Abstract:Background Cardiac hemangiomas are rare in all kinds of benign cardiac tumors. Although cardiac hemangiomas affect all ages and may occur anywhere within the heart, right ventricular hemangiomas are extremely uncommon. Case presentation We report a 56-year-old woman presented with chest tightness and breath shortness for 3 months. Transthoracic echocardiography and coronary computed tomography angiography showed a mass located adjacent to the apex of the right ventricle but both failed to figure out where the mass originated from, remaining a diagnostic mystery preoperatively. The mass was removed successfully and the histopathological examination confirmed it was hemangioma. Conclusions Cardiac magnetic resonance should be the ultimate diagnostic tool of cardiac tumors. Surgical removal, associated with a low recurrence rate and long-term survival benefits, should be the first choice of therapy for cardiac hemangiomas.
Application of Fetal Echocardiography in Detection of Fetal Arrhythmia and Its Clinical Significance

Abstract:OBJECTIVE To investigate application of fetal echocardiography in diagnosis of fetal arrhythmia and its clinical significance. METHODS Fetal echocardiography was performed on 725 fetuses for evaluation of fetal heart structures and arrhythmias. Two-dimensional, M-mode, Color M-mode and pulsed Doppler echocardiography were used. RESULTS Ninety fetuses were documented with fetal arrhythmia, the commonest fetal arrhythmia encountered in 72 cases extrasystole was (65 atrial extrasystoles, 7 ventricular extrasystoles), followed by bradycardia in 9 cases, tachycardia in 6 cases, 2:1 atrioventricular block in 2 cases, atrial flutter in 1 case. There were 4 fetuses with arrhythmias and structural heart diseases: 2 fetuses were found died in uterus within two-week follow-up (1 with single ventricle and pulmonary stenosis, 1 with cardiac rhabdomyoma, fetal echocardiographic findings were confirmed at autopsy), another 2 cases (1 with mitral atresia and 1 with atrioventricular canal defect, autopsy confirmed the fetal echocardiographic diagnoses) received termination of pregnancy later. One 38-week fetus with atrial flutter underwent cesarean section, neonatal ECG confirmed the arrhythmia, and Cedilanid D induced successful conversion. All the other 85 fetuses were with intermittent arrhythmia, normal heart structures, and had no fetal hydrops. After receiving routine treatment, all of them had term deliveries and follow-up monitoring showed normal neonatal heart rhythms (auscultation). CONCLUSIONS Fetal echocardiography is the main diagnostic tool for prenatal evaluation of fetal arrhythmias. The outcomes of the vast majority of fetal arrhythmias are benign, especially for fetuses with extrasystoles. Their arrhythmias are always well tolerated and disappear during the perinatal period. Fetuses with intermittent arrhythmias without structural heart malformations, fetal hydrops or heart failure, can be followed up with routine prenatal care without the need for special intervention.
Hemangioendothelioma of the Right Atrial Appendage Associated with Pericardial Effusion in an Infant.

Abstract:BACKGROUND:Cardiac hemangioma is a rare cause for pericardial effusion. We present a case of hemangioendothelioma of the right atrial appendage associated with pericardial effusion in an infant.METHODS:The patient was a 49-day-old infant transferred to our department because of the presence of pericardial effusion. Two-dimensional transthoracic echocardiography revealed moderate pericardial effusion and a 24 mm × 16 mm hypoechoic mass located on the right side of the right ventricular outflow tract and in front of the aorta.RESULTS:The infant underwent an exploratory median sternotomy. In the pericardial cavity, 120 mL transudate was observed. A 15 mm × 15 mm encapsulated mass of soft tissue was located in the pericardial cavity and involved the right atrial appendage. The tumor with right atrial appendage was completely removed. Pathological examination revealed wide sessile implant basis of the tumor into the the myocardium of the right atrial appendage, with no affection to the endocardium. Hemangioendothelioma was confirmed histopathologically. Echocardiographic examination 2 years after operation revealed that the infant was free from tumor recurrence.CONCLUSIONS:Pericardial effusion may be caused by hemangioma of the right atrial appendage. The diagnosis of cardiac hemangioma is based on imaging examination and histopathological studies.
Ultrasound and computed tomography angiography diagnosis of fetal right atrial isomerism with cardiac total anomalous pulmonary venous connection and intestinal malrotation: a case report and literature review

Abstract:Right atrial isomerism is a rare and severe isomerism. It is frequently associated with complex congenital heart disease and various extracardiac anomalies. Imaging diagnosis of right atrial isomerism is a challenge. Multisystem and complex anomalies in a 24-week-old fetus were diagnosed with prenatal ultrasound, postnatal computed tomography angiography (CTA), and autopsy. The ultrasound detected most major cardiovascular anomalies including right atrial isomerism and total anomalous pulmonary venous connection. The CTA further detected thoracic and abdominal malformations such as bilateral morphologically right bronchus, diaphragmatic hernia, asplenia, midline liver, and intestinal malrotation. The autopsy confirmed both ultrasound and CTA findings with additional findings, namely, bilateral trilobed lungs and bilateral morphological right auricles. Prenatal ultrasound and postnatal CTA can be complementary to each other in detecting multi-system complex anomalies. Their combined use can be useful for prenatal counseling and postpartum management.

