force patterns observed in vivo in experimental studies were also corroborated by the simulation. These findings, contrary to previous deterministic models' predictions but in agreement with experimental measurements, explained the observed coactivation of FDS and FDP, and resolved the controversy regarding the roles of the flexors in finger movement dynamics.
Estimating in Vivo Passive Forces of the Index Finger Muscles: Exploring Model Parameters.

Abstract:We compared predicted passive finger joint torques from a biomechanical model that includes the exponential passive muscle force-length relationship documented in the literature with finger joint torques estimated from measures in ten adult volunteers. The estimated finger joint torques were calculated from measured right index fingertip force, joint postures, and anthropometry across 18 finger and wrist postures with the forearm muscles relaxed. The biomechanical model predicting passive finger joint torques included three extrinsic and three intrinsic finger muscles. The values for the predicted passive joint torques were much larger than the values calculated from the fingertip force and posture measures with an average RMS error of 7.6Ncm. Sensitivity analysis indicated that the predicted joint torques were most sensitive to passive force-length model parameters compared to anthropometric and postural parameters. Using Monte Carlo simulation, we determined a new set of values for the passive force-length model parameters that reduced the differences between the joint torques calculated from the two methods to an average RMS value of 0.5Ncm, a 94% average improvement of error from the torques predicted using the existing data. These new parameter values did vary across individuals; however, using an average set for the parameter values across subjects still reduced the average RMS difference to 0.8Ncm. These new parameters may improve dynamic modeling of the finger during sub-maximal force activities and are based on in vivo data rather than traditional in vitro data.
Identification and validation of the dynamic model of a tendon-driven anthropomorphic finger

Abstract:This study addresses the absence of an identification framework to quantify a comprehensive dynamic model of human and anthropomorphic tendon-driven fingers, which is necessary to investigate the physiological properties of human fingers and improve the control of robotic hands. First, a generalized dynamic model was formulated, which takes into account the inherent properties of such a mechanical system. This includes rigid-body dynamics, coupling matrix, joint viscoelasticity, and tendon friction. Then, we propose a methodology comprising a series of experiments, for step-wise identification and validation of this dynamic model. Moreover, an experimental setup was designed and constructed that features actuation modules and peripheral sensors to facilitate the identification process. To verify the proposed methodology, a 3D-printed robotic finger based on the index finger design of the Dexmart hand was developed, and the proposed experiments were executed to identify and validate its dynamic model. This study could be extended to explore the identification of cadaver hands, aiming for a consistent dataset from a single cadaver specimen to improve the development of musculoskeletal hand models.

Abstract:Mechanical behavior of skeletal muscles is commonly modeled under the assumption of mechanical independence between individual muscles within a muscle group. Epimuscular myofascial force transmission via the connective tissue network surrounding a muscle challenges this assumption as it alters the force distributed to the tendons of individual muscles. This study aimed to derive a lumped estimate of stiffness of the intermuscular and extramuscular connective tissues and to assess changes in such stiffness in response to a manipulation of the interface between adjacent muscles. Based on in situ measurements of force transmission in the rat plantar flexors, before and after resection of their connective tissue network, a nonlinear estimate of epimuscular myofascial stiffness was quantified and included in a multi-muscle model with lumped parameters which allows for force transmission depending on the relative position between the muscles in the group. Such stiffness estimate was assessed for a group with normal intermuscular connective tissues and for a group with increased connectivity, mimicking scar tissue development. The model was able to successfully predict the amount of epimuscular force transmission for different experimental conditions than those used to obtain the model parameters. The proposed nonlinear stiffness estimates of epimuscular pathways could be integrated in larger musculoskeletal models, to provide more accurate predictions of force when effects of mechanical interaction or altered epimuscular connections, e.g. after surgery or injury, are substantial.

