Abstract:Therapeutic endoscopic ultrasound has become widespread as an effective procedure for biliary drainage; however, it is rarely used to remove foreign bodies such as a biliary stent. A 57-year-old man was referred to our hospital for a benign biliary stricture in the left hepatic duct after hepatectomy. Initially, a 7-Fr plastic stent was placed in the left hepatic duct with the distal end set above the papilla, and it was replaced with an 8.5-Fr stent as the stricture remained after 3 months. Endoscopic retrograde cholangiopancreatography was performed to retrieve the plastic stent 3 months later; however, the stent could not be moved because the proximal flap was caught in the stricture. Attempts using various devices failed to retrieve the stent; thus, endoscopic ultrasound-guided hepaticogastrostomy was performed to create a route for stent retrieval. Eventually, the plastic stent was successfully retrieved with biopsy forceps through a fully covered self-expandable metallic stent located in a transgastric fistula. We propose our new method involving endoscopic ultrasound-guided hepaticogastrostomy for endoscopic stent retrieval that fails via the transpapillary route.

Salvage technique for covered metal stent migration during endoscopic reintervention after endoscopic ultrasound-guided hepaticogastrostomy

Abstract:Endoscopic ultrasound (EUS)-guided hepaticogastrostomy (EUS-HGS) is an alternative drainage method for malignant biliary obstruction (MBO) when endoscopic retrograde cholangiopancreatography (ERCP) has failed [1]. A partially covered self-expandable metal stent (PCSEMS) is often used for EUS-HGS, but it cannot be removed. A new metal or plastic stent is therefore placed as endoscopic reintervention following EUS-HGS [2] [3] [4]; however, troubleshooting after endoscopic reintervention remains problematic because of a paucity of reported cases. An 81-year-old woman who had previously undergone ERCP for MBO due to pancreatic cancer presented with recurrent biliary obstruction and duodenal stricture. An EUS-HGS using a PCSEMS (EGIS biliary stent, double-covered, 8 mm × 12 cm; S&G Biotech Inc., Yongin, South Korea) and duodenal stent placement were successfully performed. After 5 months, the patient underwent endoscopic reintervention for recurrent biliary obstruction. An additional fully covered SEMS (FCSEMS; HANAROSTENT benefit, 8 mm × 8 cm; Boston Scientific Co., Tokyo, Japan) was deployed through the stent mesh of the EUS-HGS PCSEMS because of the difficulty removing the PCSEMS, along with placement of an antegrade stent across the MBO ([Fig. 1]). A second endoscopic reintervention was required for SEMS occlusion, during which new plastic stents (Through & Pass Type-IT; Gadelius Medical, Tokyo, Japan) were placed through the distal end of the EUS-HGS SEMS after the stent mesh had been broken using argon plasma coagulation ([Fig. 2]). After 2 months, the patient developed acute cholangitis due to migration of the EUS-HGS SEMS placed during the first endoscopic reintervention and cholecystitis due to the antegrade SEMS. After the plastic stents had been removed, grasping forceps (Rat Tooth; Olympus, Tokyo, Japan) were inserted via the EUS-HGS SEMS. The migrated SEMS was grabbed ([Fig. 3] a ) and removed. Additionally, the antegrade SEMS was firmly grasped and gradually removed via the EUS-HGS route ([Fig. 3] b ; [Video 1]). The successful removal of the two SEMSs was followed by the insertion of new plastic stents and the patient's cholangitis and cholecystitis subsequently improved. This technique demonstrates successful troubleshooting of endoscopic reintervention after EUS-HGS. Abstract:Video 1A novel method of bilateral biliary decompression by EUS-guided hepaticogastrostomy with bridging stenting using the partial stent-in-stent method for reintervention of multiple metal stent failure.We report a case in which anterior and posterior drainage was performed using the partial stent-in-stent method via the transpapillary approach. The patient had a bismuth type IV biliary obstruction, but only the right hepatic lobe was drained due to obstruction of the left portal vein. For the recurrent stent dysfunction, the patient underwent placement of a plastic stent within an uncovered self-expanding metal stent to correct stent dysfunction. A 7F plastic stent inside a metal stent is shown.The patient later experienced stent failure and jaundice due to tumor progression and was admitted for plastic stent replacement. Neither imaging results nor symptoms suggested duodenal stenosis. The