Abstract:Objective: To test the wear and fatigue resistance of three materials (Essix ACE®, Taglus®, and Zendura A®) for the fabrication of vacuum-formed retainers in an artificial oral environment. Material and methods: Wear resistance was tested by subjecting 21 retainers of each Essix ACE®, Taglus®, and Zendura A® to 12,000 wear cycles at 75N to simulate one year of retainer wear with moderate nighttime bruxing. Post-wear retainer thickness was compared to baseline measurements to calculate wear depth. Fatigue resistance was tested by flexing 15 retainers of each material at an angle of 25 degrees for 1,825 cycles to simulate one year of removing and reinserting a retainer five times per day. Retainers were visually inspected for fractures. Pairwise t-tests with correction using Tukey's method were used to determine significant differences between materials. Results: The mean wear depths were 0.155±0.021mm, 0.168±0.031mm, and 0.096±0.033mm for Essix ACE®, Taglus®, and Zendura A®, respectively. The wear depth of Zendura A® was significantly lower than that of both Essix ACE® (P<0.001) and Taglus® (P<0.001). There was no significant difference in wear depth between Essix ACE® and Taglus® (P=0.312). Under the parameters set for the fatigue resistance test, fractures did not occur on any of the tested materials. Conclusions: Under the assumption of moderate nighttime bruxing for one year, Zendura A® is the most wear-resistant among the materials tested. With the assumption of retainer removal and reinsertion five times per day for one year, all three materials tested have the same ability to resist fatigue.
Evaluation of the Distortion of Essix Retainer Formed From Biostar Machine Using Intraoral Digital Scans: An In Vitro Study

Abstract:Introduction Retention is essential to prevent unwanted tooth movement due to growth changes, to allow the gingival and periodontal tissues affected by orthodontic treatment to realign, and to stabilize teeth that have been moved to potentially unstable positions, thus reducing the risk of relapse. This study aimed to evaluate the distortion of Essix retainers over time to enhance their retention and stability. Methods Patients who visited the Department of Orthodontics and Dentofacial Orthopedics at Ranjeet Deshmukh Dental College & Research Centre, Nagpur, India, after completing their orthodontic treatment were included in the study, according to the established inclusion and exclusion criteria. A total of 26 patients participated. Each patient received an Essix retainer fabricated from a Duran+ Biostar round sheet (1 mm thickness) using a Biostar machine based on their post-debonded maxillary cast. The patients were instructed on the correct insertion and removal of the Essix retainer. The inner surface of the retainers was scanned at one month, three months, and six months using an intraoral digital scanner. These scans were analyzed and compared for distortion at different time intervals using Medit software. Results The Essix retainers exhibited varying degrees of distortion at different time intervals. Notably, distortion was more significant in the posterior region compared to the anterior region. Additionally, distortion increased over time, with the least amount observed at one month and progressively worsening by the sixth month. Specifically, the average distortion in the posterior region ranged from 0.133 mm after the first month to 0.304 mm after six months. In contrast, the average distortion in the anterior region was lower, ranging from 0.057 mm at one month to 0.068 mm at six months. Conclusions Distortion was more pronounced on the posterior surface of the Essix retainer compared to the anterior region.
Retentive force variations in three polyamide and a polyetheretherketone denture attachments for implant-supported overdentures: An in vitro study

