Abstract:Laboratory medicine is complex and contributes to the diagnosis, therapeutic monitoring and follow-up of acquired and inherited human disorders. The regular practice of physical exercise provides important benefits in heath and disease and sports medicine is thereby receiving growing focus from almost each and every clinical discipline, including laboratory medicine. Sport-laboratory medicine is a relatively innovative branch of laboratory science, which can provide valuable contributions to the diagnosis and follow-up of athletic injuries, and which is acquiring a growing clinical significance to support biomechanics and identify novel genomics and "exercisenomics" patterns that can help identify specific athlete's tendency towards certain types of sport traumas and injuries. Laboratory medicine can also provide sport physicians and coaches with valuable clues about personal inclination towards a certain sport, health status, fitness and nutritional deficiencies of professional, elite and recreational athletes in order to enable a better and earlier prediction of sport injuries, overreaching and overtraining. Finally, the wide armamentarium of laboratory tests represents the milestone for identifying cheating athletes in the strenuous fight against doping in sports.

Abstract:Abstract The need to evaluate the health status of an athlete represents a crucial aim in preventive and protective sports science in order to identify the best diagnostic strategy to improve performance and reduce risks related to physical exercise. In the present review we aim to define the main biochemical and haematological markers that vary significantly during and after sports training to identify risk factors, at competitive and professional levels and to highlight the set up of a specific parameter’s panel for elite athletes. Moreover, we also intend to consider additional biomarkers, still under investigation, which could further contribute to laboratory sports medicine and provide reliable data that can be used by athlete’s competent staff in order to establish personal attitudes and prevent sports injuries.

Abstract:Background: Laboratory diagnostics represent a valuable tool for the optimization and assessment of the performance and regeneration ability in professional athletes. Blood parameters play an important role in the prevention, diagnosis and rehabilitation of injuries and physical overload. Objectives: The aim of this article is to present an overview of musculoskeletal laboratory parameters and to provide relevant information for the medical care of competitive athletes. Methods: Literature search and narrative review. Results: The laboratory assessment of bone metabolism includes vitamin D, calcium and bone turnover and aims to provide a preventive benefit with respect to skeletal complications (e.g., to minimize the risk of bone stress injuries). In addition, muscular serum markers, such as lactate dehydrogenase (LDH), creatine kinase (CK), myoglobin and aspartate aminotransferase (AST) can be used to monitor metabolic adaptation to physical exercise and to obtain information about the muscular workload and potential damage. The energy availability can be estimated and optimized by appropriate balancing and laboratory determination of macro- and micronutrients. Conclusions: Laboratory diagnostics have a clinical relevance across different sport disciplines. They are intended to support athletes and medical staff on their way to the highest possible performance and help to ensure the optimal prevention of bone and muscle injuries. Parameters with deficiency results (e.g., vitamin D) should be adequately compensated. A periodization of the laboratory tests, with at least two tests per year, and the establishment of individual variability and reference ranges can improve the assessment.

Abstract:Abstract Objectives In 2019 The Croatian Working Group for Laboratory Hematology, on behalf of the Croatian Society of Medical Biochemistry and Laboratory Medicine, wanted to explore the background in field of laboratory hematology routine practice among Croatian laboratories in order to develop future strategies for producing national recommendations, if needed. Methods During April and May 2019, a comprehensive survey covering all main parts of the total testing process within the field of laboratory hematology among Croatian medical laboratories was conducted. The survey comprised 49 inquiries. Data was collected using Survey Monkey (Palo Alto, CA, USA). All collected data was anonymized. Results The response rate was 72%. There is still a substantial number of laboratories that have only three-part differential hematology analyzers (9%). Furthermore, a very high number of laboratories did not perform analyzer verification prior to implementation into routine work (31%). Out of those who have verified their analyzers, a diversity of guidelines and recommendations were used. Nearly 10% of the laboratories do not have a defined policy regarding specimen rejection. The majority of the participants perform internal quality control daily (83%), however, only 51% of respondents evaluate the agreement between different hematology analyzers on daily basis. Although more than 90% of Croatian laboratories have a defined policy regarding specimen rejection, only 61% of respondents continuously monitor quality indicators in routine practice. Conclusions The survey revealed substantial differences in all aspects of laboratory hematology practices among Croatian medical laboratories, indicating the need for universal recommendations at the national level.

