Abstract:In modern magnetic resonance imaging (MRI), patients are exposed to strong, time-varying gradient magnetic fields that may be able to induce electric fields (E-fields)/currents in tissues approaching the level of physiological significance. In this work we present theoretical investigations into induced E-fields in the thorax, and evaluate their potential influence on cardiac electric activity under the assumption that the sites of maximum E-field correspond to the myocardial stimulation threshold (an abnormal circumstance). Whole-body cylindrical and planar gradient coils were included in the model. The calculations of the induced fields are based on an efficient, quasi-static, finite-difference scheme and an anatomically realistic, whole-body model. The potential for cardiac stimulation was evaluated using an electrical model of the heart. Twelve-lead electrocardiogram (ECG) signals were simulated and inspected for arrhythmias caused by the applied fields for both healthy and diseased hearts. The simulations show that the shape of the thorax and the conductive paths significantly influence induced E-fields. In healthy patients, these fields are not sufficient to elicit serious arrhythmias with the use of contemporary gradient sets. However, raising the strength and number of repeated switching episodes of gradients, as is certainly possible in local chest gradient sets, could expose patients to increased risk. For patients with cardiac disease, the risk factors are elevated. By the use of this model, the sensitivity of cardiac pathologies, such as abnormal conductive pathways, to the induced fields generated by an MRI sequence can be investigated. Abstract:During the past decade, it became clear that the electric field elicited by non-invasive brain stimulation (NIBS) techniques such as transcranial direct current stimulation (tDCS) and transcranial magnetic stimulation (TMS) are substantially influenced by variations in individual head and brain anatomy. In addition to structural variations in the healthy, several psychiatric disorders are characterized by anatomical alterations that are likely to further constrain the intracerebral effects of NIBS. Here, we present high-resolution realistic head models derived from structural magnetic resonance imaging data of 19 healthy adults and 19 patients diagnosed with major depressive disorder (MDD). By using a freely available software package for modelling the electric fields induced by different NIBS protocols, we show that our head models are well-suited for assessing inter-individual and between-group variability in the magnitude and focality of tDCS-induced electric fields for two protocols targeting the left dorsolateral prefrontal cortex.

Abstract:This work presented a method for computation of magnetic resonance imaging (MRI) coil excited electromagnetic fields (EMFs) inside the human head. A head-sized, multilayered dielectric spherical head phantom with measured radial conductivity and permittivity profiles was constructed, and then DyadicGreen's function (DGF)-based method was used to calculate the EMFs inside the head phantom. This solution is suitable for calculation of EMFs excited by any radio-frequency (RF) resonators whose current distributions can be calculated by method of moment (MoM). The calculated EMFs can then be used to deduce MRI imaging parameters. Results show that the computation time of the proposed method is much lower than that of a full finite difference time domain (FDTD) calculation using a complex head model, and the computation speed is increased by more than ten-fold. Simulation demonstrates that the magnetic and electric fields in human head are affected by the frequency, and that field/tissue interactions increase with the frequency increases, and that the inhomogeneity of the magnetic field in human head increases, thus resulting in the distortion of MR signals. The proposed DGF/MoM method can be used for high field MRI coil design, for example, the rapid evaluation of RF resonator (surface and volume coils) and high performance gradient system designs.

Abstract:Transcranial magnetic stimulation (TMS) delivers highly localized brain stimulations via non-invasive externally applied magnetic fields. This non-invasive, painless technique provides researchers and clinicians with a unique tool capable of stimulating both the central and peripheral nervous systems. However, a complete analysis of the macroscopic electric fields produced by TMS has not yet been performed. In this paper, we addressed the importance of the secondary E-field created by surface charge accumulation during TMS using the boundary element method (BEM). 3D models were developed using simple head geometries in order to test the model and compare it with measured values. The effects of tissue geometry, size and conductivity were also investigated. Finally, a realistically shaped head model was used to assess the effect of multiple surfaces on the total E-field. Secondary E-fields have the greatest impact at areas in close proximity to each tissue layer. Throughout the head, the secondary E-field magnitudes typically range from 20% to 35% of the primary E-field's magnitude. The direction of the secondary E-field was generally in opposition to the primary E-field; however, for some locations, this was not the case (i.e. going from high to low conductivity tissues). These findings show that realistically shaped head geometries are important for accurate modeling of the total E-field.