Abstract:Statement of problem: The loss in the retentive capability of implant-supported overdenture attachments has been related to the wear of the retentive inserts. Wear of the abutment coating material when following the replacement period for the retentive inserts requires investigation. Purpose: The purpose of this in vitro study was to compare the changes in retentive force of 3 polyamide and a polyetheretherketone denture attachments under repeated insertion and removal cycles in wet conditions while following their manufacturers' replacement time recommendations. Material and methods: Four different denture attachments (LOCKiT, OT-Equator, Ball attachment, and Novaloc) with their retentive inserts were tested. Four implants were embedded into individual acrylic resin blocks, and 10 abutments for each attachment were used. Forty metal housings with their retentive inserts were attached to polyamide screws with autopolymerizing acrylic resin. A customized universal testing machine was used to simulate insertion and removal cycles. The specimens were mounted on a second universal testing machine at 0, 540, 2700, and 5400 cycles, and the maximum retentive force was recorded. The retentive inserts for LOCKiT (light retention), OT-Equator (soft retention), and Ball attachment (soft retention) were replaced every 540 cycles, while the Novaloc (medium retention) attachments were never replaced. All the abutments were weighed with a precision scale at 0, 2700, and 5400 cycles. The surface of every abutment was examined under a stereomicroscope at ×10 magnification. Data were analyzed with descriptive statistics. Two-way repeated measures ANOVA was used to compare the mean retentive force and mean abutment mass of all groups and time evaluation points. Bonferroni corrections were made to adjust for multiple tests (α=.05). Results: The mean retention loss for LOCKiT was 12.6% after 6 months and 45.0% after 5 years of simulated use. The mean retention loss for OT-Equator was 16.0% after 6 months and 50.1% after 5 years of simulated use. The mean retention loss for Ball attachment was 15.3% after 6 months and 39.1% after 5 years of simulated use. The mean retention loss for Novaloc was 31.0% after 6 months and 59.1% after 5 years of simulated use. The mean abutment mass difference was statistically significant (P<.05) for LOCKiT and Ball attachment but not statistically significant (P>.05) for OT-Equator and Novaloc at baseline, 2.5 years, and 5 years. Conclusions: All tested attachments recorded retention loss under the experimental conditions, even when their manufacturers' recommendations for replacement time for the retentive inserts were followed. Patients should be aware that implant abutments should be replaced after a recommended period since their surfaces also change over time.
Comparison of chairside rectangular chain retainers and multi-stranded conventional retainers: a randomized controlled trial

Abstract:Background: While retention appliances are widely used in orthodontics, there is still no evidence-based consensus regarding the optimal type of appliance or time of retention. Objectives: To compare chairside rectangular chain retainers, which can be placed in one sitting, with conventional multi-stranded bonded retainers regarding their levels of stability, biological side effects, complications, and patient experiences. Trial design: A single-centre, two-arm, parallel-group randomized controlled trial. Methods: In total, 48 patients were included in this single-centre, randomized controlled trial conducted in Varberg, Region Halland, Sweden. The patients were randomized to two groups: the chairside rectangular chain retainer group, using the Ortho FlexTech retainer (OFT); and the conventional retainer group, using the 0.0195 Penta One multi-stranded spiral wire (PeO). The primary outcome was Little´s irregularity index (LII) evaluated at debond (T0) and at 3 months (T3) and 12 months (T12). The secondary outcomes were inter-canine distance (ICD), plaque index (PI), calculus index (CI), bleeding on probing (BoP), and caries, evaluated at T0, T3, and T12, as well as patients' perceptions, evaluated at T3 and T12, and technical complications that were registered throughout the study period. The Mann-Whitney U-test was used for continuous variables for inter-group comparisons, and the Wilcoxon Signed Rank test was used for intra-group comparisons. Results: There were no statistically significant differences between the groups regarding LII, biological side effects, technical complications, or patients' experiences. However, there was a small but statistically significant difference between the groups regarding the maintenance of the ICD. Within the OFT group, there was a significant increase in CI, and within the PeO group, there was a significant increase in BoP. Conclusions: In terms of clinical relevance, the chairside rectangular chain retainer and the conventional multi-stranded spiral wire provide similar outcomes with respect to the stability of alignment, biological side-effects, technical complications, and patients' experiences short-term. Environmental sustainability of post-orthodontic dental retainers: a comparative life-cycle assessment of Hawley and Essix retainers