Abstract:Background and aims: Laboratory performance as a relative concept needs repetitive benchmarking for continuous improvement of laboratory procedures and medical processes. Benchmarking as such establishes reference levels as a basis for improvements efforts for healthcare institutions along the diagnosis cycle, with the patient at its center. But while this concept seems to be generally acknowledged in laboratory medicine, a lack of practical implementation hinders progress at a global level. The aim of this study was to examine the utility of a specific combination of indicators and survey-based data collection approach, and to establish a global benchmarking dataset of laboratory performance for decision makers in healthcare institutions. Methods: The survey consisted of 44 items relating to laboratory operations in general and three subscales identified in previous studies. A global sample of laboratories was approached by trained professionals. Results were analyzed with standard descriptive statistics and exploratory factor analysis. Dimensional reduction of specific items was performed using confirmatory factor analysis, resulting in individual laboratory scores for the three subscales of "Operational performance," "Integrated clinical care performance," and "Financial sustainability" for the high-level concept of laboratory performance. Results and conclusions: In total, 920 laboratories from 55 countries across the globe participated in the survey, of which 401 were government hospital laboratories, 296 private hospital laboratories, and 223 commercial laboratories. Relevant results include the need for digitalization and automation along the diagnosis cycle. Formal quality management systems (ISO 9001, ISO 15189 etc.) need to be adapted more broadly to increase patient safety. Monitoring of key performance indicators (KPIs) relating to healthcare performance was generally low (in the range of 10-30% of laboratories overall), and as a particularly salient result, only 19% of laboratories monitored KPIs relating to speeding up diagnosis and treatment. Altogether, this benchmark elucidates current practice and has the potential to guide improvement efforts and standardization in quality & safety for patients and employees alike as well as sustainability of healthcare systems around the globe.

Abstract:The article describes the main biomarkers for determining human adaptation to physical activity. Analyzed modern scientific publications on the criteria of individual tolerance of food products, studied promising directions of personalized correction of the diet. The authors have formed a group of valid (sports-specific) laboratory indicators, it is proposed to pay close attention to the reference values of laboratory equipment that have “their own” norms. A promising direction of laboratory diagnostic work is the formation of reliable and accessible complementary criteria that are at the “junction” of functional and instrumental diagnostic methods. Metabolomics is viewed as a young but highly effective science for detecting highly specific metabolic characteristics of human health. The authors proposed the main directions of scientific research in laboratory diagnostics in sports.
95th Anniversary of Pathophysiology in Croatia

