Abstract:Public Health Reports®, Ahead of Print. Social determinants of health (SDOH) are the conditions in which people are born, grow, live, work, and age that influence health outcomes, and structural and systemic drivers of health (SSD) are the social, cultural, political, and economic contexts that create and shape SDOH. With the integration of constructs from previous examples, we propose an SSD model that broadens the contextual effect of these driving forces or factors rooted in the Centers for Disease Control and Prevention's SDOH framework. Our SSD model (1) presents systems and structures as multidimensional, (2) considers 10 dimensions as discrete and intersectional, and (3) acknowledges health-related effects over time at different life stages and across generations. We also present an application of this SSD model to the housing domain and describe how SSD affect SDOH through multiple mechanisms that may lead to unequal resources, opportunities, and consequences contributing to a disproportionate burden of disease, illness, and death in the US population. Our enhanced SDOH framework offers an innovative and promising model for multidimensional, collaborative public health approaches toward achieving health equity and eliminating health disparities.

An Emerging Trend of Equal Authorship Credit in Major Public Health Journals

Abstract:Background This study aimed to identify the longitudinal trends and characteristics of the practice of explicitly giving equal credit to multiple authors of publications in public health journals. Manual searches were conducted to identify original research articles, published in five public health journals with the highest IFs according to the “2012 JCR Science Edition” between January 1, 2004 and December 31, 2013, which awarded equal credit to multiple authors ( Epidemiologic Reviews , Environmental Health Perspectives , the International Journal of Epidemiology , Epidemiology , and the Annual Review of Public Health ). The Instructions to Authors in the five journals were also examined with regard to information about giving equal credit to multiple authors. Findings Statistically significant differences were noted in the annual prevalence in Environmental Health Perspectives , International Journal of Epidemiology , and Epidemiology ( r = 0.753, P = 0.012; r = 0.894, P = 0.000; r = 0.522, P = 0.122, respectively). The first two authors listed in the by-line received equal credit in the majority of articles, but this practice was also extended to authors in nearly every position on the by-line in some publications. The authors given equal credit in articles appearing in Environmental Health Perspectives , International Journal of Epidemiology , and Epidemiology were primarily from European and North American countries. Finally, none of the journals provided specific guidance regarding this practice in their Instructions to Authors. Conclusions An emerging trend of giving equal credit to multiple authors is observed in the public health journals. This practice should be better addressed in the guidance provided by journals to authors.

Abstract:Abstract Social determinants of health (SDOH) encompass the social environmental factors and lived experiences that collectively shape an individual's health. Recently, the polysocial score approach has been introduced as an innovative method for capturing the cumulative impact of a broad spectrum of social factors. This approach offers a promising opportunity to complement and enhance conventional methodologies in the advancement of SDOH research. In this issue of the Journal, Jawadekar et al. (Am J Epidemiol. XXXX;XXX(XX):XX–X–XXXX) evaluated the value of the polysocial score for predicting cognitive performance and mortality among middle-aged and older adults. Models built on a smaller set of social determinants, including race, gender, and education, performed comparably to the polysocial score models where a more complex set of social factors were included. In this invited commentary, I welcome the evaluation of the predictive ability of the polysocial score and the discussion of its merits and limitations. I also summarize the practical utility of the polysocial score in predicting health outcomes and its mechanistic significance in unveiling the relationship between genetics, social environment, and lifestyles in shaping an individual’s health and elucidate health disparities. Lastly, I propose several avenues for future research.