Abstract:SummaryBackgroundEnvironmental sustainability has been brought into the limelight due to the global climate crisis. This crisis is driven by human activities and even the healthcare sector is no exception. Within dentistry, orthodontics is a large global market; hence, the use of post-orthodontic retainers has a significant environmental footprint. The aim of this study was to determine the environmental sustainability of post-orthodontic retention using Hawley and Essix retainers.Materials and methodsA comparative life-cycle assessment (LCA) was carried out to compare the environmental impact of both retainers. All inputs and outputs were accounted for using the Ecoinvent database, v3.7.1, and openLCA software. Sixteen impact categories were used to determine their environmental burden.ResultsOf the 16 impact categories, the Hawley had a greater environmental burden than the Essix retainer in 12 categories. The Hawley's most significant contributors to its impact values are factory manufacturing and in-house production, with an average of 41.45% and 52.52%, respectively. For the Essix, the greatest contributor is factory manufacturing, with an average of 64.63%. However, when factoring in the lifespan of the retainers, the Essix would have a greater environmental impact than the Hawley retainer.LimitationsThis study employed a comparative LCA. There were also assumptions made, but these were supported by research.ConclusionsOn the basis of the evidence gathered in this study, Hawley retainers are more environmentally sustainable than Essix retainers. These results would better enable clinicians to factor in the environmental impact and make informed decisions on the choice of retainer type.

The Micromechanical Properties and Surface Roughness of Orthodontic Retainer Wires-An In Vitro Analysis

Abstract:Background: Despite the large variety of retainer wires available, no studies could be found comparing the micromechanical properties and surface roughness of different retainer wires. Such characteristics affect the survival of the fixed retainer in terms of both fracture resistance and resistance to debonding from the tooth. Therefore, the aim of the present study was to examine and compare those characteristics in popular retainer wires. Methods: six different popular orthodontic retainer wires were subjected to instrumental indentation based on the Oliver and Pharr method. The geometric surface structure was analysed using a non-contact profilometer. Results: stainless steel wires had a higher hardness and a higher elastic modulus compared to titanium wires and white gold chain. The titanium wire and the white gold chain showed much more roughness than other wires. Conclusions: stainless steel wires are the most resistant, considering both the shape retention capacity and the ability to resist abrasive wear. The titanium wire showed the lowest hardness and, thus, the highest susceptibility to deformation. Bond-a-braid, Retainium and Orthoflex white gold are more resistant to fracture than other steel wires. Titanium wire and chain retainer wires have more roughness, which is a great advantage in terms of mechanical adhesion to composite materials.

Poly-ether-ether-ketone (PEEK) versus dead-soft coaxial bonded retainers: a randomized clinical trial. Part 1. Stability, retainer failure, and participant satisfaction

Abstract:Background: Poly-ether-ether-ketone (PEEK) was introduced in dentistry as an alternative to metal alloys. Objective: To assess the effectiveness of PEEK-fixed retainers in preserving the stability of mandibular anterior and participant satisfaction as compared to the Dead-soft coaxial fixed retainer (DSC). Trial design: A single-centre, two-arm parallel groups randomized clinical trial. Methods: The patients treated with pre-adjusted orthodontic appliances who have a Little's Irregularity Index (LII) ≤ 0.5 mm have been enrolled in the trial. PEEK retainers were prepared to round 0.8 mm wire by computer-aided design and manufacturing, and the DSC wire was carefully adapted to the lingual surface of the lower anterior teeth. The primary outcome was the stability of lower anterior teeth as assessed by LII, while the secondary outcomes were changes in occlusal parameters, retainer failure, and patient satisfaction. The data were collected at the debonding stage (T0), 1 month (T1), 3 months (T3), and 6 months (T6) after starting the trial, except for patient's satisfaction, which was recorded using an electronic form at T1 and T6. Blinding: Single blinding of participants. Results: A total of 46 participants with an age range of 12-28 years old were randomly allocated to the two groups (n = 23 in each). Only one participant dropped out; therefore, 45 participants were analysed. The DSC group showed a significant increase in LII at T3. Both retainer groups had comparable occlusal measurements, failure frequency, and survival time, with no significant difference. The patients in the DSC group reported a statistically significant perception of change in the position of their teeth compared to those in the PEEK group. Harms: No harmful effects have been reported. Limitations: Limited follow-up duration and the inability to blind the operator due to the nature of the intervention. Conclusions: After 6-month retention, the PEEK retainer was equally effective to DSC retainers in maintaining the teeth alignment, with no significant differences regarding the failure frequency, survival rate, and general patient satisfaction. Post-treatment stability, survival time and periodontal health associated with vacuum-formed, bonded and rapid prototype retainers: A prospective clinical study