Abstract:Background: Both hypoplastic left heart syndrome and ventriculo-coronary communication are extremely rare congenital cardiac conditions. Case: The diagnosis of hypoplastic left heart syndrome combined with endocardial fibroelastosis and ventriculo-coronary communication was made in a pregnant woman at 24 weeks of gestation. Interestingly, unlike diastolic flow from the coronary artery to the left ventricle in typical ventriculo-coronary communication, this fetus showed a systolic flow from the left ventricle to the coronary artery. Conclusion: To the best of our knowledge, this is the first study to present only a systolic flow signal in hypoplastic left heart syndrome combined with ventriculo-coronary communication in prenatal ultrasonography.

Abstract:A 29-year-old pregnant woman was referred to our hospital for fetal ultrasonography at 24 weeks of gestation. Her personal and family history was unremarkable. Two-dimensional ultrasound and colour Doppler were performed. The sonographic examination showed the fetus had a right-sided stomach, and the abdominal aorta and inferior vena cava were on the left side of the spine (figure 1). Fetal left ventricle was significantly smaller than the right ventricle, and the atrioventricular septal was completely defected. The aortic arch was located on the right side of the trachea, and the pulmonary artery was narrow. The pulmonary veins flowed into the right atrium. The prenatal ultrasonographic diagnosis was fetal heterotaxy syndrome and complex cardiac malformations. The parents decided to terminate the pregnancy, and the consent for post-mortem autopsy was obtained. At autopsy, organ dissection revealed a midline liver, a right-sided stomach, asplenia, two lobar bronchi and four-lobed lungs with bilateral main bronchi. Cardiovascular dissection showed a stenotic left ventricle, complete atrioventricular septal defect with common atrioventricular valve, right aortic arch with left brachiocephalic trunk, right common carotid artery and right subclavian artery, pulmonary atresia, anomalous draining of the pulmonary veins to the right atrium. The thoracic part of the inferior vena cava was on the right side of the spine, and the abdominal part is turned to the left side of the spine. Abstract:A rare congenital condition known as hydrometrocolpos (HMC) is characterized by the buildup of secretions in the vagina and uterus as a result of distal obstruction; this condition is frequently linked to imperforate hymen. This case report presents a prenatal diagnosis of isolated HMC in a 30-year-old primigravida, detected at 36 weeks gestation. Initial ultrasound revealed a large cystic mass, confirmed by fetal magnetic resonance imaging to be HMC. Postnatal examination of the new born confirmed imperforate hymen and HMC, which was effectively treated with hymenotomy, resulting in early resolution. This case underscores the importance of prenatal imaging in the early identification and management of HMC to prevent potential complications.

Abstract:Background: Left atrial myxoma during pregnancy is rare. We present three cases in order to aid in the management. Case presentation: Three cases of left atrial myxoma during pregnancy were presented in this article. Three patients all received multidisciplinary team work and acquired good outcomes. The case 1 had no symptoms and delivered before traditional cardiac surgery. The case 2 and case 3 undergone totally endoscopic minimally invasive cardiac surgery during pregnancy. The case 3 maintained pregnancy to term and gave birth to a healthy baby via vaginal delivery. No relapse of the tumor was observed. Conclusions: The management of left atrial myxoma during pregnancy ought to be individualized and combined with the gestational age. If the diagnosis was made in the first two trimesters of pregnancy, totally endoscopic minimally invasive cardiac surgery during pregnancy would be an optimal choice. The patients can benefit from the multidisciplinary team work.
Echocardiographic Evaluation of Persistent Left Superior Vena Cava in Fetus

Abstract:Objective To evaluate the ultrasonic feature and clinical significance of persistent left superior vena cava(PLSVC)in fetal life.Methods Fetal echocardiography was performed in 3368 fetuses.Thirty-one fetuses of PLSVC were confirmed.Results The dilated coronary sinus was observed in 30 of 31 fetuses.Congenital heart defects were presented in 14 of these cases,and extracardiac anomalies were presented in 6 fetuses.Both congenital heart defects and extracardiac anomalies were observed in 4 fetuses.Conclusions PLSVC is always associated with congenital heart defects.The prognosis Of affected fetuses largely depends on whether or not the PLSVC is associated with a congenital heart defect.Prenatal diagnosis of PLSVC can help US plan perinatal counseling and ameliorate the postnatal course.

Abstract:We report a case of fetal nasal chondromesenchymal hamartoma (NCMH) first noted on prenatal ultrasound at 34 weeks. A solid‐cystic mass which predominantly hyperechoicgenic and relatively clear margin, was located on the left nasal cavity and pharynx, with anterior extension and moderate blood flow. Further follow‐up ultrasound examination depicted an enlargement of the tumor. Fetal magnetic resonance imaging (MRI) showed an inhomogeneous signal lesion involving the ethmoid sinuses, nasal cavity, and pharynx. The infant, delivered via cesarean section at 37 + 5 weeks, required urgent neonatology intervention due to respiratory difficulties. Neonatal MRI and computer tomography were subsequently performed at 1 day after birth. Surgical excision occurred at 7 days, confirming NCMH via histological examination. Awareness of this entity, is essential to avoid potentially harmful therapies, especially in prenatal period. Considered NCMH in diagnosis when fetal nasal masses presenting with predominantly high‐level echo, well‐defined margins and moderate vascularity.

