Abstract:Endoscopic biliary drainage for unresectable malignant hilar biliary obstruction often requires multiple metal stents, and stent-in-stent placement is an effective drainage method [1] [2]. Although multiple stent placement requires more cholangiograms for proper positioning, post-endoscopic retrograde cholangiopancreatography (ERCP) cholangitis remains a concern. Moreover, the over-injection of contrast medium and residual contrast medium increase post-ERCP cholangitis [3] [4] [5]. To overcome these issues, we introduced a simple method using a Y-connector attached to an existing metal stent ([Fig. 1]). A 70-year-old woman undergoing chemotherapy for intrahepatic cholangiocarcinoma after a previous cholecystitis-associated endoscopic ultrasound-guided gallbladder drainage presented with obstructive jaundice for hilar biliary obstruction. Enhanced computed tomography revealed bilateral intra-bile duct dilation ([Fig. 2]). Stent-in-stent placement was performed. The first metal stent was placed, and a guidewire was inserted through the mesh gap into the bile duct, where the second stent placement was intended. By this point, most of the contrast medium had leaked out, making the length of the stenosis and the target placement position difficult to determine ([Fig. 3] a,b ). A Y-connector (Access-9TM, Hemostasis Valve; SHEEN MAN Co., Ltd., Osaka, Japan) was attached to the proximal end of the guidewire lumen of the second stent device. The stent was inserted into the bile duct through a guidewire using a Y-connector. Because contrast using the Y-connector enabled visualization of the stenosis, it was easy to determine the position of the stent ([Fig. 3] c ). Furthermore, to prevent postoperative cholangitis after deployment, the remaining contrast medium could be aspirated easily without switching to a catheter ([Fig. 3] d,e ) ([Video 1]). No post-ERCP cholangitis or elevated hepatobiliary enzyme levels were observed in this patient. The combination of the stent device and Y-connector is a convenient and an efficacious method for confirming the stent position and preventing post-ERCP cholangitis. Abstract:We developed an "olive-tipped" technique for preventing the distal filter retrieval catheter from entrapment in the stent struts during retrieval procedures after stent deployment and which we used in 2 vertebral artery stenting cases. A new wire entry port was made in the catheter at the transition of larger and smaller lumen to permit passage of a low-profile balloon into the lumen of the retrieval catheter. When the balloon was inflated across the retrieval catheter tip, a smoother profile was created which eliminated the sharp step between the catheter tip and the filter wire. The retrieval catheter could then be advanced easily through the stent struts using this modified system.
The “U-Wire” Technique During ERCP for Easy Guide Wire Passage Through Occluded Uncovered Metal Stents

Abstract:A significant number of self-expanding metal stents (SEMSs) placed as conservative therapy for malignant biliary obstruction will become occluded [1]. Stent occlusion can be treated endoscopically either by balloon trawling to remove the biliary sludge and debris or by inserting a polyethylene stent or a second stent through the occluded stent [2]. However, these treatments can be given only after a guide wire is passed through the occluded lumen of the stent into the bile duct beyond [1], and not infrequently during endoscopic retrograde cholangiopancreatography (ERCP), a guidewire accidentally passes through the mesh of an uncovered metal stent, which may result in prolongation of the ERCP procedure or fluoroscopy. Here, we present a simple method that avoids guide wires being unintentionally passed through the stent mesh: the “Uwire” technique. About 2–3cm of the guide wire is first passed out of the sphincterotome, and biliary cannulation is then completed with the sphincterotome (●" Fig.1a). The tip of the wire remains outside the papilla and the body of the wire is advanced into the lumen of the stent along with the sphincterotome. The wire is therefore formed into a “U” shape in the occluded stent and as a result, it can be easily passed through the lumen of the stent without the tip of the wire passing through the mesh (●" Fig.1b–d). This U-wire cannulation method is an extremely effective and simple technique that avoids the risk of accidental passage of a guide wire through the mesh of a metal stent.