Abstract:Background: New technologies have paved the way for newer fabrication techniques, such as rapid prototyping, which has gained popularity in the fabrication of several orthodontic appliances including retainers. Aim: To evaluate the stability, survival time and periodontal health associated with vacuum-formed retainers (VFRs), bonded retainers (BRs) and rapid prototype retainers (RPRs) over a period of 12 months in retention. Study design: Prospective clinical study. Methods: A total of 72 participants completing fixed orthodontic treatment were allocated to the following three groups by the investigators based upon their initial malocclusion, compliant and preferences: VFR; BR; and RPR. The primary outcome was Little's Irregularity Index (LII). Other stability parameters, including arch length (AL), inter-canine width (ICW) and inter-molar width (IMW), were also assessed immediately after debonding (T0) and 12 months (T2) after debonding. In addition, survival time, and calculus index and gingival index (GI) were assessed. Results: LII showed a significant difference (P = 0.00) between the groups and was increased in the VFR group (0.18 ± 0.22 mm) compared to the BR (0.03 ± 0.05 mm) and RPR (0.01 ± 0.03 mm) groups but was not clinically significant. The mean survival time of maxillary retainers was longest for the RPR group (220.63 days; 95% confidence interval [CI] = 194.1-247.16) but there was no significant difference between the three groups (P = 0.43). The mean survival time of mandibular retainers was higher and the same for the VFR and RPR groups (240 days; 95% CI = 240-240) and there was no significant difference between the three groups (P = 0.38). The calculus index score (P = 0.00) was statistically significant between the groups, with an increased score for the BR group (0.33 ± 0.27) compared to the VFR (0.07 ± 0.16) and RPR (0.13 ± 0.22) groups. Similarly, the GI score (P = 0.02) was statistically significant between the groups and was increased in the BR group (0.01 ± 0.19) compared to the VFR (-0.15 ± 0.18) and RPR (-0.06 ± 0.15) groups. The increase in calculus index and GI scores for the BR group were not clinically significant. Conclusion: There were no clinically significant differences between the three retainer groups in terms of stability, periodontal health and time to failure.
In-house 3D-printed aligners: effect of in vivo ageing on mechanical properties

Abstract:Objective: To investigate alterations in the mechanical properties of in-house three-dimensional (3D) printed orthodontic aligners after intraoral ageing. Materials and methods: Sixteen 3D-printed aligners (TC-85DAC resin, Graphy, Seoul, Korea) were used for the purpose of the study, which were divided into 10 control (not used) aligners and 6 materials retrieved from 4 patients after 1-week service (retrieved group). Samples from the control group were analysed by attenuated total reflectance-Fourier-transform infrared (ATR-FTIR) spectroscopy. Samples from control/retrieved groups were embedded resin and subjected to instrumented indentation testing (IIT) to record force-indentation depth curves, calculating the following (as per ISO 14577-1, 2002 standard): Martens hardness (HM), indentation modulus (EIT), and elastic index (ηIT), and the indentation relaxation index (RIT). Differences between control and retrieved 3D-printed aligners were checked with Mann-Whitney/t-tests at an alpha = 5%. Results: ATR-FTIR analysis showed that aligners were made of a vinyl ester-urethane material. The results of the IIT testing were: HM (control: median 91.5 N/mm2, interquartile range [IQR] 88.0-93.0/as-retrieved: median 90.5 N/mm2, IQR 89.0-93.0); EIT (control, mean 2616.3 MPa, standard deviation [SD] 107.0 MPa/retrieved, mean 2673.2 MPa, SD 149.4 MPa); ηIT (control: median 28.6%, IQR 28.2-30.9%/as-retrieved: median 29.0%, IQR 28.7-29.2%); and RIT (control: median 45.5%, IQR 43.0-47.0%/as-retrieved: median 45.1%, IQR 45.0-45.3%). No differences between as-retrieved and control aligners were found for any of the mechanical properties tested (P > 0.05 in all instances). Conclusion: The mechanical properties of the in-house 3D-printed aligners tested were not affected after 1 week in service period.

