Abstract:Recent publications have suggested that intra-ocular pressure (IOP) may be an indirect assessment of intra-cranial pressure (ICP). Both IOP and ICP have similar physiologic pressure ranges and similar responses to changes in intra-abdominal, intra-thoracic and aortic pressure. Previous studies have demonstrated the relationships between retinal arterial pressure and aortic pressure, intra-ocular pressure and retinal venous pressure, intra-cranial pressure and retinal venous pressure. Power athletes routinely utilize the Valsalva maneuver during weightlifting. In fact there are reports of stroke, cerebral hemorrhage, subarachnoid hemorrhage, conjunctival, foveal and retinal hemorrhage, retinal detachment, hiatal hernia and pneumothorax associated with weightlifting. These events are thought to occur secondary to the extreme pressure elevations that occur in the intra-abdominal, intra-thoracic, intra-cranial, intra-ocular and vascular compartments. To date no human studies have examined the IOP changes that may occur with heavy resistance exercise. Therefore, we recruited power athletes (n = 11), who had participated in prior studies, from the local metropolitan area. The athletes had blood pressure status, drug screening and medical histories performed during previous investigations. Intra-ocular pressure was measured by noncontact tonometry at rest and during maximal isometric contraction. All subjects resting IOP were within normal ranges (mean 13 +/- 2.8 mmHg). Intra-ocular pressures were significantly (p < 0.0001) elevated in each subject during maximal contraction (mean 28 +/- 9.3 mmHg). One subject's IOP reached 46 mmHg during maximal contraction. Linear regression analysis demonstrated a significant linear relationship (r = 0.62, p < 0.0001) in the net change of IOP from rest to maximal contraction for each subject. This study demonstrates that IOP elevates to pathophysiologic levels during resistance exercise. The findings of conjunctival hemorrhages in two subjects further supports IOP being reflective of retinal venous pressure. The enormous pressures generated by power athletes during weightlifting leads to elevations in ICP which obstruct venous outflow leading to hemorrhage and elevations in IOP. The question remains as to whether these intermittent bursts of elevated IOP can lead to long-term pathological sequelae.
The effect of supine cycling and progressive lower body negative pressure on cerebral blood velocity responses

Abstract:Moderate-intensity aerobic exercise increases cerebral blood velocity (CBv) primarily due to hyperpnea-induced vasodilation; however, the integrative control of cerebral blood flow (CBF) allows other factors to contribute to the vasodilation. Although lower body negative pressure (LBNP) can reduce CBv, the exact LBNP intensity required to blunt the aforementioned exercise-induced CBv response is unknown. This could hold utility for concussion recovery, allowing individuals to exercise at higher intensities without symptom exacerbation. Thirty-two healthy adults (age: 20-33 yr; 19 females/13 males) completed a stepwise maximal exercise test during a first visit to determine each participant's wattage associated with their exercise-induced maximal CBv increase. During the second visit, following supine rest, participants completed moderate-intensity exercise at their determined threshold, while progressive LBNP was applied at 0, -20, -40, -60, -70, -80, and ∼88 Torr. Bilateral middle cerebral artery blood velocities (MCAvs), mean arterial pressure (MAP), heart rate, respiratory rate, and end-tidal carbon dioxide levels were measured continuously. Two-way analysis of variance with effect sizes compared between sexes and stages. Compared with resting supine baseline, averaged MCAv was elevated during 0 and -20 Torr LBNP (q value > 7.73; P < 0.001); however, no differences were noted between baseline and -40 to -70 Torr (q value < |4.24|; P > 0.262). Differences were present between females and males for absolute MCAv measures (q value > 11.2; P < 0.001), but not when normalized to baseline (q value < 0.03; P > 0.951). Supine cycling-elicited increases in MCAv are able to be blunted during the application of LBNP ranging from -40 to -70 Torr. The blunted CBv response demonstrates the potential benefit of allowing individuals to aerobically train (moderate-intensity supine cycling with LBNP) without exacerbating symptoms during the concussion recovery phase.NEW & NOTEWORTHY The current investigation demonstrated that moderate-intensity supine cycling-induced increases in cerebral blood velocities were balanced by the lower body negative pressure-induced decreases in cerebral blood velocity. Although performed in a healthy population, the results may lend themselves to a potential treatment option for individuals recovering from concussion or experience persistent concussion symptoms.

