Abstract:Statement of problem: The loss in the retentive capability of implant-supported overdenture attachments has been related to the wear of the retentive inserts. Wear of the abutment coating material when following the replacement period for the retentive inserts requires investigation. Purpose: The purpose of this in vitro study was to compare the changes in retentive force of 3 polyamide and a polyetheretherketone denture attachments under repeated insertion and removal cycles in wet conditions while following their manufacturers' replacement time recommendations. Material and methods: Four different denture attachments (LOCKiT, OT-Equator, Ball attachment, and Novaloc) with their retentive inserts were tested. Four implants were embedded into individual acrylic resin blocks, and 10 abutments for each attachment were used. Forty metal housings with their retentive inserts were attached to polyamide screws with autopolymerizing acrylic resin. A customized universal testing machine was used to simulate insertion and removal cycles. The specimens were mounted on a second universal testing machine at 0, 540, 2700, and 5400 cycles, and the maximum retentive force was recorded. The retentive inserts for LOCKiT (light retention), OT-Equator (soft retention), and Ball attachment (soft retention) were replaced every 540 cycles, while the Novaloc (medium retention) attachments were never replaced. All the abutments were weighed with a precision scale at 0, 2700, and 5400 cycles. The surface of every abutment was examined under a stereomicroscope at ×10 magnification. Data were analyzed with descriptive statistics. Two-way repeated measures ANOVA was used to compare the mean retentive force and mean abutment mass of all groups and time evaluation points. Bonferroni corrections were made to adjust for multiple tests (α=.05). Results: The mean retention loss for LOCKiT was 12.6% after 6 months and 45.0% after 5 years of simulated use. The mean retention loss for OT-Equator was 16.0% after 6 months and 50.1% after 5 years of simulated use. The mean retention loss for Ball attachment was 15.3% after 6 months and 39.1% after 5 years of simulated use. The mean retention loss for Novaloc was 31.0% after 6 months and 59.1% after 5 years of simulated use. The mean abutment mass difference was statistically significant (P<.05) for LOCKiT and Ball attachment but not statistically significant (P>.05) for OT-Equator and Novaloc at baseline, 2.5 years, and 5 years. Conclusions: All tested attachments recorded retention loss under the experimental conditions, even when their manufacturers' recommendations for replacement time for the retentive inserts were followed. Patients should be aware that implant abutments should be replaced after a recommended period since their surfaces also change over time.

Abstract:The purpose of this research is to compare the cytotoxicity of polyetheretherketone (PEEK) and polyetherketoneketone (PEKK) with conventional dental implant–abutment materials, namely titanium alloy (Ti-6Al-4V) and yttria-stabilized tetragonal zirconia polycrystal (Y-TZP), to evaluate the cell metabolic activity, cytotoxicity, and inflammation potential of human oral fibroblasts (HOF) on these materials. Disk-shaped specimens were designed and prepared via a dental computer-aided manufacturing technology system. Surface topography, roughness, and free energy were investigated by atomic force microscope and contact angle analyzer; cell metabolic activity and cytotoxicity by MTT assay; and morphological changes by scanning electron microscopy (SEM). The effect of pro-inflammatory gene expression was evaluated by RT-qPCR. The obtained data were analyzed with one-way analysis of variance and post-hoc Tukey’s honest significant difference tests. PEEK and PEKK exhibited higher submicron surface roughness (0.04 μm) and hydrophobicity (>80°) than the control. Although the cell activity of PEEK was lower than that of Ti-6Al-4V and Y-TZP for the first 24 h (p 0.05). According to the cell cytotoxicity and the pro-inflammatory cytokine gene expression assays, there was no difference between the materials (p > 0.05). SEM observations indicated that HOF adhered poorly to PEKK but properly to Ti-6Al-4V, Y-TZP, and PEEK. PEEK and PEKK show comparable epithelial biological responses to Ti-6Al-4V and Y-TZP as implant–abutment materials. Between the two polymeric materials, the PEEK surface, where the HOF showed better cell metabolic activity and cytotoxicity, was a more promising implant–abutment material.