Abstract:Abstract Background Retainers have the potential to detrimentally impact periodontal health and contribute to tooth decay. Objectives To investigate periodontal health and bacterial biofilm related to Poly-Ether-Ether-Ketone (PEEK) fixed retainers as compared to Dead-soft coaxial fixed retainer (DSC). Trial design A two-arm parallel groups single-centre randomized clinical trial. Methods The trial included patients whose orthodontic treatment was completed and required retainers. Participants were randomly assigned into two retainer groups: PEEK retainers, prepared by computer-aided design and manufacturing into 0.8 mm wire form, and DSC retainers. The objectives included assessing periodontal health through plaque accumulation index (PI), bleeding on probing (BOP), periodontal pocket depth (PPD), gingival index (GI), calculus index (CI), and alveolar bone height (ABH) assessment. Biofilm assessment involved bacteriological screening of aerobic, facultative anaerobic, mutans streptococci, and lactobacilli. The periodontal indices and microbiological screening as well as were assessed at the debonding stage (T0), 1-month (T1), 3-month (T3), and 6-month (T6) after the commencement of the trial, except for the ABH, which was recorded using periapical radiograph at T0 and T6. Blinding Single blinding of participants in addition to the bacteriological specialist. Results Initially, the trial enrolled 46 participants, aged between 12 and 28 years, and were randomly assigned to two groups, with 23 participants in each group. Subsequently, one participant withdrew from the trial, resulting in a total of 45 participants whose data were analysed. Assessment of the periodontal indices, excluding the CI (P = .480), revealed statistically but not clinically significant differences between groups after 6-month of retention (P = .016 of PI, P = .020 of BOP, P = .05 of PPD, and P = .01 of GI). There was slight plaque accumulation, normal PPD (approximately 1 mm), healthy to mild gingivitis with a GI of less than 1 and BOP was around 10%. Concerning the ABH, there was a noticeable reduction in its score after 6 months, particularly in the PEEK group, although the difference was not statistically significant (P = .102). Furthermore, the bacteriological viable count did not show any significant difference between the groups during the recall visits. Harms There have been no reported negative consequences. Limitations Blinding the assessor of periodontal indices was not feasible due to the nature of the intervention. The trial follow-up duration was limited. Conclusions Both the PEEK and DSC retainers have comparable impacts on periodontal health and bacterial accumulation and composition during the retention period. Trial registration NCT05557136.

Abstract:Background: Most orthodontists believe that fixed retainers are necessary to maintain ideal dental relationships. However, untoward side effects might result from their long-term placement. The aim of this study was to evaluate the clinical and radiographic effect of two commonly used fixed retainers on the health of the periodontium. Methods: Thirty patients were randomly divided into two groups to receive either a fiber-reinforced composite retainer or a spiral wire retainer extended on the lingual surfaces of both maxillary and mandibular arches from canine to canine. Periapical radiographs were obtained from the patients at the time of placement of the retainers and after the 6-month period to assess the radiographic conditions of the periodontium. Clinical examination was carried out at the same two time intervals. Results: Even though there were no significant differences between the two groups of study at the beginning of the trial, there were statistically significant differences after the 6-month follow-up regarding the main outcomes of the study. Nearly all indices showed to deteriorate after 6 months in the fiber-reinforced group, while in the spiral wire group, this was not the case. As for the secondary outcomes, radiographic examination did not reveal any statistically significant differences after 6 months or between the two groups. Conclusions: It can be concluded that spiral wire retainers elicit less detrimental periodontal response in the short-term follow-up compared to fiber-reinforced composite retainers as revealed by the primary outcomes of the study. Surface wear of resin composites used for Invisalign® attachments