The effect of exercise intensity and cardiorespiratory fitness on the kinetic response of middle cerebral artery blood velocity during exercise in healthy adults

Abstract:The aim of this study was to compare the kinetic response of middle cerebral artery blood velocity (MCAv) to moderate- and heavy-intensity cycling in adults, and explore the relationship between maximal oxygen uptake (V̇o2max) and MCAv kinetics. Seventeen healthy adults (23.8 ± 2.4 yr, 9 females) completed a ramp incremental test to exhaustion on a cycle ergometer to determine V̇o2max and the gas exchange threshold (GET). Across six separate visits, participants completed three 6-min transitions at a moderate intensity (90% GET) and three at a heavy intensity (40% of the difference between GET and V̇o2max). Bilateral MCAv was measured using transcranial Doppler (TCD) ultrasonography and analyzed using a monoexponential model with a time delay. The time constant (τ) of the MCAv response was not different between moderate- and heavy-intensity cycling (25 ± 10 vs. 26 ± 8 s, P = 0.82), as was the time delay (29 ± 11 vs. 29 ± 10 s, P = 0.95). The amplitude of the exponential increase in MCAv from baseline was greater during heavy-intensity cycling (23.9 ± 10.0 cm·s-1, 34.1 ± 14.4%) compared with moderate-intensity cycling (12.7 ± 4.4 cm·s-1, 18.7 ± 7.5%; P < 0.01). Following the exponential increase, a greater fall in MCAv was observed during heavy-intensity exercise compared with moderate-intensity exercise (9.5 ± 6.9 vs. 2.8 ± 3.8 cm·s-1, P < 0.01). MCAv after 6 min of exercise remained elevated during heavy-intensity exercise compared with moderate-intensity exercise (85.2 ± 9.6 vs. 79.3 ± 7.7 cm·s-1, P ≤ 0.01). V̇o2max was not correlated with MCAv τ or amplitude (r = 0.11-0.26, P > 0.05). These data suggest that the intensity of constant-work rate exercise influences the amplitude, but not time-based, response parameters of MCAv in healthy adults, and found no relationship between cardiorespiratory fitness and MCAv kinetics.NEW & NOTEWORTHY This is the first study to model the MCAv kinetic response to moderate- and heavy-intensity cycling in healthy adults. This study found that the amplitude of the exponential rise in MCAv at exercise onset was greater during heavy-intensity exercise (∼34%) compared with moderate-intensity exercise (∼19%), but the time-based characteristics of the responses were similar between intensities. Higher cardiorespiratory fitness was not associated with a greater or faster MCAv response to moderate- or heavy-intensity exercise.

Abstract:A total of 678 women were interviewed about symptoms before as well as advantages and disadvantages after abdominal hysterectomy. Medical complications, side effects, days of hospitalization and some possible differences between supravaginal hysterectomy (SVH) and total hysterectomy (TH) were also investigated. TH had been performed in 79% and SVH in 21% of the women. Leiomyoma was the most frequent diagnosis (79%). Heavy menstrual bleeding and dysmenorrhea were the dominant symptoms before surgery. After the operation, 71% of the women had no subjective gynecological complaints. Relief from heavy bleeding and pain was considered the major advantage and as many as 29% of the women experienced no disadvantage at all with the operation. Regarding sexual life, 39% experienced intercourse as much improved or better and 40% as unchanged. An intraabdominal abscess was more often found postoperatively in patients operated with TH than with SVH but with this exception no objective finding favoured one method more than the other. When new, alternative treatments such as endometrial ablation and GnRH analogues are introduced and promoted they should be subjected to careful evaluation and compared with the therapeutic efficacy of hysterectomy.
Cerebral blood flow velocity during simultaneous changes in mean arterial pressure and cardiac output in healthy volunteers