Abstract:During percutaneous pulmonary valve implantation, it is challenging to advance the delivery system loaded with a stent-valve through the right ventricular outflow tract. An extra-stiff wire can provide enough supporting and stable positioning to prevent the displacement of the guide wire in stent-valve delivery. In the 2 cases reported here, we used a snare to trap the extra-stiff wire and a large sheath to support the snare; thus, the extra-stiff wire was pulled to the distal end and provided strong support for the advancement of the delivery system. After 8 months of follow-up, the patients were in good condition with excellent valve function. We named this novel, efficient technique the “parallel anchor sheath snare” technique. This report highlights a novel delivery strategy for patients with tortuous right ventricular outflow tracts in percutaneous pulmonary valve implantation.
Radiopaque biodegradable stent for duct-to-duct biliary reconstruction in pigs

Abstract:BackgroundBiliary stricture is a common cause of morbidity after liver transplantation. We previously developed a duct-to-duct biliary anastomosis technique using a biodegradable stent tube and confirmed the feasibility and safety of biliary stent use. However, the duration and mechanism of biliary stent absorption in the common bile duct remain unclear.Materials and methodsRadiopaque biodegradable biliary stents were created using a copolymer of L-lactide and ε-caprolactone (70: 30) and coated with barium sulfate. Stents were surgically implanted in the common bile duct of 11 pigs. Liver function tests and computed tomography (CT) scans were performed postoperatively, and autopsies were conducted 6 months after biliary stent implantation.ResultsAfter the surgery, all 11 pigs had normal liver function and survived without any significant complications such as biliary leakage. A CT scan at 2 months post-procedure showed that the biliary stents were located in the hilum of the liver. The stents were not visible by CT scan at the 6-month follow-up examination.ConclusionsThe surgical implantation of radiopaque biodegradable biliary stents in biliary surgery represents a new option for duct-to-duct biliary reconstruction. This technique appears to be feasible and safe and is not associated with any significant biliary complications. The advantage of coated biliary stent use is that it may be visualized using abdominal radiography such as CT.

Abstract:Coronary atherosclerotic lesions at the ostium and proximal coronary arteries pose significant challenges in percutaneous coronary interventions (PCI), especially in the left main coronary artery (LMCA). Guide catheter‐induced damage can lead to severe complications such as vessel dissection or myocardial infarction. Ostial stent placement with drug‐eluting stents offers mechanical support and reduces restenosis but is technically challenging due to the anatomical complexity of the ostium. Complications like longitudinal stent deformation and hydraulic dissection from contrast media are specific concerns. The aortic free‐floating wire technique, expanded in this article to Wire in Aorta for Localization and Protection of the Ostium (WALPO), provides a method for safely locating and protecting the ostium during PCI, aiming to improve the safety and efficacy of PCI at this location.

Guide Wire-Assisted Technique to Access the Bile Duct with Biopsy Forceps for Repositioning and Removal of Metal Stents

Abstract:The malposition of a self-expandable metal stent (SEMS) placed fully inside the bile duct may be disastrous, and adjusting the position of a malpositioned stent can be difficult or impossible. Fully-covered SEMSs were designed mainly to treat benign biliary strictures and to be removable [1]. The removal process can however be difficult if the stent is placed fully inside the bile duct. Grasping the end of stent inside the bile duct directly with biopsy forceps may allow metal stents to be easily adjusted or removed. However, the front tip of a biopsy forceps is straight and hard, so access to the bile duct with biopsy forceps is difficult, especially via a native papilla without a sphincterotomy.

