Abstract:In modern magnetic resonance imaging (MRI), patients are exposed to strong, time-varying gradient magnetic fields that may be able to induce electric fields (E-fields)/currents in tissues approaching the level of physiological significance. In this work we present theoretical investigations into induced E-fields in the thorax, and evaluate their potential influence on cardiac electric activity under the assumption that the sites of maximum E-field correspond to the myocardial stimulation threshold (an abnormal circumstance). Whole-body cylindrical and planar gradient coils were included in the model. The calculations of the induced fields are based on an efficient, quasi-static, finite-difference scheme and an anatomically realistic, whole-body model. The potential for cardiac stimulation was evaluated using an electrical model of the heart. Twelve-lead electrocardiogram (ECG) signals were simulated and inspected for arrhythmias caused by the applied fields for both healthy and diseased hearts. The simulations show that the shape of the thorax and the conductive paths significantly influence induced E-fields. In healthy patients, these fields are not sufficient to elicit serious arrhythmias with the use of contemporary gradient sets. However, raising the strength and number of repeated switching episodes of gradients, as is certainly possible in local chest gradient sets, could expose patients to increased risk. For patients with cardiac disease, the risk factors are elevated. By the use of this model, the sensitivity of cardiac pathologies, such as abnormal conductive pathways, to the induced fields generated by an MRI sequence can be investigated. Simulation Study of the Potential Hazards of Cardiac Stimulation by Induced Eddy-Currents in Modern MRI

Abstract:In modern MRI, patients are exposed to strong, rapidly switching magnetic gradient fields that may be able to elicit nerve stimulation and cardiac stimulation. Based on a new, High Definition, Finite-Difference Time-Domain (HD-FDTD) method and a realistic inhomogeneous 10-mm-resolution human body model with tissue parameters, this paper firstly provides numerical results of an investigation into induced current spatial distributions inside human tissues when exposed to pulsed z-gradient fields. Then, various cardiac arrhythmia have been simulated based on our realistic heart-torso model and supposed that the induced current reaches the cardiac stimulation threshold. Simulation results show that the induced current is not strong enough to elicit ventricular fibrillation under nowadays several KHz switch frequency of gradient fields, but if raise the gradient field switch frequency further, it is with great risk to elicit ventricular fibrillation.

Abstract:The purpose of the present study is to examine the effects of the White Matter (WM) anisotropy on the scalp electroencephalogram (EEG). The finite difference method (FDM) was used to model the head volume conductor with three kinds of inhomogeneous and anisotropic WM conductivities estimated by the volume constraint method. A current dipole was used to simulate the brain electrical activity with varying locations and orientations within the Gray-Matter (GM) of the cortex. The present computer simulation results suggest that the anisotropic conductivity of WM may have some effects on the scalp EEG. Further investigations are needed to fully address this issue.

Abstract:In permanent magnetic resonance imaging (MRI) systems, pulsed gradient fields induce strong eddy currents in the conducting structures of the magnet body. The gradient field for image encoding is perturbed by these eddy currents leading to MR image distortions. This paper presents a comprehensive finite element (FE) analysis of the eddy current generation in the magnet conductors. In the proposed FE model, the hysteretic characteristics of ferromagnetic materials are considered and a scalar Preisach hysteresis model is employed. The developed FE model was applied to study gradient z-coil induced eddy currents in a 0.5 T permanent MRI device. The simulation results demonstrate that the approach could be effectively used to investigate eddy current problems involving ferromagnetic materials. With the knowledge gained from this eddy current model, our next step is to design a passive magnet structure and active gradient coils to reduce the eddy current effects.

Abstract:In this paper, we present a magnetocardiogram (MCG) simulation study using the boundary element method (BEM) and based on the virtual heart model and the realistic human volume conductor model. The different contributions of cardiac equivalent source models and volume conductor models to the MCG are deeply and comprehensively investigated. The single dipole source model, the multiple dipoles source model and the equivalent double layer (EDL) source model are analysed and compared with the cardiac equivalent source models. Meanwhile, the effect of the volume conductor model on the MCG combined with these cardiac equivalent sources is investigated. The simulation results demonstrate that the cardiac electrophysiological information will be partly missed when only the single dipole source is taken, while the EDL source is a good option for MCG simulation and the effect of the volume conductor is smallest for the EDL source. Therefore, the EDL source is suitable for the study of MCG forward and inverse problems, and more attention should be paid to it in future MCG studies.