Abstract:Abstract Purpose Cerebral blood flow (CBF) needs to be precisely controlled to maintain brain functions. While previously believed to be autoregulated and near constant over a wide blood pressure range, CBF is now understood as more pressure passive. However, there are still questions regarding the integrated nature of CBF regulation and more specifically the role of cardiac output. Our aim was, therefore, to explore the effects of MAP and cardiac output on CBF in a combined model of reduced preload and increased afterload. Method 16 healthy volunteers were exposed to combinations of different levels of simultaneous lower body negative pressure and isometric hand grip. We measured blood velocity in the middle cerebral artery (MCAV) and internal carotid artery (ICAV) by Doppler ultrasound, and cerebral oxygen saturation (ScO 2 ) by near-infrared spectroscopy, as surrogates for CBF. The effect of changes in MAP and cardiac output on CBF was estimated with mixed multiple regression. Result Both MAP and cardiac output had independent effects on MCAV, ICAV and ScO 2 . For ICAV and ScO 2 there was also a statistically significant interaction effect between MAP and cardiac output. The estimated effect of a change of 10 mmHg in MAP on MCAV was 3.11 cm/s (95% CI 2.51–3.71, P < 0.001), and the effect of a change of 1 L/min in cardiac output was 3.41 cm/s (95% CI 2.82–4.00, P < 0.001). Conclusion The present study indicates that during reductions in cardiac output, both MAP and cardiac output have independent effects on CBF.

Acute Exercise Stress Reveals Cerebrovascular Benefits Associated with Moderate Gains in Cardiorespiratory Fitness

Abstract:Elevated cardiorespiratory fitness improves resting cerebral perfusion, although to what extent this is further amplified during acute exposure to exercise stress and the corresponding implications for cerebral oxygenation remain unknown. To examine this, we recruited 12 moderately active and 12 sedentary healthy males. Middle cerebral artery blood velocity (MCAv) and prefrontal cortical oxyhemoglobin (cO 2 Hb) concentration were monitored continuously at rest and throughout an incremental cycling test to exhaustion. Despite a subtle elevation in the maximal oxygen uptake (active: 52 ± 9 ml/kg per minute versus sedentary: 33 ± 5 ml/kg per minute, P < 0.05), resting MCAv was not different between groups. However, more marked increases in both MCAv (+28 ± 13% versus +18 ± 6%, P < 0.05) and cO 2 Hb (+5 ±4% versus −2 ± 3%, P < 0.05) were observed in the active group during the transition from low- to moderate-intensity exercise. Collectively, these findings indicate that the long-term benefits associated with moderate increase in physical activity are not observed in the resting state and only become apparent when the cerebrovasculature is challenged by acute exertional stress. This has important clinical implications when assessing the true extent of cerebrovascular adaptation.

Abstract:The effect that cardiorespiratory fitness has on the dynamic cerebral autoregulatory capacity during changes in mean arterial pressure (MAP) remains equivocal. Using a multiple-metrics approach, challenging MAP across the spectrum of physiological extremes (i.e., spontaneous through forced MAP oscillations), we characterized dynamic cerebral autoregulatory capacity in 19 male endurance athletes and eight controls via three methods: (1) onset of regulation (i.e., time delay before an increase in middle cerebral artery (MCA) conductance [MCA blood velocity (MCAv)/MAP] and rate of regulation, after transient hypotension induced by sit-to-stand, and transfer function analysis (TFA) of MAP and MCAv responses during (2) spontaneous and (3) forced oscillations (5-min of squat-stand maneuvers performed at 0.05 and 0.10 Hz). Reductions in MAP and mean MCAv (MCAVmean) during initial orthostatic stress (0-30 sec after sit-to-stand) and the prevalence of orthostatic hypotension were also determined. Onset of regulation was delayed after sit-to-stand in athletes (3.1 ± 1.7 vs. 1.5 ± 1.0 sec; P = 0.03), but rate of regulation was not different between groups (0.24 ± 0.05 vs. 0.21 ± 0.09 sec-1; P = 0.82). While both groups had comparable TFA metrics during spontaneous oscillations, athletes had higher TFA gain during 0.10 Hz squat-stand versus recreational controls (P = 0.01). Reductions in MAP (P = 0.15) and MCAVmean (P = 0.11) during orthostatic stress and the prevalence of initial orthostatic hypotension (P = 0.65) were comparable between groups. These results indicate an intact ability of the cerebral vasculature to react to spontaneous oscillations but an attenuated capability to counter rapid and large changes in MAP in individuals with elevated cardiorespiratory fitness.
The influence of maturation and sex on intracranial blood velocities during exercise in children