Abstract:The knowledge of local stress distribution in hand joints is crucial to understand injuries and osteoarthritis occurrence. However, determining cartilage contact stresses remains a challenge, requiring numerical models including both accurate anatomical components and realistic tendon force actuation. Contact forces in finger joints have frequently been calculated but little data is available on joint contact pressures. This study aimed to develop and assess a hybrid biomechanical model of the index finger to estimate in-vivo joint contact pressure during a static maximal strength pinch grip task. A finite element model including bones, cartilage, tendons, and ligaments was developed, with tendon force transmission based on a tendon-pulley system. This model was driven by realistic tendon forces estimated from a musculoskeletal model and motion capture data for six subjects. The hybrid model outputs agreed well with the experimental measurement of fingertip forces and literature data on the physiological distribution of tendon forces through the index finger. Mean contact pressures were 6.9 ± 2.7 MPa, 6.2 ± 1.0 MPa and 7.2 ± 1.3 MPa for distal, proximal interphalangeal and metacarpophalangeal joints, respectively. Two subjects had higher mean contact pressure in the distal joint than in the other two joints, suggesting a mechanical cause for the prevalence of osteoarthritis in the index distal joint. The inter-subject variability in joint contact pressure could be explained by different neuromuscular strategies employed for the task. This first application of an effective hybrid model to the index finger is promising for estimating hand joint stresses under daily grip tasks and simulating surgical procedures.

Abstract:Some basic research issues of biomechanics modeling and analysis were discuissed using a forward dynamics model of the metacarpophalangeal (MCP) joint of the index finger, which combines musculotendon dynamics and muscle excitation contraction dynamics. The model numerically analyzes the 60° angular fast flexion of the MCP joint of the index finger using the optimal control theory. The result shows that during a fast flexion the short time average angular speed is nearly constant, but the instantaneous speed appears to be a fluctuation wave. This model provides a basic analysis tool for the simulation of MCP joint movement including the neural control signals and is of further significance for analyzing the mechanism for redundant movement control of the human hand.
The variability of transfusion practice in coronary artery bypass surgery. Transfusion Medicine Academic Award Group.

Abstract:We audited 540 patients undergoing elective first-time coronary artery bypass grafts at 18 institutions. The purposes of the study were to describe the variability in transfusions among institutions and to determine factors that may account for variability. Mean homologous red blood cell use per patient was 2.9(+/- 0.1) U (institutional range, 0.4 to 6.3 U). One hundred seventy-seven patients (32%) received plasma (institutional range, 0% to 97%), and 119 (22%) received platelets (institutional range, 0% to 80%). After controlling for patient and surgical practice variables, transfusion practice factors still accounted for variation in red blood cell transfusions. Variation in patients receiving plasma and platelet transfusions among institutions was determined in part by prophylactic transfusions. We conclude that blood component usage for coronary artery bypass grafts differs widely among institutions. The variability in use of these components is accounted for in part by unnecessary transfusions in otherwise routine, uncomplicated coronary artery bypass graft procedures.
Extensor digitorum communis compartments with a double compensation synergy strategy for constant force control of the index finger

Abstract:Precise sustained force control of the fingers is important for achieving flexible hand movements. However, how neuromuscular compartments within a forearm multi-tendon muscle cooperate to achieve constant finger force remains unclear. This study aimed to investigate the coordination strategies across multiple compartments of the extensor digitorum communis (EDC) during index finger sustained constant extension. Nine subjects performed index finger extensions of 15%, 30%, and 45% maximal voluntary contractions, respectively. High-density surface electromyography signals were recorded from the EDC and then analyzed using non-negative matrix decomposition to extract activation patterns and coefficient curves of EDC compartments. The results showed two activation patterns with stable structures during all tasks: one pattern corresponding to the index finger compartment was named master pattern; whereas the other corresponding to other compartments was named auxiliary pattern. Further, the intensity and stability of their coefficient curves were assessed using the root mean square value (RMS) and coefficient of variation (CV). The RMS and CV values of the master pattern increased and decreased with time, respectively, while the corresponding values of the auxiliary pattern were both negatively correlated with the formers. These findings suggested a special coordination strategy across EDC compartments during index finger constant extension, manifesting as two compensations of the auxiliary pattern for the intensity and stability of the master pattern. The proposed method provides new insight into the synergy strategy across multiple compartments within a forearm multi-tendon during sustained isometric contraction of a single finger and a new approach for constant force control of prosthetic hands.
A simplified model of a multi-jointed mechanical finger calibrated with experimental data by vision system