Computation of electric and magnetic stimulation in human head using the 3-D impedance method

Abstract:A comparative, computational study of the modeling of transcranial magnetic stimulation (TMS) and electroconvulsive therapy (ECT) is presented using a human head model. The magnetic fields from a typical TMS coil of figure-eight type is modeled using the Biot-Savart law. The TMS coil is placed in a position used clinically for treatment of depression. Induced current densities and electric field distributions are calculated in the model using the impedance method. The calculations are made using driving currents and wave forms typical in the clinical setting. The obtained results are compared and contrasted with the corresponding ECT results. In the ECT case, a uniform current density is injected on one side of the head and extracted from the equal area on the opposite side of the head. The area of the injected currents corresponds to the electrode placement used in the clinic. The currents and electric fields, thus, produced within the model are computed using the same three-dimensional impedance method as used for the TMS case. The ECT calculations are made using currents and wave forms typical in the clinic. The electrical tissue properties are obtained from a 4-Cole-Cole model. The numerical results obtained are shown on a two-dimenaional cross section of the model. In this study, we find that the current densities and electric fields in the ECT case are stronger and deeper penetrating than the corresponding TMS quantities but both methods show biologically interesting current levels deep inside the brain.
Investigating the electric field distribution in the human brain model induced by a high focality transcranial magnetic coil

Abstract:Transcranial magnetic stimulation (TMS) is a noninvasive neuromodulation technique for treating various neurological disorders. The geometry of the TMS coil determines the focality, stimulation field strength, and stimulation depth of the induced electric field. In this paper, we introduce a novel coil, named the eccentric folding coil (EFC). We used Sim4Life, a three-dimensional human tissue medical electromagnetic simulation software, and the brain models of the subjects from the population head model repository to conduct finite element simulations of the eccentric folding coil, circular coil, and figure-eight coil, implying that the largest EFC has a focal area of 19.5 cm2 less than the smallest circular coil, and the EFC of any size has a smaller focal area than the figure-8 coil of the same size. Furthermore, we stimulated the model from three directions with the coil and measured the electric field responses of the model, confirming that stimulation varied in different directions. Additionally, we applied EFC to four representative human models and found that the electric field in different subjects’ brains differed by 3.03 mm in stimulation depth and 12.9 cm2 in focal area, suggesting that personalized human head modeling and electromagnetic analysis are essential for TMS treatment. In the future, we will explore using different types of TMS devices for patients with different conditions to achieve more precise and customized treatment.

Prediction of magnetically induced electric fields in biological tissue

Abstract:There are many potential medical applications in which it is desirable to noninvasively induce electric fields. One such application that serves as the backdrop of this work is that of stimulating neurons in the brain. The magnetic fields necessary must be quite high in magnitude, and fluctuate rapidly in time to induce the internal electric fields necessary for stimulation. Attention is focused on the calculation of the induced electric fields commensurate with rapidly changing magnetic fields in biological tissue. The problem is not a true eddy current problem in that the magnetic fields induced do not influence the source fields. Two techniques are introduced for numerically predicting the fields, each employing a different gauge for the potentials used to represent the electric field. The first method employs a current vector potential (analogous to A in classical magnetic field theory where DEL x A = B) and is best suited to two-dimensional (2-D) models. The second represents the electric field as the sum of a vector plus the gradient of a scalar field; because the vector can be determined quickly using Biot Savart (which for circular coils degenerates to an efficient evaluation employing elliptic integrals), the numerical model is a scalar problem even in the most complicated three dimensional geometry. These two models are solved for the case of a circular current carrying coil near a conducting body with sharp corners.

Abstract:This paper summarized the basic principle of magnetic stimulation, and established the connection between action potential of neural cell and electrical parameters by simulating the neural model. The simulation of coil electromagnetic field and calculation of circuit mathematical model were also combined to derive out the electrical parameters. They are charging voltage, discharging current, stimulating waveform, which were required by magnetic stimulation. In the end, the optimized design and circuit of magnetic stimulation system were suggested, and simple parameter simulation of the model machine was carried out.