Abstract:Cerebral blood velocity (CBv) increases in response to moderate exercise in humans, but the magnitude of change is smaller in children compared with postpubertal adolescents and adults. Whether sex differences exist in the anterior or posterior CBv response to exercise across pubertal development remains to be determined. We assessed middle cerebral artery (MCAv) and posterior cerebral artery (PCAv) blood velocity via transcranial Doppler in 38 prepubertal (18 males) and 48 postpubertal (23 males) with cerebrovascular and cardiorespiratory measures compared at baseline and ventilatory threshold. At baseline, MCAv was higher in both sexes pre- versus postpuberty. Females demonstrated a greater MCAv (P < 0.001) than their male counterparts (prepubertal females; 78 ± 11 cm·s-1 vs. prepubertal males; 72 ± 8 cm·s-1, and postpubertal females; 68 ± 10 cm·s-1 vs. postpubertal males; 62 ± 7 cm·s-1). During exercise, MCAv remained higher in postpubertal females versus males (81 ± 15 cm·s-1 vs. 73 ± 11 cm·s-1), but there were no differences in prepuberty. The relative increase in PCAv was greater in post- versus prepubertal females (51 ± 9 cm·s-1 vs. 45 ± 11 cm·s-1; P = 0.032) but was similar in males and females. Our findings suggest that biological sex alters anterior cerebral blood velocities at rest in both pre- and postpubertal youth, but the response to submaximal exercise is only influenced by sex postpuberty.NEW & NOTEWORTHY Cerebral blood velocity (CBv) in the anterior circulation was higher in females compared with males irrespective of maturational stage, but not in the posterior circulation. In response to exercise, females demonstrated a greater CBv compared with males, especially post-peak height velocity (post-PHV) where the CBv response to exercise was more pronounced. Our findings suggest that both CBv at rest and in response to acute submaximal exercise are altered by biological sex in a maturity-dependent manner.
Very-low-load resistance exercise in the upper body with and without blood flow restriction: cardiovascular outcomes

Abstract:It is proposed that, at very low loads, greater blood flow restriction (BFR) pressures might be required for muscular adaptation to occur. The cardiovascular and hyperemic response to very low loads combined with relative levels of BFR is unknown. Ninety-seven participants were recruited and assigned to 1 of 4 exercise conditions: 15% of 1-repetition maximum (1RM) without BFR (15/00), 15% 1RM with BFR at 40% of arterial occlusion pressure (AOP) (15/40), 15% of 1RM with BFR at 80% of AOP (15/80), and 70% of 1RM without BFR (70/00). Participants performed 4 sets of unilateral biceps curls. Blood pressure was measured before and after exercise; brachial artery blood flow was measured before exercise, following the second set, and 1 min following exercise. Systolic blood pressure increased following exercise in all conditions (+10 (11) mm Hg, P < 0.0005). Diastolic pressure increased in all but 70/00 (+2 (11) mm Hg, P = 0.107). Brachial artery blood flow increased following the second set of exercise in all but 15/80 (+43.4 (76.8) mL·min −1 , P = 0.348). One minute following exercise and cuff deflation, there were no differences in blood flow between conditions (P > 0.05). Similarly, artery diameter was increased in all conditions except 15/80 (+0.002 (0.041) cm, P = 0.853) following the second set, and increased in all conditions by 1 min following exercise (P < 0.05). In conclusion, exercise-induced hyperemia is blunted with increasing pressures of BFR. There is a modest increase in blood pressure at very low loads of resistance exercise in the upper body.