Abstract:The development of suitable models for mechanical fingers, whether they are part of prosthetic device or of a robotic hand, is a powerful tool to predict the behaviour of their components since the early stages of design, especially for underactuated mechanisms. Experimental data can improve the reliability of such models and promote their application to build proper control strategies especially for prosthetic hands. Here, we have developed a multi-jointed model of a mechanical finger. The finger is part of the Federica hand: an underactuated mechanical hand that was conceived for prosthetic purpose. The model accounts for friction phenomena in the finger and it is tuned with experimental data acquired through a digital image correlation device. The model allowed us to write kinematics relations of the phalanges and evaluate finger configurations in relation to the closure velocity. Moreover, it was possible to estimate the tendon force and the work analysis occurring during the closure tasks, both in free mode and in presence of objects.

Abstract:A forward dynamic model of human multi-fingered hand movement is proposed. The model represents digits 2–5 in manipulative acts as a 12-degrees-of-freedom (DOF) system, driven by torque actuators at individual joints and controlled using a parsimonious proportional–derivative (PD) scheme. The control parameters as feedback gains along with an auxiliary parameter to modulate the joint torque magnitudes and cross-coupling can be empirically identified in an iterative procedure minimizing the discrepancy between the model-prediction and measurement. The procedure is guided and computationally accelerated by pre-knowledge of relations between the parameters and kinematic responses. An empirical test based on real grasping movement data showed that the model simulated the multi-finger movements with varied inter-joint temporal coordination accurately: the grand mean of the root-mean-square-errors (RMSE) across trials performed by 28 subjects was 3.25°. Analyses of the model parameters yielded new insights into intra- and inter-person variability in multi-finger movement performance, and distinguished the less variable motor control strategy from much more variable anthropometric and physiological factors.
Influence of Force-Length Relationship and Task-Specific Constraints on Finger Force-Generating Capacities

Abstract:Grip strength loss in extended and flexed wrist postures has been explained by reduced force-generating capacities of extrinsic finger flexor resulting from non-optimal length, owing to the force-length relationship. Recent works suggested that other muscles, especially wrist extensors, participate in this grip strength loss. The objective of this study was to clarify the role of the force-length relationship in finger force production. 18 participants performed maximal isometric finger force production during pinch grip (Pinch) and four-finger pressing (Press) tasks in four different wrist postures (extended, flexed, neutral, spontaneous). The maximum finger force (MFF), finger and wrist joint angles, as well as activation of four muscles were determined using dynamometry, motion capture, and electromyography. The force and length of the four muscles were estimated from joint angles and muscle activation using a musculoskeletal model. MFF decreased for flexed wrist during Pinch but remained stable across wrist postures during Press. The results suggested that the loss of pinch grip force in deviated wrist posture is partially related to force-length relationship of finger extensors. In opposition, MFF during Press was not influenced by the modulation of muscle capacities but was probably first limited by mechanical and neural factors related to finger interdependence

Abstract:To design a controller for the purpose of control of hand exoskeleton force, the critical question is to model the human finger impedance properly. The difficulty is that the parameters of the impedance model are perturbative in different postures. In this paper, multiplicative uncertain models of human finger impedance are presented. We describe the impedance as a mass-spring-damping system. The experiments are set in extension, half-flexion, flexion and synthesis postures. The parameters of nominal model as well as their perturbation range are identified by the forgetting factor recursive least square method. The weighting functions are constructed accordingly. The results show that the uncertain model of synthesis posture can represent the magnitude-frequency characteristic at low frequency, while, for high frequency, the uncertain models composed by the weighting function of synthesis posture and the corresponding nominal model are feasible. These provide effective approaches for hand exoskeleton force control issues.