Retention and fatigue performance of modified polyetheretherketone clasps for removable prosthesis

Abstract:Purpose: Polyetheretherketone (PEEK) is considered as an alternative to metal material for removable partial denture (RPD). However, the retentive force is not strong as a metal RPD. This study investigated the retention and fatigue performance of PEEK clasps with different proportions of clasp arm engaging the undercut to verify a new strategy to improve their clinical performance. Methods: Three groups (n = 10/group) of PEEK clasps with their terminal 1/3, 2/3 and the whole of retentive arms engaging the undercut were fabricated along with a group (n = 10) of conventional cobalt-chrome (CoCr) clasps as control group. Retentive forces were measured by universal testing machine initially and at an interval of 1500 cycles for a total of 15,000 fatigue cycles. The fatigue cycles were conducted by repeated insertion and removal of the clasp using fatigue testing machine. Each clasp was scanned by Trios3 scanner before and after fatigue test to obtain digital models. The deformation of the clasp was evaluated by root mean square (RMS) through aligning the two models in Geomagic wrap (2021). Scanning electron microscopy (SEM) and finite element analysis were carried out to observe the abrasion and the von Mises stress of the clasp arm. Kruskal-Wallis H test was used to compare the retentive forces and the RMSs of the studied groups followed by Bonferroni multiple comparisons. Results: The whole of PEEK clasp arm engaging the undercut provided higher mean retentive forces (7.99 ± 2.02 N) than other PEEK clasp groups (P < 0.001) and was closer to CoCr clasps (11.88 ± 2.05 N). The RMSs of PEEK clasps were lower than CoCr clasps (P < 0.05) while the differences among PEEK clasps were of no statistical significance (P > 0.05). SEM showed that evidences of surface abrasion were observed on the section that engaged the undercut for all groups of clasps. The stress concentration mainly occurred on the initial part of the retentive arm. The maximum von Mises stress of each group was below the compressive strength of PEEK. Conclusions: Proportions of PEEK clasp arm engaging the undercut positively influenced the retentive force and the fatigue resistance of PEEK clasps was superior than CoCr clasps. It is a feasible method to improve the retention of PEEK clasps by increasing the proportion of clasp arm engaging the undercut. Clinical trials are needed to further verify this innovation.

Abstract:Objective: this study aimed to digitally compare wear behavior and retention between PEEK and nylon retentive inserts used in locator-retained, mandibular implant overdentures when attachment design and size were standardized. Materials and methods: A total of sixty-four inserts (32 PEEK and 32 nylon inserts); were picked-up in implant overdentures. Overdentures of both groups were submerged in artificial saliva and mounted to chewing simulator. After 480,000 chewing cycles (equivalent to 2 years of clinical use) all inserts were scanned by scanning electron microscope (SEM), then all acquired images were digitally analyzed by software to detect and compare quantitative and qualitative changes of inserts in both groups. On the other hand, retention of both groups was measured by universal testing machine and the collected data was statistically analyzed using one-way Analysis of Variance (ANOVA) test with significance level set at P ≤ 0.05. Results: PEEK inserts showed significantly higher mean retentive values compared to the nylon inserts in the control group. Also, PEEK retentive inserts exhibited statistically lower mean wear values than the control group P ≥ 0.000. Qualitative investigation revealed significant and more pronounced changes in the surface roughness of nylon inserts compared to PEEK ones. Conclusions: Regarding retention, wear behavior and dimension stability, PEEK can be recommended as retentive insert material in cases of locator-retained mandibular implant overdentures. Clinical relevance: PEEK inserts offer enhanced retention, reduced wear, and greater dimensional stability over two years time interval. Clinically, this reduces prosthodontic maintenance and adjustments, improving patient satisfaction and long-term prosthetic success.