Abstract:Exertional hypertension associated with acute high-intensity resistance exercise (RE) increases both intravascular and intracranial pressure (ICP), maintaining cerebrovascular transmural pressure. Carotid intravascular pressure pulsatility remains elevated after RE. Whether ICP also remains elevated after acute RE in an attempt to maintain the vessel wall transmural pressure is unknown. Optic nerve sheath diameter (ONSD), a valid proxy of ICP, was measured in 20 participants (6 female; 24±4yr, 24.2±3.9 kg m−2) at rest (baseline), following a time-control condition, and following RE (5 sets, 5 repetition maximum bench press, 5 sets 10 repetition maximum biceps curls) using ultrasound. Additionally, intracranial hemodynamic pulsatility index (PI) was assessed in the ophthalmic artery (OA) by using Doppler. Aortic pulse wave velocity (PWV) was obtained from synthesized aortic pressure waveforms obtained via a brachial oscillometric cuff and carotid pulse pressure was measured by using applanation tonometry. Aortic PWV (5.2±0.5–6.0±0.7 m s−1, P<0.05) and carotid pulse pressure (45±17–59±19 mm Hg, P<0.05) were significantly elevated post RE compared with baseline. There were no significant changes in ONSD (5.09±0.7–5.09±0.7 mm, P>0.05) or OA flow PI (1.35±0.2–1.38±0.3, P>0.05) following acute RE. In conclusion, during recovery from acute high-intensity RE, there are increases in aortic stiffness and extracranial pressure pulsatility in the absence of changes in ICP and flow pulsatility. These findings may have implications for alterations in cerebral transmural pressure and cerebral aneurysmal wall stress following RE.

Abstract:Appropriate regulation of IL-12 expression is critical for cell-mediated immune responses. In the present study, we have analyzed the regulation of IL-12 p40 promoter activity in primary human monocytes in vivo. Accordingly, we analyzed the p40 promoter by in vivo footprinting in resting and activated primary human blood CD14+ monocytes. Interestingly, footprints at binding sites for trans-activating proteins such as C/EBP, NF-κB, and ETS were only found upon stimulation with LPS and IFN-γ. In contrast, a footprint over a purine-rich sequence at −155, termed GA-12 (GATA sequence in the IL-12 promoter), was observed in resting, but not activated, cells. Further characterization of this site revealed specific complex formation at a protected GATA core motif in unstimulated primary monocytes and RAW264.7 macrophages. Mutagenesis within the GA-12 sequence caused a significant up-regulation of inducible IL-12 p40 promoter activity in both transient and stable transfection systems, suggesting a repressor function of this site. Furthermore, binding activity of the GA-12 binding protein GAP-12 was increased by treatment with two potent inhibitors of IL-12 expression, IL-4 and PGE2. Finally, we observed that IL-4-mediated repression of IL-12 p40 promoter activity is critically dependent on an intact GA-12 sequence. In summary, our data underline the complex regulation of the human IL-12 p40 promoter and identify GA-12 as a potent, novel repressor element that mediates IL-4-dependent suppression of inducible promoter activity in monocytes. Regulation of GAP-12 binding may thus modulate IL-12 p40 gene expression.
The Effects of Fatiguing Aerobic Exercise on the Cerebral Blood Flow and Oxygen Extraction in the Brain: A Piloting Neuroimaging Study