Abstract:BACKGROUND: The development trend of Chinese massage is to get rid of the experience-oriented mode and gradually gets objective, accurate, quantitative and individual. Therefore, the quantitative research of massage is emergently needed. OBJECTIVE: To build a biomechanical model of pushing manipulation with one-finger meditation (thumb flexion) and put forward a method to calculate the force of joints which can hardly be measured noninvasively in vivo. METHODS: Cameras and Chinese massage force analyzer (FZ-I) were used to collect kinematic data and force data, and then a biomechanical model was established to calculate the force of joints according to these data. RESULTS AND CONCLUSION: A biomechanical model of pushing manipulation with one-finger meditation (thumb flexion) with 4 bars and 5 nodes was established, the force of thumb joint, thumb metacarpophalangeal joint, wrist joint and elbow joint was calculated and the function of the joints in manipulation was analyzed. The results were consistent with manipulator's feeling. The force of joints in pushing manipulation with one-finger meditation (thumb flexion) can be calculated through the biomechanical model. The method may be useful for massage teaching and researching.
Dynamic modeling of three-dimensional muscle wrapping based on absolute nodal coordinate formulation

Abstract:Muscle wrapping influences its moment arm and alters the dynamic response of the musculoskeletal system. Conventional musculoskeletal multibody models often simplify muscle wrapping as the shortest path problem, while these models cannot consider the three-dimensional muscular geometry. In comparison, the finite element description of skeletal muscles using three-dimensional solid element discretization is computationally expensive. This paper establishes a novel flexible multibody modeling method of muscle wrapping based on absolute nodal coordinate formulation (ANCF), which can simultaneously deal with its fusiform geometry, distributed inertia, and active contractions. The skeletal muscle is discretized by full-parameterized ANCF beam elements with variable cross section. The three-dimensional shape of the muscle belly can be expressed by an explicit function, and its time-dependent change can be captured via the surface-to-surface contact detection algorithm. The muscle stress contains the effect of Hill-type active–passive contractions, muscular fascia hyperelasticity, and volume constancy based on continuum mechanics description. As a validation, a forward dynamics simulation of the sagittal elbow rotation driven by surface electromyography (sEMG) data was performed and compared with literature data and experimental results. The established numerical method provides an efficient analysis tool to describe muscle-bone force transmissions, presenting an approach for understanding the hidden biomechanics during human locomotion and human–machine interactions.

Abstract:Abstract This paper aims to develop and validate a subject-specific framework for modelling the human hand. This was achieved by combining medical image-based finite element modelling, individualized muscle force and kinematic measurements. Firstly, a subject-specific human hand finite element (FE) model was developed. The geometries of the phalanges, carpal bones, wrist bones, ligaments, tendons, subcutaneous tissue and skin were all included. The material properties were derived from in-vivo and in-vitro experiment results available in the literature. The boundary and loading conditions were defined based on the kinematic data and muscle forces of a specific subject captured from the in-vivo grasping tests. The predicted contact pressure and contact area were in good agreement with the in-vivo test results of the same subject, with the relative errors for the contact pressures all being below 20%. Finally, sensitivity analysis was performed to investigate the effects of important modelling parameters on the predictions. The results showed that contact pressure and area were sensitive to the material properties and muscle forces. This FE human hand model can be used to make a detailed and quantitative evaluation into biomechanical and neurophysiological aspects of human hand contact during daily perception and manipulation. The findings can be applied to the design of the bionic hands or neuro-prosthetics in the future.