Abstract:Abstract Objective Polymeric framework represent an innovative approach for implant-supported dental prostheses. However, the mechanical response of ultra-high performance polymers as frameworks for full-arch prostheses under the “all-on-four concept” remains unclear. The present study applied finite element analysis to examine the behavior of polyetherketoneketone (PEKK) and polyetheretherketone (PEEK) prosthetic frameworks. Materials and Methods A three-dimensional maxillary model received four axially positioned morse-taper implants, over which a polymeric bar was simulated. The full-arch prosthesis was created from a previously reported database model, and the imported geometries were divided into a mesh composed of nodes and tetrahedral elements in the analysis software. The materials were assumed as isotropic, elastic, and homogeneous, and all contacts were considered bonded. A normal load (500 N magnitude) was applied at the occlusal surface of the first left molar after the model was fixed at the base of the cortical bone. The microstrain and von-Mises stress were selected as criteria for analysis. Results Similarities in the mechanical response were observed in both framework for the peri-implant tissue, as well as for stress generated in the implants (263–264 MPa) and abutments (274–273 MPa). The prosthetic screw and prosthetic base concentrated more stress with PEEK (211 and 58 MPa, respectively) than with PEKK (192 and 49 MPa), while the prosthetic framework showed the opposite behavior (59 MPa for PEEK and 67 MPa for PEKK). Conclusion The main differences related to the mechanical behavior of PEKK and PEEK frameworks for full-arch prostheses under the “all-on-four concept” were reflected in the prosthetic screw and the acrylic base. The superior shock absorbance of PEKK resulted in a lower stress concentration on the prosthetic screw and prosthetic base. This would clinically represent a lower fracture risk on the acrylic base and screw loosening. Conversely, lower stress concentration was observed on PEEK frameworks.
Flexural strength, surface roughness, and biofilm formation of ceramic-reinforced PEEK: An in vitro comparative study

Abstract:Purpose: This in vitro study aimed to compare flexural strength, surface roughness, and biofilm formation of ceramic-reinforced polyetheretherketone (PEEK) with conventionally heat-compressed and milled polymethylmethacrylate (PMMA) denture base materials. Materials and methods: Thirty strips (6.4 mm × 10 mm × 3 mm) and 30 discs (10 mm × 1 mm) were fabricated from a heat-compressed PMMA, milled PMMA, and ceramic-reinforced PEEK, 10 each. One surface of each sample was polished to mimic the laboratory procedure for denture base materials. Strips were then subjected to a three-point bend test using a universal testing machine at a crosshead speed of 5.0 mm/min. An optical profilometer was used to assess the Ra value (mm) of the discs on polished and unpolished sides. Biofilm formation behavior was analyzed by measuring the colony-forming unit (CFU)/mL of Candida albicans on the unpolished surface of the discs. One-way ANOVA followed by Tukey multiple comparison tests were used to compare the flexural strength, Ra value, and biofilm formation of the studied materials (a = 0.05). Results: Ceramic-reinforced PEEK showed significantly higher flexural strength (178.2 ± 3.2 MPa) than milled PMMA (89.6 ± 0.8 MPa; p < 0.001) and heat-compressed PMMA (67.3 ± 5.3 MPa; p < 0.001). Ceramic-reinforced PEEK exhibited a significantly higher Ra value than the other groups on unpolished sides; however, the polishing process significantly reduced the Ra values of all studied groups (p < 0.05). There was no significant difference in C. albicans adhesion among the groups (p < 0.05). Conclusion: The flexural strength of tested materials was within acceptable limits for clinical use as a denture base material. Ceramic-reinforced PEEK had the highest surface roughness; however, its similarity in biofilm formation to other groups indicates its clinical acceptability as denture base material.