Abstract:The fatigue in aerobic exercise affects the task performance. In addition to the fatigue in the muscular system, the diminished performance may arise from the altered cerebral blood supply and oxygen extraction. However, the effects of the fatiguing aerobic exercise on the ability of brain to regulate the cerebral blood flow (CBF) and to extract the oxygen are not fully understood. In this pilot study, we aim to quantify such effects via advanced functional MRI techniques. Twenty healthy younger elite athletes were recruited. In the screening visit, one circle ergometer test was used to screen the maximal relative oxygen consumption (VO2max). Eleven eligible participants then completed the next MRI visit after 7 days. These participants completed a 2-min pulsed arterial spin labeling (ASL) using the PICORE/QUIPSS II and 5-min asymmetric spin echo (ASE) scan at baseline and immediately after the aerobic circle ergometer test. The CBF was then measured using the ASL images and the oxygen consumption of the brain was quantified using oxygen extraction fractions (OEF) derived from the ASE images. The test time, VO2max, and anaerobic threshold were also recorded. As compared to baseline, participants had significant reduction of global CBF (p = 0.003). Specifically, the CBF in bilateral striatum, left middle temporal gyrus (MTG) and right inferior frontal gyrus (IFG) decreased significantly (p < 0.005, K > 20). No significant changes of the OEFs were observed. Participants with greater OEF within the right striatum at baseline had longer test time, greater anaerobic threshold and relative VO2max (r2 > 0.51, p < 0.007). Those with longer test time had less reduction of CBF within the right IFG (r2 = 0.55, p = 0.006) and of OEF within the left striatum (r2 = 0.52, p = 0.008). Additionally, greater anaerobic threshold was associated with less reduction of OEF within the left MTG (r2 = 0.49, p = 0.009). This pilot study provided first-of-its-kind evidence suggesting that the fatiguing aerobic exercise alters the cerebral blood supply in the brain, but has no significant effects on the ability of brain to extract oxygenation. Future studies are warranted to further establish the CBF and OEF as novel markers for physical and physiological function to help the assessment in the sports science and clinics.

Abstract:Key points Aerobic exercise elicits increases in cerebral blood flow (CBF) as well as core body temperature; however, the isolated influence of temperature on CBF regulation during exercise has not been investigated This study assessed CBF regulation and neurovascular coupling during submaximal cycling exercise and temperature‐matched passive heat stress during isocapnia (i.e. end‐tidal PCO 2 was held constant) Submaximal cycling exercise and temperature‐matched passive heat stress provoked approximately 16% increases in vertebral artery blood flow, independent of changes in end‐tidal PCO 2 and blood pressure External carotid artery blood flow increased by approximately 43% during both exercise and passive heat stress with no change in internal carotid artery blood flow Neurovascular coupling (i.e. the relationship between local increases in cerebral metabolism and appropriately matched increases in regional cerebral blood flow) is preserved during both exercise and temperature‐matched passive heat stress Acute moderate‐intensity exercise increases core temperature (Tc; +0.7‐0.8°C); however, such exercise increases cerebral blood flow (CBF; +10‐20%) mediated via small elevations in arterial PCO 2 and metabolism. This study aimed to isolate the role of Tc from PCO 2 on CBF regulation during submaximal exercise. Healthy adults ( n = 11; 10M/1F; 26 ± 4 years) participated in two interventions each separated by ≥48 h: 1) 60 mins semi‐recumbent cycling (EX; 50% workload max); and 2) 75 mins passive heat stress (HS; 49°C water‐perfused suit) to match the exercise‐induced increases in Tc (EX: Δ0.75 ± 0.33°C vs. HS: Δ0.77 ± 0.33°C, P = 0.855). Blood flow ( Q ) in the internal and external carotid (ICA and ECA, respectively) and vertebral (VA) arteries (Duplex ultrasound) was measured. End‐tidal PCO 2 and PO 2 were effectively clamped to resting values within each condition. The Q ICA was unchanged with EX and HS interventions ( P = 0.665), consistent with the unchanged end‐tidal PCO 2 ( P = 0.327); whereas, Q VA was higher throughout both EX and HS (EX: Δ16±21% vs. HS: Δ16±23%, time effect: P = 0.006) with no between condition differences ( P = 0.785). These increases in Q VA contributed to higher global CBF throughout both EX and HS (EX: Δ12 ± 20% vs. HS: Δ14±14%, time effect: P = 0.029; condition effect: P = 0.869). The Q ECA increased throughout both EX and HS (EX: Δ42 ± 58% vs. HS: Δ53 ± 28%, time effect: P < 0.001; condition effect: P = 0.628). Including blood pressure as a covariate did not alter these CBF findings (all P > 0.05). Overall, these data provide new evidence for temperature‐mediated elevations in posterior CBF during exercise that are independent of changes in PCO 2 and blood pressure. Abstract:Applied Physiology, Nutrition, and Metabolism, Ahead of Print. Cerebral hemodynamics have been quantified during exercise via transcranial Doppler ultrasound, as it has high-sensitivity to movement artifacts and displays temporal superiority. Currently, limited research exists regarding how different exercise modalities and postural changes impact the cerebrovasculature across the cardiac cycle. Ten participants (4 females and 6 males) ages 20–29 completed three exercise tests (treadmill, supine, and upright cycling) to volitional fatigue. Physiological data collected included middle cerebral artery velocity (MCAv), blood pressure (BP), heart rate, and respiratory parameters. Normalized data were analyzed for variance and effect sizes were calculated to examine differences between physiological measures across the three exercise modalities. Systolic MCAv was greater during treadmill compared to supine and upright cycling (p < 0.001, (large) effect size), and greater during upright versus supine cycling (p < 0.017, (large)). Diastolic MCAv was lower during treadmill versus cycling exercise only at 60% maximal effort (p 0.05, (negligible)). Systolic BP was lower during treadmill versus supine cycling at 40% and 60% intensity (p < 0.05, (moderate–large)) and greater during supine versus upright at only 60% intensity (p < 0.003, (moderate)). The above differences were not explained by partial pressure of end-tidal carbon dioxide levels (main effect: p = 0.432). The current study demonstrates the cerebrovascular and cardiovascular systems respond heterogeneously to different exercise modalities and aspects of the cardiac cycle. As physiological data were largely similar between tests, differences associated with posture and modality are likely contributors.