Abstract:Transcranial magnetic stimulation (TMS) delivers highly localized brain stimulations via non-invasive externally applied magnetic fields. This non-invasive, painless technique provides researchers and clinicians with a unique tool capable of stimulating both the central and peripheral nervous systems. However, a complete analysis of the macroscopic electric fields produced by TMS has not yet been performed. In this paper, we addressed the importance of the secondary E-field created by surface charge accumulation during TMS using the boundary element method (BEM). 3D models were developed using simple head geometries in order to test the model and compare it with measured values. The effects of tissue geometry, size and conductivity were also investigated. Finally, a realistically shaped head model was used to assess the effect of multiple surfaces on the total E-field. Secondary E-fields have the greatest impact at areas in close proximity to each tissue layer. Throughout the head, the secondary E-field magnitudes typically range from 20% to 35% of the primary E-field's magnitude. The direction of the secondary E-field was generally in opposition to the primary E-field; however, for some locations, this was not the case (i.e. going from high to low conductivity tissues). These findings show that realistically shaped head geometries are important for accurate modeling of the total E-field.

Abstract:The electric current distribution produced by dipoles in human brains is simulated by the finite-element method. Current dipoles are adopted to simulate the nerve sources in the brain. A finite-element formula related to dipole sources and anisotropic media is derived. An approach for creating a symmetrical stiffness matrix of the finite-element equation system in the computation of potential problems with anisotropic media is presented. The realistic shape of the head with anisotropic conducting media is simulated.
How Conductivity Boundaries Influence the Electric Field Induced by Transcranial Magnetic Stimulation in in vitro Experiments

Abstract:Background: Although transcranial magnetic stimulation (TMS) has become a valuable method for non-invasive brain stimulation, the cellular basis of TMS activation of neurons is still not fully understood. In vitro preparations have been used to understand the biophysical mechanisms of TMS, but in many cases these studies have encountered substantial difficulties in activating neurons. Objective/hypothesis: The hypothesis of this work is that conductivity boundaries can have large effects on the electric field in commonly used in vitro preparations. Our goal was to analyze the resulting difficulties in in vitro TMS using a simulation study, using a charge-based boundary element model. Methods: We decomposed the total electric field into the sum of the primary electric field, which only depends on coil geometry and current, and the secondary electric field arising from conductivity boundaries, which strongly depends on tissue and chamber geometry. We investigated the effect of the conductivity boundaries on the electric field strength for a variety of in vitro experimental settings to determine the sources of difficulty. Results: We showed that conductivity boundaries can have large effects on the electric field in in vitro preparations. Depending on the geometry of the air-saline and the saline-tissue interfaces, the secondary electric field can significantly enhance, or attenuate the primary electric field, resulting in a much stronger or weaker total electric field inside the tissue; we showed this using a realistic preparation. Submerged chambers are generally much more efficient than interface chambers since the secondary field due to the thin film of saline covering the tissue in the interface chamber opposes the primary field and significantly reduces the total field in the tissue placed in the interface chamber. The relative dimensions of the chamber and the TMS coil critically determine the total field; the popular setup with a large coil and a small chamber is particularly sub-optimal because the secondary field due to the air-chamber boundary opposes the primary field, thereby attenuating the total field. The form factor (length vs width) of the tissue in the direction of the induced field can be important since a relatively narrow tissue enhances the total field at the saline-tissue boundary. Conclusions: Overall, we found that the total electric field in the tissue is higher in submerged chambers, is higher if the chamber size is larger than the coil and if the shorter tissue dimension is in the direction of the electric field. Decomposing the total field into the primary and secondary fields is useful for designing in vitro experiments and interpreting the results.
Investigating the electric field distribution in the human brain model induced by a high focality transcranial magnetic coil

Abstract:Transcranial magnetic stimulation (TMS) is a noninvasive neuromodulation technique for treating various neurological disorders. The geometry of the TMS coil determines the focality, stimulation field strength, and stimulation depth of the induced electric field. In this paper, we introduce a novel coil, named the eccentric folding coil (EFC). We used Sim4Life, a three-dimensional human tissue medical electromagnetic simulation software, and the brain models of the subjects from the population head model repository to conduct finite element simulations of the eccentric folding coil, circular coil, and figure-eight coil, implying that the largest EFC has a focal area of 19.5 cm2 less than the smallest circular coil, and the EFC of any size has a smaller focal area than the figure-8 coil of the same size. Furthermore, we stimulated the model from three directions with the coil and measured the electric field responses of the model, confirming that stimulation varied in different directions. Additionally, we applied EFC to four representative human models and found that the electric field in different subjects’ brains differed by 3.03 mm in stimulation depth and 12.9 cm2 in focal area, suggesting that personalized human head modeling and electromagnetic analysis are essential for TMS treatment. In the future, we will explore using different types of TMS devices for patients with different conditions to achieve more precise and customized treatment.