Abstract:Orthodontic treatments with Invisalign® require the use of attachments, which are composite resin buttons attached to tooth surfaces. Attachments constitute one of the most powerful tools for improving the efficiency of orthodontic tooth movement. The main purpose of this study was to evaluate surface wear over six months in two resin composites (Filtek Z350 XT, 3MESPE and Amelogen Plus TW, Ultradent Products Inc. ) used for making Invisalign® attachments. These composites were selected for their esthetic and mechanical properties. Forty attachments were bonded to the buccal surface of maxillary teeth. Materials were randomized and assigned in a double blind manner. Impressions were taken of the attachments using a preestablished technique immediately after bonding and following 6 months of clinical use. The impressions were examined by Scanning Electron Microscopy (SEM) at 12-fold magnification to identify changes in surface texture and attachment shape. Data were analyzed using Mann-Whitney's test on the overall ranking of detected changes in those variables. Statistically significant differences (P<0. 001) were found for surface wear, with greater changes in Amelogen Plus. Attachment shape did not change significantly in either material (P>0. 05). According to the results of this study, the alteration of the attachment surface during the first six months of treatment depends on the composite used, while attachment shape does not appear to be affected, so the clinical performance of the materials evaluated could be considered acceptable.
Digital wear analysis and retention of poly-ether-ether-ketone retentive inserts versus conventional nylon inserts in locator retained mandibular overdentures: in-vitro study

Abstract:Objective: this study aimed to digitally compare wear behavior and retention between PEEK and nylon retentive inserts used in locator-retained, mandibular implant overdentures when attachment design and size were standardized. Materials and methods: A total of sixty-four inserts (32 PEEK and 32 nylon inserts); were picked-up in implant overdentures. Overdentures of both groups were submerged in artificial saliva and mounted to chewing simulator. After 480,000 chewing cycles (equivalent to 2 years of clinical use) all inserts were scanned by scanning electron microscope (SEM), then all acquired images were digitally analyzed by software to detect and compare quantitative and qualitative changes of inserts in both groups. On the other hand, retention of both groups was measured by universal testing machine and the collected data was statistically analyzed using one-way Analysis of Variance (ANOVA) test with significance level set at P ≤ 0.05. Results: PEEK inserts showed significantly higher mean retentive values compared to the nylon inserts in the control group. Also, PEEK retentive inserts exhibited statistically lower mean wear values than the control group P ≥ 0.000. Qualitative investigation revealed significant and more pronounced changes in the surface roughness of nylon inserts compared to PEEK ones. Conclusions: Regarding retention, wear behavior and dimension stability, PEEK can be recommended as retentive insert material in cases of locator-retained mandibular implant overdentures. Clinical relevance: PEEK inserts offer enhanced retention, reduced wear, and greater dimensional stability over two years time interval. Clinically, this reduces prosthodontic maintenance and adjustments, improving patient satisfaction and long-term prosthetic success.
In vitro assessment of polyetheretherketone for an attachment component for an implant-retained overdenture

Abstract:Statement of problem: Frequent maintenance because of the limited lifetime of overdenture attachments with O-rings has led to the development of materials that might improve their functionality and longevity. However, testing of newly developed attachment materials is lacking. Purpose: The purpose of this in vitro study was to evaluate a newly developed attachment made of polyetheretherketone (PEEK) for an implant-retained overdenture. Material and methods: Specimens of PEEK, polyacetal, and Teflon O-ring materials were prepared for analysis of roughness, surface hardness, and compressive strength. For the fatigue resistance test, new specimens based on acrylic resin were subjected to 2880 insertion and removal cycles. Compression and roughness data were compared with the Kruskal-Wallis and Dunn post hoc test; hardness data with ANOVA and t test; and fatigue and stereomicroscopy with ANOVA with repeated measures, t test, and Bonferroni adjustment (α=.05). Results: Polyacetal had the lowest surface roughness (P=.038). There was a significant difference in hardness among the materials (P<.05). PEEK presented the highest compressive strength among the materials (P<.001). For the fatigue resistance, only polyacetal showed a difference between the initial time-point (P<.05) and subsequent time-points. Polyacetal had a higher fatigue resistance than Teflon (P<.001) and PEEK (P<.05). Regarding the internal deformation of the attachments, a significant difference was observed among the materials (P<.05). Conclusions: PEEK showed promising results regarding the physicomechanical properties necessary for use as an overdenture attachment when compared with other evaluated materials. In addition, the PEEK attachment showed results comparable to those in the control group (O-rings) in terms of retention.
Mechanical Properties of 3D Printed Orthodontic Retainers