Abstract:Background: Retention and stabilization of implant-supported overdenture is associated with more patient satisfaction, chewing capability, and oral health-related quality of life. There are a variety of implant retention systems which can be utilized to retain an implant overdenture. Telescopic attachments provide excellent retention resulting from friction. Objective: This study evaluates the retention force and fatigue resistance of ready-made polyether ether ketone (PEEK) telescopic attachment in different numbers and positions of dental implants in palateless maxillary overdenture. Method: Five edentulous maxillary models were fabricated from cold-cure poly methyl methacrylate resin. The study models were divided into two groups; the first group (3-implant supported overdenture) and the second group (4-implant supported overdenture). Each group is subdivided into subgroups according to the different implant positions. One palateless maxillary denture was constructed over each model. Ready-made PEEK telescope attachments correspond to implant abutments embedded in the inner surface of these overdentures. Initial anterior, posterior, and central retention values of overdentures were estimated and compared with the retention after 540 cycles of insertion and removal using a digital force gauge. One-way ANOVA testing was used to compare the means of the models of group one, an Independent sample t-test was used to compare the means of the models of group two, and a comparison of retention value between the groups and Paired sample t-test was applied for determining the difference between initial and secondary retention value (p<0.05). Results: The anterior retention value was significantly decreased when the dental implants were placed distally while the posterior and central retention values were significantly increased. Also, retention value increased as implant number increased; the lowest mean retention values were recorded with Group one and the highest values were with Group two. The final retention value was significantly decreased after 540 times insertion and removal. However, the values of secondary retention are still larger than the minimum retention value that was reported (5–7 N) to achieve sufficient patient satisfaction. Conclusion: The implant location and number affect the retention value of palateless maxillary overdenture. Continuous insertion and removal of the prosthesis for daily hygiene practice lead to decreases in retention values.

Abstract:CONCLUSIONS: The PEKK clasps did not exhibit significantly reduced retentive forces or permanent deformations after 15,000 fatigue cycles. These results suggest that the PEKK polymer displays superior mechanical properties as an esthetic clasp material, and clasps with 0.50-mm and 0.75-mm undercut depths are recommended for long-term clinical use.

Abstract:Abstract The aim of this study was to evaluate the interaction between Streptococcus oralis and Polyetheretherketone (PEEK), a novel material recently introduced in implantology. The topographical characterization and the Streptococcus oralis adhesion on this material were compared with other titanium surfaces, currently used for the production of dental implants: machined and double etched (DAE). The superficial micro-roughness of the PEEK discs was analyzed by scanning electron microscopy (SEM) and, the Energy Dispersive Spectrometer (EDS) analyzed their chemical composition. Atomic Force Microscopy (AFM) was used to characterize the micro-topography and the sessile method to evaluate the wettability of the samples. Microbiological analysis measured the colony forming units (CFUs), the biomass (OD 570 detection) and the cell viability after 24 and 48 h after Streptococcus oralis cultivation on the different discs, that were previously incubated with saliva. Results showed that PEEK was characterized by a micro-roughness that was similar to machined titanium but at nano-level the nano-roughness was significantly higher in respect to the other samples. The EDS showed that PEEK superficial composition was characterized mainly by Carbonium and Oxygen. The hydrophilicity and wetting properties of PEEK were similar to machined titanium; on the contrary, double etched discs (DAE) samples were characterized by significantly higher levels ( p < 0.05). PEEK was characterized by significant lower CFUs, biomass and viable cells in respect to the titanium surfaces. No differences were found between machined and DAE. The anti-adhesive and antibacterial properties showed by PEEK at 24 and 48 h against a pioneer such as S. oralis , could have an important role in the prevention of all pathologies connected with biofilm formation, like peri-implantitis in dentistry or prosthetic failures in orthopedics.