Evaluation and Tracing of Prenatally Diagnosed Abdominal Masses

Abstract:Objective To assess the accuracy of prenatal diagnosis in abdominal masses, natural development of the mass and the status of parent's choice for termination. Methods Clinical data of prenatally ultrasonographic diagnosed abdominal masses in recent 3 years were reviewed, including gestation age, mass size and location, mass features (cystic, mixed, solid), prenatal diagnosis, prenatal follow-up, the parents' choice for termination, postnatal diagnosis and management and follow-up.Results A total of 59 fetal abdominal masses were found. They accounted for 2. 2 ％ among the prenatally diagnosed malformation and tumors(59/2723)in same period. Of these cases, the youngest was 20 weeks and the oldest 39 weeks. Mean gestation age is 31.3 weeks. Confirmed diagnosis from autopsy, postnatal operation and image examination was gained. Only 3 cases were not match to the prenatal diagnosis. The total accordance is 91.9％. The masses presented regression before or after birth were observed in 18 cases (18/59, 3 confirmed and 15 not confirmed). Ninteen parents(19/59) refused the physician's advice and chose termination. Twenty-one (21/59) were termination without autopsy and lost follow-up. Fifty tumors diagnosed via operation or postnatal image examination were established. Conclusions Fetal abdominal mass can be found by sonography. Most prenatal diagnosis is match to postnatal result. The natural history of fetal mass is favorable. Some parents may choose termination when they know the fetal with suspected tumor in China.

Abstract:We report the case of a fetus with an ultrasonographic diagnosis of cerebellar hyperenhancement at 28 weeks' gestation. As an adjunct to ultrasound, fetal MRI can confirm the hemorrhagic origin of hyperechogenic cerebellar lesions, and postnatal MRI also confirms the prenatal diagnosis of cerebellar hemorrhage. We report the case of a fetus with a sonographic diagnosis of hyperechogenic cerebellum at 28 weeks' gestation, which was suspected to be of hemorrhagic origin on fetal ultrasound. As an adjunct to ultrasound, fetal MRI can confirm the hemorrhagic origin of hyperechogenic cerebellar lesions, particularly by showing a high signal on T1‐weighted images. No etiological factors for fetal hemorrhage were found other than maternal hypertension and aspirin use. Postnatal MRI confirmed the prenatal diagnosis of cerebellar hemorrhage without underlying vascular anomaly.

Abstract:Objective To analyze the ultrasonographic features of immune associated fetal heart disease,to im prove the prenatal diagnosis rate.Methods Retrospective analysis of the echocardiographic data of the 22 cases of fetuses whose mother suffered from autoimmune diseases diagnosed in Peking University People's Hospital from.Autopsy was performed for fetuses who underwent termination of pregnancy.Results Five among the 22 cases showed abnormal ultrasonic appearance of fetal heart whose mother suffered autoimmune diseases.4 cases were atrioventricular block,2 cases were Ⅱ degree and 2 cases were Ⅲ degree atrioventricular block;5 cases had tricuspid regurgitation;3 cases showed the echo enhancement,thickened and rough of the ventricular endocardial,especially the right ventricular wall,1 case associate with the echo enhancement of left atrial wall and primary atrial septum.2 cases associated with pericardial effusion.The other 17 fetuses showed no abnormality by echocardiography.Conclusions Congenital heart conduct block and the echo change of valvular and endocardial are the main manifestations of immune related congenital heart disease.The damage of the heart conduction system is the most common manifestation.The regurgitation of the heart valves occur in all fetuses.Prenatal echocardiographic screening is helpful for improving the early diagnosis rate.Early intervention obtains a good pregnancy outcome and a very important clinical value.

Abstract:Introduction Anomalous origin of the left pulmonary artery (AOLPA) is an exceptionally rare congenital malformation, requiring particular care to be detected during fetal echocardiography. Case presentation A 30-year-old woman, gravida 1, para 0, was referred for a mid-trimester anomaly scan. The three-vessel tracheal view in fetal echocardiographic examination led to the prenatal detection of an anomalous origin of the left pulmonary artery in the presence of a right-sided aortic arch. Additionally, a bilateral arterial duct and the ductal origin of the left pulmonary artery (LPA) were detected postnatally. Prenatal diagnosis enabled the scheduling of the delivery in a tertiary perinatal center, immediate postnatal treatment with prostaglandin E1 to avoid obstruction of the isolated LPA as well as surgical repair of the anomaly. Conclusion The rareness of the disease led to only sporadically published cases of prenatal diagnosis of AOLPA. However, early detection makes prenatal diagnosis crucial regarding the infants' outcome. This case report underlines the importance of a meticulous examination of the bifurcation of the pulmonary trunk during fetal echocardiography.