Abstract:Video 1A novel method of bilateral biliary decompression by EUS-guided hepaticogastrostomy with bridging stenting using the partial stent-in-stent method for reintervention of multiple metal stent failure.We report a case in which anterior and posterior drainage was performed using the partial stent-in-stent method via the transpapillary approach. The patient had a bismuth type IV biliary obstruction, but only the right hepatic lobe was drained due to obstruction of the left portal vein. For the recurrent stent dysfunction, the patient underwent placement of a plastic stent within an uncovered self-expanding metal stent to correct stent dysfunction. A 7F plastic stent inside a metal stent is shown.The patient later experienced stent failure and jaundice due to tumor progression and was admitted for plastic stent replacement. Neither imaging results nor symptoms suggested duodenal stenosis. The transpapillary approach was attempted first but was unsuccessful. Duodenoscopy was challenging to perform because of duodenal stenosis. Fluoroscopy confirmed the duodenal stenosis. The plastic stent was extracted using an upper endoscope. Multiple uncovered metal stents are shown (1 stent in the anterior bile duct and 2 stents in the posterior bile duct). Jaundice did not resolve despite plastic stent removal.The patient refused to undergo percutaneous biliary drainage, so a decision was made to perform an EUS-guided hepaticogastrostomy (HGS) instead. The left bile duct was observed in the stomach. The left bile duct was punctured with a 19-gauge FNA needle. A 0.025-inch hydrophilic guidewire was directed into the left bile duct. Enhancement of the bile duct showing malignant hilar biliary obstruction (bismuth IV) is seen. Insertion of the guide wire into the posterior bile duct is shown.The stent mesh was then dilated using a balloon dilator. However, there was difficulty inserting the catheter. Additional dilation was performed using a spiral dilator. This instrument is a tapered tip dilator that fits into 0.025-inch guidewires and is expandable to 7F. Insertion of a second guidewire with a larger caliber was done to straighten the bile duct and help stabilize stent insertion. A 0.035-inch hydrophilic guidewire into the posterior bile duct using a double-lumen cannula and insertion of a 0.025-inch hydrophilic guidewire into the anterior bile duct are shown.The stent mesh was then dilated using a spiral dilator. A metal stent was placed through the anterior bile duct at a steep angle. Insertion and deployment of the first uncovered self-expanding metal stent (8 × 60 mm) from the anterior bile duct into the left bile duct is shown. Multiple metal stents were implanted into the hilar area, and the new stent was placed using the partial stent-in-stent method to prevent overexpansion. Guidewire seeking the posterior bile duct from inside the deployed stent through the stent mesh is shown.The stent mesh was then dilated using a balloon dilator. Insertion and deployment of an uncovered self-expanding metal stent (8 × 60 mm) from the posterior bile duct to the left bile duct using the partial stent-in-stent method is shown. Enhancement of the bile duct shows drainage from the right bile duct. The fistula of the HGS was only dilated with the spiral dilator. The risk of bile leakage was low, so we decided to implant a plastic stent. A 7F × 15-cm plastic stent was placed from the posterior bile duct into the stomach. Anterior and posterior segment drainage by EUS-HGS with bridging stenting using the partial stent-in-stent method is shown, with left segment drainage by EUS-HGS with the plastic stent.We performed EUS-HGS on a patient with multiple metal stents in place. There were no adverse events, and total bilirubin levels were reduced by more than half within 2 weeks. Six months have passed without stent dysfunction.
Steerable catheter based on wire-driven seamless artificial blood vessel tube for endoscopic retrograde transpapillary interventions

Abstract:Purpose: Current steerable catheters (SCs) for endoscopic retrograde cholangiopancreatography (ERCP) have performance limitations caused by an asymmetric multiple-slit tube design with a small maximum bend angle, lesser curvatures, and insufficient durability. We propose a wire-driven SC for balanced bidirectional bending using artificial blood vessel material to overcome these limitations. We assess the SC prototype's steerability using phantom and animal models. Methods: The SC prototype employed a slit-less and multiple-lumen seamless tube with a polytetrafluoroethylene (PTFE) body with stretch-retractable porous expanded PTFE at the distal end, and loop-formed control wires. We evaluated the wire routing design using a static model. The bending performance was compared with conventional SCs. Feasibility studies were performed, including major duodenal papilla insertions and ductal branch selections in desktop phantoms and a mini-pig model. Results: The proposed design reduced the wire contact force by 48% compared to the single wire configuration. The maximum bend angle was 162°, almost twofold larger than that for conventional SCs. The lateral tip position in the bent shape was maximally 56% smaller. The tip flexibility was comparable to conventional SCs, and the insertion resistance was similar to the passive catheters. Phantom studies showed that the SC prototype could perform the large and protuberant papilla insertions and fine ductal branch selections without breaking; the animal study was completed successfully. Conclusion: We propose a wire-driven SC design for ERCP using a multi-lumen seamless tube and two loop-formed control wires, different from the conventional SC design with a multiple-slit tube and single control wire. The SC prototype records balanced bidirectional bending with a maximum bending angle of ± 162° without breakage risk. The phantom and animal studies show that the prototype performance potentially facilitates papilla cannulations and intrahepatic ductal branch seeking.
Sutureless Choledochoduodenostomy with an Intraluminal Degradable Stent in Dog Model.