Abstract:A new application of a polyetheretherketone (PEEK) post-core restoration combined with polyvinylsiloxane (PVS) attachments was proposed and substantiated. This study aimed to evaluate retentive characteristics of the PEEK post-core restoration with PVS attachment systems. Specimens with PVS attachments were fabricated to evaluate retention force during 10,000 cyclic dislodgements. Additionally, the retention forces of PVS attachments with three different values of Shore hardness were further measured before and after aging treatments. The results of cyclic dislodgement test indicated a strong negative linear relationship between the cyclic times and retention force (p < 0.0001, r = −0.957). Furthermore, the retention forces of the PVS were significantly improved with Shore hardness increased, which was also affected by the aging treatment (F (2, 138) = 10.95, p < 0.0001). Therefore, the PEEK post-core restoration with PVS attachments exhibited the favorable retention force, which could be a promising alternative for dental prostheses.

Abstract:Poly(aryl–ether–ketone) materials (PAEKs) are gaining interest in everyday dental practices because of their natural properties. This study aims to analyze the bonding performance of PAEKs to a denture acrylic. Testing materials were pretreated by grinding, sandblasting, and priming prior to polymerization with the denture acrylic. The surface morphologies were observed using a scanning electron microscope and the surface roughness was measured using atomic force microscopy. The shear bond strength (SBS) values were determined after 0 and 2500 thermal cycles. The obtained data were analyzed using a paired samples t-test and Tukey’s honestly significant difference (HSD) test (α = 0.05). The surface characteristics of testing materials after different surface pretreatments showed obvious differences. PAEKs showed lower surface roughness values (0.02–0.03 MPa) than Co-Cr (0.16 MPa) and zirconia (0.22 MPa) after priming and sandblasting treatments (p < 0.05). The SBS values of PAEKs (7.60–8.38 MPa) met the clinical requirements suggested by ISO 10477 (5 MPa). Moreover, PAEKs showed significantly lower SBS reductions (p < 0.05) after thermal cycling fatigue testing compared to Co-Cr and zirconia. Bonding performance is essential for denture materials, and our results demonstrated that PAEKs possess good resistance to thermal cycling fatigue, which is an advantage in clinical applications. The results imply that PAEKs are potential alternative materials for the removable of prosthetic frameworks.

Abstract:Background: Debonding of ball attachments is one of the complications that annoy teeth supported overdenture wearers. The polyetheretherketone (PEEK) and polyetherketoneketone (PEKK) polymers are widely applied in the dental field. The purpose of the current study was to compare the tensile bond strength of ball attachments made of such materials and the commonly used titanium ones after 5 years of overdenture insertion and removal (5000 cycles) in addition to chewing simulation (1,200,000 cycle). Methods: Extracted mandibular canines (N = 60) were randomly allocated into three groups and received ball attachments; titanium (group TI; N = 20), PEEK (group PE; N = 20), PEKK (group PK; N = 20). In each group, the samples were divided into two subgroups whereas tensile bond strength was measured pre aging (T0; n = 10) and post aging (T1; n = 10). Tensile bond strength was measured by the Pull out test using the Universal testing machine. Failure mode analysis was determined by examination of the samples' surfaces under 65X stereomicroscope. The resulting data followed normal distribution and the significance level was set at (α = 0.05). Results: One Way Anova showed statistically significant difference between the three groups (P < .00001). PostHoc Tukey test showed statistically significant difference between the groups TI and PE, TI and PK and no statistically significant difference between the groups PE and PK. Paired t test showed statistically significant difference in the tensile bond strength pre and post aging in each group. Conclusions: PEEK and PEKK ball attachments could be concluded to have a higher tensile bond strength compared to the titanium ones when bonded to root dentin. Tensile bond strength of such attachments may decrease with aging as well. Clinically, the higher tensile bond strength may have a lesser rate of debonding and thus reduced patient apprehension.