transpapillary approach was attempted first but was unsuccessful. Duodenoscopy was challenging to perform because of duodenal stenosis. Fluoroscopy confirmed the duodenal stenosis. The plastic stent was extracted using an upper endoscope. Multiple uncovered metal stents are shown (1 stent in the anterior bile duct and 2 stents in the posterior bile duct). Jaundice did not resolve despite plastic stent removal.The patient refused to undergo percutaneous biliary drainage, so a decision was made to perform an EUS-guided hepaticogastrostomy (HGS) instead. The left bile duct was observed in the stomach. The left bile duct was punctured with a 19-gauge FNA needle. A 0.025-inch hydrophilic guidewire was directed into the left bile duct. Enhancement of the bile duct showing malignant hilar biliary obstruction (bismuth IV) is seen. Insertion of the guide wire into the posterior bile duct is shown.The stent mesh was then dilated using a balloon dilator. However, there was difficulty inserting the catheter. Additional dilation was performed using a spiral dilator. This instrument is a tapered tip dilator that fits into 0.025-inch guidewires and is expandable to 7F. Insertion of a second guidewire with a larger caliber was done to straighten the bile duct and help stabilize stent insertion. A 0.035-inch hydrophilic guidewire into the posterior bile duct using a double-lumen cannula and insertion of a 0.025-inch hydrophilic guidewire into the anterior bile duct are shown.The stent mesh was then dilated using a spiral dilator. A metal stent was placed through the anterior bile duct at a steep angle. Insertion and deployment of the first uncovered self-expanding metal stent (8 × 60 mm) from the anterior bile duct into the left bile duct is shown. Multiple metal stents were implanted into the hilar area, and the new stent was placed using the partial stent-in-stent method to prevent overexpansion. Guidewire seeking the posterior bile duct from inside the deployed stent through the stent mesh is shown.The stent mesh was then dilated using a balloon dilator. Insertion and deployment of an uncovered self-expanding metal stent (8 × 60 mm) from the posterior bile duct to the left bile duct using the partial stent-in-stent method is shown. Enhancement of the bile duct shows drainage from the right bile duct. The fistula of the HGS was only dilated with the spiral dilator. The risk of bile leakage was low, so we decided to implant a plastic stent. A 7F × 15-cm plastic stent was placed from the posterior bile duct into the stomach. Anterior and posterior segment drainage by EUS-HGS with bridging stenting using the partial stent-in-stent method is shown, with left segment drainage by EUS-HGS with the plastic stent.We performed EUS-HGS on a patient with multiple metal stents in place. There were no adverse events, and total bilirubin levels were reduced by more than half within 2 weeks. Six months have passed without stent dysfunction.
Fully-Covered Self-Expandable Metal Stent for Hepaticojejunostomy Anastomotic Stricture After Living Donor Liver Transplantation: A Case Series

Abstract:Introduction: We prospectively evaluated 3 cases regarding the usefulness of fully-covered self-expandable metal stents (FCSEMSs) for hepaticojejunostomy anastomotic stricture (HAS) after living donor liver transplantation (LDLT), which could not be resolved with conventional treatment using a plastic stent. Case report: All patients underwent LDLT with Roux-en-Y reconstruction; therefore, a short-type double-balloon enteroscope was used for the endoscopic procedures. HAS was observed on enteroscopic view of endoscopy in patients 1 and 2, and cholangiography revealed dilatation of the intrahepatic bile duct. The FCSEMS was successfully placed without the report of adverse events. The FCSEMS was removed after 16 weeks, and the HAS improved in both patients. In addition, stone clearance was also achieved in patient 2. On the other hand, FCSEMS was not placed in patient 3 because there was no indication of FCSEMS placement due to the multiple segmental biliary strictures (pruned-tree appearance on cholangiography). Subsequent deceased-donor liver transplantation confirmed recurrent primary sclerosing cholangitis. In this case, magnetic resonance cholangiopancreatography (MRCP) was not performed prior to cholangiography to rule out PSC recurrence. Conclusion: FCSEMS placement may be effective and safe for HAS after LDLT, which is not resolved with conventional treatment using a plastic stent. MRCP should be used to identify HAS prior to invasive cholangiography.