Abstract:Orthodontic retention is the final important stage of orthodontic treatment, the aim of which is to consolidate the functional and aesthetic position of teeth. Among adults, fixed retainers made of different types of wires are the most common. The aim of this study was to analyse the mechanical properties of a new generation of fixed orthodontic retainers-printed by 3D printers. Materials and methods: The study was conducted using samples made of Nextdent MFH C&B N1 resin in the form of cuboid bars with nominal dimensions of width b = 3 mm, thickness d = 0.8 mm; 1 mm; 1.2 mm, length l = 30 mm for each type. The influence of the thickness of the retainers on their strength under loaded conditions was evaluated. Flexural strength, elastic properties, deflection, and creep were compared. The samples were aged in an artificial saliva bath at 37 ± 1 °C during the strength tests. Results: It was shown that differences in the thickness of the samples affected their elastic and strength properties. The highest average flexural modulus, the highest deflection, creep, and strength was characteristic of the samples with the highest thickness (1.2 mm). Samples with an average thickness of 1 mm had the lowest modulus of elasticity. Conclusions: The mechanical properties of 3D printed retainers show that they can be an alternative to metal retainers and the procedure of making new retainers, especially when patients have aesthetic requirements or allergies to metals.
Wear resistance of CAD/CAM one-piece screw-retained hybrid-abutment-crowns made from different restorative materials

Abstract:Introduction: The aim of this study was to measure the wear progress of three high performance polymers (HPP) materials as well as that of zirconia after artificial aging (simulated 2.5- and 5-year of clinical service with thermo-mechanical loading) and compare it with the well-documented wear of lithium disilicate. Methods: Forty implants were used to restore a maxillary first premolar, where the abutment and the crown were manufactured as hybrid-abutment-crown and connected to the implant using a titanium insert. The implants were randomly divided, according to the restorative materials used, into five groups: 3Y-TZP zirconia (Z), lithium disilicate (L), ceramic-reinforced polyetheretherketon (P), nano-hybrid composite resin (C) and polymer-infiltrated ceramic-network (E). All hybrid-abutment-crowns were produced using CAD/CAM technology. A design of a maxillary first premolar was created with an angle of 120° between the buccal and palatal cusps, which were designed as planes. The restorations were adhesively luted onto the titanium inserts, according to the manufacturers' recommendations for each material individually, by means of dual-curing luting resin with the exception of group P, where the blocks were pre-fitted (heat-pressed) with an integrated titanium insert. The suprastructures were assembled onto the implants through titanium screws. The screw channels were sealed with Teflon tape and composite resin filling material that was polished to high-gloss. All specimens underwent 1 200 000 thermo-dynamic loading cycles with 49 N in a dual-axis chewing simulator. Elastomeric impressions were made for all specimens after 600 000 and after 1 200 000 cycles. The corresponding impressions were imaged using a laser scanning microscope and then 3D-analyzed using the software (Geomagic Wrap) to measure the volume loss of the wear area for all specimens. Statistical analysis was performed using Wilcoxon-Test regarding the two different time measurements for each material. For the analysis of the material variable, Kruskal-Wallis test was conducted followed by Mann-Whitney test. Results: Group Z showed statistically the lowest volume loss compared to the other test materials, both after 600 000 and 1 200 000 cycles of artificial aging, with a median value of 0.002 mm3 volume loss after 1 200 000 cycles. In contrast, group E showed the highest volume loss with median values of 0.18 and 0.3 mm3 after 600 000 and 1 200 000 cycles, respectively. Artificial aging had significantly negative effect on the volume loss for all test materials. In addition, the choice of material had statistical influence on the outcome. Conclusion: Monolithic zirconia ceramic demonstrated lower wear than that reported for enamel after simulated 5-year of clinical service, whereas all other test materials showed higher volume loss after artificial aging.