Abstract:We quantified the relationship between the change in post-contraction blood flow with motor unit firing rates and metrics of fatigue during intermittent, sub-maximal fatiguing contractions of the knee extensor muscles after stroke. Ten chronic stroke survivors (>1-year post-stroke) and nine controls participated. Throughout fatiguing contractions, the discharge timings of individual motor units were identified by decomposition of high-density surface EMG signals. After five consecutive contractions, a blood flow measurement through the femoral artery was obtained using an ultrasound machine and probe designed for vascular measurements. There was a greater increase of motor unit firing rates from the beginning of the fatigue protocol to the end of the fatigue protocol for the control group compared to the stroke group (14.97 ± 3.78% vs. 1.99 ± 11.90%, p = 0.023). While blood flow increased with fatigue for both groups (p = 0.003), the magnitude of post-contraction blood flow was significantly greater for the control group compared to the stroke group (p = 0.004). We found that despite the lower magnitude of muscle perfusion through the femoral artery in the stroke group, blood flow has a greater impact on peripheral fatigue for the control group; however, we observed a significant correlation between change in blood flow and motor unit firing rate modulation (r2 = 0.654, p = 0.004) during fatigue in the stroke group and not the control group (r2 = 0.024, p < 0.768). Taken together, this data showed a disruption between motor unit firing rates and post-contraction blood flow in the stroke group, suggesting that there may be a disruption to common inputs to both the reticular system and the corticospinal tract. This study provides novel insights in the relationship between the hyperemic response to exercise and motor unit firing behavior for post-stroke force production and may provide new approaches for recovery by improving both blood flow and muscle activation simultaneously.