Abstract:Transcranial magnetic stimulation (TMS) is capable of stimulating neurons in the brain non-invasively and provides numerous possibilities for the treatment of various neurological disorders such as major depressive disorder, Parkinson's disease, obsessive compulsive disorder. TMS coils can affect the distribution of induced electric fields significantly, thus the design of TMS coils is always a popular topic in TMS studies. Yet the importance of the role of anatomical structures in the induced electric field has not been thoroughly investigated. Therefore, this work has compared the strength of electric fields induced from fifty realistic head models with twelve commercial or novel TMS coils to explore how anatomical structures affect the electric field. It has been found that the electric field strengths among the fifty head models showed highly correlated patterns. The coils were placed at two positions, where all the twelve coils were placed at the vertex and eight of them were placed at the dorsolateral prefrontal cortex of the head due to the coil geometry. Notably, fifty heterogeneous head models that are derived from MRI data were used in the simulations for examining the difference on the performance of TMS coils caused by different anatomical structures. A total of one thousand simulations have been conducted, providing a large amount of data for analysis. Clinical Relevance- This provides a basis to make treatment protocols or predictions in TMS clinical trials considering the different anatomical structures among subjects.

Abstract:OBJECTIVE: Transcranial magnetic stimulation (TMS) represents a powerful technique to noninvasively modulate cortical neurophysiology in the brain. However, the relationship between the magnetic fields created by TMS coils and neuronal activation in the cortex is still not well-understood, making predictable cortical activation by TMS difficult to achieve. Our goal in this study was to investigate the relationship between induced electric fields and cortical activation measured by blood flow response. Particularly, we sought to discover the E-field characteristics that lead to cortical activation.APPROACH: Subject-specific finite element models (FEMs) of the head and brain were constructed for each of six subjects using magnetic resonance image scans. Positron emission tomography (PET) measured each subject's cortical response to image-guided robotically-positioned TMS to the primary motor cortex. FEM models that employed the given coil position, orientation, and stimulus intensity in experimental applications of TMS were used to calculate the electric field (E-field) vectors within a region of interest for each subject. TMS-induced E-fields were analyzed to better understand what vector components led to regional cerebral blood flow (CBF) responses recorded by PET.MAIN RESULTS: This study found that decomposing the E-field into orthogonal vector components based on the cortical surface geometry (and hence, cortical neuron directions) led to significant differences between the regions of cortex that were active and nonactive. Specifically, active regions had significantly higher E-field components in the normal inward direction (i.e., parallel to pyramidal neurons in the dendrite-to-axon orientation) and in the tangential direction (i.e., parallel to interneurons) at high gradient. In contrast, nonactive regions had higher E-field vectors in the outward normal direction suggesting inhibitory responses.SIGNIFICANCE: These results provide critical new understanding of the factors by which TMS induces cortical activation necessary for predictive and repeatable use of this noninvasive stimulation modality.

Abstract:Abstract Significance. Noninvasive brain stimulation (NIBS) by quasistatic electromagnetic means is presently comprised of two methods: magnetic induction methods (transcranial magnetic perturbation or TMP) and electrical contact methods (transcranial electric perturbation or TEP). Both methods couple to neuronal systems by means of the electric fields they produce. Both methods are necessarily accompanied by a scalp electric field which is of greater magnitude than anywhere within the brain. A scalp electric field of sufficient magnitude may produce deleterious effects including peripheral nerve stimulation and heating which consequently limit the spatial and temporal characteristics of the brain electric field. Presently the electromagnetic NIBS literature has produced an accurate but non-generalized understanding of the differences between the TEP and TMP methods. Objective. The aim of this work is to contribute a generalized understanding of the differences between the two methods which may open doors to novel TEP or TMP methods and translating advances, when possible, between the two methods. Approach. This article employs a three shell spherical conductor head model to calculate general analytical results showing the relationship between the spatial scale of the brain electric fields and: (1) the scalp-to-brain mean-squared electric field ratio for the two methods and (2) TEP-to-TMP scalp mean-squared electric field ratio for similar electric fields at depth. Main results. The most general result given is an asymptotic limit to the TEP-to-TMP ratio of scalp mean-squared electric fields for similar electric fields at depth. Specific example calculations for these ratios are also given for typical TEP electrode and TMP coil configurations. While TMP has favorable mean-squared electric field ratios compared to TEP this advantage comes at an energetic cost which is briefly elucidated in this work.