Abstract:BACKGROUND: The development of new and innovative materials such as high performance polymers (PEEK) opens a wide therapeutic range in implant prostheses. They are presented as alternative materials to metal and zirconium alloys in the manufacture of structures and attachments for implant prostheses and fixed and removable dental substitutes. The objective of this review is to know the characteristics of this material and thus assess its advantages and disadvantages in its possible applications in prostheses on dental implants.MATERIAL AND METHODS: A bibliographic search was carried out through the PubMed and Scopus search engines, of articles published from 2007 to 2020, excluding all articles in which PEEK was used for tooth prostheses. The data on the sufficiency of the PEEK material were organized according to its chemical, physical and mechanical properties.RESULTS: 148 articles were found in the databases using as keywords; Polyetheretherketone; PEEK; BioHPP; healing abutments; dental prostheses; dental prosthodontics; Full-arch rehabilitation; fixed implant prosthodontics; implant-retained prostheses; implant prostheses.CONCLUSIONS: It is concluded that PEEK offers greater lightness, good aesthetics, biocompatibility, and an elastic modulus more similar to bone than other materials commonly used in implant prostheses; however, it presents a higher risk of fracture and abrasion. Abstract:Background: Preservation of the remaining structures while maintaining an esthetic appearance is a major objective in removable partial prosthodontics. So, the aim of the current study was to compare the stresses induced on the supporting structures by two digitally produced esthetic core materials; Zirconia and Polyetheretherketone when used as an extracoronal attachment in distal extension removable partial dentures using strain gauge analysis. Methods: A mandibular Kennedy class II stone cast with the necessary abutments' preparations was scanned. The mandibular left canine and first premolar teeth were virtually removed. An acrylic mandibular left canine and first premolar teeth were prepared with heavy chamfer finish line and scanned. Virtual superimposition of the acrylic teeth in their corresponding positions was done. Two strain gauge slots were designed: distal to the terminal abutment and in the residual ridge. Two models and two sets of scanned teeth were digitally printed. The printed teeth were then placed in their corresponding sockets in each model and scanned. The attachment design was selected from the software library and milled out of Zirconia in the model ZR and Polyetheretherketone in the model PE. Five removable partial dentures were constructed for each model. The strain gauges were installed in their grooves. A Universal testing machine was used for unilateral load application of 100 N (N). For each removable partial denture, five measurements were made. The data followed normal distribution and were statistically analyzed by using unpaired t test. P value < 0.05 was considered to be statistically significant. Results: During unilateral loading unpaired t test showed statistically significant difference (p = 0.0001) in the microstrain values recorded distal to the abutment between the models ZR (-1001.6 µε ± 24.56) and PE (-682.6 µε ± 22.18). However, non statistically significant difference (p = 0.3122) was observed in the residual ridge between them; ZR (16.2 µε ± 4.53) and PE (15 µε ± 3.74). Conclusions: In removable partial dentures, Polyetheretherketone extracoronal attachment induces less stress on the supporting abutments compared to the zirconia one with no difference in the stresses induced by them on the residual ridge.

Abstract:Purpose: This study was designed to evaluate the influence of utilizing Polyetheretherketone (PEEK) coping and three different dental restorative materials in the success of tooth and implant-supported fixed partial dentures (TISFPD) in the maxillary posterior region, under static loading by 3-Dimensional Finite Element Analysis (3D FEA). Materials and methods: Six 3D FEA models were designed assuming the extraction of maxillary first and second molars. Bone, implant, abutment, PEEK coping, second premolar, periodontal ligament (PDL) and six 3-unit TISFPD with different restorative materials [Porcelain-Fused-to-Metal (PFM), PEEK-Composite (PC), Monolithic Zirconia (MZ)] were modeled. Then, PEEK copings were modeled to be cemented onto the implants as a double-crown system for the first three groups (PFMPEEK, PCPEEK and MZPEEK) whereas, the next three groups (PFM, PC, MZ) excluded a PEEK coping in their designs. The prostheses were loaded twice, vertically and obliquely. From the determined points, 250 N for vertical loading (0o to the long axis) and 200 N for the oblique loading (30o to the long axis) were applied. Von Mises tension, maximum and minimum principal tension value criterias were analyzed. Results: Regardless of the material used for suprastructure, the maximum average stress was reduced by using PEEK coping. Considering the maximum stress distribution, PC appeared to have the highest stresses on the cortical bone, implant and screw. Additionally, the von Mises stresses formed in the PDL for each model were lower when PEEK coping was included in the design of the TISFPD, reducing the risk of intrusion. Conclusions: The stress distribution was positively affected by the PEEK coping in TISFPD design, reducing bone resorption and failure. This elastic material used generated lower stresses in the bone and implant, while no significant effect was found on stresses around natural teeth.
Retentive characteristics of individual and prefabricated polyvinylsiloxane overdenture attachments: alternative treatment options for geriatric patients