Abstract:Endoscopic biliary drainage for unresectable malignant hilar biliary obstruction often requires multiple metal stents, and stent-in-stent placement is an effective drainage method [1] [2]. Although multiple stent placement requires more cholangiograms for proper positioning, post-endoscopic retrograde cholangiopancreatography (ERCP) cholangitis remains a concern. Moreover, the over-injection of contrast medium and residual contrast medium increase post-ERCP cholangitis [3] [4] [5]. To overcome these issues, we introduced a simple method using a Y-connector attached to an existing metal stent ([Fig. 1]). A 70-year-old woman undergoing chemotherapy for intrahepatic cholangiocarcinoma after a previous cholecystitis-associated endoscopic ultrasound-guided gallbladder drainage presented with obstructive jaundice for hilar biliary obstruction. Enhanced computed tomography revealed bilateral intra-bile duct dilation ([Fig. 2]). Stent-in-stent placement was performed. The first metal stent was placed, and a guidewire was inserted through the mesh gap into the bile duct, where the second stent placement was intended. By this point, most of the contrast medium had leaked out, making the length of the stenosis and the target placement position difficult to determine ([Fig. 3] a,b ). A Y-connector (Access-9TM, Hemostasis Valve; SHEEN MAN Co., Ltd., Osaka, Japan) was attached to the proximal end of the guidewire lumen of the second stent device. The stent was inserted into the bile duct through a guidewire using a Y-connector. Because contrast using the Y-connector enabled visualization of the stenosis, it was easy to determine the position of the stent ([Fig. 3] c ). Furthermore, to prevent postoperative cholangitis after deployment, the remaining contrast medium could be aspirated easily without switching to a catheter ([Fig. 3] d,e ) ([Video 1]). No post-ERCP cholangitis or elevated hepatobiliary enzyme levels were observed in this patient. The combination of the stent device and Y-connector is a convenient and an efficacious method for confirming the stent position and preventing post-ERCP cholangitis. Novel Sutureless Cholangiojejunostomy: Initial Experience with 11 Cases.

Abstract:Background: It has become increasingly necessary to develop a reliable sutureless technique to replace the conventional hand-sewn one for carrying out bilioenteric anastomosis.Methods: A new sutureless technique for cholangiojejunostomy is described, and a retrospective review was conducted of a prospectively collected database, which included 11 patients who underwent sutureless cholangiojejunostomy between April 2005 and July 2006.Results: All patients successfully underwent sutureless cholangiojejunostomy, including choledochojejunostomy and hepaticojejunostomy. Median operative time was 13 minutes. There were no mortalities and no postoperative morbidities directly related to cholangiojejumostomy. At a mean follow-up period of 8.2 months, no patients had evidence of anastomotic stricture, except for 1 patient who developed obstructive jaundice because the recurrent tumor was pressing against the hepaticojejumal anastomosis.Conclusions: Sutureless cholangiojejunostomy is simple, reliable, and feasible. Further larger-series studies, with longer follow-up periods and involving further improvements of the technique, are necessary before this procedure can become routine. (C) 2008 Excerpta Medica Inc. Abstract:Introduction Our aim was to introduce a simple way of making a self-releasing biliary stent for laparoscopic common biliary duct exploration. Methods To make a self-releasing biliary stent, an absorbable thread was sutured to a J-shape polyurethane biliary stent. After the evacuation of the calculi, a guide wire was placed into the duodenum antegradely, facilitating the insertion of the biliary stent. The pigtail of the stent was left in the duodenum. The choledochotomy was then sutured, with the absorbable thread left outside the bile duct. Results This study consisted of 150 patients with choledocholithiasis. The mean operating time was 126 +/- 36 minutes. The postoperative hospital stay was 6.5 +/- 3.6 days. In 149 cases, the biliary stents were released and discharged out of the body with feces. The inside body time of the rapid-releasing stent was 13.6 +/- 2.6 days, and it was 28.0 +/- 4.6 days for the slow-releasing stent. One stent was removed by endoscopy. Transient hyperamylasemia occurred in 32 patients (21.3%) without clinical onset of pancreatitis. Bile leakage occurred in three patients (2.0%), all of whom recovered after treatment. Residual lithiasis was found in one patient (0.7%) and was retracted by endoscopy 30 days after the first procedure. No infection or dislocation of the stent was found. Conclusion For laparoscopic common biliary duct exploration for choledocholithiasis, this method provides an alternative way to make a simple and safe self-releasing stent. It enables the endoscopic retraction of biliary stents to be avoided.