Abstract:A key element of risk assessment is accounting for the full range of variability in response to environmental exposures. Default dose-response methods typically assume a 10-fold difference in response to chemical exposures between average (healthy) and susceptible humans, despite evidence of wider variability. Experts and authoritative bodies support using advanced techniques to better account for human variability due to factors such as in utero or early life exposure and exposure to multiple environmental, social, and economic stressors.This review describes: 1) sources of human variability and susceptibility in dose-response assessment, 2) existing US frameworks for addressing response variability in risk assessment; 3) key scientific inadequacies necessitating updated methods; 4) improved approaches and opportunities for better use of science; and 5) specific and quantitative recommendations to address evidence and policy needs.Current default adjustment factors do not sufficiently capture human variability in dose-response and thus are inadequate to protect the entire population. Susceptible groups are not appropriately protected under current regulatory guidelines. Emerging tools and data sources that better account for human variability and susceptibility include probabilistic methods, genetically diverse in vivo and in vitro models, and the use of human data to capture underlying risk and/or assess combined effects from chemical and non-chemical stressors.We recommend using updated methods and data to improve consideration of human variability and susceptibility in risk assessment, including the use of increased default human variability factors and separate adjustment factors for capturing age/life stage of development and exposure to multiple chemical and non-chemical stressors. Updated methods would result in greater transparency and protection for susceptible groups, including children, infants, people who are pregnant or nursing, people with disabilities, and those burdened by additional environmental exposures and/or social factors such as poverty and racism.

Abstract:Future changes in exposure to risk factors should impact mortality rates and population. However, studies use mortality rates and population projections developed exogenously to the health impact assessment model used to quantify future health burdens attributable to environmental risks that are therefore invariant to projected exposure levels. This impacts the robustness of many future health burden estimates for environmental risk factors. This work describes an alternative methodology that more consistently represents the interaction between risk factor exposure, population and mortality rates, using ambient particulate air pollution (PM 2.5 ) as a case study. A demographic model is described that estimates future population based on projected births, mortality and migration. Mortality rates are disaggregated between the fraction due to PM 2.5 exposure and other factors for a historic year, and projected independently. Accounting for feedbacks between future risk factor exposure and population and mortality rates can greatly affect estimated future attributable health burdens. The demographic model estimates much larger PM 2.5 -attributable health burdens with constant 2019 PM 2.5 (∼10.8 million deaths in 2050) compared to a model using exogenous population and mortality rate projections (∼7.3 million), largely due to differences in mortality rate projection methods. Demographic model-projected PM 2.5 -attributable mortality can accumulate substantially over time. For example, ∼71 million more people are estimated to be alive in 2050 when WHO guidelines (5 μg m −3 ) are achieved compared to constant 2019 PM 2.5 concentrations. Accounting for feedbacks is more important in applications with relatively high future PM 2.5 concentrations, and relatively large changes in non-PM 2.5 mortality rates.

Abstract:Epidemiologists aim to identify modifiable causes of disease, this often being a prerequisite for the application of epidemiological findings in public health programmes, health service planning and clinical medicine. Despite successes in identifying causes, it is often claimed that there are missing additional causes for even reasonably well-understood conditions such as lung cancer and coronary heart disease. Several lines of evidence suggest that largely chance events, from the biographical down to the sub-cellular, contribute an important stochastic element to disease risk that is not epidemiologically tractable at the individual level. Epigenetic influences provide a fashionable contemporary explanation for such seemingly random processes. Chance events-such as a particular lifelong smoker living unharmed to 100 years-are averaged out at the group level. As a consequence population-level differences (for example, secular trends or differences between administrative areas) can be entirely explicable by causal factors that appear to account for only a small proportion of individual-level risk. In public health terms, a modifiable cause of the large majority of cases of a disease may have been identified, with a wild goose chase continuing in an attempt to discipline the random nature of the world with respect to which particular individuals will succumb. The quest for personalized medicine is a contemporary manifestation of this dream. An evolutionary explanation of why randomness exists in the development of organisms has long been articulated, in terms of offering a survival advantage in changing environments. Further, the basic notion that what is near-random at one level may be almost entirely predictable at a higher level is an emergent property of many systems, from particle physics to the social sciences. These considerations suggest that epidemiological approaches will remain fruitful as we enter the decade of the epigenome.