The effect of tissue anisotropy on the radial and tangential components of the electric field in transcranial direct current stimulation

Abstract:AbstractTranscranial direct current stimulation (tDCS) is considered to be a promising technique for noninvasive brain stimulation and brain disease therapy. Recent studies have investigated the distribution of the electric field (EF) magnitude over gyri and sulci and the effect of tissue homogeneity with isotropic electrical conductivities. However, it is well known that the skull and white matter (WM) are highly anisotropic electrically, requiring investigations of their anisotropic effects on the magnitude and the directional components of the induced EF due to the high dependency between neuromodulation and the EF direction. In this study, we investigated the effects of the skull and WM anisotropy on the radial and tangential components of the EF via gyri-specific high-resolution finite element head models. For tDCS, three configurations were investigated: the conventional rectangular pad electrode, a 4(cathodes) +1(anode) ring configuration, and a bilateral configuration. The results showed that the skull anisotropy has a crucial influence on the distribution of the radial EF component. The affected cortical regions by the radial EF were reduced about 22 % when considering the skull anisotropy in comparison with the regions with the skull isotropy. On the other hand, the WM anisotropy strongly alters the EF directionality, especially within the sulci. The electric current tends to flow radially to the cortical surface with the WM anisotropy. This effect increases the affected cortical areas by the radial EF component within the sulcal regions. Our results suggest that one must examine the distribution of the EF components in tDCS, not just the magnitude of the EF alone.

Prediction of magnetically induced electric fields in biological tissue

Abstract:There are many potential medical applications in which it is desirable to noninvasively induce electric fields. One such application that serves as the backdrop of this work is that of stimulating neurons in the brain. The magnetic fields necessary must be quite high in magnitude, and fluctuate rapidly in time to induce the internal electric fields necessary for stimulation. Attention is focused on the calculation of the induced electric fields commensurate with rapidly changing magnetic fields in biological tissue. The problem is not a true eddy current problem in that the magnetic fields induced do not influence the source fields. Two techniques are introduced for numerically predicting the fields, each employing a different gauge for the potentials used to represent the electric field. The first method employs a current vector potential (analogous to A in classical magnetic field theory where DEL x A = B) and is best suited to two-dimensional (2-D) models. The second represents the electric field as the sum of a vector plus the gradient of a scalar field; because the vector can be determined quickly using Biot Savart (which for circular coils degenerates to an efficient evaluation employing elliptic integrals), the numerical model is a scalar problem even in the most complicated three dimensional geometry. These two models are solved for the case of a circular current carrying coil near a conducting body with sharp corners.
Investigating the Effects of Anatomical Structures on the Induced Electric Field in the Brain in Transcranial Magnetic Stimulation

Abstract:Transcranial magnetic stimulation (TMS) is capable of stimulating neurons in the brain non-invasively and provides numerous possibilities for the treatment of various neurological disorders such as major depressive disorder, Parkinson's disease, obsessive compulsive disorder. TMS coils can affect the distribution of induced electric fields significantly, thus the design of TMS coils is always a popular topic in TMS studies. Yet the importance of the role of anatomical structures in the induced electric field has not been thoroughly investigated. Therefore, this work has compared the strength of electric fields induced from fifty realistic head models with twelve commercial or novel TMS coils to explore how anatomical structures affect the electric field. It has been found that the electric field strengths among the fifty head models showed highly correlated patterns. The coils were placed at two positions, where all the twelve coils were placed at the vertex and eight of them were placed at the dorsolateral prefrontal cortex of the head due to the coil geometry. Notably, fifty heterogeneous head models that are derived from MRI data were used in the simulations for examining the difference on the performance of TMS coils caused by different anatomical structures. A total of one thousand simulations have been conducted, providing a large amount of data for analysis. Clinical Relevance- This provides a basis to make treatment protocols or predictions in TMS clinical trials considering the different anatomical structures among subjects.
Enabling Electric Field Model of Microscopically Realistic Brain