Usefulness of Intraductal Placement of a Dumbbell-Shaped Fully Covered Self-Expandable Metal Stent for Post-Cholecystectomy Bile Leaks

Abstract:Background and aims: In the treatment of post-cholecystectomy bile leaks, endoscopic naso-biliary drainage (ENBD) or biliary stenting using plastic stents is the standard of care. Fully covered self-expandable metal stent (FCSEMS) placement across the sphincter of Oddi is considered a salvage therapy for refractory cases, but pancreatitis and migration are the major concerns. Intraductal placement of a dumbbell-shaped FCSEMS (D-SEMS) could avoid these drawbacks of FCMSESs. In this retrospective study, we investigated the usefulness of intraductal placement of the D-SEMS for post-cholecystectomy bile leaks. Methods: Six patients who underwent intraductal placement of the D-SEMS for post-cholecystectomy bile leaks were enrolled. This method was performed as initial treatment in three patients and as salvage treatment in three ENBD refractory cases. Results: Technical and clinical successes were obtained in 6 (100%) patients and 5 (83%) patients, respectively. One clinically unsuccessful patient required laparoscopic peritoneal lavage. The early adverse event was one case of mild pancreatitis (17%). The median duration of the D-SEMS indwelling was 61 days (42-606 days) with no migration cases, all of which were successfully removed. The median follow-up after index ERCP was 761 (range: 161-1392) days with no cases of recurrent bile leaks. Conclusions: Intraductal placement of the D-SEMS for post-cholecystectomy bile leaks might be safe and effective even in refractory cases.

Abstract:A 68-year-old man was admitted with vomiting and jaundice. He had undergone total gastrectomy and Roux-en-Y jejunojejunostomy for cardiac cancer a year earlier. Magnetic resonance imaging indicated abdominal tumor metastasis and dilatation of the intrahepatic bile ducts. A cap-assisted adult colonoscope was used for endoscopic biliary drainage owing to abnormal anatomy. A jejunal stenosis was found near the esophagojejunostomy and a 1.5-cm-diameter balloon was applied to dilate the stricture (▶Fig. 1). The colonoscope then passed through the stenosis into the duodenum. However, the papilla could not be reached because of the duodenal stricture and cannulation was not performed (▶Fig. 2). Abstract:BackgroundsStent migration following endoscopic ultrasonography-guided hepaticogastrostomy (EUS-HGS) may sometimes be fatal because there are no adhesions between the biliary tract and stomach. To prevent stent migration and minimize the stent length in the abdominal cavity, we recently performed EUS-HGS using the technique of releasing the stent within the scope channel.AimsTo examine the technical feasibility of the intra-scope channel stent release technique.MethodsForty-one consecutive patients who underwent EUS-HGS were enrolled. Between October 2015 and December 2015, EUS-HGS was performed using the extra-scope channel release technique, while the intra-scope channel release technique was performed between January 2016 and March 2016.ResultsThe distance between the hepatic parenchyma and the stomach wall after EUS-HGS in the intra-scope channel stent release group was significantly shorter than that in the extra-scope channel release group (0.66 ± 1.25 vs 2.52 ± 0.97, P < 0.05). Adverse events, such as biloma or stent migration, were seen in only the extra-scope channel release group.ConclusionIn conclusion, although additional cases and randomized controlled studies using metal stents of various lengths are needed, our technique is likely to be clinically useful for the prevention of early and late stent migration.