Abstract:University level of Pathophysiology research and teaching in Croatia had started with the third year of Medical School of Zagreb in academic year 1919./20. Ever since, despite historical changes of the main university stake holder, the state of Croatia, Department of Pathophysiology development progressed and has made visible academic achievements, with a broader effect in medical community. The first 95 years of academic tradition and major achievements are shortly described in this paper. Professor Miroslav Mikuličić envisioned Pathophysiology in close relations with Pharmacology and made the pioneering steps of establishing the "double" department at Šalata. His group was academically very pro-active, with strong international scientific participation and recruitment of professionals. The group published the first voluminous textbook of Pharmacology and Pathophysiology, in Croatian. In fifties, professor Pavao Sokolić established clinical pathophysiology within the Hospital Centre at Rebro. Out of "double" department two new departments were founded, the Pathophysiology one was completed with the clinical ward. That institutional move from Šalata hill to the Rebro hill was a necessary gigantic step and a prerequisite for the proper further development. It was in accordance with the concept of the Mikuličić's program of Pathophysiology from 1917. Pavao Sokolić has been remembered for his visions, deep insights into etiopathogenesis, ability to transfer knowledge and friendly relations to students. Sharp intellectual power, emanating charisma, academic erudition and unique clinical competencies made the legendary image of the "Teacher" - as students used to refer to him with admiration. He was second to no one when complex patient issues were to be resolved. Clinical Hospital Centre Zagreb and his Department at Rebro have become a referral point to whom to go to despair. Students recognized in their Teacher the landmark of Croatian medicine, which made a lasting legacy on generations to come. Professor Stjepan Gamulin made molecular medicine the working reality at Rebro. Both in clinical research, and in health system as diagnostic service and tool for all centers in Croatia, molecular measurement in tissue samples came into usage in daily physicians reasoning and therapy prescriptions. Macromolecular aspects of disease have come of age and became clinimetric signs of patients' condition. Professor Gamulin with his group and associated authors wrote the textbook of pathophysiology, which in upcoming 30 years had 7 editions, has become the bestseller in medicine. The textbook was translated and published in English and Albanian. In the most recent book professor Gamulin turned the focus of medical community to clinical epidemiology and a need for retrospective insights into medical efficiency. Medical performance can be improved with the improvement of understanding of underlying etiopathogenetic relations as the foundation of therapy-is the main message. Following the academic legacy and spirit of three charismatic authorities we established two methods of teaching/learning in medicine. The two methods opened up a new avenue, so important for the era of postgenomic plethora of information and demands of precision/personalized medicine. Methodology has been introduced timely. It is student-friendly and usable for advanced types of education. Problem based algorhytmic matrices stimulate analysis and resynthesis of etiopathogenetic pathways. Graphic presentation of the solution integrates horizontal, vertical and longitudinal aspects of the problem. The companion textbook in the form of problem solver has been published in 3 editions, and contains 128 study solved cases. It was published in English, as well. Out of algorhythmic analysis the etiopathogenetic clusters (EPCs) are composed of etiopathogenetic pathway analysis. EPCs are natural units of disease development, the crossing points of processes. They are integrative hubs which tend to make networks of EPCs. Four volume textbook has been published, which elaborates 91 EPCs with 1165 study cases. Unique approach in the first 95 years was defined as Zagreb School of Pathophysiology. It made visible effect outside academia and recognizable image at the international level, in scientific, educational and practical aspects of activities.

Abstract:The ultimate goal of value-based laboratory medicine is maximizing the effectiveness of laboratory tests in improving patient outcomes, optimizing resources and minimizing unnecessary costs. This approach abandons the oversimplified notion of test volume and cost, in favor of emphasizing the clinical utility and quality of diagnostic tests in the clinical decision-making. Several key elements characterize value-based laboratory medicine, which can be summarized in some basic concepts, such as organization of in vitro diagnostics (including appropriateness, integrated diagnostics, networking, remote patient monitoring, disruptive innovations), translation of laboratory data into clinical information and measurable outcomes, sustainability, reimbursement, ethics (e.g., patient empowerment and safety, data protection, analysis of big data, scientific publishing). Education and training are also crucial, along with considerations for the future of the profession, which will be largely influenced by advances in automation, information technology, artificial intelligence, and regulations concerning in vitro diagnostics. This collective opinion paper, composed of summaries from presentations given at the two-day European Federation of Laboratory Medicine (EFLM) Strategic Conference "A vision to the future: value-based laboratory medicine" (Padova, Italy; September 23-24, 2024), aims to provide a comprehensive overview of value-based laboratory medicine, projecting the profession into a more clinically effective and sustainable future.
Blood Biomarker Profiling and Monitoring for High-Performance Physiology and Nutrition: Current Perspectives, Limitations and Recommendations