Abstract:OBJECTIVE: To examine the acute changes in blood flow and blood pressure of very low load knee extensor exercise (15% one repetition maximum (1RM)) with and without different levels of applied pressure to determine how these effects might differ from high load exercise. We also sought to examine if this differed between men and women.APPROACH: A total of 90 participants (45 men, 45 women) were randomized into a very low load condition with no restriction 15/0, (n = 21), a very low load condition with 40% arterial occlusion pressure (15/40, n = 23), a very low load condition with 80% arterial occlusion pressure (15/80, n = 22), and a traditional high load condition (70/0, n = 24). Pre-post change in blood flow and blood pressure were compared across conditions. Evidence for or against the null hypothesis was quantified using Bayes factors (BF10).MAIN RESULTS: For blood flow, there was no evidence that the changes were different across conditions (BF10: 0.902). However, only the very low load free flow condition (15/0) had evidence to suggest a change (mean, (standard deviation)) from baseline (5.3 (9.1) ml · min-1; BF10: 3.687). Systolic pressure increased ~17 mmHg for the 15/0, 15/40, and 70/0 conditions, with no change in the 15/80 condition. There was no effect of sex for any variable.SIGNIFICANCE: There was substantial variability in our blood flow measurements, making conclusions difficult for this variable. Of note, the blood pressure response was not augmented by blood flow restriction. The hemodynamic changes were also similar between sexes, indicating that men and women were not changing differently.

Abstract:Fluctuating arterial blood pressure during high-intensity interval exercise (HIIE) may challenge dynamic cerebral autoregulation (dCA), specifically post-stroke after an injury to the cerebrovasculature. We hypothesized dCA would be attenuated at rest and during a sit-to-stand, immediately following and 30 min after HIIE in individuals post-stroke compared to age- and sex-matched controls (CON). HIIE switched every minute between 70% and 10% estimated maximal watts for 10 min. Mean arterial pressure (MAP) and middle cerebral artery blood velocity (MCAv) were recorded. dCA was quantified during spontaneous fluctuations in MAP and MCAv via transfer function analysis. For sit-to-stand, time delay before an increase in cerebrovascular conductance index (CVCi = MCAv/MAP), rate of regulation, and %change in MCAv and MAP were measured. Twenty-two individuals post-stroke (age 60±12 yrs, 31±16 months) and twenty-four CON (age 60±13 yrs) completed the study. Very low frequency (VLF) gain (p=0.02, η 2 =0.18) and normalized gain (p=0.01, η 2 =0.43) had a group-by-time interaction, with CON improving following HIIE, while individuals post-stroke did not. Individuals post-stroke had lower VLF phase (p=0.03, η 2 =0.22) following HIIE compared to CON. We found no differences in the sit-to-stand measure of dCA. Our study showed lower dCA during spontaneous fluctuations in MCAv and MAP following HIIE in individuals post-stroke compared to CON, while the sit-to-stand response was maintained.

Abstract:The circulatory effects of lifting and holding weighted suitcases of 5, 10 and 15 kg, and carrying the same suitcases at two different walking speeds were evaluated in 6 normal subjects before and after beta-adrenoceptor blockade with 160 mg oxprenolol. Lifting and holding induced significant weight-induced increments in systolic and diastolic blood pressure which were not attenuated by beta-blockade. There was a significant increase in heart rate only on lifting the 15 kg weight which was attenuated by oxprenolol. Dynamic exercise (walking) with the same weighted suitcases resulted in rate and speed related increase in systolic pressure and heart rate, the magnitude of which was greater than that of lifting alone. The isometric pressor response was attenuated when walking and carrying at 2 mph, but completely abolished by the metabolic and heat induced vasodilatation when walking at 4 mph. Following beta-blockade both the absolute blood pressure and the systolic pressor and heart rate responses to combined lifting and carrying were attenuated. The diastolic pressor response induced by lifting, which was offset in the control period by the vasodilation induced by dynamic exercise was progressively attenuated proportionate to the load carried following beta-blockade; presumably this reflected systemic vasoconstriction to maintain mean perfusion pressure in the presence of central beta-blockade. These observations suggest that the isometric component which predominates at slow walking speeds when carrying weights between 5 and 15 kg is completely suppressed by the vasodilatation at fast walking speeds. Beta-blockade, while reducing heart rate and systolic pressor response to the same stimuli, leads to an augmented systemic vascular resistance which is particularly evident at high metabolic workloads.