Abstract:Transcranial magnetic stimulation (TMS) efficiency relies on the electric field (EF) delivered to the stimulation target. Due to lack of intracranial EF measurement methods, the induced EF is often calculated using simulation models. However, the difference between simulation and practice is still unknown. Our objective is to provide a novel and feasible method of measuring induced EF in the air domain and verify the consistency between simulation and measurement. Coil voltage inside the stimulator was measured to derive the ratio of coil voltage to coil inductance, representing the current change rate (di/dt). The calculated induced EF was obtained by multiplying di/dt values with magnetic vector potentials acquired through numerical and analytical calculations. A measurement probe based on concentric circular loops measured induced electromotive force (EMF) from a figure-8 coil, validating the presented method by comparing measurement and calculation results. Using this approach, we calculated and measured the spatial distribution of induced EF generated by a self-made figure-8 coil, demonstrating consistency with relative errors below 5% in the volume of interest (VOI). Spatial distributions of induced EF from a commercial figure-8 coil, identical in outer dimensions and turns to the self-made coil, were also measured, showing agreement despite differences in pulse widths and amplitudes. The presented measurement probe allows a feasible characterization of the TMS-induced EF. The measured values are consistent with calculated values. The proposed method may provide experimental data rather than coil geometry model for intracranial EF simulation software.

Abstract:Human offspring are weaned earlier than the offspring of other great apes but take longer to reach nutritional independence. An analysis of human disorders of imprinted genes suggests genes of paternal origin, expressed in infants, have been selected to favor more intense suckling than genes of maternal origin. The same analysis suggests that genes of maternal origin may favor slower childhood growth but earlier sexual maturation. These observations are consistent with a hypothesis in which slow maturation was an adaptation of offspring that reduced maternal fitness, whereas early weaning was an adaptation of mothers that reduced the fitness of individual offspring.
A Closed Formalism for Anatomy-Independent Projection and Optimization of Magnetic Stimulation Coils on Arbitrarily Shaped Surfaces

Various applications have been established or approved for specific coil designs with their corresponding spatial electric field distributions. However, the specific coil implementation may no longer be appropriate from the perspective of available material and manufacturing opportunities or considering the latest understanding of how to achieve induced electric fields in the head most efficiently. Furthermore, in some cases, field measurements of coils with unknown winding or a user-defined field are available and require an actual implementation. Similar applications exist for magnetic resonance imaging coils. Objective: This work aims at introducing a complete formalism free from heuristics, iterative optimization, and ad-hoc or manual steps to form practical stimulation coils with individual turns to either equivalently match an existing coil or produce a given field. The target coil can reside on practically any sufficiently large or closed surface adjacent to or around the head. Methods: The method derives an equivalent field through vector projection exploiting the well-known Huygens' and Love's equivalence principle. In contrast to other coil design or optimization approaches recently presented, the procedure is an explicit forward Hilbert-space vector projection or basis change. For demonstration, we map a commercial figure-of-eight coil as one of the most widely used devices and a more intricate coil recently approved clinically for addiction treatment (H4) onto a bent surface close to the head for highest efficiency and lowest field energy. Results: The resulting projections are within ≤4% of the target field and reduce the necessary pulse energy by more than 40%.

Simulation of transcranial magnetic stimulation in head model with morphologically-realistic cortical neurons

Abstract:Background: Transcranial magnetic stimulation (TMS) enables non-invasive modulation of brain activity with both clinical and research applications, but fundamental questions remain about the neural types and elements TMS activates and how stimulation parameters affect the neural response. Objective: To develop a multi-scale computational model to quantify the effect of TMS parameters on the direct response of individual neurons. Methods: We integrated morphologically-realistic neuronal models with TMS-induced electric fields computed in a finite element model of a human head to quantify the cortical response to TMS with several combinations of pulse waveforms and current directions. Results: TMS activated with lowest intensity intracortical axonal terminations in the superficial gyral crown and lip regions. Layer 5 pyramidal cells had the lowest thresholds, but layer 2/3 pyramidal cells and inhibitory basket cells were also activated at most intensities. Direct activation of layers 1 and 6 was unlikely. Neural activation was largely driven by the field magnitude, rather than the field component normal to the cortical surface. Varying the induced current direction caused a waveform-dependent shift in the activation site and provided a potential mechanism for experimentally observed differences in thresholds and latencies of muscle responses. Conclusions: This biophysically-based simulation provides a novel method to elucidate mechanisms and inform parameter selection of TMS and other cortical stimulation modalities. It also serves as a foundation for more detailed network models of the response to TMS, which may include endogenous activity, synaptic connectivity, inputs from intrinsic and extrinsic axonal projections, and corticofugal axons in white matter.
Electric Field Models of Transcranial Magnetic Stimulation Coils with Arbitrary Geometries: Reconstruction from Incomplete Magnetic Field Measurements