Abstract:Blood test data were traditionally confined to the clinic for diagnostic purposes, but are now becoming more routinely used in many professional and elite high-performance settings as a physiological profiling and monitoring tool. A wealth of information based on robust research evidence can be gleaned from blood tests, including: the identification of iron, vitamin or energy deficiency; the identification of oxidative stress and inflammation; and the status of red blood cell populations. Serial blood test data can be used to monitor athletes and make inferences about the efficacy of training interventions, nutritional strategies or indeed the capacity to tolerate training load. Via a profiling and monitoring approach, blood biomarker measurement combined with contextual data has the potential to help athletes avoid injury and illness via adjustments to diet, training load and recovery strategies. Since wide inter-individual variability exists in many biomarkers, clinical population-based reference data can be of limited value in athletes, and statistical methods for longitudinal data are required to identify meaningful changes within an athlete. Data quality is often compromised by poor pre-analytic controls in sport settings. The biotechnology industry is rapidly evolving, providing new technologies and methods, some of which may be well suited to athlete applications in the future. This review provides current perspectives, limitations and recommendations for sports science and sports medicine practitioners using blood profiling and monitoring for nutrition and performance purposes.

Abstract:Current literature review provides an evaluation of advantages and limitations of biochemical control objects representing functional state of athletes as well as the outlook for using alternative targets regarding sports medicine. Traditionally, invasive procedures (venous blood collection, muscle biopsy) have been known as the gold standard for analyzing a wide range of biomarkers which could be employed as effective diagnostic tools to control the course of adaptation processes, monitor performance, overtraining and physical well-being of athletes, but these techniques are painful, time-consuming and place demands on storage and shipment. In this behalf finding an alternative objects for biochemical research that does not have disadvantages given above is the question of present interest. Saliva and dry blood spots (DBS) could serve as equally informative and promising targets for monitoring athletes' condition. The non-invasive nature of saliva collection allows to shorten sample collection time, reduce stress hormones levels and possible infection contamination. Moreover, collecting saliva process does not require special equipment and trained medical staff which is particularly important when athletes are at training camps. The DBS method has successfully proven itself with regard to neonatal screening and pharmacokinetics studies. Its key benefits are simplicity, small volume of bioliquid, enhanced stability of adsorbed biomarkers on the card surface, lack of special storage and transportation requirements and low costs for samples shipment to the laboratory. Taken together outlined advantages will provide the opportunity to increase the frequency of biomaterial collection to perform selective observation of training loads effects on various systems of athletes' body. The combination of DBS with immunochemical and mass-spectrometric approaches could serve as an efficient instrument to investigate the role of various biomarkers in monitoring the functional state of athletes. Abstract:The main task of an academic department of any medical discipline is an integrated development and improvement of professional, scientific and educational activities. The proces of formulation, establishment and recognition of general practice/family medicine as an academic discipline in Croatia started with postgraduate education and vocational training in 1960 at the Andrija Stampar School of Public Health. In 1980 Department of Family Medicine of the Medical School, University of Zagreb was officially established. The aim of this paper was to describe the characteristics of general practice/family medicine as a scientific discipline with its proper, independent knowledge, acquired in scientific research and applied in practice. Based on literature review, specific areas and methodology of scientific research in general practice/family medicine are presented. Scientific research in general practice/family medicine has traditionally included epidemiological, clinical, organisational and other aspects of work of general practitioners/family physicians comprising activities directed towards patient and community care, as well as towards the position and responsibilities within a health care system. Department of Family Medicine carried out its own scientific projects, participated in international scientific projects and scientific projects implemented with other partners. Some of these projects were presented in this paper with the purpose to give a brief insight into the development and current state of the scientific work within the Department of Family Medicine.

Abstract:60 years ago, Jokl addressed the nascent profession of sports medicine with the question 'What is Sports medicine?' [sic]. Is there a danger for myopia in anti-doping education? Comparative analysis of substance use and misuse in Olympic racket sports calls for a broader approach