Abstract:A 75-year-old man was referred for management of a hepaticojejunostomy (HJS) stricture following HJS performed for a bile duct injury during cholecystectomy 3 months prior. A short-type double-balloon enteroscopy (DBE) was attempted but was unsuccessful due to inability to reach the HJS. Endoscopic ultrasound (EUS)-guided transduodenal endoscopic retrograde cholangiopancreatography (ERCP) for management of the HJS stricture was planned [1]. An EUS-guided duodenum–afferent limb bypass was first performed with a lumen-apposing metal stent (LAMS) between the duodenum and the afferent limb ([Video 1]). On EUS, the afferent limb was identified from the duodenum and punctured with a 19G needle (EZshot 3; Olympus Medical, Tokyo, Japan) ([Fig. 1]).The afferent limb was distended by infusion of 500 ml of normal saline mixed with indigo-carmine and contrast medium. Over a 0.025-inch guidewire, the delivery system of the cautery-enhanced LAMS delivery system (Hanarostent Z-EUS IT; M.I. Tech, Gyeonggi-do, South Korea) was inserted and a 16 × 20-mm stent was deployed into the afferent limb ([Fig. 2], [Fig. 3]) [2]. ERCP was subsequently performed after 3 days with a dual-channel endoscope inserted into the afferent limb via the LAMS. The HJS ([Fig. 4]) was dilated with a 6-mm biliary balloon (Hurricane Biliary RX; Boston Scientific, Marlborough, Massachusetts, USA). Two plastic stents were inserted into bilateral intrahepatic ducts. The patient was discharged on post-procedure day 2. No other complications or unplanned procedure occurred on follow-up. The patient underwent stent exchange at 6 months. No residual stricture was evident on cholangiogram at 9 months post-procedure. The HJS stricture occurs in up to 12.5% of patients at 2 years post hepatico-jejunostomy [3]. In this patient with an HJS stricture after Roux-en-Y HJS, EUS-guided duodenum–afferent limb bypass was successful for access to the HJS for ERCP after failed DBE-assisted ERCP. Abstract:Abstract Cholangioscopy is reportedly useful for selective guidewire placement across difficult biliary strictures, but few methods are available for complete stricture of biliary anastomosis. This study aimed to propose a guidewire puncture technique to recanalize totally obstructed anastomosis and discuss its safety and feasibility. From January 2015 to December 2021, a total of 11 patients with complete biliary anastomotic stricture after liver transplantation were enrolled. These patients underwent peroral single operator cholangioscopy (SpyGlass), whereas two failed cases on SpyGlass finally underwent percutaneous transhepatic cholangioscopy (PTCS). The steps of the recanalization technique were as follows: the stricture was viewed carefully to detect the closure point (CP) of the scar endoscopically, then the CP was targeted by the hard tip of the guidewire and broke through under guidance of the cholangioscope and fluoroscope. Complete occlusions were confirmed by SpyGlass in all cases. A total of 13 hard-tip guidewire punctures were performed under cholangioscopy, and ten punctures were successful (technical success rate, 76.9% [10/13]). After recanalization of the occluded anastomosis, plastic stent or metallic stent was deployed in three and seven patients, respectively. No procedure-related complications occurred during or after the cholangioscopy-assisted guidewire puncture. After a mean follow-up of 12 months, stents had been removed in five patients. The other six patients were still receiving stent treatment. This study demonstrated that the guidewire puncture technique under cholangioscopy is safe and feasible for complete stricture of biliary anastomosis, and the success rate is satisfactory.