Construction of Multiplex Muscle Network for Precision Pinch Force Control

Abstract:Muscle synergy is a fundamental mechanism of motor control. Despite a number of studies focusing on muscle synergy during power grip and pinch at high-level force, relatively less is known about the functional interactions between muscles within low-level force production during precision pinch. Traditional analytical tools such as nonnegative matrix factorization or principal component analysis have limitations in processing nonlinear dynamic electromyographic signals and have confined sensitivity particularly for the low-level force production. In this study, we developed a novel method - multiplex muscle networks, to investigate the dynamical coordination of muscle activities at low-level force production during precision pinch. The multiplex muscle network was constructed based on multiplex limited penetrable horizontal visibility graph (MLPHVG). Seven forearm and hand muscles, including brachioradialis (BR), flexor carpi ulnaris (FCU), flexor carpi radialis (FCR), flexor digitorum superficialis (FDS), extensor digitorum communis (EDC), abductor pollicis brevis (APB) and first dorsal interosseous (FDI), were examined using surface electromyography (sEMG). Eight healthy subjects were instructed to perform a visuomotor force tracking task by producing higher (10% MVC) and lower (1% MVC) precision pinch. Interlayer mutual information I, average edge overlap ω weighted clustering coefficient CW, weighted characteristic path length LW were selected as network metrics. We assessed the undirected weighted network generated from multiplex muscle network after taking the I between paired muscle network layers as edge. There are significant differences between higher and lower force level with higher I, ω, CW and lower LW at higher force level. Advanced efficiency of information processing in the regional and global perspective indicated dynamical alterations when human faces the higher force tracking task. It suggested that ω may be an important characteristic to classify different force control states with the average classification accuracy of 82.21%. These findings reveal related alterations of functional interactions between muscles involved in precision pinch. The novel method for constructing multiplex muscle network may provide insights into muscle synergies during precision pinch force control.

Abstract:INTRODUCTION: Musculoskeletal modeling allows insight into the interaction of muscle force and knee joint kinematics that cannot be measured in the laboratory. However, musculoskeletal models of the lower extremity commonly use simplified representations of the knee that may limit analyses of the interaction between muscle forces and joint kinematics. The goal of this research was to demonstrate how muscle forces alter knee kinematics and consequently muscle moment arms and joint torque in a musculoskeletal model of the lower limb that includes a deformable representation of the knee.METHODS: Two musculoskeletal models of the lower limb including specimen-specific articular geometries and ligament deformability at the knee were built in a finite element framework and calibrated to match mean isometric torque data collected from 12 healthy subjects. Muscle moment arms were compared between simulations of passive knee flexion and maximum isometric knee extension and flexion. In addition, isometric torque results were compared with predictions using simplified knee models in which the deformability of the knee was removed and the kinematics at the joint were prescribed for all degrees of freedom.RESULTS: Peak isometric torque estimated with a deformable knee representation occurred between 45° and 60° in extension, and 45° in flexion. The maximum isometric flexion torques generated by the models with deformable ligaments were 14.6% and 17.9% larger than those generated by the models with prescribed kinematics; by contrast, the maximum isometric extension torques generated by the models were similar. The change in hamstrings moment arms during isometric flexion was greater than that of the quadriceps during isometric extension (a mean RMS difference of 9.8 mm compared to 2.9 mm, respectively).DISCUSSION: The large changes in the moment arms of the hamstrings, when activated in a model with deformable ligaments, resulted in changes to flexion torque. When simulating human motion, the inclusion of a deformable joint in a multi-scale musculoskeletal finite element model of the lower limb may preserve the realistic interaction of muscle force with knee kinematics and torque.

Aiming to solve this problem, this study proposes a single-DOF noncircular pulley-five-bar finger rehabilitation mechanism, obtains the pose requirements of finger movement and extracts four important poses. The solution domain of the five-bar mechanism with four poses was solved using the solution-region synthesis method, and the noncircular pulley was used to constrain two rods, making it a single-DOF mechanism. According to the requirements of the finger rehabilitation mechanism and considering the force transmission characteristics of the mechanism, the mechanism parameter optimization model is established, and the optimal parameters are optimized. The calculation model of noncircular pulley transmission is established, and the profile of noncircular pulley is solved according to the obtained rod rotation angle relationship. A finger rehabilitation device was developed, and an experimental study was performed. It was found that the device passed four poses, and the actual trajectory was consistent with the target trajectory. It is confirmed that the single-DOF noncircular pulley-five-bar finger rehabilitation mechanism proposed is effective, and solution method are correct.