Abstract:Background: Racket sports are typically not associated with doping. Despite the common characteristics of being non-contact and mostly individual, racket sports differ in their physiological demands, which might be reflected in substance use and misuse (SUM). The aim of this study was to investigate SUM among Slovenian Olympic racket sport players in the context of educational, sociodemographic and sport-specific factors. Methods: Elite athletes (N=187; mean age=22±2.3; 64% male) representing one of the three racket sports, table tennis, badminton, and tennis, completed a paper-and-pencil questionnaire on substance use habits. Athletes in this sample had participated in at least one of the two most recent competitions at the highest national level and had no significant difference in competitive achievement or status within their sport. Results: A significant proportion of athletes (46% for both sexes) reported using nutritional supplements. Between 10% and 24% of the studied males would use doping if the practice would help them achieve better results in competition and if it had no negative health consequences; a further 5% to 10% indicated potential doping behaviour regardless of potential health hazards. Females were generally less oriented toward SUM than their male counterparts with no significant differences between sports, except for badminton players. Substances that have no direct effect on sport performance (if timed carefully to avoid detrimental effects) are more commonly consumed (20% binge drink at least once a week and 18% report using opioids), whereas athletes avoid substances that can impair and threaten athletic achievement by decreasing physical capacities (e.g. cigarettes), violating anti-doping codes or potentially transgressing substance control laws (e.g. opiates and cannabinoids). Regarding doping issues, athletes' trust in their coaches and physicians is low. Conclusion: SUM in sports spreads beyond doping-prone sports and drugs that enhance athletic performance. Current anti-doping education, focusing exclusively on rules and fair play, creates an increasingly widening gap between sports and the athletes' lives outside of sports. To avoid myopia, anti-doping programmes should adopt a holistic approach to prevent substance use in sports for the sake of the athletes' health as much as for the integrity of sports.

Abstract:The use of performance‐enhancing substances in sports undermines values and risks athletes' health. This study analyzes doping literature, identifying influential documents and research domains. While prevalent doping classes are well‐studied, emerging trends like contaminated supplements receive less attention. Technological advancements drive sophisticated detection techniques. The study emphasizes the need for awareness campaigns among athletes. The use of performance‐enhancing substances not only undermines the core values of sports but also poses significant health risks to athletes. In a fast‐evolving doping environment, where sport professionals are constantly seeking novel and illegal means to bypass doping tests, and new substances are regularly detected on the drug market, it is crucial to inform authorities with updated evidence emerging from scientific research. The current study aims to (i) outline the structure of knowledge in the literature on performance enhancers in sports (i.e., most active countries, main sources, most productive authors, and most frequently used keywords); (ii) identify the most impactful documents in the field; and (iii) uncover the main domains of research in the literature. To do so, we conducted a comprehensive scientometric analysis of the literature on doping, sourcing our data from Scopus. Our research involved a document co‐citation analysis of 193,076 references, leading to the identification of the 51 most influential documents and seven key thematic areas within the doping literature. Our results indicate that the scientific community has extensively studied the most prevalent doping classes, such as anabolic agents and peptide hormones, and little is still known about the use of contaminated supplements or other types of enhancers identified as emergent trends. Concurrently, technological advancements contributed to the development of more sophisticated doping detection techniques, using blood or urine samples. More recently, the focus has shifted towards the athlete biological passport, with research efforts aimed at identifying biomarkers indicative of doping. The dynamic nature of doping methods underlines the necessity for more robust educational campaigns, aiming at raising awareness among sports professionals and their entourage about the dangers of doping and the intricacies of its control mechanisms.

Abstract:Communication &Sport, Ahead of Print. This article provides an overview and analysis of how the study of sport at the intersection with the media has developed in the Slovenian context over the last three decades. The first part of the article briefly explains the importance and role of sport and media in the broader social context. By looking in particular at sports journalism and introducing the Slovenian context, the purpose of the study is clarified. The second part identifies and reviews all academic research contributions published on the highlighted intersection and during the selected period. Based on a quantitative and qualitative analysis of the most frequently addressed aspects of the sports media system, the main research topics identified and the media outlets studied, the main findings are summarized and highlighted. While studies addressing different aspects of nationality predominate, attention to gendered nationalism is highlighted as particularly valuable in the context of studies focusing on gender and its intersections. Event-oriented and decontextualized sports content is problematized in the context of recent studies focusing on some aspects of sports journalism. In the final section of the article, some guidelines are formulated to encourage further research in (and of) this geographical area (and beyond).