Abstract:Background: Modeling brain stimulation at the microscopic scale may reveal new paradigms for a variety of stimulation modalities. Objective: We present the largest map of distributions of the extracellular electric field to date within a layer L2/L3 mouse primary visual cortex brain sample, which was enabled by automated analysis of serial section electron microscopy images with improved handling of image defects (250x140x90 μm volume). Methods: We used the map to identify microscopic perturbations of the extracellular electric field and their effect on the activating thresholds of individual neurons. Previous relevant studies modeled a macroscopically homogeneous cortical volume. Result: Our immediate result is a reduction of the predicted stimulation field strength necessary for neuronal activation by a factor of approximately 0.7 (or by 30%) on average, due to microscopic perturbations of the extracellular electric field - an electric field "spatial noise" with a mean value of zero. Conclusion: Although this result is largely sample-specific, it aligns with experimental data indicating that existing macroscopic theories substantially overestimate the electric fields necessary for brain stimulation.

Influence of segmentation accuracy in structural MR head scans on electric field computation for TMS and tES

Abstract:In several diagnosis and therapy procedures based on electrostimulation effect, the internal physical quantity related to the stimulation is the induced electric field. To estimate the induced electric field in an individual human model, the segmentation of anatomical imaging, such as (magnetic resonance image (MRI) scans, of the corresponding body parts into tissues is required. Then, electrical properties associated with different annotated tissues are assigned to the digital model to generate a volume conductor. An open question is how segmentation accuracy of different tissues would influence the distribution of the induced electric field. In this study, we applied parametric segmentation of different tissues to exploit the segmentation of available MRI to generate different quality of head models using deep learning neural network architecture, named ForkNet. Then, the induced electric field are compared to assess the effect of model segmentation variations. Computational results indicate that the influence of segmentation error is tissue-dependent. In brain, sensitivity to segmentation accuracy is relatively high in cerebrospinal fluid (CSF), moderate in gray matter (GM) and low in white matter for transcranial magnetic stimulation (TMS) and transcranial electrical stimulation (tES). A CSF segmentation accuracy reduction of 10% in terms of Dice coefficient (DC) lead to decrease up to 4% in normalized induced electric field in both applications. However, a GM segmentation accuracy reduction of 5.6% DC leads to increase of normalized induced electric field up to 6%. Opposite trend of electric field variation was found between CSF and GM for both TMS and tES. The finding obtained here would be useful to quantify potential uncertainty of computational results.

Abstract:OBJECTIVE: Transcranial magnetic stimulation (TMS) represents a powerful technique to noninvasively modulate cortical neurophysiology in the brain. However, the relationship between the magnetic fields created by TMS coils and neuronal activation in the cortex is still not well-understood, making predictable cortical activation by TMS difficult to achieve. Our goal in this study was to investigate the relationship between induced electric fields and cortical activation measured by blood flow response. Particularly, we sought to discover the E-field characteristics that lead to cortical activation.APPROACH: Subject-specific finite element models (FEMs) of the head and brain were constructed for each of six subjects using magnetic resonance image scans. Positron emission tomography (PET) measured each subject's cortical response to image-guided robotically-positioned TMS to the primary motor cortex. FEM models that employed the given coil position, orientation, and stimulus intensity in experimental applications of TMS were used to calculate the electric field (E-field) vectors within a region of interest for each subject. TMS-induced E-fields were analyzed to better understand what vector components led to regional cerebral blood flow (CBF) responses recorded by PET.MAIN RESULTS: This study found that decomposing the E-field into orthogonal vector components based on the cortical surface geometry (and hence, cortical neuron directions) led to significant differences between the regions of cortex that were active and nonactive. Specifically, active regions had significantly higher E-field components in the normal inward direction (i.e., parallel to pyramidal neurons in the dendrite-to-axon orientation) and in the tangential direction (i.e., parallel to interneurons) at high gradient. In contrast, nonactive regions had higher E-field vectors in the outward normal direction suggesting inhibitory responses.SIGNIFICANCE: These results provide critical new understanding of the factors by which TMS induces cortical activation necessary for predictive and repeatable use of this noninvasive stimulation modality.