Abstract:Background: Calculation of the electric field induced by transcranial magnetic stimulation (TMS) in the brain requires accurate models of the stimulation coils. Reconstructing models from the measured magnetic fields of coils so far worked only for flat coil geometries and required data of their full magnetic field distribution. Objective: To reconstruct models of coils with arbitrary winding geometries from spatially incomplete magnetic field measurements. Methods: Dipole approximation via minimum norm estimation with a cross validation procedure simultaneously assessing predictability and reproducibility of the field approximation. The methods were validated on both simulated and acquired magnetic flux density data. Furthermore, reconstruction of coil models from sparsely sampled data is investigated and a procedure for obtaining sparse dipole coil models based on orthogonal matching pursuit is suggested. Results: Given that measured data from regions around the coil are available, the magnetic vector potential can accurately be represented by fitted dipole models even from sparsely sampled data. Considerable sparsification of dipole models is possible while retaining accurate representation of the field also close to the coil. Conclusion: Our versatile approach removes an important hurdle for constructing coil models from measurement data and enables a flexible trade-off between the accuracy and computational efficiency of the reconstructed dipole models.

Abstract:Neurostimulation devices that use rotating permanent magnets are being explored for their potential therapeutic benefits in patients with psychiatric and neurological disorders. This study aims to characterize the electric field (E-field) for ten configurations of rotating magnets using finite element analysis and phantom measurements. Various configurations were modeled, including single or multiple magnets, bipolar or multipolar magnets, rotated at 10, 13.3, and 400 Hz. E-field strengths were also measured using a hollow sphere ( = 9.2 cm) filled with a 0.9% sodium chloride solution and with a dipole probe. The E-field spatial distribution is determined by the magnets’ dimensions, number of poles, direction of the magnetization, and axis of rotation, while the E-field strength is determined by the magnets’ rotational frequency and magnetic field strength. The induced E-field strength on the surface of the head ranged between 0.0092 and 0.59 V/m. At the range of rotational frequencies applied, the induced E-field strengths were approximately an order or two of magnitude lower than those delivered by conventional transcranial magnetic stimulation. The impact of rotational frequency on E-field strength represents a previously unrecognized confound in clinical trials that seek to personalize stimulation frequency to individual neural oscillations and may represent a mechanism to explain some clinical trial results.

Abstract:Abstract Background Multichannel transcranial electrical stimulation (tES) modeling and optimization have been widely studied in recent years. Its theoretical bases include quasi-static assumption and linear superposition. However, there is still a lack of direct in vivo evidence to validate the simulation model and theoretical assumptions. Methods We directly measured the multichannel tES-induced voltage changes with implanted stereotactic-electroencephalographic (sEEG) electrodes in 12 epilepsy subjects. By combining these measured data, we investigate the linear superposition and prediction accuracy of simulation models for multi-electrode stimulation and further compare the induced EF differences between transcranial direct current stimulation (tDCS) and transcranial alternating current stimulation (tACS). Results Our in vivo measurements demonstrated that the multi-electrode tES-induced voltages were almost equal to the sum of the voltages generated independently by bipolar stimulation. Both measured voltages and electric fields obtained in vivo were highly correlated with the predicted values in our cohort (Voltages: r = 0.92, p < 0.001; electric fields: r = 0.74, p < 0.001). Under the same stimulation intensity, the tDCS-induced peak-zero voltages were highly correlated with the values of tACS ( r = 0.99, p < 0.001; s = 0.99). Conclusions The in vivo measurements provides confirmatory results for linear superposition and quasi-static assumption within the human brain. Furthermore, we found that the individualized simulation model reliably predicted the multi-electrode tES-induced electric fields.