Abstract:Since ancient times sports has drawn the attention of scientists from a large number of disciplines, including social sciences, physiology, medicine and rehabilitation. Although it is now unquestionable that regular performance of physical exercise is associated with a lower risk of developing many human diseases such as cancer, cardiovascular disorders, diabetes, rheumatic diseases, sarcopenia and frailty, some issues remain to be addressed. These typically include the amount of exercise needed to improve health and fitness without overcoming the putative health befits, along with the insufficient recognition of biological pathways that can be modulated by active lifestyle. The still incomplete knowledge of these aspects is contributing to the growth of several scientific initiatives, specifically aimed to study changes and individual adaptations underlying regular sport practice. The bases, context and early achievements of one of such initiatives, called “Run for Science”, will be summarized in this article. In particular, the various editions of this event have allowed to make new discoveries and tangible advancements in human biology and personalized laboratory medicine, including the observation that cardiac troponins transitorily increase in blood after endurance exercise, that running performance is influenced by two previously unappreciated laboratory parameters (i.e., serum alpha-amylase and mean platelet volume) and that the DNA may undergo a transitory oxidative damage during exercise. More recently, the information gathered from the Run for Science helped discovering important aspects linking physical exercise with the lower risk of cancer, cardiovascular disease and asthmatic reactions. Additional analyses are underway, and specific focus will be placed on sports epigenetics, with the aim to define how recreational sport interplays with gene expression and modifies the individual response to exercise.

Abstract:Positive trends in the development of medical and technological sciences are associated with an increase in the costs of the health care system, which have increased in many countries over the last decade and, accordingly, increased the cost of medical care, the purchase of modern medical equipment, the introduction of pharmacological treatments, etc. This caused a significant increase in investments in research and implementation of developments in the latest promising medical and technological directions, for example, such as information medical technologies. In developed countries, where the total "consumption of medical devices" is tens of billions of dollars per year, the costs of medical information technology reach 10-15 percent of the total costs of the health care system. Over the last decade, this indicator has more than doubled, which equated it with the amount of investments in medical products, including in the practice of sports medicine. The purpose of the research is to determine the directions of introduction of biomedical technologies into the practice of modern sports medicine. Research materials and methods: analysis and generalization of data from special scientific and methodical literature; monitoring of Internet information resources; systematization method; content analysis. The results. It was determined that the current trends and development directions of the application of biomedical engineering in the practice of sports medicine are the development of the latest biomedical devices and technologies to improve the effectiveness of training and preserve the health of athletes; using artificial intelligence to analyze large volumes of data and improve the training process; to create personalized training programs and adapt to the individual needs of athletes; development of professional prostheses and exoskeletons for persons with limited mobility. Conclusions. It has been established that simultaneously with the differentiation of sciences, their increasing specialization on traditional boundaries, new interdisciplinary directions appear, which become bridges of mutual penetration of different areas of knowledge in the name of universal human progress. Medical and technical sciences, such as biomedical cybernetics, bionics, biomechanics and others, are also an example of the interpenetration of medical and technical sciences for the purpose of learning the nature of health, human diseases, understanding the mechanisms of diagnosis, treatment and prevention of body injuries, including in the practice of sports of medicine.

Abstract:In 2019, three regulations of the Minister of Health regarding sports medicine examinations in children, adolescents and young athletes came into force. The publication presents in detail the current scope of tests and the frequency of required medical examinations necessary to obtain medical certificates qualify- ing patients to practice sports. The publication also presents the Regulation of the Minister of Health on the required qualifications of doctors authorized to issue medical certificates to athletes. It is very important to properly assess the health of potential and current athletes to ensure their safety while participating in sports competitions. There are diseases that increase the risk of sudden cardiac death which doctors should keep in mind when qualifying athletes for competition. The publication draws attention to the underestimated role of echocardiography and electrocardiographic stress testing.