Abstract:OBJECTIVE:To compare the retentions of different designs of cobalt-chromium (Co-Cr), pure titanium (CP Ti), and titanium alloy (Ti-6Al-4V) removable partial denture (RPD) circumferential clasps manufactured by selective laser melting (SLM) and to analyze the stress distribution of these clasps during the removal from abutment teeth. METHODS:Clasps with clasp arm size A (1.9 mm width/1.1 mm thickness at the body and 0.8-taper) or B (1.2 times A) and 0.25 mm or 0.50 mm undercut engagement were modeled on a prepared first premolar die, named as designs A1, A2, A3, and A4, respectively. The density and elastic modulus of SLM-built Co-Cr, CP Ti, and Ti-6Al-4V were measured and given to different groups of clasps. The density, elastic modulus, and Poisson ' s ratio of enamel were given to the die. The control group was the cast Co-Cr clasp with design A1, to which the density and elastic modulus of cast Co-Cr alloy were given. The Poisson's ratio of all metals was 0.33. The initial 5 N dislodging force was applied, and the maximum displacement of the clasp along the insertion path was computed. The load was reapplied with an increment of 5 N than in the last simulation until the clasp was completely dislodged. The retentive force range of different groups of clasps was obtained. The retentive forces of the SLM-built Co-Cr, CP Ti, and Ti-6Al-4V clasps with equivalent computed retentive force range to the control group were validated through the insertion/removal experiment. The von Mises stress distributions of these three groups of SLM-built clasps under 15 N loads were analyzed. RESULTS:SLM-built Co-Cr, CP Ti, and Ti-6Al-4V clasps with designs B1 or B2, and Co-Cr clasps with design A2 had higher retentive forces than those of the control group. SLM-built CP Ti and Ti-6Al-4V clasps with design A1 had lower retentive forces than those of the control group. SLM-built Co-Cr clasp with design A1 and SLM-built CP Ti and Ti-6Al-4V clasps with design A2 had equivalent retentive forces to those of the control group. The insertion/removal experiment showed that the measured retentive forces of these three groups of SLM-built clasps were (21.57±5.41) N, (19.75±4.47) N, and (19.32±2.04) N, respectively. No statistically significant measured retentive force difference was found among these three groups of SLM-built clasps (P>0.05). The maximum von Mises stress of these three groups of SLM-built clasps exceeded their responding yield strength except for the Ti-6Al-4V one. CONCLUSION:SLM-built Co-Cr circumferential clasps had higher retention than CP Ti and Ti-6Al-4V ones with the same clasp arm size and undercut engagement. The retention of SLM-built circumferential clasps could be adjusted by changing the undercut engagement and clasp arm size. If SLM-built circumferential clasps are used in clinical practice, the Ti-6Al-4V clasp with clasp arm size A and 0.50 mm undercut engagement is recommended considering the long-term use of RPD in the patient's mouth.
Evaluation of wear resistance and surface properties of additively manufactured restorative dental materials

Abstract:Objectives To evaluate the wear resistance of three additively manufactured dental crown materials (NextDent C&B MFH, Saremco print CROWNTEC and Bego VarseoSmile Crown) under two environmental conditions (dry and artificial saliva), two loads (49 N and 70 N) and two surface treatments (polished and glazed). Methods A total of 120 specimens were divided into 24 groups and tested for wear under two loads (49 N and 70 N), surface treatment (polished or glazed), and environment (dry or submerged in artificial saliva). All samples underwent reciprocating wear testing at 1 Hz using a wear simulator, replicating 48 months of In Vivo conditions with a stainless-steel ball as the antagonist. The coefficient of friction (CoF), surface roughness, volumetric and vertical wear loss were measured and statistically analysed. Confocal microscopy assessed the surface properties of crown materials and the antagonists. Results The NextDent material demonstrated the most homogenous wear, with relatively low vertical and volumetric loss across all groups ( p < 0.004). NextDent and Bego materials performed similarly in artificial saliva regardless of the load type ( p > 1.000). The CoF remained below 0.3 for all groups. All groups exhibited significant increases in surface roughness after testing, however, this did not correlate with an increase in the CoF. Confocal analysis revealed material deformities due to load and notable scratch marks on the stainless-steel antagonists. Conclusion It was found that all investigated addtively manufactured materials can be suggested for provisional use. Both vertical loss and volumetric loss results should be included for material evaluation. CoF and surface roughness should be implemented into wear evaluation. Clinical significance This study highlights the practical value of additively manufactured dental crown materials, particularly for provisional restorations. However, their extended use requires careful consideration of individual patient needs, emphasising the need for judicious clinical application evaluation.