An investigation of tendon strains in jersey finger injury load cases using a finite element neuromuscular human body model

Abstract:Introduction: A common hand injury in American football, rugby and basketball is the so-called jersey finger injury (JFI), in which an eccentric overextension of the distal interphalangeal joint leads to an avulsion of the connected musculus flexor digitorum profundus (FDP) tendon. In the field of automotive safety assessment, finite element (FE) neuromuscular human body models (NHBMs) have been validated and are employed to evaluate different injury types related to car crash scenarios. The goal of this study is to show, how such a model can be modified to assess JFIs by adapting the hand of an FE-NHBM for the computational analysis of tendon strains during a generalized JFI load case. Methods: A jersey finger injury criterion (JFIC) covering the injury mechanisms of tendon straining and avulsion was defined based on biomechanical experiments found in the literature. The hand of the Total Human Model for Safety (THUMS) version 3.0 was combined with the musculature of THUMS version 5.03 to create a model with appropriate finger mobility. Muscle routing paths of FDP and musculus flexor digitorum superficialis (FDS) as well as tendon material parameters were optimized using literature data. A simplified JFI load case was simulated as the gripping of a cylindrical rod with finger flexor activation levels between 0% and 100%, which was then retracted with the velocity of a sprinting college football player to forcefully open the closed hand. Results: The optimization of the muscle routing node positions and tendon material parameters yielded good results with minimum normalized mean absolute error values of 0.79% and 7.16% respectively. Tendon avulsion injuries were detected in the middle and little finger for muscle activation levels of 80% and above, while no tendon or muscle strain injuries of any kind occurred. Discussion: The presented work outlines the steps necessary to adapt the hand model of a FE-NHBM for the assessment of JFIs using a newly defined injury criterion called the JFIC. The injury assessment results are in good agreement with documented JFI symptoms. At the same time, the need to rethink commonly asserted paradigms concerning the choice of muscle material parameters is highlighted.
Principles of the Mechanism for Epimuscular Myofascial Loads Leading to Non-uniform Strain Distributions Along Muscle Fiber Direction: Finite Element Modeling

Abstract:Sarcomere lengths and their changes are key determinants of muscle active force production. Recent studies indicate inhomogeneity of sarcomere lengths within the muscle. Studies utilizing magnetic resonance imaging (MRI) analyses for quantifying local muscle tissue strains and diffusion tensor imaging (DTI) analyses allowing for determination of their components along muscle fascicles show that those length changes can be non-uniform. Specifically, two questions arise regarding the muscle's length change heterogeneities along the muscle fiber direction: (1) How can a passively lengthened muscle show shortened regions? (2) How can an isometric contracting muscle show lengthened parts? Using finite element modeling and studying principles of the mechanism of strain heterogeneity along the muscle fiber direction, the aim was to test the following hypothesis: epimuscular myofascial loads can lead locally to strains opposing those elsewhere within the muscle that are determined by the globally imposed conditions. The geometry of the model was defined by the contour of a longitudinal slice of the rat extensor digitorum longus (EDL) muscle belly. Three models were studied: (1) isolated muscle (muscle modeled fully isolated from its surroundings) and models aiming at representing the principles of a muscle in its in vivo context including (2) extramuscularly connected muscle (muscle's connections to non-muscular structures are modeled exclusively) and (3) epimuscularly connected muscle (additionally muscle's connections to neighboring muscle are modeled). Three cases were studied: passive isometric muscle with imposed relative position change (Case I), passive lengthened muscle (Case II), and active isometric muscle with imposed relative position change (Case III). The findings indicated non-uniform strains for all models except for zero strain in model (1) in Case I, but models (2) and (3) also showed strains opposing the imposed effect. Case I: model (3) showed shortened and lengthened sections (up to 35.3%), caused exclusively by imposed relative position change. Case II: models (2) and (3) showed shortened sections (up to 12.7 and 19.5%, respectively) in addition to lengthened sections. Case III: models (2) and (3) showed lengthened sections (up to 5 and 23.4%, respectively) in addition to shortened sections. These effects get more pronounced with stiffer epimuscular connections. Assessments of forces exerted on the muscle by the epimuscular connections showed that such strain heterogeneities are ascribed to epimuscular myofascial loads determined by muscle relative position changes.