Abstract:Background It is difficult and time-consuming for carrying out conventional hand-sewn bilioenteric anastomosis, especially for small bile duct anastomosis and laparoscopic procedure. In order to simplify it, we have developed a novel procedure of sutureless bilioenteric anastomosis with an intraluminal degradable stent. This study aimed to evaluate the feasibility and safety of this technique with cholangioduodenostomy in dog model.Methods A patent intraluminal degradable stent tube for sutureless choledochoduodenostomy in dog model was made with polylactic acid in diameter of 3 mm or 4 mm. Thirty-eight dogs were randomly divided into to a stent group (SG, n=20) and a control group (CG, n=18). Dogs in the SG underwent sutureless choledochoduodenostomy with intraluminal stent, while the CO underwent conventional choledochoduodenostomy (single layer discontinuous anastomosis with absorbable suture). Dogs of each group were divided into 4 subgroups according to time of death (1, 3, 6, and 12 months postoperatively) to evaluate the healing of anastomosis. Operation time, intraoperative tolerance pressure of anastomosis, rate of postoperative bile leakage, bursting pressure of anastomosis were compared between the two groups. Anastomosis tissue was observed afterwards by pathology evaluation, hydroxyproline content, serum bilirubin, liver enzyme level and magnetic resonance cholangio-pancreatography (MRCP) to assess the stricture.Results All procedures were completed successfully. The surgical time of the SG was significantly less than the CG (SG: (19.2+/-4.3) minutes, vs. CG: (29.2+/-7.1) minutes, P=0.000). One bile leakage was occurred in either group. No significant difference of intraoperative tolerance pressure of anastomosis, rate of bile leakage and postoperative bursting pressure of anastomosis, anastomotic stricture, hydroxyproline content, serum bilirubin and liver enzyme level was found between the two groups. MRCP showed no anastomosis stricture and obstruction during months of follow-up.Conclusion The technique of sutureless choledochoduodenostomy with a degradable intraluminal stent is feasible and a safe procedure in this dog model. Abstract:Guide extension catheters are useful in PCI procedures when device passage is difficult. However, in complex lesions, such as severely tortuous vessels, deep insertion of the guide extension catheter cannot always be achieved even when balloon anchoring technique is applied. In such cases, in our hospital, a guidewire is added as a buddy wire and the balloon anchoring technique is applied. The advantage of this method is that the anchoring force is strengthened by anchoring another wire with the wire. By pulling the wire that has been anchored with a balloon, anchoring force is fortified, enabling deep insertion of the guide extension catheter and subsequent stent delivery. This method, which we refer to as the Buddy-wire anchoring technique, can be useful in cases of complex lesions, especially those with severely tortuous vessels, where device passage is difficult. It is crucial to note that their efficacy tends to decline after approximately 60 days, underscoring the importance of timely removal. LAMS are also increasingly being used off-label for addressing gastrointestinal strictures that do not respond to conventional endoscopic interventions.

Abstract:We report two case series in which we encountered the difficulties to retrieve disrupted devices during percutaneous coronary intervention and endovascular treatment. The broken devices were successfully and safely retrieved by the "flower technique" that creates multiple cut and slit in the tip of the guiding catheter. This technique can be easily and safely applied in situations where a balloon catheter or stent has been disrupted and device entrapment has occurred, as the technique simply involves manually placing multiple cuts or slits in the tip of the guiding catheter. Transcatheter interventionists should be familiar with this technique because it can be bailout from device entrapment without additional cost or clinical experience.

Abstract:Objective: Repair of medial canthus injury involving canaliculus is an emergency indication for canalicular intubation to restore lacrimal drainage. Herein, the author has described an innovative but simple technique for this reconstruction. Method and result: A small, blunt pigtail probe was gently passed through the opposite canaliculus in a rotational manner. A silicon stent was threaded inside canaliculi by reverse rotation of the pigtail in an atraumatic way. The technique was used on 4 pediatric cases without any postoperative complication or epiphora. Conclusions: This technique of intubation is simple, cheap, and useful in canalicular emergencies, including "distal" canaliculus lacerations.