Abstract:Objectives: Stud attachments are often too adhesive and too susceptible to damage for use in geriatric patients, especially when implants are angulated. This study aimed to evaluate alternative anchoring systems comprising individual and prefabricated polyvinylsiloxane (PVS) attachments. Materials and methods: A total of 182 specimens with individual PVS (IPVS) attachments (Shore hardness [SH] 25, SH50, SH65], prefabricated PVS (PPVS) attachments (SM green, yellow, and red), and Locator attachments (LR blue) were fabricated (n = 7 per group). Retention force was measured using the following parameters: insertion/removal (100, 200, 500, 1000, and 5000 cycles), thermal undulation (10,000 cycles at 5-55 °C; one implant per specimen), implant angulation (0°, 5°, and 10° convergence and divergence; two implants per specimen), and artificial saliva. Results: Insertion/removal and thermal undulation caused no changes in retention force in SM green and IPVS subgroups; conversely, LR blue, SM red, and SM yellow attachments exhibited significant decreases in retention force of up to 66% (all P ≤ 0.001). Implant angulation produced relevant changes in retention force only in LR blue attachments. Artificial saliva caused a general decrease in retention force. Conclusions: Retention force of low-retentive PVS attachments proved to be comparatively immune to dislocation and thermal undulation, as well as to implant angulation up to 10°. Clinical relevance: Low-retentive PVS attachments could be a treatment option if reduced denture retention is required and/or if angulated implants are in place. Clinical studies are necessary to evaluate the materials' durability under oral conditions.

Abstract:Statement of problem Computer aided technologies have been used to fabricate occlusal devices. However, the clinical behavior of the newly developed materials developed for occlusal devices is unknown. Purpose The purpose of this prospective, double-blind study was to assess the clinical efficacy of recently introduced computer-aided design and computer-aided manufacturing (CAD-CAM) materials for the fabrication of occlusal devices. Material and methods A total of 24 participants were divided randomly into 2 study groups; polyetheretherketone (PEEK) and polymethyl methacrylate (PMMA), and a control group (CG). Conventional impressions and gypsum casts were obtained from all participants. In the study groups, the casts were digitalized with an extraoral digital scanner, designed with a software program (Bite Occlusal Device Module; exocad GmbH) and milled from PEEK and PMMA blocks. Clear resin sheets were used for occlusal device fabrication in the CG. The baseline measurements were made during the initial appointments. After 6 months, the participants returned for follow-up evaluations. Clinical performance based on surface roughness, wear of the antagonist teeth, occlusal device fit and therapeutic effect, as well as participant satisfaction were compared using the 1-way ANOVA test between the main groups (α=.05). The post hoc and Kruskal Wallis-H tests were used to compare the nonparametric group. Results The therapeutic effects of the occlusal devices did not differ. All participants showed improvement in palpation and mandibular movement scores, but no statistically significant differences were found among the groups ( P >.05). PEEK and PMMA had statistically less surface wear than CG ( P <.001) and led to less antagonist tooth wear. No significant participant satisfaction difference was seen among the groups ( P >.05). The control group had the best fit ( P <.001). Conclusions Recent CAD-CAM materials exhibit clinically acceptable outcomes, and their performance is comparable with that of traditional materials. CAD-CAM materials appear suitable in terms of accuracy, surface wear, and therapeutic efficacy.