Abstract:Transcranial magnetic stimulation (TMS) is an increasingly popular tool for stroke rehabilitation. Consequently, researchers have started to explore the use of TMS in pediatric stroke. However, the application of TMS in a developing brain with pathologies comes with a unique set of challenges. The effect of TMS-induced electric fields has not been explored in children with stroke lesions. Here, we used finite element method (FEM) modeling to study how the electric field strength is affected by the presence of a lesion. We created individual realistic head models from MRIs (n = 6) of children with unilateral cerebral palsy due to perinatal stroke. We conducted TMS electric field simulations for coil locations over lesioned and non-lesioned hemispheres. We found that the presence of a lesion can strongly affect the electric field distribution. On the group level, the mean electric field strength did not differ between lesioned and non-lesioned hemispheres but exhibited a greater variability in the lesioned hemisphere. Other factors such as coil-to-cortex distance have a strong influence on the TMS electric field even in the presence of lesions. Our study has important implications for the delivery of TMS in children with brain lesions with respect to TMS dosing and coil placement.

Abstract:Tumor treating fields (TTFields) are a non-invasive, anti-mitotic and approved treatment for recurrent glioblastoma multiforme (GBM) patients. In vitro studies have shown that inhibition of cell division in glioma is achieved when the applied alternating electric field has a frequency in the range of 200 kHz and an amplitude of 1-3 V cm(-1). Our aim is to calculate the electric field distribution in the brain during TTFields therapy and to investigate the dependence of these predictions on the heterogeneous, anisotropic dielectric properties used in the computational model. A realistic head model was developed by segmenting MR images and by incorporating anisotropic conductivity values for the brain tissues. The finite element method (FEM) was used to solve for the electric potential within a volume mesh that consisted of the head tissues, a virtual lesion with an active tumour shell surrounding a necrotic core, and the transducer arrays. The induced electric field distribution is highly non-uniform. Average field strength values are slightly higher in the tumour when incorporating anisotropy, by about 10% or less. A sensitivity analysis with respect to the conductivity and permittivity of head tissues shows a variation in field strength of less than 42% in brain parenchyma and in the tumour, for values within the ranges reported in the literature. Comparing results to a previously developed head model suggests significant inter-subject variability. This modelling study predicts that during treatment with TTFields the electric field in the tumour exceeds 1 V cm(-1), independent of modelling assumptions. In the future, computational models may be useful to optimize delivery of TTFields.

Its most common usage is validation of experimental methods as EEG and extracellular spike recordings or modeling of physiological phenomena which can not be easily determined empirically. Continuous experimental work has been re-raising the importance of polarization effects between neuronal structures to neuronal communication. As this effects relies on very small potential changes, better modeling methods are necessary to quantify the weak electrical fields in the microscopic scale in a more realistic way. An important factor of influence on local field effects in the hippocampal formation is the heterogeneous resistivity of extracellular tissue. The vast majority of modeling studies consider the extracellular space to be homogeneous while experimentally, it has been shown that the stratum pyramidale has two times higher resistivity then other hippocampal layers. Common simulation methods for extracellular electrical fields based on the point source approximation are bound to describe the resistance of the space with a single, linear factor. We propose that models should be based on the space- and time-dependent Maxwell equations in order to account for heterogeneous properties of the extracellular space and specific arrangements of neurons in dense hippocampal layers. To demonstrate the influence of heterogeneous extracellular resistivity and neuronal spatial orientation on modeling results, we combine solutions of classical compartment models with spatiotemporal PDEs solved by the FEM. With the help of these methods, we show that the inclusion of heterogeneous resistivity has a substantial impact on voltages in close proximity to emitting neurons, increasing the extracellular potentials substantially compared to the homogeneous variant.

Computation of electric and magnetic stimulation in human head using the 3-D impedance method

Abstract:A comparative, computational study of the modeling of transcranial magnetic stimulation (TMS) and electroconvulsive therapy (ECT) is presented using a human head model. The magnetic fields from a typical TMS coil of figure-eight type is modeled using the Biot-Savart law. The TMS coil is placed in a position used clinically for treatment of depression. Induced current densities and electric field distributions are calculated in the model using the impedance method. The calculations are made using driving currents and wave forms typical in the clinical setting. The obtained results are compared and contrasted with the corresponding ECT results. In the ECT case, a uniform current density is injected on one side of the head and extracted from the equal area on the opposite side of the head. The area of the injected currents corresponds to the electrode placement used in the clinic. The currents and electric fields, thus, produced within the model are computed using the same three-dimensional impedance method as used for the TMS case. The ECT calculations are made using currents and wave forms typical in the clinic. The electrical tissue properties are obtained from a 4-Cole-Cole model. The numerical results obtained are shown on a two-dimenaional cross section of the model. In this study, we find that the current densities and electric fields in the ECT case are stronger and deeper penetrating than the corresponding TMS quantities but both methods show biologically interesting current levels deep inside the brain.
